Abstract:Background: Abnormal sleep duration predicts depression and anxiety. We seek to evaluate the impact of sleep duration before stroke on the occurrence of depression and anxiety at 3 months after acute ischemic stroke (AIS). Methods: Nationally representative samples from the Third China National Stroke Registry were used to examine cognition and sleep impairment after AIS (CNSR-III-ICONS). Based on baseline sleep duration before onset of stroke as measured by using the Pittsburgh Sleep Quality Index (PSQI), 1,446 patients were divided into four groups: >7, 6–7, 5–6, and <5 h of sleep. Patients were followed up with the General Anxiety Disorder-7 (GAD-7) and Patient Health Questionnaire-9 (PHQ-9) for 3 months. Poststroke anxiety (PSA) was defined as GAD-7 of ≥5 and poststroke depression (PSD) as PHQ-9 of ≥5. The association of sleep duration with PSA and PSD was evaluated using multivariable logistic regression. Results: The incidences of PSA and PSD were 11.2 and 17.6% at 3 months, respectively. Compared to a sleep duration of >7 h, 5–6 h, and <5 h of sleep were identified as risk factors of PSA [odds ratio (OR), 1.95; 95% confidence interval (CI), 1.24–3.07; P < 0.01 and OR, 3.41; 95% CI, 1.94–6.04; P < 0.01) and PSD (OR, 1.47; 95% CI, 1.00–2.17; P = 0.04 and OR, 3.05; 95% CI, 1.85–5.02; P < 0.01), while 6–7 h of sleep was associated with neither PSA (OR, 1.09; 95% CI, 0.71–1.67; P = 0.68) nor PSD (OR, 0.92; 95% CI, 0.64–1.30; P = 0.64). In interaction analysis, the impact of sleep duration on PSA and PSD was not affected by gender ( P = 0.68 and P = 0.29, respectively). Conclusions: Sleep duration of shorter than 6 h was predictive of anxiety and depression after ischemic stroke.

Impact of Persistent Poor Sleep Quality on Post-Stroke Anxiety and Depression: A National Prospective Clinical Registry Study

Abstract:Purpose: The impact of poor sleep quality after stroke, especially persistent poor sleep quality, on poststroke anxiety and depression is unclear. We seek to investigate the impact of baseline and persistent poor sleep quality on short-term poststroke anxiety and depression. Patients and methods: Data were analyzed for 1619 patients with acute ischemic stroke from the Impairment of Cognition and Sleep after Acute Ischemic Stroke or Transient Ischemic Attack in Chinese Patients study (ICONS). The sleep quality was assessed at 2 weeks and 3 months using the Pittsburgh Sleep Quality Index scale (PSQI). Poor sleep quality was defined as a PSQI score of >5, and persistent poor sleep quality was defined as a PSQI score of >5 at each time point. Patients were divided into three groups according to the quality of sleep: good sleep quality, baseline poor sleep quality and persistent poor sleep quality. Patient Health Questionnaire-9 (PHQ-9), General Anxiety Disorder-7 scale (GAD-7), and Modified Rankin Scale (mRS) at 3 months after stroke were taken as the study outcomes. Results: Persistent poor sleep quality was present in 70.2% of patients after stroke. Compared to those with good sleep quality, patients with baseline poor sleep quality did not show significant differences in disability, anxiety and depression. However, patients with persistent poor sleep were at increased risk of depression (odds ratio, OR 3.04, 95% confidence interval, CI 1.66-5.57, P < 0.01) and anxiety (OR 3.20, 95% CI 1.42-7.19, P < 0.01) at 3 months after stroke. Persistent poor sleep quality was not identified as a risk factor for functional disability at 3 months. Conclusion: Patients with persistent poor sleep quality are at added risks for depression and anxiety after stroke.
Association Between Pre-Stroke Subjective Sleep Status and Post-Stroke Cognitive Impairment: A Nationwide Multi-Center Prospective Registry

Abstract:Background: Although sleep disorders significantly increase the risk of cognitive impairment, literature is relatively scarce regarding the impact of sleep status on cognitive function in patients with acute ischemic stroke (AIS). We seek to study the association between pre-stroke subjective sleep status and cognitive function at 3 months after stroke. Patients and methods: Data were analyzed for 1,759 AIS patients from the Impairment of Cognition and Sleep after Acute Ischemic Stroke or Transient Ischemic Attack in Chinese Patients Study (ICONS). Pre-stroke subjective sleep status was assessed by the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS). Greater sleep fragmentation was defined as waking up in the middle of the night or early morning ≥3 times a week. Cognitive function was evaluated using the Montreal Cognitive Assessment (MoCA) at 3 months after stroke. Primary endpoint was the incidence of post-stroke cognitive impairment (PSCI) at 3 months after stroke. The association between subjective sleep status and PSCI was evaluated using multivariable logistic regression. Results: PSCI occurred in 52.1% at 3 months after stroke. Patients with very bad sleep quality before stroke were at increased risk of PSCI (OR, 2.11; 95% CI, 1.11-4.03; P=0.03). Subgroup analysis found that the association between very bad sleep quality and PSCI was more evident among patients with high school education or above (OR, 5.73; 95% CI, 1.92-17.10; P for interaction=0.02). In addition, patients with greater sleep fragmentation before stroke were also at higher risk of PSCI (OR, 1.55; 95% CI, 1.20-2.01; P<0.01). Similarly, subgroup analysis showed that the risk of PSCI was more pronounced among patients without employment (OR, 2.45; 95% CI, 1.59-3.77; P for interaction=0.01). Conclusion: Very bad sleep quality and greater sleep fragmentation before stroke were identified as independent risk factors for PSCI at 3 months after stroke.

Abstract:Objective: To investigate the effects of sleep disorders on post-stroke cognitive impairment (PSCI) and other factors affecting post-stroke cognitive impairment. Methods: A total of 1,542 first-ever stroke inpatients in department of neurology of Tianjin Huanhu Hospital from 2015.6.1 to 2016.12.31. We recorded the personal history of patients. The MMSE (mini-mental state examination), MoCA (Montreal Cognitive Assessment), HAMD (Hamilton Depression Scale), BI (Barthel index), mRS (Modified Rankin Scale), PSQI (Pittsburgh Sleep Quality Index), ESS (Epworth Sleepiness Scale), Berlin questionnaire, nocturnal TST (total sleep time) were assessed before discharge. All patients were followed up at 3 months, 6 months, and 4 years (2019–2020) after stroke. During follow-up, the above scales should be evaluated again to assess the sleep status and cognitive function of patients at that time. Results: Nocturnal TST (>8 h) (OR 3.540, 95% CI 1.692–7.406, P = 0.001) was a risk factor for cognitive impairment 3 months after stroke. Nocturnal TST (<7 h) (OR 6.504, 95% CI 3.404–12.427, P < 0.001) was a risk factor for cognitive impairment 6 months after stroke. Low sleep quality (OR 2.079, 95% CI 1.177–3.672, P = 0.012), sleepiness (OR 3.988, 95% CI 1.804–8.818, P = 0.001), nocturnal TST (<7 h) (OR 11.334, 95% CI 6.365–20.183, P 8 h) (OR 4.096, 95% CI 1.682–9.975, P = 0.002) were risk factors for cognitive impairment 4 years after stroke. The prevalence of cognitive impairment with TIA were 79.3% at admission, 68.1% at 3-months follow-up, 62.1% at 6-months follow-up and 52.2% at 4-year follow-up. Conclusion: Long or short nocturnal TST ( 8 h) was a risk factor for cognitive impairment after stroke (3 months, 6 months and 4 years). Poor sleep quality and sleepiness were shown to be risk factors for cognitive impairment at 4-year follow-up. Cognitive impairment was very common in patients with TIA.

Abstract:Objective: To investigate the sleep state and determine whether variables, such as age, functional status, walking capacity, fatigue, depressive symptoms, and quality of life were associated with sleep quality of individuals with chronic stroke. Methods: For this exploratory study, the dependent variable was sleep quality, which was measured by the Pittsburgh Sleep Quality Index (PSQI). Step-wise multiple linear regression analysis was employed to identify which of the independent variables, that is, age, functional status (Modified Rankin Scale-mRS), walking capacity (6-minute Walk Test), fatigue (Fatigue Severity Scale), depressive symptoms (Geriatric Depression Scale-GDS), and quality of life (EuroQol) were associated with sleep quality. Results: Ninety participants, 55 (61%) men, who had a mean age of 61 (SD 12) years and a mean time since the onset of the stroke of 58.2 (SD 58.7) months, were included. Sleep quality was significantly associated with depressive symptoms and functional status. Together, they explained 30% of the variance in the PSQI scores (p < .0001). The GDS scores alone explained 22% (F = 25.76; p < .0001) of the variance in sleep quality (PSQI). When mRS scores were included in the model, the explained variance increased to 30% (F = 20,38; p < .0001). Conclusion: Depressive symptoms and functional status, which are both potentially modifiable factors, were associated with the sleep quality of individuals with chronic stroke. It is important to consider the assessment of sleep quality in the context of stroke rehabilitation.

Seasonal Changes in the Physiology of Male Virginia Opossums (Didelphis virginiana): Signs of the Dasyurid Semelparity Syndrome?

Abstract:Semelparity, which is multiplying once in a lifetime, is a rare reproductive strategy among mammals. Several species of the marsupial family Dasyuridae experience 100% male mortality following an intense mating period. We investigated seasonal physiological changes that may be associated with early mortality in the male Virginia opossum (Didelphis virginiana; Didelphidae) and compared these changes with those of semelparous, male dasyurids. Free‐ranging male Virginia opossums (n = 36) were collected during 2001 at the Oklahoma State University Cross Timbers Experimental Range. Seasonal data were collected on hematological, morphological, and helminth parameters of these individuals. We used one‐way ANOVA to determine whether there were seasonal differences among means for each parameter. It appeared that male Virginia opossums experienced some physiological changes similar to those of male dasyurids exhibiting semelparity. All males collected in summer (August) were juveniles of the year. Lack of adult males in August suggests high mortality of this cohort during the breeding season. Opossum characteristics exhibiting the dasyurid semelparity syndrome included packed cell volume, adrenal mass, and helminth numbers. Minor lymphocytopenia, neutrophilia, and testosterone concentrations also were similar to semelparous dasyurids. However, a lack of change in serum cortisol concentration and body mass and dynamics in immunoglobulin protein, serum protein, and testes mass were not consistent with previous reports of semelparous dasyurid physiology. Evolutionary divergence and differences in breeding behavior between dasyurids and didelphids may be responsible for the lack of consistency between the taxa.

Abstract:Abstract Introduction Stroke survivors frequently report sleep disturbance and feelings of not being back to where they were, physically, prior to their event. Post-stroke sleep disturbance may be a factor related to sub-optimal recovery. Methods Data came from the Reactions to Acute Care and Hospitalizations (ReACH) Stroke-Sleep Study, an observational cohort assessing the relationship of sleep with secondary cardiovascular risk in the year following stroke or transient ischemic attack (TIA). Data were collected using self-reported questionnaires at 1 month and chart reviews. Sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI), with a score >5 indicating poor sleep quality. Insomnia symptoms were assessed with a single item from the Posttraumatic Stress Disorder Checklist for DSM-5 (PCL-5): “In the past month, how much were you bothered by trouble falling or staying asleep?” with a rating of “moderately,” “quite a bit,” or “extremely” indicating presence of insomnia symptoms. Degree of disability was assessed using the Simplified Modified Rankin Scale Questionnaire (smRSq), with presence of disability defined as smRSq score >2. Binary logistic regression was used to analyze the relationships between presence of poor sleep and insomnia, separately, with presence of disability at 1-month post-hospitalization. Analyses were adjusted for age, gender, race/ethnicity, and stroke severity (NIH Stroke Scale). Results The sample included n=436 participants (mean [SD] age=60.2 [15.5] y, 52% Hispanic/Latino, 59% female). Fifty-six percent of participants reported poor sleep quality, 27% reported presence of insomnia symptoms, and 23% reported presence of disability at 1 month. Poor global sleep quality (vs. good sleep) was significantly associated with presence of disability (adjusted OR: 2.45, 95% CI: 1.48-4.04, p< 0.001). Insomnia (vs. no or mild symptoms) was also significantly associated with presence of disability (adjusted OR: 2.40, 95% CI: 1.50-3.85, p< 0.001). Conclusion More than half of participants reported poor sleep quality, and over a quarter reported insomnia symptoms, 1 month after stroke hospitalization. Compared to those without sleep problems, individuals with sleep disturbance had nearly 2.5 times higher odds of disability. While causal relationships cannot be concluded based on this cross-sectional analysis, findings suggest that sleep may play a role in stroke survivors’ outcomes and recovery. Abstract:Background Sleep quality can offer new insights into addressing depression among stroke patients. However, the current understanding of the mechanism by which sleep quality reduces depression is not clear in existing research. Objectives This study aimed to identify the relationships and mechanisms among perceived stress, sleep quality, social support, and depression in stroke patients. Methods A multicenter cross-sectional study was conducted from January to May 2023. Cluster random sampling was used to recruit 500 stroke patients from five hospitals in Henan Province, China. The Chinese Perceived Stress Scale (CPSS), Pittsburgh Sleep Quality Index (PSQI), Social Support Rating Scale (SSRS), and Hamilton Depression Scale (HAMD-24) were employed to assess perceived stress, sleep quality, social support, and depression, respectively. Data were analyzed using descriptive analysis, Pearson’s correlation analysis, and moderated mediation analysis. The study adhered to the STROBE checklist for reporting. Results Out of 500 participants, 471 completed the survey (94.2%). After controlling for sex and age, mediation analysis revealed that poor sleep quality partially mediated the relationship between perceived stress and depression ( β = 0.184, 95% CI: 0.110, 0.359). Additionally, social support played a moderating role in the mediation model. Conclusion This study explained the moderated mediation of sleep quality and social support between perceived stress and depression. It provided a theoretical basis for the development of a sleep quality intervention program for reducing depression among stroke patients.

Impact of sleep disturbance on longitudinal cognitive performance in patients with transient ischemic attack or mild stroke

Abstract:Introduction: Sleep disturbances including obstructive sleep apnea (OSA) and poor sleep quality are common after stroke, while its association with cognitive changes following transient ischemic attack (TIA) or mild stroke remains unclear. We aim to determine whether sleep duration, OSA parameters, or nocturnal hypoxemia is associated with a greater cognitive decline after stroke. Methods: We prospectively followed-up patients with acute TIA/mild stroke [National Institute Health Stroke Scale (NIHSS) < 7] who underwent baseline sleep questionnaire [Pittsburgh Sleep Quality Index (PSQI)], and serial cognitive assessments [Montreal Cognitive Assessment (MoCA) 5-min, Stroop Test] at baseline and one-year. We also evaluated apnea-hypopnea index (AHI) and nocturnal hypoxemia by Home Sleep Apnea Test (HSAT) at one-year. Primary outcome was one-year change in MoCA 5-min score. Results: One hundred and five patients with TIA/mild stroke (mean age 63 years, 65 % male) were included. Baseline short sleep (< 6 hour/night) and AHI ≥ 20/hour at one-year were independently associated with a decline in the MoCA 5-min total score after covariates adjustment [short sleep: β = -2.36 95 % confidence interval (CI) (-4.13, -0.59), p = 0.009; AHI ≥ 20/hour: β = -1.79 (-3.26, -0.32), p = 0.017; remained significant after multiple comparisons correction]. A lower mean MinSpO2 was associated with a decline in executive function [Stroop interference index: β = 0.29 (0.04, 0.53), p = 0.021], but not with MoCA 5-min score at one-year. Moderation analysis indicated AHI ≥ 20/hour was associated with a pronounced decline in executive function only in men. Conclusions: Short sleep after stroke onset, AHI ≥ 20/hour and nocturnal hypoxemia at one-year contributed to an impaired cognitive trajectory at one-year following stroke in patients with TIA/mild stroke.
Resilience as the Mediating Factor in the Relationship Between Sleep Disturbance and Post-stroke Depression of Stroke Patients in China: A Structural Equation Modeling Analysis

Abstract:Background: Stroke patients may suffer from a variety of symptoms which can result in sleep disturbance and post-stroke depression (PSD). Whereas, resilience can alleviate sleep disturbance and help maintain well-being after stroke. Objective: The aim of this study is to explore whether resilience plays a mediating role in the relationship between sleep disturbance and PSD of stroke patients in China. Methods: A cross-sectional study with a multi-stage sampling was carried out in Liaoning Rehabilitation Center and the Third People's Hospital of Chongqing in China from May to September 2019. A total of 353 stroke patients were enrolled in this study. Structural equation model (SEM) was used to test the mediating effect of resilience on the relationship between sleep disturbance and PSD. Results: The prevalence of PSD of stroke patients was 34.56%. Sleep disturbance contributed most to the variance of PSD and had a significantly positive association with PSD among stroke patients ( P < 0.01). Resilience was negatively associated with PSD, and acted as a mediator between sleep disturbance and PSD (a * b = 0.201, BCa 95% CI: 0.156~0.254). Conclusions: The prevalence of PSD was high among the Chinese stroke patients. Sleep disturbance was highly associated with PSD, resulting in the increased risk of PSD. Furthermore, resilience has a mediating effect on the relationship between sleep disturbance and PSD, and could reduce the negative effect of sleep disturbance on the development of PSD.

Abstract:PurposeSleep disturbances are frequently reported in stroke patients and associated with the outcome of strokes. Using sleep questionnaires, we investigated the prevalence of classified sleep disturbance and the influence of sleep disorders upon a stroke prognosis.MethodsPatients with acute ischemic strokes or transient ischemic attacks (TIA) were included. We investigated the prevalence of sleep disturbance and the association of outcomes resulting from strokes. The National Institutes of Health Stroke Scale score at day 7 (NIHSS-7) and modified Rankin Scale score at month 3 (mRS-3) stood for short- and long-term outcomes. A series of questionnaires including all Korean versions of the Pittsburgh Sleep Quality Index (PSQI-K), Insomnia Severity Index (ISI-K), Epworth Sleepiness Scale (ESS-K), Berlin Questionnaire, Sleep Obstructive apnea score optimized for Stroke (SOS), Beck Depression Inventory-2, and Hospital Anxiety and Depression Scale were used.ResultsA total of 241 (mean age was 64.2 ± 11.9, 146 males; 60.6%) consecutive acute ischemic stroke patients, including 36 TIAs, were enrolled. The NIHSS score at admission, NIHSS-7, and mRS-3 were 3.26 ± 3.64, 1.72 ± 2.29, and 0.21 ± 0.82, respectively. PSQI-K ≥8.5 was reported in 79 subjects (32.8%), ISI-K ≥15.5 in 29 (12.0%), ESS-K ≥11 in 21 (8.7%), and SOS ≥11 in 48 (20.3%). The NIHSS-7 was associated with the SOS (standardized β = 0.281, p < 0.001) and the mRS-3 with the ISI-K (standardized β = 0.219, p = 0.001) and the SOS (standardized β = 0.171, p = 0.011).ConclusionsScreening for and intervening in the sleep problems of stroke patients could improve their outcome. As sleep disturbances are associated with short-term and/or long-term outcomes of strokes, active screening and intervention for sleep disturbances after strokes are needed.

Structural and functional characterization of calponin fragments.

Abstract:Calponin from chicken gizzard consists of two principal components, possibly isoforms, separable by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and isoelectric focusing. Cleavage with 2-nitro-5-thiocyanobenzoic acid indicated that calponin contains 2 cysteine residues. Purified fragments of 30 and 21 kDa retained the following properties of the intact protein: actin-, tropomyosin- and calmodulin-binding, and ability to inhibit the actin-activated MgATPase activity of smooth muscle myosin. Both fragments, like intact calponin, were phosphorylated by protein kinase C which inhibited their binding to actin and relieved their inhibition of the ATPase. Tryptic digestion of calponin phosphorylated by protein kinase C generated 3 phosphopeptides with the following N-terminal sequences: FASQQGMTAYGTR, GASQQGMTVYGLP, and NHSGHVQ, each possessing a single phosphoserine.

Abstract:Introduction. Stroke is a major cause of morbidity and mortality in various populations. Sleep disturbances are common in patients who have suffered an acute cerebral circulatory disorder, complicating the recovery of motor functions, worsening the emotional state and being associated with poorer health and quality of life in general. Aim. To investigate the impact of sleep disturbance on the emotional state of stroke patients. Materials and methods. The study included 100 people who were divided into two groups. 50 patients who had suffered an acute cerebral circulatory disorder and who suffered from insomnia formed the main group, which was divided into 2 subgroups. MG 1 – main subgroup 1 (26 people), who received a course of short-term (5 sessions) cognitive-behavioural therapy for insomnia in combination with the drug Valocordin-Doxylamine 25 mg as part of complex therapy. – MG 2 main subgroup 2 (24 people), who received only standard treatment of the main disease. Patients in the comparison group who underwent acute cerebral circulatory disorder without concomitant insomnia (CG, 50 people) also received only standard treatment for the primary condition. In addition to clinical examination, the Hospital Anxiety and Depression Scale (HADS), the Pittsburgh Sleep Quality Questionnaire (PSQI) and the Insomnia Severity Scale (ISI) were used to assess patients. Results. Initially, the main group had a statistically significant higher level of depression (clinically expressed depression) than the comparison group (subclinically expressed depression) (U = 54.6; p = 0.012). As a result of treatment in main subgroup 1, where insomnia was corrected, there was not only an improvement in sleep (median PSQI score before treatment 8 (6; 10) and after treatment 5 (5; 6), p = 0.022), but also a reduction in depression (HADS-D score before treatment 12 (10; 14) and after treatment 12 (10; 14)): 12 (10; 14) and post-treatment – 5 (4; 6), p = 0.022) and anxiety (HADS-A pre-treatment: 9 (7; 10) and post-treatment – 5 (4; 6), p = 0.013). In the main subgroup 2, where insomnia was not corrected, there was no improvement in sleep (PSQI before treatment 8 (6; 11) and after treatment 8 (7; 10), p = 0.433), depression (HADS-D before treatment: 9 (8; 11) and after treatment – 8 (6; 9), p = 0.171) and anxiety (HADS-A before treatment: 9 (6; 10) and after treatment – 8 (6; 9), p = 0.064) were less pronounced than in the main subgroup 1 and the comparison group (HADS-D: H (2, N = 100) = 25.73829, p = 0.016; HADS-A: H (2, N = 100) = 28.42621, p = 0.004). Conclusion. Complex therapy for stroke patients suffering from insomnia, including drug and non-drug methods of insomnia correction, increases the effectiveness of treatment by not only improving the quality of sleep, but also reducing the severity of anxiety and depression.

Abstract:Background and purpose Whilst sleep disturbances are associated with stroke, their association with stroke severity is less certain. In the INTERSTROKE study, the association of pre‐morbid sleep disturbance with stroke severity and functional outcome following stroke was evaluated. Methods INTERSTROKE is an international case–control study of first acute stroke. This analysis included cases who completed a standardized questionnaire concerning nine symptoms of sleep disturbance (sleep onset latency, duration, quality, nocturnal awakening, napping duration, whether a nap was planned, snoring, snorting and breathing cessation) in the month prior to stroke (n = 2361). Two indices were derived representing sleep disturbance (range 0–9) and obstructive sleep apnoea (range 0–3) symptoms. Logistic regression was used to estimate the magnitude of association between symptoms and stroke severity defined by the modified Rankin Score. Results The mean age of participants was 62.9 years, and 42% were female. On multivariable analysis, there was a graded association between increasing number of sleep disturbance symptoms and initially severe stroke (2–3, odds ratio [OR] 1.44, 95% confidence interval [CI] 1.07–1.94; 4–5, OR 1.66, 95% CI 1.23–2.25; >5, OR 2.58, 95% CI 1.83–3.66). Having >5 sleep disturbance symptoms was associated with significantly increased odds of functional deterioration at 1 month (OR 1.54, 95% CI 1.01–2.34). A higher obstructive sleep apnoea score was also associated with significantly increased odds of initially severe stroke (2–3, OR 1.48; 95% CI 1.20–1.83) but not functional deterioration at 1 month (OR 1.19, 95% CI 0.93–1.52). Conclusions Sleep disturbance symptoms were common and associated with an increased odds of severe stroke and functional deterioration. Interventions to modify sleep disturbance may help prevent disabling stroke/improve functional outcomes and should be the subject of future research.

Abstract:IntroductionThere is mounting evidence that sleep plays an important role in the rehabilitation and recovery process following acute stroke. Following acute care, many patients with stroke are admitted to inpatient rehabilitation facilities (IRFs), where they undergo intensive, interdisciplinary therapy to recover or relearn functional skills to minimize long-term disability. The role and impact of sleep in this early stage of stroke rehabilitation, however, is poorly understood. The purpose of this study is to investigate the relationship between sleep quality and clinical outcomes in the IRF setting following acute stroke.MethodsPatients wore a collection of wearable sensors to measure sleep and wake throughout their IRF stay. Linear mixed-effect models (LMEMs) were built to determine the relationship between functional outcomes and sleep quality. Independent variables were total sleep time (TST) and sleep efficiency (SE) derived from wearable sensors, calculated between two clinical measures. Dependent variables included scores from repeated measures of the 6-Minute Walk Test (6MWT), 10-Meter Walk Test (10MWT), Berg Balance Scale (BBS), and Action Research Arm Test (ARAT). Covariates included demographics such as age and stroke type.ResultsFifty-three individuals with stroke (age: 58.26±15.57 years; BMI: 28.27±6.16 kg/m2) consented to participate during their IRF program within 7 days of admission. All individuals were recruited from a single-site IRF between July 2020 and August 2021. The average length of stay was 17.85±6.99 days. There were no significant differences in TST between the first three nights and the last three nights (5.1±1.9 hours vs. 5.2±2.0 hours) or SE (67.8±17.7% vs. 69.0±17.8%). The greater standard deviation of TST was associated with lower 6MWT scores (R2=0.77, beta=−0.48, p=0.06), while the greater standard deviation of SE was associated with lower 10MWT scores (R2=0.80, beta=−0.20, p=0.18).ConclusionOur preliminary findings indicate that greater variability in TST and SE are associated with walking endurance and mobility recovery. Future analyses will investigate additional measures of sleep and activity in IRF settings and their relationship with patient outcomes. Abstract:Patterned "bead in pore" composite film with hemispherical or mushroomlike TiO(2) microparticles lying in the holes of a honeycomblike polystyrene matrix has been fabricated by a template-free bottom-up approach from a homogeneous solution of TiCl(4)/polystyrene/CHCl(3) using the breath figures method. It is a very simple way to prepare hemispherical or mushroomlike TiO(2) microparticles and to get the hexagonally nonclose-packed arrays of asymmetrical particles with or without polymer matrix, which have potential applications in photonics.

Abstract:Background To investigate the association between sleep disorder and depression in stroke population using data from the National Health and Nutrition Examination Surveys (NHANES) conducted between 2005 and 2014. Methods Participants included stroke population who reported on sleep disorder and completed the Patient Health Questionnaire-9 (PHQ-9) to assess depressive symptom. Sleep disorder was self-reported and categorized dichotomously. Multivariate logistic regression, subgroup analysis, receiver operating characteristic curve (ROC) and restricted cubic spline (RCS) were used to explore the association between sleep disorder and depression. Results The final sample included 566 stroke survivors (weighted n = 3,326,625). Multivariate logistic regression analysis confirmed that sleep disorder was independently associated with an increased risk of depression even after adjusting for potential confounders (OR = 2.616, 95% CI: 2.561 to 2.599, P < 0.0001). Subgroup analysis indicated that this association was particularly strong among female stroke population (OR = 5.515). Sleep disorder as a significant predictor of depression, while the relationship between sleep time and depression is characterized by non-linearity. Conclusions Sleep disorder is significantly associated with an increased risk of depression in stroke. This relationship remains robust across various sociodemographic and lifestyle factors, highlighting the need for integrated interventions targeting both sleep and mood disturbances in stroke care.

Abstract:Objective This study aims to investigate the effects of sleep disorders on the prognosis of neurological function after stroke and other factors affecting the prognosis after stroke. Method We designed a cohort study. A total of 1,542 patients with their first stroke were hospitalized in the department of neurology of Tianjin Huanhu Hospital from 2015.6.1 to 2016.12.31. We recorded the personal histories of patients. The MMSE (mini-mental state examination), MoCA (Montreal Cognitive Assessment), HAMD (Hamilton Depression Scale), National Institutes of Health Stroke Scale (NIHSS) score, mRS (Modified Rankin Scale), BI (Barthel Index), PSQI (Pittsburgh Sleep Quality Index), ESS (Epworth Sleepiness Scale), Berlin questionnaire, and nocturnal TST (Total sleep time) were assessed before discharge, 3 months, 6 months, and 4 years (2019–2020) after stroke. Result Low sleep quality (OR 2.019, 95%CI 1.199–3.398, p = 0.008), nocturnal TST (<7 h) (OR 4.060, 95%CI 1.494–11.034, p = 0.006), nocturnal TST (>8 h) (OR 5.928, 95% CI 2.134–16.464, p = 0.001) were risk factors for poor neurological function recovery at 3 months after stroke. Nocturnal TST (<7 h) (OR 13.042, 95%-CI 2.576–66.027, p = 0.002) and nocturnal TST (>8 h) (OR 11.559, 95%-CI 2.108–63.390, p = 0.005) were risk factors for poor neurological function at 6 months after stroke. Nocturnal TST (<7 h) (OR 2.668, 95% CI 1.250–5.698, p = 0.011) and nocturnal TST (>8 h) (OR 2.516, 95% CI 1.080–5.861, p = 0.033) were risk factors for poor neurological function at 4 years after stroke. High risk of OSA (HR 1.582, 95%CI 1.244–2.012, p < 0.001) was a risk factor for all-cause death in patients followed up for 4 years after stroke. Conclusion Low sleep quality is associated with short-term poor neurological function after stroke. Unusual nocturnal TST (long or short) is associated with short-term or long-term poor neurological function after stroke. A high risk of OSA is associated with a higher risk of all-cause death after stroke.

Abstract:Abstract Introduction Affective instability, a form of emotion dysregulation, is defined by rapid and intense shifts in affect and a known precursor of affective disorders. Poor sleep health leads to emotional dysregulation and affective instability. Poor sleep health is common after stroke, but which specific aspects of sleep health are important is unclear. We hypothesized that a multidimensional assessment of sleep health would identify specific factors that contribute to affective instability in stroke survivors. Methods Forty community-dwelling stroke survivors underwent multidimensional sleep health assessment with sleep diaries, the Pittsburgh Sleep Quality Index and ecological momentary assessment of affect and alertness eight times daily for seven days. Sleep health was quantified into 6 domains of the Regularity, Satisfaction, Alertness, Timing, Efficiency, Duration (RU-SATED) framework. Affective instability was quantified using probability of acute change (PAC) and mean squared successive difference (MSSD). Multivariable linear regressions were used to identify sleep health factors associated with affective instability, adjusting for age, sex, and race. Results Instability of depressed affect was associated with lower alertness (MSSD: B=.55, p=.001; PAC: B=.76, p<.001), lower sleep efficiency (B=.35, p=.036), and longer sleep latency (B=.38, p=0.30). Instability of cheerful affect was associated with less regular mid-sleep time (MSSD: B=.57, p<.001; PAC: B=.48, p=.004), less regular sleep duration (B=.43, p=.012), and longer sleep latency (MSSD: B=.39, p=.020; PAC: B=.39, p=.021). Conclusion Lower sleep efficiency, longer sleep latency, lower alertness, and irregular sleep contribute to affective instability among stroke survivors. Future intervention efforts managing these specific factors of sleep health to stabilize affective changes and prevent affective disorders after stroke are needed. Support (if any)
