Abstract:Self-reported cancer data from the California Teachers Study were validated by using California Cancer Registry data. The California Teachers Study cohort consists of 133,479 active and retired California teachers. In 1995-1996, data from a mailed questionnaire were linked to the California Cancer Registry data. Sensitivity and specificity of 11 types of cancer were calculated. Multivariate analyses were conducted to evaluate correlates of false-positive and false-negative reporting. Sensitivities showed great variation by cancer site. The highest sensitivities were observed for breast (96.4%) and thyroid (92.9%) cancers, whereas the lowest sensitivities were those for cervical (44.3%), endometrial (69.1%), and other skin (53.6%) cancers. The sensitivities for in situ cancers (at the time of diagnosis) were considerably lower than those for invasive cancers in about half of the cancer types surveyed. The specificities for individual cancer sites ranged from 90% to 99%; the highest were those for lung cancer, leukemia, and Hodgkin's disease (all 99.9%). The lowest specificity was for other skin cancer (90.2%). In situ stage at diagnosis and older age were significantly associated with false-positive reporting. Age and non-White race were associated with false-negative reporting. These findings suggest that the feasibility of using self-reported data without verification in epidemiologic studies of cancer varies by site.

Abstract:RATIONALE: Obesity and underweight are contraindications to lung transplantation based on their associations with mortality in studies performed before implementation of the lung allocation score (LAS)-based organ allocation system in the United States Objectives: To determine the associations of body mass index (BMI) and plasma leptin levels with survival after lung transplantation.METHODS: We used multivariable-adjusted regression models to examine associations between BMI and 1-year mortality in 9,073 adults who underwent lung transplantation in the United States between May 2005 and June 2011, and plasma leptin and mortality in 599 Lung Transplant Outcomes Group study participants. We measured body fat and skeletal muscle mass using whole-body dual X-ray absorptiometry in 142 adult lung transplant candidates.MEASUREMENTS AND MAIN RESULTS: Adjusted mortality rates were similar among normal weight (BMI 18.5-24.9 kg/m(2)), overweight (BMI 25.0-29.9), and class I obese (BMI 30-34.9) transplant recipients. Underweight (BMI < 18.5) was associated with a 35% increased rate of death (95% confidence interval, 10-66%). Class II-III obesity (BMI ≥ 35 kg/m(2)) was associated with a nearly twofold increase in mortality (hazard ratio, 1.9; 95% confidence interval, 1.3-2.8). Higher leptin levels were associated with increased mortality after transplant surgery performed without cardiopulmonary bypass (P for interaction = 0.03). A BMI greater than or equal to 30 kg/m(2) was 26% sensitive and 97% specific for total body fat-defined obesity.CONCLUSIONS: A BMI of 30.0-34.9 kg/m(2) is not associated with 1-year mortality after lung transplantation in the LAS era, perhaps because of its low sensitivity for obesity. The association between leptin and mortality suggests the need to validate alternative methods to measure obesity in candidates for lung transplantation. A BMI greater than or equal to 30 kg/m(2) may no longer contraindicate lung transplantation.
[Percutaneous interspinous dynamic stabilization (in-space) in patients with degenerative disease of the lumbosacral spine - a prospective study].

Abstract:INTRODUCTION Interspinous spacers are supposed to reduce the segmental extension with a decrease in the expansion of yellow ligaments into the spinal canal, thus avoiding the dynamic narrowing of the spinal canal and compression of nerve roots. The aim of this study was to evaluate clinical outcomes and post-operative complications during one year in patients mostly having suffered from spinal stenosis and treated by In-Space interspinous spacer (Synthes, USA). MATERIAL AND METHODS A total of 25 patients aged between 25 and 73 (average age 52.6) years, including 18 males and 7 females, with degenerative disease of the lumbosacral spine were indicated for surgery and prospectively followed up. The patients were operated on under general anaesthesia in the prone position, using a minimally invasive lateral percutaneous approach, under fluoroscopic control. The ODI and VAS values as well as X- rays (Range Of Motion and Sagittal angle of the operated segment) 6 and 12 months after the surgery were compared to each other and to those before surgery. The results were statistically analyzed. RESULTS The average ODI of the group was 47.2% before surgery and 17.48% 6 months (22.76% 12 months) after surgery, showing a statistically significant improvement by 63% (52% after 12 months). The average VAS of the group was 6.64 points before surgery and 2.96 points 6 months (2.8 points 12 months) after surgery, which showed a statistically significant improvement by 55.4% after 6 months (57.8% after 12 months) when compared to preoperative status. After surgery the lordotic sagittal angle remained in all cases; one year after surgery the angle increased due to the slight sinking of some implants. The extent of segmental motion was minimally changed (6.1° 6 months and 7.24° 12 months after surgery). No serious complications occurred. The effect of interspinous implants proved insufficient in two cases (one year and two years after surgery) and conversion to arthrodesis or decompression was performed. CONCLUSIONS 1. Percutaneous, minimally invasive insertion of an In-Space interspinous spacer is an effective and safe method of dynamic stabilization not accompanied by any serious complications. 2. ODI improved by 63% 6 months after surgery with a decrease in this effect 12 months after surgery. VAS for axial and radicular pain, as reported by patients, improved on average by 55.4% 6 months and by 57.8% 12 months after surgery. 3. In all cases, the lordotic sagittal angle remained after surgery and the extent of segmental motion from flexion to extension was minimally changed.

Abstract:Frailty is associated with increased mortality among lung transplant candidates. We sought to determine the association between frailty, as measured by the Short Physical Performance Battery (SPPB), and mortality after lung transplantation. In a multicenter prospective cohort study of adults who underwent lung transplantation, preoperative frailty was assessed with the SPPB (n = 318) and, in a secondary analysis, the Fried Frailty Phenotype (FFP; n = 299). We tested the association between preoperative frailty and mortality following lung transplantation with propensity score-adjusted Cox models. We calculated postestimation marginalized standardized risks for 1-year mortality by frailty status using multivariate logistic regression. SPPB frailty was associated with an increased risk of both 1- and 4-year mortality (adjusted hazard ratio [aHR]: 7.5; 95% confidence interval [CI]: 1.6-36.0 and aHR 3.8; 95%CI: 1.8-8.0, respectively). Each 1-point worsening in SPPB was associated with a 20% increased risk of death (aHR: 1.20; 95%CI: 1.08-1.33). Frail subjects had an absolute increased risk of death within the first year after transplantation of 12.2% (95%CI: 3.1%-21%). In secondary analyses, FFP frailty was associated with increased risk of death within the first postoperative year (aHR: 3.8; 95%CI: 1.1-13.2) but not over longer follow-up. Preoperative frailty is associated with an increased risk of death after lung transplantation.

Abstract:Rationale: Immediate graft performance after lung transplantation is associated with short- and long-term clinical outcomes. However, the biologic mechanism that determines outcomes is not fully understood. Objectives: To investigate the impact of cell death signals at 24 and 48 hours after lung transplantation on short- and long-term clinical outcomes. Methods: Plasma samples were collected pretransplantation and at 24 and 48 hours after transplant from 60 bilateral lung transplant recipients. Ten patients had primary graft dysfunction (PGD) grade 3 (PaO2/FiO2 ratio <200 or on extracorporeal membrane oxygenation support) at 72 hours after transplant (PGD group). The remaining 50 patients were defined as the control group. Levels of plasma M30 (signifying epithelial apoptosis), M65 (signifying epithelial apoptosis plus necrosis), and high-mobility group box 1 protein (HMGB-1; signifying necrosis of all cell types) were measured by ELISA and correlated with clinical outcomes. Survival analyses were performed using Kaplan-Meier curves and Cox proportional hazards regression. Prediction accuracy of markers was assessed by calculated area under the curve of receiver operating characteristic graph. Measurements and main results: The PGD group had significantly higher M30 and M65 levels at 24 and 48 hours after transplant compared with the control group. There was no significant difference in HMGB-1. Area under the curve for 1-year survival was 0.86, 0.93, and 0.51 for M30, M65, and HMGB-1 at 48 hours, respectively. Survival analysis showed that higher M30 and M65 levels at 24 and 48 hours were significantly associated with worse survival. M65 at 48 hours remained significant even after adjustment for PGD. HMGB-1 was not significantly associated with survival. Conclusions: Recipient plasma concentration of epithelial cell death markers (M30, M65) after lung transplantation is negatively correlated with early graft performance and long-term survival.
Addressing ethnic disparity in asthma trials

Abstract:Ethnic minorities experience poorer asthma outcomes. In Australia for example, there is a higher prevalence of asthma among Indigenous Australians when compared with other Australians. These Indigenous Australians with asthma have twice the rate of asthma hospitalizations. Indeed, asthma is the second most common reason for hospital admission among Indigenous Australians. Disturbingly, they also have three times the relative risk of asthma death. These ethnic disparities are not unique to Australia. Asthmatic individuals from other ethnic minorities such as African Americans and Maori in New Zealand face similar heightened risks. Such disparities are driven by a complex range of factors. This is best documented in the African American population, where first, African genetic ancestry amplifies the likelihood of developing asthma and influences the expression of disease phenotype. Second, physiology is different; African American patients perceive breathlessness less well, potentially masking the onset of asthma exacerbations. Third, lower socioeconomic status hinders health-care access. Fourth, social–environmental interactions impede medication adherence and compound obesity and smoking rates. Lastly, physician attitudes toward African American patients are worryingly negative, leading to qualitatively different medical consultations. Other ethnic minorities have not been as well studied as African Americans; however, existing data suggest many of these issues also apply to them. The factors discussed above combine to undermine effective asthma treatment. Recommended asthma action plans demand that a patient undertake a precise series of steps; early symptom recognition, adherence to agreed escalation and timely healthcare access. Yet these are the very issues where disadvantaged minorities struggle most. Medication efficacy may also vary across ethnic groups. Again, in African American asthma patients, in vitro responses to corticosteroids were diminished in T cells, findings that often reflect clinical steroid insensitivity. In a different study however, clinical responses of African Americans to inhaled corticosteroids (ICS) were preserved. More disquieting is evidence that African Americans fail treatment more frequently with long-acting beta-agonists, irrespective of concurrent ICS use. Underlying mechanisms for this remain unclear. Although betaadrenergic receptor polymorphisms do not seem to be responsible, other genetic variations may play a role. There is thus a tremendous potential for heterogeneity of asthma treatment effects in ethnic minorities. This makes their recruitment into mainstream trials a high priority, where they have traditionally been under-represented. A recent systematic review highlighted common barriers to study recruitment across four ethnic minorities. It also identified valuable facilitators to boost recruitment such as cultural congruence, community altruism and participant convenience. Even when substantial numbers of an ethnic minority are successfully recruited into asthma trials, a final challenge remains. Such trials are invariably powered to only detect a signal within the total cohort. Thus, subgroup analysis of ethnic groupspecific data is fraught with risks of type I (falsepositive) and type II (false-negative) error. Limiting the number of predefined analyses can reduce type I error. Ways of limiting type II error in this context will be discussed at the end of this article. In this issue of Respirology, Pilcher et al. report an important ethnic subgroup analysis of a large multicentre asthma trial. The parent study was a pragmatic randomized controlled trial, comparing combination budesonide/formoterol as maintenance and reliever therapy versus fixed-dose combination budesonide/formoterol plus as needed salbutamol, in individuals with poorly controlled asthma. One of the study’s strengths was the dispensing of metred dose inhalers with electronic monitoring, allowing accurate assessment of participants’ medication use. The study’s primary outcome was negative in that the frequency of at least one high-use episode of betaagonists was no different in the two arms. Secondary outcomes however were positive, with fewer days of high use of beta-agonists, and fewer severe asthma exacerbations in the budesonide/formoterol combination maintenance and reliever arm. Within this larger dataset, Pilcher et al. focus on the response of Maori patients. Three points are worthy of note. First, the authors should be congratulated for ethnic recruitment proportional to the New Zealand population. They enrolled 44 Maori out of the total 303 participants through culturally appropriate strategies. Second, their baseline data make sobering reading. Fasting Serum Low-Density Lipoprotein (LDL) Before Liver Transplantation Predicts Post-Transplant Survival

Abstract:BACKGROUND The disordered metabolism of liver function in liver cancer patients can affect postoperative survival after liver transplantation. We assessed whether the levels of various chemicals in liver metabolism prior to receiving a liver transplant were prognostic factors and metabolism markers in predicting survival rate. MATERIAL AND METHODS Seventy-seven patients received a donor liver transplant between June 2012 and April 2016. The basic level of fasting serum GLU, Crea, TBil, TC, TG, HDL, LDL, ApoA1, ApoB100, INR, and MELD scores of 77 patients were retrospectively analyzed. Each patient's survival was monitored to evaluate prognosis and long-term survival. RESULTS The overall survival rates of all patients post-transplant at 6-, 12-, 24-, and 36-month follow-up were 90.9%, 79.2%, 68.8%, and 64.9% respectively. Fasting serum levels of GLU (P=0.004), HDL (P=0.010), LDL (P=0.008), ApoA1 (P=0.028), and MELD scores (P=0.013) prior to liver transplantation were closely associated with the cumulative survival post-transplant in univariate analyses. Controlled fasting GLU of ≤5.12 mmol/L (P=0.019), LDL of ≤2.62 mmol/L (P=0.031), and MELD scores of ≤9 (P=0.013) before LT were significantly and independently associated with increased cumulative survival in the multivariate analyses. CONCLUSIONS Decreased fasting serum GLU, LDL, and MELD scores as independent risk factors prior to liver transplantation markedly increase cumulative survival.

Abstract:BACKGROUND: Chronic lung allograft dysfunction and its main phenotypes, bronchiolitis obliterans syndrome (BOS) and restrictive allograft syndrome (RAS), are major causes of mortality after lung transplantation (LT). RAS and early-onset BOS, developing within 3 years after LT, are associated with particularly inferior clinical outcomes. Prediction models for early-onset BOS and RAS have not been previously described.METHODS: LT recipients of the French and Swiss transplant cohorts were eligible for inclusion in the SysCLAD cohort if they were alive with at least 2 years of follow-up but less than 3 years, or if they died or were retransplanted at any time less than 3 years. These patients were assessed for early-onset BOS, RAS, or stable allograft function by an adjudication committee. Baseline characteristics, data on surgery, immunosuppression, and year-1 follow-up were collected. Prediction models for BOS and RAS were developed using multivariate logistic regression and multivariate multinomial analysis.RESULTS: Among patients fulfilling the eligibility criteria, we identified 149 stable, 51 BOS, and 30 RAS subjects. The best prediction model for early-onset BOS and RAS included the underlying diagnosis, induction treatment, immunosuppression, and year-1 class II donor-specific antibodies (DSAs). Within this model, class II DSAs were associated with BOS and RAS, whereas pre-LT diagnoses of interstitial lung disease and chronic obstructive pulmonary disease were associated with RAS.CONCLUSION: Although these findings need further validation, results indicate that specific baseline and year-1 parameters may serve as predictors of BOS or RAS by 3 years post-LT. Their identification may allow intervention or guide risk stratification, aiming for an individualized patient management approach.

Prognosis factors in heart-lung transplantation: 30 years experience in a reference center

Abstract:Introduction: Heart-lung transplantation has shown a progressive decrease in the number of procedures. There is a lack of information about this field in Spain. The main goal of this study is to analyze the experience of a national reference hospital. Methods: We performed a retrospective study of a historical cohort of heart-lung transplanted patients in a single center, during a 30 years period (from 1990 to 2021). The associations between variables were evaluated using the χ2 test or Fisher's exact test. Survival was analyzed using the Kaplan-Meier method. Differences were evaluated using the log-rank test and multivariate analysis with the Cox method. Results: A decrease in the number of procedures performed in the last decade was observed [2000-2009: 19 procedures (44.2%); 2010-2021: 15 procedures (34.8%)]. Early postoperative mortality was 23.3%, falling to 13.3% from 2010. In-hospital mortality was 41%, falling to 33% from 2010. Main factors related to higher mortality: previous thoracic surgery, corticosteroid therapy, extracorporeal circulation (ECLS) greater than 200 min, ischemia time greater than 300 min, and tracheal dehiscence (p < 0.005). Overall survival at one, five, and ten years was 58%, 44.7%, and 36.1%, respectively. Factors associated with lower survival rates: previous thoracic surgery, male donor, extracorporeal circulation greater than 200 min, ischemia time greater than 300 min, tracheal dehiscence and weight difference (p < 0.005). Conclusions: There has been a progressive decrease in the number of heart-lung transplantations, being more evident in the last decade, but showing an improvement in both mortality and survival.

Abstract:The effects of 900 MeV electron irradiation on different types of silicon substrates (standard and oxygenated float-zone, Czochralski, and epitaxial silicon) have been experimentally investigated. Irradiations up to a fluence of 2.1/spl times/10/sup 15/ e/cm/sup 2/ have been performed with the electron beam of the LINAC injector at the synchrotron light facility Elettra in Trieste (Italy). Irradiated devices have been electrically characterized by reverse I-V and C-V measurements. Substrate type inversion has been observed for standard and oxygenated float-zone but not for Czochralski and epitaxial devices. The effects of isothermal annealing cycles at 80/spl deg/C have also been studied, and the hardness factor of 900 MeV electrons, with respect to 1 MeV neutrons, has been experimentally estimated from the measurement of the reverse leakage current after annealing.
Implications of High Sensitivity Troponin Levels After Lung Transplantation

Abstract:Trends in high-sensitivity cardiac troponin I (hs-cTnI) after lung transplant (LT) and its clinical value are not well stablished. This study aimed to determine kinetics of hs-cTnI after LT, factors impacting hs-cTnI and clinical outcomes. LT recipients from 2015 to 2017 at Toronto General Hospital were included. Hs-cTnI levels were collected at 0–24 h, 24–48 h and 48–72 h after LT. The primary outcome was invasive mechanical ventilation (IMV) >3 days. 206 patients received a LT (median age 58, 35.4% women; 79.6% double LT). All patients but one fulfilled the criteria for postoperative myocardial infarction (median peak hs-cTnI = 4,820 ng/mL). Peak hs-cTnI correlated with right ventricular dysfunction, >1 red blood cell transfusions, bilateral LT, use of EVLP, kidney function at admission and time on CPB or VA-ECMO. IMV>3 days occurred in 91 (44.2%) patients, and peak hs-cTnI was higher in these patients (3,823 vs. 6,429 ng/mL, p < 0.001 after adjustment). Peak hs-cTnI was higher among patients with had atrial arrhythmias or died during admission. No patients underwent revascularization. In summary, peak hs-TnI is determined by recipient comorbidities and perioperative factors, and not by coronary artery disease. How safe is lung transplantation in patients of 65 years or older? A single-center retrospective cohort

Abstract:Introduction: With increasing experience in high-volume centers, age alone should not be an absolute contra-indication to lung transplantation (LT) but be considered as part of the patient's initial characteristics. The objective of this study is to provide early and long-term outcomes of LT in recipients aged 65 or older, compared with their younger counterparts. Methods: This is a retrospective study, including all patients undergoing LT in Bichat Hospital (Paris, France) from January 2014 to March 2019. Two groups were defined depending on the patients' age when they were transplanted: patients older than 65 were defined as the "elderly group" and patients younger than 65 years old were defined as the « younger group ». Primary endpoint was 90-day mortality. Secondary endpoints included 1-year mortality, 1-year FEV1 (forced expiratory volume in one second), and 5-year overall survival. Results: From September 2014 to March 2019, 22 patients were included in the "elderly group" and 213 were included in the « younger group ». The elderly group had more single LT (SLT) (82% vs. 29%, p < 0.001), with a shorter cold ischemic time (243 min vs. 310 min, p = 0.001) and a lower rate of early humoral rejection (9% vs. 30%, p = 0.045) compared to the younger group. Ninety-day mortality was not significantly different between elderly and younger group (9% vs. 14%, p = 0.95, respectively), nor were 1-year mortality (23% vs. 25%, p = 0.9, respectively) and 5-year overall survival. Six months after LT, FEV1 was significantly better in the elderly group compared to the younger group (77.0% vs. 65.5%, p = 0.037 respectively), but the difference did not reach statistical significance after one year (78.5 vs. 68.3%, p = 0.18 respectively). Conclusion: Elderly patients underwent more frequently single LT, and achieved similar short and long term postoperative outcomes compared to their younger counterparts. LT for patients 65 years or older should be routinely considered when carefully selected.

Abstract:Background In recent years, age at liver transplantation (LT) has markedly increased. In the context of organ shortage, we investigated the impact of recipient age on post‐transplantation mortality. Methods All adult patients who received a first LT between 2007 and 2017 were included in this cross‐sectional study. Recipients' characteristics at the time of listing, donor and surgery data, post‐operative complications and follow‐up of vital status were retrieved from the national transplantation database. The impact of age on 5‐year overall mortality post‐LT was estimated using a flexible multivariable parametric model which was also used to estimate the association between age and 10‐year net survival, accounting for expected age‐ and sex‐related mortality. Results Among the 7610 patients, 21.4% were aged 60–65 years, and 15.7% over 65. With increasing age, comorbidities increased but severity of liver disease decreased. Abstract:Background: High-Urgent (HU) lung transplantation (LTx) is a life-saving treatment for highly selected patients with respiratory failure. Aims and Methods: Retrospective analysis of post-LTx survival in patients who underwent HU LTx between January 1st 2014, and January 31st 2024 in a large-volume lung transplant center. Results: A total of 70 patients underwent HU LTx (39 Male/31 Female, 67 bilateral lung/2 heart-lung/1 combined liver-lungs, 6 redo-LTx), n=10 for obstructive disease (group A), n=8 for vascular disease (group B), n=10 for suppurative disease (group C), and n=42 for restrictive disease (group D). Median (IQR) age at LTx was 52 (37.8-59.3) years, BMI 23.6 (18.4-26.3), LAS score 63 (42.8-78.5), waiting list duration 42.5 (6-196) days, and HU-status 7 (2-12) days. At HU LTx, 12 patients (17%) were mechanically ventilated (MV), 9 patients (13%) were on extracorporeal life support (ECLS) alone, and 18 (26%) on both MV and ECLS, while the remaining n=31 (44%) were on nasal high flow oxygen or non-invasive ventilatory support. 20 patients (29%) were HLA-sensitized, in 9 of whom DSA were present, with a median IgG CDC-PRA of 0 (0-4.5)%. Median post-LTx follow-up is 675.5 (298.8-1690.0) days (range 6-3679), and 27 patients (39%) meanwhile have died, after a median of 579 (155-1326) days post-LTx. Overall post-LTx survival was 84.6% at 1 year, 66.9% at 3 years, and 49.6% at 5 years, but differed with age (≤50 vs. >50 y, p=0.002), need for invasive support (no vs. ECLS±MV, p=0.0362), and tended to differ with HLA-sensitization (p=0.0945). Conclusions: Long-term survival after HU LTx is limited, especially in recipients above 50 years, requiring invasive support, or with HLA-sensitization.

Abstract:The current organ allocation rules prioritize elderly and urgent patients on the lung transplantation (LT) waiting list. A steady increase in the threshold at which age is taken into consideration for LT has been observed. This retrospective cohort study recruited 166 lung transplant recipients aged ≥ 65 years between January 2016 and October 2020 in the largest LT center in China. In the cohort, subgroups of patients aged 65–70 years (111 recipients, group 65–70) and ≥ 70 years (55 recipients, group ≥ 70) were included. Group D restrictive lung disease was the main indication of a lung transplant in recipients over 65 years. A significantly higher percentage of coronary artery stenosis was observed in the group ≥ 70 (30.9% vs. 14.4% in group 65–70, P = 0.014). ECMO bridging to LT was performed in 5.4% (group 65–70) and 7.3% (group ≥ 70) of patients. Kaplan–Meier estimates showed that recipients with cardiac abnormalities had a significantly increased risk of mortality. After adjusting for potential confounders, cardiac abnormality was shown to be independently associated with the increased risk of post-LT mortality (HR 6.37, P = 0.0060). Our result showed that LT can be performed in candidates with an advanced age and can provide life-extending benefits.

Abstract:Lung transplantation aims to improve health–related quality of life (HRQL) and survival. While lung function improvements are associated with these outcomes, the association between physical functioning and these outcomes is less clear. We investigated the association between changes in patient-reported physical functioning and HRQL, chronic lung allograft dysfunction (CLAD), and survival after lung transplantation. This single–center prospective cohort study analyzed 220 lung transplant recipients who completed the 15–item Lung Transplant Valued Life Activities (LT–VLA) before and repeatedly after transplant. HRQL was assessed using generic, respiratory disease–specific, and utility measures. Associations between 0.3–point changes (the minimally important difference) in LT–VLA as a time–varying predictor on HRQL, CLAD, and mortality were tested using linear regression and Cox proportional hazard models. Models were adjusted for demographics, disease diagnosis, and post–operative lung function as a time–varying covariate. Participants were 45% female and 75% White, with a mean age of 56 (±12) years. Each 0.3–point improvement in LT–VLA was associated with substantially improved HRQL across all measures (adjusted p–values <0.01). Each 0.3–point improvement in LT–VLA was associated with a 13% reduced hazard of CLAD (adjusted HR: 0.87, 95% CI: 0.76–0.99, p=0.03) and a 19% reduced hazard of mortality (adjusted HR: 0.81, 95% CI: 0.67–0.95, p=0.01). Improvements in patient–reported physical functioning after lung transplantation are associated with improved HRQL and reduced risk of CLAD and death, independent of allograft function. The simplicity of the LT–VLA suggests it could be a valuable monitoring or outcome measure in both clinical and research settings.

Abstract:Severe pulmonary hypertension (PHT) is a contraindication to liver transplantation (LT); however, the prognostic implication of mild to moderate PHT in living‐donor LT (LDLT) is unknown. The study cohort retrospectively included 1,307 patients with liver cirrhosis who underwent LDLT. PHT was defined as a mean pulmonary artery pressure (PAP) of ≥25 mmHg, measured intraoperatively just before surgery. The primary endpoint was graft failure within one year after LDLT, including retransplantation or death from any cause. The secondary endpoints were in‐hospital adverse events. In the overall cohort, the median Model for End‐stage Liver Disease‐Sodium (MELD‐Na) score was 19, and 100 patients (7.7%) showed PHT. During 1‐year follow‐up, graft failure occurred in 94 patients (7.2%). Patients with PHT had lower 1‐year graft survival (86% vs. 93.4%, p=0.005) and survival rates (87% vs. 93.6%, p=0.011). Mean PAP was associated with a high risk of in‐hospital adverse events and 1‐year graft failure. Adding the mean PAP to the clinical risk model improved the risk prediction. In conclusion, mild to moderate PHT was associated with higher risks of 1‐year graft failure and in‐hospital events, including mortality after LDLT in patients with liver cirrhosis. Intraoperative mean PAP can help predict the early clinical outcomes after LDLT.
