Abstract:Abstract Background: Ultrasonic measurements of tongue thickness and condylar translation were recently introduced to predict difficult laryngoscopy in non-obstetric patients. We designed the present study to evaluate the performance of these two ultrasonic indicators in predicting difficult laryngoscopy in healthy parturients. Methods: The 119 parturients undergoing elective cesarean delivery were enrolled. Tongue thickness and condylar translation measured by ultrasonography, and Modified Mallampati test (MMT) score, inter-incisor distance (IID) and modified Cormack-Lehane grading system (MCLS) were measured and recorded before anesthesia. The primary outcome was difficult laryngoscopy defined as MCLS 3 or 4. The association between these variables and difficult laryngoscopy were analyzed by using multivariable logistic regression and receiver operating characteristic (ROC) curve. Results: Compared to the Easy Laryngoscopy Group, the tongue thickness was significantly higher and the condylar translation and IID were significantly lower in the Difficult Laryngoscopy Group. Tongue thickness and condylar translation but not MMT score and IID were proved to be two independent predictors for difficult laryngoscopy by multivariate logistic regression, with the odds ratios of 2.554 (95% confidence interval (CI), 1.715 to 3.802) and 0.457 (95% CI, 0.304 to 0.686). The area under the ROC curve to predict difficult laryngoscopy for tongue thickness was 0.93 (95% CI, 0.88–0.98) and for condylar translation was 0.77 (95% CI, 0.67–0.86), which were significantly higher than those for MMT score (0.67, 95% CI, 0.56–0.77) and IID (0.65, 95% CI, 0.55–0.76). Conclusions: Compared with MMT and IID, tongue thickness and condylar translation measured by ultrasonography appear to be better indicators for predicting difficult laryngoscopy in parturients. Abstract:ABSTRACT Background and Aims: Ultrasonography has emerged as a new airway assessment tool. However, its role in predicting difficult airways needs to be explored. This study aimed to evaluate the accuracy of pre-operative ultrasound assessment of the neck in predicting difficult airways in patients undergoing elective surgery under general anaesthesia. Methods: One hundred and fourteen adult patients undergoing elective surgeries under general anaesthesia were enrolled in this study. In the pre-operative room, upper airway ultrasound measurements of the neck were obtained, namely, distance from skin to the hyoid bone, distance from skin to the thyroid isthmus and thickness of the base of the tongue. Clinical airway assessment details were noted from the pre-anaesthetic evaluation form. The airway management technique was noted. Receiver operating characteristic curves were used to assess the diagnostic value of these upper airway ultrasound measurements in predicting difficult airways. Results: The distance from the skin to the thyroid isthmus in the difficult airway group (0.37 ± 0.133 cm) was significantly higher than in the non-difficult group ( P = 0.007). It appeared to be a better predictor of difficult airways and correlated better with clinical tests among the measured ultrasound parameters. The body mass index was significantly higher in the difficult airway group ( P = 0.009). Conclusion: Considering the difference in means between the two groups, distance from the skin to the thyroid isthmus should be explored as a potential predictor of a difficult airway in studies with a larger sample size.
A new score for airway assessment using clinical and ultrasound parameters

Abstract:Background Over the last few years, ultrasonography has been introduced as the fifth pillar to patient’s bedside physical examination. Clinical assessments aim to screen and look for airway difficulties to predict difficult intubations, but none have demonstrated a significant predictive capacity. Recent systematic reviews have established a correlation between ultrasound imaging and difficult direct laryngoscopy. The primary objective of this study was to determine whether the utilization of ultrasonography to examine the upper airway could accurately predict difficult direct laryngoscopy. Methods This is a prospective observational study including 102 adult patients that required general anesthesia for elective surgery. Preoperatively, clinical airway assessments were performed. Data such as Mallampati-Samsoon grade (MS), upper lip bite test (ULBT), thyromental (TMD) and sternomental distance (SMD), cervical circumference (CC) and the Arné risk index were collected. Ultrasound evaluation was taken at five different levels in two planes, parasagittal and transverse. Therefore, the following measurements were registered: distance from skin to hyoid bone (DSHB), distance from skin to thyrohyoid membrane (DSTHM), distance from skin to epiglottis (DSE), distance from skin to thyroid cartilage (DSTC) and distance from hyoid bone and thyroid cartilage (DHBTC). Patients were divided into two groups based on the difficulty to perform direct laryngoscopy, according to Cormack-Lehane (C-L) classification. Grades I and II were classified as easy laryngoscopy and grades III or IV as difficult. Logistic regression models and the Receiver Operating Characteristic (ROC) curve was employed to determine the diagnostic precision of ultrasound measurements to distinguish difficult laryngoscopy (DL). Results The following risk score for DL was obtained, DSTHM ≥ 1.60 cm (2 points), DSTC ≥ 0.78 cm (3 points) and gender (2 points for males). The score can range from 0 to 7 points, and showed and AUC (95% CI) of 0.84 (0.74–0.95). A score of 5 points or higher indicates a 34-fold increase in the risk of finding DL ( p = 0.0010), sensitivity of 91.67, specificity of 75.56, positive predictive value of 33.33, and negative predictive value of 98.55. Conclusion The use of ultrasonography combined with classic clinical screening tests are useful tools to predict difficult direct laryngoscopy.

Ultrasound-based airway assessment in obese patients as a valuable tool for predicting difficult airway: an observational study

Abstract:Background: Difficult airway, characterized by difficult mask ventilation and intubation, is common in obese patients undergoing surgery. The purpose of this study was to evaluate and compare the prognostic efficiency of ultrasound-measured anterior cervical soft tissue parameters as an indicator of difficult airway during anesthesia induction in obese patients. Methods: This prospective, double-blind, observational study was conducted at Balikesir University Faculty of Medicine Hospital between March 2020 and March 2022. A total of 157 patients age ≥ 18 (BMI ≥ 30 kg.m-2), without previous head and neck surgery were included in the study. Anterior cervical soft tissue measurements were performed at three levels; minimum distance between the hyoid bone and skin at the level of the hyoid bone; (DSHB), distance between the midpoint of the epiglottis and skin at the level of the thyrohyoid membrane; (DSE), distance between the anterior commissure of vocal cords and skin at the vocal cord level; (DSV). The Han scale was used to assess difficult mask ventilation and the Cormack-Lehane scale was used to assess difficult laryngoscopy. Results: In the difficult laryngoscopy group, the mean values of DSHB, DSE and DSV were 18.5 ± 3.5, 18.3 ± 3.8, and 18.6 ± 3.4, respectively. The AUC values for DSHB, DSE, and DSV were 0.845, 0.827, and 0.850, respectively. Anterior cervical measurements showed a better predictive value for difficult laryngoscopy compared to difficult mask ventilation. Conclusion: Ultrasonographic measurements were predictive for difficult laryngoscopy and ventilation with better correlation in laryngoscopy.

Abstract:To evaluate the utility of ultrasound for detection of the difficult intubation in a preoperative setting. PubMed, Ovid, CINAHL Plus Full Text, and Google Scholar searches using [“difficult airway” OR “difficult intubation” OR “difficult laryngoscopy” OR “difficult ventilation”] AND [ultrasonography OR sonography OR ultrasound] without date limitations. Abstracts without publications, case reports, letters, textbooks, unrelated topics, or foreign language articles were excluded. Ancestry references were included from the reviewed articles. Two reviewers independently performed the query. Each study was reviewed using the STARD checklist to assess blinding, incomplete data reporting, subject attrition, and selection of appropriate statistical tests. Ten studies were included. All used convenience sampling of adult subjects requiring direct laryngoscopy in elective surgical settings. One study is retrospective and nine are prospective observational. Populations included non-obese, obese, pregnant, and thyroidectomy patients in the United States, Turkey, Israel, Canada, Portugal, and China. Airway locations scanned are variable using different protocols and patient positioning. The outcome variable is uniformly the Cormack–Lehane Grade. 114 of the 681 total subjects had difficult laryngoscopies. Significance for sonographic prediction of difficult laryngoscopy occurred at three locations: hyomental distance [52.6 ± 5.8 mm (p < 0.01)], anterior tissue at the hyoid bone [16.9 mm (95 % CI 11.9–21.9) and 15.9 ± 2.7 mm (p < 0.0001)], and the thyrohyoid membrane [34.7 mm (95 % CI 28.8–40.7) and 23.9 ± 3.4 mm (p < 0.0001) and 28.25 ± 4.43 mm (p < 0.001)]. The vocal cords and sternal notch levels have conflicting significance. Limitations include the heterogeneous populations and lack of standard scanning protocols.

Evaluation and comparison of sonographic difficult airway assessment parameters with clinical airway predictors

Abstract:Abstract Background and Aims: None of the clinical difficult airway predictors are 100% sensitive and specific. Ultrasound is being used for airway assessment, but there is still no established parameters or model to predict difficult laryngoscopy. This observational study was planned to determine the predictive ability of clinical and sonography-based airway assessment parameters for difficult laryngoscopy and intubation. Material and Methods: A total of 130 patients of 18–60 years of age undergoing elective intubation were included. The distribution of Cormack–Lehane (CL) grade and intubation difficulty scale (IDS) was correlated with the clinical and sonographic screening parameters for difficult airways. Results The prevalence of difficult laryngoscopy and difficult intubation in our study was 17.6% and 11.5%, respectively. Mallampati grade (MMG), upper lip bite test (ULBT), neck circumference, hyomental distance ratio (HMDR), tongue thickness (TT), skin to epiglottis/epiglottis to vocal cord distance (SED/E-VC), and mandibular condylar mobility (MCM) had significant association with the difficult laryngoscopy and MMG, neck circumference, SED, SED/E-VC; MCM had significant association with the difficult intubation. The combination of these predictors showed better diagnostic ability for difficult airways. Model 1 based on ultrasound parameters showed an area under the curve (AUC) of 0.848 (CI- 0.748-0.947, P value < 0.0001) and model 2 based on combined clinical and ultrasound parameters showed an AUC of 0.755 (95% CI- 0.631-0.879, P value < 0.0001). Conclusions: Ultrasound-based airway predictors can help in predicting difficult laryngoscopy and intubation along with the clinical parameters. Individual sonographic predictors have moderately satisfactory diagnostic profiles. The models based on combined tests have better diagnostic value.

Abstract:Background: Preoperative assessment of the airway with ultrasound has come up as a viable option to aid the preanaesthetic airway assessment. Objectives: To identify reliable ultrasound parameters as predictors of difficult airway in patients undergoing surgery under general anaesthesia. Methods: A prospective analytical cross-sectional study was conducted at Kathmandu Medical College in 99 consecutive patients scheduled for elective surgery with general anaesthesia and endotracheal intubation from July 2019 to June 2020 after ethical clearance. Three ultrasound parameters were used for this study. Distance from the skin to the anterior aspect of trachea at the level of vocal cords (ANS-VC), the depth of the pre-epiglottic space (PreE), the distance from the epiglottis to the midpoint of vocal cords (EVC) was measured. The PreE/EVC and hyomental distance ratio (HMDR), which is the distance ratio of hyomental distance at neutral and extended head position were calculated. These ultrasonographic parameters were used to predict difficult laryngoscopy Cormack-Lehane (CL) grading 3, 4. Results: Difficult intubation was seen in 23 (23.2%) as CL grade 3. The authors did not encounter CL grade 4. HMDR and PreE/EVC have been shown to have significant association with CL grading, with a specificity of 71% and 77% respectively and a high negative predictive value of 84.3% and 84.2% respectively. Therefore, it is valuable in predicting difficult intubation. ANS-VC did not have a significant correlation. Conclusion: Diagnostic predictability of difficult airway is better with HMDR and PreE/E-VC.
A quantitative study of airway ultrasound in predicting difficult laryngoscopy: A prospective study

Abstract:Purpose: As common clinical screening tests cannot effectively predict a difficult airway, and unanticipated difficult laryngoscopy remains a challenge for physicians. We herein used ultrasound to develop some point-of-care predictors for difficult laryngoscopy. Methods: This prospective observational study included 502 patients who underwent laryngoscopy and a detailed sonographic assessment. Patients under 18 years old, or with maxillofacial deformities or fractures, limited mouth opening, limited neck movement or history of neck surgery were excluded from the study. Laryngoscopic views of all patients were scored and grouping using the modified Cormack-Lehane (CL) scoring system. The measurements acquired comprised tongue width, the longitudinal cross-sectional area of the tongue, tongue volume, the mandible-hyoid bone distance, the hyoid bone-glottis distance, the mandible-hyoid bone-glottis angle, the skin-thyrohyoid membrane distance, the glottis-superior edge of the thyroid cartilage distance (DGTC), the skin-hyoid bone distance, and the epiglottis midway-skin distance. ANOVA and Chi-square were used to compare differences between groups. Logistic regression was used to identify risk factors for difficult laryngoscopy and it was visualized by receiver operating characteristic curves and nomogram. R version 3.6.3 and SPSS version 26.0 were used for statistical analyses. Results: Difficult laryngoscopy was indicated in 49 patients (CL grade Ⅲ - Ⅳ) and easy laryngoscopy in 453 patients (CL grade Ⅰ - Ⅱ). The ultrasound-measured mandible-hyoid bone-glottis angle and DGTC significantly differed between the 2 groups (p < 0.001). Difficult laryngoscopy was predicted by an area under the curve (AUC) of 0.930 with a threshold mandible-hyoid bone-glottis angle of 125.5° and by an AUC of 0.722 with a threshold DGTC of 1.22 cm. The longitudinal cross-sectional area of the tongue, tongue width, tongue volume, the mandible-hyoid distance, and the hyoid-glottis distance did not significantly differ between the groups. Conclusion: Difficult laryngoscopy may be anticipated in patients in whom the mandible-hyoid bone-glottis angle is smaller than 125.5° or DGTC is larger than 1.22 cm.

Abstract:Background The study was conducted with the aim of determining the usefulness of ultrasonography in assessment of difficult airway preoperatively to compare and correlate airway assessment done clinically and airway viewed ultra sonographically with Cormack–Lehane classification of the direct laryngoscopy. Methodology This prospective, observational trial consists of total 150 patients undergoing elective surgeries under general anesthesia. The measurements recorded were interincisor gap, modified Mallampati's classification, and thyromental distance and the airway assessment of ultrasound done. Based on the Cormack–Lehane classification of laryngoscopic view, patients were classified into different groups: group A—easy intubation and group B—difficult intubation, at the end of the study. Results In both the groups, demographic data were similar except weight, which was significant in group B. Ultrasound measurements of airway done at four levels—hyoid bone, suprasternal-notch, thyroid isthmus, and thyroid—were increased in group B compared with group A, with p -values 0.0002, 0.0001, 0.001, and 0.0001, respectively, showing significant results. Conclusion On the basis of our study, we conclude that by measuring the thickness of soft tissues in the anterior part of neck with ultrasound difficult airway can be predicted, thus ultrasound can be used for assessing difficult airway preoperatively. Comparison of Ultrasound Parameters and Clinical Parameters in Airway Assessment for Prediction of Difficult Laryngoscopy and Intubation: An Observational Study

Abstract:Background and objective The primary responsibility of the anesthesiologist is to provide adequate oxygenation and ventilation to the patient by securing the airway. Prediction of Cormack-Lehane (CL) grading preoperatively helps patients' airway management during anesthesia induction, particularly in difficult intubations. Our study aims to evaluate airway assessment modalities using ultrasound and conventional clinical screening methods for predicting difficult laryngoscopy and intubation. Materials and methods This prospective observational study was conducted on 100 patients aged between 18 and 70 years belonging to ASA classes I, II, and III scheduled for elective surgery requiring general anesthesia under endotracheal intubation was included in the study. Patients who needed rapid sequence induction and had a history of difficult intubation, obese patients with a body mass index (BMI) of more than 40, patients with notable swelling in the neck region (thyroid), pregnant patients, and patients with maxillofacial anomalies were excluded from the study. Clinical parameters such as body mass index, neck circumference, modified Mallampati grading, thyromental distance, and ultrasound parameters such as anterior neck soft tissue thickness at the level of the thyrohyoid membrane (ANS-TM) and anterior neck soft tissue thickness at the level of vocal cord (ANS-VC) were obtained preoperatively. After intubation, the CL grading was noted and categorized into two groups: easy (classes 1 and 2) and difficult (classes 3 and 4). Descriptive statistics included frequency and percentage for categorical variables and mean±standard deviation for continuous variables. The chi-square test was applied to find the relationship between easy and difficult laryngoscopy when compared with the outcome for categorical variables. A P value of less than 0.05 was considered significant throughout the study. The receiver operating characteristics curve (ROC curve) was used to determine the sensitivity and specificity to predict the outcomes. Results Ultrasound-guided measurements of ANS-TM and ANS-VC are independent predictors of difficult laryngoscopy compared with clinical screening tests. Of the two parameters, we found that ANS-TM has a better diagnostic value for predicting a difficult airway with an area under the ROC curve (AUC) of 91% compared with ANS-VC, which has an AUC of 84%. Of the clinical parameters, the modified Mallampati grading has an AUC of 81%, leading to better diagnostic value in the prediction of a difficult airway. Conclusion Our study demonstrated that ANS-TM and ANS-VC are independent predictors of a difficult airway. ANS-TM has a better correlation with CL grading. Clinical screening tests should be combined with ultrasound measurements to aid in the better prediction of difficult laryngoscopy.

Abstract:BackgroundUltrasound is a simple, noninvasive complementary tool for preoperative airway assessment. This study compared airway ultrasound and conventional airway assessments to predict difficult intubation.MethodsAirway assessment was performed in 99 patients scheduled for bariatric surgery using the following two parameters: El-Ganzouri score (EGS) and oblique transverse ultrasound view. The following measurements were gathered with ultrasound: pre-epiglottic space depth (pre-E), distance from the epiglottis to the mid-vocal cords (E-VC), mean distance between the skin and epiglottis (mDSE), distance between the skin and hyoid bone (DSHB), and tongue volume. After induction of anesthesia, direct laryngoscopy was performed and Cormack–Lehane (CL) classification was reported.CL grades 1 and 2 were classified as the easy laryngoscopy (EL) group, Grades 3 and 4 were classified as having a difficult laryngoscopy (DL). The crucial performance measure was the E-ability VC's to anticipate difficult intubation. Association between these factors and the CL categorization was a significant indication.ResultsIn 12.12 percent of cases, hard intubation was seen. The DL group had a lower E-VC than the EL group (0.9 ± 0.28 cm vs. 1.52 ± 0.3 cm; P -value less than0.001). The DL cutoff point was <1.22 cm, with 91.7% sensitivity and 83.9% specificity. The CL classification indicated a strongly positive association with the EGS and strongly negative association with the E-VC measurement (r = 0.700 and –0.828, consecutively; P-value less than 0.001), moderately positive correlation with mDSE and DSHB (r = 0.433 and 0.566, consecutively; P-value less than 0.001), and weakly positive correlation with pre-E and tongue volume (r = 0.331 and 0.344, respectively; P-value less than 0.001).ConclusionsUltrasound as a complementary measure to anticipate difficult intubation is beneficial with high sensitivity and low specificity in avoiding the hazards of unanticipated, difficult intraoperative laryngeal visualization.

Abstract:Objective To evaluate the efficacy of ultrasound for placement of laryngeal mask airway (LMA) in comparison with fiberoptic bronchoscope in adult patients.Methods Fifty-eight ASA physical status Ⅰ or Ⅱ patients,aged 18-57 yr,weighing 50-70 kg,with body mass index 20-30 kg/m2,scheduled for gynecological operation under general anesthesia,underwent standard general anesthetic technique with 4 # LMA-ClassicTM.After clinical assessment of correct placement of LMA,the position of the LMA was confirmed by transverse neck ultrasound,and reconfirmed using fiberoptic bronchoscopy.The peak airway pressure and oropharyngeal leak pressure were recorded.Results When the outcome of clinical assessment of correct placement of LMA was considered as satisfactory,the rate of ideal anatomic placement assessed by ultrasound evaluation was 69 %,and the rate of ideal anatomic placement assessed by fiberoptic bronchoscopy was 59 %,and it showed that there was no significant difference in detecting the ideal placement of the LMA (P ＞ 0.05).The oropharyngeal leak pressure was significantly higher when ultrasound was used for confirmation of the correct placement of LMA than that when fiberoptic bronchoscope was used (P ＜ 0.05).Conclusion Ultrasound provides better efficacy for placement of LMA in indicating sealing of LMA than fiberoptic bronchoscope and is helpful in improving ventilation conditions in adult patients.

Abstract:BACKGROUND: Ultrasound is evolving as a probable tool in airway assessment. The upper airway is a superficial structure and has sonographically identifiable structures which makes it ideal for evaluation with the ultrasound.AIMS: The aim of this study was to evaluate the role of skin to hyoid and skin to thyrohyoid membrane distance in prediction of difficult laryngoscopy.SETTINGS AND DESIGN: This is a prospective observational study included 150 patients aged 18-60 years of American Society of Anesthesiologists Physical Status I and II scheduled to undergo surgery under general anesthesia requiring laryngoscopy and endotracheal intubation.MATERIALS AND METHODS: The modified Mallampati score, mouth opening, mentohyoid distance, thyromental distance were noted. Skin to hyoid bone distance and skin to thyrohyoid membrane distance were measured by ultrasound. Patients were clubbed retrospectively into easy and difficult laryngoscopy groups on the basis of Cormack Lehane grading, and the characteristics of both groups were compared.STATISTICAL ANALYSIS: Statistical Package for the Social Sciences, Version 23 was used for statistical analysis. Independent samples t-test was used to compare the means between difficult and easy laryngoscopy patients. Diagnostic accuracy of the significant (P < 0.05) variables between difficult and easy laryngoscopy patients was calculated using receiver operating characteristics curve in terms of their area under curve. Appropriate cutoff values (with corresponding sensitivity, specificity, and overall accuracy) were also identified.RESULTS: Out of 150 patients, 13 (8.7%) were identified as difficult laryngoscopy whereas 137 patients (91.3%) were identified as easy laryngoscopy. The demographics of both groups were comparable. Mentohyoid distance, skin to hyoid bone distance, and skin to thyrohyoid distance were statistically different between easy and difficult laryngoscopy patients, with lower mentohyoid distance and higher skin to hyoid bone distance and skin to thyrohyoid distance in difficult laryngoscopy patients. Diagnostic accuracy of the mentohyoid distance (70.3%) was slightly superior to skin to hyoid bone distance (67.1%) and skin to thyrohyoid distance (68.1%).CONCLUSION: Ultrasound measurements of skin to hyoid bone and skin to thyrohyoid membrane distance fail to eclipse clinical parameters in accurately predicting a difficult laryngoscopy.

Abstract:Background: Several clinical tools are used for preoperative airway assessment. Ultrasound is an emerging tool in anesthesia, and its role in the upper airway needs to be explored. Aims and Objectives: We aimed to assess the role of airway ultrasound in predicting difficult laryngoscopy and airway changes following induction of anesthesia. Materials and Methods: Clinical airway assessment was performed with a modified Mallampati score in 100 elective surgical patients. Ultrasound variables in pre- and post-induction of anesthesia were: distance between the skin and vocal cords at the hyoid bone (DSVC hb), distance from the skin to the thyroid isthmus, distance from the skin to the tracheal ring at suprasternal notch level, and distance between the skin and the cricothyroid (DSCM). Cormack–Lehane (CL) grades 3b and 4 were classified as having a DL. Association of clinical, ultrasonography (USG) indicators and CL grading was correlated. Dynamic changes in anterior neck tissues following induction of anesthesia were also measured. Results: Pre-induction USG variables were significantly higher in the DL group compared to the NDL group (P<0.05), except DSCM. Post-induction all USG parameters were significantly higher in the DL group. Among USG variables, DSVC hb and DTSI had the highest AUCs of 0.801 and 0.772, respectively. DSVC hb had a sensitivity of 76% and a specificity of 100% for DL. DTSI had a sensitivity of 92% and a specificity of 72%. There was a significant increase in depth after anesthetic induction of all USG parameters except DSCM. Conclusions: Ultrasound of anterior airway tissues can be considered for prediction of DL and allows dynamic airway assessment.
Evaluation of neck ultrasound measurements as a difficult airway predictor: A prospective observational study

Abstract:This study aimed to investigate the relationship between cervical measurements and difficult airways using ultrasonographic measurements. American Society of Anesthesiologists grade I to III, male or female, 120 adult patients, undergoing elective surgery were enrolled in the study. The study involved measuring the distance of the trachea, cricoid cartilage, thyroid cartilage, vocal cord anterior commissure, and hyoid bone to the skin using a 10 to 13 MHz linear ultrasound probe in the transverse plane. Additionally, the length of the cricothyroid and thyrohyoid membranes, along with their distance from the skin, were measured using the probe in the sagittal plane. Subsequently, another experienced anesthesiologist conducted mask ventilation and intubation after the patient's induction of general anesthesia. Throughout this process, the patient was assessed for difficulties in mask ventilation, laryngoscopy, and intubation. 28 (23.3%) patients had a difficult airway. Analyzing the measurements associated with difficult airways, the most reliable predictor was the epiglottis midline-skin distance [AUC (area under the curve): 0.847, P < .001, cutoff: >19.9, sensitivity: 78.6%, specificity: 79.4%]. Additionally, other factors such as hyoid bone to skin distance, thyroid cartilage to skin distance, thyrohyoid membrane to skin distance, and vocal cord anterior commissure-skin distance were also identified as predictors for a difficult airway. The increase in the distance of the epiglottis midline, vocal cord anterior commissure, hyoid bone, thyrohyoid membrane, and thyroid cartilage to the skin at the level of the isthmus measured by ultrasonography is predictive of difficult airways. Based on our study outcomes, we assert that ultrasonographic evaluation can be used in the prediction of difficult airways.

Abstract:Background Clinical airway screening tests used to predict difficulties during airway management have low sensitivity and specificity. Point-of-care airway ultrasound has described measurements related to problems with difficult direct laryngoscopy. Nevertheless, the correlation between ultrasound parameters and videolaryngoscopy has not been published yet. The aim of this multicenter, prospective observational pilot study was to evaluate the applicability of clinical parameters and ultrasound measurements to find potential tracheal intubation difficulties when videolaryngoscopy is used. Methods Preoperatively, six clinical airway assessments were performed: (1) modified Mallampati score, (2) thyromental distance, (3) sternomental distance, (4) interincisal distance, (5) upper lip bite test, and (6) neck circumference. Six ultrasound parameters were measured in awake patients: (1) distance from skin to hyoid bone, (2) distance from skin to epiglottis, (3) hyomental distance in neutral head position, (4) hyomental distance in head-extended position, (5) distance from skin to the deepest part of the palate, and (6) sagittal tongue area. And finally, there was one ultrasound measure obtained in anesthetized patients, the compressed sagittal tongue area during videolaryngoscopy. The difficulty for tracheal intubation using a McGrathTM Mac videolaryngoscope, the percentage of glottic opening, and Cormack-Lehane grade were also assessed. Results In this cohort of 119 subjects, tongue dimensions, particularly the sagittal tongue area, showed a robust association with increased intubation difficulty using videolaryngoscopy. A multiparametric model combining the following three ultrasound variables in awake patients: (a) the distance from skin to epiglottis, (b) the distance from skin to the deepest part of the palate, and (c) the sagittal tongue area, yielded a sensitivity of 92.3%, specificity of 94.5%, positive predictive value of 82.8%, and negative predictive value of 97.8% ( p < 0.001). Conclusion Point-of-care airway ultrasound emerges as a more useful tool compared to traditional clinical scales to anticipate possible challenges during videolaryngoscopic intubation.

Ultrasound Guided Measurement of Anterior Neck Tissue for the Prediction of Difficult Airway: A Prospective Observational Study

Abstract:Objectives: To determine that ultrasound (US) measurements of anterior neck soft tissue thickness at hyoid bone, thyrohyoid membrane, and anterior commissure levels can be used to predict difficult laryngoscopy. Materials & methods: The present study included 100 patients of age group 18-60 years undergoing elective surgery under general anaesthesia. It was a prospective observational study which included patients with ASA physical status I and II. Excluded patients were with facial and neck deformities, neck trauma, or those undergoing surgery of the larynx, epiglottis and pharynx. Comparison analysis was performed using t-test for continuous variables and chi-square or Fisher exact test for non-continuous variables. Correlation analysis performed using Pearson test. Results: There were 39 out of 100 patients categorised as difficult laryngoscopy. Thickness at hyoid bone (DSHB), thyrohyoid membrane (DSEM) and anterior commissure (DSAC), MMS (modified Mallampati score), and BMI (body mass index) were greater in the difficult laryngoscopy group (p < 0.001). TMD (thyromental distance) was less in the difficult laryngoscopy group (p < 0.001). There was a strong positive correlation between DSEM and DSAC (r = 0.784). Moderate positive correlation was between DSEM and DSHB (r = 0.559), DSEM and MMS(r=0.437). The area under curve (AUC) of DSHB, DSEM, DSAC, TMD and MMS is >0.7. The optimal cut-off values for DSEM, DSHB, DSAC and TMD were 1.34 cm, 0.98 cm, 1.68 cm and 6.59 cm, respectively, in predicting difficult airway. Conclusion: Ultrasound measurement of soft tissue thickness at hyoid bone, thyrohyoid membrane, and anterior commissure of vocal cord are good independent predictors for difficult laryngoscopy. When combined with traditional screening tests it improves the ability to predict difficult laryngoscopy.

Abstract:The laryngeal mask airway (LMA) is a supraglottic device that is commonly used to provide lung ventilation during general anesthesia. LMA placement needs to be confirmed to provide adequate lung ventilation. To investigate the feasibility of using ultrasound examination, compared with clinical tests and fiberoptic laryngoscopy, to confirm LMA placement, we performed a clinical study of 64 female patients classified as American Society of Anesthesiologists Physical Status I or II who were scheduled for gynecologic surgery with LMA insertion for airway management. After insertion, placement of the LMA was confirmed by clinical tests, ultrasound examination and fiberoptic laryngoscopy. Of the 64 women, placement was confirmed as acceptable in 89.1% by clinical tests, in 59.4% by fiberoptic laryngoscope assessment and in 67.2% by ultrasound examination. With respect to patients with oropharyngeal leaks classified as high, there were no differences in confirmation of acceptable placement between clinical tests and ultrasound examinations (p = 0.092), but the number of patients determined to have acceptable placement by ultrasound examination was greater than that determined by fiberoptic laryngoscopy (p = 0.034). Thus, ultrasound examination is a superior technique for confirming the seal on the LMA.

Abstract:Abstract Background Unanticipated difficult airway remains a challenge for the anesthesiologist with no established standard criteria to predict difficulty in intubation. Our aim was to correlate the pre-anesthetic ultrasonographic (USG) airway assessment parameters with Cormack-Lehane (CL) grade at direct laryngoscopy view under general anaesthesia. This was a prospective, observational study on 150 adult patients between 18 and 70 years with the American Society of Anesthesiologist—Physical Status 1–2 requiring general endotracheal anesthesia for elective surgery. Results The incidence of difficult laryngoscopy was 22.7%. The sonographic distance from anterior neck surface to epiglottis (ANS-E) > 1.67 cm was observed to be a statistically significant USG predictor of difficult laryngoscopic view with sensitivity of 64.71% and specificity of 78.45% ( p = 0.000). The sonographic distance from anterior neck surface to hyoid bone (ANS-H) or to anterior commissure (ANS-AC) did not corelate with difficult laryngoscopy. The ultrasound (US) parameters had higher negative than positive predictive value. Conclusions We found ANS-E distance to be the most significant predictor of difficult laryngoscopy in our study. USG is a useful tool to identify the “at-risk” patients for difficult airway.
