Abstract:Artificial intelligence is rapidly expanding in diagnostic imaging. The musculoskeletal field has been showing high developmental potential. This review describes the technical background of artificial intelligence, machine learning, and deep learning and illustrates the main potential applications for musculoskeletal imaging. Artificial intelligence is rapidly expanding in all technological fields. The medical field, and especially diagnostic imaging, has been showing the highest developmental potential. Artificial intelligence aims at human intelligence simulation through the management of complex problems. This review describes the technical background of artificial intelligence, machine learning, and deep learning. The first section illustrates the general potential of artificial intelligence applications in the context of request management, data acquisition, image reconstruction, archiving, and communication systems. In the second section, the prospective of dedicated tools for segmentation, lesion detection, automatic diagnosis, and classification of musculoskeletal disorders is discussed.

Abstract:Ultrasound imaging is a patient-friendly and robust technique for studying physiological and pathological muscles. An automatic deep learning (DL) system for the analysis of ultrasound images could be useful to support an expert operator, allowing the study of large datasets requiring less human interaction. The purpose of this study is to present a deep learning algorithm for the cross-sectional area (CSA) segmentation in transverse musculoskeletal ultrasound images, providing a quantitative grayscale analysis which is useful for studying muscles, and to validate the results in a large dataset. The dataset included 3917 images of biceps brachii, tibialis anterior and gastrocnemius medialis acquired on 1283 subjects (mean age 50 ± 21 years, 729 male). The algorithm was based on multiple deep-learning architectures, and its performance was compared to a manual expert segmentation. We compared the mean grayscale value inside the automatic and manual CSA using Bland-Altman plots and a correlation analysis. Classification in healthy and abnormal muscles between automatic and manual segmentation were compared using the grayscale value z-scores. In the test set, a Precision of 0.88 ± 0.12 and a Recall of 0.92 ± 0.09 was achieved. The network segmentation performance was slightly less in abnormal muscles, without a loss of discrimination between healthy and abnormal muscle images. Bland-Altman plots showed no clear trend in the error distribution and the two readings have a 0.99 Pearson's correlation coefficient (p < 0.001, test set). The ICC(A, 1) calculated between the z-score readings was 0.99. The algorithm achieves robust CSA segmentation performance and gives mean grayscale level information comparable to a manual operator. This could provide a helpful tool for clinicians in neuromuscular disease diagnosis and follow-up. The entire dataset and code are made available for the research community.

Abstract:Pretreatment risk stratification is key for personalized medicine. While many physicians rely on an “eyeball test” to assess whether patients will tolerate major surgery or chemotherapy, “eyeballing” is inherently subjective and difficult to quantify. The concept of morphometric age derived from cross-sectional imaging has been found to correlate well with outcomes such as length of stay, morbidity, and mortality. However, the determination of the morphometric age is time intensive and requires highly trained experts. In this study, we propose a fully automated deep learning system for the segmentation of skeletal muscle cross-sectional area (CSA) on an axial computed tomography image taken at the third lumbar vertebra. We utilized a fully automated deep segmentation model derived from an extended implementation of a fully convolutional network with weight initialization of an ImageNet pre-trained model, followed by post processing to eliminate intramuscular fat for a more accurate analysis. This experiment was conducted by varying window level (WL), window width (WW), and bit resolutions in order to better understand the effects of the parameters on the model performance. Our best model, fine-tuned on 250 training images and ground truth labels, achieves 0.93 ± 0.02 Dice similarity coefficient (DSC) and 3.68 ± 2.29% difference between predicted and ground truth muscle CSA on 150 held-out test cases. Ultimately, the fully automated segmentation system can be embedded into the clinical environment to accelerate the quantification of muscle and expanded to volume analysis of 3D datasets.

Abstract:Deep learning is one of the most exciting new areas in medical imaging. This review article provides a summary of the current clinical applications of deep learning for lesion detection, progression, and prediction of musculoskeletal disease on radiographs, computed tomography (CT), magnetic resonance imaging (MRI), and nuclear medicine. Deep‐learning methods have shown success for estimating pediatric bone age, detecting fractures, and assessing the severity of osteoarthritis on radiographs. In particular, the high diagnostic performance of deep‐learning approaches for estimating pediatric bone age and detecting fractures suggests that the new technology may soon become available for use in clinical practice. Recent studies have also documented the feasibility of using deep‐learning methods for identifying a wide variety of pathologic abnormalities on CT and MRI including internal derangement, metastatic disease, infection, fractures, and joint degeneration. However, the detection of musculoskeletal disease on CT and especially MRI is challenging, as it often requires analyzing complex abnormalities on multiple slices of image datasets with different tissue contrasts. Thus, additional technical development is needed to create deep‐learning methods for reliable and repeatable interpretation of musculoskeletal CT and MRI examinations. Furthermore, the diagnostic performance of all deep‐learning methods for detecting and characterizing musculoskeletal disease must be evaluated in prospective studies using large image datasets acquired at different institutions with different imaging parameters and different imaging hardware before they can be implemented in clinical practice.Level of Evidence: 5 Technical Efficacy Stage: 2 J. Abstract:Deep learning has been widely used for medical image segmentation and a large number of papers has been presented recording the success of deep learning in the field. A comprehensive thematic survey on medical image segmentation using deep learning techniques is presented. This paper makes two original contributions. Firstly, compared to traditional surveys that directly divide literatures of deep learning on medical image segmentation into many groups and introduce literatures in detail for each group, we classify currently popular literatures according to a multi‐level structure from coarse to fine. Secondly, this paper focuses on supervised and weakly supervised learning approaches, without including unsupervised approaches since they have been introduced in many old surveys and they are not popular currently. For supervised learning approaches, we analyse literatures in three aspects: the selection of backbone networks, the design of network blocks, and the improvement of loss functions. For weakly supervised learning approaches, we investigate literature according to data augmentation, transfer learning, and interactive segmentation, separately. Compared to existing surveys, this survey classifies the literatures very differently from before and is more convenient for readers to understand the relevant rationale and will guide them to think of appropriate improvements in medical image segmentation based on deep learning approaches.

Abstract:As an emerging biomedical image processing technology, medical image segmentation has made great contributions to sustainable medical care. Now it has become an important research direction in the field of computer vision. With the rapid development of deep learning, medical image processing based on deep convolutional neural networks has become a research hotspot. This paper focuses on the research of medical image segmentation based on deep learning. First, the basic ideas and characteristics of medical image segmentation based on deep learning are introduced. By explaining its research status and summarizing the three main methods of medical image segmentation and their own limitations, the future development direction is expanded. Based on the discussion of different pathological tissues and organs, the specificity between them and their classic segmentation algorithms are summarized. Despite the great achievements of medical image segmentation in recent years, medical image segmentation based on deep learning has still encountered difficulties in research. For example, the segmentation accuracy is not high, the number of medical images in the data set is small and the resolution is low. The inaccurate segmentation results are unable to meet the actual clinical requirements. Aiming at the above problems, a comprehensive review of current medical image segmentation methods based on deep learning is provided to help researchers solve existing problems.

Abstract:Image segmentation is a branch of digital image processing which has numerous applications in the field of analysis of images, augmented reality, machine vision, and many more. The field of medical image analysis is growing and the segmentation of the organs, diseases, or abnormalities in medical images has become demanding. The segmentation of medical images helps in checking the growth of disease like tumour, controlling the dosage of medicine, and dosage of exposure to radiations. Medical image segmentation is really a challenging task due to the various artefacts present in the images. Recently, deep neural models have shown application in various image segmentation tasks. This significant growth is due to the achievements and high performance of the deep learning strategies. This work presents a review of the literature in the field of medical image segmentation employing deep convolutional neural networks. The paper examines the various widely used medical image datasets, the different metrics used for evaluating the segmentation tasks, and performances of different CNN based networks. In comparison to the existing review and survey papers, the present work also discusses the various challenges in the field of segmentation of medical images and different state-of-the-art solutions available in the literature.

Abstract:Deep learning techniques have rapidly become important as a preferred method for evaluating medical image segmentation. This survey analyses different contributions in the deep learning medical field, including the major common issues published in recent years, and also discusses the fundamentals of deep learning concepts applicable to medical image segmentation. The study of deep learning can be applied to image categorization, object recognition, segmentation, registration, and other tasks. First, the basic ideas of deep learning techniques, applications, and frameworks are introduced. Deep learning techniques that operate the ideal applications are briefly explained. This paper indicates that there is a previous experience with different techniques in the class of medical image segmentation. Deep learning has been designed to describe and respond to various challenges in the field of medical image analysis such as low accuracy of image classification, low segmentation resolution, and poor image enhancement. Aiming to solve these present issues and improve the evolution of medical image segmentation challenges, we provide suggestions for future research.

Abstract:Interactive segmentation is a crucial research area in medical image analysis aiming to boost the efficiency of costly annotations by incorporating human feedback. This feedback takes the form of clicks, scribbles, or masks and allows for iterative refinement of the model output so as to efficiently guide the system towards the desired behavior. In recent years, deep learning-based approaches have propelled results to a new level causing a rapid growth in the field with 121 methods proposed in the medical imaging domain alone. In this review, we provide a structured overview of this emerging field featuring a comprehensive taxonomy, a systematic review of existing methods, and an in-depth analysis of current practices. Based on these contributions, we discuss the challenges and opportunities in the field. For instance, we find that there is a severe lack of comparison across methods which needs to be tackled by standardized baselines and benchmarks.

Abstract:<p>The review covers automatic segmentation of images by means of deep learning approaches in the area of medical imaging. Current developments in machine learning, particularly related to deep learning, are proving instrumental in identification, and quantification of patterns in the medical images. The pivotal point of these advancements is the essential capability of the deep learning approaches to obtain hierarchical feature representations directly from the images, which in turn is removing the need for hand crafted features. Deep learning is expeditiously turning into the state of the art for medical image processing and has resulted in performance improvements in diverse clinical applications. In this review, the basics of deep learning methods are discussed along with an overview of successful implementations involving image segmentation for different medical applications. Finally, some research issues are highlighted and future need for further improvements is pointed out.</p>

Abstract:Clinical diagnosis of the pediatric musculoskeletal system relies on the analysis of medical imaging examinations. In the medical image processing pipeline, semantic segmentation using deep learning algorithms enables an automatic generation of patient-specific three-dimensional anatomical models which are crucial for morphological evaluation. However, the scarcity of pediatric imaging resources may result in reduced accuracy and generalization performance of individual deep segmentation models. In this study, we propose to design a novel multi-task, multi-domain learning framework in which a single segmentation network is optimized over the union of multiple datasets arising from distinct parts of the anatomy. Unlike previous approaches, we simultaneously consider multiple intensity domains and segmentation tasks to overcome the inherent scarcity of pediatric data while leveraging shared features between imaging datasets. To further improve generalization capabilities, we employ a transfer learning scheme from natural image classification, along with a multi-scale contrastive regularization aimed at promoting domain-specific clusters in the shared representations, and multi-joint anatomical priors to enforce anatomically consistent predictions. We evaluate our contributions for performing bone segmentation using three scarce and pediatric imaging datasets of the ankle, knee, and shoulder joints. Our results demonstrate that the proposed approach outperforms individual, transfer, and shared segmentation schemes in Dice metric with statistically sufficient margins. The proposed model brings new perspectives towards intelligent use of imaging resources and better management of pediatric musculoskeletal disorders.

Abstract:The medical imaging literature has witnessed remarkable progress in high-performing segmentation models based on convolutional neural networks. Despite the new performance highs, the recent advanced segmentation models still require large, representative, and high quality annotated datasets. However, rarely do we have a perfect training dataset, particularly in the field of medical imaging, where data and annotations are both expensive to acquire. Recently, a large body of research has studied the problem of medical image segmentation with imperfect datasets, tackling two major dataset limitations: scarce annotations where only limited annotated data is available for training, and weak annotations where the training data has only sparse annotations, noisy annotations, or image-level annotations. In this article, we provide a detailed review of the solutions above, summarizing both the technical novelties and empirical results. We further compare the benefits and requirements of the surveyed methodologies and provide our recommended solutions. We hope this survey article increases the community awareness of the techniques that are available to handle imperfect medical image segmentation datasets.

Abstract:Medical image segmentation plays a critical role in accurate diagnosis and treatment planning, enabling precise analysis across a wide range of clinical tasks. This review begins by offering a comprehensive overview of traditional segmentation techniques, including thresholding, edge-based methods, region-based approaches, clustering, and graph-based segmentation. While these methods are computationally efficient and interpretable, they often face significant challenges when applied to complex, noisy, or variable medical images. The central focus of this review is the transformative impact of deep learning on medical image segmentation. We delve into prominent deep learning architectures such as Convolutional Neural Networks (CNNs), Fully Convolutional Networks (FCNs), U-Net, Recurrent Neural Networks (RNNs), Adversarial Networks (GANs), and Autoencoders (AEs). Each architecture is analyzed in terms of its structural foundation and specific application to medical image segmentation, illustrating how these models have enhanced segmentation accuracy across various clinical contexts. Finally, the review examines the integration of deep learning with traditional segmentation methods, addressing the limitations of both approaches. Abstract:Automatic segmentation of knee bony anatomy is essential in orthopedics, and it has been around for several years in both pre-operative and post-operative settings. While deep learning algorithms have demonstrated exceptional performance in medical image analysis, the assessment of fairness and potential biases within these models remains limited. This study aims to revisit deep learning-powered knee-bony anatomy segmentation using plain radiographs to uncover visible gender and racial biases. The current contribution offers the potential to advance our understanding of biases, and it provides practical insights for researchers and practitioners in medical imaging. The proposed mitigation strategies mitigate gender and racial biases, ensuring fair and unbiased segmentation results. Furthermore, this work promotes equal access to accurate diagnoses and treatment outcomes for diverse patient populations, fostering equitable and inclusive healthcare provision.

Abstract:Deep learning algorithms, in particular convolutional networks, have rapidly become a methodology of choice for analyzing medical images. This paper reviews the major deep learning concepts pertinent to medical image analysis and summarizes over 300 contributions to the field, most of which appeared in the last year. We survey the use of deep learning for image classification, object detection, segmentation, registration, and other tasks. Concise overviews are provided of studies per application area: neuro, retinal, pulmonary, digital pathology, breast, cardiac, abdominal, musculoskeletal. We end with a summary of the current state-of-the-art, a critical discussion of open challenges and directions for future research.

Artificial intelligence in musculoskeletal imaging: realistic clinical applications in the next decade

Abstract:This article will provide a perspective review of the most extensively investigated deep learning (DL) applications for musculoskeletal disease detection that have the best potential to translate into routine clinical practice over the next decade. Deep learning methods for detecting fractures, estimating pediatric bone age, calculating bone measurements such as lower extremity alignment and Cobb angle, and grading osteoarthritis on radiographs have been shown to have high diagnostic performance with many of these applications now commercially available for use in clinical practice. Many studies have also documented the feasibility of using DL methods for detecting joint pathology and characterizing bone tumors on magnetic resonance imaging (MRI). However, musculoskeletal disease detection on MRI is difficult as it requires multi-task, multi-class detection of complex abnormalities on multiple image slices with different tissue contrasts. The generalizability of DL methods for musculoskeletal disease detection on MRI is also challenging due to fluctuations in image quality caused by the wide variety of scanners and pulse sequences used in routine MRI protocols. The diagnostic performance of current DL methods for musculoskeletal disease detection must be further evaluated in well-designed prospective studies using large image datasets acquired at different institutions with different imaging parameters and imaging hardware before they can be fully implemented in clinical practice. Future studies must also investigate the true clinical benefits of current DL methods and determine whether they could enhance quality, reduce error rates, improve workflow, and decrease radiologist fatigue and burnout with all of this weighed against the costs.
Biomedical Image Segmentation: A Systematic Literature Review of Deep Learning Based Object Detection Methods

Abstract:Biomedical image segmentation plays a vital role in diagnosis of diseases across various organs. Deep learning-based object detection methods are commonly used for such segmentation. There exists an extensive research in this topic. However, there is no standard review on this topic. Existing surveys often lack a standardized approach or focus on broader segmentation techniques. In this paper, we conducted a systematic literature review (SLR), collected and analysed 148 articles that explore deep learning object detection methods for biomedical image segmentation. We critically analyzed these methods, identified the key challenges, and discussed the future directions. From the selected articles we extracted the results including the deep learning models, targeted imaging modalities, targeted diseases, and the metrics for the analysis of the methods. The results have been presented in tabular and/or charted forms. The results are presented in three major categories including two stage detection models, one stage detection models and point-based detection models. Each article is individually analyzed along with its pros and cons. Finally, we discuss open challenges, potential benefits, and future research directions. This SLR aims to provide the research community with a quick yet deeper understanding of these segmentation models, ultimately facilitating the development of more powerful solutions for biomedical image analysis.

Automatic multi-structure pediatric knee bone segmentation using optimal multi-level Otsu thresholding to tackle intensity homogeneity in bone structures

Abstract:Recent studies in medical image segmentation involve new automatic approaches where active learning models are useful with less training samples. Presence of homogenous and heterogenous intensities for a single anatomical structure in pediatric musculoskeletal MR images affects the accuracy in terms of segmentation and classification of labels. This study addresses the homogeneity in intensity issues and introduces a new pre-training pipeline framework of Multi-level Otsu thresholding image as separate channel for 3D UNet model training. The proposed framework achieved higher performance of up to 85% when compared with the Baseline 3D UNet model and the Histogram threshold with 3D UNet. All algorithms are run through MONAI core framework.Clinical Relevance- This study will be of major interest to practicing pediatric clinicians and surgeons for its ability to provide accurate morphological assessment of underlying musculoskeletal structure. For researchers, it provides a new approach in dealing with heterogeneity in intensity problem which is common in pediatric MR imaging.

Abstract:Abstract Segmentation of anatomical components is a major step in creating accurate and realistic 3D models of the human body, which are used in many clinical applications, including orthopedics. Recently, many deep learning approaches have been proposed to solve the problem of manual segmentation, which is time-consuming and operator-dependent. In the present study, SegResNet has been adapted from other domains, such as brain tumors, for knee joints, in particular, to segment the femoral bone from magnetic resonance images. This algorithm has been compared to the well-known U-Net in terms of evaluation metrics, such as the Dice similarity coefficient and Hausdorff distance. In the training phase, various combinations of hyperparameters, such as epochs and learning rates, have been tested to determine which combination produced the most accurate results. Based on their comparable results, both U-Net and SegResNet performed well in accurately segmenting the femur. Dice similarity coefficients of 0.94 and Hausdorff distances less than or equal to 1 mm indicate that both models are effective at capturing anatomical boundaries in the femur. According to the results of this study, SegResNet is a viable option for automating the creation of 3D femur models. In the future, the performance and applicability of SegResNet in real-world settings will be further validated and tested using a variety of datasets and clinical scenarios.
