Abstract:AbstractBackgroundAlzheimer’s disease (AD) is a progressive and debilitating neurological disorder that impacts millions of individuals globally. Prompt and accurate detection and diagnosis of Alzheimer’s disease are pivotal for providing effective care and management to patients. However, early identification of the disease remains challenging due to the complexity of diagnostic procedures and the similarity of symptoms with age‐related cognitive decline. With the world transitioning towards a more digital‐focused environment, there is an increasing potential to utilize human‐computer interaction technologies to aid in the early detection of Alzheimer’s disease.MethodWe recruited 40 healthy individuals, 40 patients with mild cognitive impairment (MCI), and 40 patients with early AD. By incorporating fusion perception‐enhancing technologies into intelligent identification devices, such as eye‐tracking and body motion detection, peripheral modal biometric information can be gathered from subjects.ResultThe study found that in human‐computer interaction experiments using eye‐tracking, the patients with early AD showed significantly longer completion times and free rates of eye motion and movement trajectory than the normal group and those with MCI. The correlation between eye motion trajectory and movement trajectory was notably lower as compared to the normal group and the MCI group. During the human‐computer interaction physical movement experiment, the AD group recorded lower target accuracy rate and screen click rate as compared to the normal group and patients with MCI. Similarly, in the human‐computer interaction clock drawing experiment, the AD group demonstrated longer completion time and pen‐up thinking time, as well as lower clock drawing scores. Furthermore, the total completion time of human‐computer interaction tasks was notably shorter as compared to conventional cognitive screening tests. In conclusion, peripheral biomarkers obtained from the aforementioned human‐computer interaction task were analyzed using machine learning techniques, resulting in the identification of early‐stage Alzheimer’s disease with a classification accuracy of 91.1%.ConclusionHence, this non‐invasive and accurate technique may prove instrumental in identifying potential cognitive deficits associated with this disease while also being widely applicable for screening early AD.
Effect of maternal social support on attachment: experimental evidence.

Abstract:The present study used an experimental format to evaluate the influence of maternal social support on the development of attachment. 46 WIC primigravidas were randomly assigned to an experimental or control condition. Volunteer coaches, trained to provide maternal support and information, met with experimentals prenatally and during the first postpartum year. Experimentals and controls did not differ on demographics, ego level, verbal ability, affect, or perceived social support. At 14 months, experimental infants scored higher on an Attachment Ratings cluster from Waters and Deane's new Q-sort procedure. This cluster was more sensitive than a Criterion Sort measure in detecting the effect of the manipulation and as or more sensitive to maternal affect, perceived support, and the HOME. These findings provide experimental evidence regarding the importance of social support on infant attachment and support for an alternative approach for reducing data from the attachment Q-sort.

Abstract:AIM To illustrate with the example of a secondary data analysis study the use of the multiple imputation method to replace missing data. BACKGROUND Most large public datasets have missing data, which need to be handled by researchers conducting secondary data analysis studies. Multiple imputation is a technique widely used to replace missing values while preserving the sample size and sampling variability of the data. DATA SOURCE The 2004 National Sample Survey of Registered Nurses. REVIEW METHODS The authors created a model to impute missing values using the chained equation method. They used imputation diagnostics procedures and conducted regression analysis of imputed data to determine the differences between the log hourly wages of internationally educated and US-educated registered nurses. DISCUSSION The authors used multiple imputation procedures to replace missing values in a large dataset with 29,059 observations. Five multiple imputed datasets were created. Imputation diagnostics using time series and density plots showed that imputation was successful. The authors also present an example of the use of multiple imputed datasets to conduct regression analysis to answer a substantive research question. CONCLUSION Multiple imputation is a powerful technique for imputing missing values in large datasets while preserving the sample size and variance of the data. Even though the chained equation method involves complex statistical computations, recent innovations in software and computation have made it possible for researchers to conduct this technique on large datasets. IMPLICATIONS FOR RESEARCH/PRACTICE The authors recommend nurse researchers use multiple imputation methods for handling missing data to improve the statistical power and external validity of their studies.

Abstract:Background: Alzheimer's disease (AD) affects a significant portion of the aging population, presenting a serious challenge due to the limited availability of effective therapies during its progression. The disease advances rapidly, underscoring the need for early diagnosis and the application of preventative measures. Current diagnostic methods for AD are often expensive and invasive, restricting access for the general public. One potential solution is the use of biomarkers, which can facilitate early detection and treatment through objective, non-invasive, and cost-effective evaluations of AD. This review critically investigates the function and role of biofluid biomarkers in detecting AD, with a specific focus on cerebrospinal fluid (CSF), blood-based, and saliva biomarkers. Results: CSF biomarkers have demonstrated potential for accurate diagnosis and valuable prognostic insights, while blood biomarkers offer a minimally invasive and cost-effective approach for diagnosing cognitive issues. However, while current biomarkers for AD show significant potential, none have yet achieved the precision needed to replace expensive PET scans and CSF assays. The lack of a single accurate biomarker underscores the need for further research to identify novel or combined biomarkers to enhance the clinical efficacy of existing diagnostic tests. In this context, artificial intelligence (AI) and deep-learning (DL) tools present promising avenues for improving biomarker analysis and interpretation, enabling more precise and timely diagnoses. Conclusions: Further research is essential to confirm the utility of all AD biomarkers in clinical settings. Combining biomarker data with AI tools offers a promising path toward revolutionizing the personalized characterization and early diagnosis of AD symptoms.

Abstract:Abstract Blood-biomarkers for Alzheimer’s Disease and related neuropathological processes, including amyloidosis, tau deposition, and neurodegeneration, may revolutionize the monitoring of risk and progression of neurocognitive outcomes in older adults. At visit 5 (2011-13) of the ARIC study, a multi-center longitudinal cohort study, amyloid-beta (Aβ42:Aβ40), ptau-181, glial fibrillary acidic protein (GFAP), and neurofilament light (NFL) were measured in plasma using an ultrasensitive single-molecule array (Simoa; Quanterix N4PE + p-tau-181 single-plex) in a sample of 1,711 participants (Age=76±5 years, 60% Female, 28% Black). Neurocognitive outcomes included adjudicated dementia and mild cognitive impairment (MCI) through 2019 for a median follow-up of 5.6 years. Multinomial logistic regressions estimated Relative Risk Ratios (RRR) of the four-category outcome of MCI (n=543), dementia (n=371), and death (n=59) vs. cognitively unimpaired (n=736) for each base(2) log-transformed biomarker, adjusted for age, sex, education, site-race, hypertension, diabetes, BMI, APOE, and eGFR. When estimates appeared similar for MCI and dementia, an additional analysis modeled RRR of a three-category outcome of combined MCI/dementia, death, and cognitively unimpaired. Higher plasma Aβ42:Aβ40 ratio was associated with lower MCI/dementia risk (RRR=0.48, 95%CI: 0.32,0.73). Higher levels of all other biomarkers were associated with higher risk of MCI [p-tau181 (RRR=1.57, 95%CI: 1.23,2.00); GFAP (RRR=1.31, 95%CI: 1.03,1.67); NFL (RRR=1.64, 95%CI: 1.26,2.13)] and dementia [p-tau181 (RRR=3.16, 95%CI: 2.43,4.11); GFAP (RRR=2.40, 95%CI: 1.80,3.20); NFL (RRR=3.62, 95%CI: 2.58,5.09)]. These results indicate blood-biomarkers representative of multiple neuropathological processes were strongly associated with both MCI and all-cause dementia outcomes, emphasizing their potential role as tools in risk identification, early diagnosis, and possibly guiding interventions.

Abstract:Background: There are no obvious clinical signs and symptoms in the early stages of Alzheimer’s disease (AD), and most patients usually have mild cognitive impairment (MCI) before diagnosis. Therefore, early diagnosis of AD is very critical. This paper mainly discusses the blood biomarkers of AD patients and uses machine learning methods to study the changes of blood transcriptome during the development of AD and to search for potential blood biomarkers for AD. Methods: Individualized blood mRNA expression data of 711 patients were downloaded from the GEO database, including the control group (CON) (238 patients), MCI (189 patients), and AD (284 patients). Firstly, we analyzed the subcellular localization, protein types and enrichment pathways of the differentially expressed mRNAs in each group, and established an artificial intelligence individualized diagnostic model. Furthermore, the XCell tool was used to analyze the blood mRNA expression data and obtain blood cell composition and quantitative data. Ratio characteristics were established for mRNA and XCell data. Feature engineering operations such as collinearity and importance analysis were performed on all features to obtain the best feature solicitation. Finally, four machine learning algorithms, including linear support vector machine (SVM), Adaboost, random forest and artificial neural network, were used to model the optimal feature combinations and evaluate their classification performance in the test set. Results: Through feature engineering screening, the best feature collection was obtained. Moreover, the artificial intelligence individualized diagnosis model established based on this method achieved a classification accuracy of 91.59% in the test set. The area under curve (AUC) of CON, MCI, and AD were 0.9746, 0.9536, and 0.9807, respectively. Conclusion: The results of cell homeostasis analysis suggested that the homeostasis of Natural killer T cell (NKT) might be related to AD, and the homeostasis of Granulocyte macrophage progenitor (GMP) might be one of the reasons for AD.

Abstract:Abstract Background With new drug treatments emerging, the need for early Alzheimer’s disease (AD) detection methods that can be done in a primary care setting has become urgent. Significant advances have been made in validating blood‐based AD biomarkers, but the absence of a validated, non‐invasive clinical indicator remains a barrier for determining those who might be optimal candidates for treatment. The digital clock drawing test (dCDT) has been previously shown to be related to AD PET biomarkers. However, it is unknown whether and the extent to which dCDT would predict AD as compared to blood‐based AD biomarkers. The current study used an interpretable machine learning approach to determine whether the dCDT, independently and in combination with blood‐based AD biomarkers (e.g., amyloid‐β 42 (Aβ42); total tau (T‐tau)), was predictive of incident AD. Method This study included participants from the Framingham Heart Study Generation 2 cohort (Gen 2; n = 527) who had dCDT measures, plasma Aβ42, and T‐tau. The fast interpretable greedy‐tree sums method was used to identify clinical decision rules for AD prediction. Synthetic minority oversampling technique was employed to handle the class imbalance. The predictive ability of plasma Aβ42, T‐tau and dCDT measures was evaluated by both area under the curve (AUC) and standardized partial AUC. The permutation feature importance method was used to show the importance of each measure. Result Interpretable clinical decision rules were generated as multivariate biomarker signatures for AD prediction ( Figure 1 ). The dCDT (AUC 0.859) was better at incident AD prediction than AD blood‐based biomarkers (AUC 0.839). The combination of dCDT, Aβ42, and T‐tau best predicted incident AD (AUC 0.884) ( Figure 2 ). The combination of dCDT and blood‐based biomarkers also has higher partial AUC than individual ones ( Table 1 ). The total score and information processing scores were the most important contributing dCDT measures. Conclusion These results highlight the efficiency of detecting early AD through the integration of digital measures as a clinical indicator combined with AD specific biomarkers. The interpretable decision rules have the potential to provide automated results that can be used in practical clinical settings, although external validation is warranted.

Driving Automation for Older Adults with Preclinical Alzheimer's Disease

Abstract:Introduction: Older adults with preclinical Alzheimer's disease (AD) show changes in on-road driving performance. The impact of preclinical AD on using automated vehicle (AV) technology is unknown. The aim was to evaluate safety and cognitive workload while operating AV technology in drivers with preclinical AD. Introduction: This cross-sectional study included 40 participants: 19 older adults (age 74.16 ± 4.78; MOCA scores 26.42 ± 2.52) with preclinical AD, evidenced by elevated cortical beta-amyloid; and 21 controls (age 73.81 ± 5.62; MOCA scores 28.24 ± 1.67). All participants completed two scenarios in a driving simulator. Scenario 1 included conditional automation with an emergency event that required a manual take-over maneuver. Scenario 2 was identical but with a cognitive distractor task. Emergency response time was the main safety outcome measure. Cognitive workload was calculated using moment-to-moment changes in pupillary size and converted into an Index of Cognitive Activity (ICA). Mann-Whitney U and independent t tests were used to compare group differences. Results: Emergency response times were similar between drivers with preclinical AD and controls in scenario 1 (20.85 s ± 1.08 vs. 20.52 s ± 3.18; p = 0.83) and scenario 2 (14.83 s ± 7.37 vs. 13.45 s ± 10.43; p = 0.92). Likewise, no differences were found in ICA between drivers with preclinical AD and controls in scenario 1 (0.34 ± 0.08 vs. 0.33 ± 0.17; p = 0.74) or scenario 2 (0.30 ± 0.07 vs. 0.29 ± 0.17; p = 0.93). Conclusions: Older drivers with preclinical AD may safely operate AV technology, without increased response times or cognitive workload. Future on-road studies with AV technology should confirm these preliminary results in drivers with preclinical AD.

Abstract:Alzheimer's disease (AD), a primary cause of dementia globally, is traditionally diagnosed via cerebrospinal fluid (CSF) measures and positron emission tomography (PET). The invasiveness, cost, and limited accessibility of these methods have led to exploring blood-based biomarkers as a promising alternative for AD diagnosis and monitoring. Recent advancements in sensitive immunoassays have identified potential blood-based biomarkers, such as Aβ42/Aβ40 ratios and phosphorylated tau (p-tau) species. This paper briefly evaluates the clinical utility and reliability of these biomarkers across various AD stages, highlighting challenges like refining plasma Aβ42/Aβ40 assays and enhancing the precision of p-tau, particularly p-tau181, p-tau217, and p-tau231. The discussion also covers other plasma biomarkers like neurofilament light (NfL), glial fibrillary acidic protein (GFAP), and synaptic biomarkers, assessing their significance in AD diagnostics. The need for ongoing research and development of robust assays to match the performance of CSF and PET biomarkers is underscored. Abstract:Background: The development of biomarkers that are easy to collect, process, and store is a major goal of research on current Alzheimer's Disease (AD) and underlies the growing interest in plasma biomarkers. Biomarkers with these qualities will improve diagnosis and allow for better monitoring of therapeutic interventions. However, blood collection strategies have historically differed between studies. We examined the ability of various ultrasensitive plasma biomarkers to predict cerebral amyloid status in cognitively unimpaired individuals when collected using acid citrate dextrose (ACD). We then examined the ability of these biomarkers to predict cognitive impairment independent of amyloid status. Methods: Using a cross-sectional study design, we measured amyloid beta 42/40 ratio, pTau-181, neurofilament-light, and glial fibrillary acidic protein using the Quanterix Simoa® HD-X platform. To evaluate the discriminative accuracy of these biomarkers in determining cerebral amyloid status, we used both banked plasma and 18F-AV45 PET cerebral amyloid neuroimaging data from 140 cognitively unimpaired participants. We further examined their ability to discriminate cognitive status by leveraging data from 42 cognitively impaired older adults. This study is the first, as per our knowledge, to examine these specific tests using plasma collected using acid citrate dextrose (ACD), as well as the relationship with amyloid PET status. Results: Plasma AB42/40 had the highest AUC (0.833, 95% C.I. 0.767-0.899) at a cut-point of 0.0706 for discriminating between the two cerebral amyloid groups (sensitivity 76%, specificity 78.5%). Plasma NFL at a cut-point of 20.58pg/mL had the highest AUC (0.908, 95% CI 0.851- 0.966) for discriminating cognitive impairment (sensitivity 84.8%, specificity 89.9%). The addition of age and apolipoprotein e4 status did not improve the discriminative accuracy of these biomarkers. Conclusion: Our results suggest that the Aβ42/40 ratio is useful in discriminating clinician-rated elevated cerebral amyloid status and that NFL is useful for discriminating cognitive impairment status. These findings reinforce the growing body of evidence regarding the general utility of these biomarkers and extend their utility to plasma collected in a non-traditional anticoagulant.

Abstract:Potential biomarkers for Alzheimer’s disease (AD) include amyloid β1–42 (Aβ1–42), t-Tau, p-Tau181, neurofilament light chain (NFL), and neuroimaging biomarkers. Their combined use is useful for diagnosing and monitoring the progress of AD. Therefore, further development of a combination of these biomarkers is essential. We investigated whether plasma NFL/Aβ1–42 can serve as a plasma-based primary screening biomarker reflecting brain neurodegeneration and amyloid pathology in AD for monitoring disease progression and early diagnosis. We measured the NFL and Aβ1–42 concentrations in the CSF and plasma samples and performed correlation analysis to evaluate the utility of these biomarkers in the early diagnosis and monitoring of AD spectrum disease progression. Pearson’s correlation analysis was used to analyse the associations between the fluid biomarkers and neuroimaging data. The study included 136 participants, classified into five groups: 28 cognitively normal individuals, 23 patients with preclinical AD, 22 amyloid-negative patients with amnestic mild cognitive impairment, 32 patients with prodromal AD, and 31 patients with AD dementia. With disease progression, the NFL concentrations increased and Aβ1–42 concentrations decreased. The plasma and CSF NFL/Aβ1–42 were strongly correlated (r = 0.558). Plasma NFL/Aβ1–42 was strongly correlated with hippocampal volume/intracranial volume (r = 0.409). In early AD, plasma NFL/Aβ1–42 was associated with higher diagnostic accuracy than the individual biomarkers. Moreover, in preclinical AD, plasma NFL/Aβ1–42 changed more rapidly than the CSF t-Tau or p-Tau181 concentrations. Our findings highlight the utility of plasma NFL/Aβ1–42 as a non-invasive plasma-based biomarker for early diagnosis and monitoring of AD spectrum disease progression.

Abstract:Physiological signals are immediate and sensitive to neurological changes resulting from the mental workload induced by various driving environments and are considered a quantifying tool for understanding the association between neurological outcomes and driving cognitive workloads. Neurological assessment, outside of a highly-equipped clinical setting, requires an ambulatory electroencephalography (EEG) headset. This study aimed to quantify neurological biomarkers during a resting state and two different scenarios of driving states in a virtual driving environment. We investigated the neurological responses of seventeen healthy male drivers. EEG data were measured in an initial resting state, city-roadways driving state, and expressway driving state using a portable EEG headset in a driving simulator. During the experiment, the participants drove while experiencing cognitive workloads due to various driving environments, such as road traffic conditions, lane changes of surrounding vehicles, the speed limit, etc. The power of the beta and gamma bands decreased, and the power of the delta waves, theta, and frontal theta asymmetry increased in the driving state relative to the resting state. Delta-alpha ratio (DAR) and delta-theta ratio (DTR) showed a strong correlation with a resting state, city-roadways driving state, and expressway driving state. Binary machine-learning (ML) classification models showed a near-perfect accuracy between the resting state and driving state. Moderate classification performances were observed between the resting state, city-roadways state, and expressway state in multi-class classification. An EEG-based neurological state prediction approach may be utilized in an advanced driver-assistance system (ADAS).

Abstract:The field of blood-based biomarkers for Alzheimer's disease (AD) has advanced at an incredible pace, especially after the development of sensitive analytic platforms that can facilitate large-scale screening. Such screening will be important when more sophisticated diagnostic methods are scarce and expensive. Thus, blood-based biomarkers can potentially reduce diagnosis inequities among populations from different socioeconomic contexts. This large-scale screening can be performed so that older adults at risk of cognitive decline assessed using these methods can then undergo more complete assessments with classic biomarkers, increasing diagnosis efficiency and reducing costs to the health systems. Blood-based biomarkers can also aid in assessing the effect of new disease-modifying treatments. This paper reviews recent advances in the area, focusing on the following leading candidates for blood-based biomarkers: amyloid-beta (Aβ), phosphorylated tau isoforms (p-tau), neurofilament light (NfL), and glial fibrillary acidic (GFAP) proteins, as well as on new candidates, Neuron-Derived Exosomes contents (NDEs) and Transactive response DNA-binding protein-43 (TDP-43), based on data from longitudinal observational cohort studies. The underlying challenges of validating and incorporating these biomarkers into routine clinical practice and primary care settings are also discussed. Importantly, challenges related to the underrepresentation of ethnic minorities and socioeconomically disadvantaged persons must be considered. If these challenges are overcome, a new time of cost-effective blood-based biomarkers for AD could represent the future of clinical procedures in the field and, together with continued prevention strategies, the beginning of an era with a lower incidence of dementia worldwide.

Abstract:INTRODUCTION With emergence of disease‐modifying therapies, efficient diagnostic pathways are critically needed to identify treatment candidates, evaluate disease severity, and support prognosis. A combination of plasma biomarkers and brief digital cognitive assessments could provide a scalable alternative to current diagnostic work‐up. METHODS We examined the accuracy of plasma biomarkers and a 10‐minute supervised tablet‐based cognitive assessment (Tablet‐based Cognitive Assessment Tool Brain Health Assessment [TabCAT‐BHA]) in predicting amyloid β positive (Aβ+) status on positron emission tomography (PET), concurrent disease severity, and functional decline in 309 older adults with subjective cognitive impairment (n = 49), mild cognitive impairment (n = 159), and dementia (n = 101). RESULTS Combination of plasma pTau181, Aβ42/40, neurofilament light (NfL), and TabCAT‐BHA was optimal for predicting Aβ‐PET positivity (AUC = 0.962). Whereas NfL and TabCAT‐BHA optimally predicted concurrent disease severity, combining these with pTau181 and glial fibrillary acidic protein was most accurate in predicting functional decline. DISCUSSION Combinations of plasma and digital cognitive markers show promise for scalable diagnosis and prognosis of ADRD. Highlights The need for cost‐efficient diagnostic and prognostic markers of AD is urgent. Plasma and digital cognitive markers provide complementary diagnostic contributions. Combination of these markers holds promise for scalable diagnosis and prognosis. Future validation in community cohorts is needed to inform clinical implementation.

Abstract:Alzheimer's disease (AD) is an incurable neurodegenerative disorder that leads to dementia. This study employs explainable machine learning models to detect dementia cases using blood gene expression, single nucleotide polymorphisms (SNPs), and clinical data from Alzheimer's Disease Neuroimaging Initiative (ADNI). Analyzing 623 ADNI participants, we found that the Support Vector Machine classifier with Mutual Information (MI) feature selection, trained on all three data modalities, achieved exceptional performance (accuracy = 0.95, AUC = 0.94). When using gene expression and SNP data separately, we achieved very good performance (AUC = 0.65, AUC = 0.63, respectively). Using SHapley Additive exPlanations (SHAP), we identified significant features, potentially serving as AD biomarkers. Notably, genetic-based biomarkers linked to axon myelination and synaptic vesicle membrane formation could aid early AD detection. In summary, this genetic-based biomarker approach, integrating machine learning and SHAP, shows promise for precise AD diagnosis, biomarker discovery, and offers novel insights for understanding and treating the disease. This approach addresses the challenges of accurate AD diagnosis, which is crucial given the complexities associated with the disease and the need for non-invasive diagnostic methods.

Abstract:Background: Reliable, widely accessible and affordable biomarkers for predicting Alzheimer’s disease (AD) brain pathology status are a necessity to aid development of prevention strategies in cognitively healthy at-risk older adults, at the right timepoint. Measurements of the key neuropathological hallmark beta-amyloid (Aβ) by PET neuroimaging or cerebrospinal fluid measures reflect its accumulation in the brain, yet recent methodological advancements now enable blood-based measures reflecting cerebral amyloid burden. Objectives: The current study validated the capacity of plasma Aβ42/Aβ40 measured using six different assays to predict amyloid positivity in a subgroup of cognitively unimpaired (CU) participants in the ADNI study and assessed its ability to discriminate CU from AD cases. We also explored economic viability of using two different plasma amyloid assays for pre-screening in AD prevention trials and as routine clinical diagnostic tool, versus amyloid PET alone. Design: A cross-sectional analysis of plasma and brain amyloid data, including comparative cost analysis of the plasma biomarkers in relation to brain amyloid PET. Setting: Alzheimer’s Disease Neuroimaging Initiative (ADNI). Participants: ADNI participants consisting of 115 CU, mild cognitive impairment and AD cases who had plasma Aβ42/Aβ40 measured with six platforms. Measurements: Plasma Aβ42/Aβ40 was measured via six different platforms: three immunoassays (Roche, Quanterix and ADx Neurosciences) and three mass spectrometry (MS) based assays (WashU, Shimadzu and Gothenburg). Aβ-PET imaging was conducted within three months of plasma sampling using [18F]florbetapir. Results: There was a weak to moderate correlation of plasma Aβ42/Aβ40 ratio between platforms. The MS-based WashU test had the highest capacity to discriminate between CU and AD (area under the curve, AUC = 0.734, 95% CI: 0.613-0.854; P = 0.008). Within the CU group, the WashU plasma amyloid test had the best discriminative capacity to distinguish Aβ+ from Aβ– (AUC = 0.753, 95% CI: 0.601-0.905; P = 0.003) closely followed by the immunoassay from Roche (AUC = 0.737, 95% CI: 0.597-0.877; P = 0.006). The exploratory economic analyses showed that the use of Roche or WashU plasma amyloid assay as a pre-screening tool prior to Aβ-PET scans for clinical trial recruitment significantly reduced total screening cost (saving up to $5882 per recruited patient) expected in an AD prevention trial. Conclusions: With few available treatment strategies, dementia prevention is a global priority. CU individuals at risk for AD are the target population for dementia prevention but have been poorly studied. Our findings confirming diagnostic value of ultrasensitive immunoassays and high-performance immunoprecipitation coupled with MS for measurement of plasma Aβ42/Aβ40 to detect PET amyloid positivity in CU participants allude to potential clinical utility of this biomarker. Plasma Aβ42/Aβ40 could be optimal for pre-selecting at-risk candidates for more invasive and expensive investigations across AD prevention clinical trials and clinical care for a rapidly ageing population.

Abstract:Background Accurate prediction of cerebral amyloidosis with easily available indicators is urgently needed for diagnosis and treatment of Alzheimer’s disease (AD). Methods We examined plasma Aβ42, Aβ40, T-tau, P-tau181, and NfL, with APOE genotypes, cognitive test scores and key demographics in a large Chinese cohort ( N = 609, aged 40 to 84 years) covering full AD spectrum. Data-driven integrated computational models were developed to predict brain β-amyloid (Aβ) pathology. Results Our computational models accurately predict brain Aβ positivity (area under the ROC curves (AUC) = 0.94). The results are validated in Alzheimer’s Disease Neuroimaging Initiative (ADNI) cohort. Particularly, the models have the highest prediction power (AUC = 0.97) in mild cognitive impairment (MCI) participants. Three levels of models are designed with different accuracies and complexities. The model which only consists of plasma biomarkers can predict Aβ positivity in amnestic MCI (aMCI) patients with AUC = 0.89. Generally the models perform better in participants without comorbidities or family histories. Conclusions The innovative integrated models provide opportunity to assess Aβ pathology in a non-invasive and cost-effective way, which might facilitate AD-drug development, early screening, clinical diagnosis and prognosis evaluation.

Abstract:Abstract Recent technological advances have improved the sensitivity and specificity of blood-based biomarkers for Alzheimer’s disease and related dementias. Accurate quantification of amyloid-ß peptide, phosphorylated tau (pTau) isoforms, as well as markers of neurodegeneration (neurofilament light chain [NfL]) and neuro-immune activation (glial fibrillary acidic protein [GFAP] and chitinase-3-like protein 1 [YKL-40]) in blood has allowed researchers to characterize neurobiological processes at scale in a cost-effective and minimally invasive manner. Although currently used primarily for research purposes, these blood-based biomarkers have the potential to be highly impactful in the clinical setting – aiding in diagnosis, predicting disease risk, and monitoring disease progression. Whereas plasma NfL has shown promise as a non-specific marker of neuronal injury, plasma pTau181, pTau217, pTau231, and GFAP have demonstrated desirable levels of sensitivity and specificity for identification of individuals with Alzheimer’s disease pathology and Alzheimer’s dementia. In this forward looking review, we (i) provide an overview of the most commonly used blood-based biomarkers for Alzheimer’s disease and related dementias, (ii) discuss how comorbid medical conditions, demographic, and genetic factors can inform the interpretation of these biomarkers, (iii) describe ongoing efforts to move blood-based biomarkers into the clinic, and (iv) highlight the central role that clinical neuropsychologists may play in contextualizing and communicating blood-based biomarker results for patients.

Abstract:This work motivates and details the concept of QoEaware sustainable throughput in the area of video streaming. Sustainable throughput serves as a mean to compare video streaming solutions in terms of Quality of Experience (QoE) and energy efficiency (EE). It builds upon the QoE ProvisioningDelivery Hysteresis (PDH) and denotes the maximal throughput at which QoE deteriorations can be kept below a quantifiable level, which in turn allows to compare the EE of different video streaming solutions on QoE-fair grounds. In this work, we particularly focus on delivery problems stemming from outage-prone links, as they are typical for mobile systems. Welladapted to the nature of the video-associated data streams and disturbances, a stochastic fluid flow model is used that allows for straightforward calculation of sustainable throughput values. We also discuss the application of the sustainable throughput for comparisons among different streaming solutions and their offered QoE and EE, respectively.

Abstract:Recent FDA approvals of new disease-modifying therapies for Alzheimer disease (AD) has increased the clinical demand for tests of amyloid pathology that can be scaled for wider use. Concurrent advances in blood-based biomarker (BBB) assays have made it possible to perform minimally invasive and highly accessible laboratory testing for core biomarkers of this disease. While there are currently no FDA-approved blood tests for AD, many of the major in vitro diagnostic (IVD) companies have products in development (including several with breakthrough device status), and thus new IVD products are anticipated in the not-too-distant future. Existing diagnostic approaches, such as cerebrospinal fluid (CSF) analysis and positron emission tomography (PET) imaging, are effective but are invasive and often more expensive (especially the latter) compared to blood tests. Additionally, these tests may not be widely accessible based on socioeconomic and geographic factors, as well as regulatory and reimbursement status. PET in particular requires administration of a radiolabeled tracer prior to imaging, necessitating proximity to a tracer production site or a well-validated supply chain, which often limits this technology to tertiary care centers in predominantly major urban centers. The future availability of plasma assays is poised to fundamentally change the diagnostic process for AD, as it may lower some of the barriers to testing faced by CSF and PET testing.
