Abstract:Preclinical data have shown a synergistic effect of radiotherapy (RT) combined with immunotherapy. However, the effect of combining RT with immunotherapy remains unknown for advanced NSCLC with brain metastases (BMs). This study aimed to assess the preliminary efficacy and safety of brain RT combined with camrelizumab plus platinum-based chemotherapy as first-line treatment in NSCLC patients (pts) with BMs. Treatment-naïve pts with NSCLC and BMs were adults who had no known EGFR/ALK/ROS1 alterations and had at least one measurable intracranial and extracranial lesions per RECIST 1.1. Eligible pts received camrelizumab (200 mg) and platinum-based chemotherapy intravenously every 3 weeks for 4-6 cycles after stereotactic radiosurgery or whole-brain RT. Subsequently, pts without disease progression received maintenance treatment with camrelizumab ± pemetrexed every 3 weeks up to 24 months, or until disease progression or intolerance. The primary endpoint was 6-month progression-free survival (PFS) rate. Between May 6, 2020 and April 25, 2023 (data cutoff), 65 pts were enrolled and treated. The primary endpoint 6-month PFS rate was 70.2% (95% CI 56.3-80.4). With a median follow-up of 11.2 months (95% CI 9.0-17.0), the median PFS was 8.7 months (95% CI 7.5-15.7), and median overall survival was 21 months (95% CI 15.1-NE). Of the 65 pts, 49 (75.4%) pts had a confirmed partial response and 14 (21.5%) had stable disease, with the overall objective response rate (ORR) and disease control rate of 75.4% (95% CI 63.1%-85.2%) and 96.9% (95% CI 89.3%-99.6%). The intracranial ORR was 81.5% (95% CI 70%-90.1%). Adverse events (AEs) of any grade occurred in 64 (98.5%) of the 65 pts, and grade 3 or 4 AEs were observed in 32 (49.2%) pts, with the most common being neutrophil count decreased (14 [21.5%]) and white blood cell count decreased (11 [16.9%]). No treatment-related deaths occurred. The combination of brain radiotherapy, camrelizumab and platinum-based chemotherapy showed promising efficacy and manageable toxicity for treatment-naïve advanced NSCLC with BMs.
Stereotactic radiosurgery for brain metastases from small cell lung cancer without prior whole-brain radiotherapy: A meta-analysis

Abstract:Objectives: Assess upfront Stereotactic radiosurgery (SRS) effectiveness for small cell lung cancer (SCLC) brain metastases (BM). Where possible, a comparison with whole-brain radiotherapy (WBRT) was performed. Methods: Following PRISMA and MOOSE guidelines, eligible studies were identified on Medline, Embase, Cochrane Library, and proceedings of annual meetings between inception and July 01, 2020. Results: Nine observational studies with 1638 patients were included. The median overall survival (OS) was 8.3 months (95% CI 7.1-9.5 months, I2 = 0%). OS rate at 12 months was 39% (95% CI 31-44%, I2 = 0%). The relative risk between SRS and WBRT for the OS at 12 months was 1.33 (95% CI 1.13-1.51, P = 0.0001). The projected OS for 6, 12, 18- and 24-months comparing SRS with WBRT was 67% vs. 57%, 39% vs. 29%, 22% vs. 15% and 15% vs 9%, favoring SRS (P < 0.001). The LC rate at 12 months was 93% (95% CI 91-94%, I2 = 0%). The distant brain failure rate (DBFR) at 12 months was 41% (95% CI 33-48%, I2 = 52%, P = 0.08). The SRS or WBRT as salvage treatment after upfront SRS was 32% and 19%, respectively. The freedom from neurologic death at 12 months was 87% (95% CI 84-89%). Conclusion: Based on the pooling of a large sample of retrospective studies our meta-analysis suggests that for high selected SCLC patients with limited BM upfront SRS produces favorable lesion control and survival outcomes. These findings support the design of randomized clinical trial to confirm the role of SRS in this clinical scenario.
Upfront Stereotactic Radiosurgery or Fractionated Stereotactic Radiotherapy in Elderly Patients with Brain Metastases from Non-small cell lung cancer: A Retrospective Analysis of a 10-Year Bi-Institutional Experience

Abstract:Purpose: Stereotactic radiosurgery (SRS) or fractionated stereotactic radiotherapy (FSRT) are increasingly used as initial therapies for brain metastases (BM). We aimed to assess the outcomes of SRS/FSRT in patients aged ≥65 years who had 1-10 BM from non-small cell lung cancer (NSCLC). Materials and methods: We retrospectively reviewed 91 elderly NSCLC patients with 222 BM who were treated with SRS/FSRT at two institutions between 2010 and 2020. The primary endpoint was overall survival (OS) after SRS/FSRT. In addition, in-field local control (IFLC) within the treated field was evaluated. Statistical analysis was performed to identify the prognostic factors affecting OS and IFLC. Results: During a median follow-up of 18 months, the median OS was 32 months. The 1- and 2-year survival rates were 69.8 and 56.1%, respectively. In multivariate analysis, the NSCLC-specific graded prognostic assessment (GPA) score (p=0.007) and administration of systemic therapy (p=0.039) were defined as prognosticators affecting OS. The median IFLC period was 31 months, and the 1- and 2-year IFLC rates were 75.9 and 57.6%, respectively. The total BM volume (p=0.042) significantly affected IFLC. No severe adverse events were reported after SRS/FSRT. Conclusion: SRS/FSRT is an effective upfront treatment option for BM arising from NSCLC in elderly patients, with a good OS without severe side effects. Higher GPA score and active systemic treatment were associated with improved OS, indicating that elderly patients are significant candidates for SRS/FSRT.

Abstract:Abstract Background Stereotactic radiosurgery (SRS) is preferred to whole brain radiotherapy (WBRT) for the treatment of patients with a limited number of brain metastases, both as primary treatment and after surgery. SRS can be delivered in a single fraction (SF) or in multiple fractions (MF) depending on tumor size and location. The aim of this single-center retrospective study was to evaluate intracranial control and overall survival (OS) in patients who received SF and MF SRS for brain metastases. Material and Methods All patients treated with primary SRS for brain metastases at Aarhus University Hospital between 2015 and 2020 were identified. Independent of undergoing surgery, SF SRS (20 Gy) was administered for targets (gross tumor volume) <2 cm in diameter and MF SRS (24-27 Gy in 3 fractions) for targets >2 cm. SRS was not combined with WBRT. Treatment response was evaluated by 3-monthly MRI-scan. To evaluate intracranial control, first events were scored as local-only failure (tumor progression at the SRS treatment site), distant failure (tumor progression not at the SRS site), and combined failure (local and distant failure). Actuarial incidence of local failure (local-only and combined) and OS were estimated using the Kaplan-Meier method. To analyze the association of SF and MF SRS with local failure and OS, multivariable Cox regression analyses were used. Results The consecutive cohort consisted of 196 patients treated for 275 brain metastases. The most frequent primary disease was non-small cell lung cancer (NSCLC n=96), followed by renal cell carcinoma (n=26) and breast cancer (n=22). SRS was delivered to 1 (n=133), 2 (n=49), 3 (n=12), and 4 (n=2) targets per patient. SF and MF SRS was used in 99 (51%) and 97 (49%) patients, respectively. In 40 patients, SRS was administered after surgery (SF n=7, MF n=33). At a median (range) follow-up of 10 (0-93) months, 111 (57%) patients had intracranial progression, and 159 (81%) patients died. Local-only/distant/combined failure rate was 23 and 42%/59 and 35%/18 and 23%, in SF and MF SRS respectively. The median time to local failure was respective 9.0 (95% CI 5.4-12.6) and 6.0 (95% CI 5.0-7.0) months in the SF and MF SRS group. SF SRS and surgery were independently associated with a longer time to local failure. Salvage WBRT, SRS, and re-operation were performed in respective 36, 13, and 26 patients. Histopathological radiation necrosis was observed in 5 patients. All patients with necrosis had a primary diagnosis of NSCLC, and 4/5 patients were treated with SF SRS. For the total group, the median OS was 10.0 (95% CI 8.0 - 12.0) months. PS<2, surgery, stable extracranial disease and SF SRS were associated with a longer survival. Conclusion MF SRS was associated with a higher rate of and shorter time to local failure, and was a poor prognostic factor for survival, which is in line with the selection of patients with more unfavorable brain metastases for this treatment.

Multi-institutional retrospective review of stereotactic radiosurgery for brain metastasis in patients with small cell lung cancer without prior brain-directed radiotherapy

Abstract:Introduction: Patients with small cell lung cancer (SCLC) brain metastasis (BM) typically receive whole brain radiotherapy (WBRT) as data regarding upfront radiosurgery (SRS) in this setting are sparse. Methods: Patients receiving SRS for SCLC BM without prior brain radiation were identified at three U.S. institutions. Overall survival (OS), freedom from intracranial progression (FFIP), freedom from WBRT (FFWBRT), and freedom from neurologic death (FFND) were determined from time of SRS. Results: Thirty-three patients were included with a median of 2 BM (IQR 1-6). Median OS and FFIP were 6.7 and 5.8 months, respectively. Median FFIP for patients with ≤2 versus >2 BM was 7.1 versus 3.6 months, p=0.0303. Eight patients received salvage WBRT and the 6-month FFWBRT and FFND were 87.8%. and 90.1%, respectively. Conclusions: Most SCLC patients with BM who received upfront SRS avoided WBRT and neurologic death, suggesting that SRS may be an option in select patients.

Abstract:Brain metastases (BM) frequently occur in non-small cell lung cancer (NSCLC) patients. Most patients with BM have a limited life expectancy, measured in months. Selected patients may experience a very long progression-free survival, for example, patients with a targetable driver mutation. Traditionally, whole-brain radiotherapy (WBRT) has been the cornerstone of the treatment, but its indication is a matter of debate. A randomized trial has shown that for patients with a poor prognosis, WBRT does not add quality of life (QoL) nor survival over the best supportive care. In recent decades, stereotactic radiosurgery (SRS) has become an attractive non-invasive treatment for patients with BM. Only the BM is irradiated to an ablative dose, sparing healthy brain tissue. Intracranial recurrence rates decrease when WBRT is administered following SRS or resection but does not improve overall survival and comes at the expense of neurocognitive function and QoL. The downside of SRS compared with WBRT is a risk of radionecrosis (RN) and a higher risk of developing new BM during follow-up. Currently, SRS is an established treatment for patients with a maximum of four BM. Several promising strategies are currently being investigated to further improve the indication and outcome of SRS for patients with BM: the effectivity and safety of SRS in patients with more than four BM, combining SRS with systemic therapy such as targeted agents or immunotherapy, shared decision-making with SRS as a treatment option, and individualized isotoxic dose prescription to mitigate the risk of RN and further enhance local control probability of SRS. This review discusses the current indications of SRS and future directions of treatment for patients with BM of NSCLC with focus on the value of SRS.

Abstract:Stereotactic radiosurgery (SRS) is an option for brain metastases (BM) not eligible for surgical resection, however, predictors of SRS outcomes are poorly known. The aim of this study is to investigate predictors of SRS outcome in patients with BM secondary to non-small cell lung cancer (NSCLC). The secondary objective is to analyze the value of volumetric criteria in identifying BM progression. This retrospective cohort study included patients >18 years of age with a single untreated BM secondary to NSCLC. Demographic, clinical, and radiological data were assessed. The primary outcome was treatment failure, defined as a BM volumetric increase 12 months after SRS. The unidimensional measurement of the BM at follow-up was also assessed. One hundred thirty-five patients were included, with a median BM volume at baseline of 1.1 cm 3 (IQR 0.4–2.3). Fifty-two (38.5%) patients had SRS failure at follow-up. Only right BM laterality was associated with SRS failure ( p =0.039). Using the volumetric definition of SRS failure, the unidimensional criteria demonstrated a sensibility of 60.78% (46.11%–74.16%), specificity of 89.02% (80.18%–94.86%), positive LR of 5.54 (2.88–10.66) and negative LR of 0.44 (0.31–0.63). SRS demonstrated a 61.5% local control rate 12 months after treatment. Among the potential predictors of treatment outcome analyzed, only the right BM laterality had a significant association with SRS failure. The volumetric criteria were able to identify more subtle signs of BM increase than the unidimensional criteria, which may allow earlier diagnosis of disease progression and use of appropriate therapies.

Abstract:Purpose: To evaluate the efficiency and side effects of stereotactic radiation therapy (SRT) with or without other treatments for brain metastases (BM) from various primary tumors. Methods: This was a retrospective analysis of 161 patients with brain metastases treated with SRT. Clinical data, EGFR mutation status and survival data were collected. Follow-up data was analyzed until December 2018. Kaplan-Meier and Cox proportional hazards regression analyses were used for the survival analysis. Results: The median overall survival (OS) was 19 months. No difference was observed in OS between SRT group and SRT + whole brain radiation therapy (WBRT) groups (p = 0.717). Statistically significant factors of better OS after univariable analysis were no extracranial metastases (p = 0.016), BED10-SRT >= 50Gy (p = 0.049), oligometastases (1-3 brain metastases) (p < 0.001), GPA score >= 2.5 (p = 0.003), RPA class I (p = 0.026), NSCLC tumor type (p = 0.006), targeted therapy (p < 0.001) and controlled extracranial disease (p = 0.011). Multivariate analysis indicated that higher BED10-SRT (>= 50Gy, HR = 0.504, p = 0.027), controlled extracranial disease (HR = 0.658, p = 0.039) and targeted therapy (HR = 0.157, <0.001) were independent favorable predictors for OS. Besides that, we also find that the median overall survival (OS) was 22 months in NSCLC patients and controlled extracranial disease (HR = 0.512, p = 0.012) and targeted therapy (HR = 0.168, < 0.001) were independent favorable predictors for OS. Conclusion: For patients with brain metastases, stable extracranial disease, higher BED10-SRT (>= 50Gy) and targeted therapy may predict a favorable prognosis.
Up-front single-session radiosurgery for large brain metastases-volumetric responses and outcomes

Abstract:Background: Patients presenting with large brain metastases (LBM) pose a management challenge to the multidisciplinary neuro-oncologic team. Treatment options include surgery, whole-brain or large-field radiation therapy (WBRT), stereotactic radiosurgery (SRS), or a combination of these. Objective: To determine if corticosteroid therapy followed by SRS allows for efficient minimally invasive care in patients with LBMs not compromised by mass effect. Methods: We analyzed the change in tumor volume to determine the efficacy of single-session SRS in the treatment of LBM in comparison to other treatment modalities. Twenty-nine patients with systemic cancer and brain metastasis (≥ 2.7 cm in greatest diameter) who underwent single-session SRS were included. Results: Among 29 patients, 69% of patients had either lung, melanoma, or breast cancer. The median initial tumor size (maximal diameter) was 32 mm (range 28-43), and the median initial tumor volume was 9.56 cm3 (range 1.56-25.31). The median margin dose was 16 Gy (range 12-18). The average percent decrease in tumor volume compared to pre-SRS volume was 55% on imaging at 1-2 months, 58% at 3-5 months, 64% at 6-8 months, and 57% at > 8 months. There were no adverse events immediately following SRS. Median corticosteroid use after SRS was 21 days. Median survival after radiosurgery was 15 months. Conclusion: Initial high-dose corticosteroid therapy followed by prompt single-stage SRS is a safe and efficacious method to manage patients with LBMs (defined as ≥ 2.7 cm).

Abstract:What's New? This international multicenter study investigates the impact of stereotactic radiosurgery (SRS) and fractionated stereotactic radiotherapy (FSRT) on brainstem metastases (BSM). Our results, analyzing 136 patients with 144 BSM from nine institutions, underscore the long‐term benefits in local control and the robust safety profile despite the delicate location of the treated metastases. Based on this analysis and review of other studies, the use of stereotactic radiotherapy for BSM in prospective SRS and FSRT trials appears justified. Brainstem metastases (BSM) present a significant neuro‐oncological challenge, resulting in profound neurological deficits and poor survival outcomes. Stereotactic radiosurgery (SRS) and fractionated stereotactic radiotherapy (FSRT) offer promising therapeutic avenues for BSM despite their precarious location. This international multicenter study investigates the efficacy and safety of SRS and FSRT in 136 patients with 144 BSM treated at nine institutions from 2005 to 2022. The median radiographic and clinical follow‐up periods were 6.8 and 9.4 months, respectively. Predominantly, patients with BSM were managed with SRS (69.4%). The median prescription dose and isodose line for SRS were 18 Gy and 65%, respectively, while for FSRT, the median prescription dose was 21 Gy with a median isodose line of 70%. The 12‐, 24‐, and 36‐month local control (LC) rates were 82.9%, 71.4%, and 61.2%, respectively. Corresponding overall survival rates at these time points were 61.1%, 34.7%, and 19.3%. In the multivariable Cox regression analysis for LC, only the minimum biologically effective dose was significantly associated with LC, favoring higher doses for improved control (in Gy, hazard ratio [HR]: 0.86, p

Long-Term Results of Stereotactic Radiotherapy in Patients with at Least 10 Brain Metastases at Diagnosis

Abstract:Purpose: to evaluate an SRT approach in patients with at least 10 lesions at the time of BM initial diagnosis. Methods: This is a monocentric prospective cohort of patients treated by SRT, followed by a brain MRI every two months. Subsequent SRT could be delivered in cases of new BMs during follow-up. The main endpoints were local control rate (LCR), overall survival (OS), and strategy success rate (SSR). Acute and late toxicity were evaluated. Results: Seventy patients were included from October 2014 to January 2019, and the most frequent primary diagnosis was non-small-cell lung cancer (N = 36, 51.4%). A total of 1174 BMs were treated at first treatment, corresponding to a median number of 14 BMs per patient. Most of the patients (N = 51, 72.6%) received a single fraction of 20–24 Gy. At 1 year, OS was 62.3%, with a median OS of 19.2 months, and SSR was 77.8%. A cumulative number of 1537 BM were treated over time, corresponding to a median cumulative number of 16 BM per patient. At 1-year, the LCR was 97.3%, with a cumulative incidence of radio-necrosis of 2.1% per lesion. Three patients (4.3%) presented Grade 2 toxicity, and there was no Grade ≥ 3 toxicity. The number of treated BMs and the treatment volume did not influence OS or SSR (p > 0.05). Conclusions: SRT was highly efficient in controlling the BM, with minimal side effects. In this setting, an SRT treatment should be proposed even in patients with ≥10 BMs at diagnosis.

Abstract:In this paper results are presented from investigations on partial discharges (PD) in insulation systems, resembling the stator insulation in high voltage rotating machines. A model, simulating a stator winding in a slot, has been developed, consisting of simple rotating machine insulation test bars with epoxy/mica insulation, mounted between steel sheets forming a dot, in order to investigate the fundamental behaviour of PD in insulation defects in epoxy/mica insulation and the characteristics of the resulting electrical pulses. Stator slot couplers (SSC) were used to detect pulses coming from artificial defects implemented in the model as well as from the existing voids in the test bars' insulation. Some characteristic pulse shapes were found, which could be related partly to the defect type. The high frequency detection method, with directional couplers, made it possible to localize the defects thereby determining the pulse distortion in the chosen geometry. The influence of some parameters was investigated, such as the voltage applied, the location of the defect with respect to the sensor and the orientation of the defect in the slot. This information is essential for PD-detection systems where the noise/signal separation is based on the pulse shape, such as the SSC-technology used in the present investigations.<<ETX>>

Abstract:P turfgrasses are characterized by an extremely fine, fibrous root system. This leads to considerable frustration on the part of researchers due to the time-consuming, costly procedures required to accurately study and measure turfgrass roots. Nevertheless, the monitoring of rooting depth and functional viability on a regular basis is an important dimension in any turf manager's information assemblage for key decision-making activities. Roots are a hidden, but major component of a turfgrass plant. Roots function in water absorption, nutrient uptake, anchorage, and sod strength. Actually, the unique, extensive, fibrous root system of turfgrass has a extraordinary ability to take up nutrient elements, and thereby protects the quality of groundwater. Turfgrass shoots/leaves have their meristematic region and allied cell division and elongation activities located at the base. Thus, the leaf tip is the oldest portion of the leaf and the leaf base the youngest. As a result the oldest part of the leaf is cut off during mowing. Just the opposite exists for roots, where the tip of the root most distant from the originating turfgrass crown or stem node region is the youngest part of the root. Thus, in harvesting sod the meristems or growing tips of the roots are cut off. Consequently, new root initiation must originate from the meristematic tissues in the crowns and the nodes on lateral stems, such as stolons and rhizomes. This basic principal dictates that the shallower the soil depth cut during sod harvesting, the more rapid the rate of sod transplant rooting, assuming there is no water stress. The principle sites through which water and nutrients are taken up from the soil solution into the roots and translocated upward to the shoots are via the root hairs. These are very delicate, whitish, outward extensions of individual epidermal or surface root cells. These root hairs are microscopic and thus cannot be seen by the naked eye. However, the surface area of these roots is 5 to 10 times greater than the primary and secondary root system that is visible to the eye. Therefore, understanding how environmental factors, cultural practices, and chemicals affect root hairs is a major concern in relation to maximizing the ability of turfgrasses to take up vital water and nutrients. Most turfgrasses are perennials, but certain perennial species have root systems that behave as an annual. Included among the annual-type root systems are such coolseason turfgrasses as perennial ryegrass (Lolium perenne), bentgrass (Agrostis spp.), and rough bluegrass (Poa trivialis). The critical period in the loss of root systems of cool-season turfgrasses is during mid-summer when soil temperatures at a 4-inch (100 mm) depth exceed 80°F (17°C) (Figure 1). Warm-season turfgrasses may be characterized as having annual root systems on occasions when spring root decline (SRD) occurs. The critical time in terms of a limited root system for warm-season turfgrasses is in the spring when soil temperatures at a 4-inch (100-mm) depth reach 64°F (18°C) (Figure 2). The genetic rooting ability of various turfgrass species and cultivars in terms of depth and mass varies considerably (Table 1). Abstract:Background and purpose To evaluate modern clinical outcomes for patients with brain-only metastatic non-small cell lung cancer (NSCLC) treated with intracranial stereotactic radiosurgery (SRS) with or without definitive treatment of the primary site. Materials and methods Patients with synchronously diagnosed NSCLC and brain-only metastatic disease treated with intracranial SRS at a single institution were retrospectively identified. Patients were stratified based on whether they did (A) or did not (B) receive definitive primary site treatment. Patient characteristics and clinical outcomes were compared. Results From 2008 to 2022, 103 patients were identified, 53 of whom received definitive primary site treatment. Median follow-up was 2.1 y (A) and 0.8 y (B) (p < 0.001). 28 (53 %) patients in Group A received immune checkpoint inhibitor (ICI) therapy versus 19 (38 %) in Group B (p = 0.13) and there were no other statistically significant baseline or treatment characteristic differences between the groups. 5-year local-PFS was 34.5 % (A) versus 0 % (B) (p < 0.001). 5-year regional-PFS was 33.0 % (A) versus 0 % (B) (p < 0.001). 5-year distant body-PFS was 34.0 % (A) versus 0 % (B) (p < 0.001). 5-year CNS-PFS was 14.7 % (A) versus 0 % (B) (p = 0.12). 5-year OS was 40.2 % (A) versus 0 % (B) (p = 0.001). 5-year CSS was 67.6 % (A) versus 0 % (B) (p = 0.002). On multivariable analysis, lack of definitive treatment to the primary site (HR = 2.40), AJCC T3-4 disease (HR = 2.73), and lack of ICI therapy (HR = 2.86) were significant predictors of death. Conclusion Definitive treatment to the thoracic primary site in patients with brain-only metastatic NSCLC after intracranial radiosurgery was associated with slower progression of disease and improved survival.

Abstract:This study aims to evaluate the clinical outcome of stereotactic radiosurgery as the sole treatment for brain metastases and to assess prognostic factors influencing survival. A total of 108 consecutive patients with 213 metastases were retrospectively analyzed. Treatment was determined with close-meshed MRI follow-up. Various prognostic factors were assessed, and several prognostic indices were compared regarding their reliability to estimate overall survival. Median overall survival was 15 months; one-year overall survival was 50.5%. Both one- and two-year local controls were 90.9%. The rate of new metastases after SRS was 49.1%. Multivariate analysis of prognostic factors revealed that the presence of extracranial metastases, male sex, lower KPI, and progressive extracranial disease were significant risk factors for decreased survival. Of all evaluated prognostic indices, the Basic Score for Brain Metastases (BSBMs) showed the best correlation with overall survival. A substantial survival advantage was found for female patients after SRS when compared to male patients (18 versus 9 months, p = 0.003). SRS of brain metastasis is a safe and effective treatment option when frequent monitoring for new metastases with MRI is performed. Common prognostic scores lack reliable estimation of survival times. Female sex should be considered as an additional independent positive prognostic factor influencing survival.

Upfront Brain Radiotherapy May Improve Survival for Unfavorable Prognostic Breast Cancer Brain Metastasis Patients with Breast-GPA 0-2.0

Abstract:In this study, we attempted to assess the efficacy of upfront brain radiotherapy (RT) in breast cancer (BC) patients with brain metastases (BM). Medical records of 111 consecutive BC patients treated with WBRT or SRS between August 2009 and November 2017 in single center were retrospectively reviewed. Eighty patients received upfront brain RT after BM diagnosis and 31 had delayed RT. The median age at BM was 54 years (22-77), with median KPS 80 (50-90). The molecular BC subtypes of Luminal A, Luminal B, triple-negative and HER2 overexpression were 16, 47, 27, and 19, respectively, with 2 unknown. Of them, 83 received WBRT after BM and 28 SRS. Median overall survival (OS) was significantly related to Breast-GPA, as following: 6.5, 9.9, 14.4, and 24.5 months in 0-1.0, 1.5-2.0, 2.5-3.0, and 3.5-4.0 subgroups, respectively (P = 0.007). Univariate and multivariate analysis showed that KPS <= 70, infratentorial involvement, extracranial metastases, and no continuing systemic therapy were independent risk factors for OS. In the whole group, no significant difference in OS was found between upfront or delayed RT. For Breast-GPA 0-2.0 subgroup, upfront RT was associated with increased median OS (3.3 vs 9.8 months, P = 0.04). In GPA 2.5-4.0 subgroup, the median OS for upfront and delayed RT was 13.8 and 16.5 months, respectively (P = 0.58). In conclusion, BCBM patients with better KPS, systemic therapy, without infratentorial involvement and extracranial metastases are associated with better OS. Patients with Breast-GPA 0-2.0 might benefit from upfront brain RT.

Abstract:Background/Objectives: Around 20% of cancer patients will develop brain metastases (BrMs), with 15–25% occurring in the posterior fossa (PF). Although the effectiveness of systemic therapies is increasing, surgery followed by stereotactic radiosurgery (S+SRS) versus definitive SRS remains the mainstay of treatment. Given the space restrictions within the PF, patients with BrMs in this location are at higher risk of brainstem compression, hydrocephalus, herniation, coma, and death. However, the criteria for treating large PF BrMs with S+SRS versus definitive SRS remains unclear. Methods: We reviewed a prospective registry database (2009 to 2020) and identified 64 patients with large PF BrMs (≥4 cc) treated with SRS or S+SRS. Clinical and radiological parameters were analyzed. The two endpoints were overall survival (OS) and local failure (LF). Results: Patients in the S+SRS group were more highly symptomatic than patients in the SRS group. Gait imbalance and intracranial pressure symptoms were 97% and 80%, and 47% and 35% for S+SRS and SRS, respectively. Radiologically, there were significant differences in the mean volume of the lesions [6.7 cm3 in SRS vs. 29.8 cm3 in the S+SRS cohort, (p < 0.001)]; compression of the fourth ventricle [47% in SRS vs. 96% in S+SRS cohort, (p < 0.001)]; and hydrocephalus [0% in SRS vs. 29% in S+SRS cohort, (p < 0.001)]. Patients treated with S+SRS had a higher Graded Prognostic Assessment (GPA). LF was 12 and 17 months for SRS and S+SRS, respectively. Moreover, the S+SRS group had improved OS (12 vs. 26 months, p = 0.001). Conclusions: A higher proportion of patients treated with S+SRS presented with hydrocephalus, fourth-ventricle compression, and larger lesion volumes. SRS-alone patients had a lower KPS, a lower GPA, and more brain metastases. S+SRS correlated with improved OS, suggesting that it should be seriously considered for patients with large PF-BrM.

Abstract:Background and purpose: After surgical resection of brain metastases (BM), radiotherapy (RT) is indicated. Postoperative stereotactic radiosurgery (SRS) reduces the risk of local progression and neurocognitive decline compared to whole brain radiotherapy (WBRT). Aside from the optimal dose and fractionation, little is known about the combination of systemic therapy and postoperative fractionated stereotactic radiotherapy (fSRT), especially regarding tumour control and toxicity. Methods: In this study, 105 patients receiving postoperative fSRT with 35 Gy in 7 fractions performed with Cyberknife were retrospectively reviewed. Overall survival (OS), local control (LC) and total intracranial brain control (TIBC) were analysed via Kaplan-Meier method. Cox proportional hazards models were used to identify prognostic factors. Results: Median follow-up was 20.8 months. One-year TIBC was 61.6% and one-year LC was 98.6%. Median OS was 28.7 (95%-CI: 16.9-40.5) months. In total, local progression (median time not reached) occurred in 2.0% and in 20.4% radiation-induced contrast enhancements (RICE) of the cavity (after median of 14.3 months) were diagnosed. Absence of extracranial metastases was identified as an independent prognostic factor for superior OS (p = <0.001) in multivariate analyses, while a higher Karnofsky performance score (KPS) was predictive for longer OS in univariate analysis (p = 0.041). Leptomeningeal disease (LMD) developed in 13% of patients. Conclusion: FSRT after surgical resection of BM is an effective and safe treatment approach with excellent local control and acceptable toxicity. Further prospective randomized trials are needed to establish standardized therapeutic guidelines.
