Abstract:Intracranial electroencephalography (iEEG) provides the best precision in estimating the location and boundary of an epileptogenic zone. Analysis of iEEG in the routine EEG frequency range (0.5–70 Hz) remains the basis in clinical practice. Low-voltage fast activity is the most commonly reported ictal onset pattern in neocortical epilepsy, and low-frequency high-amplitude repetitive spiking is the most commonly reported ictal onset pattern in mesial temporal lobe epilepsy. Recent studies using wideband EEG recording have demonstrated that examining higher (80–1000 Hz) and lower (0.016–0.5 Hz) EEG frequencies can provide additional diagnostic information and help to improve the surgical outcome. In addition, novel computational techniques of iEEG signal analysis have provided new insights into the epileptic network. Here, we review some of these recent advances. Although these sophisticated and advanced techniques of iEEG analysis show promise in localizing the epileptogenic zone, their utility needs to be further validated in larger studies.
Electroencephalogram (EEG) With or Without Transcranial Magnetic Stimulation (TMS) as Biomarkers for Post-stroke Recovery: A Narrative Review

Abstract:Stroke is one of the leading causes of death and disability. Despite the high prevalence of stroke, characterizing the acute neural recovery patterns that follow stroke and predicting long-term recovery remains challenging. Objective methods to quantify and characterize neural injury are still lacking. Since neuroimaging methods have a poor temporal resolution, EEG has been used as a method for characterizing post-stroke recovery mechanisms for various deficits including motor, language, and cognition as well as predicting treatment response to experimental therapies. In addition, transcranial magnetic stimulation (TMS), a form of non-invasive brain stimulation, has been used in conjunction with EEG (TMS-EEG) to evaluate neurophysiology for a variety of indications. TMS-EEG has significant potential for exploring brain connectivity using focal TMS-evoked potentials and oscillations, which may allow for the system-specific delineation of recovery patterns after stroke. In this review, we summarize the use of EEG alone or in combination with TMS in post-stroke motor, language, cognition, and functional/global recovery. Overall, stroke leads to a reduction in higher frequency activity (≥8 Hz) and intra-hemispheric connectivity in the lesioned hemisphere, which creates an activity imbalance between non-lesioned and lesioned hemispheres. Compensatory activity in the non-lesioned hemisphere leads mostly to unfavorable outcomes and further aggravated interhemispheric imbalance. Balanced interhemispheric activity with increased intrahemispheric coherence in the lesioned networks correlates with improved post-stroke recovery. TMS-EEG studies reveal the clinical importance of cortical reactivity and functional connectivity within the sensorimotor cortex for motor recovery after stroke. Although post-stroke motor studies support the prognostic value of TMS-EEG, more studies are needed to determine its utility as a biomarker for recovery across domains including language, cognition, and hemispatial neglect. As a complement to MRI-based technologies, EEG-based technologies are accessible and valuable non-invasive clinical tools in stroke neurology.

Abstract:▪ Abstract Quantitative electroencephalogram (qEEG) plays a significant role in EEG-based clinical diagnosis and studies of brain function. In past decades, various qEEG methods have been extensively studied. This article provides a detailed review of the advances in this field. qEEG methods are generally classified into linear and nonlinear approaches. The traditional qEEG approach is based on spectrum analysis, which hypothesizes that the EEG is a stationary process. EEG signals are nonstationary and nonlinear, especially in some pathological conditions. Various time-frequency representations and time-dependent measures have been proposed to address those transient and irregular events in EEG. With regard to the nonlinearity of EEG, higher order statistics and chaotic measures have been put forward. In characterizing the interactions across the cerebral cortex, an information theory-based measure such as mutual information is applied. To improve the spatial resolution, qEEG analysis has also been combined with medical imaging technology (e.g., CT, MR, and PET). With these advances, qEEG plays a very important role in basic research and clinical studies of brain injury, neurological disorders, epilepsy, sleep studies and consciousness, and brain function.

Abstract:Recent advances in signal analysis have engendered EEG with the status of a true brain mapping and brain imaging method capable of providing spatio-temporal information regarding brain (dys)function. Because of the increasing interest in the temporal dynamics of brain networks, and because of the straightforward compatibility of the EEG with other brain imaging techniques, EEG is increasingly used in the neuroimaging community. However, the full capability of EEG is highly underestimated. Many combined EEG-fMRI studies use the EEG only as a spike-counter or an oscilloscope. Many cognitive and clinical EEG studies use the EEG still in its traditional way and analyze grapho-elements at certain electrodes and latencies. We here show that this way of using the EEG is not only dangerous because it leads to misinterpretations, but it is also largely ignoring the spatial aspects of the signals. In fact, EEG primarily measures the electric potential field at the scalp surface in the same way as MEG measures the magnetic field. By properly sampling and correctly analyzing this electric field, EEG can provide reliable information about the neuronal activity in the brain and the temporal dynamics of this activity in the millisecond range. This review explains some of these analysis methods and illustrates their potential in clinical and experimental applications.

Abstract:Background: With the development of quantitative electroencephalography (QEEG), an increasing number of studies have been published on the clinical use of QEEG in the past two decades, particularly in the diagnosis, treatment, and prognosis of neuropsychiatric disorders. However, to date, the current status and developing trends of this research field have not been systematically analyzed from a macroscopic perspective. The present study aimed to identify the hot spots, knowledge base, and frontiers of QEEG research in neuropsychiatric disorders from 2000 to 2021 through bibliometric analysis. Methods: QEEG-related publications in the neuropsychiatric field from 2000 to 2021 were retrieved from the Web of Science Core Collection (WOSCC). CiteSpace and VOSviewer software programs, and the online literature analysis platform (bibliometric.com) were employed to perform bibliographic and visualized analysis. Results: A total of 1,904 publications between 2000 and 2021 were retrieved. The number of QEEG-related publications in neuropsychiatric disorders increased steadily from 2000 to 2021, and research in psychiatric disorders requires more attention in comparison to research in neurological disorders. During the last two decades, QEEG has been mainly applied in neurodegenerative diseases, cerebrovascular diseases, and mental disorders to reveal the pathological mechanisms, assist clinical diagnosis, and promote the selection of effective treatments. The recent hot topics focused on QEEG utilization in neurodegenerative disorders like Alzheimer's and Parkinson's disease, traumatic brain injury and related cerebrovascular diseases, epilepsy and seizure, attention-deficit hyperactivity disorder, and other mental disorders like major depressive disorder and schizophrenia. In addition, studies to cross-validate QEEG biomarkers, develop new biomarkers (e.g., functional connectivity and complexity), and extract compound biomarkers by machine learning were the emerging trends. Conclusion: The present study integrated bibliometric information on the current status, the knowledge base, and future directions of QEEG studies in neuropsychiatric disorders from a macroscopic perspective. It may provide valuable insights for researchers focusing on the utilization of QEEG in this field.

Abstract:Purpose of review Clinical electroencephalography (EEG) is a conservative medical field. This explains likely the significant gap between clinical practice and new research developments. This narrative review discusses possible causes of this discrepancy and how to circumvent them. More specifically, we summarize recent advances in three applications of clinical EEG: source imaging (ESI), high-frequency oscillations (HFOs) and EEG in critically ill patients. Recent findings Recently published studies on ESI provide further evidence for the accuracy and clinical utility of this method in the multimodal presurgical evaluation of patients with drug-resistant focal epilepsy, and opened new possibilities for further improvement of the accuracy. HFOs have received much attention as a novel biomarker in epilepsy. However, recent studies questioned their clinical utility at the level of individual patients. We discuss the impediments, show up possible solutions and highlight the perspectives of future research in this field. EEG in the ICU has been one of the major driving forces in the development of clinical EEG. We review the achievements and the limitations in this field. Summary This review will promote clinical implementation of recent advances in EEG, in the fields of ESI, HFOs and EEG in the intensive care.

Abstract:The electroencephalogram (EEG) is a cornerstone tool for the diagnosis, management, and prognosis of selected patient populations. EEGs offer significant advantages such as high temporal resolution, real-time cortical function assessment, and bedside usability. The quantitative EEG (qEEG) added the possibility of long recordings being processed in a compressive manner, making EEG revision more efficient for experienced users, and more friendly for new ones. Recent advancements in commercially available software, such as Persyst, have significantly expanded and facilitated the use of qEEGs, marking the beginning of a new era in its application. As a result, there has been a notable increase in the practical, real-world utilization of qEEGs in recent years. This paper aims to provide an overview of the current applications of qEEGs in daily neurological emergencies and ICU practice, and some elementary principles of qEEGs using Persyst software in clinical settings. This article illustrates basic qEEG patterns encountered in critical care and adopts the new terminology proposed for spectrogram reporting.

This technique measures scalp potentials with high temporal resolution, which permits the non‐invasive detection and analysis of neural oscillations. hdEEG data analyses can be conducted at the sensor level, as well as at the source level. Accurate localization of neural sources is a specific feature of hdEEG compared to standard low‐density systems. Notably, hdEEG source localization is achieved by combining realistic head models, which incorporate detailed information about the subject's anatomy and electrode positions, with innovative methods for inverse problem solutions. hdEEG offers a powerful and versatile tool for investigating neural correlates in motor and cognitive neuroscience experiments, as well as for clinical investigations. Electroencephalography (EEG) is a technique for non‐invasively measuring neuronal activity in the human brain using electrodes placed on the participant's scalp. With the advancement of digital technologies, EEG analysis has evolved over time from the qualitative analysis of amplitude and frequency modulations to a comprehensive analysis of the complex spatiotemporal characteristics of the recorded signals. EEG is now considered a powerful tool for measuring neural processes in the same time frame in which they happen (i.e. the subsecond range). However, it is commonly argued that EEG suffers from low spatial resolution, which makes it difficult to localize the generators of EEG activity accurately and reliably. Today, the availability of high‐density EEG (hdEEG) systems, combined with methods for incorporating information on head anatomy and sophisticated source‐localization algorithms, has transformed EEG into an important neuroimaging tool. hdEEG offers researchers and clinicians a rich and varied range of applications. It can be used not only for investigating neural correlates in motor and cognitive neuroscience experiments, but also for clinical diagnosis, particularly in the detection of epilepsy and the characterization of neural impairments in a wide range of neurological disorders. Notably, the integration of hdEEG systems with other physiological recordings, such as kinematic and/or electromyography data, might be especially beneficial to better understand the neuromuscular mechanisms associated with deconditioning in ageing and neuromotor disorders, by mapping the neurokinematic and neuromuscular connectivity patterns directly in the brain.

Abstract:Electroencephalography (EEG) is a non-invasive diagnostic tool, enabling us to assess the electrical activity of the brain and its disturbance in a great number of psychiatric conditions. This paper provides a narrative overview of the most recent advantages of EEG use in various psychiatric disorders that are associated. This article analyses selected psychiatric disorders. We discussed anxiety disorders characterized by chronic fear, dementia leading to cognitive decline, schizophrenia disrupting logical thinking, bipolar affective disorder with alternating episodes of mania and depression, and depression manifested by gradual loss of vital energy. We have shown that EEG testing, by monitoring the electrical activity of the brain, is a helpful tool in identifying specific brain wave patterns associated with these disorders. In the study of mental illness the EEG variant called quantitative EEG (QEEG) is of particular avail as to the spatial discrimination. QEEG is based on digital coding and transformation according to Fourier algorithm. Recently, even more complex methods of EEG data analysis have been implemented, including artificial intelligence (AI), deep learning (DL) and its branch: machine learning (ML). The use of sophisticated EEG postprocessing adds important information about the pathophysiology of mental disorders. More so, EEG/QEEG recording (in particular spectral analysis), if repeated over time may help to follow up the treatment results and to establish a prognosis as to the course of the given condition. Reliability, safety and availability of EEG makes it to be an indispensable tool in modern psychiatry. Use of EEG, QEEG may lead to faster and more precise differential diagnostic workup. Use of EEG/QEEG changes as an objective outcome measure in clinical trials may support the development of personalized pharmacotherapy or psychotherapy.

Abstract:The electroencephalography (EEG) signal is a noninvasive and complex signal that has numerous applications in biomedical fields, including sleep and the brain–computer interface. Given its complexity, researchers have proposed several advanced preprocessing and feature extraction methods to analyze EEG signals. In this study, we analyze a comprehensive review of numerous articles related to EEG signal processing. We searched the major scientific and engineering databases and summarized the results of our findings. Our survey encompassed the entire process of EEG signal processing, from acquisition and pretreatment (denoising) to feature extraction, classification, and application. We present a detailed discussion and comparison of various methods and techniques used for EEG signal processing. Additionally, we identify the current limitations of these techniques and analyze their future development trends. We conclude by offering some suggestions for future research in the field of EEG signal processing.

Abstract:Clinical EEG and Neuroscience, Ahead of Print. Over the last few decades, electroencephalography (EEG) has evolved from being a method that purely relies on visual inspection into a quantitative method. Quantitative EEG, or QEEG, enables the assessment of neurological disorders based on spectral features, dynamic characterizations of EEG resting-state activity, brain connectivity analyzes or quantification of EEG signal complexity. The information contained in EEG is multidimensional: Electrodes, positioned at different scalp locations, provide a spatial dimension to the analysis of EEG while time provides a dynamic dimension: This multidimensional property of EEG makes its quantification a challenging task. In this narrative review we present quantitative models focused on different aspects of EEG: While microstate models focus more on the quantification of the dynamic aspects of EEG, spectral methods, connectivity analysis and entropy based models are more concerned with its spatial aspects. Nevertheless, these diverse approaches have provided neurophysiology based biomarkers, especially for monitoring and predicting the course of various neurodegenerative disorders. However, their translation into clinical practice crucially depends on the ability to automate the analysis of EEG in a user-friendly manner, without compromising on the validity of the provided results. Once this has been accomplished, EEG would provide an inexpensive and widely available method for monitoring disease progression, identifying patients at risk of neurodegeneration—especially before the onset of clinical symptoms, and predicting future cognition. For stratification of patients to clinical trials, EEG would allow shortening the trial duration and lowering the number of necessary participants by identifying patients at risk of fast cognitive decline.

Taking the EEG Back Into the Brain: The Power of Multiple Discrete Sources

Abstract:Background: In contrast to many neuroimaging modalities, clinical interpretation of EEG does not take advantage of post-processing and digital signal analysis. In most centers, EEG is still interpreted at sensor level, exactly as half a century ago. A major task in clinical EEG interpretation is the identification of interictal epileptiform discharges (IEDs). However, due to the overlap of background activity, IEDs can be hard to detect in the scalp EEG. Since traditional montages, like bipolar and average reference, are linear transformations of the recorded channels, the question is whether we can provide linear transformations of the digital EEG to take it back into the brain, at least on a macroscopic level. The goal is to improve visibility of epileptiform activities and to separate out most of the overlap. Methods: Multiple discrete sources provide a stable linear inverse to transform the EEG into source space with little cross-talk between source regions. The model can be based on a few dipoles or regional sources, adapted to the individual EEG and MRI data, or on selected standard sources evenly distributed throughout the brain, e.g. below the 25 EEG standard electrodes. Results: Auditory and somatosensory evoked potentials serve as teaching examples to show how various source spaces can reveal the underlying source components including their loss or alteration due to lesions. Source spaces were able to reveal the propagation of source activities in frontal IEDs and the sequential activation of the major nodes of the underlying epileptic network in myoclonic epilepsy. The power of multiple discrete sources in separating the activities of different brain regions was also evident in the ongoing EEG of cases with frontal cortical dysplasia and bitemporal lobe epilepsy. The new source space 25 made IEDs more clearly visible over the EEG background signals. The more focal nature of source vs. scalp space was quantitatively confirmed using a new measurement of focality. Conclusion: Multiple discrete sources have the power to transform the EEG back into the brain by defining new EEG traces in source space. Using standard source space 25, these can provide for improved clinical interpretation of EEG.

Deep learning-based electroencephalography analysis: a systematic review

Abstract:Abstract Context . Electroencephalography (EEG) is a complex signal and can require several years of training, as well as advanced signal processing and feature extraction methodologies to be correctly interpreted. Recently, deep learning (DL) has shown great promise in helping make sense of EEG signals due to its capacity to learn good feature representations from raw data. Whether DL truly presents advantages as compared to more traditional EEG processing approaches, however, remains an open question. Objective . In this work, we review 154 papers that apply DL to EEG, published between January 2010 and July 2018, and spanning different application domains such as epilepsy, sleep, brain–computer interfacing, and cognitive and affective monitoring. We extract trends and highlight interesting approaches from this large body of literature in order to inform future research and formulate recommendations. Methods . Major databases spanning the fields of science and engineering were queried to identify relevant studies published in scientific journals, conferences, and electronic preprint repositories. Various data items were extracted for each study pertaining to (1) the data, (2) the preprocessing methodology, (3) the DL design choices, (4) the results, and (5) the reproducibility of the experiments. These items were then analyzed one by one to uncover trends. Results . Our analysis reveals that the amount of EEG data used across studies varies from less than ten minutes to thousands of hours, while the number of samples seen during training by a network varies from a few dozens to several millions, depending on how epochs are extracted. Interestingly, we saw that more than half the studies used publicly available data and that there has also been a clear shift from intra-subject to inter-subject approaches over the last few years. About of the studies used convolutional neural networks (CNNs), while used recurrent neural networks (RNNs), most often with a total of 3–10 layers. Moreover, almost one-half of the studies trained their models on raw or preprocessed EEG time series. Finally, the median gain in accuracy of DL approaches over traditional baselines was across all relevant studies. More importantly, however, we noticed studies often suffer from poor reproducibility: a majority of papers would be hard or impossible to reproduce given the unavailability of their data and code. Significance . To help the community progress and share work more effectively, we provide a list of recommendations for future studies and emphasize the need for more reproducible research. We also make our summary table of DL and EEG papers available and invite authors of published work to contribute to it directly. A planned follow-up to this work will be an online public benchmarking portal listing reproducible results.

Abstract:Electroencephalogram (EEG) is an important biomarker for neonatal encephalopathy (NE) and has significant predictive value for brain injury and neurodevelopmental outcomes. Quantitative analysis of EEG involves the representation of complex EEG data in an objective, reproducible and scalable manner. Quantitative EEG (qEEG) can be derived from both a limited channel EEG (as available during amplitude integrated EEG) and multi-channel conventional EEG. It has the potential to enable bedside clinicians to monitor and evaluate details of cortical function without the necessity of continuous expert input. This is particularly useful in NE, a dynamic and evolving condition. In these infants, continuous, detailed evaluation of cortical function at the bedside is a valuable aide to management especially in the current era of therapeutic hypothermia and possible upcoming neuroprotective therapies. This review discusses the role of qEEG in newborns with NE and its use in informing monitoring and therapy, along with its ability to predict imaging changes and short and long-term neurodevelopmental outcomes.

Shapes In a Box: Disassembling 3D Objects for Efficient Packing and Fabrication

Abstract:Modern 3D printing technologies and the upcoming mass‐customization paradigm call for efficient methods to produce and distribute arbitrarily shaped 3D objects. This paper introduces an original algorithm to split a 3D model in parts that can be efficiently packed within a box, with the objective of reassembling them after delivery. The first step consists in the creation of a hierarchy of possible parts that can be tightly packed within their minimum bounding boxes. In a second step, the hierarchy is exploited to extract the (single) segmentation whose parts can be most tightly packed. The fact that shape packing is an NP‐complete problem justifies the use of heuristics and approximated solutions whose efficacy and efficiency must be assessed. Extensive experimentation demonstrates that our algorithm produces satisfactory results for arbitrarily shaped objects while being comparable to ad hoc methods when specific shapes are considered.
Pitfalls in scalp EEG: Current obstacles and future directions

Abstract:Although electroencephalography (EEG) serves a critical role in the evaluation and management of seizure disorders, it is commonly misinterpreted, resulting in avoidable medical, social, and financial burdens to patients and health care systems. Overinterpretation of sharply contoured transient waveforms as being representative of interictal epileptiform abnormalities lies at the core of this problem. However, the magnitude of these errors is amplified by the high prevalence of paroxysmal events exhibited in clinical practice that compel investigation with EEG. Neurology training programs, which vary considerably both in the degree of exposure to EEG and the composition of EEG didactics, have not effectively addressed this widespread issue. Implementation of competency-based curricula in lieu of traditional educational approaches may enhance proficiency in EEG interpretation amongst general neurologists in the absence of formal subspecialty training. Efforts in this regard have led to the development of a systematic, high-fidelity approach to the interpretation of epileptiform discharges that is readily employable across medical centers. Additionally, machine learning techniques hold promise for accelerating accurate and reliable EEG interpretation, particularly in settings where subspecialty interpretive EEG services are not readily available. This review highlights common diagnostic errors in EEG interpretation, limitations in current educational paradigms, and initiatives aimed at resolving these challenges.

Abstract:Electroencephalography (EEG) is a mechanism to understand the brain’s functioning by analyzing brain electrical signals. More recently, it has been more commonly used in studies that are focused on the causation and effect of dementia. More tools are now available to gather EEG data. This brings about the challenge of understanding brain signals, which involves signal processing. Professionals with an electrical engineering background are very comfortable analyzing EEG data. Still, scientists in computer science and related fields need a source that can identify all the tools available and the process of analyzing the data. This paper deals specifically with the existing EEG data analysis tools and the processes involved in analyzing the EEG data using these tools. Furthermore, the paper goes in-depth into identifying the tools and the mechanisms of data processing techniques. In addition, it lists a set of definitions required for a better understanding of EEG data analysis, which can be challenging. The purpose of this paper is to serve as a reference for not only scientists that are new to EEG data analysis but also seasoned scientists that are looking for a specific data component in EEG and can go straight to the section of the paper that deals with the tool that they are using.

Abstract:Aim. Identification of the main trends of the latest studies of human neurophysiological characteristics using EEG. Materials and methods. Theoretical overview of modern scientific works on the topic of research, which is freely available in institutional repositories and catalogs, scientific information and search systems, international databases of scientific information. Results. The main areas of neurophysiological research using EEG are considered in the work. A review of scientific works over the past five years has shown that a significant place among research is traditionally occupied by the issue of identifying and evaluating pathological conditions: epilepsy, apnea, paroxysmal and vegetative states, Alzheimer's disease, and neuropsychiatric disorders. The practice of combining EEG with other methods of recording biological signals, such as ECG, EMG, etc., to increase the specificity of the obtained signs, is deepening. Multi-complex methods are becoming widespread. There is also a high interest in the study of psychophysiological processes depending on age, gender, profession and in human-machine interaction. There has been an increased interest in assessing the condition of persons who have been affected by traumatic events. A special interest of scientists was found in the application of EEG for research that can be attributed to the military sphere: determining the attention index of UAV operators, assessing the quality of aiming during the training of shooters, etc. The latest direction is the use of EEG for the assessment of neurological disorders and neurorehabilitation in the case of COVID-19. Conclusions. In general, the analysis of the latest works in the field of the application of EEG for the assessment of neurophysiological characteristics showed that there is still interest in the traditional directions of research into pathological conditions, such as epilepsy, Alzheimer's disease. In the research of psychophysiological processes, the leading role continues to be occupied by works dedicated to identifying the features of cognitive processes and mnestic functions under the influence of various stimuli, during training or performance of specific professional duties. Research using EEG in the military sphere received a new impetus. One of the areas of application of EEG to assess disorders after COVID-19.
