Abstract:Background and Aims: Current cardiovascular disease (CVD) prevention is based on diagnosis and treatment of specific disease. Little is known for high risk people with CVD at the community level. In rural China, health records of all residents were established after the recent health reforms. This study aims to describe the characters of the rural population with high CVD risk regarding their clinical indicators, disease patterns, drug treatment and adherence.Methods and Results: 17042 (87%) of all the 19500 rural residents in the two townships had valid health records in 2009. We employed a validated tool, the Asian Equation, to screen 8182 (48%) resident health records of those aged between 40-75 years in 2010. Those who were identified with a CVD risk of 20% or higher were selected for a face-to-face questionnaire survey regarding their diagnosed disease and drug treatment. 453 individuals were identified as high risk of CVD, with an average age of 53 years, 62% males, 50% smoking rate and average systolic blood pressure of 161 mmHg. 386 (85%) participated in the survey, while 294 (76%) were diagnosed with and 88 (23%) were suspects of CVD, hypertension, diabetes or hyperlipidaemia. 75 (19%) took drug regularly and 125 (32%) either stopped treatment or missed drugs. The most often used drugs were calcium channel blockers (20%). Only 2% used aspirins and 0.8% used statins. The median costs of drugs were 17 RMB (USD2.66) per month.Conclusion: The majority of the high risk population in our setting of rural China had already been diagnosed with a CVD related disease, but very few took any drugs, and less still took highly effective drugs to prevent CVD. A holistic strategy focused on population with high risk CVD and based on the current China public health reform is suggested in the context of primary care.
Ideal Cardiovascular Health in Chinese Urban Population.

Abstract:Background: The American Heart Association's 2020 Strategic Goals define a new concept of cardiovascular health. However, the prevalence of ideal cardiovascular health outside of the United States is unclear, and its relationship with psychological status has not been reported.Method: We included 9962 participants (mean age 47.1 years; 44.2% women) from the survey of the Disease Risk Evaluation and Health Management study from October 2009 to Feb 2012. The prevalence of poor, intermediate, and ideal cardiovascular health levels were calculated, and psychological status was assessed using the 21-item Depression Anxiety Stress Scales.Results: After adjusting for sex and age, only 0.5% of the participants met ideal levels of all 7 cardiovascular health metrics, and 26.9% presented with 5 to 7 ideal health metrics. Fasting plasma glucose was the most prevalent ideal metric (71.2%), whereas physical activity was the least prevalent (18.1%). Women had a significantly higher proportion of 5 to 7 ideal health metrics compared with men (40.4% versus 13.4% after adjusting for age), and the proportion of participants who had 5 to 7 ideal health metrics significantly decreased with age (P<0.001 for trend). Furthermore, the scores for depression, anxiety, and stress showed a negative correlation with the number of ideal health metrics, with regression coefficients of -0.07, -0.07, and -0.11, respectively (P<0.05).Conclusions: Few adults met ideal levels of cardiovascular health. Individuals, communities, and health-care providers in China should be better integrated to pay closer attention to primordial prevention of unhealthy lifestyles and psychological problems. Evaluation of a systematic cardiovascular disease risk reduction strategy in primary healthcare: an exploratory study from Zhejiang, China.

Abstract:In China, cardiovascular disease (CVD) risk reduction strategies are not systematically implemented in primary healthcare (PHC). We conducted an exploratory study to evaluate the preliminary effectiveness of our systematic CVD risk reduction package in one township hospital of Zhejiang. Using the Asian Equation, we selected subjects aged 40-74 years with a calculated 10-year CVD risk of 20% or higher from the existing resident health records and research checkup. The subjects were provided, as appropriate, with the low-dose combination of CVD-preventive drugs (antihypertensive drugs, aspirin, statin), lifestyle modification and adherence strategies monthly. The intervention was piloted for three months in 2012, preceding the conduct of a cluster-based randomized controlled trial (RCT). A total of 153 (40%) subjects were recruited, with an average total 10-year risk of CVD of 28.5 +/- 7.9%. After intervention, the appointment rate was up to 90%. An upward trend was observed for the use of CVD-preventive drugs. The smoking rates significantly reduced from 38 to 35%, with almost no change for salt reduction. The systolic blood pressure (BP) and diastolic BP decreased slightly. A holistic CVD risk reduction approach shows preliminary effects in a rural PHC setting of Zhejiang, China. However, further understanding is needed regarding its long-term effectiveness and feasibility in PHC practices. Our cluster-based RCT will provide the highest level of evidence for the policy development of preventing CVD in a rural PHC of China.
Associations among Risk Perception, Health Efficacy, and Health Behaviors for Cardiovascular Disease: an Application of Risk Perception Attitude Framework.

Abstract:Background:There is currently a pervasive prevalence of cardiovascular disease (CVD) risk worldwide and an inadequate amount of action is being taken to promote healthy lifestyle habits. The risk perception attitude (RPA) framework, which classifies individuals based on their risk perception and efficacy belief, enables us to predict their preventive behaviors. We applied the RPA framework to analyze CVD prevention behaviors among Chinese adults and extended its application to CVD objective risk. Methods:A cross-sectional survey was performed in two sites in Zhejiang Province, from March to August 2022, which contained self-reported CVD risk perception, objective CVD risk, efficacy belief, physical activity, healthy diet, and covariates. We used the RPA framework to categorize participants into four groups, then analysis was conducted to estimate inter-group differences in healthy behaviors. We further conducted a hierarchical logistic regression analysis with individuals' health behaviors as the dependent variable, using three blocks of independent variables. Results:Among 739 participants, healthy physical activity and healthy diet had significant differences among four RPA groups, post hoc tests clarified that the proportion of respondents with healthy PA in the responsive group (61.6%) was significantly higher than that in the other three groups. Risk perception and efficacy belief significantly predicted health behavior against CVD; the relationship between absolute CVD risk and health behavior was moderated by efficacy belief. Conclusions:Early CVD risk screening is crucial, but tailored support and a proper understanding of personal risk are essential to promote healthy behaviors. Developing communication and behavioral counseling intervention strategies on the basis of the RPA framework has the potential to promote healthy behaviors for CVD prevention.
Socioecological influencers of health-promoting lifestyles in Chinese: a preliminary survey using convenient samples

Abstract:Background: Healthy lifestyles are considered important means to reduce the burden of diseases. This cross-sectional study was conducted based on the Ecological Model of Health Behavior (EMHB) to analyze the factors associated with the health-promoting lifestyles of Chinese residents. Methods: We carried out a cross-sectional investigation in July 2023. Our investigated factors included social-demographic characteristics (including sex, age, education level, employment status, marital status, personal monthly income, and daily behavioral habits [which were measured by a questionnaire)], health literacy [which was measured by the Chinese version of the Health Literacy Scale Short-Form scale (HLS-SF12)], and family health [which was measured by the Chinese version of the Short-Form of the Family Health Scale (FHS-SF)]. Our outcome was health promoting lifestyle, which was measured by a revised version of Health Promoting Lifestyle Profile-II (HPLP-IIR). Data were analyzed using stepwise regression. Results: A total of 1,402 participants were enrolled. Higher scores of HLS-SF12 ( β = 0.467), having regular exercise ( β = 0.212), and regular physical examination ( β = 0.088) were associated with better health-prompting lifestyles. However, older age (≥60 years) ( β = −0.046), drinking ( β = −0.066), and sleeping time (5–6 h/day) ( β = −0.048) were associated lower levels of health-prompting lifestyles. Living with family ( β = 0.077), FHS-SF ( β = 0.104), and married ( β = −0.077) were significant influencers. Unemployed ( β = −0.048), receiving retirement pay ( β = −0.053), and economic support provided by parents ( β = 0.094) were associated with better health-prompting lifestyles. There were multiple influencing factors of the six dimensions of the HPLP-IIR. Our findings indicate that community residents with higher health literacy, better family health, and health-related behaviors tend to have better health-promoting lifestyles. Conclusion: Our findings have confirmed the complex impacts of social-ecological factors on health-promoting lifestyles, which may help policy makers with health-promotion strategies making and also help researchers to control for confounding in study design.

Abstract:Objective:To investigate and analyze the status quo of health promoting lifestyles of 670 people with high normal blood pressure in Shenzhen city and provide scientific basis for early intervention.Methods:A total of 670 people with high normal blood pressure were investigated by Health promoting lifestyles scale.Re-sults:The total score of lifestyles of 670 people with high normal blood pressure was among 78~1 67,4.78%of them got excellent scores,57.76% of them got good scores,37.1 6% of them got ok scores,0.30% of them got poor scores.All dimensions ranked according to the standardized scores:self realization ranked the first, followed by interpersonal relationships,nutrition,stress management and health responsibility,and the exer-cise ranked the last.Conclusion:The lifestyle of people with high normal blood pressure needs to be improved. Nurses should guide the patients to eliminate the bad personal habits,keep a reasonable diet,create good mood and keep exercising,and then to prevent the progress of high normal blood pressure.
Influence of health promoting lifestyle on health management intentions and behaviors among Chinese residents under the integrated healthcare system

Abstract:Background Health promoting lifestyle is an important influencing factor of individual health status. This study aims to assess the health promoting lifestyle of community residents in China, and explore its association with their health management intention and behaviors during the integrated healthcare system reform. Methods A total of 666 residents were recruited from six county level hospitals and 12 community health centers from July to August 2019 in Zhejiang Province, China. Health promoting lifestyle was measured by the Chinese version Health Promoting Lifestyle Profile-II scale (HPLP-II). Results The average total score of HPLP-II among our sample was 130.02±23.19. Among the six domains, interpersonal relationship had the highest average score (2.68±0.50), and physical activity scored the lowest (2.21±0.59). Total score of HPLP-II scale was negatively associated with being male (β = -0.13, p<0.01; Ref: female), positively associated with being students (β = 0.15, p<0.01; Ref: self-employed), and positively associated with a monthly per capita income of more than 8000 RMB (β = 0.15, p<0.01; Ref: less than 3000 RMB). The domain scores of HPLP-II were significantly correlated with residents’ health management intention and their behavior on following doctors’ advice or not. Conclusions The health promoting lifestyles of community residents in China are at moderate levels. Improving residents’ healthy lifestyle levels might be helpful for changing their health management intentions or behaviors.

Abstract:Objectives: This study aims to explore the status of Shandong Province, China residents' health promotion lifestyle and its influencing factors, especially to explore how health attitude affects health promotion lifestyle, thus can make targeted recommendations for health promotion in China and similar areas. Methods: 1800 adults were selected from urban and rural areas of Shandong Province, China, using multistage stratified, cluster random sampling method. A survey was conducted face-to-face from March to May, 2018, using Health Promotion Lifestyle Profile and Health Attitude Questionnaire. The between-group measured data were compared by One-way ANOVA or t-tests. The correlation between the health attitude and health promotion lifestyle was examined by Pearson correlation. Logistic regression model was used to examine the related factors influencing health promotion lifestyle. Health promotion lifestyle is the dependent variable, and gender, education level, annual family per capita income and health attitude are the independent variables. Results: The mean (SD) of HPLP-IICR total score of the participants was 82.12(16.63). 54.50% of the participants had poor or average health promotion lifestyle, while 45.50% had good or excellent health promotion lifestyle. Significant differences existed in health promotion lifestyle among different gender, education level, income level, marital status, and health attitude (Ps < 0.001). Multivariable Logistic regression model found that male (OR = 0.35, 95% CI: 0.12-0.34), high school education level (OR = 0.57, 95% CI:0.17-0.41), junior middle school & below (OR = 0.42; 95% CI:0.12-0.33), annual family per capita income with < 10,000 CNY (OR = 2.53, 95% CI:1.24-2.06; OR = 2.14, 95% CI:1.08-3.12), low health affection (OR = 0.39, 95% CI:2.15-4.22), and low health behavioral intention (OR = 0.21; 95% CI: 2.33-5.29) were statistically significant correlates of average or poor health promotion lifestyle. Conclusions: The health lifestyle needs to be further promoted in Shandong Province, China. The government and social sectors are encouraged to make more efforts to improve the accessibility and quality of health services. Meanwhile, individual responsibility cannot be ignored as well. More affective factors and operable measures should be added to enhance health affection and health behavioral intention, so as to enhance health promotion lifestyle.
Individuals at High Risk of Cardiovascular Disease and Risk Factors in Rural China:A Survey in Two Counties in Hebei Province

Abstract:Objective: To investigate the prevalence of individuals at high risk of cardiovascular disease and the risk factors for cardiovascular disease in rural China. Methods: 2033 subjects above the age of 20 were surveyed.These subjects came from Luquan and Anguo counties in Henan( 12 villages per county). The method of used was stratified cluster random sampling. All subjects received standardized physical examinations and were asked to complete a questionnaire. Multivariable logistic regression was used to analyze the associated risk factors. Results: The weighted detection rate of individuals at high risk of cardiovascular disease was 20. 0%,with significantly higher rates for older ages( P 0. 001). In addition to sex and age,obesity,low levels of education,and lack of physical activity all significantly increased cardiovascular risk. The risk of cardiovascular disease in an overweight person was 2. 3times that of someone of normal weight. The risk of cardiovascular disease in a person with no education was 3. 1times that of someone with a high school education or above. The risk of cardiovascular disease in someone who didnon-laboring work was 2. 1 times that of a laborer. Conclusions: There are many individuals at high risk of cardiovascular disease among adults in rural China,especially for people with low education. Policies for preventing chronic disease are urgently needed to protect disadvantaged populations and improve their health.

Abstract:Objective To investigate the current situation of health-promoting lifestyles among people receiving health examination in Shenzhen,in order to provide a theoretical basis for nursing intervention.Methods Totally 720 people receiving health examination in outpatient department were investigated by using Health-Promoting Lifestyle Profile(HPLP)and general information questionnaire.Results The total score of lifestyle was 136.45±20.51,and the average score of the items was 2.62±0.40.In the six subscales,the excellent rate of interpersonal relationships,self-actualization,stress management,nutrition,health responsibility,and physical movement accounted for 99.31%,96.25%,93.75%,92.36%,64.86%,and 52.78% respectively.The factor scores were ranked as interpersonal relationships 26.54±4.00,self-realization 25.99±4.37,nutrition 24.42±4.19,stress management 21.11±3.53,health responsibility 20.68±4.54,and physical movement 17.73±4.96.Conclusion The people receiving health examina-tion in Shenzhen have poor knowledge in physical movement and health responsibility.Nurses should focus on the related factors influencing physical movement and health responsibility,guide the people to adopt healthy behavior,promote the people to cultivate good exercise habits,and raise self-consciousness of health-promoting,in order to improve people living standard.
Epidemic of chronic diseases and the related healthy lifestyle interventions in rural areas of Shandong Province, China

Abstract:Background: There were amounts of previous studies on chronic diseases, but few studies on the prevalence of chronic disease and the healthy lifestyle intervention in recent years, China. This study aimed to investigate the prevalence of chronic disease and the implementation of healthy lifestyle intervention in rural areas of China, so as to put forward health promotion measures to control the chronic diseases effectively. Method: A large cross-sectional study (N = 2168) on community diagnosis and chronic disease was carried out in Shandong province, China. The chronic disease questionnaire and the healthy lifestyle intervention questionnaires were recruited to survey the chronic diseases and the implementation of healthy lifestyle intervention. Physical examination and biochemical indicators examination were carried out by the medical staffs and clinical laboratory. Results: The current diagnosed prevalence of hypertension, diabetes, hyperlipidemia for total sample, female, male were 24.97, 24.6, 25.5, 7.60, 8.9, 6.0 and 40.27%, 45.9, 33.3% respectively in rural China. The one-year prevalence of myocardial infarction (MI) and stroke of the total sample, female, male were 1.06, 1.0, 1.1 and 2.09%, 2.2, 2.0% respectively. Healthy lifestyles interventions were not effective in rural China. The current active smoking rate and passive smoking rate were 25.68 and 42.65%. 27.86% of the population drunk alcohol within a month and 47.01% of them participated in the actions to control salt daily intake. Only 1.07 and 7.89% of the population participated in medium to high intensity physical exercises. Conclusions: The prevalence of common chronic diseases were still high and the implementation of healthy lifestyle intervention were not optimal in rural area, China. Challenges to prevent chronic diseases were still severe, so medical institutes, government and individuals would put forward effective strategies to reduce the prevalence and public health promotion project should be effectively strengthened.
The association between a combination of healthy lifestyles and the risks of hypertension and dyslipidemia among adults-evidence from the northeast of China

Abstract:Background and aims: There is increasing evidence that lifestyle factors play an important role in the development of hypertension and dyslipidemia. However, existing research usually evaluated these risk factors individually (such as physical activity, smoking, drinking, obesity and so on), rather than joint evaluation. The aim of this study was to quantify the association between a combination of a healthy lifestyle and the risk of hypertension and dyslipidemia. Methods and result: A healthy lifestyle score was created based on 4 factors: never smoking, moderate to high-intensive physical activity, no alcohol drinking, and normal body mass index. We calculated the healthy lifestyle score using the cumulative number of health factors for each individual. Also, a multivariate analysis was used to assess the relationship between healthy lifestyle and hypertension and dyslipidemia. Among 6446 participants, 650 (10.08%) had lowest healthy lifestyle score (0) and 627 (9.72%) had highest healthy lifestyle score (4), respectively. The adjustment model indicated that participants with the highest score (score: 4), which integrated the four lifestyles, had significantly lower ORs for hypertension compared with the lowest score (score: 0) (0.21; (95%CI: 0.10, 0.43 P-trend< 0.001)). In the adjustment models, compared with lowest healthy lifestyle score, the ORs of highest healthy lifestyle score was: 0.17; (95%CI: 0.07, 0.42 P-trend<0.001) for dyslipidemia. Conclusion: Hypertension and dyslipidemia were negatively correlated with healthy lifestyle score. Interventions with healthy lifestyle to reduce hypertension, dyslipidemia and promote population health are warranted. Correlations between health-promoting lifestyle and health-related quality of life among elderly people with hypertension in Hengyang, Hunan, China

Abstract:Hypertension is considered as an important public health problem in developed and developing countries. The disease is closely associated with health-promoting lifestyle (HPL), and it seems that HPL plays an important role in improving health-related quality of life (HRQOL). This cross-sectional study is to investigate the effects of health-promoting lifestyle (HPL) on health-related quality of life in elderly people with hypertension from a community health service center in Hengyang, Hunan, PR China.Totally 530 elderly patients with hypertension from the community health service center were included in this study, who were asked to fill in a questionnaire (504 patients responded). HPL was assessed by the health-promoting lifestyle profile II (HPLP-II), and HRQOL was measured by the Short Form Health Survey Questionnaire (SF-36).HPL among these elderly people with hypertension was at moderate level (125.02 ± 21), with the highest score for nutrition and the lowest score for health responsibility. Moreover, HRQOL among these elderly hypertensive people was at moderate level (54.36 ± 21.18). Role-emotional domain score was far below average, vitality domain was a little below average, social functioning and general health domains were a little above average, and other domains were far above average. Furthermore, HPL and HRQOL were positively correlated (P < .01). According to the standardized regression coefficients, the influencing factors for HRQOL included (in a descending order) the health responsibility, physical activity, interpersonal relationships, stress management, spiritual growth, and nutrition.HPL and HRQOL were both relatively poor in the elderly people with hypertension from the community health service center. HPL represents an important factor affecting HRQOL of elderly people with hypertension. HRQOL could be improved through promoting HPL (such as health responsibility and physical activity).

Abstract:Background Coronary heart disease (CHD) is the most common type of heart disease and cause of heart attacks. This study investigated the epidemiological characteristics of CHD and its risk factors in Jiaozhou, Shandong province, to ultimately find a way of reducing the prevalence of cardiovascular disease, and to provide a theoretical basis for establishing a cardiovascular disease management path under the regional medical collaborative mechanism. Methods A questionnaire survey was performed including 1 952 people aged 35 years or older who were questioned by means of stratified, cluster, proportional sampling to investigate the prevalence of CHD and its risk factors. The data were inputted into SPSS11.0 statistical software for processing and analysis. We advised the local medical institutions to establish health files for the residents with CHD and risk factors. They were followed up regularly. Their risk factors and life-style were monitored, and advice was given as to proper medications. Green channels were established, and the patients were transmitted in a timely manner to superior hospitals for better treatment if the necessary treatments were not available in the local hospitals. The control of risk factors was observed after the follow-up for half a year. Results In Jiaozhou, the rates of coronary artery disease, hypertension, diabetes, hyperlipidemia and overweight were 8.15%, 28.54%, 11.43%, 35.46%, and 18.70% respectively. The rates of hypertension, diabetes, hyperlipidemia and overweight were higher than the data published in “The report of Chinese cardiovascular disease 2012”; which are 24%, 9.7%, 18.6%, and 9.7%, respectively. The control of risk factors improved significantly after the guidance of the residents lifestyle and medication for six months. Conclusions The high prevalence of coronary artery disease in Jiaozhou is closely related to age, gender, diet structure, family history of cardiovascular disease, hypertension, diabetes, hyperlipidemia, overweight, and unhealthy lifestyle. Under the regional medical coordination mechanism, the collaborative management of cardiovascular disease can provide new management concepts for the areas short of medical resources, so as to reduce the prevalence of cardiovascular disease.

Evaluating a Simplified Method for Identifying High-Risk Individuals for Cardiovascular Diseases in the Resource-Constrained Rural Areas of China

Abstract:OBJECTIVE:This research project aimed to evaluate whether a simplified method for identifying high-risk individuals for cardiovascular diseases proposed by the China Rural Health Initiative (CRHI) was feasible in the rural areas in China.METHODS:2 036 adults, aged 20 years or older were stratified-randomly sampled from 12 villages in Luquan county and Anguo county (Hebei province) respectively, to receive physical examination and filling in administered questionnaires. "Gold Standard" was used on high risk in Chinese Guidelines on Prevention and Treatment of Dyslipidemia in Adults to evaluate the CRHI standards:people with a history of coronary heart disease or stroke, or elderly as men ≥50 years or women ≥60 years with diabetes, or elderly with systolic blood pressure ≥160 mmHg. Sensitivity, specificity, positive and negative predictive values related to the identification of high-risk individuals for cardiovascular diseases were assessed.RESULTS:The concordance rate between the CRHI standard and the gold standard was 92.9% , with sensitivity as 77.2%, specificity as 98.5%, Youden's Index as 0.76, positive predictive value as 94.7% and negative predictive value as 92.5%. Under CRHI standard, 21.3% of the adults were identified as high risk. The rate was increasing with age (P < 0.001), reaching 44% among those who were over 60 years old.CONCLUSION:The CRHI standard seemed simple and easy and was suitable for identifying high-risk individuals for cardiovascular diseases in the resource-constrained rural areas.
Association of Healthy Lifestyle with Incident Cardiovascular Diseases among Hypertensive and Normotensive Chinese Adults.

Abstract:BackgroundWhether lifestyle improvement benefits in reducing cardiovascular diseases (CVD) events extend to hypertensive patients and whether these benefits differ between hypertensive and normotensive individuals is unclear. This study aimed to investigate the associations of an overall healthy lifestyle with the subsequent development of CVD among participants with hypertension and normotension.MethodsUsing data from the Suzhou subcohort of the China Kadoorie Biobank study of 51,929 participants, this study defined five healthy lifestyle factors as nonsmoking or quitting for reasons other than illness; nonexcessive alcohol intake; relatively higher physical activity level; a relatively healthy diet; and having a standard waist circumference and body mass index. We estimated the associations of these lifestyle factors with CVD, ischemic heart disease (IHD) and ischemic stroke (IS).ResultsDuring a follow-up of 10.1 years, this study documented 6,151 CVD incidence events, 1,304 IHD incidence events, and 2,243 IS incidence events. Compared to those with 0–1 healthy lifestyle factors, HRs for those with 4–5 healthy factors were 0.71 (95% CI: 0.62, 0.81) for CVD, 0.56 (95% CI: 0.42, 0.75) for IHD, and 0.63 (95% CI: 0.51, 0.79) for IS among hypertensive participants. However, we did not observe this association among normotensive participants. Stratified analyses showed that the association between a healthy lifestyle and IHD risk was stronger among younger participants, and the association with IS risk was stronger among hypertensive individuals with lower household incomes.ConclusionAdherence to a healthy lifestyle pattern is associated with a lower risk of cardiovascular diseases among hypertensive patients, but this benefit is not as pronounced among normotensive patients.

Abstract:Background To plan long-term prevention strategies and develop tailored intervention activities, it is important to understand the socio-demographic characteristics of the subpopulations at high risk of developing chronic diseases. This study aimed to examine the socio-demographic characteristics associated with multiple lifestyle risk factors and their clustering. Methods We conducted a simple random sampling survey to assess lifestyle risk factors in three districts of Hangzhou, China between 2008 and 2009. A two-step cluster analysis was used to identify different health-related lifestyle clusters based on tobacco use, physical activity, fruit and vegetable consumption, and out-of-home eating. Multinomial logistic regression was used to model the association between socio-demographic factors and lifestyle clusters. Results A total of 2016 eligible people (977 men and 1039 women, ages 18-64 years) completed the survey. Three distinct clusters were identified from the cluster analysis: an unhealthy (UH) group (25.7%), moderately healthy (MH) group (31.1%), and healthy (H) group (43.1%). UH group was characterised by a high prevalence of current daily smoking, a moderate or low level of PA, low FV consumption with regard to the frequency or servings, and more occurrences of eating out. H group was characterised by no current daily smoking, a moderate level of PA, high FV consumption, and the fewest times of eating out. MH group was characterised by no current daily smoking, a low or high level of PA, and an intermediate level of FV consumption and frequency of eating out. Men were more likely than women to have unhealthy lifestyles. Adults aged 50-64 years were more likely to live healthy lifestyles. Adults aged 40-49 years were more likely to be in the UH group. Adults whose highest level of education was junior high school or below were more likely to be in the UH group. Adults with a high asset index were more likely to be in the MH group. Conclusions This study suggests that Chinese urban people who are middle-aged, men, and less educated are most likely to be part of the cluster with a high-risk profile. Those groups will contribute the most to the future burden of major chronic disease and should be targeted for early prevention programs.

Abstract:Background Cardiovascular disease is the primary cause of death and Disease burden of Chinese residents,and the situation of prevention and control is grim.Management of risk factors for cardiovascular disease is an important foundation for preventing cardiovascular disease.However,there is currently limited research on the influencing risk factors of cardiovascular disease risk in elderly patients with chronic disease in primary care using single lead wearable electrocardiogram devices both domestically and internationally,and previous studies have not ranked the importance of variables containing different quantities of risk in patients from a holistic perspective.Objective The aim of this study is to analyze the influencing factors of the comprehensive risk of cardiovascular disease in elderly patients with chronic disease aged 65 and above in primary care in Ningxia,and to provide objective basis and assistance for the comprehensive prevention and control of cardiovascular disease in primary care.Methods From December 2021 to September 2022,totally 3 039 patients over 65 years old with hypertension,diabetes and coronary heart disease,including at least one chronic disease,from 26 primary care health center in Ningxia were selected as the research subjects.According to the analysis of the 72 hour electrocardiogram,there were 632 cases in the normal group and 2 407 cases in the risk group.Analyze the basic information of two groups of patients to determine the best λ Value,draw a model,and use LASSO regression and multivariate Logistic regression to analyze the comprehensive risk factors of cardiovascular disease in elderly patients with chronic diseases;and rank the variables that affect the overall risk of cardiovascular disease and include different numbers of risks among patients.Results The differences in age,gender,BMI,education level,occupation,urban-rural distribution,smoking,exercise,coronary heart disease combined with diabetes,hypertension combined with coronary heart disease and diabetes between the two groups of patients were statistically significant(P<0.05);optimum λvalue was 0.015 685 31.LASSO regression and multivariate logistic regression model analysis showed that age,BMI,urban-rural distribution,smoking Hypertension combined with coronary heart disease and diabetes is a influencing factor for cardiovascular disease(P<0.05).Auc was 0.650(95%CI=0.627-0.673,P<0.001)The top five variables that affect comprehensive risk and different types of risk are age,BMI,urban-rural distribution,tea drinking,and exercise;Age,hypertension combined with coronary heart disease,gender,urban-rural distribution,smoking;Age,hypertension combined with coronary heart disease,gender,BMI,urban-rural distribution;Diabetes combined with coronary heart disease,age,hypertension combined with coronary heart disease,hypertension combined with coronary heart disease and diabetes,hypertension combined with diabetes.Conclusion Age,BMI,urban and rural distribution,smoking,hypertension with coronary heart disease and diabetes are the influencing factors of cardiovascular disease risk in patients over 65 years old with chronic diseases.In addition to age,BMI and lifestyle habits have a significant impact on the overall risk of cardiovascular disease.As the number of comorbidities increases,the impact of chronic diseases,especially chronic disease comorbidities,increases.Primary care medical teams should regularly conduct comprehensive CVD risk management for elderly patients with multiple chronic diseases using single lead wearable devices.This not only enables efficient and low-cost implementation of primary and secondary prevention and health management of CVD risks,but also accelerates the transformation of primary medical services from inconsistent diagnosis and treatment services to full process health management.
Correlation Analysis of Lifestyle and Blood Lipid Level among Health Examination Population in Shenzhen

Abstract:Objective To explore the blood lipid level,lifestyle and its correlation among health examination population in Shenzhen,so as to provide a scientific basis for prevention and treatment of dyslipidemia.Methods Coin-tossing method was used to recruited 720 cases from health examination population,and were investigated with the general questionnaire and health-promoting lifestyle profile ( HPLP),detected the four items of fasting blood lipid and analyzed the correlation between the lifestyle and blood lipid level.Results The total average score of lifestyle was ( 136.45 ± 20.51 ),with the interpersonal relations average score (26.53 ±4.00) was highest,and physical exercise average score ( 17.70 ±4.96) was lowest,and the percent of unhealthy lifestyle was 36.11％ in 720 health examination person.The mean values of total cholesterol (TC),triglycerides (TG),low density lipoprotein (LDL-C) and high density lipoprotein (HDL-C)respectively were ( 5.75 ± 1.09 ),( 2.47 ± 1.09 ),(4.00 ± 0.84 ),( 1.27 ± 0.77 ) mmol/L.The lifestyle was overall negatively correlated with TC,TG,LDL-C ( r =-0.814,-0.690,-0.798,respectively;P ＜ 0.01 ),and positively correlated with HDL-C ( r =0.799,P ＜ 0.01 ).Conclusions Nursing personnel should implement specific health education and take intervention measures so as to change their unhealthy lifestyle,improve the blood lipid level,prevent or reduce the clinical cardiovascular events happening and improve their living quality.
