Abstract:Introduction Sudden cardiac death (SCD) in athletes is a devastating, highly publicized event that sparks ongoing debate and research into identifying potential risk factors, and its preventative screening is a controversial topic among cardiologists. With the lack of a world-renowned screening program model for athletes, the need for an adequate one is compelling. Several cases of sudden death in young Jordanian athletes were reported in the news, with no comprehensive data on SCDs. Thus, it is difficult to quantify the scope of the problem, making it all the more imperative to address this matter. This paper aims to shed light on a potential comprehensive universal cardiovascular screening model that could reduce the prevalence of SCD in athletes, focusing on its potential applicability in Jordan. Incidence Studies show a strong relationship between athleticism and the risk of SCD. In the UK, over 7,675 cases of SCD were reported by the national cardiac pathology center from 1994 to June 2022. American student-athletes were found to be 3.6 times more likely to face SCD compared to their non-athletic counterparts. Startlingly, up to 80% of athletes who suffered SCD had no prior warning manifestations. Etiology Hypertrophic cardiomyopathy (HCM), arrhythmogenic cardiomyopathy, and familial long QT syndrome are the most extensively studied conditions that may cause SCD. In the US, Autopsy-negative sudden unexplained death and the lack of mandatory pre-participation cardiac screening (PPCS) account for a significant proportion of SCDs in athletes. Current Methods & Limitations Current screening protocols have limited efficacy in detecting cardiovascular disease (CVD) in athletes. The American Heart Association (AHA) protocol involves a 14-point history and a physical exam (H&P), while European and other sporting societies recommend using ECG. However, a screening protocol rooted in H&P alone resulted in a high number of false negative results, with a sensitivity as low as ∼20%. Another limitation is having underreported symptoms due to familial and societal pressures to perform. Without an echocardiogram, some electrically silent pathologies can be missed. Our Proposed Model Including an echocardiogram in the screening of athletes can bring a more comprehensive form of evaluation to the PPCS program, particularly in countries where it is not as costly, such as Jordan. Additionally, using ECG could raise the sensitivity of CVD detection by 45.4%, as demonstrated in the UAE. This proposed model is a program unified for all athletes, with more elements than what is provided in the literature, which ensures reaching a proper diagnosis, regardless of expertise.
