[Social inequality in home care: A scoping review on the impact of socio-economic resources on home care arrangements]

Abstract:Introduction: Inequalities in long-term home care are still rarely considered in the discourse on health inequalities, although there is reason to assume that opportunities for a successful home care arrangement are not equally distributed among those in need of it. This paper pursues the question how socio-economic resources of people in need of care and their family caregivers are influencing the utilization of care services in Germany. Methods: A scoping review has been conducted to analyse the current state of research. To identify relevant papers the online databases CINAHL including MEDLINE, PubMed, LIVIVO and Web of Science were searched and supplemented by internet research. According to Bourdieu, the research studies included were arranged in economic, educational and social resources. Results: 29 qualitative and quantitative research papers were included in the analysis. 14 papers represent quantitative research results, nine papers are based on qualitative research. Six further papers are analyses based on an international data record. The utilization of care services increases with the level of income and wealth as well as education. On closer examination of individual care services, however, the evidence is inconsistent and only the so-called 24-hour care service is distinguishable as a care arrangement for high-status groups. A compensation of the lower utilization of professional care by informal support cannot be described in quantitative terms. Caregivers with low income and education levels seem to be more likely to not only provide care but also to provide higher-intensity care. Discussion: Despite increasing research reflected in the literature, the evidence remains incomplete and shows inconsistencies so that a valid statement on the degree of inequalities in care provision is not possible. A conceptual basis for the definition of social inequality in the context of long-term home care is lacking as well as a common understanding of equity in care provision. The perspective of people in need of care and their caregivers has hardly been addressed. Conclusion: Home care is not only determined by individual need but seems to be decisively influenced by socio-economic restrictions. For a more targeted approach, further research on the use of care services depending on socio-economic resources is needed, explicitly taking into account the user perspective.

Abstract:BackgroundWe see a growing number of older adults receiving long-term care in industrialized countries. The Healthcare Utilization Model by Andersen suggests that individual need characteristics influence utilization. The purpose of this study is to analyze correlations between need characteristics and service utilization in home care arrangements.Methods1,152 respondents answered the questionnaire regarding their integration of services in their current and future care arrangements. Care recipients with high long-term care needs answered the questionnaire on their own, the family caregiver assisted the care recipient in answering the questions, or the family caregiver responded to the questionnaire on behalf of the care recipient. They were asked to rank specific needs according to their situation. We used descriptive statistics and regression analysis.ResultsRespondents are widely informed about services. Nursing services and counseling are the most used services. Short-term care and guidance and training have a high potential for future use. Day care, self-help groups, and mobile services were the most frequently rejected services in our survey. Women use more services than men and with rising age utilization increases. Long waiting times and bad health of the primary caregiver increases the chance of integrating services into the home care arrangements.ConclusionThe primary family caregiver has a high impact on service utilization. This indicates that the whole family should be approached when offering services. Professionals should react upon the specific needs of care dependents and their families.

Socioeconomic Inequalities in Home-Care Use Across Regional Long-term Care Systems in Europe

Abstract:Objectives: We examine whether socioeconomic inequalities in home-care use among disabled older adults are related to the contextual characteristics of long-term care (LTC) systems. Specifically, we investigate how wealth and income gradients in the use of informal, formal, and mixed home-care vary according to the degree to which LTC systems offer alternatives to families as the main providers of care ("de-familization"). Method: We use survey data from SHARE on disabled older adults from 136 administrative regions in 12 European countries and link them to a regional indicator of de-familization in LTC, measured by the number of available LTC beds in care homes. We use multinomial multilevel models, with and without country fixed-effects, to study home-care use as a function of individual-level and regional-level LTC characteristics. We interact financial wealth and income with the number of LTC beds to assess whether socioeconomic gradients in home-care use differ across regions according to the degree of de-familization in LTC. Results: We find robust evidence that socioeconomic status inequalities in the use of mixed-care are lower in more de-familized LTC systems. Poorer people are more likely than the wealthier to combine informal and formal home-care use in regions with more LTC beds. SES inequalities in the exclusive use of informal or formal care do not differ by the level of de-familization. Discussion: The results suggest that de-familization in LTC favors the combination of formal and informal home-care among the more socioeconomically disadvantaged, potentially mitigating health inequalities in later life.
Informal and formal long-term care utilization and unmet needs in Europe: examining socioeconomic disparities and the role of social policies for older adults

Abstract:The rising number of older adults with limitations in their daily activities has major implications for the demands placed on long-term care (LTC) systems across Europe. Recognizing that demand can be both constrained and encouraged by individual and country-specific factors, this study explains the uptake of home-based long-term care in 18 European countries with LTC policies and pension generosity along with individual factors such as socioeconomic status. Using data from the Survey of Health, Ageing and Retirement in Europe conducted in 2019, we apply a two-part multilevel model to assess if disparities in use of LTC are driven by disparities in needs or disparities in use of care when in need. While individual characteristics largely affect the use of care through its association with disparities in need, country-level characteristics are important for the use of care when in need. In particular, the better health of wealthier and more educated individuals makes them less likely to use any type of home-based personal care. At the country level, results show that the absence of a means-tested benefit scheme and the availability of cash-for-care benefits (as opposed to in-kind) are strongly associated with the use of formal care, whether it is mixed (with informal care) or exclusive. LTC policies are, however, shown to be insufficient to significantly reduce unmet needs for personal care. Conversely, generous pensions are significantly associated with lower unmet needs, underscoring the importance of considering the likely adverse effects of future pension reforms.

Abstract:This article addresses inequalities in elderly care from the perspective of family caregivers. By applying an intersectional perspective, we examine the interdependencies of, and interrelations between, the four structural categories of family caregivers’ socio-economic status, gender, ethnicity and employment status, as well as differences in their ability to cope with providing care. We have analysed 20 in-depth semi-structured interviews with family carers in Germany. A high socio-economic status alone did not prove to be sufficient for the good functioning of care arrangements, and ethnicity as a dividing line turned out to be of limited importance. The core result is a typology of five ‘types of coping with care’, three of them with ‘rather successful’ and two with ‘rather precarious’ coping strategies. The types differ in the way difference categories interact. Across all types, it became evident that self-care orientation, self-care action as well as self-determination and control are central for a ‘rather successful’ coping.
Inequities in home care use among older Canadian adults: Are they corrected by public funding?

Abstract:Background Although care use should parallel needs, enabling and predisposing circumstances including the socio-demographic inequities of socioeconomic status (SES), gender, or isolation often intervene to diminish care. We examine whether availability of state-funded medical and support services at home can rebalance these individual and social inequities, and do this by identifying if and how intersecting social identities predict homecare use among older Canadian adults. Methods Using the Canadian Longitudinal Study on Aging (CLSA) of 30,097 community-dwelling adults aged 45 to 85, we performed recursive partitioning regression tree analysis using Chi-Squared automatic interaction detection (CHAID). Combinations of individual and social characteristics including sociodemographic, family-related, physical and psychological measures and contextual indicators of material and social deprivation were explored as possible predictors of formal and informal care use. Results Diminished function i.e. increased need, indicated by Activities of Daily Living, was most strongly aligned with formal care use while age, living arrangement, having no partner, depression, self-rated health and chronic medical conditions playing a lesser role in the pathway to use. Notably, sex/gender, were not determinants. Characteristics aligned with informal care were first—need, then country of birth and years since immigration. Both ‘trees’ showed high validity with low risk of misclassification (4.6% and 10.8% for formal and informal care, respectively). Conclusions Although often considered marginalised, women, immigrants, or those of lower SES utilised formal care equitably. Formal care was also differentially available to those without the financial or human resources to receive informal care. Need, primarily medical but also arising from living arrangement, rather than SES or gender predicted formal care, indicating that universal government-funded services may rebalance social and individual inequities in formal care use.

Abstract:Ziel Pflegebedürftige Menschen in Deutschland werden im Rahmen der häuslichen Pflege hauptsächlich von ihren Angehörigen versorgt, die dabei unterschiedliche Entlastungs- und Unterstützungsangebote in Anspruch nehmen. Ziel dieser Studie ist es, die Häufigkeit der tatsächlichen Nutzung sowie die gewünschte Nutzung von ambulanten Entlastungs- und Unterstützungsangeboten zu ermitteln. Als Ursache für die Pflegebedürftigkeit werden dabei Demenzerkrankungen und Nicht-Demenzerkrankungen unterschieden. Methodik Die Daten stammen aus einer repräsentativen Stichprobe pflegender Angehöriger von gesetzlich versicherten Pflegebedürftigen, die vom MD Bayern zur Pflegegradeinstufung begutachtet wurden (N=958). Die Nutzung folgender ambulanter Entlastungs- und Unterstützungsangebote wurde untersucht: ambulanter Pflegedienst, Haushaltshilfe, Tagespflege, Essen auf Rädern, Fahrdienst, Betreuungsdienst, 24-Stunden-Betreuung und Betreuungsgruppe. Charakteristika der pflegebedürftigen Personen, der pflegenden Angehörigen und der Pflegesituation wurden erfasst. Unterschiedsanalysen wurden mittels Chi2-Tests sowie t-Tests durchgeführt. Ergebnisse Die Inanspruchnahme der ambulanten Unterstützungsangebote fiel trotz hoher Pflegebelastung der pflegenden Angehörigen eher gering aus: Sie reichte von 1,7% bei der Betreuungsgruppe bis 38,4% beim ambulanten Pflegedienst. Über 40% der Befragten nutzten keines der acht Angebote. Von diesen Nicht-Nutzern hatten jedoch 72% den Wunsch, in Zukunft mindestens eines dieser Angebote nutzen zu wollen. Die Haushaltshilfe und der ambulante Pflegedienst waren von den Nicht-Nutzern die am häufigsten gewünschten Angebote. Bei Demenzerkrankungen war die tatsächliche und gewünschte Nutzung, insbesondere bei Tagespflege, Betreuungsgruppe und Betreuungsdienst, häufiger als bei anderen Ursachen der Pflegebedürftigkeit. Schlussfolgerung Der Wunsch nach Nutzung ist deutlich höher als die Wirklichkeit der Inanspruchnahme, die bei ambulanten Entlastungsangeboten von niedrigen Nutzungsraten geprägt ist. Es gilt, die Ursachen für diese Diskrepanz zu erforschen. Daraus sollten wirksame Strategien entwickelt werden, die pflegenden Angehörigen so zu beraten, dass passende Entlastungsangebote die pflegenden Familien auch erreichen und damit die häusliche Pflegesituation stärken. Aim In Germany, people in need of care are mainly cared for by their relatives who make use of various outpatient relief and support services. The aim of this study was to determine the frequency of actual use as well as the desired use of outpatient relief and support services. Dementia and non-dementia as causes behind need for care are distinguished. Methods A representative sample of informal caregivers of statutorily insured care recipients assessed by the MD Bayern during application for a care level classification (n=958) was analyzed. The use of the following outpatient relief and support services was investigated: outpatient care service; domestic help; day care centre; meals on wheels; driving service; care service; 24-hour care; and care group. Characteristics of the care receiver, the informal caregiver and the care situation were recorded. Difference analyses were carried out using Chi2 tests and t-tests. Results The use of outpatient support services was low despite the high care burden on informal caregivers ranging from 1,7% for the care group to 38,4% for the outpatient care service. More than 40% of respondents did not use any of the eight services. However, from this non-user group, 72% had a desire to use at least one of the eight services in the future. Abstract:Hintergrund In Deutschland können pflegebedürftige Personen durch Leistungen der Pflegekassen unterstützt werden. Dafür ist die offizielle Feststellung der Pflegebedürftigkeit notwendig, die seit Inkrafttreten des zweiten Pflegestärkungsgesetzes durch die Zuweisung von Pflegegraden – 1 bis 5 – erfolgt. Mit der Einführung der Pflegegrade sollten unter anderem Menschen mit Demenz stärker berücksichtigt werden. In diesem Beitrag werden die Pflegegrade durch Merkmale der Angehörigen- und Versorgungssituation beschrieben, um den Ist-Zustand abzubilden und Ansatzpunkte zur bedarfsorientierten Versorgung zu bieten. Methodik Die Datenerhebung erfolgte im Rahmen der Querschnittsstudie "Benefits of being a caregiver" durch eine bayernweite schriftliche Befragung pflegender Angehöriger zu Aspekten der Pflegesituation und dem gegenwärtigen und zukünftig gewünschtem Nutzungsverhalten von 15 Versorgungsangeboten. Zum Vergleich der Pflegegrade wurden Gruppenunterschiede mittels χ2-Tests und einfaktoriellen Varianzanalysen ermittelt. Die Effektstärkenmaße Odds Ratio und Cohen's d wurden bei signifikanten Gruppenunterschieden zum Vergleich zwischen den Pflegegraden 1 und 4 angegeben. Die Stichprobe umfasste 958 Fälle pflegebedürftiger Personen im Alter von mindestens 65 Jahren. Ergebnisse Der Anteil von Menschen mit Demenz stieg mit dem Pflegegrad signifikant an. Bei den pflegenden Angehörigen nahmen die subjektive Belastung, funktionale Copingstrategien und das Pflegemotiv die pflegebedürftige Person nicht in ein Heim geben zu wollen, mit dem Pflegegrad zu. Bei höherem Pflegegrad wandten pflegende Angehörige signifikant mehr Zeit für die Unterstützung bei Aktivitäten des täglichen Lebens und für Beaufsichtigung auf; sie erhielten häufiger informelle Hilfe und wohnten häufiger mit der pflegebedürftigen Person zusammen. Bei 9 der 15 untersuchten Versorgungsangebote (z. B. 24-h-Betreuung) stieg der gegenwärtige Nutzungsgrad signifikant mit Zunahme des Pflegegrads an. Insgesamt lag die Nutzungsrate der Angebote jedoch über alle Pflegegrade hinweg auf einem niedrigen Niveau ( M =2,20; SD =1,90). Schlussfolgerung Infolge des zweiten Pflegestärkungsgesetzes wird ein guter Zugang zu Leistungen der Pflegekasse für Menschen mit Demenz ermöglicht. Der steigende Zeitaufwand für die Unterstützung bei Aktivitäten des täglichen Lebens und für Beaufsichtigung wird durch die Pflegegrade abgebildet. Trotz steigender Belastung wurden Entlastungsangebote selten genutzt. Es empfiehlt sich, Maßnahmen zur Erhöhung der Inanspruchnahme von Entlastungs- und Beratungsangeboten auszubauen. One of the reasons for introducing care levels was to pay more attention to the needs of people with dementia. In this article, characterization of care levels were based on characteristics of care recipients, their relatives, and the care situation. The aim was to map the current state of health care and providing practical suggestions to improve health care. Methods Data was collected as part of the cross-sectional study "Benefits of being a caregiver" by a written survey of informal caregivers throughout Bavaria on aspects of the caregiving situation as well as on current and desired future use of 15 care services. Abstract:We performed an optical study on the electronic properties of perovskite zirconate AZrO3 (A = Ca and Ba) nanocrystals (NCs), which depend strongly on the size of A ion as well as the post-annealing process. The zirconate NCs were synthesized in the combustion method, which was found to be quite effective for synthesizing highly orthorhombic CaZrO3 NCs in the sense that their grain sizes are relatively larger and as-synthesized NCs do not include any raw material phase in the XRD pattern, compared with the cubic BaZrO3 NCs. The room-temperature visible emission is commonly quite strong in both NCs. Interestingly, the dominance of the green-yellow emission over the violet-blue emission with increasing the post-annealing temperature (Tanneal) appears to be in agreement with the formation of the mid-gap state identified in the absorption spectra. While the optical bandgaps of NCs increased with increasing Tanneal, a systematic trend that the optical bandgap in the AZrO3 NCs decreased with the increasingly larger size of the A ion occurred. We discuss our findings in relation to the oxygen vacancy formation, lattice distortion, and size effect.
Home care in Europe: a systematic literature review

Abstract:BackgroundHealth and social services provided at home are becoming increasingly important. Hence, there is a need for information on home care in Europe. The objective of this literature review was to respond to this need by systematically describing what has been reported on home care in Europe in the scientific literature over the past decade.MethodsA systematic literature search was performed for papers on home care published in English, using the following data bases: Cinahl, the Cochrane Library, Embase, Medline, PsycINFO, Sociological Abstracts, Social Services Abstracts, and Social Care Online. Studies were only included if they complied with the definition of home care, were published between January 1998 and October 2009, and dealt with at least one of the 31 specified countries. Clinical interventions, instrument developments, local projects and reviews were excluded. The data extracted included: the characteristics of the study and aspects of home care 'policy & regulation', 'financing', 'organisation & service delivery', and 'clients & informal carers'.ResultsSeventy-four out of 5,133 potentially relevant studies met the inclusion criteria, providing information on 18 countries. Many focused on the characteristics of home care recipients and on the organisation of home care. Geographical inequalities, market forces, quality and integration of services were also among the issues frequently discussed.ConclusionsHome care systems appeared to differ both between and within countries. The papers included, however, provided only a limited picture of home care. Many studies only focused on one aspect of the home care system and international comparative studies were rare. Furthermore, little information emerged on home care financing and on home care in general in Eastern Europe. This review clearly shows the need for more scientific publications on home care, especially studies comparing countries. A comprehensive and more complete insight into the state of home care in Europe requires the gathering of information using a uniform framework and methodology.

Factors associated with preferences for long-term care settings in old age: evidence from a population-based survey in Germany

Abstract:BackgroundLong-term care is one of the most pressing health policy issues in Germany. It is expected that the need for long-term care will increase markedly in the next decades due to demographic shifts. The purpose of this study was to investigate the factors associated with preferences for long-term care settings in old age individuals in Germany.MethodsBased on expert interviews and a systematic review, a questionnaire was developed to quantify long-term care preferences. Data were drawn from a population-based survey of the German population aged 65 and over in 2015 (n = 1006).ResultsIn multiple logistic regressions, preferences for home care were positively associated with providing care for family/friends [OR: 1.6 (1.0–2.5)], lower self-rated health [OR: 1.3 (1.0–1.6)], and no current need of care [OR: 5.5 (1.2–25.7)]. Preferences for care in relatives’ homes were positively associated with being male [OR: 2.0 (1.4–2.7)], living with partner or spouse [OR: 1.8 (1.3–2.4)], having children [OR: 1.6 (1.0–2.5)], private health insurance [OR: 1.6 (1.1–2.3)], providing care for family/friends [OR: 1.5 (1.1–2.0)], and higher self-rated health [OR: 1.2 (1.0–1.4)]. Preferences for care in assisted living were positively associated with need of care [OR: 1.9 (1.0–3.5)] and higher education [for example, University, OR: 3.5 (1.9–6.5)]. Preferences for care in nursing home/old age home were positively associated with being born in Germany [OR: 1.8 (1.0–3.1)] and lower self-rated health [OR: 1.2 (1.0–1.4)]. Preferences for care in a foreign country were positively associated with lower age [OR: 1.1 (1.0–1.2)] and being born abroad [OR: 5.5 (2.7–11.2)].ConclusionsNumerous variables used are sporadically significant, underlining the complex nature of long-term care preferences. A better understanding of factors associated with preferences for care settings might contribute to improving long-term care health services.

Abstract:Person-centered care demands the evaluation of needs and preferences of the patients. In this study, we conducted a segmentation analysis of a large sample of older people based on their bio-psycho-social-needs and functioning. The aim of this study was to clarify differences in health care use and costs of the elderly in Germany.Data was derived from the 8-year follow-up of the ESTHER study—a German epidemiological study of the elderly population. Trained medical doctors visited n = 3124 participants aged 57 to 84 years in their home. Bio-psycho-social health care needs were assessed using the INTERMED for the Elderly (IM-E) interview. Further information was measured using questionnaires or assessment scales (Barthel index, Patients Health Questionnaire (PHQ) etc.). The segmentation analysis applied a factor mixture model (FMM) that combined both a confirmatory factor analysis and a latent class analysis.In total, n = 3017 persons were included in the study. Results of the latent class analysis indicated that a five-cluster-model best fit the data. The largest cluster (48%) can be described as healthy, one cluster (13.9%) shows minor physical complaints and higher social support, while the third cluster (24.3%) includes persons with only a few physical and psychological difficulties ("minor physical and psychological complaints"). One of the profiles (10.5%) showed high and complex bio-psycho-social health care needs ("complex needs") while another profile (2.5%) can be labelled as "frail". Mean values of all psychosomatic variables—including the variable health care costs—gradually increased over the five clusters. Use of mental health care was comparatively low in the more burdened clusters. In the profiles "minor physical and psychological complaints" and "complex needs", only half of the persons suffering from a mental disorder were treated by a mental health professional; in the frail cluster, only a third of those with a depression or anxiety disorder received mental health care.The segmentation of the older people of this study sample led to five different clusters that vary profoundly regarding their bio-psycho-social needs. Informal and formal care preferences and expected willingness of providing elderly care in Germany: protocol for a mixed-methods study

Abstract:Introduction In Germany, the number of elderly people in need of care is expected to increase from 2.4 million in 2015 to 3.2 million in 2030. The subsequent rise in demand for long-term care facilities is unlikely to be met by the current care structures and available staff. Additionally, many Germans still prefer to be cared for at home for as long as possible. In light of recent changes, such as increasing employment rates of women and growing geographical distances of family members, informal caregiving becomes more challenging in the future. The aim of this study is to explore preferences for informal and formal care services in the German general population, as well as the expected willingness of providing elderly care. Methods and analysis A mixed-methods approach will be used to explore care preferences and expected willingness of providing elderly care in the German general population. A systematic literature review will be performed to provide an overview of the current academic literature on the topic. Qualitative interviews will be conducted with informal caregivers, care consultants and people with no prior caregiving experiences. A labelled discrete choice experiment will be designed and conducted to quantitatively measure the preferences for informal and formal care in the German general population. People between 18 and 65 years of age will be recruited in cooperation with a (regional) statutory health insurance (AOK Lower Saxony). A mixed multinomial logit regression model and a latent class finite mixture model will be used to analyse the data and test for subgroup differences in care preferences. Ethics and dissemination The study has been approved by the Committee for Clinical Ethics of the Medical School in Hannover. Data will be treated confidential to ensure the participants' anonymity. The results will be discussed and disseminated to relevant stakeholders in the field. Trial registration number DRKS00012266.

The roles of employment status and income in the mental health of informal caregivers in Germany

Abstract:Background: Informal caregivers often experience multiple negative consequences as a result of the informal care they provide. Among other factors, employment status, financial resources, and mental health are related to informal caregiving. This analysis examined the association between informal caregivers' employment status and their mental health, as well as the moderating effect of net household income on this relationship. Methods: The research question was addressed with data from the German Socio-Economic Panel (SOEP) survey, comprising 3,053 informal caregivers (1,007 male; 2,046 female). Data were obtained through self-reports, and mental health was measured with the Summary Scale Mental Score. Stepwise adjusted multiple linear regression models were used to examine the association between employment status and mental health. The moderating effects were tested with interaction terms. All analyses were also stratified for gender. Results: Informal caregivers with full-time jobs reported better mental health than unemployed or marginally employed caregivers (β = 0.077, p < 0.001). The significant interaction term for full-time (β=-0.066, p = 0.001) and part-time workers (β=-0.066, p = 0.003) indicated a moderating effect of net household income on the association between employment status and mental health. This finding was especially evident in women. Conclusions: Employment appears to be a relevant protective factor for informal caregivers' mental health. However, if informal caregivers are not employed, a low net household income might additionally restrict their mental health. Therefore, welfare policy structures must be created to reduce the negative financial consequences for informal caregivers and enable them to achieve work-life-care balance.

Abstract:Abstract Background Most older people, and especially those in need of long-term care, suffer from one or more chronic diseases. Consequently, older people have an increased need of medical care, including specialist care. There is little evidence as yet whether older people with greater medical care needs obtain adequate medical care because existing studies do not sufficiently control for differences in morbidity. In this study we investigate whether differences in medical specialist utilization exist between older people with and without assessed long-term care need in line with Book XI of the German Social Code, while at the same time controlling for individual differences in morbidity. Methods We used data from the 11 German AOK Statutory Health and Long-term Care Insurance funds of 100,000 members aged 60 years or over. Zero-inflated Poisson regression analyses were applied to investigate whether the need for long-term care and the long-term care setting are associated with the probability and number of specialist visits. We controlled for age, gender, morbidity and mortality, residential density, and general practitioner (GP) utilization. Results Older people in need of long-term care are more likely to have no specialist visit than people without the need for long-term care. This applies to nearly all medical specialties and for both care settings. Yet, despite these differences in utilization probability the number of specialist medical care visits between older people with and without the need for long-term care is similar. Conclusion Older people in need of long-term care might face access barriers to specialist care. Once a contact is established, however, utilization does not differ considerably between those who need long-term care and those who don’t; this indicates the importance of securing an initial contact.

What determines the use of home care services by elderly people?

Abstract:The objective of the present study was to investigate the determinants of use of statutory and private home care services by older people living in the community. A questionnaire was distributed to a stratified random sample of 2,000 elderly people living in the community registered with 11 general practices in a British city (equal numbers of men and women, aged 65-74 years, and 75 years or over). The outcome measures were the use of statutory or private home care services in the previous 3 months. Logistic regression was used to explore potential determinants of the use of these services. The response rate was 79%. Increasing age, not owning a car and being a widow(er) were associated with greater use of both statutory and private home care services, as was worse self-reported overall health. Worse physical functioning, worse emotional health, problems with cognition, foot problems and a greater number of falls were determinants of use of statutory and private services. Older age on leaving full-time education was associated with increased use of private home care services. Problems with eyesight were determinants for both types of home care services for women, but only private services for men. For women, leakage of urine was associated with greater use of private services. Social networks and social support were not generally associated with use of these services after controlling for demographic factors. Understanding the determinants for the use of both statutory and private home care services is important because of the increasing numbers of elderly people in the population and the policy to maintain older people in their own homes. Purchasers and providers should be able to address at least some of the modifiable predictors.

Abstract:Background: Although the increasing dissemination and use of health-related information on the Internet has the potential to empower citizens and patients, several studies have detected disparities in the use of online health information. This is due to several factors. So far, only a few studies have examined the impact of socio-economic status (SES) on health information seeking on the Internet. This study was designed to identify sociodemographic and health-(care-)related differences between users and non-users of health information gleaned from the Internet with the aim of detecting hard-to-reach target groups. Methods: This study analyzed data from the NRW Health Survey LZG.NRW 2011 (n = 2,000; conducted in North Rhine-Westphalia, Germany, via telephone interviews). Logistic regression analysis was used to examine the determinants of online health information seeking behavior. Results: 68% of Internet users refer to the Internet for health-related purposes. Of the independent variables tested, SES proved to exert the strongest influence on searching the Internet for health information. The final multivariate regression model shows that people from the middle (OR: 2.2, 95% CI: 1.6-3.2) and upper (OR: 4.0, 95% CI: 2.7-6.2) social classes are more likely to seek health information on the Internet than those from the lower class. Also, women are more likely to look for health information on the Internet than men (OR: 1.5, 95% CI: 1.1-2.1). Individuals with a migration background are less likely to conduct health searches on the Internet (OR: 0.6, 95% CI: 0.4-0.8). Married people or individuals in a stable relationship search the Internet more often for health information than do singles (OR: 1.9, 95% CI: 1.2-2.9). Also, heavy use of health-care services compared to non-use is associated with a higher likelihood of using the Internet for health-related matters (OR: 1.7, 95% CI: 1.2-2.5). Conclusions: In order to achieve equity in health, health-related Internet use by the socially deprived should be promoted through measures to increase their level of e-health literacy. Furthermore, longitudinal studies are needed in order to gain reliable data/results on determinants of health-related Internet use.
Relationships between the shedding of Coxiella burnetii, clinical signs and serological responses of 34 sheep

Abstract:Two abortions associated with Coxiella burnetii occurred in a group of 34 pregnant ewes. The seroprevalence of C bumetii infection was studied by using an ELISA and the immunofluorescence (IF) assay was applied to the contents of vaginal swabs. In addition, a KCR assay, with primers based on a transposonlike repetitive region of the C bumetii genome (trans-PCR), was used for the highly sensitive and specific detection of C bumetii in vaginal swabs, milk and faeces. Of the 34 animals tested at parturition, eight (24 per cent) were positive by ELISA, 11 (32 per cent) were positive by IF, and 15 (44 per cent) were positive when the vaginal swab extract was subjected to the trans-PCR assay. C bumetii was therefore detected by PCR in the vaginal swabs of seven seronegative ewes. However, five weeks after lambing, 16 (47 per cent) of the animals tested were ELISA positive but only two animals (6 per cent) were positive by PCR. Among the ELISAand PCR-positive animals, eight (25 per cent) shed coxiella in their milk and six (18 per cent) did so in their faeces.

Abstract:<p>Large cross‐country variation in long‐term‐care (LTC) policy in conjunction with household‐level data on caregiving provides a valuable laboratory for policy analysis. However, comprehensive cross‐country data on how care is provided are lacking. In order to close this gap, we draw on data from the Survey of Health, Ageing, and Retirement in Europe (SHARE) and the Health and Retirement Study (HRS) in the U.S. Since care hours are missing for some care forms (especially for nursing‐home residents), we propose a selection model to impute these. The model allows selection into care forms to differ by country. Our estimates imply that nursing‐home residents have higher care needs, even when conditioning on observed characteristics. In contrast to the bulk of the literature, we also take into account care provision from persons in the same household, which we find to contribute one‐third of all care hours. Informal‐care provision in Europe follows a steep North‐South gradient, the U.S. falling in between Central‐European and Southern‐European countries.
