Abstract:Objective To summarize the results of surgical management of different types of transposition of great arteries with different procedures.Methods Two hundred and seventy-five patients with transposition of great arteries and other congenital heart defects (ventricular septal defect,pulmonary artery stenosis,atrial septal defect,patent ductus arteriosus,interrupted inferior vana cava,persistent left superior vena cava,coronary artery anomalies and coarctation of the aortic arch) were surgically corrected.Different procedures,such as arterial switch operation,bidirectional cavo-pulmonary anastomosis,Rastelli procedure,Fontan operation and Senning operation,were adopted according to different malformations.Clinical data was reviewed and postoperative survival was evaluated.Results There were 14 cases of early deaths (5.09％) in this group with 10 cases occurred after arterial switch operation (10/243,4.12％) and 4 cases following other procedures (4/32,12.50％).Surviving patients following Glenn/modified Fontan procedures had significant improvement in oxygen saturation.Patients who undertook arterial switch operation and Senning operations could grow and develop normally.Conclusions Complete transposition of great arteries can be treated with a variety of procedures according to the combined anomalies.Satisfactory results can be achieved in neonates with arterial switch operation.

Abstract:Background: Arterial switch operation (ASO) has been optimal for children with transposition of great arteries (TGA) in either simple or complex form with an excellent survival rate. This operation was introduced late in China, but there has been a decreasing mortality in recent years. Optimizing the postoperative management has been essential to improve the survival rate after ASO. This study summarizes the experience in the management of the postoperative complications after ASO. Methods: Twenty-eight infants with TGA underwent ASO from January 2004 through December 2006. These patients aged 1-70 days (median, 6 days) had a body weight of 3.36±0.57 kg on average. Before operation, continuous intravenous infusion of prostaglanding E1 was routinely used to keep the ductus open with a SpO2 of 75%-90%. Ten patients required tracheal intubation and mechanical ventilation. Two patients underwent emergency ASO under general anesthesia and low-flow cardiopulmonary bypass or hypothermic circulatory arrest. All the 28 patients were further treated with modified ultrafiltration and delayed sternal closure after cessation of cardiopulmonary bypass. After ASO, they were on circulatory and respiratory support, with antibiotics, nutritional supplement in the ICU. Results: The patients had a cardiopulmonary bypass time of 167±32 minutes and an aortic crossclamping time of 101±24 minutes. Delayed sternal closure was carried out on 3.63±1.49 days after ASO. They had a mechanical ventilation of 5.89±3.02 days and an ICU stay of 10.12±3.25 days. There were 5 deaths after ASO with a mortality rate of 17.9%. Fifteen patients developed low cardiac output syndrome. In 12 patients presenting with cardiac arrhythmias, 9 had paroxysmal supraventricular tachycardia, 1 had frequent ventricular premature beats, 1 had ventricular tachycardia, and 1 had ventricular fibrillation. One patient suffered from pulmonary hypertensive crisis. Three patients with major bleeding and tamponade required emergency mediastinal exploration and finally survived. There were 6 patients with ventilatorassociated pneumonia, 6 with delayed incision healing, and 1 with chylothorax. Conclusions: The complications after ASO are common and complicated. Understanding of the physiological characteristics of infants and the pathophysiological changes, and optimizing postoperative treatment help to improve the survival rate after ASO.
Surgical Strategy for the Children with Double-Outlet Right Ventricle: Our Experience in 118 Cases

Abstract:Objective To summarize our experience of surgical treatment for double-outlet right ventricle (DORV) in children. Methods From August 1984 to June 2010, 118 patients with DORV underwent surgical repair at this center. Among the 118 patients, 62 were males and 56 were females.Their ages ranged from 10 days to 12 years old (mean age, 3. 2 ± 2. 6). Their weight ranged from 3. 0to 22. 5 kg (mean weight, 9. 6 ± 3. 7 kg). According to the STS-EACTS international nomenclature,86 patients were DORV associated with ventricular septal defect (VSD), 22 were DORV with tetralogy of Fallot (TOF), 3 were DORV with transposition of the greater arteries (TGA), and 7 were DORV with remote VSD. Corrective surgeries included 22 interventricular repairs, 84 interventricular repairs plus right ventricular outflow tract reconstruction (pericardium was used for the reconstruction in 82 cases, and valved extracardiac conduits was used in 2 cases), 4 switch procedures, 6 bidirectional Glenn procedures, 1 Fontan procedure, 1 Pulmonary artery banding procedure, and 1 Blalock-Taussig Shunt procedure. The surgical safety and efficacy were studied by retrospectively analyzing the clinical data of the 118 cases. Results Nine patients died after surgery (7. 6%). The leading cause of death was low cardiac output syndrome. Of the 51 patients underwent corrective surgery before 2004,7 died after surgery (13. 7%). However, only 2 died in the 67 patients who had surgery after 2005(2. 9%). The patients were followed up for 2 months to 10 years. Cardiac ultrasonography didn't show any obstruction of left ventricular outflow tract, but 1 patient developed left ventricular outflow tract obstruction 2 years after surgery and received corrective surgery. No long-term death and other complications were noted. Conclusions Surgical strategy for the children with double-outlet right ventricle should be made based on patients individual anatomic anomalies.

Abstract:Abstract OBJECTIVES This study aimed to evaluate the short-/mid-term outcome of patients with complex dextro (d)-/levo (l)-transposition of the great arteries (TGA), ventricular septal defect and left ventricular outflow tract obstructions. METHODS A single-centre, retrospective review of all complex dextro-TGA (n = 85) and levo-TGA (n = 22) patients undergoing different surgeries [Arterial switch operation + left ventricular outflow tract obstruction-resection (ASO-R), half-turned truncal switch/Mair (HTTS), Nikaidoh and Rastelli] between May 1990 and September 2022 was performed. Groups were analysed using Kruskal–Wallis test with post hoc pairwise comparison and Kaplan–Meier time-to-event models. RESULTS A total of 107 patients [ASO-R (n = 20), HTTS (n = 23), Nikaidoh (n = 21), Rastelli (n = 43)] were included, with a median age of 1.0 year (0.5–2.5) and surgical repair median follow-up was 3.8 years (0.3–10.5). Groups did not differ in respect to early postoperative complications/early mortality. Five-year overall survival curves were comparable: ASO-R 78.9% (53.2–91.5), HTTS 75.3% (46.8–89.9), Nikaidoh 85% (60.4–94.9) and Rastelli 83.9% (67.5–92.5), P = 0.9. Highest rates of right ventricular outflow tract (RVOT) reinterventions [33.3% and 32.6% (P = 0.04)] and reoperations [28.6% and 32.6% (P = 0.02)] occurred after Nikaidoh and Rastelli procedures. However, overall freedom from RVOT reinterventions and RVOT reoperations at 5 years did not differ statistically significantly between the groups (ASO-R, HTTS, Nikaidoh and Rastelli): 94.4% (66.6–99.2), 69.1% (25.4–90.5), 67.8% (34–86.9), 64.4% (44.6–78.7), P = 0.2, and 90.0% (65.6–97.4), 91% (50.8–98.7), 65.3% (32.0–85.3) and 67.0% (47.4–80.6), P = 0.3. CONCLUSIONS Surgical repair of complex dextro-/levo-TGA can be performed with satisfying early/mid-term survival. RVOT reinterventions/reoperations were frequent, with highest rates after Nikaidoh and Rastelli procedures. Left ventricular outflow tract obstruction reoperations were rare with zero events after Nikaidoh and HTTS procedures.

Abstract:
Aortic root replacement with the pulmonary autograft in children with complex left heart obstruction

Abstract:Background: The optimal surgical treatment of complex (multiple level or recurrent) left ventricular outflow tract obstruction (LVOTO) in infancy is controversial. Staged procedures expose the children to the need for reoperation, and currently available techniques of aortoventriculoplasty are associated with the morbidities of biological and mechanical prostheses. Methods: Between July 1992 and January 1996, we have performed 24 aortic root replacements with the pulmonary autograft in pediatric patients (< 18 years). Of this group, 8 were infants and children with complex LVOTO aged 9 days to 22 months (mean, 8.6 +/- 8 months) and weighing 3.3 to 10.2 kg (mean, 6.3 +/- 2.6 kg). The diagnoses were interrupted aortic arch/ventricular septal defect/subaortic stenosis in 3, recurrent aortic stenosis in 2, aortic stenosis and subaortic stenosis in 1, and aortic stenosis/subaortic stenosis/mitral stenosis/regurgitation in 2. All patients had undergone one to three previous operative procedures (mean, 1.5 +/- 0.8 procedures/patient). Preoperative echocardiographic peak LVOT gradient was 71.7 +/- 25 mm Hg (range, 40 to 110 mm Hg) and aortic annulus size was 7.2 +/- 2.3 mm (range, 4 to 10.6 mm). The surgical technique included replacement of the aortic root with the pulmonary autograft combined with incision of the conal septum to relieve subaortic stenosis or accommodate for size discrepancy between the aortic and pulmonary autograft root and a pulmonary homograft placed in the right ventricular outflow tract. Results: There were no perioperative or late deaths at follow-up (range, 2 to 25 months; mean, 13.5 +/- 8 months). Mean hospital stay was 15 +/- 17 days (range, 4 to 53 days). Three children had the following complications: diaphragmatic paresis (1), delayed pericardial effusion (1), and atrioventricular block requiring a pacemaker (1). In follow-up, echocardiographic findings showed absent aortic regurgitation in 3 and trivial aortic regurgitation in 5, and no significant LVOTO (mean peak gradient, 6.2 +/- 7.6 mm Hg; range, 0 to 16 mm Hg). Pulmonary homograft regurgitation was absent in 5, trivial in 2, and moderate in 1. Peak right ventricular outflow tract gradient by echocardiogram was trivial in 7, and a significant gradient of 55 mm Hg has developed in 1 infant. There were no infective or embolic complications during follow-up. Conclusions: Our experience shows that aortic root replacement with the pulmonary autograft can be performed in children with excellent clinical results. The technique of root replacement combined with ventriculoplasty allows definitive and simultaneous relief of complex and multiple-level obstructive lesions. Considering the growth potential of the pulmonary autograft, this should be regarded as the optimal treatment modality in infants with complex LVOTO:

Abstract:ABSTRACT Large-scale storage of CO2 within sub-sea geological formations is a viable option for reducing the volume of this greenhouse gas released directly to the atmosphere from anthropogenic activities. Risks to benthic marine life following possible leakage of gas through the seabed from this carbon capture and storage (CCS) initiative are not yet well established. This study examined behavior (activity patterns) in brown shrimp (Crangon crangon), exposed to a range of reduced seawater pH conditions (7.6, 7, or 6.5) simulating leakage scenarios of varying scales. Brown shrimp have an endogenous rhythmicity associated with their activity, which dictates they are most active during hours of darkness, presumably as protection against vision-dependent predators. This endogenous rhythm in activity continues to be expressed when shrimp are held under constant low-light conditions in the lab and provides an ecologically relevant endpoint to measure when examining the influence of reduced pH on the behavior of these animals. No marked differences in activity pattern were observed between control shrimp maintained at pH 8.1 and those at pH 7.6. However, changes in activity were evident at pH 7 and pH 6.5, where significant shifts in timing and intensity of activity occurred. There was an unexpected increase in activity within periods of expected light, probably signaling efforts by shrimp to migrate away from reduced seawater pH conditions. The loss of this important member of the benthic community due to migration may have important consequences for many of the resilient species that remain.

Clinical analysis of surgical treatment of congenital atrioventricular septal defect

Abstract:Objective:To retrospectively explore clinical features, surgical treatments, mid/long-term survival rates and risk factors for reoperation in children with congenital atrioventricular septal defect(AVSD).Methods:From January 2009 to December 2019, 125 hospitalized children(46 boys, 79 girls)were operated.There were complete atrioventricular septal defect(C-AVSD, n=72), partial atrioventricular septal defect(P-AVSD, n=34)and intermediate atrioventricular septal defect(I-AVSD, n=19). The median operative age was 14 months.According to anatomical classifications, atrial defect repair, atrioventricular valve reshaping and modified mono/double-patching were performed.Demographic data, in-hospital mortality and reoperation rate were recorded.Univariate/multivariate correlation and Cox regression analyses were performed.Results:Atrioventricular septal defect repair and atrioventricular valvuloplasty were successfully performed under cardiopulmonary bypass with cardiopulmonary bypass time(102±35)min, aortic arrested time(67±26)min, ventilator assist time(35±18)hours and postoperative hospitalization stay(13.8±4.5)days.Postoperative complications included low cardiac output(n=8), severe left atrioventricular valve regurgitation(n=3), left atrioventricular valve regurgitation with stenosis(n=1), residual atrial septal shunt(n=1), left ventricular outflow tract stenosis(n=1), arrhythmia(n=1), intracranial hemorrhage(n=1), lung infection(n=7, including 1 case of severe pneumonia)and sepsis(n=2). Overall postoperative mortality rate was 6.4%(8/125)and mortality rate 4.0%(2/46)in the past 5 years.All death cases belonged to C-AVSD and death cause was low cardiac output syndrome(n=3), sepsis(n=2), arrhythmia(n=1), intracranial hemorrhage(n=1)and severe pneumonia(n=1). The reoperative rate was 6.4%(8/125), including early operation(n=6, during hospitalization or within 1 month post-discharge), including left atrioventricular regurgitation(n=3), left ventricular outflow tract obstruction(n=1)and arrhythmia(n=1), intracranial hemorrhage(n=1); late operation for severe left atrioventricular valve regurgitation(n=2, 1 month post-discharge)and 1 case with concurrent left atrioventricular valve stenosis.Conclusions:The mid/long-term surgical outcome of congenital atrioventricular septal defect is excellent.And left ventricular dysplasia is a risk factor for reoperation and long-term survival.

Abstract:While the long-term outcome of surgical aortic valvotomy (SAV) appears to be better than that of balloon aortic valvuloplasty (BAV) as the primary procedure of aortic valve stenosis, the surgical strategies and techniques of treating aortic valve disease in children in other situations remain controversial.Valve repair should be first considered while replacement is still unavoidable in some cases, and new repair techniques developed by innovative surgeons are gradually becoming adopted.Some complex repair procedures such as cusp extension, leaflet replacement/reconstruction have provided satisfactory outcomes.The Ozaki technique replaces aortic valve leaflets with glutaraldehyde-treated autologous pericardium instead of replacing the valve entirely.Special instruments have been developed to make the Ozaki technique more reproducible and standardized.Neonates and infants undergoing aortic valve replacement (AVR) are a high-risk group, where repair should be the primary consideration rather than replacement.Several systematic reviews reveal that all currently available aortic valve substitutes such as pulmonary autograft, mechanical prosthesis, homograft and bioprosthesis are associated with suboptimal results in children, but pulmonary autograft appeared to be superior with high freedom from reintervention and better hemodynamic performance.The strategy for treatment of aortic valve disease should be specifically analyzed based on the brief of being beneficial for children.

Abstract:In this invited expert review, we focus on evolving lifetime management strategies for adolescents and young adults with congenital aortic valve disease, acknowledging that these patients often require multiple interventions during their lifetime. Our goal is to preserve the native aortic valve when feasible. Leveraging advanced multimodality imaging, a detailed assessment of the aortic valve and root complex can be obtained, and a surgical approach tailored to an individual patient's anatomy and pathology can be employed. In turn, aortic valve repair and reconstruction can be offered to a greater number of patients, either as a definitive strategy or as a component of a staged strategy to delay the need for aortic valve replacement until later in life when more options are available. In adolescents and young adults with unrepairable aortic valve disease, our preferred strategies are aortic valve replacement with pulmonary autograft (Ross procedure) or autologous pericardium (Ozaki procedure). Aortic valve replacement with a bioprosthetic or mechanical valve is a safe and reproducible option reserved as a "last resort" in adolescents and young adults due to the cumulative lifetime risk of valve-related complications and ongoing attrition of these patients during follow-up.

Outcomes of treatment pathways in 240 patients with congenitally corrected transposition of great arteries

Abstract:Objective: Congenitally corrected transposition of the great arteries (ccTGA) encompasses a diverse morphologic cohort, for which multiple treatment pathways exist. Understanding surgical outcomes among various pathways and their determinants are challenged by limited sample size and follow-up, and heterogeneity. We sought to investigate these questions with a large cohort of ccTGA patients presenting at different ages and representing the full therapeutic spectrum. Methods: Retrospective review of 240 patients diagnosed with ccTGA from Cleveland Clinic coupled with prospective cross-sectional follow-up. Forty-six patients whose definitive procedure was completed elsewhere were excluded. Time-related survival was described among treatment pathways using actuarial, time-varying covariate, and competing risks analyses. Temporal trends in longitudinal valve and ventricular function were assessed using nonlinear mixed-effects models. Results: Median follow-up was 10 years. Seventy-nine patients with ccTGA underwent anatomic repair, 45 physiologic repair, 24 Fontan palliation, and 6 primary transplant. Forty patients managed expectantly had excellent long-term survival when considered from time of presentation, but benefited from failures captured following transition to physiologic repair or transplant. Morphologic right ventricular dysfunction after physiologic repair increased from 68% to 85% after 5 years, whereas morphologic left ventricular function was stable in anatomic repair, especially with early surgery. Transplant-free survival at 15 years for anatomic and physiologic repair was 80% and 71%, respectively. Conclusions: Early anatomic repair may be preferable to physiologic repair for select ccTGA patients. Late attrition after physiologic repair represents failure of expectant management and progressive tricuspid valve and morphologic right ventricular dysfunction compared with anatomic repair, where morphologic left ventricular function is relatively preserved.
Contemporary results after repair of partial and transitional atrioventricular septal defects

Abstract:Objective: The exact incidence and risk factors for reoperation in partial and transitional atrioventricular septal defects are unclear. The goal of this study was to assess risk factors for left atrioventricular valve and left ventricular outflow tract reoperation in partial and transitional atrioventricular septal defects. Methods: All patients undergoing partial and transitional atrioventricular septal defects repair between 1995 and 2017 were reviewed. Patients were classified as infants (<1 year), toddlers (1-3 years), children (3-17 years), and adults (≥18 years). Survival and reoperation were assessed using log-rank test and Cox models for univariate and multivariable analyses, respectively. Results: Overall, 265 patients underwent partial and transitional atrioventricular septal defects repair (partial: 177 [67%]). Median age was 2 years. The cohort included 73 infants (28%), 85 toddlers (32%), 94 children (35%), and 13 adults (5%). Trisomy 21 was present in 76 patients (29%), and in 216 patients (83%), the zone of apposition was completely closed. Perioperative mortality was 0.8%. Complete heart block did not develop in any patients. Ten-year survival and freedom from reoperation were 98% and 81%, respectively. On multivariable analysis, trisomy 21 (hazard ratio [HR], 0.16) and older age compared with infants (toddlers: HR, 0.35; children: HR, 0.25) were protective for any reoperation, whereas heterotaxy (HR, 3.43) was a risk factor. For left atrioventricular valve reoperation, toddlers (HR, 0.35), children (HR, 0.25), and trisomy 21 (HR, 0.16) remained protective, whereas left atrioventricular valve anomaly was a risk factor (HR, 2.61). Likewise, for left ventricular outflow tract reoperation, toddlers (HR, 0.24) and children (HR, 0.06) were protective. Conclusions: Mortality after partial and transitional atrioventricular septal defects repair is minimal, yet reoperation for left atrioventricular valve disease and left ventricular outflow tract obstruction remains significant. Patients requiring repair during infancy are at higher risk of reoperation.

Abstract:BACKGROUND Congenital aortic and truncal valve disease is challenging in infants and children given the lack of available prostheses in very small sizes and the limited durability of homograft aortic valve replacement. METHODS A comprehensive literature search was performed using the PubMed database. Studies were included either if the report included patients less than 1 year of age or if the technique was tailored to accommodate for somatic growth. RESULTS Techniques for aortic and truncal valve repair addressing each aspect of the aortic valve complex – the aorta, aortic annulus, commissures, and cusps – were reviewed. The incidence of reoperation following aortic or truncal valve repair is significant at 10-years ranging from 30% to 70% depending on the underlying diagnosis and the repair technique utilized. A significant challenge in interpreting the published literature relates to the lack of anatomic data available in the publications limiting the ability to make direct comparisons between operative techniques and also limits the ability to draw conclusions regarding these techniques as applied to varied etiologies. CONCLUSIONS A comprehensive understanding of the aortic valve complex is necessary to achieve adequate results in pediatric aortic valve repair given the high variability in these valves.

Association of tetralogy of Fallot and complete atrioventricular canal: a single-centre 40-year experience

Abstract:Abstract OBJECTIVES Outcome data in tetralogy of Fallot (ToF) and complete atrioventricular canal (CAVC) are limited. We report our experience for over 40 years in this patient population. METHODS Single-centre, retrospective analysis of patients who underwent surgical repair with the diagnosis of ToF-CAVC from 1979 to 2022, divided into 2 different periods and compared. RESULTS A total of 116 patients were included: 1979–2007 (n = 61) and 2008–2021 (n = 55). Balanced CAVC (80%) and Rastelli type C CAVC (81%) were most common. Patients in the later era were younger (4 vs 14 months, P < 0.001), fewer had trisomy 21 (60% vs 80%, P = 0.019) and fewer had prior palliative prior procedures (31% vs 43%, P < 0.001). In the earlier era, single-patch technique was more common (62% vs 16%, P < 0.001), and in recent era, double-patch technique was more common (84% vs 33%, P < 0.001). In the earlier era, right ventricular outflow tract was most commonly reconstructed with transannular patch (51%), while in more recent era, valve-sparing repairs were more common (69%) (P < 0.001). In-hospital mortality was 4.3%. The median follow-up was 217 and 74 months for the first and second eras. Survival for earlier and later eras at 2-, 5- and 10-year follow-up was (85.1%, 81.5%, 79.6% vs 94.2%, 94.2%, 94.2% respectively, log-rank test P = 0.03). CONCLUSIONS The surgical approach to ToF-CAVC has evolved over time. More recently, patients tended to receive primary repair at younger ages and had fewer palliative procedures. Improved surgical techniques allowing for earlier and complete repair have shown a decrease in mortality, more valve-sparing procedures without an increase in total reoperations. Abstract:Objectives: Children with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) are at risk for myocardial infarction and death. This retrospective study shows the mid-term follow-up after the use of aortic implantation and alternative methods to achieve coronary transfer. Methods: Since 1990 seven consecutive children underwent primary repair of ALCAPA. Age at operation ranged from 2 to 71 months (median 11 months). Operative techniques included ligation (n = 1), intrapulmonary tunnel (n = 1), and aortic implantation (n = 5). One patient with severe mitral valve incompetence underwent additional mitral valve replacement. A 4-month-old patient was successfully treated after the operation with a left heart assist device. Results: One death in the series occurred at 2 weeks after intrapulmonary tunneling. The mid-term results were evaluated in the six survivors with a follow-up mean of 98 months (ranged 58-168). In all surviving patients with two-vessel coronary blood supply, left ventricular end-diastolic volume and left ventricular ejection fraction returned to near normal values 2-12 months postoperatively. The mitral valve incompetence decreased in all patients with a native mitral valve. One patient with coronary ligation showed severe mitral valve regurgitation and received additional mitral valve replacement concomitantly. Six years after primary valve replacement of a 21 mm SJM (Saint Jude Medical) a change of the mechanical valve to a 27 mm valve was necessary because of development of severe stenosis due to growth. Conclusions: It is always preferable to establish an antegrade flow of oxygenated blood through the coronary arteries and to create a two-coronary artery system. Mitral valve annuloplasty or replacement may be necessary for patients with severe mitral valve incompetence.
Birth in the Operating Room for Immediate Cardiac Surgery: A Rare but Effective Strategy

Abstract:Background: With significant advancements in fetal cardiac imaging, patients with complex congenital heart disease (CHD) carrying a high risk for postnatal demise are now being diagnosed earlier. We sought to assess an interdisciplinary strategy for delivering these children in an operating room (OR) adjacent to a cardiac OR for immediate surgery or stabilization. Methods: All children prenatally diagnosed with CHD at risk for immediate postnatal hemodynamic instability and cardiogenic shock who were delivered in the operating room (OR) between 2012 and 2023 in which the senior author was consulted were included. Results: Eight patients were identified. Six (75%) patients were operated on day-of-life zero, all requiring obstructed total anomalous pulmonary venous return (TAPVR) repair. Of these six patients, 2 (33%) required a simultaneous Norwood procedure, 2 (33%) required pulmonary artery unifocalization and modified Blalock-Taussig-Thomas shunt, and 2 (33%) patients had repair of obstructed mixed TAPVR. The remaining 2 patients potentially planned for immediate surgery had nonimmune hydrops fetalis and went into cardiogenic shock at 12 and 72 hours postnatally, requiring a novel Norwood procedure with left-ventricular exclusion for severe aortic/mitral valve insufficiency. The median ventilation and inpatient durations were 19 [IQR: 11-26] days and 41 [IQR: 32-128] days, respectively. Three(38%) patients required one or more in-hospital reoperations. Subsequent staged procedures included Glenn (n = 5), Fontan (n = 3), biventricular repair (n = 2), ventricular assist device placement (n = 1), and heart transplant (n = 1). Median follow-up was 5.7 [IQR:1.3-7.8] years. The five-year postoperative survival was 88% (n = 7/8). Conclusion: While children with these diagnoses have historically had poor survival, the strategy of birth in the OR adjacent to a cardiac OR where emergent surgery is planned is a potentially promising strategy with excellent clinical outcomes. However, this is a high-resource strategy whose feasibility in any program requires thoughtful assessment.
Outcomes After Repair of Pulmonary Atresia With Ventricular Septal Defect and Major Aortopulmonary Collateral Arteries: A Tailored Approach in a Developing Setting

Abstract:Objectives: Pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries (PA/VSD/MAPCAs) is complex and diverse that has led to a variety of treatment strategies. Experience has been largely obtained in the advanced countries. The clinical diversity is greater in China. We evaluated our surgical approaches and outcomes of these patients. Methods: We reviewed 127 patients undergoing varied surgeries in our center in 2010–2019. Results: Thirty patients underwent single-stage complete repair by unifocalizing MAPCAs and VSD closure (aged 3.9–131.4 months, median 22) with 3 (10%) early deaths. Ninety-seven underwent the first-stage rehabilitation strategy including systemic-to-pulmonary shunt in 29 (aged 0.5–144 month, median 8), and palliative RV-PA conduit in 68 (aged 2.2–209.6 months, median 14) with 5 (5.2%) early deaths. Eight-one patients (63.8%) eventually achieved complete repair with a median right/left ventricular (RV/LV) pressure ratio of 0.7 (ranged 0.4–1.0). Fourteen patients (11.0%) accepted palliation as final destination. Survival for the entire cohort was 89.5, 85.2, and 76.1% at 1, 5, and 10 years, respectively. Survival for those undergoing complete repair was 88.2 and 76.6% at 1 and 5 year, respectively. RV/LV pressure ratio ≥0.8 was risk factor for mortality (HR10.3, p = 0.003). Conclusions: Our cohort, the largest from China, had distinctive clinical features with substantially wider age range and higher RV/LV pressure ratio. Using the combined approaches tailored to individual patients, complete repair was achieved in 64% of patients. The early and intermediate outcomes are acceptable compared to many of the previous reports.
