Abstract:the vulva, penis, scrotum and anal region. Only a few cases with cutaneous involvement different from the genital area have been reported. To the best of our knowledge, the present case is the first reported showing a widespread cutaneous involvement and concomitant lesions affecting the genital and oral mucosa. Based on this multifocal mucocutaneous involvement, we suggest the designation of ‘disseminated verruciform xanthoma’ for this rare clinical variant of VX. Clinically, VX is usually described as a solitary verrucous, papillomatous, tumour, papule or plaque with a yellow–red to yellow–white colour, mimicking a viral wart, a verrucous carcinoma or a squamous cell carcinoma. Our patient showed a polymorphous clinical picture with hundreds of small yellow hyperkeratotic papules that coalesced forming plaques with a linear distribution resembling a linear epidermal naevus. The typical polypoid lesions were also present in the genital area. Nevertheless, similar cases with a linear pattern have been reported rarely but these were usually associated with other local disorders such as lymphoedema, or a true epithelial naevus, while the present case arose on normal skin. Histopathologically, VX is characterized by hyperkeratosis, focal parakeratosis, uniform epithelial acanthosis without atypia and large groups of foam cells typically confined to the papillary dermis. Additional lymphocytes, plasma cells, neutrophils and a few eosinophils can be observed under and within the xanthomatous cells. Occasionally, exocytosis of neutrophils in the upper epidermis may be observed. The present case shows all the typical features of VX with no histopathological differences between the cutaneous and mucosal lesions. The aetiology of VX remains unknown. Although a few cases have been reported in association with metabolic abnormalities, most patients show no underlying disorders of lipid metabolism. Thus, our patient displayed a normal serum lipid profile. The clinical picture and growth pattern of VX suggest a possible viral aetiology, but most reports including the present one have failed to demonstrate an association with human papillomaviruses. Two pathogenetic mechanisms have been proposed for the development of VX. Zegarelli et al. suggested that injury caused by a local irritation or trauma may induce epidermal degeneration with subsequent ‘lipid incontinence’ that is scavenged by dermal histiocytes. However, other authors considered that the accumulation of foam cells is the primary event, while epithelial hyperplasia and inflammation are secondary processes. The disseminated distribution of the lesions, the appearance on normal skin and the absence of previous local irritation or trauma of the present patient supports this last theory. In summary, we present an exceptionally rare variant of ‘disseminated verruciform xanthoma’ with multiple concomitant cutaneous, oral and genital lesions which has never to our knowledge been reported previously.
