Abstract:Background: Families of refugee background have complex, multigenerational mental health and developmental needs that are not accounted for in current programming frameworks in Canada. Providing appropriate support services and educational resources that address the unique concerns of families of refugee background will allow for improved family cohesion and developmental outcomes for children. Parenting programs have been shown to be successful in improving parental stress, parental efficacy, and children's mental health and well-being. This study gathers data about the experiences of caregivers of refugee background in order to develop a novel, multi-dimensional parenting program model using Community-Based Participatory Research (CBPR) principles. Methodology: This was a qualitative, CBPR study using a formative research framework. In-depth interviews (IDIs) were conducted with caregivers of refugee background and service providers that work closely with this population. Data were recorded, transcribed, and coded using deductive and inductive coding methods by two independent coders. Results: A total of 20 IDIs were conducted (7 caregivers and 13 service providers). The main topics that were identified to be incorporated into the program include: features of child development, how to address resettlement issues, child advocacy, and parenting after resettlement. Participants felt that tackling language barriers, addressing the overlapping responsibilities of caregivers attending the sessions, providing incentives, increasing awareness of the program, and using an anti-racist and anti-oppressive approach was key to the program's success. Participants emphasized the need for trauma-informed mental health supports within the program model. Conclusion: This study describes the key considerations for a novel parenting program for families of refugee background, by engaging them as key stakeholders in the program design process. Future iterations of this project would involve a pilot and evaluation of the program.

Abstract:The rapid response to settle Syrian refugees in Canada has had a profound effect on communities, schools, and social service agencies. This article discusses a research program that examined the integration and settlement of Syrian children and youth in Winnipeg and Calgary. Through the examination of the school and community contexts, the research focused on the educational and psychosocial needs of re-settled Syrian refugees and the reciprocal learning between refugee, immigrant and Canadian-born students. With contributions from youth, parents, and relevant stakeholders, the research identified gaps in programming and services as well as promising practices that support newcomers. Issues surrounding trauma, interrupted schooling, separation and loss, racism and discrimination complicated the settlement and integration efforts. Findings indicate that Canadians and Canadian service providers have a major role in supporting the successful integration of refugees. Cultural support workers, cultural brokers, and community liaison personnel are paramount to bridging the school to families and community agencies.

Abstract:Background: Refugees tend to have greater health needs and pre-existing medical conditions due to poor living conditions, lack of health services, exposure to a variety of risk factors, and a high level of stress and trauma prior to entry to a host country. Notwithstanding distinctive needs and inherent conditions, there is a paucity of literature on refugee maternal health, especially for Arabic-speaking refugee women resettled in the USA. Methods and analysis: The paper delineates a qualitative study protocol to explore the experiences of Arabic-speaking refugee women in the USA when accessing maternal care. Informed by social cognitive theory, the study will employ two qualitative research methods; in-depth interviews and 'go-along' interviews with Arabic-speaking refugee women. Go-along interview will be used to elicit spatial experiences in situ to explore perceptions of environments among study participants and environmental and structural barriers. 20 refugee women who meet the inclusion criteria will be recruited through snowball sampling with support from community partners. Two researchers will code the transcription and fieldnotes using MAXQDA 2020 (VERBI Software, 2019). The analysis will involve deductive content analysis using a structured categorisation matrix based on the theory while also incorporating inductive codes that may emerge through the process. Ethics and dissemination: The study has been reviewed and approved by the Human Subjects Protection Programme at the University of Arizona (IRB 2104716241). The study results will be condensed in a summary report, which will be shared with community partners, including refugee resettlement agencies and relevant staff at the state department. Also, community feedback will be garnered from the dissemination workshops to inform community discussions for actions and an intervention to address the identified needs.
Onychomycosis by Paecilomyces lilacinus.

Abstract:The authors describe a case of onychomycosis by Paecilomyces lilacinus in an 81-year-old Caucasian woman. The infection was characterized clinically by yellow-brown discoloration of the nail plate, distal-lateral onycholysis and subungual hyperkeratosis of the first left toenail. Microscopic examination showed irregular hyphae. Four cultures showed the development of lilac and powdery colonies. Conidiophores with divergent long narrow terminal phialides were observed at microscopic examination of culture samples. Therapy with amorolofine nail laquer, itraconazole and terbinafine was uneffective. This is the sixth case published in the literature of Paecilomyces lilacinus onychomycosis, and the second Italian case of Paecilomyces lilacinus infection.
Resettlement Experiences of Syrian Refugees in the United States: Policy Challenges and Directions

Abstract:There is a scarcity of phenomenological studies that have incorporated the voices of Syrian refugees concerning their resettlement experiences in the United States. This phenomenological study explored the challenges, forms of support, and ways whereby they interacted to shape the resettlement experiences of Syrian refugees in the United States. Face-to-face and telephone interviews were conducted. The participants were adult Syrian refugees who resettled in the United States following the war in Syria in 2011 and provided their informed consent. Fourteen interviewees participated (four females and ten males) with a mean age of 40.4 ± 10.0 from Florida, New York, New Jersey, and Nebraska. Principles of interpretative phenomenological analysis were employed. Six main themes and thirteen subthemes were generated. The main themes were the United States refugee policy, separation from family, deficient support, deficient preparatory programs, lack of psychological support, and the language barrier. The lack of linguistic, social, economic, and psychological support found in the United States refugee policies exacerbated the participants' living difficulties. The sociocultural needs and characteristics of refugees need to be acknowledged. Support programs need to incorporate sociocultural strategies and training activities on daily living skills. The United States refugee allocation policy and focus on early economic self-sufficiency need to be re-evaluated.

Abstract:The involvement of refugee parents in their children’s education is crucial for academic success and social integration. However, school personnel often seem to struggle to find approaches that will increase recently arrived refugee parents’ involvement with the school. While refugee parents are not a homogenous group, many face uniquely similar and intersecting challenges. Using a qualitative research approach, specifically semi-structured interviews, this study examines the perceptions of one group of Arabic-speaking, recently arrived refugee parents’ perceptions of being and becoming involved in their children’s education after their arrival in Canada. The findings suggest the following factors act as barriers to their involvement: (1) limited language proficiency, (2) competing basic needs, (3) lack of homework, (4) teachers’ limited cross-cultural and interreligious understanding, and (5) sexual health education being a contested shared space. While the findings illuminate what some recently arrived refugee parents regard as barriers, the discussion challenges educational leaders and policy makers to critically question the dominant, normative model of parental involvement that is employed and recommends they reframe their conceptions of parental involvement as social engagement with a school community. Abstract:Transcultural Psychiatry, Ahead of Print. This prospective study examined the psychosocial adaptation of a community sample of newly resettled Syrian refugees in Canada (N = 235). Specifically, depressive symptoms, perceived stress, and perceived control were collected in Arabic at baseline and 1-year follow-up. Two theory-informed, cross-lagged panel models demonstrated that higher baseline depressive symptoms predicted lower perceived self-efficacy and lower perceived control at 1-year follow-up. Similarly, baseline depressive symptoms were concurrently correlated with higher perceived helplessness, lower perceived self-efficacy, and lower perceived control. Secondary regression analyses further demonstrated that baseline depressive symptoms predicted lower perceived social support and higher anxiety symptoms, though neither were assessed at baseline. Empirical results identify a potentially broad, precipitating, and persistent effect of depressive symptoms on Syrian refugees' psychosocial resources and adaptation post-migration, which is consistent with both the transactional model of stress and coping and the self-efficacy theory of depression, respectively. Clinically, the study results highlight the importance of early screening for depressive symptoms among refugee newcomers within a culturally and trauma-informed, integrated health setting. Furthermore, this study underscores the value and need for theoretically guided longitudinal studies to advance future research on refugee mental health and psychosocial adaptation.

Abstract:Newcomers (immigrants, refugees, and international students) face many personal, gender, cultural, environmental and health system barriers when integrating into a new society. These struggles can affect their health and social care, reducing access to mental health care. This study explores the lived experiences of African and Middle Eastern newcomers to Ontario, Canada. An understanding of newcomer integration challenges, successes and social justice issues is needed to improve health equity and social services. In this qualitative study, we used a participatory research approach to collect stories reflecting participants' integration perspectives and experiences. Beginning with our immigrant community network, we used snowball sampling to recruit newcomers, ages 18 to 30, originating from Africa or the Middle East. We used qualitative narrative analysis to interpret stories, identifying context themes, integrating related barriers and facilitators, and resolutions and learnings. We shared our findings and sought final feedback from our participants. A total of 18 newcomers, 78% female and approximately half post-secondary students, participated in the study. Participants described an unknown and intimidating migration context, with periods of loneliness and isolation aggravated by cold winter conditions and unfamiliar language and culture. Amidst the struggles, the support of friends and family, along with engaging in schoolwork, exploring new learning opportunities, and participating in community services, all facilitated integration and forged new resilience. Community building, friendships, and local services emerged as key elements for future immigrant service research. Utilizing a participatory health research approach allowed us to respond to the call for social justice-oriented research that helps to generate scientific knowledge for promoting culturally adaptive health care and access for marginalized populations.

Abstract:Immigrant and refugee populations experience life stressors due to difficult migration journeys and challenges in leaving one country and adapting to another. These life stressors result in adverse mental health outcomes when coupled with a lack of adequate support-enhancing resources. One area of support is access to and use of mental health services to prevent and address mental health concerns. Immigrant service providers in Canada support the integration and overall well-being of newcomers. This study focuses on immigrant service providers' perceptions of access to and use of mental health services for immigrants and refugees in Alberta. A qualitative descriptive design was used to collect and analyse the perspectives of 53 immigrant service providers recruited from nine immigrant serving agencies in Alberta between November 2016 and January 2017. Data were collected using a combination of individual interviews and focus groups, followed by thematic data analysis to identify relevant themes. Barriers to access and use of mental health services include language barriers, cultural interpretations of mental health, stigma around mental illness, and fear of negative repercussions when living with a mental illness. Strategies to improve mental health service delivery include developing community-based services, attending to financial barriers, training immigrant service providers on mental health, enhancing collaboration across sectors in mental health service delivery, and advancing the role of interpreters and cultural brokers. Overall, immigrant service providers present a nuanced view of the complex and inter-related barriers immigrants and refugees experience and identify potential approaches to enhancing mental health service delivery.

Abstract:Refugee mothers are vulnerable to cultural stereotyping and socioeconomic hardships when they migrate to a new country. This vulnerability often has a negative impact on refugee mothers' breastfeeding practices. Saskatchewan is one of the growing provinces in Canada that has a noticeable increase in refugee population with young children and limited availability of healthcare settings with baby-friendly status. Considering existing gaps in knowledge, this critical ethnographic study aimed to explore barriers that impede the breastfeeding practices of refugee mothers in Saskatchewan. After seeking ethics approval, data were collected using multiple methods, including in-depth interviews undertaken with 27 refugee mothers with young children of age range 1 day to 24 months, a review of media communications and field observations of community-based services/facilities available to refugee mothers. Findings suggest that psychosocial barriers, healthcare barriers, environmental barriers, and maternal and child health-related barriers impede the breastfeeding practices of refugee mothers in Saskatchewan. Breastfeeding practices of refugee mothers can be promoted through healthcare support, culturally appropriate services, interpretation services in healthcare settings, implementation of baby-friendly initiatives, hospital and community-based breastfeeding campaigns, and follow-up services. Collaborative efforts by healthcare settings, healthcare providers, policymakers, public health agencies, service providers, and governments are essential to support the breastfeeding practices of refugee mothers.

Abstract:Background Refugees resettled in Australia may experience significant physical, mental and emotional health issues on arrival and difficulty accessing mainstream healthcare that often demands specialised services. It is not known if and how refugee health needs and service use change over time and generations, how this compares with the broader Australian population and what level of resourcing is required to maintain specialised services. There is also a significant knowledge gap concerning the resources and skills of refugees that can be harnessed to sustain the health and well-being of individuals and communities. Such knowledge gaps impede the ability of the health system to deliver responsive, efficient, acceptable and cost-effective care and services and limit the engagement of refugees in the coproduction of these services. Methods This study will be the first to provide comprehensive, longitudinal, population-based evidence of refugee health, service use and the accumulated resources or assets related to positive health and well-being (compared with data on deficits, illness and death) across the lifespan and generations. This will enable a comprehensive understanding of the relationships among assets, health status, service gaps and behaviours. We will identify the assets contributing to increased capacities to protect and promote health. This evidence is essential for planning health prevention programmes. This project has three phases: (1) employ national linked datasets to examine the health and social outcomes of refugees in Australia; (2) engage with refugees in a participatory manner to map the social, economic, organisational, physical and cultural assets in their communities and deliver an integrated model of health; and (3) codesign a roadmap of agreed actions required to attain health and well-being in communities and indicators to assess outcomes. Ethics and dissemination Ethics and procedures—phase I: Ethical approval for phase I was gained from the Australian Bureau of Statistics (ABS) for Person Level Integrated Data Asset microdata (unit record data) via the ABS DataLab and the NSW Population and Health Services Research Ethics Committee (2023ETH01728), which can provide a single review of multijurisdictional data linkage research projects under the National Mutual Acceptance Scheme. This will facilitate approval for the Victorian and ACT datasets. The ABS will be the integrating/linkage authority. The Centre for Health Record Linkage (CHeReL) and the Victorian Data Linkage Unit will prepare a data extract representing all data records from the dataset to provide to the ABS for linkage. Ethics and procedures—phases 2 and 3: Written consent will be obtained from all participants, as well as consent to publish. We have obtained ethical approval from the University of Technology Sydney Medical Research Ethics Committee; however, as we deepen our consultation with community members and receive input from expert stakeholders, we will likely seek amendments to hone the survey and World Café questions. We will also need to provide flexible offerings that may extend to individual interviews and online interactions. Discussion This innovative approach will empower refugees and put them at the centre of their health and decision-making.

Empowering Refugee Voices: Using Nominal Group Technique with a Diverse Refugee Patient Advisory Committee (PAC) to Identify Health and Research Priorities

Abstract:Background: Despite rising forced displacement globally, refugees health and research priorities are largely unknown. We investigated whether a diverse refugee committee could utilize participatory methods to identify health priorities and a research agenda to address them. Methods: We conducted a qualitative study with focus groups of current and former refugees, asylum claimants and evacuees from a specialized refugee clinic over a year. We collected sociodemographic data using standardized instruments, then utilized a four step nominal group technique process (idea generation, recording, discussion, and voting) to identify and rank participants health and research priorities. Participants ranked their top five priorities across three time periods: Pre migration/early arrival (0 to 3 months), post migration (3 months to 2 years), and long term health (beyond 2 years). Participants created overarching priorities and corroborated findings via a member checking step. Findings: Twenty three participants (median age 35 years) attended one or more of five focus groups. Twenty one completed sociodemographic surveys: 16/21 (76%) were women, representing 8 countries of origin. Participants identified more family physicians and improving health system navigation (11/60 votes each) as top health and research priorities respectively across all resettlement periods. Participants also prioritized pre departure healthcare system orientation and improved post arrival and long term mental health services. Twelve participants completed the member checking process, affirming the results with minor clarifications. Interpretation: This proof of concept study illustrates how refugees can use a rigorous consensus process without external influence to prioritize their healthcare needs, direct a health research agenda to address those needs, and co produce research. Abstract:Research teams in high-income countries often fail to acknowledge the capacity and contributions of Community Researchers. This qualitative exploratory study used decolonising methodology and the Foundation House 'Refugee Recovery Framework' to understand Community Researchers' perceptions and experiences of their role, and how research teams can integrate the knowledge they bring into research. Purposive sampling was used to facilitate the recruitment of eight Community Researchers from five different community groups working in Melbourne, Victoria. Semi-structured interviews lasting forty to sixty minutes occurred between December 2020 and January 2021. Data were analysed using reflexive thematic analysis. Findings reported in this paper include eight themes: 'nothing about us without us'; 'open the door'; a safe space to share; every step of the way; this does not translate; finding the right way to ask; a trauma-informed approach; and support within the workplace. The knowledge obtained demonstrates that Community Researchers facilitate meaningful participation in research for women, families, and communities of refugee or migrant background. Community Researchers' presence, knowledge, and skills are vital in establishing culturally safe research practices and developing accessible language to facilitate conversations about sensitive research topics across multiple languages. Community Researchers can make important contributions at all stages of research, including data collection and interpretation.
Refugee Healthcare Resilience and Burdens: A 10-year Mixed-Methods Analysis of System Shocks in Canada

Abstract:Background: System shocks, including sudden policy changes, refugee surges and pandemics, strain healthcare systems. These shocks compound existing vulnerabilities in refugee healthcare, limiting ability to provide patient care, but can also catalyze resilient adaptations. Investigating how local refugee health systems respond to shocks is critical to understanding resilience. Methods: We conducted a sequential explanatory mixed–methods study (2011–2020) at a specialized refugee health centre in Alberta, Canada, investigating four health system shocks: IFHP Funding Cuts (2012), Syrian Surge (2015), Yazidi Resettlement (2017), and COVID–19 (2020). We analyzed patient sociodemographic characteristics, health center utilization, and healthcare provider supply, conducting interrupted time series analysis of mean monthly appointments (total, family physicians, specialists and multidisciplinary team) and rates of change. We adapted a Health System Resilience framework to thematically analyze interviews with centre leaders and integrated these findings with quantitative findings to assess resilience and operational burdens. Findings: From 2011 to 2020, 10,661 refugees from 106 countries attended 107,642 appointments. Mean monthly appointments rose from 455 to 2,208 (3.9–fold, p<0.01). Monthly appointments increased between IFHP and Syrian periods (610.8 to 937.9, p<0.01), but not between Syrian Surge and Yazidi Resettlement (p=0.29). During COVID–19, mean appointments remained stable (1,412.4 to 1,414.0, p=0.11), but additional monthly appointments rose from 6.3 to 110.4 (17.5–fold, p<0.01). Over ten years, mean provider hours increased from 320 to 736 (2.3–fold), and from 59.5 to 871.4 (14.6–fold) for family physicians and multidisciplinary team members. Qualitative analysis revealed resilience capacities but highlighted costs such as burnout, vicarious trauma, and financial strain. Integration showed the centre developed resilience but experienced notable operational burden. Interpretation: Over a decade, a specialized refugee health centre adapted to successive shocks, transforming into a beacon clinic. It demonstrated resilience through care expansion and innovation, but with notable costs, financially and to health worker wellbeing. Funding: None

Abstract:African refugee women resettled in the United States are exposed to multiple risk factors for poor mental health. Currently, no comprehensive framework exists on which to guide mental health interventions specific to this population. Through a community-based participatory research partnership, we interviewed N = 15 resettled African refugees living in Rhode Island. Here we (1) describe how meanings of mental health within the African refugee community vary from US understandings of PTSD, depression, and anxiety and (2) generate a framework revealing how mental health among participants results from interactions between social support, African sociocultural norms, and US norms and systems. Multiple barriers and facilitators of mental wellbeing lie at the intersections of these three primary concepts. We recommend that public health and medicine leverage the strength of existing community networks and organisations to address the heavy burden of poor mental health among resettled African refugee women.

Abstract:Context: The impact of forced migration on the mental health of refugee men is far-reaching and compounded by gendered masculinity, which shapes men's access to employment and other resources. A gap in knowledge exists on the broader determinants of refugee men's mental health. Methodology: Using community-based participatory action research and the arts-based method of photovoice, this study advances knowledge about the gendered impacts of forced migration from the perspective of (n = 11) Syrian refugee men in the Canadian context. Theoretical approaches of intersectionality and masculinity were applied to understand how power relations shape Syrian men's identities, their access to employment, and impacts on their mental health. Analysis and Results: Syrian men's identities were marginalized by working in low-wage jobs because of inequitable policies that favored Canadian experience and credentialing assessment processes that devalued their knowledge. Multiple and overlapping factors shaped Syrian men's mental health including language and literacy barriers, time and stage of life, isolation and loneliness, belonging and identity, and gender-based stress. Caring masculinities performed through fathering, cultural connection, and service-based work promoted agency, hope, and resilience. Conclusions: Public health and community-based pathways must adopt gender-responsive and intersectional approaches to policy and practice. Peer-based programs may mitigate harmful forms of masculinity and promote transformative change to support refugee men's mental health.

Primary health care utilization in the first year after arrival by refugee sponsorship model in Ontario, Canada: A population-based cohort study

Abstract:Background Canada’s approach to refugee resettlement includes government sponsorship, a pioneering private sponsorship model and a third blended approach. Refugees are selected and supported differently in each approach including healthcare navigation. Little is known about how well private sponsors facilitate primary care navigation and whether this changed during the large-scale 2015 Syrian resettlement initiative characterized by civic and healthcare systems engagement. Methods and findings Population-based cohort study of resettled refugees arriving in Ontario between April 1, 2008 and March 31, 2017, with one-year follow-up, using linked health and demographic administrative databases. We evaluated associations of resettlement model (GARs, Privately Sponsored Refugees [PSRs], and Blended-Visa Office Referred [BVORs]) by era of arrival (pre-Syrian and Syrian era) and by country cohort, on measures of primary care (PC) navigation using adjusted Cox proportional hazards and logistic regression. There were 34,591 (pre-Syrian) and 24,757 (Syrian era) resettled refugees, approximately half of whom were GARs. Compared with the reference group pre-Syrian era PSRs, Syrian PSRs had slightly earlier PC visits (mean = 116 days [SD = 90]) (adjusted hazard ratios [aHR] = 1.19, 95% CI 1.14–1.23). Syrian GARs (mean = 72 days [SD = 65]) and BVORs (mean = 73 days [SD = 76]) had their first PC visit sooner than pre-Syrian era PSRs (mean = 149 days [SD = 86]), with respective aHRs 2.27, 95% CI 2.19–2.35 and 1.89, 95% CI 1.79–1.99. Compared to pre-Syrian PSRs, Syrian GARs and BVORs had much greater odds of a CHC visit (adjusted odds ratios 14.69, 95% CI 12.98–16.63 and 14.08, 95% 12.05–16.44 respectively) and Syrian PSRs had twice the odds of a CHC visit. Conclusions Less timely primary care and lower odds of a CHC visit among PSRs in the first year may be attributed to selection factors and gaps in sponsors’ knowledge of healthcare navigation. Improved primary care navigation outcomes in the Syrian era suggests successful health systems engagement.

Abstract:Middle Eastern immigrant women (MEIW) living in Canada have significantly increased. However, this group of women is under-represented in health research, and there is a gap in knowledge about their experiences when they access healthcare services for cancer care in Canada. This qualitative approach was conducted to uncover the meaning of the lived experiences of MEIW with healthcare services in Canada during their cancer survivorship (CS). Data were collected through unstructured interviews and one written description from three MEIW. Data were analyzed using a descriptive phenomenological approach developed by Giorgi. Four themes emerged to represent the essence (or meaning) of the participants’ lived experiences. Their healthcare was accompanied with delays and unmet needs. Yet, they found it helpful when they were provided with knowledge and information. The ability to communicate in English was equal to empowerment for each of them, while they faced cultural stigmatization of mental health issues. Thus, healthcare professionals need to identify immigrant women’s unmet support needs and psychosocial responses during their cancer survivorship. Language-specific and culturally competent cancer-care intervention programs must be developed within the Canadian healthcare system.
