Abstract:The purpose of this study was to use participatory systems thinking to develop a dynamic conceptual framework of racial/ethnic and other intersecting disparities (e.g., income) in food access and diet in Philadelphia and to identify policy levers to address these disparities. We conducted three group model building workshops, each consisting of a series of scripted activities. Key artifacts or outputs included qualitative system maps, or causal loop diagrams, identifying the variables, relationships, and feedback loops that drive diet disparities in Philadelphia, Pennsylvania. We used semi-structured methods informed by inductive thematic analysis and network measures to synthesize findings into a single causal loop diagram. There were twenty-nine participants with differing vantages and expertise in Philadelphia's food system, broadly representing the policy, community, and research domains. In the synthesis model, participants identified 14 reinforcing feedback loops and one balancing feedback loop that drive diet and food access disparities in Philadelphia. The most highly connected variables were upstream factors, including those related to racism (e.g., residential segregation) and community power (e.g., community land control). Consistent with existing frameworks, addressing disparities will require a focus on upstream social determinants. However, existing frameworks should be adapted to emphasize and disrupt the interdependent, reinforcing feedback loops that maintain and exacerbate disparities in fundamental social causes. Our findings suggest that promising policies include those that empower minoritized communities, address socioeconomic inequities, improve community land control, and increase access to affordable, healthy, and culturally meaningful foods.

Community-Led Interventions to Address Food Inequity

Abstract:Approximately 1 in 6 children in the United States, and 1 in 5 children in our local county (Hamilton County, Ohio), are food insecure. Here, we describe a novel community–academic partnership to address food inequity through distributed leadership and shared power with local neighborhood leaders. Using neighborhood-level data and community voice, 3 Cincinnati neighborhoods with high rates of poverty and food insecurity were selected as the primary intervention targets. Neighborhood leadership councils with community members representing each neighborhood were created. These councils requested intervention proposals and then decided which community designed interventions would receive grant funding. The academic partner provided grant funding distribution, quality improvement support, and data guidance and support for all partners, as well as community engagement support if desired by the community-led intervention leaders. In its first year (2021–2022), 9 interventions were funded, moving more than $250 000 into community-designed and community-led interventions to promote food security in 3 disadvantaged neighborhoods. Through leveraging community partnerships, these initiatives supplied 89 039 equivalent meals, including 56 244 pounds of produce, serving at least 3106 families in 3 neighborhoods in Cincinnati. Critical to the success of the initiatives were distributed leadership, shared power, word of mouth, and community engagement. The success of this type of community–academic partnership shows promise to address a wide variety of social and health challenges.

Abstract:Multiple Chemical Sensitivity (MS) is characterized by a wide range of symptoms when affected patients come into contact with several chemical products. Very little is known of either the diagnosis or the suitable treatment. In this study we report the case of a 38-year-old woman, previously diagnosed as suffering from MCS, who was referred for an oral health evaluation. After removing all the existing restorations and extracting the diseased teeth, a removable partial denture without metal structure was made after testing her hypersensitivity to these materials. The one year follow-up of the patient did not show any remarkable reaction, confirming the reduction of a large number of MCS symptoms and the increase in her quality of life.
Increasing access to healthful foods: a qualitative study with residents of low-income communities

Abstract:Background: Inadequate access to healthful foods has been identified as a significant barrier to healthful dietary behaviors among individuals who live in low-income communities. The purpose of this study was to gather low-income community members' opinions about their food purchasing choices and their perceptions of the most effective ways to increase access to healthful foods in their communities. Methods: Spanish and English focus groups were conducted in low-income, ethnically-diverse communities. Participants were asked about their knowledge, factors influencing their food purchasing decisions, and their perceptions regarding solutions to increase access to healthful foods. Results: A total of 148 people participated in 13 focus groups. The majority of participants were female and ethnically diverse (63% Hispanic, 17% African American, 16% Caucasian, and 4% “other”). More than 75% of the participants reported making less than $1999 USD per month. Participants reported high levels of knowledge and preference for healthful foods. The most important barriers influencing healthful shopping behaviors included high price of healthful food, inadequate geographical access to healthful food, poor quality of available healthful food, and lack of overall quality of the proximate retail stores. Suggested solutions to inadequate access included placement of new chain supermarkets in their communities. Strategies implemented in convenience stores were not seen as effective. Farmers’ markets, with specific stipulations, and community gardens were regarded as beneficial supplementary solutions. Conclusion: The results from the focus groups provide important input from a needs assessment perspective from the community, identify gaps in access, and offer potential effective solutions to provide direction for the future.
Qualitative research study on addressing barriers to healthy diet among low-income individuals at an urban, safety-net hospital

Abstract:Background Some American households experience food insecurity, where access to adequate food is limited by lack of money and other resources. As such, we implemented a free 6-month Fruit and Vegetable Prescription Program within a large urban safety-net hospital . Methods 32 participants completed a baseline and postintervention qualitative evaluation about food-related behaviour 6 months after study completion. Deductive codes were developed based on the key topics addressed in the interviews; inductive codes were identified from analytically reading the transcripts. Transcripts were coded in MAXQDA V.12 (Release 12.3.2). Results The information collected in the qualitative interviews highlights the many factors that affect dietary habits, including the environmental and individual influences that play a role in food choices people make. Participants expressed very positive sentiments overall about their programme participation. Conclusions A multifaceted intervention that targets individual behaviour change, enhances nutritional knowledge and skills, and reduces socioeconomic barriers to accessing fresh produce may enhance participant knowledge and self-efficacy around healthy eating. However, socioeconomic factors remain as continual barriers to sustaining healthy eating over the long term. Ongoing efforts that address social determinants of health may be necessary to promote sustainability of behaviour change.
A new approach to a wicked problem: development of a cross-sector community-centered learning network to tackle childhood food inequity

Abstract:Food insecurity is a complex societal problem that disproportionately impacts households with children and those led by minoritized populations, with negative impacts on health across the life course. System to Achieve Food Equity adapted the learning systems model, used to address similarly complex problems, to tackle food insecurity at a neighborhood level. SAFE, born out of the COVID-19 pandemic, leverages a family-centered, community-based, cross-sector network fundamentally aimed at changing the food system so that all children in Cincinnati have the food they need to thrive. Through the following principles, Community-Led Network, Co-Production with Community, Equitable Sustainability, Learning to Learn Together, Distributing Leadership and Power, and Shared Data and Governance, SAFE has grown to over 300 individuals and 100 organizations, funded 9 novel interventions, distributed over 270,000 meals, and created a collaborative of motivated like-minded stakeholders. Future work includes improved data collection and sharing, support for increased stakeholder engagement and greater distribution of leadership and power, advocacy for policy change, refining measurement tools of network maturity for community settings, and collaboration with other efforts that contribute to food security indirectly.

Abstract:Food insecurity is a broad and serious public health issue in the United States, where many people are reporting lack of access to healthy foods. The reduced availability of healthy, affordable foods has led to increased consumption of energy-dense and nutrient-poor foods, resulting in increasing the risk for many chronic diseases such as obesity, cardiovascular diseases, and type 2 diabetes mellitus. Thus, identifying promising approaches to increase access to healthy foods through improving the food environment is of importance. The purpose of this review article is to highlight how the food environment affects directly a person's food choices, and how to increase access to healthy foods through improving environmental approaches. The literature search was focused on finding different approaches to improve food security, primarily those with an impact on food environment. Overall, potential solutions were gathered through multilevel environmental approaches, including nutrition education and peer education, community-based participatory research, and policy changes in supplemental nutrition programs. A recommendation to reduce food insecurity is learning to create meals with a variety of seasonal fruits and vegetables purchased from affordable farmers' markets.

Closer to the Root: The Potential of Traditional Food Systems Interventions for Obesity Prevention

Abstract:Abstract <sec> Objectives We review the project design, implementation experiences, and study findings from obesity prevention trials (OPTs) in Native American communities to identify future directions for obesity and chronic disease prevention. </sec> <sec> Methods We use a case study methodology to highlight three OPTs and identify themes across studies. We included three OPTs: Navajo Healthy Stores (NHS), OPREVENT1, and OPREVENT2. NHS was a food-store based environmental intervention, while OPREVENT1 and OPREVENT2 were multi-level, multi-component interventions with store, school, worksite, and media components (OPREVENT1) and combined with a policy-based component (OPREVENT2). </sec> <sec> Results All studies showed mixed success in achieving behavioral and health outcomes. NHS intervention exposure was associated with reduced BMI, and found improvements in intentions, cooking methods, and food getting comparing intervention with control regions. The program was sustained for several years after the trial was completed. OPREVENT1 trial resulted in significant reductions in waist circumference, reduced soda consumption, and increased physical activity (PA) comparing intervention and comparison communities. The OPREVENT2 trial had significant reductions in PA, calorie intake, carbohydrate, and dietary fat intake comparing intervention and comparison communities. </sec> <sec> Conclusions Working with health staff can enhance intervention sustainability, which is also impacted by health center capacity and funding. All studies had strong evaluation designs, with high retention and low refusal rates. All studies included store components that aimed to improve healthy food access on-reservation and point-of-purchase promotion. Although all studies demonstrated impacts on behaviors and modest impacts on health outcomes, taking a food-systems approach could increase the likelihood of improved outcomes by addressing the root of healthy food access and obesity: diminished traditional food systems. These interventions hold great potential for supporting significant health outcomes by supporting traditional food-getting practices, preparation, and intake. Such interventions require strong community-academic partnerships since these food systems and knowledge belong to tribal nations and communities. </sec> <sec> Funding Sources NHLBI & USDA. </sec>

Abstract:Background: Research on childhood nutritional environments, which are predictive of a variety of child and family health and wellbeing outcomes, has primarily focused on parenting behaviors and the home food environment. However, broader social and community factors play an important role in shaping nutritional environments during early childhood. Food security is a key example that is closely linked to nutrition-related health disparities, which can arise from constrained financial resources and material deprivations. Understanding parent and caregiver lived experiences related to social and community components of family nutritional environments in the context of economic constraints could help to support multi-level nutritional practices that are optimal for healthy child development. Method: Semi-structured in-depth interviews were conducted using a narrative approach with parents (n = 21) of pre-school aged children living near or below the poverty line. Participants were recruited from social and community service sites pertinent to maternal and child health in Philadelphia where health disparities are highly concentrated. Analysis was conducted using MAX QDA analytic software following a systematic approach for thematic analysis in narrative research. Results: Three themes describe the feeding and eating environments in the home and community context: 1) Food Procurement and Meal Planning; 2) Family Togetherness; 3) Creativity, Variety, and Flexibility. Creative decision making and food procurement strategies responsive to child preferences were balanced by shopping to save and visiting multiple stores for the best foods and savings. Logistical (e.g., transportation, childcare) and emotional supports facilitated food shopping habits and routines, while lack of trusted childcare, reliable transportation, and social support barriers challenged feeding and eating decisions. Conclusion: Strategies for improving cost and time-efficiencies could help support health-promoting nutritional environments of families with food insecurity. Building social and community capacity and facilitating connections to practical supports that include trustworthy childcare and peer support may be particularly important for this population.

Abstract:Objective: To examine associations between Theory of Planned Behavior constructs related to parent engagement in health-promoting behaviors and food resource management with household food security status. Methods: A cross-sectional secondary analysis of baseline data from 1622 parents enrolled in the ENCIRCLE study, a pragmatic cluster-randomized controlled trial to prevent obesity among preschool-aged children from rural lower-income families. Logistic regression models examined associations between parent engagement in health-promoting behaviors (attitude, subjective norm, perceived behavioral control) and food resource management (self-confidence, behaviors) with household food security status. Results: Parents with greater perceived behavioral control (OR=1.44, CI:1.14-1.82) and food resource management self-confidence (OR=2.08, CI:1.80-2.41) had significantly higher odds of experiencing household food security in adjusted models. Conclusions and Implications: Findings suggest that perceived behavioral control and food resource management self-confidence may help safeguard lower-income families. Health promotion efforts should target these factors to help parents engage in health-promoting behaviors during times of economic hardship.

The Development of a Satellite Food Pantry Through Community-Based Participatory Action Research

Abstract:Food for a Long Life (FFLL) is a community– university–extension project that utilizes intergenerational strategies and a communitybased participatory action research approach to increase healthy food access, consumption, and education for preschoolers and older adults. As a Children, Youth, and Families at Risk sustainable community project of the U.S. Department of Agriculture, FFLL is within its fifth and final year working collaboratively at an intergenerational site in a midwestern United States community identified as being food insecure. During Project Years 2–4, FFLL engaged in iterative communication and relationship-building with stakeholders and a local food bank to identify and address community needs, resulting in the creation of a satellite food pantry. Prior to the COVID-19 pandemic, the Food Satellite served approximately 60 family members each month. FFLL strives for program sustainability beyond the grant funding period by continuing community partnerships and training staff to facilitate the satellite food pantry operations.
Engaging Youth in Food Activism in New York City: Lessons Learned from a Youth Organization, Health Department, and University Partnership

Abstract:Research indicates that insufficient emphasis on community collaboration and partnership can thwart innovative community-driven work on the social determinants of health by local health departments. Appreciating the importance of enhancing community participation, the New York City Department of Health and Mental Hygiene (DOHMH) helped lead the development of the Health Equity Project (HEP), an intervention aimed at increasing the capacity of urban youth to identify and take action to reduce food-related health disparities. DOHMH partnered with the City University of New York School of Public Health and several local youth organizations to design and implement the intervention. HEP was conducted with 373 young people in 17 cohorts at 14 unique sites: six in Brooklyn, six in the Bronx, and two in Harlem. Partnered youth organizations hosted three stages of work: interactive workshops on neighborhood health disparities, food environments, and health outcomes; food-focused research projects conducted by youth; and small-scale action projects designed to change local food environments. Through these activities, HEP appears to have been successful in introducing youth to the social, economic, and political factors that shape food environments and to the influence of food on health outcomes. The intervention was also somewhat successful in providing youth with community-based participatory research skills and engaging them in documenting and then acting to change their neighborhood food environments. In the short term, we are unable to assess how successful HEP has been in building young leaders who will continue to engage in this kind of activism, but we suspect that more extended interactions would be needed to achieve this more ambitious goal. Experiences at these sites suggest that youth organizations with a demonstrated capacity to engage youth in community service or activism and a commitment to improving food or other health-promoting community resources make the most suitable and successful partners for this kind of effort.

Abstract:Local effects of major cations on coronary vascular resistance were studied in the beating dog heart. This was accomplished by shunting the blood around the heart and lungs, clamping the arch of the aorta and perfusing arterial blood at a constant rate into the ascending aorta. Perfusion pressure was measured during intracoronary infusion of isotonic solutions of NaCl, KCl, CaCl 2 , MgCl 2 , and MgSO 4 and hypertonic solutions of NaCl and KCl at rates which raised cation concentrations in coronary blood without significantly affecting concentrations generally within the body. Coronary vascular resistance decreased as a function of the infusion rate of isotonic solutions of KCl, MgCl 2 , and MgSO 4 and increased as a function of the infusion rate of an isotonic solution of CaCl 2 . Isotonic NaCl had no effect. Resistance changes occurred before measurable change in the proportion of a minute spent in electrical systole by the ventricle. These findings, together with those previously reported for the dog foreleg, suggest that the coronary vascular bed is actively dilated by localized slight increase in plasma concentration of K or Mg and actively constricted by localized slight increase in plasma concentration of Ca.
Examining the impact of a school-based fruit and vegetable co-op in the Hispanic community through documentary photography

Abstract:Hispanic families are at elevated risk for certain diet related illnesses including obesity and diabetes. Brighter Bites is a 16-week school-based program that delivers produce and nutrition education to high-need areas. The purpose of this study was to explore the adoption of Brighter Bites healthy eating strategies in low-income Spanish-speaking families as well as barriers to the sustainability of improved dietary behaviors using Photovoice. Two researchers worked with a predominately Hispanic, low-income public school to conduct the project. Spanish speaking parents participating in Brighter Bites were recruited and five mothers completed the study. Participants developed research questions, took documentary photos, attended group analysis sessions, and organized a community event. Photos from participants were gathered at two time points: i) during the Brighter Bites produce distributions, and ii) when the distributions were no longer in session. Participants organized resulting photos into themes during discussion groups that proceeded each photo-taking period. Themes that emerged during the produce distribution period included: cost savings, increased variety and accessibility of fresh produce, and ability to practice healthy eating. Themes that emerged when the weekly produce distributions were no longer in session included: increased costs, lack of variety, the continued effect of Brighter Bites, and innovative ways to cook with produce. Overall, participants were enthusiastic about the benefits they experienced with the program, but noted hardships in accessing nutritious food without the produce distributions in session. Innovative approaches are needed to create sustainable pathways to healthful food access after community nutrition interventions are complete.
Leveraging Nutrient-Rich Traditional Foods to Improve Diets among Indigenous Populations in India: Value Chain Analysis of Finger Millet and Kionaar Leaves

Abstract:Many indigenous foods are nutrient-rich but are often underutilized even among populations at high risk of malnutrition. The aims of this study were to conduct value chain analysis of one cultivated crop (finger millet among the Munda tribe) and one wild green leafy vegetable (Koinaar leaves among the Sauria Paharia tribe) of two Indigenous communities in Jharkhand state, India and to identify entry points for interventions aimed at supporting production and consumption. Semi-structured interviews were conducted with stakeholders among each tribal group and transcripts were open coded and organized based on key themes across the steps of the value chain for each food independently. Improved storage techniques and infrastructure, machinery for processing and improved cooking fuel would help reduce barriers across the finger millet supply chain related to postharvest losses, processing labor and safety concerns related to cooking. For Koinaar leaves, improving drying techniques to increase consumption across seasons and providing training and support to increase opportunities for selling leaves in local markets, where participants mentioned potential language barriers, could strengthen the supply chain. Improving extension services and focusing beyond production has potential to improve the production and consumption of both nutrient-rich crops among Indigenous communities in India.
Midventricular obstruction causing abnormal flow patterns in hypertrophic cardiomyopathy.

Abstract:In rare cases of hypertophic cardiomyopathy a midventricular obstruction can exist, dividing the left ventricle into an apical and a basal part. Abnormal flow between these two compartments can be detected by Doppler echocardiography. The apical chamber can develop into an apical aneurysm. We describe two patients with midventricular obstruction and two distinct flow patterns in the leftventricle. Both patients (female, 70 and 82 years) presented with chest pain. One ofthem had a left bundle branch block, the other had an abnormal ECG consistent with left ventricular hypertrophy. Both had normal coronary arteries on angiography. The upper panel of figure 1 shows a continuous wave Doppler signal through the left ventricle. It clearly demonstrates an abnormal flow that is directed from the apex towards the base ofthe heart. The flow starts in the second halfof systole and continues through diastole up to the start of the A wave. After the A wave, it appears again as a proto-systolic flow from apex to base. The lower panel shows a dynamic gradient of 43 mmHg in the outflow tract and the same abnormal midventricular flow in a pulsed Doppler signal employing a high pulse repetition frequency (HPRF) to depict high velocity signals. In figure 2, an angiogram ofthe left ventride, the double-chambered left ventricle can be appreciated. Patient 2 was different in that the abnormal midventricular flow was directed from base to apex, as can be seen in the upper panel of figure 3, a pulsed Doppler reading from the mid left ventricle. The lower panel shows the pulsed Doppler signal obtained from the mitral valve orifice in which the abnormal flow is absent. Qualitative Exploration of Cultural Influence on a Rural Health-Promotion Initiative

PSE initiatives to increase access to healthy food and physical activity opportunities through a community coalition model. Phenomenon of interest: Social norms and cultural influences surrounding community members' food preferences, physical activity behavior, and future hopes for community development. Analysis: Abductive content analysis. Results: Major categories on food social norms (subcategories: physical health, eating habits, and food preference), race relations, generational factors, physical activity social norms, and hopes for the community (subcategories: increased engagement, health, awareness, cohesion, and inspiration) were discussed in relation to the progress of PSE initiatives. Conclusions and implications: Because of community member perceptions, PSE initiatives became associated with factors beyond food and fitness, such as race relations, generational differences, and community cohesion. A focus on increased youth and church involvement, community values, relationship building, and input from diverse voices can be foundational to culturally-appropriate PSE efforts in rural settings.
Understanding Barriers and Facilitators to Healthy Eating and Active Living in Rural Communities

Abstract:Objective . Studies demonstrate that people’s food and physical activity (PA) environments influence behavior, yet research examining this in rural communities is limited. Methods . Focus groups of 8–15 women were conducted in rural communities in seven US states. Questions were designed to identify factors within residents’ food and PA environments they felt helped or hindered them from eating healthfully and being physically active. Results . Participants were aged 30–84 years; mean (SD) = 61 (14) ( N = 95 ). On average, communities had fewer than 5,000 residents. Limited time, social norms, and distances from or lack of exercise facilities were common PA barriers. Facilitators for PA included social support, dog walking, and availability of affordable facilities. Healthy eating barriers included the perception that healthy foods were too expensive; calorically dense large portion sizes served at family meals; and frequency of eating foods away from home, which were perceived as generally unhealthy. Healthy eating supports included culture/value around local food gathering (e.g., hunting and gardening) and preservation (e.g., canning and smoking). Friends and family were frequently identified as key influencers of eating and PA behavior. Conclusions . Targeting both social and built environment factors, particularly those unique to rural locales, may enhance support for healthy eating and PA behavior change interventions.
Feminist action research in response to food insecurity among college students

Abstract:Feminist action research can instigate community, organizational, and policy changes to address social issues. We describe how we considered inclusion, participation, action, social change, and researcher reflexivity, as well as researcher positionality, in design and implementation of the 2016 Michigan College Campuses Food Pantry Summit. Specifically, in this case study, we describe ways we used these principles to plan this event around concept mapping, an interactive, collaborative research approach. Our event led to the formation of a regional community of practitioners, enabled an exchange of institutional best practices among campus pantries, and informed federal policy on food access for college students.

Abstract:Early childhood provides an opportunity to optimize growth and development and parents play a fundamental role in forming healthy eating habits in their children. A healthy diet improves quality of life and wellbeing and reduces the risk of chronic disease. The aim of this research was to explore parents’ experiences of feeding 0–5-year-old children and food literacy behaviors. This qualitative study employed a general inductive inquiry approach. Participants were recruited through community-based parenting organizations in disadvantaged areas. Eight focus groups were conducted with 67 parents (92.5% female) living in socially disadvantaged areas within metropolitan Perth of Western Australia. Ten themes emerged from the preliminary analysis and were aligned with domains of relatedness, autonomy, and competence within the self-determination theory. Themes included relatedness (1) feeding is emotional, (2) variations in routine and feeding structures, (3) external influences, autonomy (4) power struggles, (5) it must be quick and easy, (6) lack of strategies for feeding autonomy, competency (7) whatever works, (8) healthy is important but for some unattainable, (9) improvements in food literacy skills, and (10) conflicting information overload. This research informed the development of a food literacy program for parents. Parents faced many challenges when trying to provide healthy food. This research has shown parents would benefit from support to achieve healthy eating practices for their families.
