Abstract:The incidence of accessory pathways in patients with Ebstein's anomaly is high, ranging from 20% to 30%, with right and multiple pathways being more commonly encountered. Radiofrequency ablation can eliminate the accessory pathway, but success rates of ablation treatment and the chances of recurrence are generally less satisfactory as compared to those observed when the procedure is performed on structurally normal hearts. We report the case of a 56-year-old man with Ebstein's anomaly and refractory atrial tachycardia. The special abnormality of the tricuspid valve led to the failure of preoperative electrophysiological mapping and radiofrequency ablation treatment. Finally, the anomalies observed in this patient were corrected with simultaneous surgery and pathway resection.

Abstract:We report a patient with atrioventricular reentrant tachycardia (AVRT) with bidirectional conduction over an anteroseptal accessory pathway (AP) who underwent successful ablation in the non-coronary aortic sinus (AS). In three previous attempts, the intracardiac recordings showed an anteroseptal, AP with antegrade and retrograde conduction that failed to be ablated in spite of radiofrequency (RF) applications from the right and left anteroseptal regions. During the study, the earliest atrial. activation during tachycardia was recorded in the non-coronary AS preceding the atrial activation at the His bundle (HB) region by 24 ms, and the anteroseptal AP was successfully blocked by one single ablation in the non-coronary AS. These data strongly suggest that careful mapping of an anteroseptal. AP in the non-coronary AS may provide an alternative ablation approach in patients with previously failed ablation.
New-onset Heart Failure after Permanent Right Ventricular Apical Pacing in Patients with Acquired High-Grade Atrioventricular Block and Normal Left Ventricular Function.

Abstract:Introduction: Emerging data have suggested that right ventricular (RV) apical pacing results in progressive left ventricular (LV) dysfunction and contributes to the development of heart failure (HF). This study aimed to investigate the prevalence and clinical predictors for the development of new‐onset HF after long‐term RV apical pacing in patients with acquired atrioventricular (AV) block who require permanent pacing. Methods: We studied the clinical outcomes after long‐term RV apical pacing for acquired AV block in 304 patients without a prior history of HF. All patients had >90% ventricular pacing as determined by device diagnostic data. Results: After a median follow‐up of 7.8 years, 79 patients (26.0%) developed new‐onset HF after RV apical pacing. Univariate Cox‐regression analysis revealed that older age at the time of pacemaker implantation (P < 0.001), the presence of coronary artery disease (CAD) (P < 0.001) or atrial fibrillation (P = 0.03), VVI pacemaker (P < 0.001), wider paced QRS duration (P < 0.001), and new‐onset myocardial infarction (P < 0.001) were predictors for HF. Multivariate analysis revealed that older age at implantation (Hazard ratio [HR] 1.06, 95% confidential interval [CI] 1.04–1.09, P < 0.001), CAD (HR 1.98, 95% CI 1.12–3.50, P < 0.05), and a wider paced QRS duration (HR 1.27 for each 10 ms increment, 95% CI 1.11–1.45, P = 0.001) were independent predictors of HF. Furthermore, cardiovascular mortality was significantly increased in those with HF (36.7% vs. 2.7%, P < 0.001). Conclusions: After a median follow‐up of 7.8 years, permanent RV apical pacing was associated with HF in 26% of patients. Elderly age at the time of implant, a wider paced QRS duration and the presence of CAD independently predicted new‐onset HF. More importantly, HF after RV apical pacing was associated with a higher cardiovascular mortality.
Accurate localization and catheter ablation of superoparaseptal accessory pathways

Abstract:Background: There remains some confusion delineating the accurate location and the detailed anatomical relationship between atrioventricular accessory pathways (APs) located in the superoparaseptal region. Objective: The purpose of this article was to detail the anatomical relationship and accurate location of APs located in the superoparaseptal region. Methods: Between May 1, 2009 and November 30, 2016, 11 patients with superoparaseptal APs (SPS-APs) were identified in 129 consecutive patients who underwent catheter ablation for APs in our center. Results: A single SPS-AP was detected in all patients (manifest, n = 5; concealed, n = 6). The location of all 11 APs were precisely identified at the region millimeters superior to the His bundle recording site at the tricuspid annulus (S-HB, n = 6; manifest, n = 4); the area millimeters behind the His bundle recording site, adjacent to the right atrial aspect of the noncoronary aortic cusp (B-HB, n = 2; manifest, n = 1); and the true para-His bundle region (P-HB, n = 3). The electrocardiogram of all 5 manifest APs conformed to the typical "anteroseptal AP" pattern: a positive delta wave in leads I, II, avF, and avL; a narrow positive delta wave in lead V1; and a precordial QRS transition at lead V3. All APs were successfully eliminated by catheter ablation. After 54 ± 26 months of follow-up, all patients were free of arrhythmia. Conclusion: Three distinct regions are identified for localization of SPS-APs. Careful mapping and a detailed understanding of the anatomy of this region as well as distinct electrocardiographic characteristics are essential to eliminate such APs safely and effectively.

Abstract:Abstract Background Implantable cardioverter defibrillators (ICDs) are most effective in treating sudden cardiac death. However, accurate diagnostic workup of broad complex tachycardia is crucial to ensure correct indication for ICD treatment and to avoid unnecessary invasive treatment and device-associated morbidity. Case summary We present a case of atypical atrial flutter with 2:1 atrioventricular (AV) conduction via a left-posterior accessory pathway (AP), leading to the diagnosis of Wolff-Parkinson-White (WPW) syndrome. Upon admission, the 72-year-old patient showed a regular broad complex tachycardia with superior axis and positive concordance in precordial leads, suggestive of either ventricular tachycardia (VT), antidromic AV re-entrant tachycardia (AVRT), or supraventricular tachycardia with antegrade conduction via a left-posterior AP. Interrogation of the two-chamber ICD, which was very likely implanted unjustified in a peripheral clinic before, revealed atrial flutter with 2:1 AV conduction. Remarkably, after the restoration of sinus rhythm, no classic echocardiogram (ECG) criteria for preexcitation syndrome were detected. An invasive electrophysiological study proved the diagnosis of a bidirectionally conducting, left-posterior AP, which was successfully ablated. Discussion Differential diagnosis of broad complex tachycardia with superior axis and positive concordance of chest leads consists of i) VT with a left ventricular exit at the posterior mitral annulus, ii) antidromic AVRT involving a left-posterior AP, and iii) supraventricular tachycardia predominantly conducted via a left-posterior AP. The absence of classic ECG criteria for preexcitation syndrome does not rule out AP sufficiently, highlighting the importance of minimal surface-ECG preexcitation criteria. In the case of detection of minimal surface-ECG preexcitation criteria, administration of adenosine rules out or proves the existence of an AP noninvasively and cost-effectively.

[Clinical and Prognostic Analysis of Accessory Pathway-Induced Dilated Cardiomyopathy after Catheter Ablation in Children].

Abstract:Objective: To investigate the clinical characteristics and prognostic factors after catheter ablation of accessory pathway (AP)-induced dilated cardiomyopathy (DCM) in children. Methods: Data were collected and analyzed on 147 consecutive pediatric patients (81males and 66 females), who hospitalized in our pediatric heart center between January 2009 and August 2018 and received catheter ablation for ventricular pre-excitation (right AP). Thirty-one children were diagnosed as AP-induced DCM and 116 children with normal cardiac function served as control. Data including clinical characteristics, electrocardiogram (ECG), echocardiography, electrophysiological examination (EPS), successful ablation and follow up were analyzed. Results: The median age at first examination was 3.07 (0.09, 5.83) years. The pre-hospital misdiagnosis rate was 87.1% (27/31). The incidence of the AP-induced DCM was 21.1% (31/147) and the rate of right anterior free wall accessory pathway (AP) was the highest (50.0%, 10/20). AP-induced DCM was more common in right free wall Aps (41.9%, 13/31) and right anterior free wall (32.3%, 10/31). 48.4% (15/31) cases had no supraventricular tachycardia (SVT). EPS showed that 29% (9/31) of the AP did not have retrograde conduction. The median follow-up time after successful catheter ablation was 8 (2, 36) months of AP-induced DCM group, LVDd was significantly decreased ((41.6±12.8)mm vs. (45.6±13.9)mm, P<0.01) and LVEF ((56.2±11.8)% vs. (40.8±12.5)%, P<0.01) was significantly increased after ablation. Cox regression analysis showed that LVEF was the predictor of the duration of cardiac function recovery (HR=1.08, 95%CI 1.01-1.15, P=0.03). Conclusions: Misdiagnosis rate is high for children with AP-induced DCM, leading to the delayed treatment. All of the AP-induced DCM occurred in right APs and right anterior free wall APs is the highest. Right free wall APs and right anterior free wall are most common in AP-induced DCM. Catheter ablation is a safe and effective treatment option for these patients. The lower the LVEF, the longer the cardiac function recovery.
Outcomes of Different Ablation Approaches for Para-Hisian Accessory Pathway and Ablation Safety at Each Site

Abstract:Background This study describes the electrophysiologic characteristics of the para-hisian accessory pathway (AP), the outcome of different ablation approaches, and ablation safety at different sites. Method A total of 120 patients diagnosed as para-hisian AP were included in this study. The electrophysiologic characteristics and outcomes at different ablation sites were analyzed. Results In total, 107 APs and 13 APs were diagnosed as right anteroseptal (RAS) and right midseptal (RMS), respectively. The significant ECG difference between RAS and RMS was lead III, which mainly manifested as positive and negative delta waves, respectively. Catheter trauma to AP was recorded in 21 of 120 (17.5%) patients. The recurrence rate of direct ablation at the “bumped” sites was higher than the conventional ablation method (37.5 vs. 14.1 %, p = 0.036). For RAS APs, there was no significant difference in the success rate between the inferior vena cava (IVC) and superior vena cava (SVC) approaches (76.6 vs. 73.3%, p = 0.63). The RAS was separated into three regions: (1) Site 1: superior part above the real “His” recorded site with far-field “His” potential; (2) Site 2 (true para-hisian): the site with near-field “His” potential; and (3) Site 3: inferior part below the biggest real “His” with far-field “His” potential. Mid-septal was defined as an area that is bounded anteriorly by His recording location and posteriorly by the roof of coronary sinus (CS) ostium. The incidence of atrioventricular (AV) conduction injury at different sites was as follows: 3 of 6 (50%) at Site 2, 4 of 13 (30.8%) at RMS, 7 of 34 (20.6%) at Site 3, and 3 of 46 (6.5%) at Site 1. Even if ablation was performed at the atrial side of the para-hisian region, the right bundle branch block (RBBB) was caused in 6 patients (5%). Conclusion Ablation via IVC or SVC was comparative for para-hisian APs, but not for the noncoronary cusp (NCC) approach. The AV conduction injury risk ranks as follows: Site 2 > RMS > Site 3 > Site 1. RBBB could be caused while ablating at the atrial side, which could further demonstrate the His bundle longitudinal dissociation theory.

Abstract:Circulation: Arrhythmia and Electrophysiology, Ahead of Print. BACKGROUND:The anatomy of myocardial fibers around the right cardiac veins (RCVs) and their roles in accessory pathways (APs) are rarely reported.METHODS:Six RCV-APs were identified from 566 patients with right-sided APs. Mapping of retrograde atrial activation was performed using CARTO 3 system under orthodromic tachycardia or right ventricular pacing. Venography of RCVs was acquired at the earliest retrograde atrial activation.RESULTS:Patients enrolled had a median age of 30 (11–51) years, 5 of them were male. Venography of RCVs could be classified into 3 distinct patterns based on the identified ventricular branches, right marginal vein only (type I; n=3), both right marginal vein and anterior cardiac veins (type II; n=2), and anterior cardiac vein only (type III; n=1). Patients with type I venography had rS QRS pattern in lead V1, negative delta wave in lead III and negative or isoelectric delta wave in lead aVF. However, patients with type II and III venography had QS QRS patterns in lead V1and variable patterns of delta wave in inferior leads. Earliest retrograde atrial activation was found at a median of 16.75 (14.60–20.00) mm away from the tricuspid annulus, all with A larger than V. At the earliest retrograde atrial activation, far-field ventricular electrogram was found 30 ms later than QRS onset in 1 patient under sinus rhythm. AP conduction was eliminated by mechanical pressure in 2 and by radiofrequency ablation in 4 at the ostium of the veins colocalizing with the earliest retrograde activation of the right atrium. No recurrence observed during 36 (10–60) months follow-up.CONCLUSIONS:The RCV-AP is a rare form of right-sided APs characterized by atrial insertions distant from the annulus. ECG-speculated ventricular insertion sites conformed to the location of identified RCVs.

Right ventriculography to guide left bundle branch pacing in pacing-induced cardiomyopathy: a novel case report

Abstract:Background: There is emerging evidence for the potential utility of left bundle branch area pacing (LBBAP), as an alternative to conventional cardiac resynchronization therapy (CRT). The utility of right ventriculography by way of power injector to facilitate lead placement has not yet been reported in the literature. Case summary: A 79-year-old female, with a background of poorly rate-controlled atrial fibrillation, presented with worsening dyspnoea. She had recently undergone single-chamber pacemaker insertion prior to an atrioventricular nodal (AVN) ablation, owing to failure in achieving successful CRT coronary sinus lead placement. She had clinical evidence of volume overload, and her electrocardiogram demonstrated right ventricular pacing. Echocardiography demonstrated left ventricular (LV) impairment, with an ejection fraction (EF) of 35%, and severe functional mitral regurgitation (MR). Her diagnosis was overall consistent with pacing-induced cardiomyopathy (PIC). In this patient, the use of right ventriculography, using power-injector-delivered contrast, successfully facilitated placement of an LBBAP lead, with confirmation of good threshold and sensing parameters. Following an upgrade to conduction system pacing, the patient recovered well. On recent follow-up, repeat echocardiography (24 months post initial presentation) demonstrated improved LV function (EF 45% from 35%) and only mild-to-moderate MR. Discussion: In conclusion, we demonstrate the utility of right ventriculography to facilitate placement of an LBBAP lead, successfully treating a patient who developed PIC from chronic right ventricular pacing following AVN ablation.

Abstract:Abstract A dyssynchronous biventricular activation, which can be determined by left bundle branch block, chronic right ventricular pacing, frequent premature ventricular complexes, or pre-excitation, can cause a global abnormal contractility, thus leading to systolic dysfunction and left ventricular remodelling in a unique nosological entities: abnormal conduction-induced cardiomyopathies. In this clinical scenario, the mainstay therapy is eliminating or improving LV dyssynchrony, removing the trigger. This usually ensures the improvement and even recovery of cardiac geometry and left ventricular function, especially in the absence of genetic background. A multidisciplinary approach, integrating advanced multimodal imaging, is essential for the systematic aetiological definition and the subsequent evaluation and aetiology-guided therapies of patients and their families. This review aims to describe mechanisms, prevalence, risk factors, and diagnostic and therapeutic approach to the various abnormal conduction-induced cardiomyopathies, starting from reasonable certainties and then analysing the grey areas requiring further studies.

Abstract:Background: Mahaim-type accessory pathways (MAPs) are rare accessory pathways (APs) with specific properties. They are mostly located in the right side of the heart but rarely exist on the left side. Objectives: This study aims to analyze the clinical and electrophysiological (EP) characteristics of both-sided MAPs. Methods: A total of 2,249 patients with AP from our center were enrolled between 1 January 2011 and 27 March 2022. During the EP study (EPS) 17 patients were diagnosed with MAPs (right-sided: n = 13, left-sided: n = 4) according to the properties of Mahaim fibers. Results: MAPs constitute 0.75% of all APs. Out of 1,553 patients with left-sided APs, four patients (0.26%) were diagnosed with Mahaim fiber-mediated tachycardia. Out of 696 patients with right-sided APs, 13 patients (1.9%) were diagnosed with Mahaim fiber. Most Mahaim fibers were located at the free wall of the tricuspid and mitral annuli. Seven patients of right-sided MAPs were of atriofasicular type, six patients had right-sided MAPs, and all of the patients with left-sided MAPs were of atrioventricular (AV) type. The M potential only was detected in long-length MAPs. Coexistence with other supraventricular tachycardias (SVTs) was also observed both in patients with right-sided and left-sided MAPs. All the patients underwent radiofrequency ablation successfully. Only one patient had tachycardia recurrence during a follow-up. Conclusion: Although MAPs are commonly located at right sides, left sites are not impossible. The M potential contributes to the improvement of the successful ablation.

Abstract:] Objective Incomplete potential accessory pathway(IPAP) may make the resting electrocardiogram normal,but differences do exist after radiofrequency catheter ablation(RCA).The present study aims to investigate the clinical features of IPAP.Methods We retrospectively analyzed 159 cases of single accessory pathway treated successfully by RCA.The patients were divided into three groups: manifested pre-excitation(n=46),IPAP(n=32),and concealed accessory pathway(n=81).Comparisons were made in the electrocardiographic parameters of different groups before and after RCA,with analysis of the locations of accessory pathway.Results Before RCA,the PR intervals of the manifested pre-excitation group were shorter,while the QRS durations and QTc intervals were longer than those of the IPAP and concealed accessory pathway groups,with no significant differences in PR intervals,QRS durations and QTc intervals between the latter two groups.The IPAP group showed increased PR intervals,reduced QRS durations and similar QTc intervals and heart rate after RCA.The proportion of the right atrioventricular accessory pathway was relatively larger in the manifested pre-excitation group,and that of the left obviously higher in the IPAP group.Conlusion IPAP is not rare clinically,which is often located in the left atrioventricular accessory pathway,and tachycardia frequently occurs as orthodromic atrioventricular reentry tachycardia.Although the electrocardiographic parameters of these patients are normal before RCA,changes do occur in the vector of QRS after it.

Abstract:Objective To evaluate the ratio of patients excluding organic heart diseases who have atypical manifestation in surface electrocardiogram(ECG) of left accessory pathway(AP) which have antegrade conduction and the cause of this phenomenon was analyzed simultaneously to help to understand the mechanism and locate the AP Position.Methods Forty-six patients with antegrade conductive AP admitted from Jan.2007 to Nov.2009 underwent radiofrequency catheter ablation(RFCA) were analyzed retrospectively.The ratio of the atypical ECG manifested as downward main wave of QRS complex of lead V1 in patients(atypical AP group) of left accessory pathway(AP) which have antegrade conduction before ablation was calculated.The AH intervals of sinus rhythm were collected.The earliest origin of P wave in ECG to A wave of orifice of coronary sinus(PA intervals) were collected as the duration of sinoatrial node to left atrium.The distance of AP to orifice of coronary sinus(CSo)(DAP-CSo) was gathered also.Two tailed t test was used to compare all the variations.Results The ratio of R<S of lead V1 was 19.6% in left antegrade conduction AP.The difference of PR after ablation and AH were significant between two groups while no difference was observed in PR before ablation between two groups.The PA was not prolonged in atypical AP group.There was no significant difference of DAP-CSo between the two groups.There was no significant difference between two groups.Conclusions There was 19.6% patients with left antegrade conduction AP was atypical in ECG.The better conduction of atrioventricular node was the important cause of less ventricular excitation originated from AP and R(r)<S(s) in V1 in these patients.

Abstract:A 53-year-old male patient with dextrocardia and situs viscera inversus, a congenitally corrected transposition of the great arteries (c-TGA), and atrial septum defect (ASD), secundum type, post patch repair was referred to our laboratory for treatment of frequent episodes of arrhythmia. In the previous fourteen years, the patient had experienced multiple supraventricular tachycardias (SVTs), which were successfully ablated 14, 12, and 2 years prior, respectively. This time in the electrophysiology laboratory, a narrow QRS tachycardia (cycle length=380 msec) was induced by right ventricular and right atrial incremental pacing with concentric activation. A bi-atrial activation map was constructed using a NavX system (St. Jude Medical, St. Paul, MN, USA) via a transseptal procedure to approach the systemic left atrium, which showed a centrifugal activation from a focal origin with mapped cycle length 121 ms (32% of the tachycardia cycle length) on the systemic high left atrial septum where a large low voltage zone was observed. The bipolar electrograms at the origin of the focal atrial tachycardia (AT) demonstrated fractionated and low-amplitude potentials. Successful ablation was carried out at the AT origin resulting in the non-inducibility of the AT. No recurrence of the tachycardia was observed during one year of follow-up.

Abstract:Aberrant conduction during orthodromic reciprocating tachycardia (ORT) can prolong the ventriculo-atrial conduction time, which is found essential for the reentry in certain situations. To investigate the prevalence and different mechanism of aberrancy-dependent ORT. ORT cases relying on aberrancy from 220 cases in our center were searched using the following criteria: (1) Multiple episodes of ORT with aberrant conduction in ECG ; (2) No baseline BBB during sinus rhythm; (3) Absence of narrow QRS form during tachycardia unless isoproterenol used; (4) Restoration of bundle branch conduction giving rise to tachycardia termination or circuit alteration. ECGs and intracardiac tracings during study were carefully examined. 3 patients from 220 patients (1.4%) with ORT showed the phenomenon of dependence on aberrancy. Clinical tachycardia episodes of the patients were short in time. All accessory pathways (APs) were located at anterolateral regions of atrioventricular annulus. None of them had baseline bundle branch block. Creating functional bundle branch block was necessary to induce the tachycardias. In the first patient with ORT and left bundle branch block (LBBB), a late-delivered premature ventricular contraction (PVC) was able to advance the next A and reset the tachycardia, which was diagnostic for ORT with aberrancy. Meanwhile, left bundle branch conduction was restored which narrowed the next QRS complex. Tachycardia was then terminated by the advanced wavefront reaching the ventricular insertion site of AP which was refractory. In the second case, termination of the tachycardia was always preceded by a spontaneous LBBB restoration, with the V and subsequent A at MA advanced, suggesting anterograde block in the AV node. In the other case was, induction required both right BBB and slow pathway conduction. When isoproterenol was given, fast pathway recovered followed by BBB restoration, which made anterograde limb again switched to slow pathway. All pathways were successfully ablated during the procedure. All patients have been free of arrhythmias for at least 6 months. Extra transseptal time caused by aberrancy can be an integral part of ORT, which can explain the infrequent or late onset and unsustainable episodes of ORT in certain patients and is useful in understanding the circuit and localizing the pathway.
Role of oxidative mixed-disulfide formation in elastase-serine proteinase inhibitor (serpin) complex.

Abstract:To understand the role of thiol and oxidative mixed-disulfide exchange reaction in serpins, we analyzed the conformation of native and mixed-disulfide forms of alpha 1-proteinase inhibitor (alpha 1-PI), alpha 1-antichymotrypsin (alpha 1-ACT), alpha 2-antiplasmin (alpha 2-AP), angiotensinogen, and ovalbumin. The conformation of native and oxidized mixed-disulfide serpins was measured by transverse urea gradient (TUG) gels. The results suggest that the acute phase proteins alpha 1-PI and alpha 1-ACT undergo conformational changes following oxidative mixed-disulfide formation and that alpha 2-AP and angiotensinogen do not. The kinetics of disulfide formation was followed by measuring changes in absorbance at 412 nm resulting from Ellman's reaction of disulfide exchange. The rate of mixed-disulfide formation in albumin was 10-fold faster than in the serpin tested. The rate of disulfide exchange in alpha 1-PI was 2-fold faster than that of alpha 1-ACT. However, disulfide formation in alpha 1-PI and alpha 1-ACT was much slower than for any other serpin, e.g., alpha 2-AP and angiotensinogen. We present evidence that alpha 1-PI forms a dimer sensitive to thiol reduction, suggesting cysteinyl-mediated dimerization of alpha 1-PI. The alpha 1-PI also demonstrated two types of inter-protein disulfide linkages: one resulting in homodimer and other involving heterodimer formation. TUG-Western immunoblot methodology was developed to identify the conformational changes in serpins. We found that the conformational changes in serpins by mixed-disulfide formation are due to unfolding and not to decomposition or degradation in TUG gels. Using fluorescence measurements with isolated tryptic fragments of fluorescence-labelled elastase, we observed that the cysteinyl232 in alpha 1-PI interacted with the cysteinyl168 of elastase in the proteinase-inhibitor complex. Our data suggests that serpin thiols may play an important role in forming a stable serpin-proteinase complex.
Case Report: Pulsed field ablation for epicardial right-sided accessory pathway

Abstract:We present a case of a 32-year-old male with a history of palpitations and preexcitation on ECG who underwent altogether four failed catheter ablations using different approaches in the two other electrophysiology centers within two years. ECG showed overt preexcitation with a positive delta wave in lead I and negative in leads V1–V3, suggesting a right free wall accessory pathway. During the electrophysiological study, the accessory pathway was localized on the free lateral wall. However, the electrograms and mapping during atrial and ventricular pacing suggested the presence of true epicardial accessory pathway. Repeated radiofrequency energy delivery with the support of the steerable sheath and excellent contact (as assessed by intracardiac echocardiography) at the earliest ventricular activation was not successful. Therefore, the Farawave catheter (Boston Scientific, Inc) was used, and a flower configuration with the intention to cover the entire atrial attachment of the pathway during ventricular pacing was selected. Application of pulsed field resulted in interruption of accessory pathway conduction. An electrophysiological study one year later confirmed the persistent effect of ablation. This case illustrates the potential utility of pulsed field energy for the ablation of atrial insertion of the accessory pathway with an epicardial course. Such an approach can avoid epicardial mapping and access and may improve the safety of the procedure.

Abstract:A 21 year-old college athlete presented with palpitations during exercise. ECG and transthoracic echo were normal. He underwent electrophysiology study. With catheter placement, the patient had atrial fibrillation without preexcitation. On isoproterenol, spontaneous salvos of supraventricular tachycardia (SVT) occurred. Coronary sinus activation was concentric. SVT never sustained, limiting diagnostic maneuvers. In SVT, the ventriculoatrial (VA) time was 145 msec, and there was no decrement of VA conduction with ventricular extrastimuli. Para-Hisian pacing (PHP) was performed. At high output, local myocardial + Purkinje (LM+P) capture was observed, with a VA time of 130 msec; at lower output, selective Purkinje (SP) capture was observed, with a narrower QRS (Figure 1A), and paradoxically longer VA time (160 msec) (Figure 1B). The diagnosis of a retrograde conducting-only accessory pathway (AP) was made, mapped with ventricular pacing, and ablated on the posteroseptum. SVT was no longer inducible. Discussion: PHP is employed to identify presence of a septal retrograde conducting AP based on findings of equal VA times with LM and LM+P capture. Here, the traditional findings of PHP (LM and LM+P capture) were absent because LM capture did not occur, and a third, distinct manner of activating the ventricle (via SP capture) produced unexpected results. In the presence of a retrograde conducting AP, the VA time with narrow-QRS by way of SP capture (sequential activation of the Purkinje system then myocardial conduction) was paradoxically longer than that of wider-QRS LM+P capture (concurrent Purkinje and myocardial conduction), because the path of VA conduction via the AP is “closer” to the site of first ventricular activation. The comparison of SP to LM+P capture may in and of itself declare the presence of an AP; however, LM, LM+P, and SP capture together may be useful to identify a second concealed AP providing incremental diagnostic information to conventional PHP.
