Accessible design features and home modifications to improve physical housing accessibility: A mixed-methods survey of occupational therapists

Abstract:Background: Despite the number of older people and people with disabilities increasing in Australia, it is unclear which housing design features are needed to support physical housing accessibility for people with and without disabilities across the lifespan. Objective: This study drew upon the experience of occupational therapists to investigate accessible housing design features and home modifications to support aging in place and discharge from hospital to home. Methods: A cross-sectional online survey exploring housing design features and home modifications was completed by 144 Australian occupational therapists over six weeks in 2021. Descriptive quantitative and qualitative data analyses were used. Results: For both aging in place and hospital discharge, the most important housing design features included step-free access to the dwelling, large step-free showers, and bathroom and bedroom space on the ground floor. Qualitative findings also highlighted the importance of preparing for home modifications, such as reinforcing bathroom walls to support the post-build installation of grab rails. The most frequently needed modifications were for bathroom features, while structural changes to the dwelling were the most time-intensive modifications, requiring more than six weeks to be completed. Conclusions: External access to the home and internal access to bedroom and bathroom facilities can support aging in place and hospital discharge and mitigate the need for costly and time-intensive home modifications. While this study was conducted in Australia, the findings have relevance outside of this context, and are important for ensuring equitable accessibility for people with and without disabilities across the lifespan.

Abstract:As part of a knowledge-transfer project consisting of a series of three talks on the topic of healthy ageing and ageing in place, we explored what participants (older adults, students, the general public, as well as professionals in architecture, urban planning and property management) consider to be key requirements for ageing in place and healthy ageing. Feedback is captured using survey questionnaires and a post-talk discussion group. Safety, a comfortable and spacious environment, age-friendly facilities and meeting the needs of older adults, the availability of caring support and home maintenance services were the most frequently mentioned desirable features of ageing in place. Future models for different types of support for ageing in place may be explored by management companies working with the residents themselves, to develop a sustainable business model.

Abstract:Aging individuals may face difficulty with independently navigating and interacting with their home environment. Evidence-based interventions promoting home modifications are needed to support aging-in-place across the lifespan. This study identified the facilitators and barriers to implementing home modifications from the perspectives of residents and professionals (N = 16). Guided by a social-ecological model, researchers utilized directed content analysis of focus group interviews. While participants discussed facilitators and barriers mainly on the individual level, factors were presented at the relationship, community, and societal level of the model. Overall, the findings suggest a potential for targeted interventions on all levels of the model to promote adoption of home modifications.

Designing Interiors to Mitigate Physical and Cognitive Deficits Related to Aging and to Promote Longevity in Older Adults: A Review

Abstract:Background: With the increasing global population of older adults, there is a need for environmental interventions that directly affect their physical, psychological, and emotional well-being to help them maintain or regain their independence and autonomy - all of which promote longevity. Methods: To better understand potential opportunities and challenges associated with interior design and "future homes" that may promote well-being, aging in place, and independent living in older adults, the authors reviewed relevant literature and included their own expert opinions from a multidisciplinary point of view including interior design, wellness, and engineering. Results: After summarizing existing environmental interventions for the aging population and their effectiveness, this review reveals knowledge gaps in interior design for the well-being and longevity of older adults followed by a discussion of opportunities for future research that may fill these gaps. Some of these opportunities include finding habilitative design strategies that identify and address unique situational needs of each user, advancing multidisciplinary fields such as environmental gerontology that recreate security and independence for older adults even outside of their homes, implementing technically advanced design strategies, which are flexible and adaptive to individual needs; and integrating the Internet of things (IoT) into living environments, including voice-activated command technologies to improve seniors' central role in enabling an optimized healthcare ecosystem. Conclusions: Knowledge of current evidence regarding the impact of different environmental factors may hasten adaptation of well-designed innovations that can provide optimal healing and living environments for the aging population. By effectively addressing older adults' unique and specialized needs, design practitioners can become an indispensable part of their medical, social, and environmental team. One of the rapidly developing infrastructures promising to revolutionize the design of "future homes" is the IoT. While it is at an early stage of development, ultimately we envisage a connected home using voice-controlled technology and Bluetooth-radio-connected add-ons, to augment much of what home health does today. Bringing these approaches together into an effective strategy for a model of effective geriatric care is important and needs to become an integral part of both design education and practice.

Abstract:Australia's population is ageing, and a key question is how can people remain socially active as they grow older? To answer this question, this paper offers a critical review of the literature looking at the mechanisms through which an urban neighbourhood environment can influence social life, an important component of social health. The aim of this review is to develop a general framework that can be applied to the studies on the social health of the elderly. The framework illustrates the main elements that potentially promote social life, namely: a safe, accessible, walkable and relatively dense neighbourhood in addition to green spaces, third places, soft edges and streets. This framework is then applied to a small case study, looking particularly at the social life of the elderly and how it can be supported by the physical urban neighbourhood environment. Data are collected by conducting in-depth interviews with 19 elderly residents (aged 65 years and over) in a neighbourhood located in the city of Gold Coast, Queensland. The results from this study demonstrate the importance of neighbourhood design in the social life of the elderly, interviewed. The study also identifies some of the key barriers to the elderly's social interaction.
Phylogenetic relationships among european Polistes and the evolution of social parasitism (Hymenoptera: Vespidae, Polistinae)

Abstract:Cladistic analysis of the European species ofPolistes is used to investigate the evolution .of social parasitism in the genus. The three species of social parasites (formerly the genus Sulcopolistes) are all inquilines: lacking a worker caste, and dependent on uSUIping the colony of a host species to obtain a worker force. El:vlERY'S Rule states that social parasites are more closely related to their hosts than to any other species. Previously published allozyme (CARPENTER et aI., 1993) and mtDNA (CHOUDHARY et aI., 1994) data did not support this hypothesis, but did not resolve relationships among the species of social parasites. Morphological characters are adduced which resolve the phylogenetic relationships among these three species, and the combination of the morphological and molecular data sets largely resolves the relationships among all nine European species. Cladistic optimization of traits associated with social parasitism on the resulting cladogram shows: (1) El:vlERy's'Rule is rejected; (2) the scenario proposed by TAYLOR (1939) for the evolution of social parasitism is not supported either. The predatory behavior of the inquiline P. atn"mandibularis, with separate "supply" and "nursery" nests, is evidently secondary, as .. is its initially passive invasion behavior. La phylogenie des Polistes d'Europe et revolution du parasitisme social (Hymenoptera: Vespidae, Polistinae) L'analyse cladistique des especes de Polistes europeennes est utilisee pour etudier l'evolution du parasitisme social dans Ie geme. A Critical Approach to Aging in Place: A Case Study Comparison of Personal and Professional Perspectives from the Minneapolis Metropolitan Area

Abstract:The concept of aging in place attracts older adults, scholars, policymakers, and service providers alike. Interviews with 125 independent-dwelling men and women (mean age 71 years) and ten policymakers/community service providers queried for elements of urban and suburban contexts that strengthen or weaken desires and abilities to age in place. Overall, interviewees emphasized the need for accessible and affordable housing, reliable services, robust transportation infrastructure, and suitable options for health and care. Perspectives of low-income participants diverged notably from mainstream conceptions: those in perilous-subsidized housing desired to move to safer and more comfortable settings and homeless participants did not have a stable home or community to age in place. Planning and implementation of aging in place framed in highly individualistic, resourced, and ableist conception did not address their everyday struggles, including the lack of affordable housing and defunding of supportive social services. The manuscript complicates idealized notions of aging in place and suggests new theoretical and empirical directions to expand the concept to become more inclusive and socially just.

Abstract:Worldwide, growth in the older population creates a pressing need to develop supportive environments that enhance quality of life as people age. Too often, built environments present barriers and challenges to older adults that compromise independent living and adversely affect health and life outcomes. Designing homes, buildings, and neighborhoods with older adults, through exercises in participatory or co-design, could help ensure that environments are better able to facilitate healthy aging. However, while it is potentially advantageous to involve this age group in environmental design decisions, doing so can be difficult. Analysis of and guidance on effective ways to involve older adults in these activities could make the challenge easier. With this aim in mind, this article provides critical perspectives on eight “less traditional” engagement techniques—walking interviews, photovoice, photo-elicitation, Talking Mats®, participatory mapping, drawing, model-making, and the “Design Fair”. Insights into the strengths and limitations of these techniques, gained from observation of their use in participatory design activities, as well as feedback collected from older co-design participants, are presented. The article concludes by offering a number of practical recommendations for those interested in designing age-friendly homes and neighborhoods with older people.

Abstract:Journal of Applied Gerontology, Ahead of Print. Housing solutions for older adults aim at providing a safe environment to live in. The construction is heavily based on aspects of physical safety, often disregarding the social aspects of feeling safe. The older adults themselves are looking for a living environment that supports their individual capacities and social networks. The living environment can contribute to their physical, social, and emotional safety by providing accessible housing and spaces for community building. Interviews with older adults who had recently lived through a building fire highlighted the importance of the community in daily life, in emergency as well as in recovery from hazards. The results show that community building is related to access to common-use spaces and daily interaction with neighbors. The shared spaces in the immediate surroundings can enhance community cohesion and generate peer support. The common-use spaces and public facilities in urban environment have a significant role in emergencies and in the process of recovery from adversities.

Abstract:In 2005, the Healthcare Information Management Systems Society (HIMSS) Nursing Informatics Community developed a survey to measure the impact of health information technology (HIT), the I-HIT Scale, on the role of nurses and interdisciplinary communication in hospital settings. In 2007, nursing informatics colleagues from Australia, England, Finland, Ireland, New Zealand, Scotland and the United States formed a research collaborative to validate the I-HIT across countries. All teams have completed construct and face validation in their countries. Five out of six teams have initiated reliability testing by practicing nurses. This paper reports the international collaborative's validation of the I-HIT Scale completed to date.

Abstract:Background: There are few studies that focus explicitly on the impact of the home environment on older adults' sedentary behaviour (SB) and physical activity (PA) using the socio-ecological model (SEM). This study aims to investigate older adults' PA and SB within the home environment integrating the SEM as a theoretical framework. Methods: A qualitative exploratory research design was employed to conduct 33 in-depth interviews (IDIs) and five focus group (FGs; n = 16) with multi-diverse ethnic older adults (mean age 72±5 years). Using reflexive thematic analysis themes were generated from the data set and were interpreted using the SEM. Results: The findings indicate that different levels of the SEM had an impact on older adults' PA and SB. These include the 1) Individual level: Attitude, perception and motivation 2) Interpersonal level: Family and Friends: a motive to remain active 3) Organisational level: healthcare institutes, 4) Community level factors: Significance of social groups, 5) Physical Environment: Microenvironment and 6) Policy level factors (lockdown restrictions and healthcare system). This model can be utilised to foster activity within the home by focusing on the facilitators and barriers identified at each of these levels of influence. Conclusion: The study findings suggest that modifying PA and SB in the home environment is complex and is influenced across different levels of the SEM. Therefore, a holistic approach is required that integrates these multiple influences. This understanding can inform the design of interventions that seek to optimize PA and minimize SB within the home environment.

Abstract:"Ageing in place" is a policy initiative strongly advocated by the World Health Organization to face the challenge of an ageing population. This pilot study used a "photovoice" approach, aiming to explore aspects of the housing environment considered by older people as important in facilitating ageing in place. It enabled participants to express their ideas through photographs. Each participant was asked to take photos that illustrated age-friendly features they considered crucial for supporting their lives in the community. A total of 44 older people participated in the pilot study, and 300 photos were collected. Participants were invited to describe the reasons for taking these photos by filling in a journal sheet. A semi-structured interview was then conducted with individual participants, who were asked to elaborate on the meaning of their photos. The analysis revealed three themes: (1) age-friendly housing design; (2) supportive neighborhood; and (3) connection to family and the community. These three themes are pillars of an age-friendly city, which are important to seniors to facilitate ageing in place.

Abstract:Population ageing presents significant challenges for many countries, one of which is the provision of adequate housing. Developing understanding of the needs and preferences of ageing societies will be crucial in order to assist in the provision of suitable housing and communities that are sustainable in the long term. While a preference to ‘age in place’ is clear in the literature, comparatively less academic research is available on older people’s preferences for more specific housing and environment attributes. The aim of this study is to identify the main housing and environment characteristics that are linked to the health and wellbeing of the elderly and determine the preferences for such characteristics via a survey with UK residents aged 55+. The results indicate a strong preference for independent living and an increasing desire for bungalows in later life. Housing conditions, energy efficiency, thermal comfort, and home adaptions to facilitate ageing in place are particularly important housing characteristics to older people. The location and environment are also key drivers of housing preferences; a safe neighbourhood, accessibility to amenities, public transport, and a clean and walkable environment are particularly important. Preferences varied with age, but gender has a less significant impact on the preferences expressed. The findings of this study will be valuable for stakeholders engaged in housing policy and provision for older people.

Dwellings occupied by mobility-limited older people emerge as strong control centers and more age-friendly places

Abstract:The future will witness the substantial worldwide growth of older people with functional limitations or disabilities who have difficulties leaving their dwellings and traveling to their neighborhoods or other community destinations to realize their obligatory and discretionary needs and goals. This commentary offers conceptual arguments and literature findings proposing that the dwellings of this vulnerable population deserve new scrutiny because they have become more salient and positively experienced places to live where their occupants can maintain their independence and age in place. The catalyst for this commentary is the emergence of gerontechnological innovations relying on digital and sensor technologies, offering these older occupants a new category of dwelling connectivity solutions-constituting a paradigm shift-whereby goods, care, services, social supports, and information and leisure activities can be delivered to their houses and apartments. Incorporating this technological component has transformed their dwellings into dynamic "control centers," connecting their occupants in real-time with the resources and activities offered in other places. These solutions enable older people to cope more effectively with declines and losses because their ability to live independently is less threatened by challenges they face accessing destinations with inadequate transportation options and less age-friendly land use or physical design features. By occupying more supportive, safer, and connected dwellings, these older people have overall more positive and salient residential mastery emotional experiences and feel more competent and in control of their lives and environment. Planning or policy recommendations directed to the World Health Organization (WHO) and its age-friendly city/community agenda follow from its conclusions. They highlight how dwelling environments containing gerontechnological solutions are becoming more critical influences of "active aging." The commentary recommends that WHO allocates more resources to dwelling interventions that increase the awareness, availability, usability, and acceptability of these gerontechnological solutions, thus reducing the disincentives for older people to be adopters.

Abstract:Supporting people to 'age in place' – to live independently at home and remain connected to the community – is an international policy priority. But the process of ageing in place is mediated in a socio-cultural context where neoliberal tropes of successful ageing reproduce a pervasive model about 'ageing well' by elevating ideals of individualised choice and self-governance. Based on two waves of qualitative interviews and interim observations, we employ a Bourdieusian logic to explore the ramifications of this context on the experiences of 46 people in later older age (80+) ageing in place in North East England. All participants enacted everyday improvisatory practices to render their homes habitable. But our participants – most of whom were located in middle-class social positions – supplemented such improvisions with a strategic disposition to plan for and actively shape their ageing-in-place futures. Our participants conveyed a distinct sense of agency over their ageing futures. Underpinning their orientations to practice was an awareness of the value attached to individually 'ageing well' and a distancing from the agedness associated with the fourth age. Our analysis demonstrates the role of capital, accrued throughout the life course, in bringing such future trajectories into effect. The central argument of this paper therefore is that the embodiment of (neoliberal) ideals of successful ageing in place requires the deployment of classed capital. In sum, contrary to the individualising narratives ubiquitous in policy pertaining to ageing well, we show the importance of classed structural moorings in this process.

Abstract:Home modification or adaptation is an important global issue, especially for older people living with disabilities in ordinary housing of varying ages, and pre-dating concerns about accessibility in design. Comparisons of research from five developed nations identify common themes: variation in integrated service development; public and private financial investment; deferred health costs; workforce expansion and training; and the value of the user perspective. Further discussion identifies the importance of retrofitting alongside new build, and argues for sustainable housing that recognizes population aging but also issues of climate change and the need for more inclusive design of housing for all ages.

Abstract:A policy of aging in place should be accompanied by physical and social environments that support healthy aging. This article describes how a property development company in Hong Kong sought to elicit the views of older people and their caregivers towards elderly services through a market research company, using questionnaire surveys followed by focus groups. Over 80% of all participants rated healthy dietary habits and exercise, maintaining mental and spiritual health, and maintaining a generally healthy lifestyle as important. Current health concerns include long waiting times for care at public hospitals, lack of carer should dependency occur, and lack of information about what services are available in the community. Interests in services in their neighbourhood include medical care (82%), healthy lifestyle activities (66%), and home care support (55%). There was considerable interest in the provision of services that improve brain and physical function, as well as general health checks. Carers were willing to pay more for services compared with older adults themselves. The findings inform the development of pilot models of aging in place as a sustainable financial model.

Abstract:Introduction: Modification becomes a critical issue in a supportive home environment for older adults. Methods: This study examined if modifications in different spaces at home are facilitated to provide a supportive environment for older adults to feel in control in a cultural context. Data was utilized using field research with a mixed method design: Structured and open-ended interviews as well as surveys using a questionnaire. The hypothesis states that older adults have a supportive environment at home when the overall sense of control is associated with home modification. Results: The result reveals that overall sense of control with home modification and its components: kitchen, bedroom, living room, and reception room have a significant effect on older adults' wellbeing. Discussion: Therefore, older adult supported environment in Jordan can only take place by boosting awareness about the existing problems in the physical environment, by describing the benefits of home modification and by reducing the barriers of home modifications. This process can enhance the housing market and provide better urban policies for urban design and urban planning based on cultural heritage and values.

The poor design of housing for the elderly has led to the charge that the needs of aging people have not been adequately anticipated. There is little focus on design features that facilitate aging in place, hence improving the quality of life for the elderly, for example. This study examines the impact of interior housing design on the lives of the elderly, through a review of research on the subject and an evaluation of current design trends. Using a keyword search, 51 empirical studies from 2013 to 2022 that focus on housing for the elderly were identified and analyzed in terms of architectural features, home environment, and energy efficiency. The review found minimal connections have been made between interior design efforts and the actual housing needs of the elderly. This thematic review provides a summary of this literature for use by researchers and designers and recommends future studies for the use of inclusive designs in housing interiors in order to better meet the needs of elderly individuals.

Abstract:In article describes the choice of methods for caring for the elderly and the comfort of living spaces are closely linked to aging-friendly design. Aging leads to a decline in physical functions, resulting in numerous obstacles in life. Therefore, the article analyzes the activity of housing for the elderly, taking aging-friendly design as a starting point. The development status of elderly housing needs in management, safety, social interaction, respect, and other aspects are examined. Design strategies conducive to aging, in terms of lighting, ventilation, detailed environment, public spaces, and intellectual services, are proposed. This aims to enhance comfort, safety, and livability in housing for the elderly and ensure the quality of life for older people. With the rapid growth in demand for elderly care, the vacancy rate in elderly housing reaches 50%. Due to changes in social and family structure, most elderly people live alone and lack security. Elderly homes have become ideal places for them. Currently, most care facilities lack professional management, and the services provided are too uniform. This leads to a weak sense of identity, with resources remaining underutilized. For semi-disabled individuals, standardized conditions do not always meet their expectations for rehabilitation care, medical services, and spiritual comfort. Elderly people face behavioral disorders affecting sight, hearing, smell, taste, and touch. Declines in the five senses, including speech impairments, mobility difficulties, and physiological disorders, lead to psychological needs such as socialization and self-realization. Aging-friendly design is based on the physiological and psychological cognitive characteristics of elderly people. It aims to create an environment that meets their needs and ensures effective care for them. The aim of the study is to analyze the problems associated with designing buildings for the elderly and to determine strategies for creating a supportive design that considers the needs and characteristics of the target audience. The work is aimed at improving the comfort, safety, and quality of life of elderly people by providing them with an appropriate living environment.
