Abstract:Background: The identification of variables affecting suicidality and the search for interventions to reduce suicide risk are priorities among mental health researchers. A promising direction for such research is schema therapy and its two main constructs, i.e., early maladaptive schemas (EMSs) and schema modes. Methods: This systematic review was designed in accordance with the PRISMA guidelines. It summarizes the studies conducted to date that describe the relationship between EMSs and schema modes and measures of suicidality in individuals over the age of 16. Results: The review confirmed that there are many significant associations between EMSs (especially from the Disconnection/Rejection domain) and suicide risk. Although only one study was found that explores the association between schema modes and suicidality, the correlations it identified are also confirmed here. Discussion: The results show the unquestionable importance of EMSs and schema modes in assessing suicide risk. The co-occurrence of these variables represents the first step in further assessing causality and introducing schema therapy techniques into work with patients who are at risk of suicide. This issue requires more extensive experimental research.

Abstract:This study investigated the risk factors of suicide ideation, suicide behaviors, and self-harm, particularly the role of emotional schemas. Three hundred seventy-five university students participated and completed the Leahy Emotional Schema Scale (LESS), the Beck Scale for Suicide Ideation (BSSI), the Self-Harm Inventory (SHI), and the Suicide Behaviors Questionnaire-Revised (SBQ-R). The findings indicated that there was a significant correlation between emotional schemas and suicidal ideation, self-harm behaviors, and suicidal behaviors. Specially, rumination and invalidation had a significant role in predicting which individuals have elevated levels of suicidal ideation and behaviors and self-harm. Invalidation and rumination, among other possible emotional schemas, could be considered as suicidal emotional schemas (SESs).

Abstract:Early maladaptive schemas (EMSs) may be closely related to nonsuicidal self-injury (NSSI). The present study aimed to discuss the relationship between EMS and a personal history of NSSI. This was a retrospective study. A total of 1339 Chinese college students between 16 and 29 years old were asked to complete a questionnaire survey regarding their personal history of NSSI and EMSs. 116 college students reported a history of NSSI (NSSI group), who differed significantly in terms of all EMS-related scores than non-NSSI group(n = 1223). Logistic regression analysis showed that the scores in the disconnection/rejection schema domain exhibited by subjects who were the only child in their family could help differentiate between college students with or without NSSI. The emotional deprivation schema was significantly associated with the lifetime frequency of NSSI behaviors; the vulnerability to harm or illness schema was significantly associated with the internal emotion regulation function and pain associated with NSSI; and the enmeshment/underdeveloped self schema was significantly associated with the addictive features of NSSI. The self-sacrifice schema was significantly associated with the external emotion regulation function of NSSI; the enmeshment/underdeveloped self schema was positively related with the sensation-seeking function of NSSI; and the abandonment/instability schema was negatively related the sensation-seeking function of NSSI. The disconnection/rejection schema domain was highly related with NSSI behaviors. EMS is significantly associated with the history, functions, addictive features, and severity of NSSI. Every EMS is worthy of further investigation and discussion with patients in the context of NSSI behaviors during clinical therapy.
Early maladaptive schemas and harm avoidance as mediating factors between early life stress and psychiatric symptoms in adults

Abstract:OBJECTIVE: This study investigated how factors of temperament and early maladaptive schemas predict psychiatric symptoms, as well as how they mediate the relation between early life stress and psychiatric symptoms in adults.METHODS: A cross-sectional study was conducted with a sample of 200 university students. Data was collected through a sociodemographic questionnaire, the Adult Self-Report Inventory, the Childhood Trauma Questionnaire, the Young Schema Questionnaire, and the Temperament and Character Inventory-Revised.RESULTS: A model including early maladaptive schemas, harm avoidance (temperament factor), and early life stress explained 69% of the variation of the psychiatric symptoms; among the predictors, early maladaptive schemas explained 31% of psychiatric symptoms, while harm avoidance explained 25%. Most of the predictive power associated with early life stress can be better explained by early maladaptive schemas and, to a lesser extent, harm avoidance.CONCLUSION: By managing these processes therapeutically, deleterious effects associated with early life stress can be minimized.

Abstract:Early maladaptive schemas (EMSs) are broad and pervasive themes regarding oneself and one's relationships with others originating from adverse childhood experiences. Although the concept of EMSs was initially developed for the treatment of personality disorders, the associations of EMSs with a variety of other mental disorders have been investigated. The goal of the present study was to summarize and analyze the EMSs-disorder associations in studies in which patients with specific psychiatric diagnoses were compared to healthy controls. Of the 28 studies that met the inclusion criteria, 27 were included in a meta-analysis. Across diagnoses, all EMSs were elevated in the clinical groups. The largest effect sizes were observed for the social isolation, the negativity/pessimism, the defectiveness/shame, and social undesirability schemas. Depression (n = 8), borderline personality disorder (n = 5), and obsessive–compulsive disorder (n = 5) were the most frequently studied mental disorders. Heterogeneity between studies was high. Results suggest that mental disorders are not characterized by specific EMSs.

Abstract:This study examines the frequency of consanguineous marriage and coefficient of inbreeding in the State of Qatar. The study was conducted in semi-urban areas of Doha between January and May 2004. A sample of 1515 married Qatari females aged 15 years and over participated. The degree of consanguinity between each female and her spouse, and degree of consanguinity between their parents were recorded. The rate of consanguinity in the present generation was high (54·0%) with a coefficient of inbreeding of 0·02706. The commonest type of consanguineous marriage was between first cousins (34·8%). Double first cousin marriages were common (3·1%) compared with other populations. The consanguinity rate in the State of Qatar has increased from 41·8% to 54·5% in one generation.

Abstract:Early maladaptive schemas (EMS) are dysfunctional structures formed by emotions, cognitions, memories, and physical sensations. The Young schema questionnaire—short form (YSQ-S3), a 90-item self-report instrument with a 6-point Likert scale, was developed to measure the 18 EMS. This study aimed to present evidence of the validity of the YSQ-S3 instrument for use in Brazil, involving a general population sample of 1,050 individuals. The YSQ-S3 and symptom checklist (SCL-90) forms were applied. Confirmatory factor analysis was used to assess the factor structure and validity evidence based on relations to other variables through correlating with clinical psychological symptoms. The results indicated good evidence of validity. Cronbach’s alpha was satisfactory for all EMS (α between 0.74 and 0.94). The fit of the factor model were shown to be adequate and the instrument had good validity. It was possible to verify satisfactory evidence of validity for use of the instrument in the Brazilian population. The importance of future studies involving clinical samples is emphasized.

Abstract:Background: Non-suicidal self-injury (NSSI) is a maladaptive strategy mainly regulating emotions in youth and adolescents. Early maladaptive schemas (EMSs) were identified to affect NSSI. This review aimed to provide a synthesis of the literature on the relationship between EMS and NSSI. Methods: A systematic review was conducted via the electronic databases PsycINFO, PubMed, Science Direct, and Google Scholar. The inclusion criteria were (i) the study examined the relationship between EMSs and NSSI, (ii) full texts were available in the English language, and (iii) the study was a piece of published empirical quantitative research. The authors also evaluated the risk of bias assessment. Results: Ten studies were investigated. The relationship between EMS and NSSI was identified from different points of view. The results confirmed a developmental model of NSSI and shame that focuses on the function of this behavior to regulate emotions (especially shame) in individuals with a predisposition for emerging EMS and schema modes. A major limitation of this review is the absence of longitudinal research. Conclusion: It is important to address EMSs and modes that lead to NSSI engagement in both preventive and therapeutic interventions.

Abstract:The main purpose of this preliminary study was to investigate a potential relationship between early maladaptive schemas (EMSs) and impulsive and compulsive buying tendencies in a sample of young adults (college students). This research adds to the cognitive perspective of consumer behavior that the cognitive schemas putatively associated with early experiences may have a strong impact on impulsive and compulsive buying. Data was obtained from 365 participants in a cross-sectional study design. Participants completed an online survey with the following instruments: Young Schema Questionnaire; Impulsive Buying Tendency Measurement Scale; Richmond Compulsive Buying Scale; and Hospital Anxiety and Depression Scale. Using multiple linear hierarchical regressions, we confirmed that the domain of over vigilance and inhibition schemas was positively associated with impulsive and compulsive buying tendencies, while an opposite association was found for the domain of impaired limits. Being a female was also a predictor of impulsive buying and compulsive buying. The results were discussed in terms of the coping mechanisms to deal with negative emotions, as a way to obtain rewards, or as a way to escape painful self-awareness. Other mechanisms related to the internalization of perfectionist expectations and the propensity to shame were also explored.

Early maladaptive schemas and child and adult attachment: a 15-year longitudinal study

Abstract:Objectives: To examine the relation between early maladaptive schemas (EMS), as defined in schema therapy, and both child and adult attachment. Design: A 15-year longitudinal design in which child attachment groups (secure, avoidant, ambivalent, and disorganized) were compared in early adulthood on their profile of scores across EMS domains. A similar strategy was used to examine EMS profiles as a function of adult attachment groups (secure, preoccupied, and fearful). Methods: Sixty participants, recruited from Montreal day-care centres, were assessed at 6 (Time 1) and 21 years of age (Time 2). Time 1 attachment was assessed using a separation-reunion procedure and Time 2 attachment, using the Experiences in Close Relationships questionnaire. EMS were evaluated with the Young Schema Questionnaire (Time 2). Results: There were more signs of EMS among young adults with either an insecure ambivalent child attachment, or an insecure preoccupied adult attachment style, compared to their secure peers. These differences were not specific to one domain of EMS; they were reported for various EMS. Conclusions: The results suggest that specific elements of representational models are more likely to be related to the development of EMS: high anxiety over abandonment, negative self-view, and explicit manifestations of personal distress. Unmet childhood needs for secure attachment may lead to a large variety of EMS as defined in schema therapy.

Abstract:According to Young's schema model (Young, J. E., Klosko, J. S., & Weishaar, M. E. (2003). Schema therapy: A practitioner's guide. New York: Guilford Press), innate personality tendencies are important for the understanding of early maladaptive schemas (EMS). The current study examined the relations between EMS and the dimensions of the five-factor model of personality (FFM). One hundred and forty-seven adult outpatients completed the NEO PI-R, the Schema Questionnaire-Short Form (SQ-SF), and the Beck Depression Inventory (BDI). Correlational analyses showed a substantial overlap between EMS and the FFM, neuroticism in particular. EMS predicted depressive symptoms above and beyond the FFM personality dimensions. Implications of these findings are discussed.

Abstract:The present study explored the Early Maladaptive Schemas (EMS) of individuals who engage in self-mutilation. One hundred five participants (34 males and 71 females) from a community site and from two clinical sites participated in the study. Four EMS differentiated self-mutilators from nonmutilators: Mistrust/Abuse, Emotional Deprivation, Social Isolation/Alienation, and Insufficient Self-Control/Self-Discipline. The following schemas were also found to differentiate repetitive self-mutilators from nonmutilators and from self-mutilators who had engaged in only one episode of self-mutilation: Emotional Deprivation, Social Isolation/Alienation, Defectiveness/Shame, and Insufficient Self-Control/Self-Discipline. Finally, the Social Isolation/Alienation schema was found to be endorsed more strongly as the number of self-mutilative episodes increased. The results are largely in accord with the theoretical suppositions of schema theory. The clinical implications of these findings are discussed in the context of schema therapy.

Abstract:<span><b>Background:</b> Behavioral addiction (BA) is a recent concept in psychiatry. Few studies have investigated the relationship between BA and early maladaptive schemas (EMSs). EMS is the core of Schema Therapy (ST). According to the ST model, psychiatric disorders result from the development of EMSs in response to unmet emotional needs in childhood. <a href="#B4">Bach et al. (2018)</a> grouped the 18 EMSs into four domains: (1) <i>disconnection and rejection</i>; (2) <i>impaired autonomy and performance</i>; (3) <i>excessive responsibility and standards</i>; and (4) <i>impaired limits</i>. This study aims to assess the possible association of the most frequent BAs with EMSs in a large group of late adolescents and young adults and to evaluate their self-perceived quality of life (QoL).<b>Methods:</b> A battery of psychological tests assessing food addiction (FA), gambling disorder (GD), internet addiction (IA), and QoL was administered to 1,075 late adolescents and young adults (<i>N</i> = 637; 59.3% women). A forward-stepwise logistic regression model was run to identify which variables were associated with BAs.<b>Results:</b> Food addiction was more frequent among women and GD among men, while IA was equally distributed. Regarding the EMSs, participants with FA or IA showed significantly higher scores on all four-schema domains, whereas those with GD exhibited higher scores on <i>impaired autonomy and performance</i> and <i>impaired limits</i>. Besides, average scores of all domains increased with the association of two or more comorbid BAs. Self-perceived QoL was lower for participants with FA and IA, but not for those with GD; the presence of comorbid BAs was associated with lower Physical Component Summary (PCS) and Mental Component Summary (MCS) scores. Finally, specific EMS domains and demographic variables were associated with each BA.<b>Conclusion:</b> Late adolescents and young adults with FA or IA have a lower perception of their mental and physical health. The most striking result is that FA appears to be associated with the <i>disconnection and rejection</i> schema domain, IA with all the schema domains (except for <i>impaired autonomy and performance</i>), and GD with <i>impaired autonomy and performance</i> schema domain. In conclusion, our findings suggest that EMS should be systematically assessed during psychotherapy of patients with BAs.</span>

Early maladaptive schemas in patients with somatoform disorders and somatization

Abstract:Maladaptive schemas are stable relational patterns that develop through harmful childhood experiences with primary caregivers. Schemas within somatoform disorders are rarely explored even though these disorders are clinically important due to high prevalence, co-morbidity, and cost for the health care system. This study investigates schemas according to Young's schema theory in patients with somatoform disorders in comparison with healthy controls and patients with depressive or anxiety disorders. Further associations between schemas and somatization were explored. We included 134 patients with a somatoform disorder and 39 age-matched healthy controls, 83 patients with a unipolar depression, and 34 patients with an anxiety disorder. The clinical sample consists of day care patients, diagnosed with the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, but without a personality disorder. Primary measures were the Young Schema Questionnaire (YSQ-S2), the Screening for Somatoform Disorders (SOMS-7T), the Beck Depression Inventory, Second Edition, and the Childhood Trauma Questionnaire. Analyses of variance indicated that somatoform patients scored higher on almost all schemas than do healthy controls (p < 0.001, η2 = 0.148). The highest mean scores were reached for the schemas "self-sacrifice" and "unrelenting standards," with significant higher values in the patient sample. However, when compared with patients with depressive or anxiety disorders, somatoform patients scored equally or even lower. High somatization was associated with generally higher schema activation. This effect was to a great extent mediated by depressive symptoms. Only the schema "vulnerability to harm or illness" was exclusively related to somatization. These findings suggest that schemas should be systematically assessed within psychotherapy of somatoform patients.
Early maladaptive schemas in adult survivors of interpersonal trauma: foundations for a cognitive theory of psychopathology

Abstract:BACKGROUND: Although the association between psychological trauma and early maladaptive schemas (EMS) is well established in the literature, no study to date has examined the relationship of EMS to PTSD and psychopathologies beyond depression and anxiety in a sample of adult survivors of interpersonal trauma. This information may be useful in helping our understanding on how to best treat interpersonal trauma.OBJECTIVE: We set out to investigate the association between EMS and common forms of psychopathology in a sample of women with a history of interpersonal trauma (n=82). We have hypothesised that survivors of interpersonal trauma will present with elevated EMS scores compared to a non-clinical control group (n=78). We have also hypothesised that unique schemas will be associated with unique psychopathological entities and that subgroups of interpersonal trauma survivors would be present in our sample, with subgroups displaying different profiles of schema severity elevations.METHOD: Participants completed measures of trauma, psychopathology, dissociation, self-esteem, and the Young Schema Questionnaire.RESULTS: It was found that survivors of interpersonal trauma displayed elevated EMS scores across all 15 schemas compared to controls. Although the pattern of associations between different psychopathological features and schemas appears to be rather complex, schemas in the domains of Disconnection and Impaired Autonomy formed significant associations with all psychopathological features in this study.CONCLUSIONS: Our findings support the usefulness of cognitive behavioural interventions that target schemas in the domains of Disconnection and Impaired Autonomy in an effort to modify existing core beliefs and decrease subsequent symptomatology in adult survivors of interpersonal trauma.

Abstract:BACKGROUND: Early maladaptive schemas (EMSs) are hypothesized to be stable, trait-like, enduring beliefs underlying chronic and recurrent psychological disorders. We studied the relation of EMSs with depressive symptom severity and tested the stability of EMSs over a course of evidence-based outpatient treatment for depression in a naturalistic treatment setting.METHODS: The sample consisted of depressed outpatients (N=132) treated at a specialized mood disorders treatment unit in The Netherlands. Participants completed measures of depressive symptom severity and maladaptive schemas before treatment and 16-weeks after starting with treatment.RESULTS: Specific maladaptive schemas (failure, emotional deprivation, abandonment/instability) were cross-sectionally related to depressive symptom severity. Moreover, the schema domain impaired autonomy & performance at pre-treatment related positively to depression levels at the 16-week follow-up assessment, whereas the schema domain overvigilance & inhibition at pre-treatment related negatively to depression levels at the follow-up assessment when controlling for pre-treatment depression severity. Finally, all EMSs demonstrated good relative stability over the course of treatment.CONCLUSIONS: Our results suggest that specific EMSs are related to depressive symptom severity in clinically depressed patients, that specific schema domains predict treatment outcome, and that schemas are robust to change over time, even after evidence-based outpatient treatment for depression.

Abstract:The schema model proposed by Young et al., (2003) suggests that past trauma could form maladaptive unconscious beliefs, known as Early Maladaptive Schemas (EMS). The EMS was hypothesised to be a suitable trauma theory for measuring the impacts of traumatic experiences on traumatised individuals. Currently, there are 18 types of EMS. This systematic review narratively synthesised 23 studies published from 1990 to 2022, that investigated EMS in the population who has been traumatised or experienced PTSD symptoms/diagnosis. The review excluded studies that did not specify the types of EMS (that only provide a total score), or involved traumatic experiences outside of the definition by DSM-5. The review identified that the nine EMS from the domain of Disconnection/Rejection and Impaired Autonomy/Performance were common among the traumatised population. Thus, more specialised support and intervention are warranted for traumatised populations, based on the identified EMS.

Included studies were peer-reviewed journal articles that examined the relationship between one or more EMS and depression in adulthood in participants aged 18 years or older.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>A total of 51 studies were included (<i>k</i> = 743; Pooled <i>N</i> = 17,830). All 18 EMS were positively correlated with depression, with effect sizes ranging from small (<i>r</i> = .23 [.17, .29]; Entitlement) to large (<i>r</i> =.53 [.46, .60]; Social Isolation; <i>r</i> = .50, 95% CI [.45, .54]; Defectiveness/Shame). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The evidence suggests that individuals who feel like they do not belong, or that they are flawed, bad, or unlovable, report higher levels of depression. However, most studies used cross-sectional designs and further longitudinal research is needed to establish the direction of the relationship between EMS and depression. These findings can guide preventative and treatment approaches. Focusing treatment on the Social Isolation and Defectiveness/Shame EMS may aid in relieving depressive symptoms.</p></section>
