Abstract:Objective: To assess oral hygiene behavior among patients undergoing orthodontic treatment with fixed appliances. Material and Method: One hundred and five orthodontic patients (33 males and 72 females) treated with fixed appliances at the Orthodontic Clinic, Department of Orthodontics, Faculty of Dentistry, Mahidol University, Thailand were studied. Patients were requested to fill out a structured questionnaire regarding the reason for seeking orthodontic treatment, the necessity of orthodontic treatment in daily life, oral hygiene behavior after placement of fixed orthodontic appliances. The data was analyzed to determine the frequency distribution and percentage ratio for each of the variables. Results: Approximately 83% of the patient sought orthodontic treatment for esthetic tooth alignment and 85.7% reported orthodontic treatment was necessary in their daily lives. During orthodontic treatment, 61% of the patients used orthodontic toothbrush, 81.9% used fluoride toothpaste. Percentage of regular and occasional uses of dental floss were 19% and 60% Similarly the use of regular and occasional use of mouth wash was 23.8% and 55.2%, toothpick 21% and 51.4%, and interdental brush 36.2% and 51.4%. Approximately 45% of the patients brushed their teeth in the morning and before bed, 28.6% brushed after meals, and 25.7% brushed every time after meals or snacks. About 83% reported that their orthodontists or dental assistants instructed them how to brush their teeth. Only 52.4% of the patients received instructions about oral hygiene care supplements. Conclusion: Most patients sought orthodontic treatment for esthetic tooth alignment and reported the necessity of orthodontic treatment in daily life. Although most of the patients used fluoride toothpaste, orthodontists and dental assistants should increase their awareness for instructing their patients in oral hygiene care to prevent caries and periodontal disease during fixed orthodontic treatment. Oral hygiene instruction is essential in all cases of orthodontic treatment and the use of adjuncts must be reinforced.

Abstract:Aim: Awareness on oral hygiene is important during the orthodontic phase. The aim of the present study was to assess the effectiveness of an oral hygiene program (knowledge, attitude, and practice of oral hygiene) for patients seeking fixed orthodontic treatment. Evaluating the knowledge, attitude, and practice on gingival health was the aim of the present study.Materials and Methods: The sample consisted of 150 patients who had a full complement of permanent dentition. One hundred and fifty orthodontic patients from the Department of Orthodontics and Dentofacial Orthopedics, Rural Dental College, Loni, Maharashtra, India were included in the questionnaire study. Questions were based on the knowledge, attitude, and practice on oral hygiene, i.e., gingival health at the time of orthodontic treatment. All answers were recorded and analyzed statistically.Results: The results of the study showed that nearly 50% of the patients were unaware about gingival health, 78% did not follow the oral hygiene instructions, and despite being given instructions, most of them were reluctant in practicing them strictly. It was very good that all patients used the toothbrush as an aid used to clean teeth and 72% of the patient did not have any previous dental exposure.Conclusion: The knowledge, attitude, practice of oral hygiene among orthodontic patients were poor and showed the need of better education and motivation. Many factors may be responsible for the non-maintenance of gingival health practices. There is a need to incorporate and evaluate more intense oral hygiene programs in the future.

Abstract:Objectives This study aimed to determine the overall oral health statuses of patients with fixed orthodontic appliance and clear aligner, as well as their oral hygiene management behaviours and satisfaction. Methods We selected 40 participants (20 each with fixed orthodontic devices and clear aligners) who visited a dental clinic. We conducted a survey to determine the oral hygiene management behaviours of the subjects and their satisfaction with their orthodontic treatment. Three measurements were also conducted at 4‐week intervals to determine the oral health statuses of the subjects. Abstract:The prevalence of malocclusion and a need for orthodontic treatment is high. Orthodontic appliances increase biofilm accumulation by expanding plaque retention sites. The aim of this study was to investigate the self-reported changes in oral hygiene habits among adolescents receiving orthodontic treatment. A cross-sectional study of 291 patients aged 10–17 years (mean (M) = 12.98; standard deviation (SD) = 2.36) was conducted in the Department of Orthodontics, Lithuanian University of Medical Sciences (LSMU) Hospital (Kaunas, Lithuania) during the fall semester (October–January) of the 2017/2018 study year. An anonymous self-administered questionnaire covered background information, experience of orthodontic treatment, oral hygiene habits and the seeking of professional dental care. Statistical data analysis was performed using SPSS version 22. To establish relationships between categorical variables, Chi-squared tests (χ2) were used. A p-value ≤ 0.05 was set to indicate statistically significant difference. The univariate logistic regression analysis evaluated the probability of an event given a certain risk indicator, including odds ratio (OR) and its confidence interval (95% CI). Associations were found between the usage of auxiliary measures (OR = 1.797 (1.118–2.887), p = 0.015), tongue cleaning (OR = 1.712 (1.059–2.767), p = 0.028), mouth rinsing after meals (OR = 1.707 (1.048–2.781), p = 0.032) and experience of orthodontic treatment, respectively. More orthodontic patients underwent professional oral hygiene regularly than non-orthodontic patients (p = 0.024). More patients with fixed orthodontic appliances reported significantly changed oral hygiene habits, while more orthodontic patients with removable appliances did not change their oral hygiene habits.

Abstract:ABSTRACT: The foundation of excellent oral health is proper oral hygiene, especially in patients with fixed orthodontic appliances (devices) where regular oral hygiene may be difficult to maintain due to the device. The objective of this study was to assess oral hygiene and gingival health in patients undergoing fixed orthodontic treatment at four different time points, with repeated oral hygiene guidance provided during each visit. In this prospective study, 41 patients between the ages of 18 and 22 were divided into two groups, one of which had just begun fixed orthodontic treatment (New) and the other have passed five months from the beginning of the treatment (Old). All patients got traditional oral hygiene training with oral hygiene kit and (SENSODYNE) toothpaste. The plaque index (PI) and Marginal bleeding index (MBI) were measured. Both groups showed increase in plaque values without statistical differences. Whereas MBI showed statistical differences (P=0.010) at new group only after 4 weeks. We think that oral hygiene enforcement at each visit not enough to improve oral hygiene status and need to seek for other strategies to enhance oral health like using software reminders.

Abstract:Abstract Objective: to evaluate the oral hygiene habits among patients experiencing fixed orthodontic treatment Material and method: The study was conducted in the orthodontic department of Multan medical and dental college, Multan. One hundred and five patients undergoing fixed orthodontic treatment were included in the study. Duration of study was 5months and 12 days. Patients were asked to fill out a questionnaire about the reason for undergoing treatment, the importance of orthodontic treatment in life, and oral hygiene practice after having fixed orthodontic treatment. Statistical data were analyzed by SPSS version 22 to determine the frequency distribution and percentage ratio for each variable. A Chi-square test was applied to compare gingivitis and frequency of brushing and a p-value ≤ 0.05 was considered significant. Results: The mean age of participants was 20± 4.7 years, with females predominant (71%). Most of the patients were undergoing treatment due to a friend’s suggestion (32%) and a dentist’s suggestion (17%). Most patients brushed twice daily (25%) or after every meal (24.4%) and still have gingivitis which represents brushing alone is not enough. Chi- square test was utilized between frequency of brushing and gingivitis and p- value was > 0.05 which is considered insignificant. The most preferred toothbrush was the general toothbrush (28%) and unspecified brush (23%). Which means proper knowledge of the type of brush was lacking. Only 41% of patients were sure about using fluoridated toothpaste which again shows a lack of awareness. 35% of patients were using a toothpick and 22% were using pins to clean their teeth in absence of a brush which again shows a lack of proper knowledge. 37% of patients were using their mouth wash and 24% were using interdental brushes occasionally. Conclusion: orthodontic treatment itself causes plaque retention and gingivitis so in order to maintain good oral hygiene self-motivation and proper knowledge given by the care provider is very necessary to improve gingival outcomes.

Patients' Perspectives and Attitudes About the Relationship Between Fixed Orthodontic Treatment and Oral Hygiene

Abstract:Aim: The aim of this study was to evaluate patients' attitudes and perspectives on the relationship between fixed orthodontic treatment and oral hygiene. Material and methods: The study included 175 patients undergoing fixed orthodontic treatment. Data was collected through questionnaires that included patient demographics, oral hygiene behaviours, perspectives on white spot lesions (WSL) and periodontal problems, and attitudes towards orthodontic treatment. The chi-square test was used to analyse the relationship between categorical data. The significance level was set at p<0.05. Results: The mean age of the patients (109 males, 66 females) was 17.8±6.4 years. Most of the patients who received orthodontic treatment brushed their teeth twice a day. Traditional and orthodontic toothbrushes were preferred over electric ones. The most common oral hygiene tools were toothbrushes and interdental brushes. A total of 151 patients (86.28%) indicated that their primary source of oral hygiene guidance was the orthodontist. A total of 113 patients (64.57%) had no knowledge of dental plaque, while 117 patients (66.85%) had no knowledge of WSL. While 80 patients (84%) with knowledge about periodontal problems believed that the primary cause of periodontal problems was inadequate oral hygiene, 43 patients (74.85%) with knowledge about WSL attributed the main cause of WSL to the use of braces. Negative attitudes towards orthodontic treatment and oral hygiene were more prevalent among adolescents (p<0.05), with no difference in gender or treatment duration. Patients who received information about WSL and periodontal problems exhibited a more positive attitude towards treatment and oral hygiene (p<0.05). Conclusion: This study highlights the limited patient awareness of plaque and WSL during fixed orthodontic treatment. Physician guidance has a significant impact on patient attitudes, and specific educational programmes are required in orthodontic treatment.
Assessment of oral self-care in patients with periodontitis: a pilot study in a dental school clinic in Japan

Abstract:Background: Oral hygiene education is central to every stage of periodontal treatment. Successful management of periodontal disease depends on the patient's capacity for oral self-care. In the present study, the oral self-care and perceptions of patients attending a dental school clinic in Japan were assessed using a short questionnaire referring to existing oral health models. Methods: A cross-sectional study design was used. The study population consisted of sixty-five patients (age range 23-77) with chronic periodontitis. The pre-tested 19-item questionnaire comprised 3 domains; 1) oral hygiene, 2) dietary habits and 3) perception of oral condition. The questionnaire was used as a part of the comprehensive assessment. Results: Analyses of the assessment data revealed no major problems with the respondents' perceived oral hygiene habits, although their actual plaque control levels were not entirely adequate. Most of the respondents acknowledged the importance of prevention of dental caries and periodontal diseases, but less than one third of them were regular users of the dental care system. Twenty-five percent of the respondents were considered to be reluctant to change their daily routines, and 29% had doubts about the impact of their own actions on oral health. Analyzing the relationships between patient responses and oral hygiene status, factors like 'frequency of tooth brushing', 'approximal cleaning', 'dental check-up' and 'compliance with self-care advice' showed statistically significant associations (P < 0.05) with the plaque scores. Conclusion: The clinical utilization of the present questionnaire facilitates the inclusion of multiple aspects of patient information, before initiation of periodontal treatment. The significant associations that were found between some of the self-care behaviors and oral hygiene levels document the important role of patient-centered oral health assessment in periodontal care.
Brushing Characteristics and Dietary Patterns of Orthodontic Patients with Fixed Appliances

Abstract:Background: Proper brushing practices are required to maintain good oral hygiene. This is of utmost importance in patients with fixed orthodontic appliances as these appliances can facilitate plaque accumulation and make cleaning process difficult. Hence if good oral hygiene is not maintained with these appliances then deterioration of gingival and periodontal health will result. Frequent intake of acidic drinks and sweet food can further harm the teeth. Aim: To determine tooth brushing characteristics of orthodontic patients with fixed appliances. Determine dietary patterns of such patients. Determine other oral hygiene measures used by them. Study Design: Cross sectional study Duration of the study: 6 months Methodology: Questionnaires were conveniently distributed among 104 orthodontic patients who met the inclusion criteria and provided consent to provide the information. Data collected was analyzed using SPSS 2020. Results: Most (70%) of the responding patients brush two times daily. Correct brush type (soft) is in use by 77% of the individuals. Approximately 64-66% of individuals combine vertical and horizontal technique of tooth brushing, use interdental toothbrush and report that they hardly intake fizzy drinks. However, only 55% patients have reported brushing for total duration of 2 minutes. Around 30% of the respondents have frequent intake of junk food and sugary food. Practical Implication: According to our knowledge, this study is the first study to date to evaluate the brushing habits and dietary patterns in Pakistani orthodontic patients with fixed appliances. Conclusions: The present study shows that majority of the patients show good brushing practices but need to improve their total duration of brushing. Frequent intake of junk food, sweets and fizzy drinks are also avoided by most patients. Use of fluoride mouthwashes and floss was seen in lesser proportion of surveyed patients. Abstract:Background: Orthodontic treatment requires good oral hygiene for successful completion of treatment. As protocol, patients are usually given instructions for oral hygiene and diet at the start of treatment, however, they are not fully followed. Different methods are employed in order to increase patient compliance including digital means, however, these are not possible in teaching hospitals with high burden of patient care and limited resources. The present study aims to correlate the patient reported behavior with their clinical findings and treatment need. This will enable us to identify potential sources of motivation which will be incorporated in daily practice and enable us to improve methods to enhance patient's behavior. Method: A cross-sectional study was conducted in the orthodontic department of a semi-government teaching hospital from August to October 2023 using a modified questionnaire. The clinical examination was done using a Community Periodontal Index for Treatment Need-C (CPITN-C) probe. The diagnosis of presenting clinical conditions and treatment need was done using Community Periodontal Index for Treatment Need (CPITN) and Gingival Bleeding Index (BI). Data collected was analyzed for frequencies and correlation was done using Spearman Correlation Coefficient. P- value ≤ 0.05 was taken as statistically significant. Result: The sample size consisted of 110 patients of which 60% were entitled to receive treatment. The predominant age group was 15-20 years (39.1%). Approximately 70% patients were in code 1 for CPITN and GI and in need of oral hygiene instructions. Overall patients' showed good level of awareness, however, they were not compliant in behavior. Conclusion: Although patients showed a good level of awareness towards oral hygiene practices, there was a lack of compliance in following them. Patients were more concerned for being affected by dental caries due to poor oral hygiene than its effect on overall treatment outcome.

Abstract:A survey of current knowledge about oral hygiene has been carried out, with emphasis on practical measures of proven value for orthodontic patients. The review covers various aspects of toothbrushing, complementary aids (dental floss, single-tufted brushes, toothpicks, water irrigators, disclosing agents, salivary stimulation, tooth positioners) and chemical cleaning (urea peroxide, chlorhexidine). Some advice is presented to facilitate teaching the patients to understand, master, and practice good oral hygiene, using toothbrushing with a horizontal scrub technique as the basis of the oral hygiene program. The unique potential of the orthodontist to make his patients hygiene minded and the longrange benefits of such awareness, as shown by recent clinical and experimental findings, are stressed.

Abstract:Abstract Introduction The positive attitude and behaviour of dental students can be improved during their undergraduate studies and is considered an essential factor in promoting the oral self-care habits of their patients and society in general. Aim This study was conducted to evaluate the oral health knowledge, attitudes and behaviours among undergraduate dental students at a private university, based on the year of undergraduate studies. Material and methods A self-administered questionnaire based on the Hiroshima University-Dental Behavioural Inventory was distributed among 783 undergraduate dental students from 1st to 5th year. Statistical analysis was performed using SPSS version 20. The Mann–Whitney U test and one-way ANOVA tests (Kruskal–Wallis test) were used. Results The scores of oral health knowledge, attitude and behaviour between preclinical and clinical dental students were found to have statistically significant differences ( P < 0.001). The variation of knowledge, attitude and behaviour scores from 1st to 5th year undergraduate studies was shown to be statistically significant with the year of study ( P < 0.001). The study showed significant improvement in the knowledge, attitude and behaviour of the final year dental students as compared with the 1st year dental students. Conclusion Preclinical students need properly designed oral health educational programs to increase their attitude and behaviour toward oral health.
Development and validation of the Oral health behavior questionnaire for adolescents based on the health belief model (OHBQAHBM)

Abstract:Abstract Background Oral health belief is a prerequisite of changing oral health behaviors especially during adolescence. However, there is a paucity of well-established questionnaire for use among adolescents. This study aimed to develop and validate an instrument to evaluate adolescents’ beliefs about oral health behaviors using health belief model. Methods A preliminary 43-item questionnaire was developed by an expert panel. Then the questionnaire was finalized by decreasing the number of items to 35 by analyzing the results from face validity and factor analysis from 421 Hong Kong secondary school students. The content validity were evaluated by a panel of 2 behavioral scientists, 2 dentists, 2 schoolteachers and 10 adolescents. The construct validity of the questionnaire was assessed by performing exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). The Cronbach’s alpha coefficient, item-total correlation and intraclass coefficient were used to test its reliability. In addition, to confirm its applicability, multiple regression analysis and path analysis were used to evaluate the possibility of HBM as predictors for oral health behaviors and oral hygiene status. Results The initial analysis extracted six factors that jointly accounted for 62.47% of the variance observed. Based on CFA, the final version of the questionnaire consisted of 35 items and the data of the final version fitted the model well. The Cronbach’s alpha coefficient for the subscale (> 0.7), item-total correlations (0.47–0.91) and the intraclass coefficient (0.82–0.91) were all above acceptable thresholds. The results of multiple regression analysis and path analysis confirmed its ability to predict oral health behaviors and status. Conclusions The present findings indicate satisfactory validity, reliability and applicability of the proposed Oral Health Behavior Questionnaire for Adolescents based on the Health Belief Model (OHBQAHBM) for measuring oral health beliefs of adolescents. This questionnaire can be used as an instrument to measure oral health beliefs and predict oral health behavior and oral hygiene status of adolescents.

[Does the oral microbiota evolve differently during orthodontic treatment with vestibular multi-attachment fixed appliances versus aligners? A systematic review of the literature]

Abstract:Introduction: Oral microbiota of patients is impacted during orthodontic treatment. The objective of this systematic review was to observe the evolution of oral microbiota (primary objective) and periodontal health (secondary objective) during orthodontic treatment, and to compare these changes during treatment with vestibular fixed appliances and aligners. Materials and methods: In accordance with PRISMA guidelines, an electronic search was performed in four databases until January 2022, completed by a manual search, including all prospective controlled studies, randomized or not, on the subject. Two independent authors were involved in the selection of studies, and a third author was consulted in case of disagreement. The Cochrane Collaboration's tool and ROBINS-I tool was used to assess the risk-of-bias in randomized and non-randomized trials, respectively. Finally, the risk of bias graphs were made with the robvis visualization tool. Results: Out of the 994 results obtained from these searches, 11 eligible articles were included (4 randomized clinical trials and 7 non-randomized controlled studies) with varying levels of bias. Results suggested that patients treated with aligner appliances have more favorable microbial flora and less biofilm mass during their treatment compared with those treated with fixed appliances. In addition, inflammatory marker cytokines and periodontal indices were higher in fixed orthodontic treatment compared to aligners treatment. Conclusion: Considering the limitations of this systematic review of the literature, the results suggested that aligners have a more favorable impact on the oral microbiota and periodontium compared to vestibular fixed appliances. Abstract:Objectives: The purpose of the study is to investigate the subjective oral status perception and OHIP according to the oral health managementamong international and domestic university students. Methods: self-reported questionnaire was completed by 176 domestic and 175 international university students in Daegu and Gyeongbuk from August 25 to November 1, 2014. The informed consent was received after explanation of the purpose of the study by the researcher. The questionnaire consisted of general characteristics of the subjects, toothbrushing behavior, oral health management, subjective oral status perception. and oral health impact profile (OHIP). The subjective oral status perception and OHIP was measured by Likert 5 point scale. Results: The international students tended to have higher subjective oral status perception when they had longer stay with the roommate in Korea. The female international students tended to have higher OHIP than the male students. The domestic students with lower grades and roommates showed higher OHIP. The international students had a poorer practice of oral health management than domestic students. The overall mean of subjective oral status perception was 3.13 in domestic students, and 3.09 and 3.22 in international students. The overall mean of OHIP was 4.21 in domestic students and 4.25 and 4.16 in the international students, Conclusions: International students had a higher subjective oral status perception than domestic students, but their wrong oral health management lowered their quality of life. It is necessary to provide the oral health management for the international students continuously.

Abstract:Objectives: This study analyzed the factors influencing satisfaction with clinical practice among dental hygiene students. Methods: A questionnaire survey was conducted from September 11 to September 22, 2023. Data were analyzed using descriptive analyses, t-test, ANOVA, Pearson's correlation, and multiple regression with the SPSS 20.0 program. Results: Factors influencing clinical practice satisfaction were major satisfaction, clinical practice stress, interpersonal relationships, critical thinking tendencies, and communication skills. Conclusions: It is necessary to establish effective strategies to increase critical thinking tendencies and interpersonal skills, and to prepare teaching and learning methods to improve the major satisfaction and communication skills of dental hygiene students.

Abstract:Background: Orthodontic treatment involves the use of appliances such as braces or aligners to straighten teeth and improve bite. Maintaining good oral hygiene during treatment is crucial to prevent the development of dental caries and periodontal disease. Methods: A literature search was conducted using PubMed, Cochrane Library, and Google Scholar for studies published between 2018 and 2022. The search terms used included "orthodontic treatment," "oral hygiene," "dental caries," and "periodontal disease." Inclusion criteria for studies were: (1) written in English, (2) published in peer-reviewed journals, (3) focused on orthodontic treatment and oral hygiene, and (4) included original research. Exclusion criteria were: (1) case reports, (2) reviews or meta-analyses, (3) not focused on orthodontic treatment or oral hygiene. Results: A total of 28 studies were included in the systematic review. The majority of the studies were observational in nature and conducted on adolescent patients. The studies consistently demonstrated that poor oral hygiene during orthodontic treatment is associated with an increased risk of dental caries and periodontal disease. Specifically, a poor oral hygiene status was found to be associated with a higher incidence of white spot lesions, gingivitis, and periodontitis during treatment with fixed appliances. Conclusion: The results of this systematic review demonstrate the importance of oral hygiene in orthodontic treatment. Patients undergoing orthodontic treatment should be educated about the importance of maintaining good oral hygiene to prevent the development of dental caries and periodontal disease. Dentists and orthodontists should closely monitor the oral hygiene status of their patients during treatment and provide appropriate interventions to improve oral hygiene and prevent the development of oral health complications.

Abstract:Background: Patient’s perception of own facial appearance is also an important contributing factor for treatment planning. Also, knowledge and attitude of patients to orthodontic treatment is becoming more important in orthodontics these days. The study aimed to assess self-perception, awareness and attitude towards orthodontic treatment among dental students and interns. Methods: Data was collected through cross-sectional survey conducted among the dental students and dental interns in KIST Medical College through online google form. Study duration was from August 2021 - October 2021. The data was entered in SPSS software 20. Frequency distribution was generated for all variables. Categorical variables were evaluated using Chi-square test. Results: About 96.7% of the participants believe that orthodontic treatment will improve facial appearance. 61.5% believe that orthodontic treatment is a lengthy and painful procedure. 44.5% does not think orthodontic treatment weakens the teeth. 50% of female believe that orthodontic treatment will give the promising result. Statistically significant difference was obtained with respect to preference of different types of braces among students of different years. 54.9% of the participants are satisfied with the arrangements of their teeth, 20.9% had avoid smiling and 22.5% felt lack of confidence because of the appearance of their teeth. Almost half of the participants (50.5%) think they should have orthodontic treatment and 76.2% readily agree to orthodontic treatment if suggested by the dentist. Conclusions: Dental students and interns had a good knowledge about orthodontic treatment and positive attitude towards it. Gender difference exists regarding the awareness of orthodontic treatment. Statistically significant difference was found in preference of different types of braces in students of different years and interns.

Abstract:Objective The aim of this study was to assess the relationship between the oral health habit and halitosis. It can provide an advisory basis for nurse giving individualized oral health instruction to patients of halitosis and can improve the result of prevention and treatment for halitosis accordingly. Methods 202 patients with periodontal disease were chosen to participate in this study. The study included the questionnaire of oral health habit and the halitosis diagnosis by organoleptic assessment.Comparisons of difference in the knowledge of oral health habit between the subjects with and without halitosis were conducted. Results Brushing times in subjects without halitosis were significantly more than that in subjects with halitosis. Brushing teeth in evening and interproximal brush usage were much better in subjects without halitosis than that in subjects with halitosis,respectively. Conclusions Good oral health habits could decrease the incidence of halitosis.
