Abstract:Abstract Immune thrombotic thrombocytopenic purpura (iTTP) is a rare and potentially life-threatening disorder. Treatment advances have lowered morbidity rates, but past acute events can still cause long-term consequences, reducing health-related quality of life (HRQoL) and determining cognitive impairment, anxiety, and depression. We aimed to investigate these aspects and the role of caplacizumab and rituximab: 39 patients were evaluated using the Medical Outcomes Study 36-Item Short Form Health Survey (SF-36), the FACIT-Fatigue, the Hospital Anxiety and Depression Scale, and the Functional Assessment in Cancer Therapy-Cognitive Function questionnaires. The median age at study inclusion was 50 years (IQR 38–60), and the median follow-up from diagnosis was 97 months (IQR 14–182); 82% of patients were female, and 36% had one or more recurrences. Caplacizumab was administered in 16 patients (41%), as well as rituximab. ITTP patients reported lower physical and mental HRQoL scores than the general population. No differences in physical or mental domains were observed between patients treated or not with caplacizumab, while those who received rituximab reported lower scores in mental health. Neurological impairment at diagnosis correlated with worse fatigue. The majority of patients (72%) reported anxiety or depression (82%). ITTP had a significant impact on the long-term cognitive function, fatigue, depression, and anxiety levels of patients, with a negative effect on their HRQoL. Our findings underscore the need to pay special attention to patients’ long-term physical and mental health, regardless of the medical treatments received.

Abstract:In the air-polluted area or period, the outdoor activities are unhealthy and even become the triggers for different kinds of diseases because of the inhalable particles. Most of the outdoor activities are transferred into indoor lifestyle, such as entertainments, sports and working. And it is highly unlikely to get fresh air by ventilating directly with outside air, while CO2 content will persistently increase in the enclosed space so as to oppress people of oxygen-poor feeling. On the other hand, the rapid technology development and the urgent requirement growth of wireless sensors simultaneously offer a possible solution for such contradiction. For the sake of the health and comfort of the indoor inhabitants, a fuzzy control method is proposed in this paper for controlling the actuators of air cleaner and ventilator. With a good coordination between the air-cleaners and the ventilator, an intelligent environment which meets the comfort level with good air quality can be achieved. Exercise experiments which last for six days are carried out with comparisons among three controlling algorithms in the changing outdoor environments. Results show a satisfaction from overlapping working mode based on fuzzy control algorithm. Such algorithm has a broad application prospect on the existing buildings for airpolluted area.

Abstract:Microtubules are ordered polymers of the protein tubulin that are part of the flexible, plastic, and even self-effacing skeleton of eukaryotic cells [1,2]. They are hollow cylinders, 25 nm in diameter and tens of microns long [3]. Microtubules have large bending rigidity, with a persistence length of the order of millimeters [4]. The elastic properties of microtubules and their interaction with the cell membrane are essential in defining cell morphology. This Letter is concerned with microtubules within artificial phospholipid vesicles, their shapes, and the shape transformations induced by membrane tension. A single microtubule deforms a vesicle into an oval. Under tension, the microtubule buckles and the vesicle recovers a spherical shape. This allows a measurement of the microtubule bending rigidity. Abstract:Background: Mental health conditions such as depression, anxiety and post-traumatic stress disorder (PTSD) are being increasingly recognised as common adverse outcomes for critical illness survivors. These conditions can have lasting consequences on their wellbeing, ability to return to their pre-illness level of functioning and health-related quality of life. There is a paucity of South African data in terms of the psychological aspects of the post-intensive care unit (ICU) syndrome. Objective: To identify and characterise patients who suffered from significant symptoms of mental health disease and poor mental health- related quality of life after ICU admission and who may benefit from appropriate referral, further investigation and medical intervention. Methods: Critically ill patients in a multidisciplinary tertiary ICU were prospectively enrolled upon ICU discharge. Survivors were screened for anxiety, depression and post-traumatic stress syndrome at 6 weeks and 6 months after hospital discharge. The Hospital Anxiety and Depression Score (HADS) and the Impact Of Events Scale - Revised (IES-R) were used as screening tools. The mental component summary score (MCS) of the RAND short form-36 was used to determine the effect of psychological symptoms on health-related quality of life (HRQOL). Results: The median age of the 107 ICU survivors was 42 years, and half of them were admitted for complications of COVID-19. Six out of every 10 ICU survivors experienced significant symptoms of anxiety, depression and/or PTSD at follow-up. At the 6-month study visit, 4 out of every 10 patients were experiencing significant psychological symptoms. Those affected had a significantly lower mental HRQOL when compared with those who were unaffected. More than half of those affected had co-occurrence of psychological symptoms. Significant symptoms of anxiety were common, with 5 out of every 10 participants experiencing significant symptoms at either, or both, of the study visits. More than 3 out of 10 of affected patients' symptoms only occurred after the 6-week visit. Associations for significant psychological symptoms comprised female sex, younger age, a diagnosis of trauma and frightening memories of the ICU admission. Conclusion: The burden of mental health disease in ICU survivors is substantial, affecting their HRQOL. Six out of 10 patients experienced symptoms of anxiety, depression and PTSD, and more than half of those affected experienced co-occurrence of the conditions. A third of those affected developed these symptoms only beyond 6 weeks after hospital discharge. A potentially modifiable association with psychological symptoms was frightening memories of the ICU stay.

Abstract:BackgroundSurvivors of intensive care are known to be at increased risk of developing longer-term psychopathology issues. We present a large UK multicentre study assessing the anxiety, depression and post-traumatic stress disorder (PTSD) caseness in the first year following discharge from an intensive care unit (ICU).MethodsDesign: prospective multicentre follow-up study of survivors of ICU in the UK.Setting: patients from 26 ICUs in the UK.Inclusion criteria: patients who had received at least 24 h of level 3 ICU care and were 16 years of age or older.Interventions: postal follow up: Hospital Anxiety and Depression Score (HADS) and the Post-Traumatic Stress Disorder (PTSD) Check List-Civilian (PCL-C) at 3 and 12 months following discharge from ICU.Main outcome measure: caseness of anxiety, depression and PTSD, 2-year survival.ResultsIn total, 21,633 patients admitted to ICU were included in the study. Postal questionnaires were sent to 13,155 survivors; of these 38% (4943/13155) responded and 55% (2731/4943) of respondents passed thresholds for one or more condition at 3 or 12 months following discharge. Caseness prevalence was 46%, 40% and 22% for anxiety, depression and PTSD respectively; 18% (870/4943 patients) met the caseness threshold for all three psychological conditions. Patients with symptoms of depression were 47% more likely to die during the first 2 years after discharge from ICU than those without (HR 1.47, CI 1.19–1.80).ConclusionsOver half of those who respond to postal questionnaire following treatment on ICU in the UK reported significant symptoms of anxiety, depression or PTSD. When symptoms of one psychological disorder are present, there is a 65% chance they will co-occur with symptoms of one of the other two disorders. Depression following critical illness is associated with an increased mortality risk in the first 2 years following discharge from ICU.Trial registrationISRCTN Registry, ISRCTN69112866. Abstract:Background: Post-traumatic stress disorder (PTSD) is the most common psychiatric sequelae among novel coronavirus disease (COVID-19) patients. The aim of this study was to determine the prevalence of PTSD symptoms, PTSD-related factors, and its relationship with quality of life at long-term follow-up in hospitalized COVID-19 survivors. Methods: A cross-sectional study was undertaken to evaluate the health consequences of hospitalized COVID-19 survivors. All participants were interviewed face-to-face through a series of questionnaires: a researcher-developed symptom questionnaire, the Post-traumatic Stress Disorder Checklist–Civilian Version, the Generalized Anxiety Disorder 7-item, and the 36-item Short Form. Results: A total of 574 participants were enrolled with an average age of 57 years. The median follow-up time post-discharge was 193.9 days (SD = 15.32). Among the participants, 77.9% of survivors presented with at least one symptom, where fatigue or muscle weakness (47.9%) was reported the most frequently, followed by chest distress (29.4%) and sleep difficulty (29.4%). The prevalence of PTSD was 11.15% [95% confidence interval (CI): 8.56, 13.73] with a cut-off score of 44. Factors such as respiratory symptoms [odds ratio (OR): 3.53; 95% CI: 1.68–7.42], anxiety (OR: 14.64; 95% CI: 7.09–30.21), and sleep difficulty (OR: 2.17; 95% CI: 1.14–4.16) were positively related to PTSD. Those COVID-19 survivors with potential PTSD had significantly lower quality of life than those without ( P < 0.05). Conclusion: Our study illustrated that a significant number of COVID-19 survivors were suffering from physical or mental distress to varying degrees at 6 months post-discharge. People with PTSD were more likely to experience persistent respiratory symptoms and sleep difficulty, as well as anxiety and a decreased quality of life. Such survivors require greater attention to their mental health, particularly the PTSD symptoms at the early phase, which may play an important role in the recovery of both the physical and psychological health of COVID-19 survivors.

Abstract:Objectives. The present study was undertaken to evaluate the long-term health-related quality of life (HRQOL) as well as the employment status in survivors of severe sepsis up to 6 years afterwards.Material and methods. From January 2003 to December 2008 a total of 112 severe sepsis and 112 age, gender and Charlson comorbidity index-matched non-septic critically ill patients from 4 university hospital intensive care units (ICU) were enrolled in the study and 126 age and gender-matched community residents were interviewed as the community control group.Results. A total of 66 (58.9%) severe sepsis and 80 (71.4%) non-sepsis critically ill patients survived during the long-term follow-up time. Between August and December 2010 a total of 75 patients including 42 survivors of severe sepsis and 33 critically ill controls completed the face-to-face interview. There were no differences in the long-term HRQOL in terms of Short-Form 36 criteria between severe sepsis and non-sepsis critically ill survivors. However, when compared with the community controls, HRQOL in survivors of severe sepsis showed a significantly and clinically meaningful decrease, with a lower physical functioning (p=0.016), vitality (p=0.037), role-emotional (p=0.043), mental health (p=0.038) and mental component scores (p=0.042). In addition, the criteria returning to work at 1 year and at the time of interview in severe sepsis survivors were similar with those in critically ill survivors (60.5% vs. 70.0%, p=0.41 and, 71.1% vs. 76.7%, p=0.602).Conclusions. The HRQOL in survivors of severe sepsis was impaired even up to 6 years after hospital discharge.
Prospective Association of Psychological Distress and Sexual Quality of Life Among Hematopoietic Stem Cell Transplant Survivors

Abstract:Sexual health concerns are one of the most common late effects facing hematopoietic stem cell transplant (HSCT) survivors. The current study tested whether self-reported depression and anxiety symptoms before transplant were associated with embedded items assessing two specific areas of sexual health—sexual interest and sexual satisfaction—one year post-HSCT. Of the 158 study participants, 41% were diagnosed with a plasma cell disorder ( n = 60) and most received autologous transplantation ( n = 128; 81%). At post-HSCT, 21% of participants reported they were not at all satisfied with their sex life, and 22% were not at all interested in sex. Greater pre-HSCT depressive symptomology was significantly predictive of lower sexual interest (β = −.27, p < .001) and satisfaction (β = −.39, p < .001) at post-HSCT. Similarly, greater pre-HSCT trait anxiety was significantly predictive of lower sexual interest (β = −.19, p = .02) whereas higher levels of state and trait anxiety were both predictive of lower satisfaction (β = −.22, p = .02 and β = −.29, p = .001, respectively). Participant sex significantly moderated the relationship between state anxiety and sexual satisfaction ( b = −.05, t = −2.03, p = .04). Additional research examining the factors that contribute to sexual health post-HCST is needed to inform and implement clinical interventions to address these commonly overlooked survivorship concerns.

Abstract:Background Post‐intensive care syndrome (PICS) is characterized by all three adverse survivorship dimensions: physical function, cognitive function and mental health status. Aim This review aimed to describe the quality of life (QoL) of Intensive Care Unit (ICU) survivors with PICS after discharge and of their relatives with Family Post‐intensive care syndrome (PICS‐F) and to report anxiety, depression and Post‐Traumatic Stress Disorders (PTSD) in studies investigating PICS. Study Design A systematic review was carried out. We searched PubMed, Scopus, Web of Science and the Cumulative Index to Nursing and Allied Health Literature. This review was registered in the PROSPERO database (CRD42022382123). Results We included 19 studies of PICS and PICS‐F in this systematic review. Fourteen observational studies report the effects of PICS on depression, 12 studies on anxiety and nine on post‐traumatic stress disorder and 10 on QoL. Mobility, personal care, usual activities and pain/discomfort in QoL were the domains most affected by PICS. A significant association was demonstrated between a high level of ICU survivors' anxiety and high levels of ICU relatives' burden. Strain‐related symptoms and sleep disorders were problems encountered by ICU relatives with PICS‐F. Conclusion PICS and PICS‐F were widespread experiences among ICU survivors and their ICU relatives, respectively. The results of this review showed the adverse effects of PICS and PICS‐F on QoL. Relevance to Clinical Practice PICS and PICS‐F strongly impact the rehabilitation process and are measured in terms of health costs, financial stress and potentially preventable readmission.

Drug-induced immunological tolerance to sheep red blood cells in mice: effective combination therapy with vincristine and cyclophosphamide.

Abstract:Studies involving the combined use of cyclophosphamide (Cy) and vincristine (Vc) suggest a further step towards the induction of immunological unresponsiveness in mice sensitized with sheep red blood cells (SRBC). The usefulness of this combination has been confirmed by testing the effect of different timing and dosing schedules on the plaque-forming cell capacity as well as on the uptake of 14C-thymidine into spleen cell suspensions. A temporary but marked increase in DNA synthesis could be demonstrated in mice treated with a small dose of Vc 1 day after primary immunization with SRBC. Additional challenge with a single injection of 1 mg Cy per animal at this finite period of time resulted in complete immunotolerance, while a transient and partial immunosuppression occurred after treatment with either Vc or Cy alone. Two different patterns of tolerance could be defined: a short-term non-specific unresponsiveness lasting for about 2 weeks and an antigen-specific unresponsiveness lasting for about 6 weeks. The proposed explanation for this tolerance, induced by dose- and time-dependent administration of two cytostatic agents acting at various stages of the cell cycle might be a more intensive effect of Cy on immunocompetent cells growing (partially) synchronously after treatment with Vc.

Abstract:Posttraumatic stress disorder (PTSD) reflects a mental illness that can develop after a person has experienced or witnessed a traumatic event such as a natural disaster, accident, terrorist attack, war/combat, sexual assault, or other violent acts.[1] Symptoms usually begin within 3 months of experiencing the traumatic episode(s), although these may occasionally develop earlier, and last for over a month. The most typical symptoms fall into four categories, encompassing intrusive memories (e.g., periodic distressing reminders of the traumatic episode), avoidance behaviors (avoidance of people or places reminiscent of the traumatic episode), negative changes in thinking and mood, along with changes in physical and emotional responses (i.e., arousal symptoms).[1] The historical epidemiology of PTSD suggests that between 5 and 10% of the general population may be affected by this condition at some point in their lives,[2] although coronavirus disease 2019 (COVID-19) has contributed to significantly increase the prevalence of PTSD, with almost 20% of COVID-19-discharged patients showing symptoms.[3] Given this concerning epidemiology, the search for effective treatment options for patients with PTSD has become increasingly important in recent years. Among the various agents that have been proposed, heparin represents an intriguing perspective. Kondashevskaya conducted a study with 40 sexually mature male Wistar rats that were randomly divided into four groups[4]: a first control group, a second untreated group including modeled PTSD rats, a third group consisting of modeled PTSD rats treated with 64 U/kg low-molecular-weight heparin (LMWH), and a fourth group with normal rats that received only 64 U/kg LMWH. PTSD was achieved by maintaining the animals under a permanent "predator stress" (i.e., cat urine odor) for 10 minutes daily for 10 days. The behavioral disturbances were associated with persistent increase in psychoemotional symptoms, accompanied by a decrease in serum corticosterone levels. In the PTDS-LMWH-treated group, behavioral and psychoemotional parameters (e.g., number of entries and time spent into the open arm of the maze as the more anxious rodents are less likely to do this, number of active and passive defensive responses) improved significantly, to the point of normalization, while at the same time corticosterone increased to levels comparable to those of the control group. The same author conducted another study in adult male Wistar rats, which were divided into three groups, i.e., a control group ( n = 20), a group with untreated modeled PTSD ( n = 30), and a third group with modeled PTSD receiving 36 mg/kg unfractionated heparin (UFH).[5] PTSD was again reproduced by the validated "predator stress" model (exposure to cat urine). Consistent with previous findings, the authors found a consistent reduction in plasma corticosterone levels in the PTSD rats, which was reversed with complete normalization in the group of rats receiving UFH. All psychoemotional responses were also profoundly altered in PTDS rats, while they did not differ significantly between UFH-administered PDTS rats and the control group. Of note, there was a consistent reduction in the thickness of the adrenal cortex (particularly the zona fasciculata and zona reticularis) in the untreated PDTS group, whereas this abnormality was not observed in the UFH-treated PDTS group. Finally, plasma levels of some inflammatory cytokines (namely interleukin [IL]-2 and IL-6), that were consistently elevated in the untreated PDTS group, were similar in the control and UFH-treated PDTS groups. Although the exact mechanisms by which heparin (both LMWH and UFH) could contribute to the relief of symptoms of PTSD are not precisely known, some hypotheses can be proposed. Based on the assumption that the mechanisms responsible for alteration of the central nervous system in PTSD patients are relatively similar to those causing lipopolysaccharide-induced disorders,[6] it is conceivable that the anti-inflammatory effect of heparin,[7] exerted mainly through the modulation (downregulation) of various proinflammatory cytokines,[8] would be effective in attenuating the systemic low-grade inflammatory state characteristic of PTSD.[9] Another possible mechanism is the heparin-dependent increase in brain-derived neurotrophic factor,[8] which was found to be significantly decreased in human patients with PDTS in the recent meta-analysis by Mojtabavi et al.[10] Of further interest, there is now consolidated evidence that PTSD is an independent risk factor for cardiovascular disease (CVD), other than a common psychiatric sequela of CVD events that may negatively impact the prognosis of these patients.[11] Moreover, the large Nurses' Health Study II, which included nearly 50,000 young and middle-aged women,[12] estimated that those with multiple PTSD symptom -Abstract Truncated-

Impact of COVID-19 on posttraumatic stress disorder in ICU survivors: a prospective observational comparative cohort study

Abstract:Posttraumatic stress disorder (PTSD) after a stay in the intensive care unit (ICU) can affect one in five ICU survivors. At the beginning of the coronavirus disease 2019 (COVID-19) pandemic, admission to the ICU for COVID-19 was stressful due to the severity of this disease. This study assessed whether admission to the ICU for COVID-19 was associated with a higher prevalence of PTSD compared with other causes of ICU admission after adjustment for pre-ICU psychological factors.

Abstract:Background: The prevalence of post-traumatic stress symptoms (PTSS) in COVID-19 survivors is unclear. This study examined the prevalence of PTSS and its association with quality of life (QOL) among COVID-19 survivors during the post-COVID-19 era in China. Methods: This was a comparative, cross-sectional study. PTSS, depressive symptoms, and QOL were assessed with standardized instruments. Results: A total of 134 COVID-19 survivors and 214 non-infected controls (healthy controls hereafter) were recruited. Among COVID-19 survivors, the PTSS prevalence was 18.66% (95%CI: 11.98–25.34%), which was significantly higher than that (5.61%, 95%CI: 2.50–8.71%) of healthy controls ( P < 0.001). After controlling for covariates, an analysis of covariance (ANCOVA) showed that COVID-19 survivors had a higher PTSS total score than did healthy controls [ F (1,348) = 4.664, P = 0.032]. A separate ANCOVA revealed there were no significant differences in overall QOL between COVID-19 survivors with and without PTSS [ F (1,348) = 1.067, P = 0.304]. A multiple logistic regression analysis showed that more severe depressive symptoms were significantly associated with PTSS in COVID-19 survivors (OR = 1.425, P < 0.001). Conclusions: PTSS were more severe in COVID-19 survivors compared to healthy controls in the post-COVID-19 era. Considering their negative impact on daily life and functional outcomes, regular assessment and appropriate treatments of PTSS should be conducted in COVID-19 survivors.

Abstract:Background Systemic sclerosis (SSc) is a chronic autoimmune disease characterised by microvascular damage and fibrosis. Mortality in patients with SSc has significantly decreased. Consequently, patients with SSc have longer life expectancy, and health-related quality of life (HrQoL) has become more relevant in the comprehensive management of the disease. Objective To evaluate the impact between gastrointestinal (GI) symptom burden and psychological well-being on HrQoL in patients with SSc. Design Nested cross-sectional study conducted between January and July 2022. Participants A single-centre cohort of 166 patients with SSc, including 103 (55%) with limited cutaneous SSc, 43 (24%) with diffuse SSc and 37 (21%) with sine-sclerosis SSc. Main measures GI symptom burden was assessed using the University of California Los Angeles Scleroderma Clinical Trial Consortium gastrointestinal tract 2.0 (UCLA SCTC GIT 2.0) questionnaire, psychological well-being was measured with the Hospital Anxiety and Depression Scale (HADS), and HrQoL was evaluated using the Short Form 36 (SF-36) questionnaire. Demographic, clinical and immunological data were collected from a prospectively maintained database. Key results Patients with moderate to severe GI symptoms (UCLA SCTC GIT 2.0 score >0.5, n=95, 57%) reported decreased HrQoL in all subdomains except vitality by SF-36, and higher anxiety and depression scores by HADS (all p<0.05). The severity of GI symptom burden and depression were independently associated with a decline in the physical component of QoL (β=–0.273 and β=–0.411, respectively, p<0.01 for both). Only the severity of depression and anxiety (β=–0.482 and β=–0.213, respectively, p<0.05), but not GI symptom burden, were independently associated with a decline in the mental component of QoL. Conclusions Our data suggest that in patients with SSc, GI and psychological burden negatively influence quality of life independently, highlighting the need for a holistic approach to patient's care.

Abstract:Post‐traumatic stress symptoms are highly prevalent in the pediatric solid organ transplant population, as measured by the Child Trauma Screening Questionnaire. Risk factors for post‐traumatic stress include recent ICU days, the number of medications, and involvement with the foster care system, whereas higher family functioning appears to be protective. Background Life with end‐stage organ failure is accompanied by an accumulation of traumatic medical or surgical experiences. Despite recovery after solid organ transplantation (SOT), many children and adolescents develop post‐traumatic stress symptoms (PTSS). PTSS remain underappreciated as a major comorbidity in SOT programs, despite their association with decreased quality of life. Methods We conducted a retrospective, cross‐sectional study of 86 pediatric SOT recipients (17 heart, 44 kidney, and 25 liver) to evaluate potential determinants of PTSS. Trauma symptoms were measured by the Child Trauma Screening Questionnaire (CTSQ). Demographic, baseline, and contemporaneous factors were tested for independent association with CTSQ scores. Results The median post‐transplant CTSQ score was 2 (IQR 1–4), and 22% were identified as high risk (score ≥5) for PTSD. Higher CTSQ scores were independently associated with the number of ICU days within the previous 12 months, the number of medications (complexity), and involvement with foster care in the primary model (R2 [adj.] = 0.26). The addition of the Family Impact Module improved the overall model (R2 [adj.] = 0.33), wherein higher family functioning was independently associated with lower CTSQ scores. An exploratory analysis of pre‐transplant patients (n = 34) found a median pre‐transplant CTSQ of 2 (IQR 1–6), suggesting that PTSS are onset before transplant and persist afterward. Conclusions PTSS are highly prevalent in the SOT population. Risk factors include recent adverse medical experiences and complexity, whereas family stability may be protective. Additional research is needed to improve early ascertainment and support for patients at high risk of developing PTSS throughout their transplant journey.

Abstract:Introduction: Exploring the connections between traumatic experiences and subsequent health outcomes is vital for informing clinical practices and public health policies. The study aimed to investigate the relationship between lifetime trauma exposure and posttraumatic stress disorder (PTSD), depressive and cardiovascular disease (CVD) symptoms. Methods: A total of 171 patients who received treatment in a local heart centre were included in this study. Several questionnaires such as the Life Event Checklist-5, Posttraumatic Stress Disorder Checklist for DSM-5 and Patient Health Questionnaire-9 were used to measure their traumatic experiences and PTSD and depressive symptoms, respectively. Physiological measures were also examined. Data were analysed using SPSS. Results: The chi-square test showed significant differences in the percentage of reported PTSD symptoms among the patients with CVD (24.0%), patients with kidney disease (4.3%) and patients with other health problems (7.1%). The patients with CVD reported having a significantly higher percentage of PTSD and depressive symptoms than the patients with other medical conditions. The patients with CVD who reported having PTSD symptoms had significant systolic blood pressure (SBP) and heart rate changes compared to the patients who did not. The patients who reported PTSD symptoms had a significantly shorter sleep duration than their counterparts. The SBP and diastolic blood pressure differed significantly between the patients with and without PTSD symptoms. Conclusion: Earlier detection, prevention and intervention related to trauma exposure and PTSD symptoms are suggested to reduce the CVD risk.

Abstract:ABSTRACT Introduction The long-term impact of deployment-related trauma on mental and physical health-related quality of life (HRQoL) among military personnel is not well understood. We describe the mental and physical HRQoL among military personnel following deployment-related polytrauma after their discharge from the hospital and examine factors associated with HRQoL and longitudinal trends. Materials and Methods The U.S. military personnel with battlefield-related trauma enrolled in the Trauma Infectious Diseases Outcomes Study were surveyed using SF-8 Health Surveys at 1 month post-discharge (baseline) and at follow-up intervals over 2 years. Inclusion in the longitudinal analysis required baseline SF-8 plus responses during early (3 and/or 6 months) and later follow-up periods (12, 18, and/or 24 months). Associations of demographics, injury characteristics, and hospitalization with baseline SF-8 scores and longitudinal changes in SF-8 scores during follow-up were examined. Survey responses were used to calculate the Mental Component Summary score (MCS) and the Physical Component Summary score (PCS). The MCS focuses on vitality, mental health, social functioning, and daily activity limitations, whereas PCS is related to general health, bodily pain, physical functioning, and physical activity limitations. Longitudinal trends in SF-8 scores were assessed using chi-square tests by comparing the median score at each timepoint to the median 1-month (baseline) score, as well as comparing follow-up scores to the immediately prior timepoint (e.g., 6 months vs. 3 months). Associations with the 1-month baseline SF-8 scores were assessed using generalized linear regression modeling and associations with longitudinal changes in SF-8 were examined using generalized linear regression modeling with repeated measures. Results Among 781 enrollees, lower baseline SF-8 total scores and PCS were associated with spinal and lower extremity injuries (P < .001) in the multivariate analyses, whereas lower baseline MCS was associated with head/face/neck injuries (P < .001). Higher baseline SF-8 total was associated with having an amputation (P = .009), and lower baseline SF-8 total was also associated with sustaining a traumatic brain injury (TBI; P = .042). Among 524 enrollees with longitudinal follow-up, SF-8 scores increased, driven by increased PCS and offset by small MCS decreases. Upward SF-8 total score and PCS trends were associated with time post-hospital discharge and limb amputation (any) in the multivariate analyses (P < .05), whereas downward trends were independently associated with spinal injury and developing any post-discharge infection (P ≤ .001). Patients with lower extremity injuries had lower-magnitude improvements in PCS over time compared to those without lower extremity injuries (P < .001). Upward MCS trend was associated with higher injury severity (P = .003) in the multivariate analyses, whereas downward trends were independently associated with having a TBI (P < .001), time post-hospital discharge (P < .001), and occurrence of post-discharge infections (P = .002). Conclusions Overall, HRQoL increased during the 2-year follow-up period, driven by PCS improvement. Increasing HRQoL was associated with time since hospital discharge and limb amputation, whereas a downward trend in HRQoL was associated with spinal injury and post-discharge infection. The longitudinal decline in MCS, driven by TBI occurrence, time since hospital discharge, and developing post-discharge infections, emphasizes the importance of longitudinal mental health care in this population.

Abstract:While HSCT continues to remain a promising form of treatment and advances in transplant medicine have led to reduced morbidity and mortality from HSCT, HRQOL remains an important consideration. Our study suggests that addressing possibly modifiable psychological risk factors prior to and/or during recovery from HSCT has the potential to improve this important aspect of patient experience during transplant and also health outcomes. Aim The aim of study was to investigate whether depression and anxiety symptoms and illness perception prior to hematopoietic stem cell transplantation (HSCT) predict health related quality of life (HRQOL) at Day 100 and 1 year following HSCT. Methods A total of 205 patients who underwent HSCT (N = 127 autologous transplants, N = 78 allogeneic transplants) were included in this prospective study. Baseline assessment was assessed prior to transplantation and post HSCT data were collected at Day 100 and 1 year. At baseline we assessed depressive symptoms (Patient Health Questionnaire‐9), anxiety symptoms (Generalized Anxiety Disorder‐7), illness perception (Brief Illness Perception Questionnaire), and HRQOL (Functional Assessment of Cancer Therapy‐BMT). Results Patients who expressed a greater level of concern about the severity, course, and ability to exert control over one's illness (i.e., illness perception) and who reported a greater level of depression and anxiety symptoms prior to HSCT reported lower HRQOL at both Day 100 and 1 year posttransplant, with a similar degree of association observed at the two follow‐up time points. Conclusions Our findings suggest that pretransplant perceptions about their illness and negative mood are significant predictors of HRQOL following HSCT. Illness perception, depression, and anxiety are potentially modifiable risk factors for less than optimal outcome after HCSCT and intervention strategies should be explored.
