Abstract:Background: Hypnic headache is a neurological disorder characterized by recurrent headache attacks that occur exclusively during sleep, leading to awakening. Synthesizing the available epidemiological data might inform clinical decision-making. Methods: We searched PubMed and Embase for observational studies on hypnic headache published between 1 May 2004, and 22 December 2022. Two investigators independently screened titles, abstracts, and full-text articles. We performed a random-effects meta-analysis with meta-regression to estimate the prevalence of hypnic headache and its clinical features based on epidemiologic data from population-based and clinic-based studies. Results: Fourteen studies, one population-based and 13 clinic-based, met our eligibility criteria. The population-based study did not identify any people with hypnic headache. From 11 clinic-based studies, the pooled relative frequency of hypnic headache was 0.21% (95%CI, 0.13 to 0.35%; I2 = 87%) in adult patients evaluated for headache. The pooled mean age of onset was 60.5 years, with a slight female predisposition. Hypnic headache was typically bilateral (71%), pressing (73%), of moderate (38%) or severe (44%) pain intensity, and lasted about 115 minutes per attack. Conclusions: Our data should be cautiously interpreted due to between-study heterogeneity. The identified clinical presentation of hypnic headache can guide clinical diagnosis, in addition to the International Classification of Headache Disorders.
Narcolepsy and Other Central Hypersomnias

Abstract:Purpose of review: This article focuses on the clinical presentation, pathophysiology, diagnosis, differential diagnosis, and management of narcolepsy type 1 and narcolepsy type 2, idiopathic hypersomnia, Kleine-Levin syndrome, and other central disorders of hypersomnolence, as defined in the International Classification of Sleep Disorders, Third Edition (ICSD-3). Recent findings: In ICSD-3, the names of some central disorders of hypersomnolence have been changed: narcolepsy with cataplexy and narcolepsy without cataplexy have been renamed narcolepsy type 1 and narcolepsy type 2, respectively. A low level of hypocretin-1/orexin-A in the CSF is now theoretically sufficient to diagnose narcolepsy type 1, as it is a highly specific and sensitive biomarker. Conversely, other central hypersomnias are less well-defined disorders with variability in the phenotype, and few reliable biomarkers have been discovered so far. The epidemiologic observation that influenza A (H1N1) infection and vaccination are potential triggering factors of narcolepsy type 1 (discovered during the 2009 H1N1 pandemic) has increased interest in this rare disease, and progress is being made to better understand the process (highly suspected to be autoimmune) responsible for the destruction of hypocretin neurons. Treatment of narcolepsy remains largely symptomatic, usually initially with modafinil or armodafinil or with higher-potency stimulants such as methylphenidate or amphetamines. Several newer wake-promoting agents and psychostimulants have also been developed, including sodium oxybate, which has a role in the treatment of cataplexy and as an adjunctive wake-promoting agent, and pitolisant, a selective histamine H3 receptor inverse agonist that is currently only available in Europe. Summary: Although far less common than many other sleep disorders, central hypersomnias are among the most severe and disabling diseases in the field of sleep medicine, and their early recognition is of major importance for patients, especially children, to maximize their quality of life and functioning in activities of daily living.

Abstract:Idiopathic hypersomnia was first described in 1976 under two forms: polysymptomatic, characterized by excessive daytime sleepiness, long and unrefreshing naps, nocturnal sleep of abnormally long duration and signs of sleep drunkenness upon awakening; monosymptomatic, manifested by excessive daytime sleepiness only. Yet, after 45 years, this sleep disorder is still poorly delineated and diagnostic criteria produced by successive International Classifications of Sleep Disorders are far from satisfactory. The first part of this review is a historical account of the successive names and descriptions of idiopathic hypersomnia: monosymptomatic and polysymptomatic idiopathic hypersomnia in 1976; central nervous system idiopathic hypersomnia in 1979; idiopathic hypersomnia in 1990; idiopathic hypersomnia with and without long sleep time in 2005; idiopathic hypersomnia again in 2014; and, within the last few years, the proposal of separating idiopathic hypersomnia into a well-defined subtype, idiopathic hypersomnia with long sleep duration, and a more heterogeneous subtype combining idiopathic hypersomnia without long sleep duration and narcolepsy type 2. The second part is a critical review of both current ICSD-3 diagnostic criteria and clinical features, scales and questionnaires, electrophysiological and circadian control tests, research techniques, currently used to diagnose idiopathic hypersomnia. The third part proposes a diagnostic evaluation of idiopathic hypersomnia, in the absence of biologic markers and of robust electrophysiological diagnostic criteria.

Abstract:Migraine with and without aura, cluster headache, hypnic headache, and paroxysmal hemicranias are each reported as intrinsically related to sleep. Chronic migraine, chronic tension-type headache, and medication overuse headache may cause sleep disturbance. Otherwise, both headache and sleep disorder may be manifestations of a same systemic dysfunction. There is a vicious cycle linking sleep disorders and migraine. The poor quality or poor duration of sleep could be a trigger of migraine attack and migraineurs with poor sleep reported a higher headache frequency. Moreover, coping behaviors of migraineurs (e.g., going to sleep early to relieve migraine attacks) can be factors precipitating and perpetuating sleep disturbances themselves. During cluster headache, patients report a poor quality of sleep correlated with the amount of daylight. In particular, it was demonstrated that melatonin levels have influences on cluster headache attacks. Concerning the pathophysiology of hypnic headache, it has been hypothesized a possible role of obstructive sleep apnea in triggering nocturnal attacks: an increased number of apnea episodes has been reported in hypnic headache patients, but a lack of a temporal correlation of headache attacks with the drop of oxygen saturation has been observed. Tension-type headache is the most common headache with sleep dysregulation (lack of sleep or oversleeping) frequently reported as a triggering factor for acute attacks: management of sleep disturbances seems crucial in this form of headache.

Purpose: One-third to one-half of ambulatory care patients report insomnia. The objective of this clinical review is to detail the many causes and evidence-based treatment options for insomnia in outpatients and to recommend screening and summarize evidence for the place in therapy of prescription and nonprescription treatments. Summary: This work provides an overview of the literature on insomnia regarding causes, patient assessment, and nonpharmacological and pharmacological treatments. Patients who present with insomnia should be assessed for sleep apnea, restless legs syndrome, narcolepsy, and all contributing medications as well as medical, psychiatric, and substance use disorder diagnoses. The type of insomnia, namely difficulty falling asleep, difficulty maintaining sleep, and early morning awakening with resulting functional impairment, should be documented in addition to whether insomnia is short term or persistent. Cognitive behavioral therapy for insomnia (CBT-I) or digital CBT-I is first-line treatment for all patients with insomnia irrespective of the cause or type. Nonprescription treatments such as antihistamines or melatonin are for select populations. Prescription hypnotics are best utilized on an as-needed basis or for nightly use for less than 6 weeks. Z-hypnotics are safe and effective for insomnia in persons with depression or an anxiety disorder but should be avoided in older individuals or if there is respiratory or cognitive impairment. Orexin receptor antagonists are effective for sleep initiation and maintenance in healthy persons or if there is mild cognitive impairment, but they require further study in individuals with psychiatric and medical diagnoses. Trazodone is the most prescribed off-label treatment due to its efficacy for sleep initiation and maintenance and its lack of abuse potential. Conclusion: Insomnia treatment should be guided by patient age, diagnoses, and type of insomnia. Pharmacological treatments should be used at the lowest effective dose for the shortest duration of time.

Abstract:Abstract Insomnia is defined as difficulties of initiating and maintaining sleep, early awakening and poor subjective sleep quality despite adequate opportunity and circumstances for sleep with impairment of daytime performance. These components of insomnia – namely persistent sleep difficulties despite of adequate sleep opportunity resulting in daytime dysfunction - appear secondary or co-morbid to neurological diseases. Comorbid insomnia originates from neurodegenerative, inflammatory, traumatic or ischemic changes in sleep regulating brainstem and hypothalamic nuclei with consecutive changes of neurotransmitters. Symptoms of neurological disorders (i.e motor deficits), co-morbidities (i.e. pain, depression, anxiety) and some disease-specific pharmaceuticals may cause insomnia and/or other sleep problems. This guideline focuses on insomnias in headaches, neurodegenerative movement disorders, multiple sclerosis, traumatic brain injury, epilepsies, stroke, neuromuscular disease and dementia. The most important new recommendations are: Cognitive behavioral therapy (CBTi) is recommended to treat acute and chronic insomnia in headache patients. Insomnia is one of the most frequent sleep complaints in neurodegenerative movement disorders. Patients may benefit from CBTi, antidepressants (trazodone, doxepin), melatonin and gaba-agonists. Insomnia is a frequent precursor of MS symptoms by up to 10 years. CBTi is recommended in patients with MS, traumatic brain injury and. Melatonin may improve insomnia symptoms in children with epilepsies. Patients with insomnia after stroke can be treated with benzodiazepine receptor agonists and sedating antidepressants. For patients with dementia suffering from insomnia trazodone, light therapy and physical exercise are recommended.

The International Classification of Headache Disorders, 3rd edition provided the framework for the review of primary headache disorders (migraine, tension-type headache and cluster headache).ResultsAttack presentation: Migraine attacks exhibit significant circadian and infradian rhythms, influenced by hormonal levels, light sensitivity, and hypothalamic activation. Tension-type headache lacks clear chronobiological patterns, with limited understanding of its underlying mechanisms. Cluster headache displays a distinct circannual pattern, with attacks often occurring at night and relevant involvement of the hypothalamus. Disease characterization: Age of onset exhibits the earliest peak in migraine; frequency and typical features of primary headache disorders decrease over time.ConclusionThis comprehensive analysis of time patterns in primary headache disorders underscores their role in phenotyping, understanding and treating primary headache disorders, offering promising avenues for advancing and tailoring headache management.

Abstract:Purpose: The purpose of this paper is to review some of the causes of secondary headache particularly focusing on the subcategories of secondary headache in the International Classification of Headache Disorders, 3rd edition, the clinical features of these headaches, and their associated features and management. Overview: Headache attributed to trauma or injury to the head and/or neck, headache attributed to cranial or cervical vascular disorder, headache attributed to non-vascular intracranial disorder, headache attributed to a substance or its withdrawal, headache attributed to infection, headache attributed to disorder of homeostasis, and headache or facial pain attributed to disorder of the cranium, neck, eye, ears, nose, sinuses, teeth, mouth, or other facial or cervical structure are discussed in this paper. Discussion: Headache is a common symptom of multiple medical conditions. Although a minority of headache patients have a secondary basis for their headaches, it is important to identify clinical features of secondary headache disorders including both the headache and non-headache features of the condition, diagnose the secondary etiology correctly, and treat them appropriately.

Abstract:Sleeping disorders have been troubling human beings with their continuous involvement in increased stress-level activities. Despite the technological advancement of practices for its diagnosis, assessment and treatment, these have been posing challenges to medical science. The present paper therefore attempts to critically review the valuable research contributions of scholars, especially on two approaches for the treatment of insomnia: hypnotherapy and cranial electrotherapy stimulation (CES). The review framework operates on the themes of mechanisms of action, treatment outcomes and efficacy of these approaches. Following a systematic method for the inclusion and exclusion of research papers, the shortlisted 26 papers reveal helpful insights about the differential impact of the application of hypnotherapy and CES. Wherein, the former approach is observed to promote informed decision-making about the efficacy of insomnia management strategies, while CES seems to demand further research.

Abstract:Abstract Introduction Sleep-related hallucinations, encompassing visual and auditory experiences pose a complex clinical challenge. Here, we illustrate 2 cases with diverse underlying etiologies. Report of case(s) Case 1: 76-year-old male with 3 months of nocturnal visual hallucinations, occurring 3-5 nights per week. These ranged from familiar and unfamiliar faces and characters reminiscent of movies. Symptoms occurred when waking up in the middle of the night, with difficulty returning to sleep. Polysomnography (PSG) unveiled moderate obstructive sleep apnea (OSA) without findings indicative of parasomnias and no symptoms on the night of PSG. Brain MRI revealed chronic infarcts involving right basal ganglia and bilateral optic radiations. Lesions of structures that commonly cause visual hallucinations (optic lens, retina, thalamus, midbrain) were absent. The frequency of episodes decreased to 1-2 per month after initiating CPAP therapy. Potential mechanism of improvement could be PAP mediated optimization of perfusion to the ischemic penumbra. Case 2: 93-year-old male with an eight-year history of insomnia due to musical hallucinations (MH). Auditory experiences included church and semi-classical music, with 80% being familiar tunes. The patient, a former church choir member, has a history of progressive hearing loss and tinnitus. Brain MRI/MRA revealed chronic periventricular microvascular ischemic changes. Extended EEG showed no evidence of seizures. Audiometry confirmed moderate-severe bilateral sensorineural hearing loss. No response was seen to Levetiracetam, Gabapentin, Donepezil and behavioral therapy. Home sleep study revealed severe OSA. CPAP therapy did not result in improvement of MH. MH improved significantly with Lorazepam. Hypoacusis, psychiatric disorders, focal brain lesions, epilepsy and intoxication are common etiologies for MH. The etiology in this case is very likely due to hypoacusis. The hypothesis is deafferentation resulting in release phenomenon or spontaneous activity in the auditory pathway, manifesting as MH. Conclusion Sleep-related and nocturnal hallucinations warrant a comprehensive evaluation. Differential diagnoses include neurological causes, including stroke, seizure, Multiple Sclerosis, tumor, vascular lesions, and neurodegenerative disorders. Other etiologies include psychiatric disorders, medications, alcohol, otologic and ophthalmic pathologies. It is also reasonable to evaluate for comorbid OSA as appropriate treatment may decrease the frequency of episodes and improve overall sleep in some patients. Support (if any)

Abstract:Headache is one of the most common reasons for which people seek medical attention. Migraine, tension-type, and other primary headache disorders account for the majority of cases. Still, approximately 2–5% of patients presenting with headache will ultimately be diagnosed with secondary headache, many of which are serious and even life-threatening. This distinction between primary and secondary headache is the most important and often the most challenging step in the management of these patients. This chapter aims to review and clarify the worrisome headache “red flags” that can alert healthcare providers to a potential secondary etiology. The widely utilized mnemonic “SNOOP” was created over a decade ago to help providers remember these warning signs. As our knowledge base has increased, the mnemonic has gone through multiple iterations – most recently, “SNNOOP10” – but, for the sake of simplicity and clarity, we will use “SNOOPP” as our guide: systemic symptoms, neurologic signs and symptoms, onset, older age, pattern change, and positional quality. It is important to remember that when patients have a primary headache syndrome, most commonly migraine, they are more likely to develop a headache as a symptom of these new disorders. The headache they develop is often an amplification of the preexisting one.
Insights from a 10-year Australasian idiopathic hypersomnia patient data registry study

Abstract:Study objectives: Idiopathic hypersomnia (IH) is a disorder of central hypersomnolence that results in excessive daytime sleepiness in the absence of another identifying cause. Case studies from sleep clinic patients may not be a fair representation of the wider IH population. This study aims to better characterize patients diagnosed with IH in Australia and New Zealand using online patient-driven survey data. Methods: A retrospective analysis of 686 participants from the Hypersomnolence Australia Patient Data Registry diagnosed with either IH (n = 554), narcolepsy type 1 (NT1, n = 54) or narcolepsy type 2 (NT2, n = 78) between January 2013 and October 2022 was performed. Results: Participants with IH reported additional sleep disorders such as OSA (16.4%) and restless legs syndrome (7.9%) and notable comorbidities included depression (46.2%) and anxiety (50%). There was a mean delay in diagnosis of 10 years in participants with IH, when compared to symptom onset. IH presents with unique but also overlapping symptomatology with NT2, with similar reporting of long daytime naps, unrefreshed sleep and automatic behavior. Modafinil was the most common medication (45.5%) used by participants with IH followed by dexamphetamine (44.2%). Most participants with IH reported receiving physician advice regarding positive lifestyle changes but recommend that newly diagnosed patients be given more advice about medication use. Conclusions: This study demonstrates a delay in IH diagnosis when compared to symptom onset and overlapping features of IH and NT2. It also highlights the heterogeneous presentation of IH and the value of large patient registries in future research.
Secondary intracranial causes for headaches in children

Abstract:Although migraines are the most common cause of headaches in children, it is important to be cognizant of other, secondary causes of headaches. Secondary headaches are caused by an underlying etiology that may be systemic (medical) or due to a problem inherent in the central nervous system. Common intracranial etiologies for headache include structural (eg, tumor, hydrocephalus, Chiari malformation), infection (encephalitis or meningitis), inflammatory (acute disseminated encephalomyelitis, multiple sclerosis, vasculitis), and epilepsy. In some situations, early identification and appropriate treatment of these underlying conditions can result in complete headache resolution. This article discusses these secondary causes of headaches due to primary brain etiologies, focusing on aspects of the history that should lead a neurologist to order neuroimaging or electroencephalographic studies for these children.

Abstract:Cephalalgia, Volume 44, Issue 8, August 2024. BackgroundTo synthesize the available epidemiologic data on short-lasting unilateral neuralgiform headache attacks (SUNHA). This, in turn, might inform diagnostic work-up and clinical decision-making.MethodsEMBASE and PubMed were searched for observational studies reporting on the prevalence or relative frequency of SUNHA or its individual clinical features. Two investigators independently conducted title and abstract screening, full-text review, data extraction, and risk of bias assessment, and random-effects meta-analyses were performed to estimate the prevalence or relative frequency of SUNHA and its individual clinical features.ResultsFifteen clinic-based studies met our eligibility criteria. Of these, five studies reported estimates on the relative frequency of SUNHA among adults evaluated for headache or facial pain, yielding a pooled relative frequency as 0.32% (95% confidence interval = 0.17–0.62; I2 = 89.9%). Most often, SUNHA presented as episodic, side-locked stabbing headache of severe pain intensity, predominantly affecting the ophthalmic and/or maxillary branch of the trigeminal nerve. The most common cranial autonomic features were lacrimation, conjunctival injection, rhinorrhea and nasal congestion.ConclusionsSUNHA is a rare headache disorder with distinct clinical features. However, our findings must be interpreted with caution as a result of between-study heterogeneity and lack of population-based studies, underscoring the need for further epidemiologic research.

Treatment of insomnia in elderly patients with headaches

Abstract:The article describes clinical cases of elderly patients with primary headaches and insomnia managed in neurological practice. The first clinical case provides details of the typical medical history of a female patient with acute insomnia and episodic tension-type headache, the second one describes the typical medical history of a patient with chronic insomnia and chronic migraine. Clinical examples demonstrate common errors in the management of patients with insomnia and primary headaches in real-life medical practice. The influence of psychosocial (stress, anxiety, negative misconceptions about the disease, dysfunctional behaviour, symptom catastrophizing) and iatrogenic (ordering excessive additional diagnostic tests, over-diagnosing of detected structural changes in the spine, neck vessels, brain, diagnostic errors, prescribing of ineffective treatment) factors on the development of insomnia and increase of headaches in frequency is shown. The features of the presented clinical cases are to demonstrate the relationship between insomnia and increased frequency of primary headaches. Patients were treated using the interdisciplinary program comprising non-drug treatments and drug therapy. It has been shown that the treatment of insomnia can not only improve the quality of sleep, but also reduce the frequency of headaches. SonNorm Duo containing melatonin, motherwort extract and peppermint leaf oil was used as a drug therapy for acute and chronic insomnia. Melatonin and substances of plant origin have a high level of effectiveness in the treatment of insomnia and are recommended as drugs to be used first to treat insomnia. The drug therapy of acute and chronic insomnia was combined with highly effective non-drug treatments: an educational program to improve sleep hygiene, cognitive behavioural therapy, and relaxation exercises. During treatment, patients reported an improvement in their sleep quality as early as in the first month. A stable normalization of sleep and a significant decrease in headache frequency in a patient with acute insomnia and episodic tension-type headache occurred after the fifth week of treatment and in a patient with chronic insomnia and chronic migraine after the second month of treatment. A 12-month follow-up of patients showed that the positive effect of the treatment was maintained.

Abstract:Although secondary headaches due to e.g. temporal arteritis or a brain tumor are common among the elderly, primary headache disorders also occur in this age group, albeit less frequently than in younger individuals. A different presentation in the older age groups often makes a diagnosis difficult. Some headache syndromes, such as hypnic headache, are typical for the elderly. Furthermore, age-related physiologic changes, co-morbidities and contra indications require appropriate and targeted treatment in the elderly. Although treatments for the most common primary headaches are available, many limitations hamper their use in this group. For many headaches syndromes randomized controlled treatment trials in elderly are not available. In this article we review the clinical aspects of common primary headaches and medication overuse headache in the elderly and their treatments, with emphasis on age-specific problems. <b>Relevance for patients:</b> Primary headache syndromes in older patients ask for specific treatment considerations due to comorbidity, polypharmacy and an increased risk of side effects. Clinically, the headaches can be different and atypical. Results from clinical trials cannot be generalized to the elderly because these groups usually are not included in studies. In migraine, non-pharmacologic treatment should be considered, with depression and cerebrovascular disease as major comorbidities. Tension type headache, being the most common headache presentation in elderly, also includes a relatively large proportion of patients with a secondary headache warranting further testing. Trigeminal autonomous cephalalgias are rare, and can present with pseudo dementia. Medication overuse and medication-induced headaches are relatively common, for which patient education, ceasing drugs and withdrawal from caffeine containing substances are pivotal. Furthermore, hypnic headache, exploding head syndrome and benign thunderclap headache are specific for this patient group and require specific treatment.

Abstract:Narcolepsy is a primary neurological sleep disorder characterized by excessive daytime sleepiness, and in a majority of patients, cataplexy, which is sudden muscle weakness brought on by strong emotion. While often thought of as an adult condition, narcolepsy can be present at any age and may occur as often as 1 in 2000 individuals. Regardless of age, its effects are far-reaching, resulting in missed school or work days, poor school or work performance and all associated ramifications. In children and young-adults, narcolepsy can masquerade for years as anxiety, depression, attention deficit disorder and even bipolar disorder, thus delaying diagnosis. Most patients diagnosed with narcolepsy receive significant benefit from standard medication. In addition to medication, however, this article describes promising roles for clinical hypnosis in the care of young patients with narcolepsy, including through teaching them how to have more control over their sleepiness and the emotions that can induce cataplexy, while minimizing side effects that can occur when using narcolepsy medication.

Abstract:Abstract Introduction Narcolepsy and idiopathic hypersomnia (IH) are central disorders of hypersomnolence. People with narcolepsy type 1 (NT1), narcolepsy type 2 (NT2), and IH endure long diagnostic delays (up to 15 years), have many comorbidities, and suffer considerable, poorly recognized disease burden. Administrative health data present an opportunity to characterize these patients, their diagnosis, and treatment journey in real-world clinical practice. Methods This retrospective cohort study included patients aged ≥2 years with (non-diagnostic) medical claims for narcolepsy or IH between January 01, 2014 and December 31, 2021 in Optum’s Market Clarity integrated claims and EHR database. Patients with ≥24 months continuous enrollment prior to last narcolepsy/IH diagnosis (index date) were classified into NT1; NT2; IH; NT2/IH cohorts. Patient journeys, including diagnostic evaluations and treatments, were described using all enrolled time, while comorbidities were described during a 12-month baseline period. Results Overall, 51,548 patients (median age: 45 years [IQR 32–58]; female: 64%) were included with a median pre-index observation period of 52 months: 7,742 (15%) NT1; 31,132 (60%) NT2; 12,287 (24%) IH; and 387 (1%) NT2/IH. Cardiovascular disease (52%), depression (39%), anxiety disorders (38%), obesity (32%) and sleep apnea (40%) were similarly common across cohorts during the 12 months pre-index. Substance-abuse disorders were reported among 18%–19% of narcolepsy and 13%–14% of IH cohorts. Most patients (81%) had a consistent diagnosis (NT1;NT2;IH) throughout the study period, but 9,608 patients had diagnosis switches, most commonly NT2->NT1 (n=3,355), NT1->NT2 (n=2,656), NT2->IH (n=1,187), IH->NT2 (n=859) or NT1->IH (n=205). Among patients with these most common switches (n=8,262), 68% had no record of sleep tests between the switches and 38% had no sleep test recorded during all observed time. Common treatments included SSRI/SNRI (57%), stimulants (41%), and wake-promoting agents (39%); 15% of NT1 patients received oxybate and 2.6% pitolisant. Among patients without a diagnosis switch (n=41,940), 26% did not receive any NT/IH medication after diagnosis. Conclusion In a large real-world US population, sleep diagnostic evaluations were underutilized and many patients were untreated. A small proportion received narcolepsy-specific medications. These results indicate an unmet need for effective diagnostic and treatment strategies for patients with narcolepsy/IH. Support (if any) Funded by Takeda

Abstract:Headache associated with sexual activity (HAWSA) has accompanied humanity since ancient times. However, it is only since the 1970s that it has become the subject of more extensive and detailed scientific interest. The purpose of this review is to provide an overview of the development of the concept of HAWSA, its clinical presentation, etiopathogenesis, diagnosis and treatment especially from the research perspective of the last 20 years. Primary HAWSA is a benign condition, whose etiology is unknown; however, at the first occurrence of headache associated with sexual activity, it is necessary to exclude conditions that are usually immediately life-threatening. Migraine, hypnic headache or hemicrania continua have been reported to co-occur with HAWSA, but their common pathophysiologic basis is still unknown. Recent advances in the treatment of HAWSA include the introduction of topiramate, progesterone, and treatments such as greater occipital nerve injection, arterial embolization, and manual therapy. Whether these new therapeutic options will stand the test of time remains to be seen.
