Abstract:Teaching hospitals represent a major segment of the Canadian health system, accounting for a disproportionate number of beds, patient days, and separations. Thus, although only six percent of hospitals are classified as teaching hospitals, they are responsible for about 36 percent of total hospital operating expenses. While affiliation with a medical school presents unique opportunities for the teaching hospital and increases its prestige, there are clear costs associated with affiliation. Administrators have less control over resource allocation decisions, including the types of teaching programs offered. Teaching hospitals cannot unilaterally design their own teaching programs around specialties and subspecialties of their own choosing; decisions related to teaching programs have a direct impact on the services provided by the hospital and may negatively affect the hospital's ability to fulfill its patient care mission. As education budgets are constrained, teaching hospitals are expected to assume outstanding teaching-related expenses. Teaching hospitals are also expected to shift some of their teaching to alternative settings, such as the community. Thus, teaching hospital administrators will require a strong background in finance as well as negotiation and political skills.
A smart-device news recommendation technology based on the user click behavior

Abstract:Studies have been conducted in regard to personalized news recommendation using collaborative filtering mechanisms based on users' click behaviors. However, few existing studies have focused on news recommendations depending on the rates of news-category interests. In this paper, we present a personalized news-recommendation system that builds profiles of users' news-category interests, determines the number of news articles to be recommended for each news category in proportion to news-category interests and ranks news articles according to the user's news interests. In order to find the news categories that are interesting to read, the smart device collects the web pages viewed by the user and classifies the contents. We use machine learning techniques in order to classify web pages into different categories, and the results of our experimentation show that Naive Bayes achieved the highest F-measure. The news preference is automatically calculated by the news reading time and the length of the news text. News articles with high preference generated by the collaborative filtering technique are recommended based on the rate of each category. The recommendation system we have implemented is based on collaborative filtering using Mahout Recommender API on Hadoop. Through user testing, we also assess whether the proposed system is useful.

Abstract:Medical school graduates are avoiding primary care. The very aspects that once attracted students have been subverted. The breadth of practice that was once appealing has become the breadth of heavy-handed scrutiny, as politicians and business leaders have demanded quality--simplistically defined as dogmatic adherence to a standard. Individualized clinical judgment has been devalued; thinking has been replaced by algorithms. Practice guidelines have been usurped by pay-for-performance police, on patrol for deviations--not understanding that knowing and allowing for exceptions is the heart and soul of primary care. The coercive surveillance of "Quality Improvement" has become oppressive, making single organ-system specialties increasingly attractive (or at least more tolerable). Generalists are spending so much time proving they are good doctors, they don't have time to be good doctors. A remedy is suggested: a pilot project of volunteer salaried internists (more trusted, less audited) commissioned to our expandable national health care program, Medicare.

Abstract:The current dilemmas in primary care stem from 1) the unintended consequences of forces thought to promote primary care and 2) the "disruptive technologies of care" that attack the very function and concept of primary care itself. This paper suggests that these forces, in combination with "tiering" in the health insurance market, could lead to the dissolution of primary care as a single concept, to be replaced by alignment of clinicians by economic niche. Evidence already exists in the marketplace for both tiering of health insurance benefits and corresponding practice changes within primary care. In the future, primary care for the top tier will cater to the affluent as "full-service brokers" and will be delivered by a wide variety of clinicians. The middle tier will continue to grapple with tensions created by patient demand and bureaucratic systems but will remain most closely aligned to primary care as a concept. The lower tier will become increasingly concerned with community health and social justice. Each primary care specialty will adapt in a unique way to a tiered world, with general internal medicine facing the most challenges. Given this forecast for the future, those concerned about primary care should focus less on workforce issues and more on macro health care financing and organization issues (such as Medicare reform); appropriate training models; and the development of a conception of primary care that emphasizes values and ethos, not just function.

The preeminence of primary care within Cuban predoctoral medical education

Abstract:As a result of a massive public health system and state-of-the-art tertiary medical care services, Cuba's health profile now indicates a high incidence of chronic noncommunicable diseases; Cuban health authorities concluded that this required a community-based primary care solution. In the early 1980s the Ministry of Health (MINSAP) implemented a national family medicine program with the goal of providing one "Medico de la Familia" (family physician) for every 700 persons. To attain and ensure the sustainability of this primary care model, MINSAP reformed the medical education curriculum. The reform strategies created a primary care "specialty" and provided a broad-based interdisciplinary approach to primary care training. The reforms place primary care in a prominent position within the predoctoral medical education curriculum. Cuba's approach is an innovation for a country that attained impressive medical advances through a specialized high-technology medical system.

Abstract:National health policies of many countries stress priority for primary health care (PHC). This emphasis has arisen as a reaction to large expenditures on hospitals and sophisticated technology in major cities of developing countries, while vast rural populations have been virtually ignored. The paradox developed from colonial and neo-colonial emulation of European and North American medical models. In 1978, an international conference of WHO/UNICEF at Alma-Ata, USSR defined the meaning of PHC, along with several principles of organization and equity under which it should be provided. To reach rural people with PHC, thousands of community health workers have been prepared and stationed in villages. Their training, however, is very brief and, with weak supervision, their performance has been disappointing. To achieve the WHO goal of "Health for All" through PHC requires greatly expanded education of public health leaders, who can supervise and inspire community personnel.

Abstract:The article explores the relations between teaching, research, and health care through an analysis of documents and reports by a group of professors with the Department of Clinical Medicine at the Universidade Federal do Rio de Janeiro's School of Medicine. Findings suggest that the two-hundred year old medical school has a strong tradition in teaching and health care and that although research predated inauguration of the thirty-year-old Clementino Fraga Filho University Hospital, this academic activity has only gradually occupied a larger space within the institution following creation of the hospital, which brought a radical change in clinical research methodology, concomitant with the development of a graduate program and the emergence of clinical epidemiology.

Abstract:The cysteine endopeptidase legumain was recently discovered in mammalian cells, predominantly localized in the lysosomal system where it is believed to contribute to antigen processing for MHC class II. Here we describe rapid assay procedures for the enzyme in 96-well microplates with two substrates, a novel compound, succinyl-Ala-Ala-Asn-4-methoxy-2-naphthylamide, and benzyloxycarbonyl-Ala-Ala-Asn-4-methyl-7-coumarylamide. Both substrates are suitable for fluorimetric assays, but the naphthylamide also allows colorimetric detection of legumain activity, since the released 4-methoxy-2-naphthylamine gives a red product when coupled with the Fast Garnet color reagent. We show that the naphthylamide substrate can be used to visualize active legumain after electrophoresis in polyacrylamide gel. Both substrates provide assays that are reproducible and sufficiently sensitive to allow the assay of legumain in crude samples such as tissue homogenates, although the coumarylamide is the more sensitive. The specificity of both assay methods for legumain was verified by the lack of inhibition by E-64 and total inhibition by egg white cystatin.

Abstract:I suggest a spiral-in process in which a stellar companion grazes the envelope of a giant star while both the orbital separation and the giant radius shrink simultaneously, forming a close binary system. The binary system might be viewed as evolving in a constant state of “just entering a common envelope (CE) phase.” In cases where this process takes place, it can be an alternative to CE evolution where the secondary star is immersed in the giant's envelope. Grazing envelope evolution (GEE) is made possible only if the companion manages to accrete mass at a high rate and launches jets that remove the outskirts of the giant envelope, hence preventing the formation of a CE. The high accretion rate is made possible by the accretion disk launching jets which efficiently carry the excess angular momentum and energy from the accreted mass. The orbital decay itself is caused by the gravitational interaction of the secondary star with the envelope inward of its orbit, i.e., dynamical friction (gravitational tide). Mass loss through the second Lagrangian point can carry additional angular momentum and envelope mass. The GEE lasts for tens to hundreds of years. The high accretion rate, with peaks lasting from months to years, might lead to a bright object referred to as the intermediate luminosity optical transient (Red Novae; Red Transients). A bipolar nebula and/or equatorial ring are formed around the binary remnant.

Abstract:This article examines the past, present and future of primary care dentistry. It provides a historical background of primary care dentistry and describes stages of its evolution. It further reviews the purpose and mission of contemporary primary care dentistry and outlines a vision for the development of primary care dentistry in the future. The type and extent of innovations and technological advances that have impacted – and improved – primary care dentistry revolutionising clinical activities, ranging from early computerised tomography to modern digital systems and workflows are summarised. A discussion of current scientific evidence base pertinent to primary care dentistry highlighting the need for 'effectiveness' rather than 'efficacy' studies is included in order to provide research data pertinent to the primary care dentistry setting where most dental patients receive most of their care most of the time.

Abstract:Primary health care has been held to be the foundation of any rational health system. The principle was fully endorsed by the Alma-Ata Conference in 1978, and has become the main policy of the World Health Organization. Important implications are involved for the education and training of doctors and other health care professions. An enquiry was conducted by personal interviews of those most responsible for the teaching of primary health care in the United Kingdom, to enquire about the status of primary health care in the curricula of U.K. medical schools, and about the standing of general practice. The enquiry also explored the degree of awareness among medical educators about the Alma-Ata Declaration. The leading representatives of primary health care in the medical schools made it clear that the teaching of primary health care varied greatly in the importance accorded to it and the resources made available for it by medical schools. Almost half the respondents were unaware of the support for greater emphasis on primary health care that had been specified in the Alma-Ata Declaration.

Historical Roots of Hospital Centrism in China (1835-1949): A Path Dependence Analysis.

Abstract:Stronger primary care has been associated with important contributions to health system performance, yet countries struggle to resource it adequately, given competing demands from hospitals. Although historically China has originated influential models of primary health care, it has an enduring problem with hospital dominance in health service delivery. This paper is a historical analysis of the co-evolution of hospitals and primary care providers in China from 1835 (the year when the first hospital was built in mainland China) to 1949 (the year when the People's Republic of China was founded), which aims to shed light on approaches to primary care strengthening. We develop and use a path dependence analytic framework, specifying the critical juncture, conjuncture and post-juncture development of the institutions shaping the balance between hospitals and primary care providers in China. We find that China had historically formed the hospital-centric model involving four sets of regenerating and mutually reinforcing institutions: 1) financial resources were being disproportionally distributed to hospitals; 2) high-quality medical professionals were largely concentrated in hospitals; 3) large outpatient departments were incorporated in hospitals, which functioned as a first point of care for many patients; 4) hospitals answered primarily to the demand of the more privileged social group. The early institutionalization of a hospital-centric model of Western medicine in China from 1835 became resistant to change, and efforts to strengthen primary care eventually took a divergent low-cost and de-professionalized developmental path towards 1949. As China still has a hospital-centric health system seeded in the nineteenth century, these findings can inform the framing of contemporary options for primary care strengthening. Without addressing these deep regenerating causes using a whole-system approach, China is unlikely to achieve a primary care orientation for health system development.

Abstract:AbstractOver the past 50 years, the CTU has remained a core part of training for residents within internal medicine training programs. At the same time, the needs of society have changed significantly over the years, and current training needs to reflect this change. This is not a novel idea, but one that we feel requires greater attention to adequately address the needs of academic teaching hospitals across the country. Abstract:The birth of hospital medicine has represented a challenge to general practice, since practising and learning medicine has gradually moved from patient-bedside model to laboratory, peer-based hospital model. At the turn of the 20th century, GPs managed to survive the hospital era due to a patronage contractual arrangement and patients' preference for home-based and well-known types of care. Patients were quite suspicious of the cold, isolated, and powerless position in which they found themselves in hospital. In this context, consultants had to rely on GPs referrals as they had control over clients.1 However, the hospital-based biomedical model became stronger after the two great wars and the inception of the NHS. Abstract:Nodular regenerative hyperplasia (NRH) is a rare liver condition in which widespread benign transformation of the hepatic parenchyma into small regenerative nodules occur, leading to development of non-cirrhotic portal hypertension. Conditions associated with NRH include rheumatologic, hematological, autoimmune, infectious, neoplastic, or drug-related etiology. Accurate diagnosis is made on liver biopsy, showing diffuse micronodular transformation in the absence of fibrosis. Here, we present the second case in world literature of a middle-aged man presenting with recent onset sleep reversal and memory loss with darkening of skin for four years associated with systemic manifestations, in whom porto-systemic shunt syndrome due to NRH secondary to Behcet’s disease was eventually diagnosed.
