Abstract:Background: BMS-986156 is an agonist of the glucocorticoid-induced tumor necrosis factor receptor (TNFR)-related protein (GITR) and promotes increased effector T-cell activation. Combined anti-GITR, anti-programmed death-1, anti-cytotoxic T-lymphocyte-associated protein 4 antibodies and radiotherapy improve tumor control in preclinical studies. Herein we describe the results of the safety and efficacy of BMS-986156+ipilimumab or nivolumab with/without stereotactic ablative radiotherapy (SABR) in patients with advanced solid cancers (NCT04021043). Methods: This open-label, multigroup, single-center phase I/II study enrolled patients with histologically-confirmed stage IV solid cancers resistant to standard treatments. Group 1 (G1, n=20) received four cycles of ipilimumab (3 mg/kg) plus BMS-986156 (30 mg as dose level 1 (L1) or 100 mg as dose level 2 (L2)), every 3 weeks (Q3W). Group 2 (G2, n=10) received four cycles of ipilimumab (3 mg/kg) plus BMS-986156 (dose as determined in G1, Q3W) with SABR (50 Gy/4 fx or 60-70 Gy/10 fx to liver/lung lesions. Group 3 (G3, n=20) received four cycles of nivolumab (480 mg) plus BMS-986156 (30 mg), every 4 weeks with SABR. Maintenance nivolumab could be given up to 2 years. Tumor responses were assessed every 1-3 months until progression, using immune-related response criteria. Results: 50 patients were enrolled between 10/2019 and 12/2021. Patients received a median of 3 (IQR 2-4.25) initial treatment cycles. 100 mg BMS-986156 with ipilimumab was tolerated well. Five discontinued BMS-986156 with ipilimumab due to treatment-related adverse events (TRAEs), with three in G1/L1, one in G1/L2 and one in G2, respectively. 22 patients (44%) experienced Grade 1-3 TRAEs (6, 4, 5, 7 patients for G1/L1, G1/L2, G2, G3). Six (12%) had Grade 3 TRAEs (2, 2, 1, 1 for G1/L1, G1/L2, G2, G3), with elevated alanine aminotransferase (n=3, in G1/L2, G2 and G3) and aspartate aminotransferase (n=2, in G2 and G3) being the most common. There was no Grade 4-5 TRAEs. Overall, 19/39 (48.7%) patients eligible for efficacy analysis had stable disease and 3 (7.7%) achieved a partial response. Out-of-field (abscopal) disease control rate (ACR) and out-of-field (abscopal) response rate (ARR) were 38.5% and 7.7%, respectively, with the highest ACR (50%, 9/18) and ARR (11.1%, 2/18) in G3. Conclusions: BMS-986156 was well-tolerated with ipilimumab, nivolumab, with or without SABR. Outcomes were encouraging in this population, as more than half of patients had stable disease/partial response.

Abstract:Purpose: This phase I/IIa study (NCT02737475) evaluated the safety and activity of BMS-986178, a fully human OX40 agonist IgG1 mAb, ± nivolumab and/or ipilimumab in patients with advanced solid tumors. Patients and Methods: Patients (with non–small cell lung, renal cell, bladder, other advanced cancers) received BMS-986178 (20–320 mg) ± nivolumab (240–480 mg) and/or ipilimumab (1–3 mg/kg). The primary endpoint was safety. Additional endpoints included immunogenicity, pharmacodynamics, pharmacokinetics, and antitumor activity per RECIST version 1.1. Results: Twenty patients received BMS-986178 monotherapy, and 145 received combination therapy in various regimens (including two patients receiving nivolumab monotherapy). With a follow-up of 1.1 to 103.6 weeks, the most common (≥5%) treatment-related adverse events (TRAEs) included fatigue, pruritus, rash, pyrexia, diarrhea, and infusion-related reactions. Overall, grade 3–4 TRAEs occurred in one of 20 patients (5%) receiving BMS-986178 monotherapy, six of 79 (8%) receiving BMS-986178 plus nivolumab, zero of two receiving nivolumab monotherapy, six of 41 (15%) receiving BMS-986178 plus ipilimumab, and three of 23 (13%) receiving BMS-986178 plus nivolumab plus ipilimumab. No deaths occurred. No dose-limiting toxicities were observed with monotherapy, and the MTD was not reached in either the monotherapy or the combination escalation cohorts. No objective responses were seen with BMS-986178 alone; objective response rates ranged from 0% to 13% across combination therapy cohorts. Conclusions: In this study, BMS-986178 ± nivolumab and/or ipilimumab appeared to have a manageable safety profile, but no clear efficacy signal was observed above that expected for nivolumab and/or ipilimumab.

Abstract:Abstract Background: Mesothelin, a GPI-anchored cell surface protein, is highly expressed in several tumor types and can be targeted for antibody (ab)-based cancer therapy. BMS-986148 is a fully human IgG1 anti-mesothelin monoclonal ab conjugated to tubulysin to promote selective cytotoxic delivery to tumor cells. In preclinical models, combination of anti–mesothelin-tubulysin with anti–PD-1 promoted a synergistic antitumor response and influx of tumor-infiltrating lymphocytes. Here, we present initial data for BMS-986148 ± nivolumab (NIVO; anti–PD-1) from a phase 1/2a trial in a biomarker-defined population of patients (pts) with select advanced solid tumors (NCT02341625). Methods: During dose escalation (ESC), pts received BMS-986148 0.1 to 1.6 mg/kg IV Q3W, BMS-986148 0.4 or 0.8 mg/kg IV Q1W, or BMS-986148 0.8 mg/kg + NIVO 360 mg IV Q3W. During monotherapy (mono) dose expansion (EXP), pts with mesothelin-selected mesothelioma (ie, H score ≥ 100 for tumor mesothelin expression), non-small cell lung cancer (NSCLC), or ovarian cancer received BMS-986148 1.2 mg/kg mono IV Q3W, and during combination (combo) dose EXP, pts with mesothelin-selected mesothelioma or pancreatic cancer received BMS-986148 0.8 mg/kg + NIVO 360 mg combo IV Q3W. The primary endpoint was safety. Secondary endpoints included pharmacokinetics (PK) and antitumor activity. Results: As of April 1, 2019, 126 pts have been treated in this trial; 96 pts received BMS-986148 mono (Q3W, n = 84; Q1W, n = 12), and 30 pts received BMS-986148 + NIVO combo. PK analysis in pts treated during ESC demonstrated that total antibody and active ADC exposures increased in a dose-proportional manner. Unconjugated tubulysin concentrations were low but sustained over the dosing interval. The maximum tolerated dose (MTD) for mono was 1.2 mg/kg Q3W, and the 0.8 mg/kg dose was determined to be tolerable for the combo. Across all cohorts, any-grade and grade 3/4 treatment-related AEs (TRAEs) were reported in 87% and 44% of all pts, respectively. Overall, the most common TRAEs (any grade; grade 3-4) reported in ≥ 15% of pts were AST increased (43%; 17%), ALT increased (41%; 16%), fatigue (37%; 5%), nausea (29%; 0%), decreased appetite (22%; 1%), and blood alkaline phosphatase increased (18%; 4%). The frequency of TRAEs and dose-limiting toxicities related to liver function tests increased with increasing dose of BMS-986148. Most TRAEs resolved with dose interruption, dose reduction, or treatment discontinuation. Serious TRAEs were reported in 18% of pts with mono and 23% of pts with combo. Fifteen percent of pts discontinued due to TRAEs, and 1 pt died due to a TRAE (pneumonitis; 1.2 mg/kg Q3W mono). The ORR was 6% (0 CR, 3 PR) with mono in EXP and 20% (0 CR, 6 PR) with the combo in ESC and EXP. Among pts with mesothelioma, the ORR was 4% with mono in EXP and 31% with combo in ESC and EXP. Among pts with ovarian carcinoma, the ORR was 9% with mono in EXP. Of note, durable responses lasting up to ≈ 20 mo in pts with ovarian cancer with mono and ≈ 9 mo in pts with mesothelioma with mono and combo were observed. Conclusions: BMS-986148 ± NIVO was tolerable and demonstrated a clinically manageable safety profile. Preliminary clinical activity was observed with BMS-986148 ± NIVO in select patient populations, including pts with mesothelioma. Abstract:2582 Background: The Phase 1/2, dose escalation/expansion study evaluates NGM707, a dual anti-ILT2/ILT4 humanized monoclonal antibody, as monotherapy or in combination with pembrolizumab in patients (pt) with advanced solid tumors. Methods: We enrolled pt with locally advanced or metastatic solid tumors into dose-escalating cohorts of 6-1800 mg NGM707 monotherapy and 200-1800 mg NGM707 combined with 200 mg pembrolizumab, administered Q3W IV. Primary aim was to assess safety/tolerability and dosing of expansion cohorts. Secondary/exploratory objectives included pharmacokinetics, biomarkers, and preliminary antitumor activity per RECIST v1.1. Results: As of November 6, 2023, we treated 82 pt with NGM707 monotherapy or combination at dose levels up to 1800 mg; five pt crossed over from monotherapy to combination. Primary tumor types included colorectal, NSCLC, gastric, pancreatic, and melanoma. Median age 59 yrs [28-85]; ECOG PS 0 (18.3%), 1 (81.7%). Pt received a median of 4 prior therapies (range 1-16) and 91.5% had metastatic disease. Fifty percent of the pt were pre-treated with anti-PD(L)1. Peripheral RO was dose-dependent, with NGM707 doses ≥200 mg maintaining full ILT2 and ILT4 RO. PK was typical for monoclonal antibodies, with a half-life of 12.8 days. Paired tumor biopsies showed evidence of myeloid and T cell activation. Treatment(tx)-related adverse events (TEAEs) any grade/grade ≥3 occurred in 46.3%/4.8% of pt in the monotherapy and 41.3%/4.4% of pt in the combination. Fatigue (12.2%), arthralgia (9.8%), nausea (9.8%) were reported in monotherapy; fatigue (17.4%), diarrhea (6.5%) were reported in combination. One dose-limiting toxicity (DLT; pneumonitis) occurred in monotherapy and no DLTs in combination. MTD was not reached for both tx; MAD was 1800 mg NGM707. Of 35 response-evaluable monotherapy pt, best overall responses (BOR) were one confirmed PR in pt with melanoma, SD (n=9) and non-CR/non-PD (n=1), leading to DCR ~31%. Eight pt had reduced target lesion (TL) size with maximum reduction of 71%. Of 37 response-evaluable pt in combination, the BOR to date are PR (confirmed; n=4), and SD (n=12), representing DCR ~43%. Nine pt had reduced TL size with a maximum reduction of 100%. Of the 4 pt who had PR, 3 pt were pre-treated with anti-PD(L)1. Two pt with MSS CRC achieved PR, one of them with liver/adrenal TL reduction allowing surgical resection of all residual disease with pCR; ctDNA was not detected. Durable response in this pt led to PFS of 11 months prior to the surgery and ongoing DFS post-surgery. Conclusions: NGM707 as monotherapy and in combination with pembrolizumab was safe and well tolerated at all dose levels. In heavily pretreated advanced and metastatic solid tumor malignancies, we observed early efficacy and biomarker signals, including in tumors considered unresponsive to anti-PD(L)1. Abstract:2646 Background: IBI319 is a recombinant fully human anti-CD137/PD-1 bispecific antibody. In preclinical studies, IBI319 demonstrated a better anti-tumor efficacy and safety. Here we report the initial results from the first-in-human study of IBI319 in patients with advanced malignancies. Methods: Enrolled patients (pts), ECOG PS ≤ 1, had advanced solid tumors or hematological malignancies and failed standard treatment. This phase 1 monotherapy dose-escalation includes accelerated titration (0.03 and 0.1mg/kg) and conventional 3+3 dose escalation (0.3, 1.0, 3.0, 6.0 and 10.0 mg/kg). IBI319 is administered intravenously every 14 days after dose-limiting toxicity (DLT) evaluation. Objectives include evaluation of safety, DLT, pharmacokinetic (PK) and antitumor activity. Adverse events (AEs) were reported per CTCAE v5.0 and DLTs evaluated within a 28-day window. Results: At cutoff date of February 10, 2022, 21 pts were enrolled (median age 55 years; 81.0% male; 19.1% ECOG PS 1; 52.4% advanced solid tumors). The following dose levels have been evaluated; 0.03 mg; 0.1 mg; 0.3 mg; 1 mg; 3 mg and 6 mg. Dose escalation is ongoing. The maximum tolerated dose has not been reached. Treatment-emergent adverse events (TEAEs) was reported in 15 out of 21 (71.4%) pts, among which 1 pts had back pain and nausea of grade 3. The most common TEAEs ( > 10%) were interleukin level increased (6 pts, 28.6%) and C-reactive protein increased (3 pts, 14.3%). Eleven pts (52.4%) experienced treatment-related AEs; none were grade 3 or higher. The most common TRAEs ( > 10%) were interleukin level increased (6 pts, 28.6%) and C-reactive protein increased (3 pts, 14.3%). Importantly, no drug-related elevations in transaminases (ALT, AST) or bilirubin have been seen. No DLT was observed. PK analysis of IBI319 demonstrated consistent exposure with linear PK. For 14 evaluable pts, 1 classical Hodgkin lymphoma patient achieved PR as best response. Conclusions: IBI319 demonstrates good tolerability in patients with advanced solid tumors or hematological malignancies, with a safety profile characterized by a lack of hepatotoxicity frequently observed with CD137-targeting antibodies. Abstract:Introduction: Blockade of the CTLA-4 pathway with ipilimumab (IPI), alone and in combination with nivolumab (anti-programmed death-1 antibody), has shown clinical benefit in multiple tumor types. However, not all tumors respond to IPI ± nivolumab, and peripheral effects can lead to immune-related adverse events. To enhance the therapeutic index of CTLA-4-directed therapy, a proprietary Probody® therapeutics technology platform was used to develop a modified version of IPI. BMS-986288 is an anti-CTLA-4 antibody that combines a Probody® therapeutic masking peptide and protease-cleavable linker, designed to localize anti-CTLA-4 activity to the tumor microenvironment, with an NF Fc region hypothesized to enhance antigen-presenting cell (APC)-mediated T-cell priming and regulatory T cell (Treg) modulation via increased binding to the FcγRIIIA (CD16) receptor. Here, we present the characterization of the mechanism of action and pharmacodynamic (PD) response of BMS-986288 in preclinical models. Methods: APC-mediated T-cell priming and antigen-specific responses were evaluated in a superantigen model using peripheral blood mononuclear cells (PBMCs) and human CTLA-4 knock-in (KI) mice. Antitumor activity and immune cell population changes were assessed in an MC38 tumor model implanted in human CTLA-4 KI mice. Intratumoral Treg depletion was further investigated using patient-derived dissociated tumor samples as a physiologically relevant model. Peripheral PD effects and tolerability of BMS-986288 were evaluated in a non-human primate (NHP) Ad5 vaccine model. Results: Protease-cleaved BMS-986288 demonstrated enhanced APC-mediated T-cell priming compared with IPI in superantigen-stimulated PBMCs. Similarly, administration of superantigen staphylococcal enterotoxin B (SEB) peptide elicited an increased antigen-specific T-cell receptor (Vβ8+) T-cell response after treatment with cleaved BMS-986288 vs IPI in human CTLA-4 KI mice. In the MC38 tumor-bearing mouse model, BMS-986288 showed enhanced antitumor activity as compared to IPI, with tumor clearance in all mice. Although BMS-986288 enhanced intratumoral CTLA-4+ Treg depletion in the MC38 model, limited Treg depletion was observed in the patient-derived dissociated tumor model with an NF anti-CTLA-4 antibody without the masking peptide, potentially reflecting differences in CTLA-4 expression on human vs mouse Tregs. BMS-986288 reduced PD effects in the peripheral blood compartment and increased tolerability in NHP relative to IPI. Conclusion: BMS-986288 leverages unique characteristics to differentiate it from IPI, enhancing APC-mediated T-cell priming and anti-tumor activity while also reducing peripheral activity in preclinical models. These data support an ongoing phase 1/2 clinical trial of BMS-986288 (NCT03994601) in patients with advanced solid tumors. Abstract:3002^ Background: Blockade of programmed death-1 (PD-1), a co-inhibitory receptor expressed by activated T cells, can overcome immune resistance and mediate tumor regression (Topalian et al., NEJM 2012). Here we present long-term safety and efficacy outcomes from a phase I study of nivolumab, a PD-1 blocking mAb, in patients (pts) with advanced solid tumors. Methods: Pts enrolled between 2008-2012 received nivolumab (0.1−10 mg/kg IV Q2W) during dose escalation and/or cohort expansion. Tumors were assessed by RECIST 1.0 after each 4-dose cycle. Pts received ≤12 cycles until unacceptable toxicity, confirmed progression, or CR. Results: 304 pts with non-small cell lung cancer (NSCLC, n=127, squamous and nonsquamous), melanoma (MEL, n=107), renal cell (RCC, n=34), colorectal (n=19) or prostate cancer (n=17) were treated. Abstract:Abstract Background: NGM707 is a dual antagonist antibody that binds a shared epitope of the immune inhibitory receptors ILT2 (LILRB1) and ILT4 (LILRB2). In preclinical studies, NGM707 reprogrammed suppressive myeloid cells via blockade of ILT2/ILT4 and enhanced NK and CD8+ T cell activity via blockade of ILT2; furthermore, NGM707 and pembrolizumab acted additively in vitro to enhance CD4+ T cell activation in mixed lymphocyte reactions. NGM707-IO-101 (NCT04913337) is a first-in-human Phase 1/2 clinical trial of NGM707 monotherapy and combination with pembrolizumab. Here we report pharmacodynamic and predictive biomarker data from NGM707-IO-101 monotherapy and combination dose escalation cohorts. Methods: Eligible patients with advanced or metastatic solid tumors were enrolled into escalating dose cohorts from 6 to 1800 mg NGM707, alone or in combination with 200 mg pembrolizumab, administered Q3W iv. Biomarker assessments of tumors comprised RNA-sequencing and immunohistochemistry (including ILT2, ILT4, CD163) of archival/baseline (N= 41 IHC, 47 RNA) and paired on-treatment tumor biopsies (N=13 IHC, 21 RNA). Peripheral blood biomarker samples were collected on days 1, 4, 8, and 15 of cycle 1 and prior to dosing at each subsequent cycle. Blood biomarker assessments included circulating cytokines/chemokines, PBMC RNA-sequencing, and flow cytometry immunophenotyping. Baseline biomarkers were associated with anti-tumor activity per RECIST v1.1. Results: Pre-treatment biopsies from patients with disease control (DC) showed higher expression of genes in the 18-gene tumor inflammation signature (TIS) than patients with progressive disease (PD). A composite gene signature incorporating gene sets related to TIS, NK cell activity, and tumor cell proliferation had a statistically stronger association with DC than TIS genes alone. Baseline ILT2/ILT4 target expression (≥1% of tumor area) was observed in 31 (75.6%) patients, with similar levels observed in patients with DC and PD. Pharmacodynamic changes were observed in tumor samples following treatment, including increased expression of gene signatures that reflect myeloid cell activation and a proinflammatory tumor microenvironment. Decreased protein expression of the immunosuppressive myeloid marker CD163 was observed following treatment in both blood and tumor for a subset of patients. Proinflammatory chemokines including CXCL9 increased in serum following treatment. Conclusion: Baseline gene expression signatures have shown an encouraging association with clinical benefit from NGM707 and merit further investigation as promising predictive biomarkers. Protein and gene expression changes reflecting the expected modes of action of NGM707 and pembrolizumab were observed in both peripheral blood and tumor biopsies following treatment. Abstract:Abstract Introduction: LAG3 is an immune checkpoint receptor, often co-expressed with PD-1 on immune cell surfaces. PD-1 and LAG-3 signaling contribute to immune cell exhaustion in the tumor microenvironment. Dual blockade of PD-1 and LAG-3 is thus expected to improve response versus PD-1 blockade alone. A dose escalation/expansion trial (NCT03156114) is evaluating BI 754111, an anti-LAG-3 mAb, plus BI 754091, an anti-PD-1 mAb, in patients (pts) with advanced solid tumors. Here, we report data in pts with MSS mCRC, for whom checkpoint inhibitors (CPIs) have shown limited activity. We broadly evaluated biomarkers to provide information on mode of action and for their potential to predict responses. Methods: In this dose expansion cohort, pts with PD-(L)1-naïve, previously treated MSS mCRC tumors were enrolled. Pre- and on-treatment tumor biopsies and blood samples for biomarker analyses were collected. Cytokines, including interferon-gamma (IFN-γ), were quantified in plasma samples by multiplexed and high-sensitivity immunoassays. Activated effector memory T cell and other peripheral blood mononuclear cell counts were determined by flow cytometry. PD-L1, LAG-3 and CD8 expression was determined by immunohistochemistry (IHC). Exploratory gene expression analyses to determine immuno-oncology (IO)-related markers were conducted. Results: 40 pts with MSS mCRC received BI 754111 600 mg in combination with BI 754091 240 mg every 3 weeks. Pts had received a median (range) of 3.5 (1–10) prior treatments. To date (Sep 2019), 3 (7.5%) pts achieved a partial response (PR) and 11 (27.5%) had stable disease (SD) as best response. Peripheral blood analysis showed treatment led to coordinated upregulation of pro-inflammatory cytokines in some pts. Pts with greater cytokine induction were more likely to have SD. Also, upregulation of activated CD8 effector memory T cells in peripheral blood was observed in many pts. IHC analysis indicated that, in tumors that had CD8 T cells and PD-L1 expression at the tumor periphery at baseline, treatment led to infiltration of CD8 T cells into the tumor and an increase in PD-L1 expression. Analyses of pre-treatment tumors suggested that high PD-L1 gene expression was associated with clinical benefit. In addition, a treatment-associated increase of the IFN-γ gene signature scores was observed, supporting a treatment-induced activation of the immune system in the tumor; the effect was more pronounced in pts with PR/SD versus those with progressive disease. LAG-3 IHC levels at baseline were not predictive of outcome in this anti-PD-(L)1 naïve setting. Conclusion: BI 754111 plus BI 754091 showed encouraging results in this IO-refractory MSS mCRC population. Abstract:Homozygous deletion of MTAP gene (MTAP del) occurs in ∼10% of all cancers. BMS- 986504 is a second-generation MTA-cooperative PRMT5 inhibitor designed to preferentially bind to the PRMT5-MTA complex which accumulates in MTAP del cancer cells and has demonstrated clinical proof-of-concept (POC) for selectively inhibiting PRMT5 activity in MTAP del patients as monotherapy. While immunotherapy (IO) has increased patient survival in many cancers and is now the standard of care in first and later lines, most patients ultimately relapse. Thus, there remains an unmet need to improve response for these patients receiving IO. BMS-986504 in combination with anti- PD1 has the potential to increase efficacy, but there is a limited understanding of the interaction between MTAP/PRMT5 biology and PD-1 blockade. Here, we evaluated the BMS-986504-induced immunological and cell killing effects on MTAP del tumor cells and T cells to understand potential mechanisms to support BMS-986504 combination opportunities with anti-PD1 therapy. Using a panel of human PBMC-derived T cells, we first demonstrate that non-activated T cells and T cells activated via anti-CD3/CD28 are insensitive to BMS-986504-induced cell killing at in vitro concentrations equivalent to clinical exposure, supporting the significantly improved selectivity margin of BMS- 986504 relative to first-generation PRMT5 inhibitors that showed T cell toxicity in vitro and on-target dose-limiting hematological toxicity in the clinic. Since previous studies have suggested that MTAP wild-type cells may be affected by MTA secreted from surrounding MTAP del tumors, we assessed T cell viability in the presence of MTA. We found that inactivated T cells maintain insensitivity to BMS-986504 treatment with or without extracellular MTA, but T cells activated via anti-CD3/CD28 are partially sensitive to BMS-986504-induced killing in the presence of MTA only. Using flow cytometry, we characterized the surface expression of immune-related markers on tumor cells treated with BMS-986504 and identified that BMS-986504 dose-dependently increases PD-L1 expression on MTAP del tumor cells, suggesting a potential immune evasion mechanism. Next, using human PBMC-derived T cell and tumor cell co-culture assays, we evaluated the ability of BMS-986504 and anti-PD1 combination to mediate both tumor-intrinsic killing and anti-tumor immune activation in MTAP del cancer cell lines. Taken together, we provide translational proof-of-concept data demonstrating that BMS- 986504 induces tumor cell-intrinsic immunological effects in MTAP del tumors, while having limited impact on T cell viability. This data supports the rationale for BMS- 986504 and IO combination, which may potentially lead to anti-tumor immune activation and improved outcome for MTAP del patients. Abstract:Modulating T cell homeostatic mechanisms with checkpoint blockade can efficiently promote endogenous anti-tumor T cell responses1-11. However, many patients still do not benefit from checkpoint blockade12, highlighting the need for targeting of alternative immune pathways13. Glucocorticoid-induced tumor necrosis factor receptor-related protein (GITR) is an attractive target for immunotherapy, owing to its capacity to promote effector T cell (Teff) functions14,15 and hamper regulatory T cell (Treg) suppression16-20. On the basis of the potent preclinical anti-tumor activity of agonist anti-GITR antibodies, reported by us and others16,21,22, we initiated the first in-human phase 1 trial of GITR agonism with the anti-GITR antibody TRX518 ( NCT01239134 ). Here, we report the safety profile and immune effects of TRX518 monotherapy in patients with advanced cancer and provide mechanistic preclinical evidence to rationally combine GITR agonism with checkpoint blockade in future clinical trials. We demonstrate that TRX518 reduces circulating and intratumoral Treg cells to similar extents, providing an easily assessable biomarker of anti-GITR activity. Despite Treg reductions and increased Teff:Treg ratios, substantial clinical responses were not seen. Similarly, in mice with advanced tumors, GITR agonism was not sufficient to activate cytolytic T cells due to persistent exhaustion. We demonstrate that T cell reinvigoration with PD-1 blockade can overcome resistance of advanced tumors to anti-GITR monotherapy. These findings led us to start investigating TRX518 with PD-1 pathway blockade in patients with advanced refractory tumors ( NCT02628574 ).

Abstract:2505 Background: GB1275 is a first-in-class, oral CD11b modulator that reduced myeloid-derived suppressor cells (MDSCs) and tumor associated macrophages (TAMs), repolarized M2 immunosuppressive TAMs to an M1 phenotype, resulting in increased tumor infiltration of activated CD8+ T cells and antitumor efficacy in preclinical models. Here, we report preliminary results from an ongoing, first-in-human dose-escalation study in specific advanced tumors using GB1275 alone or with pembrolizumab. (NCT04060342) Methods: Phase 1 comprises dose escalation and expansion. During dose escalation, cohorts of 3 to 6 subjects were sequentially assigned to ascending dose levels of GB1275 from 100 mg to 1200 mg BID in one of two dosing regimens: Regimen A [GB1275 monotherapy orally (PO) twice a day (BID)] and Regimen B [GB1275 PO BID plus pembrolizumab 200 mg IV every 3 weeks (q3wks)]. Dose escalation was based on safety including dose-limiting toxicities (DLTs). Following dose escalation, up to 40 subjects with specific tumor types are to be treated in expansion with the selected GB1275 dose plus pembrolizumab to assess safety, pharmacokinetics, and preliminary clinical and biomarker activity. Results: As of January 8, 2021, 45 subjects were treated [44 in dose escalation: 23, Regimen A; 21, Regimen B. 1 in expansion, Regimen B], with median (range) GB1275 exposure of 42.0 days (4-263). No DLTs were reported. GB1275-related adverse events occurred in 24/45 (53.3%) subjects; photosensitivity reaction (20.0%), dysesthesia (13.3%) and pruritus (13.3%) were most frequent (≥10%). Stable disease was reported in 6/19 (31.6%) response-evaluable subjects in Regimen A and 9/16 (56.3%) in Regimen B. In Regimen B (800 mg), one partial response was reported in a subject with MSS-CRC treated for 263 days, and one prolonged stable disease (227 days) was reported in a gastric cancer (GC) subject previously treated with pembrolizumab plus bavituximab for less than 3 months due to progression; both subjects are continuing study treatment. A dose-dependent increase in GB1275 systemic exposure was observed up to 800 mg BID. Down-regulation of peripheral MDSCs was seen with both regimens. Regimen-dependent gene clusters in whole blood were noted. An increase in tumor infiltrating lymphocyte (TIL) counts was noted in both Regimens A and B. Conclusions: Dose escalation of GB1275, up to 1200 mg in Regimens A and B, demonstrated tolerability as monotherapy and combined with pembrolizumab in subjects with advanced cancers. Encouraging antitumor activity in Regimen B (800 mg) was observed in subjects with MSS-CRC and GC. Biological activity reflected by MDSC modulations in blood and TIL Increases in tumor biopsies with GB1275 alone and with pembrolizumab supports the mechanism of GB1275. Abstract:e14501 Background: KD6001, a novel fully human anti-CTLA4 IgG1 monoclonal antibody, exhibits immunomodulatory effects, enhances T-cell-mediated antitumor immune response and shows promising activity in preclinical models of solid tumors. Here we present the pharmacokinetics, safety and tolerability data from the phase 1 study of KD6001 (NCT05230290) in patients with advanced solid tumors. Methods: patients were enrolled in a 3+3 dose escalation phase 1 study with the following pre-defined KD6001 cohorts: 0.2mg/kg, 1mg/kg, 3mg/kg or 6mg/kg, every 2 weeks via intravenous infusion for 4 cycles. The primary objective was to evaluate the safety and tolerability of KD6001 and determine the MTD/RP2D. The secondary objectives were to measure antitumor activity, immunogenicity and pharmacokinetics (PK). All AEs are graded using NCI CTCAE v5.0. Efficacy assessments are per RECIST 1.1. Blood samples are obtained for pharmacokinetics (PK) and for immunogenicity assessments, measured by the development of anti-drug antibodies. Results: The study had been completed. Thirteen pts with melanoma had been enrolled and treated at 0.2 mg/kg (n = 1), 1mg/kg (n = 3), 3 mg/kg (n = 6) and 6mg/kg (n = 3) at Beijing Cancer Hospital. The average age was 57.2 years (range 34-68). Baseline ECOG scores were 0 (n = 5), 1(n = 8) with all pts. Seven pts (53.85%) received ≥ 2 lines of previous systemic therapies and 12 pts (92.31%) received previous PD-1/PD-L1 treatment. KD6001 was well-tolerated with no dose-limiting toxicities (DLTs) observed, nor was the MTD reached at the highest doses administered. All the thirteen pts had at least one adverse event during treatment, and eleven of them (84.62%) had Treatment Related Adverse Events (TRAEs), most of which were Grades 1-2. There were no permanent drug discontinuation or deaths due to adverse events. Pharmacokinetic analysis indicated a linear correlate between the drug exposure (Cmax, AUC0-t and AUC0-inf) and study drug dosages evaluated. The geometric mean T1/2 was 10 days (CV%: 31%). The peak concentration and exposure level increased proportionally to dose from 0.2mg/kg to 6 mg/kg. No anti-drug antibodies were detected in any of the patient’s serum samples. Clinical efficacy was observed in 12 evaluable pts: 3 patients at 3mg/kg had a stable disease (SD) and 1 patient at 3mg/kg had a confirmed partial response (PR). The ORR of the 3mg/kg dose group was 16.7% (1/6), the DCR was 66.67% (4/6). The median PFS was 109 days and the median OS was not reached. Conclusions: KD6001 showed promising antitumor activity and a tolerable safety profile in patients with advanced solid tumors. Hence, 3mg/kg was selected as the RP2D. Abstract:Purpose: Programmed death-1 (PD-1), an inhibitory receptor expressed on activated T cells, may suppress antitumor immunity. This phase I study sought to determine the safety and tolerability of anti-PD-1 blockade in patients with treatment-refractory solid tumors and to preliminarily assess antitumor activity, pharmacodynamics, and immunologic correlates. Patients and methods: Thirty-nine patients with advanced metastatic melanoma, colorectal cancer (CRC), castrate-resistant prostate cancer, non-small-cell lung cancer (NSCLC), or renal cell carcinoma (RCC) received a single intravenous infusion of anti-PD-1 (MDX-1106) in dose-escalating six-patient cohorts at 0.3, 1, 3, or 10 mg/kg, followed by a 15-patient expansion cohort at 10 mg/kg. Patients with evidence of clinical benefit at 3 months were eligible for repeated therapy. Results: Anti-PD-1 was well tolerated: one serious adverse event, inflammatory colitis, was observed in a patient with melanoma who received five doses at 1 mg/kg. One durable complete response (CRC) and two partial responses (PRs; melanoma, RCC) were seen. Two additional patients (melanoma, NSCLC) had significant lesional tumor regressions not meeting PR criteria. The serum half-life of anti-PD-1 was 12 to 20 days. However, pharmacodynamics indicated a sustained mean occupancy of > 70% of PD-1 molecules on circulating T cells > or = 2 months following infusion, regardless of dose. In nine patients examined, tumor cell surface B7-H1 expression appeared to correlate with the likelihood of response to treatment. Conclusion: Blocking the PD-1 immune checkpoint with intermittent antibody dosing is well tolerated and associated with evidence of antitumor activity. Exploration of alternative dosing regimens and combinatorial therapies with vaccines, targeted therapies, and/or other checkpoint inhibitors is warranted.

Abstract:Abstract Background Triple-negative breast cancer (TNBC) is an aggressive breast cancer subtype with limited systemic treatment options. RX-5902 is a novel anti-cancer agent that inhibits phosphorylated-p68 and thus attenuates nuclear β-catenin signaling. The purpose of this study was to evaluate the ability of β-catenin signaling blockade to enhance the efficacy of anti-CTLA-4 and anti-PD-1 immune checkpoint blockade in immunocompetent, preclinical models of TNBC. Methods Treatment with RX-5902, anti-PD-1, anti-CTLA-4 or the combination was investigated in BALB/c mice injected with the 4 T1 TNBC cell line. Humanized BALB/c-Rag2 null Il2rγ null SIRPα NOD (hu-CB-BRGS) mice transplanted with a human immune system were implanted with MDA-MB-231 cells. Mice were randomized into treatment groups according to human hematopoietic chimerism and treated with RX-5902, anti-PD-1 or the combination. At sacrifice, bone marrow, lymph nodes, spleen and tumors were harvested for flow cytometry analysis of human immune cells. Results The addition of RX-5902 to CTLA-4 or PD-1 inhibitors resulted in decreased tumor growth in the 4 T1 and human immune system and MDA-MB-231 xenograft models. Immunologic analyses demonstrated a significant increase in the number of activated T cells in tumor infiltrating lymphocytes (TILs) with RX-5902 treatment compared to vehicle ( p < 0.05). In the RX-5902/nivolumab combination group, there was a significant increase in the percentage of CD4+ T cells in TILs and increased systemic granzyme B production ( p < 0.01). Conclusions Conclusions: RX-5902 enhanced the efficacy of nivolumab in a humanized, preclinical model of TNBC. Several changes in immunologic profiles were noted in mice treated with RX-5902 and the combination, including an increase in activated TILs and a decrease in human myeloid populations, that are often associated with immunosuppression in a tumor microenvironment. RX-5902 also was shown to potentiate the effects of checkpoint inhibitors of CTLA4 and the PD-1 inhibitor in the 4 T-1 murine TNBC model. These findings indicate that RX-5902 may have important immunomodulatory, as well as anti-tumor activity, in TNBC when combined with a checkpoint inhibitor.

Abstract:BACKGROUND: NGM707-IO-101 (NCT04913337) is a Phase 1/2, dose escalation/expansion study evaluating NGM707, a humanized monoclonal antibody that blocks ILT2 and ILT4 receptors, as monotherapy or in combination with pembrolizumab. Here we report data from the combination part of the study. METHODS: Eligible patients (pts) with advanced/metastatic solid tumors were enrolled into escalating dose cohorts of 200-1800 mg NGM707 combined with 200 mg pembrolizumab, administered Q3W iv. Assessment of safety and tolerability and recommended expansion doses for NGM707 in combination with pembrolizumab were the primary objectives of the study. Secondary and exploratory objectives included assessment of pharmacokinetics/receptor occupancy (RO), immunogenicity, biomarkers, and preliminary antitumor activity per RECIST v1.1. RESULTS: As of 06 Nov 2023, 46 pts have been enrolled in the combination escalation at dose levels up to 1800 mg; 5 pts crossed over from monotherapy. Median age 58.5 yrs [28-81]; ECOG PS 0 (19.6%), 1 (80.4%). Pts had received a median of 3 prior therapies (range 1-6) and all pts had metastatic disease. Overall, 20/45 pts had prior exposure to anti-PD1/anti-PD-L1. Treatment-related adverse events (any grade/grade ≥3) occurred in 41.3%/4.4% of pts. The most common treatment-related adverse events were fatigue (17.4%), diarrhea (6.5%), myalgia (4.3%), arthralgia (4.3%), and nausea (4.3%). No dose-limiting toxicities were observed. A maximum tolerated dose was not reached, and the maximum administered dose was 1800 mg NGM707 in combination with 200 mg pembrolizumab. Peripheral immune cells showed dose-dependent RO, with doses ≥ 200 mg maintaining ILT2 and ILT4 RO for the entire dosing interval. PK of NGM707 was typical for monoclonal antibodies, with a half-life of 12.8 days. Gene expression changes associated with myeloid cell and T cell activation were observed in tumor biopsies post-treatment. Of 37 response-evaluable pts, best overall responses to date are PR (confirmed) in 4 pts, SD in 12 pts, representing a DCR of ~43%. Nine pts had reduced target lesion (TL) size with a maximum reduction of 100%. Out of the 4 patients who had PR, 3 patients were pre-treated with anti-PD1/anti-PD-L1. Two subjects with MSS CRC achieved confirmed PR, one of them with TL reduction that allowed subsequent surgical resection of all gross residual disease and confirmed pathological CR with no active tumor cells and ctDNA below detection. CONCLUSIONS: NGM707 in combination with pembrolizumab appears to be safe and well tolerated at all dose levels. In this advanced and metastatic solid tumor cohort, early efficacy signals have been observed. Preliminary evidence of myeloid cell reprogramming was observed in paired tumor biopsies. Abstract:2574 Background: OX40 is an immune costimulatory receptor, expressed on activated CD4 + and CD8 + T cells, which promotes T cell proliferation and survival in the tumor microenvironment. BGB-A445 is a novel monoclonal antibody (mAb) OX40 agonist that does not compete with endogenous OX40 ligand binding. BGB-A445 has demonstrated antitumor activity as a single agent and in combination with an anti-PD-1 mAb in preclinical studies. Here, we report data from the ongoing dose-escalation part of a multicenter, phase 1 dose escalation/expansion study (NCT04215978) of BGB-A445 alone or in combination with tislelizumab (TIS) in patients (pts) with advanced solid tumors. Methods: Eligible pts were enrolled into seven sequential dose escalation cohorts of BGB-A445 IV as monotherapy (Part A) or five increasing dose levels of BGB-A445 IV in combination with TIS 200 mg IV (Part B) on day 1 of 21-day cycles. Dose escalation was guided by a Bayesian (mTPI-2) approach. This study assessed safety, tolerability, pharmacokinetics (PK) profile, and preliminary antitumor activity (RECIST v1.1). Results: As of August 31, 2022, 59 pts were enrolled in Part A and 32 pts in Part B. Median (range) age was 59 years (28-80) and 61 years (37-75) in Parts A and B, respectively; 26 (44%) and 13 (41%) pts were male, respectively. ≥Grade 3 treatment-emergent AEs (TEAEs) were reported in 24 (41%) and 17 (53%) pts in Parts A and B, respectively; the most commonly reported (≥3 reported) ≥grade 3 TEAEs were gastrointestinal disorders (diarrhea, nausea, and abdominal pain). Serious TEAEs were reported in 23 (39%) pts in Part A and 16 (50%) in Part B. Treatment-related AEs leading to treatment discontinuation were reported in 1 (2%) pt in Part A and 0 (0%) pts in Part B. In Part A, all grade immune-mediated AEs (imAEs) were reported in 11 (19%) pts; no pts reported ≥grade 3 imAEs. In Part B, all grade imAEs were reported in 14 (44%) pts; ≥grade 3 imAEs were reported in 1 (3%) pt (one event each of rash maculopapular and diarrhea). There were no dose-limiting toxicities or concerning patterns of safety/tolerability observed. BGB-A445 PK (maximum concentration [C max ] and area under the curve from day 0 to 21 [AUC 0-21 ]) were linear and dose proportional in all tested dose ranges. In the efficacy-evaluable population of Parts A (n=50) and B (n=30), respectively, partial response was observed in 2 (4%) pts (unconfirmed) and 7 (23%) pts (confirmed); stable disease in 18 (36%) and 13 (43%) pts (confirmed), and progressive disease in 26 (52%) and 8 (27%) pts. Conclusions: In the dose escalation part of this study, BGB-A445 alone or in combination with TIS was generally well tolerated across all doses in pts with advanced solid tumors, and demonstrated preliminary antitumor activity as both a single and combination agent. The dose expansion part of the study is ongoing in pts with non-small cell lung cancer and head and neck squamous cell carcinoma. Clinical trial information: NCT04215978 .

Abstract:Background In this open-label, randomized, non-comparative, multicenter phase II study (Alliance A091401) we report on three expansion cohorts treated with nivolumab (N) with and without ipilimumab (N+I) and provide a multi-omic correlative analysis of actionable biomarkers. Methods Patients were randomized (non-comparative) to receive either N or N+I. The primary endpoint was a 6-month confirmed response rate (CRR) defined by Response Evaluation Criteria in Solid Tumors version 1.1. Secondary endpoints included treatment-related adverse events (TRAEs), progression-free survival, and overall survival. Multi-omic correlative analyses were conducted using samples from both the primary and expansion cohorts. Results A total of 66 patients were evaluated for the primary endpoint with disease including gastrointestinal stromal tumor (GIST, n=18), undifferentiated pleomorphic sarcoma (UPS, n=24), and dedifferentiated liposarcoma (DDLPS, n=24). Neither N nor N+I achieved a complete or partial response in the GIST expansion cohort. In DDLPS and UPS, the primary response endpoint of CRR was met with N+I (both 16.6%, 2/12) but not with N alone (both 8.3%, 1/12). In the GIST cohort, TRAE was higher with N+I treatment, halting enrollment as required per protocol. In a correlative analysis of patients for the expansion cohort and the original cohort (n=86), traditional biomarkers of immunotherapy response were not correlated with response in any histological subtype. Markers of genomic instability including the presence of gene fusions and increased subclonal mutations correlated with improved clinical outcomes. Conclusions This expansion cohort reaffirms the outcomes of A091401. There remains a pressing need to determine the role of and predictive biomarkers for immunotherapy in sarcoma. Abstract:Abstract We have previously described a novel compound, BTM-3566, that exhibits robust single agent activity in xenograft and PDX models of DLBCL. BTM-3566 is effective on all types of DLBCL, independent of cell of origin (COO) or genotype. Drug activity depends on activation of the mitochondrial protease OMA1 followed by activation of HRI and induction of the ATF4 ISR. To ascertain whether ATF4 ISR induction occurs in vivo, we determined the pharmacokinetics (PK) and pharmacodynamic (PD) responses of BTM-3566 in mouse xenograft models of DLBCL using the double hit lymphoma SU-DHL-10. To establish PK parameters, tumor bearing mice were dosed orally for 1 or 5 consecutive days with 20 mg/kg BTM-3566 and blood sampled over 28 hours. Plasma levels of BTM-3566 were determined and analyzed using a non-compartment model. Half-life in blood plasma was 5.4 and 4.5 hours on days 1 and 5 respectively. Total exposure (AUCinf) was 524164 and 429485 hr*nM on day 1 and day 5 respectively. Drug AUCinf in tumor after a single oral dose of BTM-3566 was 151260 hr*nM. Free drug was estimated to be above the free IC90 for approximately 16 hours post-dose in both plasma and tumor. PD responses to BTM-3566 treatment were determined by measuring biomarkers of the therapeutic cascade: OPA1 cleavage is used as a surrogate for target engagement and activation of OMA1; increased phosphorylation of eIF2α represents pathway activation; induction of ATF4 regulated transcripts and the decrease in the anti-apoptotic protein MCL1 represent pathway effect. A single 20/mg/kg dose of BTM-3566 on day 1 resulted in cleavage of OPA1 by 0.5 hour, with maximal cleavage seen by 12 hours. Phosho-eIF2α was observed by 0.5 hours, peaking at 6 hours. Induction of the ATF4 transcripts ATF4, DDIT3, TRIB3, and CDKN1A was observed within 2 hours with a maximum at 6-12 hours followed by a reduction in transcript levels. MCL1 protein levels were reduced consistent with suppression of translation and protein turnover. By day 5, repeat dosing shows evidence of cumulative pathway effects: OPA1 is extensively cleaved; despite generally depressed levels, phosphorylation of eIF2α remains dynamic and MCL1 levels are markedly suppressed. Induction of ATF4 transcripts, however, follow largely the same kinetics as seen on day 1. The data suggests that in vivo anti-tumor activity of BTM-3566 is associated with activation of the mitochondrial protease OMA1 and induction of the ATF4 ISR. Robust tumor regression is correlated with both blood plasma levels of BTM-3566 and induction of the ATF4 ISR pathway. The data support the use of both ATF4 transcripts and OPA1 cleavage as robust PD markers of target engagement and pathway effect in human clinical trials. An Investigational New Drug application for BTM3566 in B-cell malignancies has been approved with initiation of first in human clinical trials planned for spring 2023. Abstract:Background: BC3402, an IgG4 monoclonal antibody (mAb) that targets TIM-3, is being developed for solid and hematological malignancies. In preclinical studies in immunocompetent cancers, BC3402 demonstrated synergy when combined with mAbs to PD-1 and/or CTLA-4, supporting a rationale for combination strategies in the clinic. As a first step, the safety, tolerability, pharmacokinetics (PK), pharmacodynamics, and preliminary anti-tumor activity of BC3402 monotherapy was evaluated in patients (pts) with advanced solid malignancies. Methods: In this phase I, open-label study, multiple doses of BC3402 were explored in pts with advanced solid cancers who had been previously treated with standard therapies to evaluate its safety and tolerability and recommend doses for further development. An accelerated titration design was used to select doses in the first two dose levels (0.02 and 0.2mg/kg IV Q3W) followed by a standard "3+3" design in the remainder of the study (1, 5, 10, 20 and 30 mg/kg IV Q3W). Results: Nineteen pts (median [range] age: 59 [29-73]; 63.2% male; 94.7% ECOG 1) were treated with at least one 3-week cycle of BC3402. DLT was not observed, and adverse events (AEs) were not dose related. The most common TEAEs (incidence ≥20%) and TRAEs (incidence ≥10%) were grade 1-2. The most common (incidence ≥10%) TEAE ≥ grade 3 was dysphagia (2/19 pts, 10.5%), which occurred in the 5 mg/kg and 10 mg/kg dose cohorts. Two pts reported grade 3 or above TRAEs – grade 3 hypertension (5 mg/kg) and grade 3 gamma-glutamyl transferase increase (10 mg/kg), There were neither dose interruptions nor treatment discontinuation due to AEs. There were no objective responses. Seven of 18 pts who were evaluable for tumor assessment had stable disease as best response with one pt (20 mg/kg dose cohort) with NSCLC having sustained reductions (32 weeks) in target lesions. Median Tmax values ranged from 1.08 to 2.96 hours, and mean T1/2 values ranged from 119 to 182 hours at doses ranging from 5-30 mg/kg. Exposure was linear in the dose range of 1 -30 mg/kg, and there was no drug accumulation after the second cycle. Receptor occupancy saturation was observed within the tested dose range. Based on these data, the RP2D was determined as 20mg/kg every 2 weeks or 30mg/kg every 3 weeks. Conclusions: BC3402 exhibited favorable safety profile and PK characteristics when administered as single infusion at doses ranging from 0.02 mg/kg to 30 mg/kg Q3W in pts with advanced solid cancers, and its safety profile supports further disease-directed studies in combination with other immune checkpoint inhibitors and/or rational therapeutic modalities.
