Abstract:Following the introduction of a combined EIPV and OPV schedule in 1990, no more cases of paralytic poliomyelitis were diagnosed and no wild poliovirus strain was isolated in the sewage. 100% seropositivity and high geometric mean titers (GMTs) to the three types of poliovirus vaccine strains and to the type 1 poliovirus 1988 epidemic strain were observed at the completion of the immunization schedule and one year later. The study of the immune profile of various age groups vaccinated with either EIPV or OPV had shown that the immunity to the epidemic poliovirus strain was less satisfactory, in terms of percent of seropositivity and GMTs, than to the poliovirus vaccine strains. In individuals previously immunized with either EIPV or OPV a high and quick immune response to both the 1988 epidemic and vaccine strains was observed after a booster dose of OPV. Follow-up to four years after the booster pointed to some decrease in the extent and intensity of the seropositivity to the 1988 epidemic strain as compared to the immune profile to the poliovirus vaccine strains. Maintaining Israel in the phase of polio elimination requires: > 95% national coverage, homogenously distributed; maintaining high immune profile to the three poliovirus types, including a satisfactory immune level to the 1988 epidemic strain in all the age groups at risk of polio; early detection of the introduction of wild poliovirus into the environment and of the occurrence of foci of indigenous activity; reducing the risk of exposure to massive infectious doses.(ABSTRACT TRUNCATED AT 250 WORDS)
[Anti-angiogenic therapies for metastatic colorectal, breast and lung cancer: benefits and risks].

Abstract:Anti-angiogenic therapies have recently enriched the therapeutic armentarium against the most common cancers. Among these, bevacizumab, a monoclonal antibody against vascular endothelial growth factor, is currently used most frequently. While the addition of bevacizumab to chemotherapy improves overall survival in first and second line treatment of metastatic colorectal cancer, its effect in metastatic breast cancer is limited to improvements in tumor response and progression-free-survival. In non-small-cell lung cancer, the positive results of a first American phase III study have not been confirmed by a second European study and are subject to controversies. A summary of the data concerning anti-angiogenic therapies in these three cancers is presented including safety information.
Immune response to an intercalated enhanced inactivated polio vaccine/oral polio vaccine programme in Israel: impact on the control of poliomyelitis

Abstract:A combined enhanced inactivated polio vaccine (EIPV) and oral polio vaccine (OPV) programme was introduced in Israel in 1990, with the purpose of providing a solution to the persistent polio morbidity in spite of a 30 year long OPV programme. The schedule comprised two doses of EIPV administered at the age of 2 and 4 months, intercalated with two doses of OPV at 4 and 6 months, followed by a reinforcing dose with the two vaccines simultaneously administered at 12 months. The 5-year evaluation of the programme included: the assessment of clinical suspicions of polio, early immune response in successive cohorts administered the new schedule, dynamics of the immune profile in a cohort followed up to the age of 5, and monitoring of wild poliovirus excretion in sewage specimens collected in 25 permanent sites throughout Israel as well as from the Palestinian Authority. No paralytic polio cases associated with a wild or vaccinal poliovirus strain were detected since the introduction of the programme. At the age of 4 months, one week after administration of the second EIPV and first OPV dose, 100% seropositivity and high geometric mean titres (GMTs) of neutralizing antibody (NA) to the three vaccinal and to the wild poliovirus type 1, responsible for the 1988 polio outbreak, were observed. No change in percent of seropositivity occurred between the age of 6 and 12 months. Thirty days after the IPV and OPV reinforcing doses, GMTs to each of the four poliovirus strains were > or = 3037. Up to the age of 5, the seropositivity was unchanged. After a 2.5-10-fold decline in the first year following the completion of the programme, GMTs to the three vaccinal and the wild poliovirus strain levelled off at rather high values, considered protective. Between 1990 and 1995, 16 wild poliovirus type 1 strains were isolated in three separate episodes in Gaza Strip sewage and once only in one Israeli site very close to Gaza City. The rapidly established, high and persistent NA titre to the vaccinal and wild poliovirus strains and the presence of immunological memory are indicative of high individual protection throughout the first 5 years of life. The only one-time introduction, without circulation, of a wild poliovirus strain in a single Israeli settlement suggests community protection. The intercalated programme offers a contribution to polio eradication by providing a solution to the primary and secondary failure associated with OPV, as well as to the control of vaccine-associated paralytic poliomyelitis.

Abstract:The Global Polio Eradication Initiative (GPEI), launched in 1988, has successfully reduced wild poliovirus (WPV) cases by over 99.9%, with WPV type 2 and WPV3 declared eradicated in 2015 and 2019, respectively. However, as of 2024, WPV1 remains endemic in Afghanistan and Pakistan. Since 2000, outbreaks of circulating virus derived of polio vaccines (cVDPVs) have emerged in multiple regions, primary driven by low vaccine coverage rates (VCRs). The COVID-19 pandemic disrupted routine immunization, resulting in millions of unvaccinated children, and leaving many countries vulnerable to both WPV1 and cVDPVs outbreaks. This paper reviews the epidemiological landscape of poliomyelitis post the COVID-19 pandemic, and the strategies and challenges to achieve the global polio eradication.

Abstract:Brine flotation and gravity sedimentation coproscopical examinations were performed in stool samples from 69 of the 147 Iaualapiti Indians of the Xingu Park, Mato Grosso State, Brazil. Intestinal parasites were present in 89.9% of the population examined. High rates of prevalence were found for some parasite species: Ancylostomidae, 82.6%; Enterobius vermicularis, 26.1%; Ascaris lumbricoides, 20.3%; and Entamoeba coli, 68.1%. Infection by Trichuris trichiura, Schistosoma mansoni, Taenia spp. and Hymenolepis nana was not detected. Helminth's prevalence in children aged one year or less was comparatively low (33.3%). Quantitative coproscopy was done in positive samples for Ascaris and Ancylostomidae and the results expressed in eggs per gram of feces (EPG). Quantitative results revealed that worm burdens are very low and overdispersed in this Indian tribe, a previously unreported fact.

Abstract:To systematically review the evidence on the association between knowledge of HIV-positive status or use of antiretroviral therapy (ART) and high-risk transmission behaviors, we searched Ovid MEDLINE from 2004 to February 2012 and the Cochrane Library Database through the first quarter of 2012. Four observational studies meeting inclusion criteria addressed HIV-positive status and seven addressed the use of ART and effects on behavior. Studies including both average and high-risk populations were conducted in developed countries and were rated at least fair quality. Overall, knowledge of HIV-positive status was associated with less engagement in high-risk transmission behaviors, and the use of ART was not found to increase participation in high-risk transmission behaviors by HIV-positive individuals.

Abstract:Japan experienced a large outbreak of wild poliovirus in the 1960s. The government made an exceptional decision to import oral polio vaccines (OPVs) from the Soviet Union and Canada while bypassing the usual approval process for medical products. Mass vaccination and subsequent, routine immunization successfully contained the wild poliovirus; the last case in Japan was reported in 1980. Domestic OPV had been used to sustain Japan’s polio-free state. In 2012, the world’s first inactivated polio vaccine developed from the Sabin vaccine replaced OPVs. Domestic vaccines combined with Sabin-derived inactivated polio vaccine are currently used in Japan. When the World Health Assembly announced the Global Polio Eradication Initiative in 1988, the Japanese government made a commitment to support endemic countries. The Japan International Cooperation Agency supported the establishment of microbiological laboratories, surveillance, distribution of polio vaccines and capacity building. Vaccine-derived poliovirus emerged as a new, international risk in the early 2000s. Vaccine-derived poliovirus was also detected in several Asian countries and required an outbreak response with additional vaccinations and strengthened surveillance. Genetically stable, novel, oral polio vaccine type 2 became available for use in outbreak responses and was used in Indonesia. Japan maintains its commitment to work toward the eradication of the poliovirus.

Abstract:The recent discovery of type 2 vaccine-derived poliovirus (VDPV2) in sewage water samples from Khan Younis and Deir al Balah in Gaza is a critical public health emergency demanding immediate intervention 1 . This finding exposes thousands of Gazans, particularly those living in densely populated displaced persons' camps, to this highly infectious disease, which can cause severe deformities and paralysis. There is no cure for polio; vaccination is the most effective way to prevent infection and subsequent transmission. The aim of this report is to underscore the urgency of intervention and draw attention from The United Nations Children's Fund (UNICEF), the World Health Organization (WHO), and other international health organizations to prevent a potential outbreak that could jeopardize the global polio eradication program.Gaza's overcrowded living conditions, combined with inadequate sanitation and healthcare facilities, create an ideal environment conducive to the rapid spread of the virus. Such conditions have historically fueled polio outbreaks linked to contaminated water sources. In 2021, Nigeria similarly faced a resurgence of polio due to circulating vaccine-derived poliovirus (cVDPV) in areas with poor sanitation 2 . This reemergence was attributed to low vaccination coverage and inadequate water, sanitation, and hygiene (WASH) infrastructure, which allowed the virus to circulate and mutate 3 .In Gaza, the situation is more severe. In 2022, Gaza had high vaccination coverage-at approximately 99% -with polio nearly being eradicated 1 . However, the escalation of hostilities has led healthcare services to collapse, making comprehensive vaccination campaigns difficult to undertake. Close to 17,000 children have missed routine vaccinations, leaving them vulnerable 4 .The few operating health facilities are only partially functional, and there is a dire shortage of medical supplies and vaccines. Humanitarian efforts, such as those by UNICEF and the WHO, are essential but are hampered by the security situation and access problems.Wars and conflicts create fertile ground for infectious diseases to spread, through the destruction of health infrastructure, contamination of water sources, and the displacement of people. This opens up avenues for outbreaks of many largely preventable diseases, as historically observed in other nations in conflict-Iraq, Syria, and Yemen-with reemergence of polio, cholera, and measles in the wake of vaccination disruption and poor sanitary conditions 5 .Preventing a devastating polio outbreak in Gaza requires immediate, coordinated efforts from global health organizations, governments, and non-profits. The top priority must be improving WASH infrastructure, as inadequate access to clean water and poor sanitation significantly increase the risk of virus transmission. In Gaza, over 1.6 million civilians lack access to clean water, making it difficult to maintain basic hygiene and dramatically increasing the risk of waterborne diseases 6 . Furthermore, intensified surveillance and vaccination efforts should target all children under five years of age and will require support from the international community for vaccine supply and logistical assistance. Cross-border entry of medical supplies and vaccines into Gaza must be made possible against the background of ongoing conflict. Strengthening Gaza's health infrastructure is also important to contain the virus -this includes equipping and training health personnel to effectively screen and isolate polio cases, as well as conducting public awareness campaigns on the importance of vaccination and hygiene practices to prevent polio transmission.The presence of the polio virus in Gaza's sewage is a grave indicator of the region's vulnerability and the urgent need for comprehensive public health interventions. Without swift and decisive action, and the provision of essential aid, a polio outbreak could erupt, leaving the health and lives of Gaza's population hanging in the balance.

Polio eradication in India: current status

Abstract:India has reached the final stage of polio eradication. The polio partnership in India, under the leadership of the Government of India, mounted tremendous response to the outbreak. The progress since 2003 is the most significant in the history of polio eradication in India. Surveillance sensitivity was increased to reach the goal for polio eradication. Since nearly all polio cases now occurring in India are caused by type 1 poliovirus in children, monovalent oral polio vaccine type 1 (mOPV1) was introduced in select high-risk districts of UP, Bihar and Mumbai-Thane during the April and May 2005 National Immunisation Days and the June and August 2005 in 6 sub-national immunisation rounds. Strategies were also being implemented to improve the impact of supplementary immunisation activities in the high-risk areas. As a result of supplementary immunisation activities targeted using surveillance data, India has made striking progress towards polio eradication.
Two-stage system for robust neutral/lombard speech recognition

Abstract:Performance of current speech recognition systems is significantly deteriorated when exposed to strongly noisy environment. It can be attributed to background noise and Lombard effect (LE). Attempts for LE-robust systems often display a tradeoff between LE-specific improvements and the portability to neutral speech. Therefore, towards LE-robust recognition, it seems effective to use a set of conditionsdedicated subsystems driven by a condition classifier, rather than attempting for one universal recognizer. Presented paper focuses on a design of a two-stage recognition system (TSR) comprising talking style classifier (neutral/LE) followed by two style-dedicated recognizers differing in input features. First, the binary neutral/LE classifier is built, with a particular interest in developing suitable features for the classification. Second, performance of common speech features (MFCC, PLP), LE-robust features (Expolog) and newly proposed features is compared in neutral/LE digit recognition tasks. In addition, robustness to the changes of average speech pitch and various noise backgrounds is evaluated. Third, the TSR is built, employing two recognizers, each using style-specific features. Comparison of the proposed system with either neutralspecific or LE-specific recognizer on a joint neutral/LE speech shows an improvement 6.5o4.2 % WER on neutral and 48.1o28.4 % WER on LE Czech utterances.

Abstract:A simple and effective method of measuring diffusion rates of various acylating reagents in macro beads (in this work diameters greater than 570 microm) of amino-functionalized resins is presented. The extent of diffusion at various points of time was determined by treating beads with a staining reagent ("chloranil", 2,3,5,6-tetrachloro-1,4-benzoquinone) that colored the regions of the beads that had not yet been permeated by reagent (the central part of the beads). The volume of unstained resin (permeated part) was compared with the volume of the stained part, and diffusion rate constants were calculated. Factors influencing diffusion such as reagent concentration, solvent, temperature, resin type, and mechanical effects were investigated. The results showed that diffusion was promoted by increased temperature, good swelling of the resin, small reagents, and high concentrations but not by sonication or mechanical agitation.

Abstract:Neuroendocrine gastroenteropancreatic tumors constitute a heterogeneous group of neoplasms, with the primary tumors being located in the gastric mucosa, pancreas, and small and large intestine. The development of a second primary malignancy in patients with these tumors is a well-described phenomenon, and the reported incidence ranges from 12% to 46%. The most common site of associated noncarcinoid malignancies is the gastrointestinal tract, which involves from 30% to 60% of the tumors. We report a case of concurrent colon carcinoma and two neuroendocrine tumors of the duodenum.
Exploring the path to polio eradication: insights from consecutive seroprevalence surveys among Pakistani children

Abstract:Introduction: After trivalent oral poliovirus vaccine (tOPV) cessation, Pakistan has maintained immunity to type 2 poliovirus by administering inactivated polio vaccine (IPV) in routine immunization, alongside monovalent OPV type 2 (mOPV2) and IPV in supplementary immunization activities (SIAs). This study assesses the change in poliovirus type 2 immunity after tOPV withdrawal and due to SIAs with mOPV2 and IPV among children aged 6–11 months. Methods: Three cross-sectional sequential serological surveys were conducted in 12 polio high-risk areas of Pakistan. 25 clusters from each geographical stratum were selected utilizing probability proportional to size. Results: Seroprevalence of type 2 poliovirus was 49%, with significant variation observed among surveyed areas; 80% in Killa Abdullah, Mardan & Swabi, and Rawalpindi. SIAs with IPV improved immunity from 38 to 57% in Karachi and 60 to 88% in Khyber. SIAs with IPV following mOPV2 improved immunity from 62 to 65% in Killa Abdullah, and combined mOPV2 and IPV SIAs in Pishin improved immunity from 28 to 89%. Results also reflected that immunity rates for serotypes 1 and 3 were consistently above 90% during all three phases and across all geographical areas. Conclusion: The study findings highlight the importance of implementing effective vaccination strategies to prevent the re-emergence of poliovirus. Moreover, the results provide crucial information for policymakers working toward achieving global polio eradication.

Abstract:Background: The European region, certified as polio free in 2002, had recent wild poliovirus (WPV) introductions, resulting in a major outbreak in Central Asian countries and Russia in 2010 and in current widespread WPV type 1 circulation in Israel, which endangered the polio-free status of the region. Methods: We assessed the data on the major determinants of poliovirus transmission risk (population immunity, surveillance, and outbreak preparedness) and reviewed current threats and measures implemented in response to recent WPV introductions. Results: Despite high regional vaccination coverage and functioning surveillance, several countries in the region are at high or intermediate risk of poliovirus transmission. Coverage remains suboptimal in some countries, subnational geographic areas, and population groups, and surveillance (acute flaccid paralysis, enterovirus, and environmental) needs further strengthening. Supplementary immunization activities, which were instrumental in the rapid interruption of WPV1 circulation in 2010, should be implemented in high-risk countries to close population immunity gaps. National polio outbreak preparedness plans need strengthening. Immunization efforts to interrupt WPV transmission in Israel should continue. Conclusions: The European region has successfully maintained its polio-free status since 2002, but numerous challenges remain. Staying polio free will require continued coordinated efforts, political commitment and financial support from all countries.

As natural infection and different vaccines have different effects on the immune system, the patterns of humoral and mucosal immunity to polioviruses in the world today are complex but are crucial to the ultimate success of the eradication initiative. This paper describes the background of the current situation and current immunological patterns and discusses their implications for managing population immunity to polioviruses in the years ahead.

Abstract:Social science research on polio has been centred in the global south, where countries that remain endemic or vulnerable to outbreaks are located. However, closely-related strains of poliovirus were detected in the sewage systems of several New York State counties and London boroughs in 2022. These detections constituted the first encounters with polio in the United States and United Kingdom for a generation - for both public health agencies and publics alike. This paper takes the transnational spread of poliovirus in 2022 as an opportunity to critique how public health memories of twentieth-century polio epidemics were mobilised to encourage vaccine uptake among groups considered vulnerable to transmission, notably Orthodox Jewish families. The study integrates data collected in London and New York as part of academic engagement with health protection responses to the spread of polio. Methods in both settings involved ethnographic research, and a total of 59 in-depth semi-structured interviews with public health professionals, healthcare providers, and Orthodox Jewish community partners and residents. Analysis of results demonstrate that narratives of epidemiological progress were deployed in public health responses in London and New York, often through references to sugar cubes, iron lungs, and timelines that narrate the impact of routine childhood immunisations. While memories of polio were deployed in both settings to provoke an urgency to vaccinate, vulnerable publics instead considered the more recent legacy of the COVID-19 pandemic when deciding whether to trust recommendations and responses. Critical attention to memory places analysis on the divergences between institutional (public health agencies) and peopled (publics) responses to disease events. Responses to re-emerging infectious disease outbreaks engender a temporal dissonance when historical narratives are evoked in ways that contrast with the contemporary dilemmas of people and parents.
