Abstract:The aim of the present study was to identify a novel strategy that predicts the metastatic status of lymph nodes (LNs) in patients diagnosed with colorectal cancer, using detailed characteristics of contrast-enhanced CT scan images. A total of 284 preoperative CT scans derived from patients diagnosed with colorectal cancer at Second Affiliated Hospital, Zhejiang University School of Medicine between January 2013 and July 2018 were retrospectively reviewed. A total of 794 LNs were assessed for size, margins, morphology and subtle internal enhancements in the equilibrium phase. Imaging features were analyzed by two abdominal radiologists (Department of Radiology, Second Affiliated Hospital, Zhejiang University School of Medicine and Departments of Radiology; Shaoxing Second Hospital Departments of Radiology, Shaoxing Second Hospital) in a blind manner. If the conclusions were not concordant, the final score was determined by a senior radiologist who specialized in abdominal radiology for ≥30 years. According to the histopathology results, 27.3% (217/794) of LNs were metastatic (LN+). In addition, LNs >10 mm in size demonstrated sensitivity, specificity, positive predictive values (PPVs) and negative predictive values (NPVs) of 47.0, 80.9, 48.1 and 80.2%, respectively [odds ratio (OR), 3.77; 95% confidence interval (CI), 2.69-5.28]. LNs in the shape of a kidney bean (middle fat depression like kidney) and/or those with an oblong shape were more likely to be metastasis negative LNs (LN-), while lobulated and irregular LNs were more likely to be LN+. In magnified images, internal enhancement characteristics of LN- were defined as homogeneous, spotted, striped and core enhancing. By contrast, rim and heterogeneity enhancement features for LN+ demonstrated sensitivity, specificity, PPVs and NPVs of 46.5, 89.9, 63.5 and 81.7%, respectively (OR, 7.79; 95% CI, 5.33-11.40). The results demonstrated that the internal enhancement features of LNs may be used as a predictor of metastasis. The detailed benign characteristics, such as homogeneity, spotted, striped and core enhancement of LNs may facilitate the identification of LN- in patients with colorectal cancer.
Differential diagnosis of adrenal metastasis and lipid-poor adrenal adenoma with CT

Abstract:Objective To study the CT characteristics of adrenal metastasis (AM) and lipid-poor adrenal adenoma (Lp-AA).Methods We retrospectively analyzed clinical and CT dates of AM (n=25) and Lp-AA (n=61) which were pathologically or clinically confirmed.The size,location,number,contour,cystic degeneration,calcification,hemorrhage and the clinical data were analyzed,as well as,CT value of tumor were measured and calculated.Receiver operating characteristic (ROC) curves were employed to assess the significant continuous variables to tell AM from Lp-AA.Results 34 adrenal lesions were detected for 25 cases of AM (9 case showed bilateral with 18 lesions).There were 65 lesions for 61 cases of Lp-AA (3 cases showed bilateral and 1case showed two lesions in unilateral side).Sex and age had significant difference between the two groups (P＜0.05).On the categorical variables of CT examination,number,cystic degeneration and contour had significant difference between the two groups (P＜0.05),while tumor location,hemorrhage and calcification had no significant difference (P＞0.05).For the continuous variables,plain scan CT value,enhancement amplitude,relative percentage washout (RPW) and tumor size had significant difference between the two groups (P＜0.05).Area under ROC curve analysis showed that when the plain scan CT value＞30.4 HU,area under ROC curve,sensitivity and specificity were 0.878,91.2％,and 72.3％,respectively.When RPW was ＞-33.4％,the area under ROC curve,sensitivity and specificity were 0.643,79.4％,64.7％,respectively.When the maximum and minimum diameter of tumor were ＞2.9 cm and 2.2 cm respectively,the area under the curve,sensitivity,specificity were 0.762,73.5％,75.4％ and 0.706,64.7％,69.2％,respectively.When venous phase increase and the largest increase were ＜37.7 HU and 37.3 HU respectively,the area under the curve,sensitivity,specificity were 0.805,89.2％,64.7％ and 0.833,95.4％,64.7％ respectively.Conclusion Tumor size,number,contour,cystic degeneration,CT measured and calculated value,as well as the patients' clinical data are of great value in differentiating AM and Lp-AA.

Abstract:Purpose: Contrast-enhanced ultrasound (CEUS) is a new technique used to distinguish benign from metastatic superficial lymph nodes (LNs) with variable accuracy. The objective of this meta-analysis was to evaluate the accuracy of CEUS for the diagnosis of superficial metastatic LNs. Methods: A comprehensive literature search of PubMed, Web of Science, Medline, Embase, and the Cochrane Library was performed until April 2018. Nine articles reporting the diagnosis of 436 LNs by CEUS were investigated. The Mantel-Haenszel and DerSimonian-Laird methods were used to analyze pooled sensitivity, specificity, diagnostic OR, positive likelihood ratio (LR), and negative LR, and a summary receiver operating characteristic (SROC) curve was also evaluated. Results: The pooled sensitivity and specificity of CEUS for the differential diagnosis of benign and metastatic superficial LNs were 0.88 (95% CI, 0.83-0.92) and 0.80 (95% CI, 0.74-0.85), respectively. The pooled positive LR, negative LR, and diagnostic OR were 4.36 (95% CI, 2.38-7.99), 0.17 (95% CI, 0.10-0.31), and 32.75 (95% CI, 11.08-96.84), respectively. SROC area under the curve (AUC) was 0.9288. The subgroup analysis excluding outliers implied that the heterogeneity was almost eliminated, and the pooled sensitivity and specificity were 0.87 (95% CI, 0.81-0.92) and 0.79 (95% CI, 0.71-0.85), respectively. The SROC AUC was 0.8950. Conclusion: CEUS, a novel imaging modality for the characterization of superficial LNs, shows a high sensitivity and specificity in the diagnosis of superficial metastatic LNs. The different diagnostic standards, contrast agents, and contrast modes contribute to a considerable level of heterogeneity among studies. A uniform standard for distinguishing between benign and metastatic LNs is needed for further clinical application.
Value of dual-phase contrast-enhanced MSCT in qualitative diagnosis of adrenal nodules in patients with malignant tumor

Abstract:Objective To investigate the qualitative diagnostic value of dual-phase contrast-enhanced multislice spiral CT(MSCT) for adrenal nodules in patients with malignant tumor.Methods Thirty-seven cases of malignant tumor with adrenal nodules,confirmed by biopsy or surgery were collected.MSCT included plain CT scanning,arterial phase and portal phase scanning.CT scan results were reviewed and comparatively analyzed.Results Fifty lessions were found in all cases.Among them,8 cases(11 lessions) were benign;2 cases were lymphoma;27 cases(37 lessions) were metastatic tumors,among which 27 lesions presented with cystic necrosis.Ten cases were followed up for 1 to 3 months and found to have enlarged lesions;Two cases had adrenal invasion from lymphoma,with CT scan showing uniform density and mild-tomoderate enhancement.Two cases had adrenal nodular hyperplasia,1 case had unilateral adrenal gland enlargement,and 4cases had adrenocortical adenoma.All with CT scan showing uniform density and significant enhancement in dual phases.The tumor size of the 7 cases showed no change in the 1-3 month follow up.One case had pheochromocytoma and its CT scan showed low density and significant enhancement in solid lesions with no enhancement in cystic lesions.Conclusion MSCT can provide important information for qualitative diagnosis of adrenal nodules in patients with malignant tumor.
Utility of Quantitative Contrast-Enhanced Ultrasound for the Prediction of Lymph Node Metastasis in Patients with Papillary Thyroid Carcinoma

Abstract:OBJECTIVES: The aim of this study was to find the optimal parameters and cutoffs to differentiate metastatic lymph nodes (LNs) from benign LNs in the patients with papillary thyroid carcinoma (PTC) on the quantitative contrast-enhanced ultrasound (CEUS) features. METHODS: A total of 134 LNs in 105 patients with PTCs were retrospectively enrolled. All LNs were evaluated by conventional ultrasound (US) and CEUS before biopsy or surgery. The diagnostic efficacy of CEUS parameters was analyzed. RESULTS: Univariate analysis indicated that metastatic LNs more often manifested centripetal or asynchronous perfusion, hyper-enhancement, heterogeneous enhancement, ring-enhancing margins, higher PI, larger AUC, longer TTP and DT/2 than benign LNs at pre-operative CEUS (p < 0.001, for all). Multivariate analysis showed that centripetal or asynchronous perfusion (OR = 3.163; 95% CI, 1.721-5.812), hyper-enhancement(OR = 0.371; 95% CI, 0.150-0.917), DT/2 (OR = 7.408; 95% confidence interval CI, 1.496-36.673), and AUC (OR = 8.340; 95% CI, 2.677-25.984) were predictive for the presence of metastatic LNs. The sensitivity and accuracy of the quantitative CEUS were higher than qualitative CEUS (75% vs 55 % and 83.6% vs 76.1 %, respectively). CONCLUSIONS: Quantitative CEUS parameters can provide more information to distinguish metastatic from benign LNs in PTC patients; In particular, DT/2 and AUC have a higher sensitivity and accuracy in predicting the presence of metastatic LNs and reduce unnecessary sampling of benign LNs.
Clinical‐imaging‐radiomic nomogram based on unenhanced CT effectively predicts adrenal metastases in patients with lung cancer with small hyperattenuating adrenal incidentalomas

Abstract:The aim of the present study was to develop and evaluate a clinical‐imaging‐radiomic nomogram based on pre‐enhanced computed tomography (CT) for pre‐operative differentiation lipid‐poor adenomas (LPAs) from metastases in patients with lung cancer with small hyperattenuating adrenal incidentalomas (AIs). A total of 196 consecutive patients with lung cancer, who underwent initial chest or abdominal pre‐enhanced CT scan with small hyperattenuating AIs, were included. The patients were randomly divided into a training cohort with 71 cases of LPAs and 66 cases of metastases, and a testing cohort with 31 cases of LPAs and 28 cases of metastases. Plain CT radiological and clinical features were evaluated, including sex, age, size, pre‐enhanced CT value (CT pre ), shape, homogeneity and border. A total of 1,316 radiomic features were extracted from the plain CT images of the AIs, and the significant features selected by the least absolute shrinkage and selection operator were used to establish a Radscore. Subsequently, a clinical‐imaging‐radiomic model was developed by multivariable logistic regression incorporating the Radscore with significant clinical and imaging features. This model was then presented as a nomogram. The performance of the nomogram was assessed by calibration curves and decision curve analysis (DCA). A total of 4 significant radiomic features were incorporated in the Radscore, which yielded notable area under the receiver operating characteristic curves (AUCs) of 0.920 in the training dataset and 0.888 in the testing dataset. The clinical‐imaging‐radiomic nomogram incorporating the Radscore, CT pre , sex and age revealed favourable differential diagnostic performance (AUC: Training, 0.968; testing, 0.915) and favourable calibration curves. The nomogram was revealed to be more useful than the Radscore and the clinical‐imaging model in clinical practice by DCA. The clinical‐imaging‐radiomics nomogram based on initial plain CT images by integrating the Radscore and clinical‐imaging factors provided a potential tool to effectively differentiate LPAs from metastases in patients with lung cancer with small hyperattenuating AIs.

Adrenal metastases: early biphasic contrast-enhanced CT findings with emphasis on differentiation from lipid-poor adrenal adenomas

Abstract:Aim: To evaluate the accuracy of unenhanced attenuation and early biphasic contrast-enhanced computed tomography (CT) in differentiating adrenal metastases (AMs) from lipid-poor adrenal adenomas (AAs). Materials and methods: This retrospective study included 37 patients with 50 AMs and 86 patients with 89 lipid-poor AAs. Quantitative data including the longest diameter (LD), the shortest diameter (SD), LD/SD ratio, CT attenuation values (CTu, CTa, CTv), degree of enhancement (DEAP, DEPP, DEpeak, APW, RPW), and peak enhanced/unenhanced (PE/U) CT attenuation ratio were obtained. Qualitative data including enhancement pattern, location, shape, the presence of calcification or haemorrhage, and intra-lesion necrosis were analysed. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were also calculated. Results: The PE/U ratio (≤1.25), CTu (≥32.2 HU), DEpeak (≤43.15 HU), DEPP (≤37.65 HU), presence of intralesional necrosis, location (bilateral adrenal glands), and irregular shape were significant variables for differentiating AMs from lipid-poor AAs (p<0.05). Among them, PE/U ratio (≤1.25) was of greater value in differentiating the two adrenal diseases, with sensitivity, specificity, area under the receiver operating curve (ROC) curve (AUC) of 92%, 84%, 0.933, respectively. When at least any three of above criteria were combined, the sensitivity, specificity, PPV, and NPV for diagnosing AMs were 88%, 93%, 88%, and 88%, respectively. Conclusions: These seven CT criteria are conducive to differentiate AMs from lipid-poor AAs. Early biphasic contrast-enhanced CT is a high-efficient and practical imaging tool in differentiating them.
[Diagnostic Value of Dual Energy CT for Lymph Node Metastasis in Patients with Non-Small Cell Lung Cancer].

Abstract:OBJECTIVE:To assess the diagnostic value of dual energy CT for lymph node metastasis in patients with non-small cell lung cancer (NSCLC).METHODS:Forty NSCLC patients, including 15 cases of squamous cell carcinoma and 25 cases of adenocarcinoma, underwent dual energy CT examination in pre-contrast and venous phase contrast scans, then the CT attenuation value of the lung cancer lesions and 85 mediastinal enlarged lymph nodes (the short diameter ≥ 5 mm, 53 metastatic and 32 non-metastatic) were measured at different energy levels (40-190 keV, spacing 10 keV) in venous phase contrast. CT spectral curves of the lung cancer lesions, hilus pulmonis and mediastinal enlarged lymph nodes were produced automatically, through comparing their CT spectral curves slope to judge whether or not the lymph nodes were metastatic. Receiver operating characteristic (ROC) curve was used to evaluate the efficiency of CT spectral curve in diagnosis of lymph node metastasis.RESULTS:The CT spectral curves slopes of the lung cancer, metastatic lymph nodes and non-metastatic lymph nodes were 1.10±0.11, 1.08±0.07 and 1.54±0.17, respectively. There was no significant difference in curve slope between metastatic lymph nodes and lung cancer (t=-1.32,P>0.05); while there was significant difference between non-metastatic lymph nodes and lung cancer (t=-2.58,P<0.05). The CT spectral curve slope ratios of metastatic and non-metastatic lymph nodes to lung cancer were 0.98±0.05 and 1.40±0.12, respectively (t=-2.86,P<0.05). ROC curve showed that taking CT spectral curve slope ratio of 1.15 as cut-off value for the diagnosis of metastatic lymph nodes, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 81.1%, 87.5%, 91.5%, 73.7% and 83.5%, respectively.CONCLUSIONS:Dual energy CT is of value in improving the diagnostic accuracy of lymph node metastasis in NSCLC patients before treatment.
The Value of Nonenhanced Single-Source Dual-Energy CT for Differentiating Metastases from Adenoma in Adrenal Glands.

Abstract:Rationale and Objectives: To evaluate the value of the nonenhanced single-source dual-energy computed tomography (ssDECT) in differentiating metastases from adenomas in adrenal glands.Materials and Methods: This retrospective study was approved by our Institutional Review Board, and written informed consent was waived. One hundred twelve patients (66 men:46 women; mean age, 58 years) with 63 adrenal metastases (AMs) and 64 adrenal adenomas (AAs) underwent a plain dual-energy spectral CT imaging from August 2011 to December 2013 were included. The fat (water) density (DFa [Wa]) from the material decomposition (MD) images and CT number and effective atomic number (eff-Z) from the virtual monochromatic spectral (VMS) image sets were measured for the AMs and AAs. The spectral Hounsfield unit (HU) curve (CT number as a function of photon energy from 40 to 140 key) was generated, and its slope (K) was calculated. The difference of these parameters between AMs and AAs was statistically compared by the Wilcoxon rank sum test. Receiver operating characteristic curve (ROC) curves were used to compare the diagnostic efficacies of these measures in the identification of AAs and AMs. The distribution of spectral HU curve was analyzed using the chi-square test in terms of its slope K: ascending (K>0.1), straight (-0.1 <= K <= 0.1), and descending (K < -0.1).Results: 1) The CT number (medium, range) of metastases (50.47, 29.93 HU at 40 key and 29.00, 9.36 HU at 140 keV) was significantly higher than that of adenomas (-0.76,33.04 to 13.73, 18.96 HU) at each energy level from 40 to 140 keV (P < .05). 2) The fat concentration of metastases (-177.37, 296.38 mg/mL) was statistically lower than that of adenomas (126.73, 328.07 mg/mL; P < .05). 3) The eff-Z of metastases (7.76, 0.23) was significantly higher than that of adenomas (7.42,0.32; P < .05). 4) With CT number of VMS image at 40 keV of 21.78 HU as a threshold, the sensitivity and specificity for differentiating metastases from adenomas was 92.1% and 76.6%, respectively, and the area under the ROC curve was 0.90. 5) The spectral curve types included 3.2% (2 of 63) ascending, 20.6% (13 of 63) straight, and 76.2% (48 of 63) descending for the metastases, whereas the corresponding numbers were 60.9% (39 of 64), 21.9% (14 of 64), and 17.2% (11 of 64) for the adenomas. The difference was statistically significant (X-2 = 56.63; P < .05).Conclusions: The nonenhanced ssDECT enables a multiparametric approach to provide an excellent sensitivity for identifying AMs from AAs.

Abstract:Objective To evaluate the value of single-source dual-energy CT ( ssDECT ) in differentiating lipid-poor adenomas from metastases in adrenal glands .Methods From August 2011 to Oct 2014,63 patients with 73 adrenal nodules ( 53 metastases proven by 5-6 months follow-up, and 20 histopathologically proven adenomas , CT value >10 HU) underwent ssDECT scanning .The CT values of conventional polychromatic CT and virtual monochromatic images ( 40-140 keV) , fat-water density images and effective atomic number (eff-Z) were reconstructed on an ADW4.5 workstation and ROC curves were then constructed to evaluate the effectiveness of each parameter .The slope of spectral curve was measured and divided into 3 types:increment curve ( K>0.1) , straight curve (-0.1≤K≤0.1) and decrement curve ( K<-0.1) according to the slope ( the value of K ) of spectral curve , and the curve patterns in the two groups were compared statistically .Results There was no statistical difference between the mean CT values of metastases (35.12±5.29)HU and lipid-poor adenomas (32.48±6.94)HU by conventional polychromatic CT (P>0.05).The range of single-energy CT values of metastases [from (53.00±15.12) HU to (33.38± 5.67) HU] was significantly higher than that of lipid-poor adenomas [from (26.90±26.94) HU to (28.77± 10.66) HU] at energy levels ranging from 40 to 80 keV ( P<0.05).There was no significant difference between the single-energy CT value of metastases and lipid-poor adenomas at energy levels ranging from 90 to 140 keV (P>0.05).The median fat-water concentration of metastases was -164.61 μg/cm3, significantly lower than that of lipid-poor adenomas (114.32 μg/cm3, P<0.05).The eff-Z of metastases (7.76±0.15) was also significantly higher than that of lipid-poor adenomas (7.50±0.25, P<0.05).When the threshold of fat-water concentration was -143.89 μg/cm3 , the sensitivity, specificity and accuracy rate of metastasis diagnosis was 70.0%, 66.0%, and 76.7%, respectively.When the threshold of eff-Z was 7.63, the sensitivity , specificity and accuracy rate of metastasis diagnosis was 83.0%, 65.0%, and 80.4%, respectively .The lower the energy , the higher the diagnostic accuracy at energy levels ranging from 40 to 80 keV, and that of 40 keV was the highest .The spectral curves of metastases included 2 ( 3.8%) ascending curves, 9 (17.0%) straight curves and 42 (792.%) descending curves, while in the 20 lipid-poor adenomas, there were 9 ( 45.0%) ascending curves , 4 ( 20.0%) straight curves and 7 ( 35.0%) descending curves , showing significant differences between the two groups ( P<0.05) .Conclu sions Single-source dual-energy CT provides an effective multi-parameter approach for differentiating lipid-poor adrenal adenomas from metastases .
Differentiation of lipid-poor adenoma from pheochromocytoma on biphasic contrast-enhanced CT

Abstract:Purpose To evaluate the diagnostic performance of biphasic contrast-enhanced CT in differentiation of lipid-poor adenomas from pheochromocytomas. Methods 129 patients with 132 lipid-poor adenomas and 93 patients with 97 pheochromocytomas confirmed by pathology were included in this retrospective study. Patients underwent unenhanced abdominal CT scan followed by arterial and venous phase. Quantitative and qualitative imaging features were compared between the two groups using univariate analysis. Risk factors for pheochromocytomas were evaluated by multivariate logistic regression analysis and a diagnostic scoring model was established based on odd ratio (OR) of the risk factors. Results Pheochromocytomas were larger and showed cystic degeneration more frequently compared with lipid-poor adenomas ( p < 0.01). No significant difference was found in peak enhancement phase between the two groups ( p = 0.348). Attenuation values on unenhanced phase (CTU), arterial phase (CTA), and venous phase (CTV) of pheochromocytomas were significantly higher than that of lipid-poor adenomas while enhancement ratio on arterial and venous phase (ERA, ERV) of pheochromocytomas was significantly lower than that of lipid-poor adenomas (all p < 0.05). Multivariate analysis revealed lesion size > 29 mm (OR: 5.74; 95% CI 2.51–13.16; p < 0.001), CTA > 81 HU (OR: 2.54; 95% CI 1.04–6.17; p = 0.04), CTV > 97 HU (OR: 11.19; 95% CI 3.21–38.97; p < 0.001), ERV ≤ 1.5 (OR: 20.23; 95% CI 6.30–64.87; p < 0.001), and the presence of cystic degeneration (OR: 6.22, 95% CI 1.74–22.25; p = 0.005) were risk factors for pheochromocytomas. The diagnostic scoring model yielded an area under the curve (AUC) of 0.911. Conclusions Biphasic contrast-enhanced CT showed good diagnostic performance in differentiation of lipid-poor adenomas from pheochromocytomas.

Abstract:Background To evaluate the factors affecting the maximum standardized uptake value (SUVmax) of metastatic lymph nodes in different histological types of non-small cell lung cancer (NSCLC) on integrated positron emission tomography and computed tomography (PET-CT). Methods This was a retrospective, single-institution review of 122 patients with pathologically proven NSCLC who had PET-CT scanning at the same center. Lymph node metastases were pathologically confirmed in tissue specimens from surgical patients. Statistical evaluation of PET-CT results was performed on a per-nodal-station basis. Results The tumor SUVmax of squamous cell carcinoma (SCC) (11.0 ± 4.1) was higher than that of adenocarcinoma (AC) (7.4 ± 4.4) (P < 0.01), however, the SUVmax of the metastatic lymph nodes did not differ between the SCC (4.6 ± 3.1) and AC groups(3.6 ± 2.5) (P = 0.221). The SUVmax of metastatic lymph nodes was positively correlated with lymph node size but not with the primary tumor SUVmax, primary tumor size, tumor location and tumor differentiation. The frequency of a SUVmax of lymph nodes ≥2.5 was 44%, 80%,100% in SCC group and 39%, 59%, 90% in AC group when the short-axis diameter of metastatic lymph node was <10 mm, 10–15 mm, and > 15 mm, respectively. The low sensitivity for metastatic lymph nodes on PET-CT was increased when the SUVmax cut-off for malignancy was considered to be above the normal background compared with that when the SUVmax cut-off was above 2.5. Conclusions There was no difference in the SUVmax of metastatic lymph nodes in the SCC and AC groups. The SUVmax of metastatic lymph nodes was positively correlated with metastatic lymph node size. There was a high false negative rate if lymph nodes with a short-axis diameter less than 10 mm and a extremely low false negative rate if lymph nodes with a short-axis diameter higher than 15 mm. Although an increased sensitivity may be achieved by decreasing the SUVmax cut-off, invasive staging may still be required for negative lymph nodes due to the lower sensitivity of PET-CT in both SCC and AC.

Abstract:BACKGROUND: Lymph node (LN) metastasis is an important factor affecting the treatment of lung cancer. The purpose of this article was to investigate the benefits of dual-layer spectral detector computed tomography (SDCT) for the evaluation of metastatic LNs in lung cancer.METHODS: Data from 93 patients with lung cancer who underwent dual-phase enhanced scanning with SDCT were retrospectively analyzed. According to the pathological findings, 166 LNs were grouped as metastatic (n=80) or non-metastatic (n=86). LNs in station 4 (n=80) and station 7 (n=35) accounted for the majority of the LNs (approximately 69.23%). The short-axis diameter of the LN, arterial enhancement fraction (AEF), normalized iodine concentration (NIC), and the slope of the spectral Hounsfield unit curve (λHU) during the arterial phase (AP) and venous phase (VP) were measured. The Mann-Whitney U test was used to statistically compare these quantitative parameters. Receiver operating characteristic (ROC) curves were plotted to identify the cutoff values, and decision curve analysis (DCA) was performed to determine the net benefit of each parameter. The diagnostic performance, obtained by combining the short-axis diameter with each of the above parameters, was also studied.RESULTS: The short-axis LN diameter, AEF, NIC, and λHU during the AP and VP all showed significant differences between the metastatic and non-metastatic groups (P<0.05). Of the parameters, the AEF had the greatest diagnostic efficiency for metastatic LNs [area under the ROC curve (AUC)AEF =0.885] with a threshold of 86.40%. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and 95% confidence interval were 90.00%, 89.53%, 88.89%, 90.59%, and 0.830-0.944, respectively. When the quantitative parameters were combined with the short-axis diameter, the AUCs of the parameters, except the AEF, were significantly improved (P<0.05).CONCLUSIONS: The iodine quantitative parameters from SDCT, such as the AEF, demonstrated high diagnostic performances in the differentiation of metastatic and non-metastatic LNs.

Differentiating Hepatic Metastases Of Gastroenteropancreatic Neuroendocrine Cancer And Adenocarcinoma Using Enhanced Ct

Abstract:Recent studies have demonstrated that dynamic enhanced CT has potential for distinguishing gastroenteropancreatic neuroendocrine cancer (GEP-NEC) from gastroenteropancreatic adenocarcinoma (GEP-ADC). However, little is known about the performance of CT in differentiating hepatic metastases of those tumors. We therefore aimed to assess the capability of contrast-enhanced CT to differentiate between GEP-NEC and GEP-ADC hepatic metastases. CT images of 33 cases of GEP-NEC hepatic metastases and 33 cases of GEP-ADC hepatic metastases were retrospectively reviewed. Qualitative analysis included tumor distribution, the presence or absence of tumor-feeding arteries and intratumoral neovascularity, and dynamic enhancement patterns. Quantitative analysis included tumor size, tumor number, measurement of CT attenuation of tumors and adjacent liver parenchyma, and calculation of the enhancement ratio of the lesion versus the surrounding liver in the hepatic arterial phase (T-L/A) and portal venous phase. There was a significantly higher prevalence of tumor feeding arteries (72.7% vs. 36.4%, p=0.003) and intratumoral neovascularity (57.6% vs. 12.1%, p<0.001) and less plateau enhancement (42.4% vs. 84.8%, p=0.002) in GEP-NEC hepatic metastases than GEP-ADC hepatic metastases. The mean T-L/A in GEP-NEC hepatic metastases was significantly higher compared to GEP-ADC hepatic metastases (0.90 +/- 0.24 vs. 0.72 +/- 0.19, p=0.001). Regression analysis identified the presence of intratumoral neovascularity as the only independent predictor of GEP-NEC hepatic metastases on CT (odds ratio=7.097; 95% CI=1.852-27.196; p=0.004). Together, these results suggest that contrast-enhanced CT may have clinical application in the differential diagnosis of hepatic metastases from GEP-NEC or GEP-ADC.
Differentiation of pheochromocytoma and adrenal lipoid adenoma by radiomics: are enhanced CT scanning images necessary?

Abstract:Purpose: To establish various radiomics models based on conventional CT scan images and enhanced CT images, explore their value in the classification of pheochromocytoma (PHEO) and lipid-poor adrenal adenoma (LPA) and screen the most parsimonious and efficient model Methods: The clinical and imaging data of 332 patients (352 lesions) with PHEO or LPA confirmed by surgical pathology in the Affiliated Hospital of Qingdao University were retrospectively analyzed. The region of interest (ROI) on conventional and enhanced CT images was delineated using ITK-SNAP software. Different radiomics signatures were constructed from the radiomics features extracted from conventional and enhanced CT images, and a radiomics score (Rad score) was calculated. A clinical model was established using demographic features and CT findings, while radiomics nomograms were established using multiple logistic regression analysis.The predictive efficiency of different models was evaluated using the area under curve (AUC) and receiver operating characteristic (ROC) curve. The Delong test was used to evaluate whether there were statistical differences in predictive efficiency between different models. Results: The radiomics signature based on conventional CT images showed AUCs of 0.97 (training cohort, 95% CI: 0.95∼1.00) and 0.97 (validation cohort, 95% CI: 0.92∼1.00). The AUCs of the nomogram model based on conventional scan CT images and enhanced CT images in the training cohort and the validation cohort were 0.97 (95% CI: 0.95∼1.00) and 0.97 (95% CI: 0.94~1.00) and 0.98 (95% CI: 0.97∼1.00) and 0.97 (95% CI: 0.94∼1.00), respectively. The prediction efficiency of models based on enhanced CT images was slightly higher than that of models based on conventional CT images, but these differences were statistically insignificant(P>0.05). Conclusions: CT-based radiomics signatures and radiomics nomograms can be used to predict and identify PHEO and LPA. The model established based on conventional CT images has great identification and prediction efficiency, and it can also enable patients to avoid harm from radiation and contrast agents caused by the need for further enhancement scanning in traditional image examinations.

Abstract:Objective: To determine the capability of dynamic enhanced computed tomography (CT) to differentiate liver metastases (LMs) of well-differentiated from poorly-differentiated gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs). Methods: Patients with LMs of GEP-NENs who underwent dynamic enhanced CT examination in Peking University Cancer Hospital from January 2009 to October 2015 were included and data were retrospectively analyzed. We assessed the qualitative and quantitative CT features to identify the significant differentiating CT features of LMs of poorly-differentiated GEP-NENs from those of well-differentiated GEP-NENs using univariate analysis and a multivariate logistic regression model. Results: The study included 22 patients with LMs of well-differentiated GEP-NENs and 32 patients with LMs of poorly-differentiated GEP-NENs. Univariate analysis revealed statistically significant differences between the LMs of well-and poorly-differentiated GEP-NENs in terms of feeding arteries (36.4% vs. 75.0%, chi(2)= 8.061, P= 0.005), intratumoral neovascularity (18.2% vs. 59.4%, chi(2)= 9.047, P= 0.003), lymphadenopathy (27.3% vs. 81.2%, chi(2)= 15.733, P< 0.001), tumor-to-aortic ratio in the hepatic arterial and portal venous phase (T-A/AP: 0.297 +/- 0.080 vs. 0.251 +/- 0.059, t= 2.437, P= 0.018; T-A/PVP: 0.639 +/- 0.138 vs. 0.529 +/- 0.117, t= 3.163, P= 0.003) and tumor-to-liver ratio in the hepatic arterial phase (T-L/AP: 1.108 +/- 0.267 vs. 0.907 +/- 0.240, t= 2.882, P= 0.006). The LMs of poorlydifferentiated GEP-NENs showed more feeding arteries, more intratumoral neovascularity, more lymphadenopathy and a lower tumor-to-aortic ratio. Multivariate analysis suggested that intratumoral neovascularity [P= 0.015, OR=0.108, 95% confidence interval (95% CI), 0.018-0.646], lymphadenopathy (P= 0.001, OR= 0.055, 95% CI, 0.009-0.323) and T-A/PVP (P= 0.004, OR= 5.3E-5, 95% CI, 0.000-0.044) were independent factors for differentiating LMs of poorly-differentiated from well-differentiated GEP-NENs. Conclusions: Dynamic enhanced CT features (intratumoral neovascularity, lymphadenopathy and T-A/PVP) are useful in the pathological classification of LMs of GEP-NENs.

Abstract:Purpose The purpose of this study was to evaluate the capabilities of multiparametric magnetic resonance imaging (MRI) in differentiating between lipid-poor adrenal adenoma (LPAA) and adrenocortical carcinoma (ACC). Materials and methods Patients of two centers who underwent surgical resection of LPAA or ACC after multiparametric MRI were retrospectively included. A training cohort was used to build a diagnostic algorithm obtained through recursive partitioning based on multiparametric MRI variables, including apparent diffusion coefficient and chemical shift signal ratio ( i.e. , tumor signal intensity index). The diagnostic performances of the multiparametric MRI-based algorithm were evaluated using a validation cohort, alone first and then in association with adrenal tumor size using a cut-off of 4 cm. Performances of the diagnostic algorithm for the diagnosis of ACC vs. LPAA were calculated using pathology as the reference standard. Results Fifty-four patients (27 with LPAA and 27 with ACC; 37 women; mean age, 48.5 ± 13.3 [standard deviation (SD)] years) were used as the training cohort and 61 patients (24 with LPAA and 37 with ACC; 47 women; mean age, 49 ± 11.7 [SD] years) were used as the validation cohort. In the validation cohort, the diagnostic algorithm yielded best accuracy for the diagnosis of ACC vs. LPAA (75%; 46/61; 95% CI: 55–88) when used without lesion size. Best sensitivity was obtained with the association of the diagnostic algorithm with tumor size (96%; 23/24; 95% CI: 80–99). Best specificity was obtained with the diagnostic algorithm used alone (76%; 28/37; 95% CI: 60–87). Conclusion A multiparametric MRI-based diagnostic algorithm that includes apparent diffusion coefficient and tumor signal intensity index helps discriminate between ACC and LPAA with high degrees of specificity and accuracy. The association of the multiparametric MRI-based diagnostic algorithm with adrenal lesion size helps maximize the sensitivity of multiparametric MRI for the diagnosis of ACC.

Abstract:Objectives: The purpose of this study was to establish a risk prediction model for differential diagnosis of pheochromocytomas (PCCs) from lipid-poor adenomas (LPAs) using a grouping method based on tri-phasic CT image features. Methods: In this retrospective study, we enrolled patients that were assigned to a training set (136 PCCs and 183 LPAs) from two medical centers, along with an external independent validation set (30 PCCs and 54 LPAs) from another center. According to the attenuation values in unenhanced CT (CTu), the lesions were divided into three groups: group 1, 10 HU < CTu ≤ 25 HU; group 2, 25 HU 40 HU. Quantitative and qualitative CT imaging features were calculated and evaluated. Univariate, ROC, and binary logistic regression analyses were applied to compare these features. Results: Cystic degeneration, CTu, and the peak value of enhancement in the arterial and venous phase (DEpeak) were independent risk factors for differential diagnosis of adrenal PCCs from LPAs. In all subjects (groups 1, 2, and 3), the model formula for the differentiation of PCCs was as follows: Y = -7.709 + 3.617*(cystic degeneration) + 0.175*(CTu ≥ 35.55 HU) + 0.068*(DEpeak ≥ 51.35 HU). ROC curves were drawn with an AUC of 0.95 (95% CI: 0.927–0.973) in the training set and 0.91 (95% CI: 0.860–0.929) in the external validation set. Conclusion: A reliable and practical prediction model for differential diagnosis of adrenal PCCs and LPAs was established using a grouping method.

Integrative Predictive Models of Computed Tomography Texture Parameters and Hematological Parameters for Lymph Node Metastasis in Lung Adenocarcinomas

Abstract:Objectives The aims of the study were to integrate characteristics of computed tomography (CT), texture, and hematological parameters and to establish predictive models for lymph node (LN) metastasis in lung adenocarcinoma. Methods A total of 207 lung adenocarcinoma cases with confirmed postoperative pathology and preoperative CT scans between February 2017 and April 2019 were included in this retrospective study. All patients were divided into training and 2 validation cohorts chronologically in the ratio of 3:1:1. The χ2 test or Fisher exact test were used for categorical variables. The Shapiro-Wilk test and Mann-Whitney U test were used for continuous variables. Logistic regression and machine learning algorithm models based on CT characteristics, texture, and hematological parameters were used to predict LN metastasis. The performance of the multivariate models was evaluated using a receiver operating characteristic curve; prediction performance was evaluated in the validation cohorts. Decision curve analysis confirmed its clinical utility. Results Logistic regression analysis demonstrated that pleural thickening (P = 0.013), percentile 25th (P = 0.033), entropy gray-level co-occurrence matrix 10 (P = 0.019), red blood cell distribution width (P = 0.012), and lymphocyte-to-monocyte ratio (P = 0.049) were independent risk factors associated with LN metastasis. The area under the curve of the predictive model established using the previously mentioned 5 independent risk factors was 0.929 in the receiver operating characteristic analysis. The highest area under the curve was obtained in the training cohort (0.777 using Naive Bayes algorithm). Conclusions Integrative predictive models of CT characteristics, texture, and hematological parameters could predict LN metastasis in lung adenocarcinomas. These findings may provide a reference for clinical decision making.
