Abstract:Ear, Nose &Throat Journal, Ahead of Print. Histiocytic necrotizing lymphadenitis (HNL), also known as Kikuchi disease, is a relatively rare sub-acute necrotic localized lymphadenitis. This benign, self-limiting condition is characterized by fever, painful lymphadenopathy, skin rash, and other systemic symptoms. Due to its nonspecific presentation, unclear etiology, and pathogenesis, HNL has a low incidence rate in clinical practice. Insufficient awareness among clinicians and pathologists can easily lead to misdiagnosis. This article reported a 27-year-old female patient who was admitted to the hospital with fever, neck pain, and enlarged lymph nodes in the neck. There were no special medical or personal histories, and postoperative pathology confirmed tissue necrotizing lymphadenitis. After treatment with steroids and symptomatic therapy, she recovered and was discharged from the hospital. Follow-up to date has shown no recurrence.

The patient was a 34-year-old man presenting with fever, multiple scattered pustules on both upper limbs and several subcutaneous nodules on the left elbow and forearm. Skin biopsy of the subcutaneous nodule revealed suppurative inflammation of the lymph nodes. Pus cultures were finally identified as Nocardia brasiliensis . The patient fully recovered without relapse after receiving optimized antimicrobial therapy consisting of linezolid combined with sulfonamides. Nocardiosis is a rare opportunistic disease which may be fatal and usually affects immunocompromised hosts, resulting in suppurative and granulomatous inflammation. Nocardia has a long culture cycle, is difficult to diagnose, and is more likely to be neglected in healthy young people. The present case suggests that physicians should be aware that nocardiosis is a differential diagnosis to consider in patients with suppurative infection, especially when anti-infective treatment is ineffective.

Abstract:Variable interval (VI) responding was hypothesized to be a function of differential reinforcement susceptibilities of various unspecified behavior chains that mediate interresponse times (IRTs). To test this hypothesis, probabilities of reinforcement were regulated for the lengths of chains of key pecking responses of pigeons, analogous to the way that VI regulates probabilities of reinforcement for IRTs. This procedure generated a number of VI-like effects, supporting the notion that VI behavior can be construed as a special case of an interaction between the organism's function relating reinforcement susceptibilities to chain length and the experimenter's function relating probabilities of reinforcement to chain length.

Abstract:Epstein-Barr virus (EBV) plays an important role in various diseases. EBV-associated lymphoproliferative disease (LPD) is a rare disease with a canceration tendency. It is difficult to differentiate LPD with involvement of digestive tract from Crohn disease due to similar clinical and endoscopic manifestations. We present a case report of multiple ulcers with esophagus, small bowel and the entire colon involved, proved to be NK-Cell LPD, developed into EBV-associated NK/T Cell lymphoma, in an immunocompetent man who was initially misdiagnosed as Crohn disease. This report underscores that intestinal ulcers should be cautiously diagnosed, for it sometimes could be a precancerous lesion.
A Secret Path: Severe Deep Neck Space Infection Led To Axilla And Lateral Chest Abscesses

Abstract:Background: Deep neck space infections (DNSI) are significantly less common today than in the past. But acute severe neck abscesses are still associated with significant morbidity and mortality rates due to lack of seasonable diagnosis and treatment. Case Report: we present a rare patient with severe deep neck space infection, the abscesses spread a covert way led to axilla and lateral chest abscesses through the pre-vertebral space. Following multiple surgical procedure and antimicrobial therapy, the patient made a full recovery. Conclusion: Severe deep neck space infections usually spread to the mediastinum, anterior chest wall easily. However, because of the most insidious pre-vertebral space, the axilla and left lateral chest wall also should to be paid attention in clinical. Using corticosteroids on patient with deep neck space infections should be cautious, or else it will make the infection spread and deteriorate. Multidisciplinary of diagnosis and therapy in severe deep neck space infections may recognize again by Clinicians.

Abstract:Majority of higher plants are outbreeding, highly heterozygous and undergo a long developmental period before reaching their reproductive stage. Traditional breeding and cross- pollinating procedures are both unpredictable and time-consuming. Haploids are plants with a gametophytic chromosome number and doubled haploids are haploids that have undergone chromosome duplication, represent a particularly attractive biotechnological method to accelerate plant breeding. They can occur either spontaneously or can be induced by modified pollination methods in vivo, or by in vitro culture of immature male or female gametophytes. Biotechnologies provide powerful tools for plant breeding, and among these ones, tissue culture, particularly haploid and doubled haploid technology, can effectively help to select superior plants. In vitro haploid production is, thus, the most prolific and desirable approach of haploid production. This review describes the range of techniques available for the isolation or induction of haploids by in vivo or by in vitro, estimation of haploid lavel either through chromosome counting or flowcytometry and also discourse the value of haploids.

Abstract:Necrotizing fasciitis (NF) is a life-threatening disease with high mortality and rapidly progressive clinical manifestations1. Early detection and surgical management coupled with antibiotic treatment are crucial for the survival, and the patient survival is heavily dependent on clinical decisions2,3. However, it is not widely known that NF does not always follow a typical clinical course, and there have been no case reports of NF following an atypical clinical course. Although the course of the disease depends on the individual patient, it remains a challenge for physicians to determine the precise timing when patients are most likely to survive multiple surgical interventions. We encountered a challenging case presenting with an atypical clinical course. We herein report a 31-year-old man who followed a deteriorating biphasic-like clinical course and presented with extensive NF and streptococcal toxic shock syndrome due to Group A Streptococcus. This case serves to inform physicians of the existence of NF with an atypical and deteriorating biphasic-like clinical course, emphasizing the need for a careful evaluation of the patient condition.

Abstract:To the Editor: Neurolymphomatosis (NL) is a clinical disorder that presents with peripheral neuropathy due to lymphomatous infiltration of the nerves. NL is generally associated with B-cell lymphomas, and T-cell lymphoma is extremely rare. Nasal type extranodal natural killer (NK)/T-cell lymphoma (ENKL) is a rare lymphoma associated with Epstein-Barr virus (EBV), which is more common in East Asia than in the West.[1] Central nervous system (CNS) lesions are occasionally observed as sites of ENKL involvement, and extranodal involvement often leads to an unfavorable prognosis.[23] Here, a rare case of NL associated with ENKL that mainly presented as progressive multiplex painful mononeuropathy with multiple white matter lesions is reported. A 26-year-old woman referred to our hospital after presenting asymmetrical numbness, pain, and weakness of bilateral legs with an intermittent fever for 3 months. The patient had experienced significant weight loss, but her prior medical history was not significant. Physical examination revealed patchy erythema with blisters on her distal arms. Neurological examination revealed paralgesia in the left planta, distal arm, outer calf, and inside of the right calf. The left Achilles tendon reflexes decreased. The left upper limb muscle strength was 2/6 distally and 3/6 proximally. Positive Babinski's sign was observed on the left side. The patient was considered to have possible CNS involvement, multiplex mononeuropathy, and skin lesions. The laboratory findings included slight anemia (hemoglobin 108 g/L; normal: 115–150 g/L), thrombocytopenia (platelet [PLT]: 100 × 109/L; normal: 125–350 × 109/L), an elevated lactate dehydrogenase level (lactate dehydrogenase: 276 U/L; normal: 125–225 U/L), and an extremely elevated ferritin level (>572 µg/L; normal: 10–140 µg/L). The cerebrospinal fluid was not remarkable. Electromyography revealed diminished or no compound muscle action potential and sensory nerve action potential amplitude in the left median and ulnar nerves and the right tibialis anterior nerves, indicating multiplex mononeuropathy with axonal damage. Two courses of high-dose dexamethasone therapy resulted in transient improvement of the patient's temperature and ne urological symptoms, while the thrombocytopenia worsened (PLT: 51 × 109/L). Bone marrow puncture revealed some atypical lymphocytes, but flow cytometry was negative. Magnetic resonance imaging of the brain revealed multiple white matter lesions with no enhancement [Figure 1a and 1b]. An 18F-fluoro-2-deoxyglucose positron emission tomography (FDG-PET) scan demonstrated extensive involvement of the left brachial plexus nerve, right tibial nerve, left median nerve, left ulnar nerve, CNS, muscle, nasopharynx, and skin [Figure 1c]. Finally, skin biopsy on the left arm showed infiltration of lymphomatous cells [Figure 1d–1h]. The lymphoma cells were positive for EBV-encoded RNA upon in situ hybridization [Figure 1i]. NL is an uncommon manifestation of lymphoma characterized by infiltration of the peripheral nerves by malignant cells. However, neuropathy can be the first manifestation, as observed in the present case. Diagnosing NL is clinically challenging. FDG-PET appears to be a high-sensitive diagnostic method for identifying NL, with a positivity rate as high as 87.5–91.0%.[45] Intense FDG uptake in the involved nerve, as observed in this case, is a typical sign of NL, as indicated in a previous report.[5] Approximately 80% of the reported NL cases originate from B-cells.[4] However, NK cell lymphoma is very rare, and the present case was finally diagnosed as ENKL pathologically. ENKL is an aggressive lymphoma preferentially occurring at unusual sites, including the CNS, skin, nasopharynx, and similar areas. It deteriorates rapidly despite active treatment. In conclusion, NL is a possible cause of progressive multiplex painful mononeuropathy. The present case suggests that NL can occur with ENKL. Financial support and sponsorship Nil. Abstract:Retinoic acid (RA) is a critical signaling molecule that regulates gene transcription and the cell cycle. Understanding of RA signaling has increased dramatically over the past decades, but the connection between whole body RA homeostasis and gene regulation in individual cells is still unclear. It has been proposed that cytochrome P450 family 26 (CYP26) enzymes have a role in determining the cellular exposure to RA by inactivating RA in cells that do not need RA. The CYP26 enzymes have been shown to metabolize RA efficiently and they are also inducible by RA in selected systems. However, their expression patterns in different cell types and a mechanistic understanding of their function is still lacking. Based on preliminary kinetic data and protein expression levels, one may predict that if CYP26A1 is expressed in the liver at even very low levels, it will be the major RA hydroxylase in this tissue. As such, it is an attractive pharmacological target for drug development when one aims to increase circulating or cellular RA concentrations. To further the understanding of how CYP26 enzymes contribute to the regulation of RA homeostasis, structural information of the CYP26s, commercially available recombinant enzymes and good specific and sensitive antibodies are needed.

Abstract:N. neocaledoniensis is a very rare infectious pathogen that causes human disease, particularly in immunocompromised individuals. In this case report, we describe the successful diagnosis of N. neocaledoniensis in a patient confirmed by mNGS and the treatment of multiple skin abscesses due to N. neocaledoniensis infection. mNGS is an important diagnostic method complementary to routine bacterial culture and identification methods, especially for rare, novel, co-infected pathogens, and pathogens that are difficult to culture. This report may provide a reference for the clinical treatment and diagnosis of N. neocaledoniensis infection in humans.

Case Report: Therapeutic Strategy with Delayed Debridement for Culture-Negative Invasive Group A Streptococcal Infections Diagnosed by Metagenomic Next-Generation Sequencing

Abstract:Streptococcal toxic shock syndrome (STSS) caused by group A streptococcus is a rare condition that rapidly developed to multiple organ failure even death. Therefore, prompt diagnosis, initiate appropriate antibiotics and other supportive treatments are critical. Here we reported a case of STSS caused by group A streptococcus infection. A healthy 39-year-old man presented a sudden pain in the left lower extremity, followed by a high fever (40.0 °C) with dizziness, nausea, and shortness of breath. Twenty-four hours before the visit, the patient showed anuria. The patient was then admitted to the intensive care unit. Blood examination revealed elevated levels of inflammatory markers and creatinine. He suffered from septic shock, dysfunction of coagulation, acute kidney dysfunction, acute respiratory distress syndrome, and acute liver function injury. The diagnosis was obtained through clinical manifestation and metagenomic next-generation sequencing (mNGS) drawn from the pustule and deep soft tissue (lower limb) samples while all bacterial cultures came back negative. The pustule mNGS report detected a total of 132 unique group A streptococcus sequence reads, representing 96.3% of microbial reads while the soft tissue mNGS report identified a total of 142474 unique group A streptococcus sequence reads, representing 100% of microbial reads. The patient was treated with aggressive fluid resuscitation, antibiotics comprising piperacillin/tazobactam and clindamycin, respiratory support, following the delayed surgical debridement. Intravenous immunoglobulin was also used for 5 days. On the 14th day after admission, he was transferred to the general ward for follow-up treatment. Our case highlighted, for the first time, the key role of mNGS in the early diagnosis of culture-negative invasive group A streptococcal infection. The case also suggested that clindamycin combined with beta-lactam antibiotics and adjunction of intravenous immunoglobulin therapy with delayed debridement performed well in the management of unstable STSS patients.

Abstract:OBJECTIVE:To investigate and analyze the clinical features in patients with histiocytic necrotizing lymphadenitis (HNL). METHODS:A total of 68 HNL patients at our hospital between the years of 1999 to 2009 were enrolled. The clinical data were collected from the hospital records and the relevant literature was reviewed. RESULTS:HNL mainly affected young people with an average age of (19 ± 13) years old and a female-to-male ratio of 1:1. All patients had lymphadenectasis. And 95.6% patients (65 cases) were feverish and 36.8% patients (25 cases) had mild hepatosplenomegaly; 25.0% (17 cases) upper respiratory symptoms such as sore throat and cough; 14.7% (10 cases) skin rash in their history; 51.5% (35 cases) leucopenia; 25.0% (17 cases)hepatic dysfunctions; 72.1% (44/61) elevated erythrocyte sedimentation rate (ESR); 11.1% (6/54) positive antinuclear antibody (ANA). The final diagnosis of HNL was confirmed by pathological examination and immunohistochemical staining of biopsy specimens. And 34 (50.0%) patients received glucocorticoid for 2 weeks to 3 months. Seven (10.3%) patients relapsed in which glucocorticoid was effective. Of 6 patients with positive ANA, one case was complicated with systemic lupus erythematosus (SLE) and another case diagnosed with SLE at 2 years after HNL. CONCLUSIONS:The clinical manifestations of HNL lack specificity so that it can be easily misdiagnosed. While the etiology of HNL remains elusive, the histopathological examination of affected lymph nodes has contributed greatly to the final diagnoses of HNL. Glucocorticoid therapy is recommended for treatment. Generally, HNL has an excellent prognosis but it often relapses. It should be noted that HNL may coexist with SLE or evolve ultimately into SLE.
The evaluation of tension in an experimental model of external fixation of distal radius fractures.

Abstract:A cadavaric model was used to evaluate the previously reported methods of determining excessive tension when applying an external fixator across the wrist. An osteotomy of the distal radius was performed and tension was applied incrementally across the joint in 9 cadaveric specimens. The fingers of each specimen could fully flex to the palm at all levels of tension tested. Although the radiocarpal and midcarpal joint spaces did lengthen with incremental changes in the amount of tension, there was no statistically significant correlation that could be made about the difference between the radiocarpal and midcarpal spaces and tension applied across the wrist. Although the carpal height ratio increased significantly from 0 to 10 lb of traction, the carpal height ratio appeared to plateau after further increases in tension. In conclusion, surgeons need to be careful when using any of these previously reported techniques for determining the optimal tension to be applied with an external fixator across the wrist. These techniques may not reproducibly allow the surgeon to detect whether there is excessive distraction across the distal radius fracture.

Abstract:OBJECTIVE:To explore the causes, clinical features, diagnosis and treatment of cervical extensive necrotizing fasciitis, a rare clinical occurrence, and to improve the clinical recognition and appreciation of it.METHOD:Two cases of cervical extensive necrotizing fasciitis were studied and relevant literatures were reviewed. The causes, clinical manifestation, experience of diagnosis and treatment were summarized.RESULT:One of two cases was secondary to foreign body of hypopharynx, and the other with unknown cause. Apathy, crepitation and diffuse swelling and rubor following with abscess formation on the neck are main characteristics. Mixed synergistic infection was confirmed by drainage culturing. All two cases were treated actively by large dosage and effective broad spectrum antibiotics, and sustaining therapy and surgical treatment, including local incision and drainage, aggressive surgical debridement and tracheotomy.CONCLUSION:Cervical extensive necrotizing fasciitis is a potentially life-threatening soft tissue infection. The keys of successful treatment were early diagnosis and surgical intervention. Rational antibiotics application and systemic supporting therapeutics were also recommended.

Abstract:A patient with comorbid diabetes mellitus, obesity, and hypertension acutely presented to the ED due to labial cellulitis with rapidly progressing symptoms of systemic inflammation. Clinical examination revealed fever and groin pain that was tender to palpation. Initial contrast-enhanced CT scans showed labial cellulitis extending to the inguinal canal, with later CT imaging findings of subcutaneous air indicative of necrotizing fasciitis (NF). Antimicrobial therapy was initiated empirically and later tailored to culture antibiogram. The patient underwent acute surgical abscess drainage and tissue debridement but was transferred to the surgical intensive care unit (SICU) due to postoperative blood loss and hypotension. Two additional surgical procedures were needed before sufficient drainage was achieved, and Eggerthia catenaformis (E. catenaformis) was isolated from all samples. Due to the extent of the infection, the patient was admitted for a total of 16 days, with five days spent in the SICU. They recovered completely due to adequate surgery and antimicrobial therapy for a total of 24 days. Here, we present the third reported case of NF due to E. catenaformiswhile emphasizing timely treatment with empiric antibiotics and surgical intervention.

Abstract:The features of scattered and transmitted light by dilute suspensions of transparent submicron particles are investigated both in the spectral and in the perceived colorimetric domains, as a function of effective particle diameter D, particle-host refractive-index mismatch m, and scattering angle θ. Our results show that the wavelength λ-dependence of the scattering and extinction cross sections remains quite similar well beyond the Rayleigh regime up to particle sizes of a few hundreds nm, but only for specific scattering angles that depend on D and m, and tend to 90° on approaching the Rayleigh regime. Close to this limit (D/λ<<1), a simple criterion that relates the perceived scattering color at θ=90° and the ratio of the sample extinction coefficients at two properly selected wavelengths is demonstrated. A comparison between computed and measured data is presented.

Abstract:Tonsillitis, the inflammation of the palatine tonsils, is a common cause of throat pain. It is most often due to viruses but can also be related to bacteria. Necrotizing tonsillitis is a rare complication of acute tonsillitis with relatively few cases reported in the literature. We present a case of necrotizing tonsillitis in a patient with mononucleosis related to infection by Epstein-Barr virus, with superinfection with Streptococcus pyogenes, or group A streptococcus. To our knowledge, this is the first case of this nature to be reported in the medical literature. While the role of radiologic imaging is generally of limited diagnostic utility in cases of uncomplicated tonsillitis, its use was invaluable in ascertaining the nature and scope of necrotizing tonsillitis in this case. Further research remains to determine whether viral or bacterial superinfection can be used to further stratify patients who present with sore throat.
