Endovascular Therapy in Pediatric Stroke: Utilization, Patient Characteristics, and Outcomes

Abstract:Background and purpose: Despite strong evidence for endovascular therapy in adults with acute arterial ischemic stroke, limited data exist in children. We aimed to describe endovascular therapy utilization and explore outcomes in a national sample of pediatric arterial ischemic stroke. Methods: We queried the 2012 Kids' Inpatient Database for children aged greater than 28 days to 20 years with the International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) codes for arterial ischemic stroke and evaluated groups based on the procedure code for endovascular therapy. Poor outcome was defined as need for tracheostomy or gastrostomy, discharge to rehabilitation facility, or death. Logistic regression evaluated the association between endovascular therapy and poor outcome, adjusted for age, disease severity (hemiplegia, critical care interventions, neurosurgical interventions), and comorbidities. Results: We identified 3184 pediatric discharges with a diagnosis code for arterial ischemic stroke. Thirty-eight (1%) had an endovascular therapy procedure code. Endovascular therapy patients were older (10.2 versus 4.5 years, P < 0.001) and more likely to have hemiplegia/paresis (relative risk [RR] 3.8, 95% confidence interval [CI] 2.0-7.4), aphasia (RR 5.3, 95% CI 2.8-10.1), and facial droop (RR 4.0, 95% CI 1.9-8.7). Endovascular therapy was not associated with critical care and neurosurgical interventions or intracranial hemorrhage. Length of hospitalization, mortality, and discharge disposition were similar between groups. In a multivariable model, endovascular therapy was not associated with poor outcome (adjusted odds ratio 1.7, 95%, CI 0.7-4.1). Conclusions: In a national sample of children with a diagnosis of arterial ischemic stroke, endovascular therapy was infrequently utilized. Patients with a procedure code for endovascular therapy had significant stroke-related deficits, but outcomes were similar to those in children who did not receive endovascular therapy. Our data, in conjunction with evidence of benefit in adults, support consideration of endovascular therapy for select children with acute stroke.

Abstract:Background Pediatric acute ischemic stroke is a rare diagnosis that requires timely recognition and definitive management to prevent morbidity and mortality. Children often present to primary care offices, urgent care clinics, and adult emergency departments for evaluation of symptoms that may be signs and symptoms of stroke. Currently, there are no published prehospital or transport protocols specific to pediatric acute ischemic stroke. The Children's Mercy Hospital Critical Care Transport Team (CMCCT) created a pediatric-specific clinical practice guideline (CPG) for suspected acute ischemic stroke. Methods This retrospective, descriptive study reports pediatric patients aged younger than 18 years who met criteria for the pediatric stroke CPG and required interfacility transport by CMCCT over a 4- year period. Large vessel occlusion (LVO) scores used in adults were calculated retrospectively. Results Seventeen patients met inclusion criteria. Four (24%) of 17 had radiographic evidence of acute thrombus, 3 of whom received alteplase and/or endovascular clot retrieval. Median age of confirmed stroke was 83 months (interquartile range, 65–148) compared with 177 months for nonstroke (interquartile range, 169–191), P = 0.126. The most common presenting symptom was hemiplegia in the confirmed stroke group. The confirmed stroke group scored significantly lower on the Glasgow Coma Scale (median of 8 vs 15, P = 0.014), significantly higher on the Los Angeles Motor Scale LVO score (median 4 vs 0, P = 0.021), and significantly higher on the Rapid Arterial Occlusion Evaluation LVO (median 4 vs 0, P = 0.036). Conclusions To our knowledge, the CMCCT CPG is the first pediatric transport protocol aimed at recognition and management of pediatric stroke described in the literature. Retrospective calculation of LVO scores show that they may be helpful in application to pediatric patients.

Abstract:Objective To determine factors associated with magnetic resonance imaging (MRI) and non-invasive diagnostic angiography among children presenting to the emergency department (ED) with acute ischemic stroke. Study Design We performed a cross-sectional study using data from >50 US children's hospitals. We included children 29 days through 17 years old hospitalized from the ED with an International Classification of Disease, 10 th revision, diagnosis code for acute ischemic stroke between October 1, 2015, and November 30, 2022. We excluded children with a principal diagnosis code of trauma/external injury, without neuroimaging on day of presentation, and into-ED transfers. Our outcomes were defined as acquisition of MRI (versus computed tomography only) and angiography (versus no angiography) on day of presentation. We performed generalized linear mixed modeling with hospital as a random effect to determine the association of demographics, known comorbidities, and treatment factors with each outcome. Results We included 1,601 children. In multivariable analysis, younger age, mechanical ventilation, and Black race were associated with lower odds of MRI acquisition, while history of moyamoya disease and sickle cell disease were associated with higher odds. Younger age, mechanical ventilation, Hispanic ethnicity, Black race, other races, history of metabolic disease, and history of seizures were associated with lower odds of angiography. Conclusions Younger and non-White children experienced lower odds of MRI and angiography, which may be driven by health system limitations or provider implicit biases or both. Our results expose risk factors for underdiagnosis of ischemic stroke and provide opportunities to tailor institutional pathways reflective of underlying pathophysiology.

Australian Clinical Consensus Guideline: The diagnosis and acute management of childhood stroke

Abstract:Stroke is among the top 10 causes of death in children and survivors carry resulting disabilities for decades, at substantial cost to themselves and their families. Children are not currently able to access reperfusion therapies, due to limited evidence supporting safety and efficacy and long diagnostic delays. The Australian Clinical Consensus Guideline for the Diagnosis and Acute Management of Childhood Stroke was developed to minimize unwarranted variations in care and document best evidence on the risk factors, etiologies, and conditions mimicking stroke that differ from adults. Clinical questions were formulated to inform systematic database searches from 2007 to 2017, limited to English and pediatric studies. SIGN methodology and the National Health and Medical Research Council system were used to screen and classify the evidence. The Grades of Recommendation, Assessment, Development, and Evaluation system (GRADE) was used to grade evidence as strong or weak. The Guideline provides more than 60 evidence-based recommendations to assist prehospital and acute care clinicians in the rapid identification of childhood stroke, choice of initial investigation, to confirm diagnosis, determine etiology, selection of the most appropriate interventions to salvage brain at risk, and prevent recurrence. Recommendations include advice regarding the management of intracranial pressure and congenital heart disease. Implementation of the Guideline will require reorganization of prehospital and emergency care systems, including the development of regional stroke networks, pediatric Code Stroke, rapid magnetic resonance imaging and accreditation of primary pediatric stroke centers with the capacity to offer reperfusion therapies. The Guideline will allow auditing to benchmark timelines of care, access to acute interventions, and outcomes. It will also facilitate the development of an Australian childhood stroke registry, with data linkage to international registries, to allow for accurate data collection on stroke incidence, treatment, and outcomes.

Background:In 2021, our Comprehensive Stroke Center (CSC) in San Antonio (SA), Texas received 7 pediatric patients suffering from signs and symptoms of stroke. These patients ranged in age from 4-17 and approximately 57% were positive for an acute ischemic stroke (AIS). During the month of March, 2 patients were brought in via EMS and both MRIs were positive for AIS, prompting our stroke team to evaluate our adult processes. We concluded that an Emergency Department (ED) Pediatric Stroke Alert Protocol was needed in order for our hospital to have the capability to care for this patient population, and also appropriately triage and offer acute therapies to give these patients the best chance possible.Purpose:Develop an ED Pediatric Stroke Alert Protocol in order to provide rapid triage and advanced therapies if warranted. The ED Pediatric Stroke Alert protocol was developed with our same stroke alert time frame in mind, 0-6 hours, and with the age range of 2-17 years.Methods:An interdisciplinary team consisting of Vascular neurology, Pediatric Neurology, Pediatric Hematology/Oncology, Pediatric Critical Care nursing, Pediatric / Adult ED nursing & Faculty, Neuro Critical Care, Pharmacy, and our Stroke Team met to discuss how to implement a pediatric protocol. It was decided that modeling it after the Adult ED Stroke Alert Protocol would be best to limit confusion and triage errors. The Cincinnati Stroke Scale was used as the tool of choice in order for our ED team to identify stroke symptoms, also a time frame of less than 6 hours from onset of symptoms AND a blood glucose between 60 and 600. This would then prompt the activation of the Pedi ED Stroke Alert Protocol. Triage on the protocol includes inclusion and exclusion criteria for IV tPA, which was also tailored to the pediatric population with the help of Pediatric Hematology/Oncology.Results:The ED Pedi SA Protocol has since been revised to meet the needs of our changing pediatric stroke population. The CSC continues to treat pediatric patients and continues to develop other tools to meet the needs of this population.Conclusion:Our CSC has been categorized by EMS as one of the only CSCs in SA, Texas that is capable of providing complete stroke care to the pediatric population.

Abstract:Patients supported on ventricular assist devices (VADs) benefit from rehabilitation while awaiting heart transplantation to recover from surgery, prevent deconditioning, and, in most cases, optimize transplant candidacy. With bleeding and neurological dysfunction as the most common VAD complications, the importance of rehabilitation dramatically increases when a patient on a VAD also suffers from a neurological injury. The rehabilitation needs for cardiac conditioning and neurological reeducation are not the same. Patients with severe neurological deficits require intense rehabilitation that often includes base-of-support challenges, usage of bolsters and balls, partial weight-bearing treadmill training, and assumption of various body positions in prone, kneeling, or quadruped for neuromotor reeducation. However, some devices are more conducive to rehabilitation than others. For children supported by the CentriMag in particular, rehabilitation is challenged by short cannula tubing, an external motor, a large interface, and an intensive care unit (ICU) admission. We report a safe and successful physical therapy course of a pediatric stroke patient with a diagnosis of Ebstein's anomaly supported by a CentriMag right VAD (RVAD) while awaiting heart transplant in the ICU.
Clinical Application and Choice of Mechanical Circulatory Support Devices in Children

Abstract:Objective To explore the clinical application of centrifugal ventricular assist device (VAD) and extracorporeal membrane oxygenation (ECMO) at Shanghai Children's Medical Center and summarize the experiences of selecting an ideal method of mechanical circulatory support (MCS) in pediatric cases. Methods From January 2005 to December 2015, 60 cases were support by MCS devices. There were congenital heart defects (n=56) and acute myocarditis (n=4). The mechanical circulatory support devices were used for supporting circulatory functions because of heart failure or arrhythmia. Thirty-two cases were supported with centrifugal VAD (VAD group) and 26 with ECMO (ECMO group) and other 2 cases were started by VAD then transferred to ECMO (VAD-ECMO group). Results The duration for VAD was from 3 to 458 hours. Twenty-two cases were weaned and 20(62.50%) were successfully discharged. The duration of ECMO was from 26 to 498 hours. Sixteen cases were weaned and 14 (53.85%) successfully discharged. None survived in VAD-ECMO group. Bleeding and neurological complications were two major mortal causes. ALCAPA (anomalous origin of coronary artery from pulmonary artery) cases had the biggest ratio in VAD group and the survival rate was 81.82%. Conclusions Pediatric patients with cardiac failure are suitable for VAD and ECMO supports. Cardiac function, respiratory function, age, complications and costs should be considered for selecting from these two methods. Determinants of Timely Access to Recanalization Treatments and Outcomes in Pediatric Ischemic Stroke

Abstract:Stroke, Ahead of Print. BACKGROUND:Timely revascularization in acute arterial ischemic stroke (AIS) is paramount for optimal outcomes. However, factors causing treatment delays in pediatric AIS remain understudied. We investigated determinants affecting the time from symptom onset or last-known-well to the start of recanalization treatment in pediatric AIS.METHODS:We conducted an ancillary analysis of the French KID-CLOT study (The National Retrospective Study of Recanalization Treatments in Pediatric Arterial Ischemic Stroke), considering patients with pediatric AIS receiving recanalization treatments (IV thrombolysis IVT and mechanical thrombectomy) from 2015 to 2018. The study assessed prehospital triage's impact, direct versus transferred admissions, and unit type (pediatric versus adult) on treatment delay and clinical outcomes using modified Rankin Scale at 1 year.RESULTS:Among 68 patients (median age, 11 [IQR, 4–16]; initial PedNIHSS, 13 [IQR, 7–19]), treatment modalities were IVT (n=31), and mechanical thrombectomy (n=23), and IVT+mechanical thrombectomy (n=14). Prehospital triage significantly reduced last-known-well to treatment delay (overall, 229 versus 270 minutes;P=0.01), most notably for and mechanical thrombectomy (P<0.001). There was no substantial delay difference between direct and transferred admissions, or between unit types, although a trend favored adult units (370.3 versus 436.73 minutes;P=0.06). Prehospital triage correlated with improved outcomes, with a shift to lower modified Rankin Scale scores (P=0.021).CONCLUSIONS:For pediatric AIS treated with reperfusion therapy, prehospital triage emerges as a pivotal factor in reducing treatment delays and enhancing outcomes. Abstract:Stroke, Volume 55, Issue Suppl_1, Page AWP216-AWP216, February 1, 2024. Background:The recognition of pediatric stroke symptoms is challenging. A regionwide pediatric stroke program utilizing an educational initiative across 15 emergency departments (ED) coupled with telemedicine was developed to standardize the process for immediate access to stroke experts and decrease door-to-intervention times. This study evaluates the impact of this intervention.Methods:Data from the pre- (1/2020-12/2021) vs post-intervention (1/2022-7/2023) including baseline demographics, volumes, and process measure timeframes: Door-to-Telestroke Page, Door-to-CT Interpretation, Door-to-Telestroke Call Back, and Door-to-Needle Time were abstracted. Chi-Square and Wilcoxon Rank Sum were used for statistical analysis.Results:There were a total of 92 Telestroke consults, with more in the post group: 35(38%) pre vs 57(62%) post (p=0.04). 52% were female with mean age of 12 (±1.2) years. 78(85%) were stroke mimics: 74% had non-cerebrovascular or migrainous neurological disorders as primary diagnoses. 7(7.6%) patients had interventions: 4(4.3%) thrombolytics, 9(9.7%) arterial or venous thrombectomies. 51% were discharged home and 43% were transferred to an acute care facility for higher level of care. There was no difference between pre- and post-implementation median [IQR] Door-to-Telestroke Page (17.5[14-21] vs 15[11-18] mins); Door-to-CT Interpretation (29[25-32] vs 27[23-29] mins) and Door-to-Needle Time. Median Post Door-to-Telestroke Call Back was faster (pre: 20[17-22] vs post: 16.0[13-18] mins [p=0.03]).Conclusion:Systemwide ED education coupled with telemedicine in implementing pediatric stroke systems of care in a large integrated healthcare system improves access to stroke specialists and time-based metrics in this low-volume, high-risk patient population.

Abstract:Objective: To assess the feasibility of endovascular thrombectomy (EVT) for the treatment of acute ischemic stroke (AIS) in children. Methods: Clinical data and follow-up information of 4 AIS children who received EVT in the Department of Intervention & Hemangioma at the Children's Hospital of the Capital Institute of Pediatrics from December 2020 to June 2021 were collected retrospectively. The vascular recanalization after EVT was assessed by the modified thrombolysis in cerebral infarction (mTICI) score. Efficacy outcomes were assessed with initial and postprocedural Pediatric National Institutes of Health Stroke Scale (PedNIHSS) score, and the modified Rankin scale (mRS) score at 3 and 6 months after treatment. Safety assessments included perioperative complications and intracranial hemorrhage post-treatment. Results: A total of 5 EVT treatment were performed on 4 children with AIS, of whom 3 were male. The age of onset was 4.6, 13.8, 7.8, 8.0, 8.9 years, respectively. The time from symptom onset to initiation of EVT was 19.0, 25.0, 22.0, 4.0, 16.5 hours, respectively and all patients achieved successful recanalization of the vessel after EVT (mTICI≥2b). The PedNIHSS score was 39, 14, 25, 39, 24 before treatment and decreased to 8, 1, 12, 39, 5 at discharge. All the procedures were performed with no perioperative complications. Only 1 patient with congenital heart disease had a recurrent AIS with malignant brain oedema and brain hernia. Although the occluded vessels were successfully recanalized,the symptoms were not improved and this patient died after treatment abandonment. The other 3 patients achieved good recovery at 6 months postoperatively. The mRS score of 3 patients was 3, 1, 2 at 3 months after EVT and decreased to 2, 1, 1 at 6 months. Conclusion: EVT treatment may be feasible and safe for pediatric AIS due to large vessel occlusion even when the treatment was initiated 6 hours post stroke, but children with heart disease may have a dismal prognosis.

Abstract:Background: Emerging evidence suggests that endovascular thrombectomy is beneficial for treatment of childhood stroke, but the safety and effectiveness of endovascular thrombectomy has not been compared with best medical treatment. We aimed to prospectively analyse functional outcomes of endovascular thrombectomy versus best medical treatment in children with intracranial arterial occlusion stroke. Methods: In this prospective registry study, 45 centres in 12 countries across Asia and Australia, Europe, North America, and South America reported functional outcomes for children aged between 28 days and 18 years presenting with arterial ischaemic stroke caused by a large-vessel or medium-vessel occlusion who received either endovascular thrombectomy plus best medical practice or best medical treatment alone. Intravenous thrombolysis was considered part of best medical treatment and therefore permitted in both groups. The primary outcome was the difference in median modified Rankin Scale (mRS) score between baseline (pre-stroke) and 90 days (±10 days) post-stroke, assessed by the Wilcoxon rank test (α=0·05). Efficacy outcomes in the endovascular thrombectomy and best medical treatment groups were compared in sensitivity analyses using propensity score matching. The Save ChildS Pro study is registered at the German Clinical Trials Registry, DRKS00018960. Findings: Between Jan 1, 2020, and Aug 31, 2023, of the 241 patients in the Save ChildS Pro registry, 208 were included in the analysis (115 [55%] boys and 93 [45%] girls). 117 patients underwent endovascular thrombectomy (median age 11 years [IQR 6-14]), and 91 patients received best medical treatment (6 years [3-12]; p<0·0001). The median Pediatric National Institutes of Health Stroke Scale (PedNIHSS) score on admission was 14 (IQR 10-19) in the endovascular thrombectomy group and 9 (5-13) in the best medical treatment group (p<0·0001). Both treatment groups had a median pre-stroke mRS score of 0 (IQR 0-0) at baseline. The change in median mRS score between baseline and 90 days was 1 (IQR 0-2) in the endovascular thrombectomy group and 2 (1-3) in the best medical treatment group (p=0·020). One (1%) patient developed a symptomatic intracranial haemorrhage (this patient was in the endovascular thrombectomy group). Six (5%) patients in the endovascular thrombectomy group and four (5%) patients in the best medical treatment group had died by day 90 (p=0·89). After propensity score matching for age, sex, and PedNIHSS score at hospital admission (n=79 from each group), the change in median mRS score between baseline and 90 days was 1 (IQR 0-2) in the endovascular thrombectomy group and 2 (1-3) in the best medical treatment group (p=0·029). Regarding the primary outcome for patients with suspected focal cerebral arteriopathy, endovascular thrombectomy (n=18) and best medical treatment (n=33) showed no difference in 90-day median mRS scores (2 [IQR 1-3] vs 2 [1-4]; p=0·074). Interpretation: Clinical centres tended to select children with more severe strokes (higher PedNIHSS score) for endovascular thrombectomy. Nevertheless, endovascular thrombectomy was associated with improved functional outcomes in paediatric patients with large-vessel or medium-vessel occlusions compared with best medical treatment. Future studies need to investigate whether the positive effect of endovascular thrombectomy is confined to older and more severely affected children. Funding: None.

Abstract:Severe disability due to large vessel occlusion (LVO) stroke can be avoided by emergency mechanical thrombectomy (MT)1 but provision varies nationally and internationally according to the availability of specialist facilities. Across England, Wales and Northern Ireland, only 26 hospitals can offer treatment,2 and the majority of stroke patients are instead admitted to local non-MT hospitals. Here, eligible LVO cases must be quickly recognised through clinical assessment and brain imaging before transfer to the nearest MT-capable centre. Although this process offers a practical solution within the existing service infrastructure, cross-organisational pathways might impede access to time-critical interventions. Abstract:Background: Rapid reperfusion with mechanical thrombectomy for ischemic stroke secondary to large vessel occlusion leads to significant reduction in morbidity and mortality. DIDO time is an important metric for stroke centers without on-site mechanical thrombectomy. Delays in treatment can occur when patients present at a Primary Stroke Center (PSC) then require transfer to the CSC for MT. Our system Comprehensive Stroke Center (CSC) receives patients from 2 of our system’s PSCs. In order to expedite care an expedited transfer protocol was implemented. Purpose: The goal of this work is to decrease the time at the PSC, known as door-in-door-out (DIDO), and decrease overall time from patient presentation to treatment with thrombectomy. Methods: Patients are screened at triage by a PSC emergency department provider for the following: aphasia, gaze preference, or neglect with unilateral weakness within 24 hours of last known normal. If these criteria are present, the emergency department provider contacts the CSC transfer center to initiate the expedited transfer process, which begins mobilizing resources to the PSC while clinical work-up continues. This expedited early transfer process was implemented in January 2021. Transfers for MT during the 12-month period before and after the implementation of the expedited transfer process were analyzed. Results: A sustained reduction in overall DIDO was observed during the time period following implementation of the expedited transfer process (Fig 1) Mean DIDO was lower when the transfer call was placed before CT vs after CT (81 min vs. 132 min respectively) (p=.0031). Conclusions: Use of the Expedited Transfer Process allows transfer resources to begin moving to the patient with a suspected LVO in parallel with ongoing evaluation, resulting in faster DIDO times leading to faster treatment aimed at reducing stroke morbidity and mortality. Further analysis of the effect of the expedited transfer process on DIDO times is planned.

Abstract:In a large registry of patients undergoing durable left ventricular assist device (d‐LVAD) implantation following micro‐axial flow‐pump (mAFP) support, the early ischemic stroke incidence was 10.8% and early hemorrhagic stroke 2.9%. Ischemic stroke, while associated with patient‐related factors, did not appear to be modulated by the temporary mechanical circulatory support strategy and the pre‐operative course. Hemorrhagic stroke was associated with both patient‐related factors and several factors related to the mAFP bundle of care, representing potentially actionable treatment targets. Both early ischemic and hemorrhagic strokes were associated with increased 1‐year mortality rate. Background Stroke after durable left ventricular assist device (d‐LVAD) implantation portends high mortality. The incidence of ischemic and hemorrhagic stroke and the impact on stroke outcomes of temporary mechanical circulatory support (tMCS) management among patients requiring bridge to d‐LVAD with micro‐axial flow‐pump (mAFP, Abiomed) is unsettled. Methods Consecutive patients, who underwent d‐LVAD implantation after being bridged with mAFP at 19 institutions, were retrospectively included. The incidence of early ischemic and hemorrhagic stroke after d‐LVAD implantation (<60 days) and association of pre‐d‐LVAD characteristics and peri‐procedural management with a specific focus on tMCS strategies were studied. Results Among 341 patients, who underwent d‐LVAD implantation after mAFP implantation (male gender 83.6%, age 58 [48–65] years, mAFP 5.0/5.5 72.4%), the early ischemic stroke incidence was 10.8% and early hemorrhagic stroke 2.9%. The tMCS characteristics (type of mAFP device and access, support duration, upgrade from intra‐aortic balloon pump, ECMELLA, ECMELLA at d‐LVAD implantation, hemolysis, and bleeding) were not associated with ischemic stroke after d‐LVAD implant. Conversely, the device model (mAFP 2.5/CP vs. mAFP 5.0/5.5: HR 5.6, 95%CI 1.4–22.7, p = 0.015), hemolysis on mAFP support (HR 10.5, 95% CI 1.3–85.3, p = 0.028) and ECMELLA at d‐LVAD implantation (HR 5.0, 95% CI 1.4–18.7, p = 0.016) were associated with increased risk of hemorrhagic stroke after d‐LVAD implantation. Abstract:Introduction: Although acute stroke endovascular therapy (EVT) has dramatically improved outcomes in acute ischemic stroke (AIS) patients with large vessel occlusions (LVO), access to EVT-capable centers remains limited, particularly in rural areas. Therefore, it is essential to optimize triage systems for EVT-eligible patients. One strategy may be the use of a telestroke network that typically consists of multiple spoke sites that receive a consultation to determine appropriateness of patient transfer to an EVT-capable hub site. Standardization of AIS protocols may be necessary to achieve target door-to-groin (DTG) times of less than 60 minutes in EVT-eligible patients upon hub arrival. Specifically, the decision to obtain vascular imaging at the transferring hub site vs delaying until arrival at the hub is controversial. The purpose of this study was to identify factors associated with reduced DTG time in LVO-AIS patients. Methods: We performed a retrospective chart review for all patients treated over a 3.5-year period at our home hub institution. Patients were classified as telestroke transfers, non-telestroke transfers, and direct-to-hub presentations. We recorded demographic information, DTG time, reperfusion status, length of stay (LOS), functional status at discharge, seven-day mortality, and the site where vascular imaging- computed tomography angiography (CTA)-was obtained. We performed binary logistic regression to identify factors associated with DTG <60 minutes. Results: In the sample of EVT-eligible patients (n = 383), CTA was performed at the spoke site prior to transfer to the hub institution in 53% of cases. Further, 59% of telestroke transfer cases received a CTA prior to transfer compared to only 40% of non-telestroke transfers (59 vs 40%, P = 0.01). A Door-to-groin time <60 minutes was achieved in 67% of transfer patients who received pre-transfer CTA compared to only 22% of transfer patients who received CTA upon hub arrival and 17% of patients who presented directly to the hub. Ultimately, transfer patients who received CTA prior to transfer were 7.2 times more likely to have a DTG <60 minutes compared to those who did not (OR 7.2, 95% confidence interval 3.5-14.7; P < 0.001). Conclusion: Pre-transfer computed tomography angiography was the only significant predictor of achieving target door-to-groin times of less than 60 minutes. Because DTG time has been well established as a predictor of clinical outcomes, including pre-transfer CTA in a standardized acute ischemic stroke protocol may prove beneficial. Our findings also illustrate the need to optimize direct-to-hub stroke alerts and telestroke relationships to minimize workflow disruptions, which became more apparent during the pandemic.
Endovascular treatment in pediatric stroke: an individual patient pooled analysis on presentation, management and outcomes

Abstract:Background: This study aimed to perform a comprehensive pooled analysis of all published series involving pediatric patients with large vessel occlusion (LVO) undergoing endovascular treatment (EVT) to study the disease landscape including presentation, management, and outcomes. Methods: The data used in this study were pooled from published literature on EVT for pediatric stroke spanning from 1980 to 2023. The relevant articles were carefully reviewed, and only those reporting at least one case of pediatric LVO undergoing EVT were included. Furthermore, pediatric patients with LVO who received EVT from the Chinese Pediatric Ischemic Stroke Registry database were also included. The primary outcome for this study was the unfavorable outcome after stroke. The risk factors influencing the outcomes of patients with pediatric stroke treated with EVT were analyzed using Cox proportional hazards models. Results: A total of 221 pediatric patients who underwent EVT were included. During an average follow-up period of 6 months, 20.8% of patients had an unfavorable outcome. Multivariate Cox regression showed that patients with a higher Pediatric National Institutes of Health Stroke Scale (pedNIHSS) score (aHR=7.54, 95% CI 2.38 to 23.58) and unsuccessful reperfusion (aHR=5.84, 95% CI 2.86 to 11.93) were associated with an increased risk of unfavorable outcomes, while older age (aHR=0.27, 95% CI 0.09 to 0.82) was associated with a lower risk of unfavorable outcomes. Conclusion: Following EVT treatment, a significant proportion of pediatric patients with LVO achieved favorable outcomes. However, factors such as the severity of the pedNIHSS score and unsuccessful reperfusion were found to be associated with a poorer prognosis.

Abstract:Background Stroke remains a devastating complication of durable left ventricular assist device (LVAD) therapy. This study evaluated the incidence and risk factors for early stroke within 7 days following LVAD implantation investigating both traditional pre-implant and new intraoperative variables collected by The Society of Thoracic Surgeons (STS) Intermacs National Database. Methods STS Intermacs was queried for patients undergoing implantation of a fully magnetically levitated centrifugal LVAD between 11/25/2020 and 6/30/2023. STS Intermacs stroke definitions were used to identify patients who suffered a stroke within the first 7 postoperative days (POD). A multivariable logistic regression model was created to generate adjusted odd ratios (OR) for variables associated with early stroke. Results Among 6950 patients in the study cohort, 5.9% (413/6950) developed a stroke after a median follow-up of 11 months, with 50% (205/413) of strokes occurring within 7 days after LVAD implantation. Of the strokes occurring during POD 0-7, 70% (144/205) occurred on POD 0-2. By multivariable analysis, the following factors were associated with early stroke: older age (70 vs. 50; OR 1.4, P=0.0129 ), white race (OR 1.5, P=0.0078 ), pre-implant temporary MCS bridge (temporary LVAD only: OR 1.6, ECMO only: OR 1.7, combination of both devices: OR 3.3; P=0.0001 ) and presence of an unremoved left atrial clot (OR 8.0, P<0.0001 ). Conclusion A significant proportion of strokes occur within the first 7 days following LVAD implantation, particularly within the first 2 days. In addition to pre-implant variables, we identified modifiable intraoperative factors associated with stroke that provide an opportunity for further risk mitigation and improvement in quality of care.

Abstract:Introduction: The launch of AHA’s Target: Stroke Phase III in 2019 challenged stroke centers to achieve DTD ≤ 90 minutes (direct arriving patients) or ≤ 60 minutes (transfer patients) in ≥ 50% of patients treated with mechanical thrombectomy (MT). At the time of Phase III launch, we were achieving the DTD goal in 45% of patients at our CSC (> 200 MT/year). Methods: Our stroke team, in collaboration with the ED and other relevant departments, analyzed the stroke alert process and implemented changes as needed to improve DTD times: • Early notification of neurointerventional surgery (NIS) by neurologist upon clinical evaluation for any possible MT candidate • CT/CTP/CTA completed simultaneously (IV lytics given in CT if eligible) • Utilization of RAPID.AI platform to allow immediate review of stroke imaging by NIS 24/7 • Decision to upgrade to MT made immediately after imaging completed • MT code activation alerts nursing supervisor, NCCU, anesthesia, and IR team • Patient transported immediately from CT to angio suite by ED nurse upon upgrade 24/7 • ED nurse transfers care to IR team and anesthesia during working hours, or to NCCU nurse and anesthesia during off hours (IR nurse assumes care upon arrival) • Focus on efficient handoff process in angio suite • Specific roles assigned to each team member • Timer on wall started upon patient arrival to angio suite, with arrival to puncture goal < 20 min • Efficiency maintained with simulation MT codes rotating members from IR, ER, NCCU, and anesthesia DTD times were collected at launch of Phase III in 2019, and after changes were implemented. Objectives were to improve DTD times from baseline, in order to optimize care and achieve Phase III goals. Results: Figure 1. Conclusion: By implementing collaborative and focused interventions, our CSC was able to improve and optimize the workflow of stroke patients undergoing MT, allowing us to achieve the rigorous Target: Stroke Phase III goals and optimize patient outcomes.
