Abstract:Introduction: Early detection of pancreatic cancer continues to be a challenge due to the difficulty in accurately identifying specific signs or symptoms that might correlate with the onset of pancreatic cancer. Unlike breast or colon or prostate cancer where screening tests are often useful in identifying cancerous development, there are no tests to diagnose pancreatic cancers. As a result, most pancreatic cancers are diagnosed at an advanced stage, where treatment options, whether systemic therapy, radiation, or surgical interventions, offer limited efficacy. Methods: A two-stage weakly supervised deep learning-based model has been proposed to identify pancreatic tumors using computed tomography (CT) images from Henry Ford Health (HFH) and publicly available Memorial Sloan Kettering Cancer Center (MSKCC) data sets. In the first stage, the nnU-Net supervised segmentation model was used to crop an area in the location of the pancreas, which was trained on the MSKCC repository of 281 patient image sets with established pancreatic tumors. In the second stage, a multi-instance learning-based weakly supervised classification model was applied on the cropped pancreas region to segregate pancreatic tumors from normal appearing pancreas. The model was trained, tested, and validated on images obtained from an HFH repository with 463 cases and 2,882 controls. Results: The proposed deep learning model, the two-stage architecture, offers an accuracy of 0.907 ± 0.01, sensitivity of 0.905 ± 0.01, specificity of 0.908 ± 0.02, and AUC (ROC) 0.903 ± 0.01. The two-stage framework can automatically differentiate pancreatic tumor from non-tumor pancreas with improved accuracy on the HFH dataset. Discussion: The proposed two-stage deep learning architecture shows significantly enhanced performance for predicting the presence of a tumor in the pancreas using CT images compared with other reported studies in the literature.

Abstract:Deep learning-based computer-aided diagnosis techniques have demonstrated encouraging performance in endoscopic lesion identification and detection, and have reduced the rate of missed and false detections of disease during endoscopy. However, the interpretability of the model-based results has not been adequately addressed by existing methods. This phenomenon is directly manifested by a significant bias in the representation of feature localization. Good recognition models experience severe feature localization errors, particularly for lesions with subtle morphological features, and such unsatisfactory performance hinders the clinical deployment of models. To effectively alleviate this problem, we proposed a solution to optimize the localization bias in feature representations of cancer-related recognition models that is difficult to accurately label and identify in clinical practice. Optimization was performed in the training phase of the model through the proposed data augmentation method and auxiliary loss function based on clinical priors. The data augmentation method, called partial jigsaw, can “break” the spatial structure of lesion-independent image blocks and enrich the data feature space to decouple the interference of background features on the space and focus on fine-grained lesion features. The annotation-based auxiliary loss function used class activation maps for sample distribution correction and led the model to present localization representation converging on the gold standard annotation of visualization maps. The results show that with the improvement of our method, the precision of model recognition reached an average of 92.79%, an F1-score of 92.61%, and accuracy of 95.56% based on a dataset constructed from 23 hospitals. In addition, we quantified the evaluation representation of visualization feature maps. The improved model yielded significant offset correction results for visualized feature maps compared with the baseline model. The average visualization-weighted positive coverage improved from 51.85% to 83.76%. The proposed approach did not change the deployment capability and inference speed of the original model and can be incorporated into any state-of-the-art neural network. It also shows the potential to provide more accurate localization inference results and assist in clinical examinations during endoscopies.

Abstract:Pancreatic cancer is one of the most malignant tumours, demonstrating a poor prognosis and nearly identically high mortality and morbidity, mainly because of the difficulty of early diagnosis and timely treatment for localized stages. It is clinically important to develop a noncontrast CT (NCCT)-based pancreatic lesion detection model that could serve as an intelligent tool for diagnosing pancreatic cancer early. However, abdominal NCCT presents low contrast intensities for the pancreas and its lesions and complex anatomical structures, developing such a model is challenging. Therefore, we design a multiscale and multiperception (MSMP) feature learning network with ResNet50 coupled with a feature pyramid network (FPN) as the backbone for strengthening feature expressions. We added multiscale atrous convolutions to expand different receptive fields, contextual attention to perceive contextual information, and channel and spatial attention to focus on important channels and spatial regions, respectively. The MSMP network then acts as a feature extractor for proposing an NCCT-based pancreatic lesion detection model with image patches covering the pancreas as its input; Faster R-CNN is employed as the detection method for accurately detecting pancreatic lesions. By using the new MSMP network as a feature extractor, our model outperforms the conventional object detection algorithms in terms of the recall (75.40% and 90.95%), precision (40.84% and 68.21%), F1 score (52.98% and 77.96%), F2 score (64.48% and 85.26%) and Ap50 (53.53% and 70.14%) at the image and patient levels, respectively. The good performance of our new model implies that MSMP can mine NCCT imaging features for detecting pancreatic lesions from complex backgrounds well. The proposed detection model is expected to be developed as an intelligent method for the early detection of pancreatic cancer.

Large-scale pancreatic cancer detection via non-contrast CT and deep learning

Abstract:Pancreatic ductal adenocarcinoma (PDAC), the most deadly solid malignancy, is typically detected late and at an inoperable stage. Early or incidental detection is associated with prolonged survival, but screening asymptomatic individuals for PDAC using a single test remains unfeasible due to the low prevalence and potential harms of false positives. Non-contrast computed tomography (CT), routinely performed for clinical indications, offers the potential for large-scale screening, however, identification of PDAC using non-contrast CT has long been considered impossible. Here, we develop a deep learning approach, pancreatic cancer detection with artificial intelligence (PANDA), that can detect and classify pancreatic lesions with high accuracy via non-contrast CT. PANDA is trained on a dataset of 3,208 patients from a single center. PANDA achieves an area under the receiver operating characteristic curve (AUC) of 0.986–0.996 for lesion detection in a multicenter validation involving 6,239 patients across 10 centers, outperforms the mean radiologist performance by 34.1% in sensitivity and 6.3% in specificity for PDAC identification, and achieves a sensitivity of 92.9% and specificity of 99.9% for lesion detection in a real-world multi-scenario validation consisting of 20,530 consecutive patients. Notably, PANDA utilized with non-contrast CT shows non-inferiority to radiology reports (using contrast-enhanced CT) in the differentiation of common pancreatic lesion subtypes. PANDA could potentially serve as a new tool for large-scale pancreatic cancer screening.

Abstract:Artificial intelligence can facilitate clinical decision making by considering massive amounts of medical imaging data. Various algorithms have been implemented for different clinical applications. Accurate diagnosis and treatment require reliable and interpretable data. For pancreatic tumor diagnosis, only 58.5% of images from the First Affiliated Hospital and the Second Affiliated Hospital, Zhejiang University School of Medicine are used, increasing labor and time costs to manually filter out images not directly used by the diagnostic model.Methods: This study used a training dataset of 143,945 dynamic contrast-enhanced CT images of the abdomen from 319 patients. The proposed model contained four stages: image screening, pancreas location, pancreas segmentation, and pancreatic tumor diagnosis.Results: We established a fully end-to-end deep-learning model for diagnosing pancreatic tumors and proposing treatment. The model considers original abdominal CT images without any manual preprocessing. Our artificial-intelligence-based system achieved an area under the curve of 0.871 and a F1 score of 88.5% using an independent testing dataset containing 107,036 clinical CT images from 347 patients. The average accuracy for all tumor types was 82.7%, and the independent accuracies of identifying intraductal papillary mucinous neoplasm and pancreatic ductal adenocarcinoma were 100% and 87.6%, respectively. The average test time per patient was 18.6 s, compared with at least 8 min for manual reviewing. Furthermore, the model provided a transparent and interpretable diagnosis by producing saliency maps highlighting the regions relevant to its decision.Conclusions: The proposed model can potentially deliver efficient and accurate preoperative diagnoses that could aid the surgical management of pancreatic tumor.

Abstract:Pancreatic cancer is one of the leading causes of cancer-related death. Accurate detection, segmentation, and differential diagnosis of the full taxonomy of pancreatic lesions, i.e., normal, seven major types of lesions, and other lesions, is critical to aid the clinical decision-making of patient management and treatment. However, existing works focus on segmentation and classification for very specific lesion types (PDAC) or groups. Moreover, none of the previous work considers using lesion prevalence-related non-imaging patient information to assist the differential diagnosis. To this end, we develop a meta-information-aware dual-path transformer and exploit the feasibility of classification and segmentation of the full taxonomy of pancreatic lesions. Specifically, the proposed method consists of a CNN-based segmentation path (S-path) and a transformer-based classification path (C-path). The S-path focuses on initial feature extraction by semantic segmentation using a UNet-based network. The C-path utilizes both the extracted features and meta-information for patient-level classification based on stacks of dual-path transformer blocks that enhance the modeling of global contextual information. A large-scale multi-phase CT dataset of 3,096 patients with pathology-confirmed pancreatic lesion class labels, voxel-wise manual annotations of lesions from radiologists, and patient meta-information, was collected for training and evaluations. Our results show that our method can enable accurate classification and segmentation of the full taxonomy of pancreatic lesions, approaching the accuracy of the radiologist's report and significantly outperforming previous baselines. Results also show that adding the common meta-information, i.e., gender and age, can boost the model's performance, thus demonstrating the importance of meta-information for aiding pancreatic disease diagnosis.

Robust Pancreatic Ductal Adenocarcinoma Segmentation with Multi-institutional Multi-phase Partially-Annotated CT Scans

Abstract:Accurate and automated tumor segmentation is highly desired since it has the great potential to increase the efficiency and reproducibility of computing more complete tumor measurements and imaging biomarkers, comparing to (often partial) human measurements. This is probably the only viable means to enable the large-scale clinical oncology patient studies that utilize medical imaging. Deep learning approaches have shown robust segmentation performances for certain types of tumors, e.g., brain tumors in MRI imaging, when a training dataset with plenty of pixel-level fully-annotated tumor images is available. However, more than often, we are facing the challenge that only (very) limited annotations are feasible to acquire, especially for hard tumors. Pancreatic ductal adenocarcinoma (PDAC) segmentation is one of the most challenging tumor segmentation tasks, yet critically important for clinical needs. Previous work on PDAC segmentation is limited to the moderate amounts of annotated patient images (n < 300) from venous or venous+arterial phase CT scans. Based on a new self-learning framework, we propose to train the PDAC segmentation model using a much larger quantity of patients (), with a mix of annotated and un-annotated venous or multi-phase CT images. Pseudo annotations are generated by combining two teacher models with different PDAC segmentation specialties on unannotated images, and can be further refined by a teaching assistant model that identifies associated vessels around the pancreas. A student model is trained on both manual and pseudo annotated multi-phase images. Experiment results show that our proposed method provides an absolute improvement of 6.3% Dice score over the strong baseline of nnUNet trained on annotated images, achieving the performance (Dice = 0.71) similar to the inter-observer variability between radiologists.

Abstract:BACKGROUND: Automated diagnosis of gastrointestinal stromal tumors' (GISTs) cancerization is an effective way to improve the clinical diagnostic accuracy and reduce possible risks of biopsy. Although deep convolutional neural networks (DCNNs) have proven to be very effective in many image classification problems, there is still a lack of studies on endoscopic ultrasound (EUS) images of GISTs. It remains a substantial challenge mainly due to the data distribution bias of multi-center images, the significant inter-class similarity and intra-class variation, and the insufficiency of training data. OBJECTIVE: The study aims to classify GISTs into higher-risk and lower-risk categories. METHODS: Firstly, a novel multi-scale image normalization block is designed to perform same-size and same-resolution resizing on the input data in a parallel manner. A dilated mask is used to obtain a more accurate interested region. Then, we construct a multi-way feature extraction and fusion block to extract distinguishable features. A ResNet-50 model built based on transfer learning is utilized as a powerful feature extractor for tumors' textural features. The tumor size features and the patient demographic features are also extracted respectively. Finally, a robust XGBoost classifier is trained on all features. RESULTS: Experimental results show that our proposed method achieves the AUC score of 0.844, which is superior to the clinical diagnosis performance. CONCLUSIONS: Therefore, the results have provided a solid baseline to encourage further researches in this field.
Generalizable Pancreas Segmentation via a Dual Self-Supervised Learning Framework

Abstract:Recently, numerous pancreas segmentation methods have achieved promising performance on local single-source datasets. However, these methods don't adequately account for generalizability issues, and hence typically show limited performance and low stability on test data from other sources. Considering the limited availability of distinct data sources, we seek to improve the generalization performance of a pancreas segmentation model trained with a single-source dataset, i.e., the single-source generalization task. In particular, we propose a dual self-supervised learning model that incorporates both global and local anatomical contexts. Our model aims to fully exploit the anatomical features of the intra-pancreatic and extra-pancreatic regions, and hence enhance the characterization of the high-uncertainty regions for more robust generalization. Specifically, we first construct a global-feature contrastive self-supervised learning module that is guided by the pancreatic spatial structure. This module obtains complete and consistent pancreatic features through promoting intra-class cohesion, and also extracts more discriminative features for differentiating between pancreatic and non-pancreatic tissues through maximizing inter-class separation. It mitigates the influence of surrounding tissue on the segmentation outcomes in high-uncertainty regions. Subsequently, a local-image-restoration self-supervised learning module is introduced to further enhance the characterization of the high-uncertainty regions. In this module, informative anatomical contexts are actually learned to recover randomly-corrupted appearance patterns in those regions. The effectiveness of our method is demonstrated with state-of-the-art performance and comprehensive ablation analysis on three pancreas datasets (467 cases). The results demonstrate a great potential in providing a stable support for the diagnosis and treatment of pancreatic diseases.

Early Detection and Localization of Pancreatic Cancer by Label-Free Tumor Synthesis

Abstract:Early detection and localization of pancreatic cancer can increase the 5-year survival rate for patients from 8.5% to 20%. Artificial intelligence (AI) can potentially assist radiologists in detecting pancreatic tumors at an early stage. Training AI models require a vast number of annotated examples, but the availability of CT scans obtaining early-stage tumors is constrained. This is because early-stage tumors may not cause any symptoms, which can delay detection, and the tumors are relatively small and may be almost invisible to human eyes on CT scans. To address this issue, we develop a tumor synthesis method that can synthesize enormous examples of small pancreatic tumors in the healthy pancreas without the need for manual annotation. Our experiments demonstrate that the overall detection rate of pancreatic tumors, measured by Sensitivity and Specificity, achieved by AI trained on synthetic tumors is comparable to that of real tumors. More importantly, our method shows a much higher detection rate for small tumors. We further investigate the per-voxel segmentation performance of pancreatic tumors if AI is trained on a combination of CT scans with synthetic tumors and CT scans with annotated large tumors at an advanced stage. Finally, we show that synthetic tumors improve AI generalizability in tumor detection and localization when processing CT scans from different hospitals. Overall, our proposed tumor synthesis method has immense potential to improve the early detection of pancreatic cancer, leading to better patient outcomes.

Generalizable Pancreas Segmentation Modeling in CT Imaging via Meta-Learning and Latent-Space Feature Flow Generation

Abstract:Accurate pancreas segmentation is highly crucial for diagnosing and treating pancreatic diseases. Although CNN has demonstrated promising outcomes, the performance on unseen data can be significantly compromised by the wide appearance-style variations induced by different imaging factors. Thus, we propose a generalizable pancreas segmentation model based on a meta-learning strategy and latent-space feature flow generation method. Our approach enhances the generalizability by systematically reducing the interference from the cluttered background and appearance-style discrepancies through a coarse-to-fine workflow. Specifically, the integrity-preserving coarse segmentation module is designed to adaptively balance the pancreas coverage and segmentation accuracy with the meta-learning strategy for filtering out background clutter. It also enhances the generalization of the coarse model to reasonably-accurate ROIs thereby promoting the stability of fine segmentation. Subsequently, the appearance-style feature flow generation method is developed to generate a series of progressively-varying style-related intermediate representations between two latent spaces. This feature flow effectively models the distribution variations caused by appearance-style discrepancies, and thus enhances the adaptability of the fine model. Our method achieves superior performance on three pancreas datasets and outperforms state-of-the-art generalization methods. Besides, it can be easily integrated into other workflows, leading to a potential paradigm for enhancing generalization performance.

Joint Shape Representation and Classification for Detecting PDAC

Abstract:We aim to detect pancreatic ductal adenocarcinoma (PDAC) in abdominal CT scans, which sheds light on early diagnosis of pancreatic cancer. This is a 3D volume classification task with little training data. We propose a two-stage framework, which first segments the pancreas into a binary mask, then compresses the mask into a shape vector and performs abnormality classification. Shape representation and classification are performed in a joint manner, both to exploit the knowledge that PDAC often changes the shape of the pancreas and to prevent over-fitting. Experiments are performed on 300 normal scans and 136 PDAC cases. We achieve a specificity of 90.2% (false alarm occurs on less than 1/10 normal cases) at a sensitivity of 80.2% (less than 1/5 PDAC cases are not detected), which show promise for clinical applications.

Abstract:Efforts should be made to develop a deep-learning diagnosis system to distinguish pancreatic cancer from benign tissue due to the high morbidity of pancreatic cancer.To identify pancreatic cancer in computed tomography (CT) images automatically by constructing a convolutional neural network (CNN) classifier.A CNN model was constructed using a dataset of 3494 CT images obtained from 222 patients with pathologically confirmed pancreatic cancer and 3751 CT images from 190 patients with normal pancreas from June 2017 to June 2018. We established three datasets from these images according to the image phases, evaluated the approach in terms of binary classification (., cancer or not) and ternary classification (., no cancer, cancer at tail/body, cancer at head/neck of the pancreas) using 10-fold cross validation, and measured the effectiveness of the model with regard to the accuracy, sensitivity, and specificity.The overall diagnostic accuracy of the trained binary classifier was 95.47%, 95.76%, 95.15% on the plain scan, arterial phase, and venous phase, respectively. The sensitivity was 91.58%, 94.08%, 92.28% on three phases, with no significant differences ( = 0.914, = 0.633). Considering that the plain phase had same sensitivity, easier access, and lower radiation compared with arterial phase and venous phase , it is more sufficient for the binary classifier. Its accuracy on plain scans was 95.47%, sensitivity was 91.58%, and specificity was 98.27%. The CNN and board-certified gastroenterologists achieved higher accuracies than trainees on plain scan diagnosis ( = 21.534, < 0.001; = 9.524, < 0.05; respectively). However, the difference between CNN and gastroenterologists was not significant ( = 0.759, = 0.384). In the trained ternary classifier, the overall diagnostic accuracy of the ternary classifier CNN was 82.06%, 79.06%, and 78.80% on plain phase, arterial phase, and venous phase, respectively. The sensitivity scores for detecting cancers in the tail were 52.51%, 41.10% and, 36.03%, while sensitivity for cancers in the head was 46.21%, 85.24% and 72.87% on three phases, respectively. Difference in sensitivity for cancers in the head among the three phases was significant ( = 16.651, < 0.001), with arterial phase having the highest sensitivity.We proposed a deep learning-based pancreatic cancer classifier trained on medium-sized datasets of CT images. It was suitable for screening purposes in pancreatic cancer detection.

Abstract:Pancreatic cancer is associated with higher mortality rates due to insufficient diagnosis techniques, often diagnosed at an advanced stage when effective treatment is no longer possible. Therefore, automated systems that can detect cancer early are crucial to improve diagnosis and treatment outcomes. In the medical field, several algorithms have been put into use. Valid and interpretable data are essential for effective diagnosis and therapy. There is much room for cutting-edge computer systems to develop. The main objective of this research is to predict pancreatic cancer early using deep learning and metaheuristic techniques. This research aims to create a deep learning and metaheuristic techniques-based system to predict pancreatic cancer early by analyzing medical imaging data, mainly CT scans, and identifying vital features and cancerous growths in the pancreas using Convolutional Neural Network (CNN) and YOLO model-based CNN (YCNN) models. Once diagnosed, the disease cannot be effectively treated, and its progression is unpredictable. That's why there's been a push in recent years to implement fully automated systems that can sense cancer at a prior stage and improve diagnosis and treatment. The paper aims to evaluate the effectiveness of the novel YCNN approach compared to other modern methods in predicting pancreatic cancer. To predict the vital features from the CT scan and the proportion of cancer feasts in the pancreas using the threshold parameters booked as markers. This paper employs a deep learning approach called a Convolutional Neural network (CNN) model to predict pancreatic cancer images. In addition, we use the YOLO model-based CNN (YCNN) to aid in the categorization process. Both biomarkers and CT image dataset is used for testing. The YCNN method was shown to perform well by a cent percent of accuracy compared to other modern techniques in a thorough review of comparative findings.

Improved Pancreatic Tumor Detection by Utilizing Clinically-Relevant Secondary Features

Abstract:Pancreatic cancer is one of the global leading causes of cancer-related deaths. Despite the success of Deep Learning in computer-aided diagnosis and detection (CAD) methods, little attention has been paid to the detection of Pancreatic Cancer. We propose a method for detecting pancreatic tumor that utilizes clinically-relevant features in the surrounding anatomical structures, thereby better aiming to exploit the radiologist's knowledge compared to other, conventional deep learning approaches. To this end, we collect a new dataset consisting of 99 cases with pancreatic ductal adenocarcinoma (PDAC) and 97 control cases without any pancreatic tumor. Due to the growth pattern of pancreatic cancer, the tumor may not be always visible as a hypodense lesion, therefore experts refer to the visibility of secondary external features that may indicate the presence of the tumor. We propose a method based on a U-Net-like Deep CNN that exploits the following external secondary features: the pancreatic duct, common bile duct and the pancreas, along with a processed CT scan. Using these features, the model segments the pancreatic tumor if it is present. This segmentation for classification and localization approach achieves a performance of 99% sensitivity (one case missed) and 99% specificity, which realizes a 5% increase in sensitivity over the previous state-of-the-art method. The model additionally provides location information with reasonable accuracy and a shorter inference time compared to previous PDAC detection methods. These results offer a significant performance improvement and highlight the importance of incorporating the knowledge of the clinical expert when developing novel CAD methods.

Multi-scale Coarse-to-Fine Segmentation for Screening Pancreatic Ductal Adenocarcinoma

Abstract:We propose an intuitive approach of detecting pancreatic ductal adenocarcinoma (PDAC), the most common type of pancreatic cancer, by checking abdominal CT scans. Our idea is named multi-scale segmentation-for-classification, which classifies volumes by checking if at least a sufficient number of voxels is segmented as tumors, by which we can provide radiologists with tumor locations. In order to deal with tumors with different scales, we train and test our volumetric segmentation networks with multi-scale inputs in a coarse-to-fine flowchart. A post-processing module is used to filter out outliers and reduce false alarms. We collect a new dataset containing 439 CT scans, in which 136 cases were diagnosed with PDAC and 303 cases are normal, which is the largest set for PDAC tumors to the best of our knowledge. To offer the best trade-off between sensitivity and specificity, our proposed framework reports a sensitivity of 94.1%$$94.1\%$$ at a specificity of 98.5%$$98.5\%$$, which demonstrates the potential to make a clinical impact.

Abstract:As Deep Convolutional Neural Networks (DCNNs) have shown robust performance and results in medical image analysis, a number of deep-learning-based tumor detection methods were developed in recent years. Nowadays, the automatic detection of pancreatic tumors using contrast-enhanced Computed Tomography (CT) is widely applied for the diagnosis and staging of pancreatic cancer. Traditional hand-crafted methods only extract low-level features. Normal convolutional neural networks, however, fail to make full use of effective context information, which causes inferior detection results. In this paper, a novel and efficient pancreatic tumor detection framework aiming at fully exploiting the context information at multiple scales is designed. More specifically, the contribution of the proposed method mainly consists of three components: Augmented Feature Pyramid networks, Self-adaptive Feature Fusion and a Dependencies Computation (DC) Module. A bottom-up path augmentation to fully extract and propagate low-level accurate localization information is established firstly. Then, the Self-adaptive Feature Fusion can encode much richer context information at multiple scales based on the proposed regions. Finally, the DC Module is specifically designed to capture the interaction information between proposals and surrounding tissues. Experimental results achieve competitive performance in detection with the AUC of 0.9455, which outperforms other state-of-the-art methods to our best of knowledge, demonstrating the proposed framework can detect the tumor of pancreatic cancer efficiently and accurately.

Annotation-Efficient Deep Learning Model for Pancreatic Cancer Diagnosis and Classification Using CT Images: A Retrospective Diagnostic Study

Abstract:The aim of this study was to develop a novel deep learning (DL) model without requiring large-annotated training datasets for detecting pancreatic cancer (PC) using computed tomography (CT) images. This retrospective diagnostic study was conducted using CT images collected from 2004 and 2019 from 4287 patients diagnosed with PC. We proposed a self-supervised learning algorithm (pseudo-lesion segmentation (PS)) for PC classification, which was trained with and without PS and validated on randomly divided training and validation sets. We further performed cross-racial external validation using open-access CT images from 361 patients. For internal validation, the accuracy and sensitivity for PC classification were 94.3% (92.8–95.4%) and 92.5% (90.0–94.4%), and 95.7% (94.5–96.7%) and 99.3 (98.4–99.7%) for the convolutional neural network (CNN) and transformer-based DL models (both with PS), respectively. Implementing PS on a small-sized training dataset (randomly sampled 10%) increased accuracy by 20.5% and sensitivity by 37.0%. For external validation, the accuracy and sensitivity were 82.5% (78.3–86.1%) and 81.7% (77.3–85.4%) and 87.8% (84.0–90.8%) and 86.5% (82.3–89.8%) for the CNN and transformer-based DL models (both with PS), respectively. PS self-supervised learning can increase DL-based PC classification performance, reliability, and robustness of the model for unseen, and even small, datasets. The proposed DL model is potentially useful for PC diagnosis.

Abstract:Background & aims: The aims of our case-control study were (1) to develop an automated 3-dimensional (3D) Convolutional Neural Network (CNN) for detection of pancreatic ductal adenocarcinoma (PDA) on diagnostic computed tomography scans (CTs), (2) evaluate its generalizability on multi-institutional public data sets, (3) its utility as a potential screening tool using a simulated cohort with high pretest probability, and (4) its ability to detect visually occult preinvasive cancer on prediagnostic CTs. Methods: A 3D-CNN classification system was trained using algorithmically generated bounding boxes and pancreatic masks on a curated data set of 696 portal phase diagnostic CTs with PDA and 1080 control images with a nonneoplastic pancreas. The model was evaluated on (1) an intramural hold-out test subset (409 CTs with PDA, 829 controls); (2) a simulated cohort with a case-control distribution that matched the risk of PDA in glycemically defined new-onset diabetes, and Enriching New-Onset Diabetes for Pancreatic Cancer score ≥3; (3) multi-institutional public data sets (194 CTs with PDA, 80 controls), and (4) a cohort of 100 prediagnostic CTs (i.e., CTs incidentally acquired 3-36 months before clinical diagnosis of PDA) without a focal mass, and 134 controls. Results: Of the CTs in the intramural test subset, 798 (64%) were from other hospitals. The model correctly classified 360 CTs (88%) with PDA and 783 control CTs (94%), with a mean accuracy 0.92 (95% CI, 0.91-0.94), area under the receiver operating characteristic (AUROC) curve of 0.97 (95% CI, 0.96-0.98), sensitivity of 0.88 (95% CI, 0.85-0.91), and specificity of 0.95 (95% CI, 0.93-0.96). Activation areas on heat maps overlapped with the tumor in 350 of 360 CTs (97%). Performance was high across tumor stages (sensitivity of 0.80, 0.87, 0.95, and 1.0 on T1 through T4 stages, respectively), comparable for hypodense vs isodense tumors (sensitivity: 0.90 vs 0.82), different age, sex, CT slice thicknesses, and vendors (all P > .05), and generalizable on both the simulated cohort (accuracy, 0.95 [95% 0.94-0.95]; AUROC curve, 0.97 [95% CI, 0.94-0.99]) and public data sets (accuracy, 0.86 [95% CI, 0.82-0.90]; AUROC curve, 0.90 [95% CI, 0.86-0.95]). Despite being exclusively trained on diagnostic CTs with larger tumors, the model could detect occult PDA on prediagnostic CTs (accuracy, 0.84 [95% CI, 0.79-0.88]; AUROC curve, 0.91 [95% CI, 0.86-0.94]; sensitivity, 0.75 [95% CI, 0.67-0.84]; and specificity, 0.90 [95% CI, 0.85-0.95]) at a median 475 days (range, 93-1082 days) before clinical diagnosis. Conclusions: This automated artificial intelligence model trained on a large and diverse data set shows high accuracy and generalizable performance for detection of PDA on diagnostic CTs as well as for visually occult PDA on prediagnostic CTs. Prospective validation with blood-based biomarkers is warranted to assess the potential for early detection of sporadic PDA in high-risk individuals.
