Abstract:Background Lumbar intervertebral disc herniation and spinal tumor are major pathologies that may cause back pain and radiculopathy. Neurological symptoms resulting from disc herniation and intradural spinal tumor together, however, are very rare. Case presentation We report a case of lumbar disc herniation which coexists with intradural schwannoma at the same spinal level in a 67-year-old man. The patient presented with persistent low back pain, sciatica, and weakness of the lower limbs. Contrast lumbar spine magnetic resonance (MR) imaging clearly delineated an intradural lesion and an extradural herniated disc at L3/4 level. Using a single posterior approach, both pathologies were addressed. Pathological studies confirmed the intradural lesion was schwannoma. Conclusion The case report highlights a rare concomitance of two symptomatic pathologies in a lumbar spine, which deserves clinical attention. Complete history, careful physical examination, and investigative measures, such as contrast MR imaging, are helpful to establish throughout diagnoses.
Lumbar spinal chondroma presenting with radiculopathy: case report

Abstract:Chondroma is a very unusual cartilagenous neoplasm of the spine. Here we are present a case of spinal chondroma with radiculopathy. A 54-year-old female patient consulted with progressive low back pain and left femoral numbness. Lumbar spinal Magnetic resonance (MR) imaging studies showed an extradural mass lesion in the left L2 body. Computerized tomography (CT) did not reveal any osteolytic lesion of the bone. The mass lesion was excised totally by left partial hemilaminectomy and the intradural compartment was also checked. The histopathology of the lesion was confirmed as chondroma. Preoperative evaluation and meticulous pathological analysis are required because of the malignant transformation potential of these rare pathologies.
Extradural chondroma presenting as lumbar mass with compressive neuropathy

Abstract:The identification of a soft tissue chondroma within the spine represents a rarity and is typically not included within the differential diagnosis for patients with sensory complaints of the leg. The authors describe 46-year-old female presenting with 3-week history of decreased sensation and paresthesias of the left leg. Magnetic resonance imaging of the lumbar spine demonstrated an L3 extradural soft tissue mass. She underwent an uncomplicated excision through an L3 laminectomy and exhibited complete resolution of symptoms. Pathologic examination revealed benign cartilaginous tissue; however, the authors recommend long-term follow-up for such lesions as the potential for malignant transformation is unknown.
Lumbar spinal chondroma presenting with acute sciatica

Abstract:A 47-year-old woman visited with lumbago and severe left leg pain that had been presented for 1 week. The patient complained of severe radiating pain on left L3 sensory dermatome area and reported aggravation of leg pain at 20 degrees of hip flexion by straight leg raising test (SLRT). However, there was no motor weakness on neurological examination. Magnetic resonance imaging (MRI) demonstrated contrast enhancing spinal extradural mass at L2-3 level that was iso-signal intensity (SI) on T1-weighted images (WI), hypo-SI on T2WI. She was not able to walk and sleep due to incapacitating pain. Thus, surgical removal was performed via left partial laminectomy. Postoperatively, the radiating pain was relieved completely. Histopathologic examination revealed that the tumor consisted of chondroma, which had mature hyaline cartilage with nests of benign-appearing cells and calcium deposits in lacunae.

Lumbar intervertebral disc cyst as a cause of radiculopathy

Abstract:Background context: Lumbar radiculopathy is commonly caused by degenerative conditions such as a herniated disc or lumbar spinal stenosis. Less common etiologies include intraspinal extradural masses such as synovial cysts and gas-containing ganglion cysts. Intraspinal extradural cysts that communicate with the intervertebral disc are a rare entity and thus, an uncommon cause of lumbar radiculopathy. There are only ten cases of an intervertebral disc cyst reported in the literature. Purpose: To document the first reported case of an intervertebral disc cyst in North America. Two series of Japanese patients with intervertebral disc cyst confirmed radiographically and intraoperatively have recently been reported. Study design: A case report of an intervertebral disc cyst at L4-5 causing an L5 radiculopathy. Outcome measures: Japanese Orthopaedic Association score for low back pain. Methods: The patient and the authors involved in the patient's management were interviewed. All medical records, radiographic imaging studies, intraoperative findings, and pertinent literature were also reviewed. Results: Preoperative magnetic resonance imaging (MRI) scan demonstrated a well-localized cyst compressing the ventral aspect of the thecal sac at L4-L5. Discography and subsequent computed tomographic (CT) scanning demonstrated the cyst communicating with an intervertebral disc herniation via an annular rupture. Decompressive discectomy and surgical excision of the disc cyst from the spinal canal resulted in complete recovery and resolution of the preoperative radiculopathy. Clinical improvement was documented using the JOA scoring system. Patient's preoperative score was 4/15, and postoperative score was 15/15. Conclusion: Although exceedingly rare, an intervertebral disc cyst should remain in the differential diagnosis of any extradural intraspinal mass ventral to the thecal sac. Diagnosis of an intervertebral disc cyst requires recognition of this uncommon entity and a high index of suspicion. Discography and post-discography computerized tomography (CT) scan confirm the diagnosis. Operative treatment includes decompression, and excision of the cyst and is reserved only for cases in which the cyst results in clinical symptoms unresponsive to nonoperative management.
Cervical spine chondroma arising from C5 right hemilamina: a rare cause of spinal cord compression. Case report and review of the literature

Abstract:Chondromas are rare primary spine tumors. Only 12 cases of chondromas located in the cervical spine have been reported in the literature up to now. The authors report a case of a cervical periosteal chondroma in a 38-year-old man presenting with neck pain and a syndrome of spinal cord compression. Magnetic resonance imaging revealed a lesion that was hypo- or isointense on T1-weighted images and iso-hyperintense on T2-weighted images. Administration of Gd-DPTA resulted in margin enhancement on T1-weighted images. CT scans revealed a hyperdense calcified lesion arising from the internal surface of C5 right hemi-lamina. The patient underwent a right C5 hemi-laminectomy, with complete en-bloc removal of the lesion. The authors emphasize that early identification of the initial lesion should be coupled with total surgical resection, as a definitive treatment, in order to prevent malignant transformation.

Abstract:Chondromas are benign cartilaginous tumors that occur very rarely in the spine. Moreover, chondromas of the extraskeletal origin are also very rare. In this case report, we describe an extremely rare case of chondroma arising from the ligamentum flavum in the cervical spine. A 67-year-old man presented to our clinic with acute-onset spastic quadriparesis. We performed emergent magnetic resonance imaging and found an epidural mass in the right ligamentum flavum at C4-C5. The acute-onset presentation was suspicious for intraligamentous hematoma in the ligamentum flavum at this level. We performed emergency decompression surgery and en bloc removal of the epidural mass with the right C4 and C5 lamina. The lesion had the appearance of cartilaginous tissue rather than hematoma. Based on the histological investigation, the final diagnosis was intraligamentous chondroma of the cervical spine. The quadriparesis improved postoperatively, as did the results of manual muscle testing in the affected area, and he was able to resume walking independently with a cane. At the one-year follow-up, the manual muscle testing results were almost normal. Surgeons should keep in mind the possibility of benign tumors including chondroma of the cervical spine when a patient presents with acute-onset quadriparesis.
Soft tissue chondroma mimicking "dumbbell" neurogenic tumour: a rare cause of lumbar radiculopathy

Abstract:Soft tissue chondromas are rare, benign extra-skeletal tumours of cartilaginous origin, which may rarely occur within the spinal canal. We report a patient who presented with left lumbar radiculopathy, and a peripherally-enhancing dumbbell shaped soft tissue mass involving both the intraspinal and extraspinal compartments on MRI. This was misdiagnosed as a neurogenic tumour and the patient underwent surgical removal. Histological examination revealed cartilaginous tissue. Although rare, soft tissue chondromas should be considered in the differential diagnosis of spinal dumbbell shaped tumours, especially if MRI shows peripheral contrast enhancement.

Abstract:A case of isolated primary non-Hodgkin's lymphoma of the spine is presented along with a review of previous cases reported in the literature. An 80-year old immunocompetent man was admitted with progressive numbness and weakness of both lower extremities of 2 weeks' duration. Magnetic resonance imaging (MRI) of the cervicothoracic spine revealed an extradural spinal mass at the T1 - T3 level, with invasion into the T3 vertebral body. The lesion was at first thought to be a metastatic tumour. Because of the underwent laminectomy and posterior decompression. Histopathological examination revealed diffuse large B-cell non-Hodgkin's lymphoma. All other investigations (including computed tomography of the chest and abdomen, bone scan, peripheral blood examination and tumour marker examination) were negative for occult disease. This case demonstrates that neurosurgeons should be aware of the occurrence of isolated primary spinal lymphoma, which should be included in the differential diagnosis of manifestations of compression, the patient metastatic tumours.
Chondrosarcoma of the dorsal spine - A rare case

Abstract:Chondrosarcomas of the spine are rare tumors and represent <10% of all chondrosarcomas. In the spine, they may arise from vertebral bodies or posterior elements. They may occur in patients ranging from 13 to 78 years of age. Here, we present a case of a 25-year-old female who presented with complaints of lower backache, stiffness in both lower limbs, and tingling sensation in the right lower limb. On examination, both power and sensations were decreased below waist. Magnetic resonance imaging spine revealed an extradural lesion at D5 vertebral body level with severe cord compression. We received the mass in multiple fragments which were grayish-white and firm to hard in consistency. Microscopically, a chondroid tumor was seen with cells arranged in lobules in abundant myxoid matrix. The neoplastic chondrocytes were large in size and had bizarre hyperchromatic nuclei. Few binucleate and multinucleate forms were also seen along with occasional atypical mitoses. There was permeation and destruction of the host bone, and the tumor was seen invading the marrow spaces. Few foci showed high cellularity. No osteoid formation was seen by the tumor. The tumor was diagnosed as Chondrosarcoma - Grade II.
Role of Veratryl Alcohol in Regulating Ligninase Activity in Phanerochaete chrysosporium

Abstract:Ligninase activity in Phanerochaete chrysosporium is stimulated by incubating cultures with various substrates for the enzyme, including veratryl (3,4-dimethoxybenzyl) alcohol, which is a secondary metabolite of this fungus. This study was designed to provide insight into the mechanism involved in this stimulation. Ligninase activity increased 2 to 4 h after the addition of exogenous veratryl alcohol to ligninolytic cultures. This increase was prevented by inhibitors of protein synthesis. Analysis of the extracellular proteins by high-performance anion-exchange liquid chromatography revealed increases in the amounts of some, but not all, ligninase species. The normal rapid decrease in ligninase activity in aging cultures was not prevented or retarded by veratryl alcohol, indicating that veratryl alcohol does not increase ligninase activity by protecting extant enzyme. We conclude that veratryl alcohol probably functions via an induction type of mechanism, affecting only certain ligninase species. Results with an isolated lignin indicate that lignin (or its biodegradation products) functions in the same way that veratryl alcohol does.

Abstract:Chondromyxoid fibroma (CMF) is a rarely documented benign osseous neoplasm, particularly with respect to its incidence in the lumbar spinal region. CMF predominantly manifests in vertebral bodies, exhibiting atypical emergence in ancillary anatomical sites. The present report describes, to the best of our knowledge, the second documented instance of CMF originating from the lumbar facet joint. The present case provides an example of CMF in the lumbar facet joint precipitating spinal canal stenosis, thereby engendering neurological manifestations in the lower extremities due to neoplastic proliferation through the intervertebral foramen. The present therapeutic intervention entailed surgical excision of the neoplasm concomitant with facet joint arthrodesis, with the objective of achieving comprehensive neoplasm eradication, ameliorating the symptomatology and safeguarding the spinal structural integrity of the patient. The present study aimed to illustrate the clinical implications of this rare neoplasm, thereby elucidating the diagnostic quandaries and therapeutic complexities associated with CMF in the lumbar facet joint. In addition, the present study aimed to augment the existing knowledge for the diagnosis and clinical management of CMF.

Clinical diagnostic and radiographic features of a primary intradural spinal chondrosarcoma in a young adult: illustrative case

Abstract:Background: Mesenchymal chondrosarcoma (MCS) is an aggressive subtype of chondrosarcoma that occurs extremely rarely in the central nervous system. Patients often present with pain or sensorimotor deficits, and resection is considered the gold standard. The role of adjuvant radiation and/or chemotherapy is largely unknown. Observations: A 22-year-old male presented with a 4-month history of progressive back and bilateral leg pain. He underwent imaging workup with magnetic resonance imaging of the lumbar spine and was found to have an intradural, extramedullary, heterogeneously enhancing mass spanning the L4-5 vertebral levels. Intraoperatively, a lobular, partially calcified mass with a ventral dural attachment displacing the nerve roots laterally was observed. The mass was removed en bloc, and the patient later underwent adjuvant radiotherapy, with no evidence of recurrence 2 years following surgery. Lessons: Spinal MCS is extremely rare and often presents with a more aggressive course than conventional chondrosarcoma. Radiological diagnosis is challenging, as the tumor mimics different pathologies. The presence of calcifications, heterogeneous enhancement, and a more rapid clinical course as well as the presence of HEY1::NCOA2 gene fusion, which can be detected by surrogate immunohistochemistry, aids in diagnosis. Resection is the standard of care, and adjuvant radiation may be considered to reduce local recurrence, although further studies are warranted.

Lumbar Vertebral Fracture Through a Pre-Existing Schmorl's Node Mimicking Histopathologically a Low-Grade Chondrosarcoma

Abstract:The aim of this paper is to present a unique, to the best of our knowledge, case of a patient with a fracture of the first lumbar vertebra (L1), which occurred through a pre-existing Schmorl's node (SN), with histopathological characteristics mimicking a low-grade chondrosarcoma that initially led to a false diagnosis. A 54-year-old woman tripped and fell to the ground, sustaining a fracture of the L1 vertebral body. She was treated conservatively with gradual mobilization using a thoracolumbar brace for six weeks. Due to persistent pain and her inability to achieve full mobilization, she was offered vertebral kyphoplasty. During the same operative session and just before the kyphoplasty, she underwent a core-needle biopsy of the affected area. Following her operation, she reported a gradual, yet quick and full remission of her symptoms. The pathology report indicated findings consistent with a low to mid-grade chondrosarcoma. A re-evaluation of the specimen by a different pathologist confirmed the diagnosis of low-grade chondrosarcoma. Subsequently, she underwent full oncological staging, which was negative for metastases. Additional imaging studies failed to show signs of local disease progression. Due to the discordance between the pathology reports and the imaging and clinical findings, her case was referred to our specialized center for spinal tumor surgery. A new pathological re-evaluation of the biopsy samples was performed, and the diagnosis of low-grade chondrosarcoma was once again confirmed. However, during the multidisciplinary tumor (MDT) meeting that followed, and after careful evaluation of subsequent imaging studies that showed signs of local improvement and due to the complete lack of symptoms, the histopathological findings were re-evaluated and attributed to the fracture occurring through a pre-existing SN penetrating the cancellous bone of the vertebra. This complex situation contributed to histopathological findings consistent with a well-differentiated chondrosarcoma. The patient remains symptom-free 10 months following her operation and has fully returned to her previous activities. Our unique case highlights the importance of an MDT meeting when evaluating patients with musculoskeletal tumors and emphasizes the need for increased awareness when clinical findings and imaging studies are in discordance with histopathology reports.
Dumbbell Schwannoma Complicated by Intradural Lumbar Disc Herniation at the Same Level—a Rare Case Report

Abstract:Introduction: Dumbbell schwannoma complicated by intradural lumbar disc herniation at the same level is extremely rare. Case presentation: We describe a 67-year-old female presented with low back pain for 10 years and weakness and numbness of the right lower extremity for one month. Contrast magnetic resonance imaging (MRI) revealed an inhomogeneous enhanced dumbbell lesion at the L4/5 right intervertebral foramen and the intradural part of the lesion is only annular enhanced. A single posterior-approach surgery, including L4 laminectomy and right L4/5 facet joint resection, intracapsular resection of extradural dumbbell tumor, and intradural nucleus pulposus resection, was performed. The patient's back pain and weakness of right lower extremity disappeared. Histological studies revealed a schwannoma and a herniated disc. Conclusions: To our knowledge, this case is the first report of dumbbell schwannoma complicated by intradural lumbar disc herniation. Contrast MRI is helpful in preoperative differential diagnosis. Careful preoperative images reading are important to prevent misdiagnosis.
Subaxial cervical foraminal chondromas: case-based discussion on surgical management

Abstract:Cervical foraminal chondromas are benign lesions that may require surgical resection when symptomatic due to radicular and/or spinal cord compression. The aim of surgery is to achieve gross tumor removal while preserving neurological function and spine stability. The authors describe a case of subaxial foraminal chondroma with a systematic review of the literature on patients with cervical chondromas. In the reported case, the authors used a retrojugular approach to remove a C6-C7 right chondroma without the need for spinal stabilization. Literature review identified a total of 11 patients who underwent surgery for subaxial foraminal chondroma. The mean age at diagnosis is 33.6 years (range: 10–73). Most patients report neurological symptoms at the time of diagnosis. The most frequently involved vertebral level is C4-C5 (54.6%, 6/11). Preoperative foraminal enlargement is present in 63.6% (7/11) of patients. Surgical resection is performed via an anterior approach in 18.2% (2/11) of patients, with vertebral body resection and concomitant cervical instrumentation. The anterolateral approach is selected in 27.2% (3/11) of patients, and the posterior approach in 54.6% (6/11) of patients, with only one patient requiring both anterior and posterior instrumentation. The choice of surgical access for subaxial foraminal chondroma can be challenging due to the anatomical location of the tumor in relation to the cervical nerve roots and spinal cord. Accurate approach selection is key to achieving complete tumor removal while preserving cervical spine stability.

Abstract:In the present work, an effort has been made to propose and implement a new steganographic technique for images by modifying existing algorithms. This technique uses LSB steganography as the basis and randomly disperses the secret message over the entire image to ensure that the secret message cannot be obtained easily from the image. Detailed visual and statistical analysis of the algorithm reveals that it yields satisfactory results. When compared with other existing algorithms, it is easy to prove that the difficulty of decoding the proposed algorithm is high.
