Abstract:Regular health monitoring can result in early detection of disease, accelerate the delivery of medical care and, therefore, considerably improve patient outcomes for countless medical conditions that affect public health. A substantial unmet need remains for technologies that can transform the status quo of reactive health care to preventive, evidence-based, person-centred care. With this goal in mind, platforms that can be easily integrated into people's daily lives and identify a range of biomarkers for health and disease are desirable. However, urine - a biological fluid that is produced in large volumes every day and can be obtained with zero pain, without affecting the daily routine of individuals, and has the most biologically rich content - is discarded into sewers on a regular basis without being processed or monitored. Toilet-based health-monitoring tools in the form of smart toilets could offer preventive home-based continuous health monitoring for early diagnosis of diseases while being connected to data servers (using the Internet of Things) to enable collection of the health status of users. In addition, machine learning methods can assist clinicians to classify, quantify and interpret collected data more rapidly and accurately than they were able to previously. Meanwhile, challenges associated with user acceptance, privacy and test frequency optimization should be considered to facilitate the acceptance of smart toilets in society.

Abstract:Technologies for the longitudinal monitoring of a person's health are poorly integrated with clinical workflows, and have rarely produced actionable biometric data for healthcare providers. Here, we describe easily deployable hardware and software for the long-term analysis of a user's excreta through data collection and models of human health. The 'smart' toilet, which is self-contained and operates autonomously by leveraging pressure and motion sensors, analyses the user's urine using a standard-of-care colorimetric assay that traces red-green-blue values from images of urinalysis strips, calculates the flow rate and volume of urine using computer vision as a uroflowmeter, and classifies stool according to the Bristol stool form scale using deep learning, with performance that is comparable to the performance of trained medical personnel. Each user of the toilet is identified through their fingerprint and the distinctive features of their anoderm, and the data are securely stored and analysed in an encrypted cloud server. The toilet may find uses in the screening, diagnosis and longitudinal monitoring of specific patient populations.

Abstract:The Internet of things (IoT) helps our everyday lives such as by monitoring objects and tracking behaviors in various settings, but studies on enhancing the bathroom experience are rare. This article describes full details about development and implementation of a smart health-monitoring bidet based on our study published previously in the conference. A smart bidet system is designed to monitor the users’ health through several contact-type sensors, such as pressure, oxygen, and thermometer. The system is equipped with a built-in artificial intelligence software platform and is designed to detect anal and spinal diseases. The attached sensors normally operate under waterproof conditions: we tested their performances under X6 international protection marking conditions. These devices were designed to operate properly even in extremely waterproof conditions. The temperature, pressure, and oxygen sensors of the bidet system had error rates of about 4.1, 0.6, and 1.1 percent, respectively.

Abstract:Smart toilet provides a feasible platform for the long-term analysis of person's health. Common solutions for identification are based on camera or radio-frequency identification (RFID) technologies, but it is doubted for privacy issues. Here, we demonstrate an artificial intelligence of toilet (AI-toilet) based on a triboelectric pressure sensor array offering a more private approach with low cost and easily deployable software. The pressure sensor array attached on the toilet seat is composed of 10 textile-based triboelectric sensors, which can leverage the different pressure distribution of individual users' seating manner to get the biometric information. 6 users can be correctly identified with more than 90% accuracy using deep learning. The signals from pressure sensors also can be used for recording the seating time on the toilet. The system integrates a camera sensor to analyze the simulated urine by comparing with urine chart and classify the types and quantities of objects using deep learning. All information including two-factor user identification and entire seating time using pressure sensor array, and data from the urinalysis and stool analysis were automatically transferred to a cloud system and were further shown in user's mobile devices for better tracking their health status.

Abstract:Background: Smart bathroom technology offers unrivaled opportunities for the automated measurement of a range of biomarkers and other data. Unfortunately, efforts in this area are mostly driven by a technology push rather than market pull approach, which decreases the chances of successful adoption. As yet, little is known about the use cases, barriers, and desires that potential users of smart bathrooms perceive. Objective: This study aimed to investigate how participants from the general population experience using a smart sensor-equipped toilet seat installed in their home. The study contributes to answering the following questions: What use cases do citizens see for this innovation? and What are the limitations and barriers to its everyday use that they see, including concerns regarding privacy, the lack of fit with everyday practices, and unmet expectations for user experience? Methods: Overall, 31 participants from 30 households participated in a study consisting of 3 (partially overlapping) stages: sensitizing, in which participants filled out questionnaires to trigger their thoughts about smart bathroom use and personal health; provotyping, in which participants received a gentle provocation in the form of a smart toilet seat, which they used for 2 weeks; and discussion, in which participants took part in a web-based focus group session to discuss their experiences. Results: Participants mostly found the everyday use of the toilet, including installation and dismantling when necessary, to be relatively easy and free of complications. Where complications occurred, participants mentioned issues related to the design of the prototype, technology, or mismatches with normal practices in using toilets and hygiene. A broad range of use cases were mentioned, ranging from signaling potentially detrimental health conditions or exacerbations of existing conditions to documenting physical data to measuring biomarkers to inform a diagnosis and behavioral change. Participants differed greatly in whether they let others use, or even know about, the seat. Ownership and control over their own data were essential for most participants. Conclusions: This study showed that participants felt that a smart toilet seat could be acceptable and effective, as long as it fits everyday practices concerning toilet use and hygiene. The range of potential uses for a smart toilet seat is broad, as long as privacy and control over disclosure and data are warranted.

Smart homes, private homes? An empirical study of technology researchers' perceptions of ethical issues in developing smart-home health technologies

Abstract:Background: Smart-home technologies, comprising environmental sensors, wearables and video are attracting interest in home healthcare delivery. Development of such technology is usually justified on the basis of the technology's potential to increase the autonomy of people living with long-term conditions. Studies of the ethics of smart-homes raise concerns about privacy, consent, social isolation and equity of access. Few studies have investigated the ethical perspectives of smart-home engineers themselves. By exploring the views of engineering researchers in a large smart-home project, we sought to contribute to dialogue between ethics and the engineering community. Methods: Either face-to-face or using Skype, we conducted in-depth qualitative interviews with 20 early- and mid-career smart-home researchers from a multi-centre smart-home project, who were asked to describe their own experience and to reflect more broadly about ethical considerations that relate to smart-home design. With participants' consent, interviews were audio-recorded, transcribed and analysed using a thematic approach. Results: Two overarching themes emerged: in 'Privacy', researchers indicated that they paid close attention to negative consequences of potential unauthorised information sharing in their current work. However, when discussing broader issues in smart-home design beyond the confines of their immediate project, researchers considered physical privacy to a lesser extent, even though physical privacy may manifest in emotive concerns about being watched or monitored. In 'Choice', researchers indicated they often saw provision of choice to end-users as a solution to ethical dilemmas. While researchers indicated that choices of end-users may need to be restricted for technological reasons, ethical standpoints that restrict choice were usually assumed and embedded in design. Conclusions: The tractability of informational privacy may explain the greater attention that is paid to it. However, concerns about physical privacy may reduce acceptability of smart-home technologies to future end-users. While attention to choice suggests links with privacy, this may misidentify the sources of privacy and risk unjustly burdening end-users with problems that they cannot resolve. Separating considerations of choice and privacy may result in more satisfactory treatment of both. Finally, through our engagement with researchers as participants this study demonstrates the relevance of (bio)ethics as a critical partner to smart-home engineering.
Digital remote monitoring for screening and early detection of urinary tract infections

Abstract:Urinary Tract Infections (UTIs) are one of the most prevalent bacterial infections in older adults and a significant contributor to unplanned hospital admissions in People Living with Dementia (PLWD), with early detection being crucial due to the predicament of reporting symptoms and limited help-seeking behaviour. The most common diagnostic tool is urine sample analysis, which can be time-consuming and is only employed where UTI clinical suspicion exists. In this method development and proof-of-concept study, participants living with dementia were monitored via low-cost devices in the home that passively measure activity, sleep, and nocturnal physiology. Using 27828 person-days of remote monitoring data (from 117 participants), we engineered features representing symptoms used for diagnosing a UTI. We then evaluate explainable machine learning techniques in passively calculating UTI risk and perform stratification on scores to support clinical translation and allow control over the balance between alert rate and sensitivity and specificity. The proposed UTI algorithm achieves a sensitivity of 65.3% (95% Confidence Interval (CI) = 64.3–66.2) and specificity of 70.9% (68.6–73.1) when predicting UTIs on unseen participants and after risk stratification, a sensitivity of 74.7% (67.9–81.5) and specificity of 87.9% (85.0–90.9). In addition, feature importance methods reveal that the largest contributions to the predictions were bathroom visit statistics, night-time respiratory rate, and the number of previous UTI events, aligning with the literature. Our machine learning method alerts clinicians of UTI risk in subjects, enabling earlier detection and enhanced screening when considering treatment.

Abstract:Background: States and families are facing growing challenges provide adequate care for older persons. Smart home health technologies (SHHTs) in the forms of sensor or robotic devices have been discussed as technical solutions for caregiving. Ethical and social concerns are raised with the use of such technologies for caregiving purposes, a particularly prominent one being privacy. This paper contributes to the literature by distinguishing privacy concerns into both the type of technologies and conceptual dimensions. Methods: Data for this paper stem from sixty semi-structured interviews with older persons, informal, and formal caregivers living in the German-speaking regions of Switzerland. All information related to privacy, that were initially inductively coded, were thematically sorted into four dimensions of privacy (physical, psychological, social, and informational) and by the type of technologies studied. Results: Participants were especially concerned about privacy intrusions from smart wearables and ambient sensors than robotic technologies, which may be due to the relative lack of familiarity with the latter. Informational privacy was evident in the context of data collection capacities and potential for misuses of data. The installation and implementation of both visual and ambient sensors induced discomfort to their senses of physical space. Alerts of smart wearables and obtrusive sightings of SHHTs garnered worries related to stigmatization and manipulation, indicating intrusions into end-users' psychological privacy. Little discussions of social dimensions of privacy were evident in the data, even toward robotic technologies for their functions to promote social interactions for older persons. Conclusions: This paper is one of the first that use the stratification approach on empirical data to highlight the multi-faceted privacy concerns when technologies may be implemented in elder care. Our paper could thus supports potential end-users in deciding which technologies to use and how to balance different privacy concerns against other values that they may hold important.

Abstract:Social health awareness in the context of medical prophylaxis influences the current trends in the design of modern domestic bathrooms. The importance of the health role of the modern bathroom also results from the current social and economic situation around the world, resulting in the need for independent health control and the developing need for medical prophylaxis, taking care of immunity and psychophysical condition in the face of public health threats. Therefore, it was necessary to investigate the health factors in bathroom design based on research and analysis of literature and materials from foreign research institutes. The study concerned design possibilities in the selection of bathroom equipment elements enabling a wide range of comfort and supporting and monitoring the health of bathroom users. The research included a review and analysis of technologically advanced solutions, intelligent solutions, including Internet of Things (IoT) technologies, ensuring health prophylaxis in the home bathroom. The potential for the use of innovative technologies and the possibility of process control, easy flow of information in relation to the external environment (e.g. remote contact with a doctor) was indicated. The need for easy and intuitive use of intelligent devices was taken into account, as well as the possibility of independent, systematic control and protection of health without the need for stationary medical visits, with a view to time and safety of healthcare professionals and patients. The research results summarize the fields of medicine where smart bathroom appliances can create new opportunities to improve user health.
Long‐term, automated stool monitoring using a novel smart toilet: A feasibility study

Abstract:Burden and inaccuracy of patients self‐reporting of stool consistency is a challenge. We developed an approach for automatic collection of stool images and computer vision classification providing long‐term monitoring and objective detection of abnormal stool consistency. Background Patients' report of bowel movement consistency is unreliable. We demonstrate the feasibility of long‐term automated stool image data collection using a novel Smart Toilet and evaluate a deterministic computer‐vision analytic approach to assess stool form according to the Bristol Stool Form Scale (BSFS). Methods Our smart toilet integrates a conventional toilet bowl with an engineered portal to image feces in a predetermined region of the plumbing post‐flush. The smart toilet was installed in a workplace bathroom and used by six healthy volunteers. Images were annotated by three experts. A computer vision method based on deep learning segmentation and mathematically defined hand‐crafted features was developed to quantify morphological attributes of stool from images. Key Results 474 bowel movements images were recorded in total from six subjects over a mean period of 10 months. 3% of images were rated abnormal with stool consistency BSFS 2 and 4% were BSFS 6. Our image analysis algorithm leverages interpretable morphological features and achieves classification of abnormal stool form with 94% accuracy, 81% sensitivity and 95% specificity. Conclusions Our study supports the feasibility and accuracy of long‐term, non‐invasive automated stool form monitoring with the novel smart toilet system which can eliminate the patient burden of tracking bowel forms.

Abstract:Most research using digital technologies builds on existing methods for staff-administered evaluation, requiring a large investment of time, effort, and resources. Widespread use of personal mobile devices provides opportunities for continuous health monitoring without active participant engagement. Home-based sensors show promise in evaluating behavioral features in near real time. Digital technologies across these methodologies can detect precise measures of cognition, mood, sleep, gait, speech, motor activity, behavior patterns, and additional features relevant to health. As a neurodegenerative condition with insidious onset, Alzheimer disease and other dementias (AD/D) represent a key target for advances in monitoring disease symptoms. Studies to date evaluating the predictive power of digital measures use inconsistent approaches to characterize these measures. Comparison between different digital collection methods supports the use of passive collection methods in settings in which active participant engagement approaches are not feasible. Additional studies that analyze how digital measures across multiple data streams can together improve prediction of cognitive impairment and early-stage AD are needed. Given the long timeline of progression from normal to diagnosis, digital monitoring will more easily make extended longitudinal follow-up possible. Through the American Heart Association-funded Strategically Focused Research Network, the Boston University investigative team deployed a platform involving a wide range of technologies to address these gaps in research practice. Much more research is needed to thoroughly evaluate limitations of passive monitoring. Multidisciplinary collaborations are needed to establish legal and ethical frameworks for ensuring passive monitoring can be conducted at scale while protecting privacy and security, especially in vulnerable populations.

Abstract:In the domain of biomedical signals measurements, monitoring human physiological parameters is an important issue. With the rapid development of wireless body area network, it makes monitor, transmit and record physiological parameters faster and more convenient. Infants and the elderly completely bedridden are two special groups of the society who need more medical care. According to researches investigating current frontier domains and the market products, the detection of physiological parameters from the excrement is rare. However, urine and faeces contain a large number of physiological information, which are high relative to health. The mainly distributed odour from urine is NH4 and the distributed odour from feces is mainly H2S, which are both could be detected by the sensors. In this paper, we introduce the design and implementation of a portable wireless device based on body area network for real time monitoring to the odour of excrement for health of infants and the elderly completely bedridden. The device not only could monitor in real time the emitted odour of faeces and urine for health analysis, but also measures the body temperature and environment humidity, and send data to the mobile phone of paramedics to alarm or the server for storage and process, which has prospect to monitoring infants and the paralysis elderly.

Abstract:Behavioral intervention studies often lack sufficiently sensitive and frequent measurements to observe an effect. Remote passive sensing offers a highly sensitive, continuous, and ecologically valid method of assessment that increases the ability to detect changes in the daily activities and function of those being monitored. To be most effectively deployed in research studies, applications of remote assessment technology must be designed with the end user in mind. User-centered design (UCD) is especially important in clinical trials where the needs and characteristics of participants and research staff need to be uniquely considered to ensure the feasibility and acceptability of the study. This paper describes UCD issues in remote passive sensing that commonly arise among older adult participants—including those living with dementia—as well as any strategies that were taken to overcome them. Using exemplars from the National Institute on Aging–funded Roybal Center ORCASTRAIT (Oregon Roybal Center for Care Support Translational Research Advantaged by Integrating Technology), as well as other experimental and observational research studies conducted in community settings, this paper brings together our collective experiences with studies using remote passive sensing technology that incorporate a UCD design approach. Although passive sensing eliminates some common UCD issues that arise with higher-touch technology, issues, such as usability, trust, and aesthetic acceptability, still need to be addressed for behavioral interventions using passive sensing technology to be potent and implementable.

An Internet of Things-Based Home Telehealth System for Smart Healthcare by Monitoring Sleep and Water Usage: A Preliminary Study

Abstract:Recently, the Internet of Things (IoT) has attracted wide attention from many fields, especially healthcare, because of its large capacities for information perception and collection. In this paper, we present an IoT-based home telehealth system for providing smart healthcare management for individuals, especially older people. Each client node of the system is mainly composed of an electronic water meter that records the user's daily water usage, in order to analyze their living patterns and lifestyle as well as ascertain their well-being, and an unobtrusive sleep sensor that monitors the user's physiological parameters during sleep, such as heart rate (HR), respiratory rate (RR), body movement (BM), and their states on the bed or outside the bed. The collected data can be transmitted to a remote centralized cloud service by a wireless home gateway for analyzing the living pattern and rhythm of users. Furthermore, the periodic feedback of results can be provided to users themselves, as well as their family and health advisers. In the present study, data was collected from a total of 18 older subjects for one year to evaluate the effectiveness of the proposed system. By analyzing living patterns and rhythm, preliminary results indicate the effectiveness of the telehealth system and suggest the potential of the system regarding improvement in the quality of life (QoL) of older people and promotion of their health.

Older Adults' Perceptions About Using Intelligent Toilet Seats Beyond Traditional Care: Web-Based Interview Survey

Abstract:Background: In contemporary society, age tech (age technology) represents a significant advancement in health care aimed at enhancing patient engagement, ensuring sustained independence, and promoting quality of life for older people. One innovative form of age tech is the intelligent toilet seat, which is designed to collect, analyze, and provide insights based on toileting logs and excreta data. Understanding how older people perceive and interact with such technology can offer invaluable insights to researchers, technology developers, and vendors. Objective: This study examined older adults' perspectives regarding the use of intelligent toilet seats. Through a qualitative methodology, this research aims to unearth the nuances of older people's opinions, shedding light on their preferences, concerns, and potential barriers to adoption. Methods: Data were collected using a web-based interview survey distributed on Amazon Mechanical Turk. The analyzed data set comprised 174 US-based individuals aged ≥65 years who voluntarily participated in this study. The qualitative data were carefully analyzed using NVivo (Lumivero) based on detailed content analysis, ensuring that emerging themes were coded and classified based on the conceptual similarities in the respondents' narratives. Results: The analysis revealed 5 dominant themes encompassing the opinions of aging adults. The perceived benefits and advantages of using the intelligent toilet seat were grouped into 3 primary themes: health-related benefits including the potential for early disease detection, continuous health monitoring, and seamless connection to health care insights. Technology-related advantages include the noninvasive nature of smart toilet seats and leveraging unique and innovative data collection and analysis technology. Use-related benefits include ease of use, potential for multiple users, and cost reduction owing to the reduced need for frequent clinical visits. Conversely, the concerns and perceived risks were classified into 2 significant themes: psychological concerns, which included concerns about embarrassment and aging-related stereotypes, and the potential emotional impact of constant health monitoring. Technical performance risks include concerns centered on privacy and security, device reliability, data accuracy, potential malfunctions, and the implications of false positives or negatives. Conclusions: The decision of older adults to incorporate intelligent toilet seats into their daily lives depends on myriad factors. Although the potential health and technological benefits are evident, valid concerns that need to be addressed remain. To foster widespread adoption, it is imperative to enhance the advantages while simultaneously addressing and mitigating the identified risks. This balanced approach will pave the way for a more holistic integration of smart health care devices into the routines of the older population, ensuring that they reap the full benefits of age tech advancements.

Abstract:The aging population has led to a widening gap between the supply and demand for defecation care. To address this issue, the development of defecation care devices is the most direct and effective solution. Pre-defecation care devices offer a more personalized and comfortable alternative to the conventional post-defecation care devices currently available on the market. Furthermore, they facilitate greater patient involvement in the care process. Real-time monitoring and accurate identification of defecation intention are key technologies in the development of pre-defecation nursing devices. Automatic and accurate online monitoring of defecation intention can provide accurate early warning information for differentiated defecation assistance and cleansing care, effectively reducing nursing workload and improving patients' quality of life. However, there are relatively few studies on real-time monitoring and accurate identification of defecation intention. This review summarizes the existing defecation intention sensing technologies and their monitoring principles and research status, and explores the potential development direction of defecation intention sensing systems by comparing the characteristics and application conditions of various sensing technologies, which provides a direction for perception strategies for future defecation intention monitoring and early warning research.
Health Monitoring Using Smart Home Technologies: Scoping Review

Abstract:Background: The Internet of Things (IoT) has become integrated into everyday life, with devices becoming permanent fixtures in many homes. As countries face increasing pressure on their health care systems, smart home technologies have the potential to support population health through continuous behavioral monitoring. Objective: This scoping review aims to provide insight into this evolving field of research by surveying the current technologies and applications for in-home health monitoring. Methods: Peer-reviewed papers from 2008 to 2021 related to smart home technologies for health care were extracted from 4 databases (PubMed, Scopus, ScienceDirect, and CINAHL); 49 papers met the inclusion criteria and were analyzed. Results: Most of the studies were from Europe and North America. The largest proportion of the studies were proof of concept or pilot studies. Approximately 78% (38/49) of the studies used real human participants, most of whom were older females. Demographic data were often missing. Nearly 60% (29/49) of the studies reported on the health status of the participants. Results were primarily reported in engineering and technology journals. Almost 62% (30/49) of the studies used passive infrared sensors to report on motion detection where data were primarily binary. There were numerous data analysis, management, and machine learning techniques employed. The primary challenges reported by authors were differentiating between multiple participants in a single space, technology interoperability, and data security and privacy. Conclusions: This scoping review synthesizes the current state of research on smart home technologies for health care. We were able to identify multiple trends and knowledge gaps-in particular, the lack of collaboration across disciplines. Technological development dominates over the human-centric part of the equation. During the preparation of this scoping review, we noted that the health care research papers lacked a concrete definition of a smart home, and based on the available evidence and the identified gaps, we propose a new definition for a smart home for health care. Smart home technology is growing rapidly, and interdisciplinary approaches will be needed to ensure integration into the health sector.
