Abstract:Background There is a dearth of research or inconsistency in findings of behaviours among children with autism spectrum disorder (ASD) and children with intellectual disability (ID) as well as children who are typically developing (TD). This study compared 7 behavioural domains of 96 behaviours among Chinese children. Method There were 803 children, including 426 children with low-functioning ASD (LFASD), 193 children with high-functioning ASD (HFASD), 128 children with ID, and 56 TD children. Ages ranged from 36 to 84 months. Each individual was administered the Chinese Autism Diagnostic Scale by a group of Chinese professionals. Results All 7 domains were found to be more impaired for the children with ASD than the other groups. Three domains contained the most distinctive behaviours: Play behaviours, Social-Interactive behaviours, and Receptive Language behaviours. The Expressive Language domain was different for the LFASD group versus the group with ID. Conclusion Children with ASD differed from children without ASD by several specific behaviours. These behaviours also differentiated performances by the children with HFASD and LFASD.
Influence of parental education on the intelligence quotient profiles and socially adaptive behavior of school-age children with autism spectrum disorder in eastern China

Abstract:Intelligence quotient (IQ) and adaptive behavior are the influencing factors of autism spectrum disorder (ASD) in children entering mainstream schools. This study explored the association between parental education level, IQ, and adaptive behavior in ASD groups. A total of 257 school-age ASD children were enrolled in our study from January 2017 to June 2021. Their parents completed a standard demographic form, including age at autism diagnosis, gender, school placement, and parents' educational background. The Chinese version of the Wechsler Intelligence Scale for Children, Fourth Edition (WISC-IV) was completed by a certified assessor for each enrolled child. Parents were interviewed on adaptive behavior using the Chinese version of the Adaptive Behavior Assessment System, Second Edition (ABAS-II). The average IQ of school-age ASD children was 76.88 (standard deviation (SD)=22.62) and boys had higher IQ levels than girls. The IQ was positively correlated with age. The General Adaptive Composite (GAC) score was 82.47 (SD=15.86) and adaptive behavior did not increase with age. ASD children who attended mainstream schools had better adaptive behavior profiles than other children. The mother's education level showed a significant correlation with the IQ and adaptive behavior of autistic children, while the father's education level did not. Consequently, better training and support for parents may help autistic children enter mainstream schools, with adaptive training being the most urgently required skill for parents.

Abstract:Introduction: Autism Spectrum Disorder (ASD) is a complex neurodevelopmental disorder characterized by a range of intellectual and language abilities. Its heterogeneity is acknowledged in modern diagnostics, complicating research and necessitating precision medicine and a multidimensional approach for individualized treatment and accurate assessment. Intellectual and language functioning influence adaptive skills and symptomatology. Thus, assessing adaptive functioning in a multidimensional and multi-informant manner is crucial, highlighting the importance of comprehensive evaluations. This study explores the interplay between autistic traits, demographic variables, IQ, adaptive functioning, and the applicability of ICD-11 classifications. Methods: We analyzed data from the initial global evaluation of 60 diverse autistic children (aged 35 to 120 months; IQ range 16 to 118). Parent-reports using the Vineland Adaptive Behavior Scale (VABS-II) were compared with standardized assessments from the PsychoEducational Profile (PEP-3). Children's intellectual levels were assessed using Griffiths Scales of Child Development (Griffiths III) and the Autism Diagnostic Observation Schedule (ADOS-2) was used for autistic traits. They were further classified according to the ICD-11 diagnostic system, functional language, and intellectual functioning levels. Correlations among variables, group comparisons, and multivariate analyses were performed. Results: The analysis indicates a linear effect of IQ on all adaptive scales and the impact of autistic traits on directly measured adaptive functioning. A factorial effect was observed due to changes concerning specific age, intellectual, and linguistic levels, which do not completely align with ICD-11 categorization. Additionally, a negative correlation between intelligence and measured autistic traits was found. Parental age, education level, and age at childbirth were also found to affect various adaptive scales. Discussion: The study questions the ICD-11's proposed distinctions in IQ and language functioning for ASD, advocating for more refined categorization and developmental considerations. It underscores the intricate relationship between autistic traits, IQ, and communication skills, casting doubt on the precision of diagnostic tools across the spectrum. Parental reports and direct assessments are essential for comprehensive evaluation, with parental education and age influencing children's behaviors and skills. The study calls for a nuanced approach to ASD assessment, integrating various metrics and sources of information for a detailed phenotyping necessary for personalized interventions and biological research.

Abstract:Dopamine-β-hydroxylase (DβH) is a copper-containing enzyme that uses molecular oxygen and ascorbate to catalyze the addition of a hydroxyl group on the β-carbon of dopamine to form norepinephrine. While norepinephrine causes vasoconstriction following reflex sympathetic stimulation, nitric oxide (NO) formation results in vasodilatation via a guanylyl cyclase-dependent mechanism. In this report, we investigated the relationship between NO and DβH enzymatic activity. In the initial in vitroexperiments, the activity of purified DβH was inhibited by the NO donor, diethylamine/NO (DEA/NO), with an IC50 of 1 mm. The inclusion of either azide or GSH partially restored DβH activity, suggesting the involvement of the reactive nitrogen oxide species, N2O3. Treatment of human neuroblastoma cells (SK-N-MC) with diethylamine/NO decreased cellular DβH activity without affecting their growth rate and was augmented by the depletion of intracellular GSH. Co-culture of the SK-N-MC cells with interferon-γ and lipopolysaccharide-activated macrophages, which release NO, also reduced the DβH activity in the neuroblastoma cells. Our results are consistent with the hypothesis that nitrosative stress, mediated by N2O3, can result in the inhibition of norepinephrine biosynthesis and may contribute to the regulation of neurotransmission and vasodilatation.
Clinical and Cognitive Functions in High Intelligence Quotient Adults with Attention Deficit Hyperactivity Disorder

Abstract:Objective To explore the clinical characters and cognitive functions in high intelligence quotient (IQ) patients with attention deficit hyperactivity disorder (ADHD) in a Chinese population sample. Methods Sixty-eight outpatients with a high IQ fulfilling the DSM-Ⅳ diagnostic criteria for ADHD,91 adults without ADHD matched by high IQ ADHD adults,42 ADHD outpatients with an average IQ and 51 normal adults with an average IQ matched were identified. All the groups were matched by age. All subjects completed the clinical interview, neuropsychological test and the Behavior Rating Inventory of Executive Function-Adult Version (BRIEF-A). Results No significant differences in ADHD subtypes between ADHD group with high IQ and with normal IQ were found.ADHD group with high IQ had more comorbidities of mood disorder and anxiety spectrum disorders than ADHD group with normal IQ(36.8%(25/68)vs.19.0%(8/42),χ2=3.880,P=0.049).The Barratt Impulsiveness Scale-Ⅱscores of non planned were significantly lower in high IQ ADHD group than in general intelligence ADHD group(29.5± 4.2 vs. 31.4 ± 4.3,t=2.102,P=0.037). ADHD group with normal IQ performed worse than control group in test of arithmetic(t=-2.335,P=0.020), digit span(t=-2.021,P=0.045)and digit symbol(t=-3.043,P<0.01).When compared with ADHD group with normal IQ,the one with high IQ had lower scores in emotion control(t=-1.976,P=0.049),working memory(t=-2.363,P=0.019)and behavior management index(t=-2.322, P=0.021)when completing BRIEF-A questionnaire. Conclusions The ADHD group with high IQ has a higher rate of comorbidities with mood disorder and anxiety spectrum disorders.ADHD adults with normal IQ have more serious cognitive damage,especially in working memory.Both ADHD groups have executive function damages.High IQ could protect the cognitive capacity of adults with ADHD,especially in working memory aspect.
Interrelations of Intelligence and Social-Adaptive Skills in Adolescents with Multiple Developmental Disorders: A Pilot Study

Abstract:Introduction The relationship between social adaptation and intelligence in adolescents with developmental disorders varies depending on various psycho-social factors. Adolescence is marked by pubertal changes in mental and physical development. Previous research has revealed a moderate correlation between intelligence and various groups of adaptive skills in adolescents with Down syndrome. However, studies involving adolescents with multiple developmental disorders are relatively scarce in the existing literature Objectives Determine the distribution of intelligence among adolescents with severe multiple disabilities; Identify the connection between intelligence and the level of adaptation in this group; explore the connection between intelligence and independence skills in the subjects. Methods The study included 11 adolescent participants enrolled in a comprehensive social skills development intervention program at the Center for Curative Pedagogics: 5 girls and 6 boys, mean age - 14,0 yrs. Age st.dev: 24,3 and 18.4. ICD-10 DS of participants were: F48.xx, F70.xx, F80.xx, F84.xx, G40.xx, G80.xx, Q74.xx, and Q90.xx. Following tools were used: Leiter-3 scales (LIQ), Vineland-3 Adaptive Behavior Scales (VSS); Perkins I.C.A.N. independence Scales (ICAN). Results Selected variables including were tested with the Shapiro-Wilk test. p-values of the SW test indicated that data were not distributed normally: LIQ (w=0.953, p=0.685); VSS (w=0.964, 0.821); ICAN (w=0.877;p=0.095). For the identification of the connections between the intelligence (LIQ) and adaptive functioning (VSS) we used r-Spearman criteria. These parameters showed significant monotonic relationship (rs = 0.961, p<0.001). Mean IQ level of the sample is characterized as mildly impaired (mean = 62.9). The adaptive and the independence skills level of the sample are also far below the low normative results (57.1 and 48.7 respectively). Images 1,2 and 3 shows the distribution of the data. The correlation between IQ (LIQ) and independence skills (ICAN) is not significant (rs = 0.671, p<0.024), as well as the correlation between adaptive and independence skills (rs = 0.733, p<0.010). Conclusions We made an exploratory study of the adolescent participants of the comprehensive social skills development intervention program at the Center for Curative Pedagogics. Results show that non-verbal intelligence of the participants shows strong connection to the adaptive skills, but not to the independence skills. Sample size is very small, which is explained by the specifics of the intervention. Further research should be focused on the increasing sample and the expanding analysis parameters, such as social and family history, intervention details and the additional variables of the existing measurements. Profile of intellectual abilities and personality traits in adults with high-functioning autism

Abstract:Background. Autism is a pervasive neurodevelopmental disorder caused by disruption of the prenatal brain development. In mild forms of autism, children may camouflage their symptoms, which may cause their mental disorder to stay undiagnosed into adulthood. Studies of personality and intelligence characteristics in adult patients with autism will reduce the difficulties of diagnosing mild forms of this disorder in a population of high-functioning socially adapted individuals. Objective. The study aims to describe the profile of intellectual abilities in adults with high-functioning autism and its relationship with the severity of clinical symptoms and personal traits. Sample. 30 participants with high-functioning autism (9 males and 21 females) aged between 20 and 44 years and 30 neurotypical controls took part in the research. Methods. The subjects underwent a short version of the Wechsler test (WAIS-III) and completed the Carver and White questionnaire “Behavioral Activation and Inhibition Systems”. Patients with autism also completed the Autism Quotient questionnaire designed by Baron and Cohen to assess severity of their symptoms, and their diagnosis was verified by an experienced psychiatrist. We analyzed intergroup differences using analysis of variance, as well as nonparametric correlational analysis between IQ, clinical symptoms, and personal traits. Results. Patients with autism did not differ from the control group in general intelligence, but they showed significantly lower verbal abilities (F(1, 42) = 8.4; p = 0.006). Reduction of verbal abilities in patients with autism directly correlated with severity of their clinical symptoms (R = –0.59; p = 0.001). We also found a decrease in the sensitivity of the behavior approach system in the clinical group (F(1.52) = 4.9; p = 0.03), manifested mostly in a low level of spontaneous fun seeking. In contrast to the neurotypical controls, where a higher sensitivity of the behavior activation system positively correlated with verbal abilities (R = 0.52; p = 0.02), in patients with autism, motivational traits were not associated with any intellectual abilities. Conclusion. The results demonstrate that adult persons with autism spectrum disorders have a relative reduction in verbal fluency. It can be assumed that this profile of intellectual abilities may be a long-term result of a delay in speech development at an early age.
Autism Spectrum Disorder-Like Symptoms in the Population with Intellectual Disability Aged 6 to 18 Years

Abstract:OBJECTIVE:To investigate the incidence of autism spectrum disorder (ASD)-like symptoms in the population with intellectual disability (ID). METHODS:The students with ASD or ID, aged 6-18 years, who studied in a special school in Shanghai from January to June, 2017, as well as the typically developing (TD) population of the same age, who studied in a general school in Shanghai during the same period, were enrolled. Social Responsiveness Scale (SRS) was completed by their parents or other guardians, and the ASD-like symptoms were evaluated. RESULTS:A total of 69 subjects with ASD, 74 subjects with ID and 177 TD subjects were enrolled. The ID group had a significantly higher SRS-positive rate than the TD group (47.3% vs 1.7%; P<0.001) and a significantly lower SRS-positive rate than the ASD group (47.3% vs 87.0%; P<0.001). The total score of SRS was 114±26 in the ASD group, 80±24 in the ID group and 38±19 in the TD group. The ID group had a significantly higher total score of SRS than the TD group (P<0.05), and the score on the subscale of social cognition showed the most significant difference between the two groups (Cohen's d=2.00). There were no significant differences in the total score of SRS and the scores on each subscale of SRS between the mild-to-moderate ID and severe-to-extremely severe ID groups (P>0.05), and there was no significant correlation between SRS score and IQ (P>0.05). CONCLUSIONS:The ID population aged 6-18 years has more ASD-like symptoms than the general population, and ASD screening and intervention should be performed for the ID population as early as possible.
Using adaptive behavior scores to convey level of functioning in children with autism spectrum disorder: Evidence from the Study to Explore Early Development

Abstract:Autistic people are often described as "low-" or "high-functioning" based on their scores on cognitive tests. These terms are common in publications and in everyday communication. However, recent research and feedback from the autistic community suggests that relying on cognitive ability alone to describe functioning may miss meaningful differences in the abilities of autistic children and adults and in the kinds of support they may need. Additional methods are needed to describe "functioning" in autistic children. We examined whether scores from a test measuring adaptive behaviors would provide information on the functional abilities of children with autism that is different from cognitive ability and autism symptom severity. Adaptive behaviors include age-appropriate skills that allow people to function in their everyday lives and social interactions. We found that a large amount of the variation in adaptive behavior scores was not explained by cognitive development, autism symptom severity, and behavioral and emotional problems. In addition, there was a wide range of adaptive ability levels in children with autism in our study, including in those with low, average, or high cognitive scores. Our results suggest that adaptive behavior scores could provide useful information about the strengths and support needs of autistic children above and beyond measures of cognitive ability and autism symptom severity. Adaptive behavior scores provide important information on the needs of autistic people.
Personality Characteristics and Neurocognitive Functions in Parents of Children with Autism Spectrum Disorder

Abstract:Background The increasing prevalence of autism spectrum disorder in children and the huge family burdens have caused concern in the academic field as well as society. Aim To study the personality characteristics and neurocognitive functions in the Chinese parents of children with autism spectrum disorder (ASD) and compare them with the parents of well-developed children. Method This study recruited 41 Chinese children who met the diagnostic criteria of autism spectrum disorder according to Diagnostic and Statistical Manual of mental disorders-IV (DSM-IV). Their 79 biological parents were evaluated by means of Eysenck Personality Questionnaire (EPQ) and a set of neuropsychological tests. The normal control group consisted of 80 parents of well-developed children, and they were matched by age and gender. Results We found that the EPQ scores in the P scale for parents of children with ASD were significantly higher than the parents of well-developed children (t=1.68, p=0.039), while their scores in E scale and L scale were significantly lower (t=1.84, p=0.035; t=2.07, p=0.023). We also identified that the parents of children with ASD took significantly longer time than the normal control to complete Trail Making Test (TMT) Part A and Part B-M (t=1.57, p=0.013; t=0.83, p=0.019). Conclusion Compared to the parents of well-developed children, the parents of children with ASD were more likely to be unconcerned, rigid, stubborn, introverted, and reticent. They displayed less novelty and thrill-seeking behaviors, and had limited social skills and maturity. Although the general cognitive functions including IQ were relatively intact in the parents of children with ASD, there were impairments in their planning, flexibility and visual processing functions.

Abstract:OBJECTIVE:To explore the practical executive function profiles in high IQ (intelligence quotient) children and adolescents with attention deficit hyperactivity disorder (ADHD) in a Chinese sample population.METHODS:For this cross-sectional study, we identified 124 outpatients aged 6.8 - 13.1 years with a high IQ fulfilling the diagnostic criteria for ADHD according to the Diagnostic and Statistical Manual of Mental Disorders-IV (DSM-IV), 68 children and adolescents without ADHD aged 6.5 - 13.1 years with a high IQ matched by high IQ children and adolescents with ADHD, 124 outpatients aged 6.8 - 13.2 years with an average IQ with ADHD and 68 normal children and adolescents aged 6.4 - 13.1 years with an average IQ matched by IQ. We operationalized high IQ as having a full scale intelligence quotient (IQ or FSIQ) ≥ 120 on Chinese version Wechsler intelligence scale for children (C-WISC) and an average IQ as 90 ≤ IQ < 110. All the above groups were matched by age. All subjects completed practical executive function tests, including Stroop color-word, trail-making, digit span, Tower of Hanoi task and verbal fluency to assess their ability in the aspects of inhibition, shifting, working memory, planning and verbal fluency.RESULTS:ADHD group with a high IQ performed worse on the Stroop color-word (3.18 ± 0.05) and trail-making tests (4.38 ± 0.55) than normal control group with a high IQ (2.92 ± 0.07 and 4.05 ± 0.07) (P < 0.01). The test performances of trail-making, digit span (4.86 ± 0.13) and fluency (23.0 ± 0.5) were significantly better in high IQ ADHD group than average IQ ADHD group (4.10 ± 0.07 and 19.9 ± 0.5) (P < 0.01).CONCLUSION:Though a bit better than average IQ ADHD group in shifting, working memory and verbal fluency, the high IQ children and adolescents with ADHD perform worse than high-IQ controls on inhibition and shifting. IQ may protect practical executive function.

Abstract:Objective: To explore the ecological executive function profile in high intelligence quotient(IQ) boys with attention-deficit/hyperactivity disorder(ADHD).Methods: Fifty male outpatients aged 7~14 were selected as the case group who were diagnosed as ADHD according to the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition(DSM-IV) and with the full scale IQ(FIQ) ≥120 in Chinese-Wechsler Intelligence Scale for Children(C-WISC).Fifty age-matched high IQ(FIQ≥120) normal boys were selected as the control groupⅠ.And 723 age-matched boys from the general population were selected as the control group Ⅱ.All the subjects' parents completed the Behavior Rating Inventory of Executive Function(BRIEF).Results: Compared with the control groupⅠand Ⅱ,the high IQ ADHD group got significantly higher scores on all subscales of the BRIEF respectively(P' s<0.05),including inhibition,shifting,emotion control,initiation,working memory,planning,organization,monitoring,behavioral regulation index(BRI),metacognition index(MI) and global executive composite(GEC) [e.g.,GEC,[(150±22) vs.(108±20),(104±21),P'(P value after the multiple-testing correction)<0.001,respectively].Conclusion: This study indicates that the high IQ boys with attention-deficit/hyperactivity disorder show significant impairments in all the aspects of ecological executive function.

Abstract:There are noteworthy sex disparities in the prevalence of autism spectrum disorders (ASD), while findings regarding the sex differences in core symptoms are inconsistent. There are few relevant studies on sex differences in mainland China. This study was dedicated to a deeper understanding of the impact of sex differences on the clinical presentation of ASD with fluent language. We retrospectively studied 301 children with ASD (58 females) and utilized raw scores from the ADI-R and ADOS and the intelligence quotient (IQ) to measure symptomatology. Based on the Full-Scale IQ (FS-IQ), a binary split of average, above-average IQ (high-IQ), and below-average IQ (low IQ) occurs at 85. Across the entire sample, males and females are comparable in the FS-IQ, while males scored higher in the Perceptual Reasoning Index (PRI) ( F = 7.812, p = 0.006). ADI-R did not find any statistically significant sex differences in the diagnostic cutoff score satisfaction or the raw domain scores. While a significant effect of sex on ADOS social affect domain scores was found in the total sample [ λ = 0.970, partial η 2 = 0.030, F (3,295) = 3.019, p = 0.030]. Tests of between-subjects effects revealed that males scored higher than females mainly in the ADOS reciprocal social interaction subcategory (partial η 2 = 0.022, F = 6.563, p = 0.011). Stratified analysis revealed that the effect of sex on ADOS reciprocal social interaction subcategory scores only significant in the low-IQ children with ASD (partial η 2 = 0.092, F = 10.088, p = 0.002). In general, overall cognitive functioning is similar across males and females with ASD, while males have a higher perceptual reasoning ability. Females with ASD are more likely to have comorbid intellectual impairment than males, and they could require additional intervention support. Autistic children with low IQs are more likely to exhibit sex differences in their core symptoms than children with high IQs. Intelligence plays a key role in sex-based differences in the core symptoms of ASD.

The importance of class I and class II HLA in cadaveric renal transplantation.

Abstract:The importance of avoiding mismatches (MM) at Class I and Class II HLA antigens in cyclosporine-treated renal allograft patients is controversial. In order to assess the role of HLA, 200 consecutive cadaveric renal allografts over a 4-year period were analysed. All patients received cyclosporine/predinisone immunosuppression and 75% were induced with ALG. Minimum follow-up period was one year. HLA A, B, DR, DQ, and DRw52/53 typing were available on 77-100% of allografts. A beneficial effect was noted at the HLA A locus. One-year survival was 87.2% in the 0 and 1 HLA A MM group combined vs 73.8% in the 2 HLA A MM group (p less than 0.05). The mean creatinine level at one year was also lower in the 0 plus 1 MM vs 2 MM group: 152.8 mumol/L vs 184.8 mumol/L, respectively (p less than 0.05). Significantly fewer rejection episodes occurred in the 0 and 1 HLA DQ MM group combined vs the 2 MM group. Steroid-resistant rejection episodes (SRRE) were not associated with the number of HLA MM. Patients who had an SRRE had significantly higher mean current and historical peak panel reactive antibodies (PRA) than patients who did not have SRRE. These results indicate that avoiding mismatches at the HLA A locus may improve renal allograft survival, and matching at HLA DQ may predispose patients to a more quiescent post-transplant course. The degree of preoperative sensitization may be an important etiologic factor in SRRE.

Abstract:The Autism Spectrum Rating Scale (ASRS) and the Social Responsiveness Scale (SRS) have been widely used for screening autism spectrum disorder (ASD) in the general population during epidemiological studies, but studies of individuals with intellectual disability (ID) are quite limited. Therefore, we recruited the parents/caregivers of 204 ASD cases, 71 ID cases aged 6–18 years from special education schools, and 402 typically developing (TD) children in the same age span from a community-based population to complete the ASRS and SRS. The results showed that the ID group scored significantly lower on total and subscale scores than the ASD group on both scales (P < 0.05) but higher than TD children (P < 0.05). Receiver operating characteristic analyses demonstrated a similar fair performance in discriminating ASD from ID with the ASRS (area under the curve (AUC) = 0.709, sensitivity = 77.0%, specificity = 52.1%, positive predictive value (PPV) = 82.2%) and the SRS (AUC = 0.742, sensitivity = 59.8%, specificity = 77.5%, PPV = 88.4%). The results showed that individuals with ID had clear autistic traits and discriminating ASD from ID cases was quite challenging, while assessment tools such as ASRS and SRS, help to some degree.

Abstract:Journal of Attention Disorders, Ahead of Print. Objective:Verbal fluency, the capacity to generate words from a designated category, predicts myriad cognitive and life outcomes. The study investigated verbal fluency in children with ADHD, autism spectrum disorder (ASD), and comorbid ADHD and ASD, to understand how ADHD- and ASD-related symptoms individually and jointly predict verbal fluency, and the underlying linguistic and cognitive substrates.Method:Thirty-three school-aged children with ADHD, 27 with ASD, 25 with comorbid ADHD and ASD, and 39 with typical development, were assessed for ADHD and ASD symptoms and completed a semantic verbal fluency task.Results:Findings indicated that ADHD and ASD symptoms, especially ADHD hyperactivity-impulsivity symptoms and language-related ASD symptoms, interactively predicted verbal fluency across diagnostic groups.Conclusion:The study implicated the potential cognitive and linguistic mechanisms underlying verbal fluency differences in ADHD and/or ASD, and clinical practices on enhancing verbal fluency in these clinical groups.

Abstract:Intellectual and developmental disabilities (IDDs) are associated with both cognitive challenges and difficulties in conceptual, social, and practical areas of living, commonly referred to as adaptive behavior (DSM–5). Although cross-sectional associations between intelligence or cognition and adaptive behavior have been reported in IDD populations, no study to date has examined whether developmental changes in cognition contribute to or track with changes in adaptive behavior. The present study sought to examine associations of longitudinal developmental change in domains of cognition (NIH Toolbox Cognition Battery, NIHTB-CB) and adaptive behavior domains (Vineland Adaptive Behavior Scales-3; VABS-3) including Socialization, Communication, and Daily Living Skills (DLS) over a two year period in a large sample of children, adolescents and young adults with IDD.

Abstract:Background: The cognitive function of children with either attention deficit hyperactivity disorder (ADHD) or learning disabilities (LDs) is known to be impaired. However, little is known about the cognitive function of children with comorbid ADHD and LD. The present study aimed to explore the cognitive function of children and adolescents with ADHD and learning difficulties in comparison with children with ADHD and healthy controls in different age groups in a large Chinese sample.Methods: Totally, 1043 participants with ADHD and learning difficulties (the ADHD + learning difficulties group), 870 with pure ADHD (the pure ADHD group), and 496 healthy controls were recruited. To investigate the difference in cognitive impairment using a developmental approach, all participants were divided into three age groups (6-8, 9-11, and 12-14 years old). Measurements were the Chinese-Wechsler Intelligence Scale for Children, the Stroop Color-Word Test, the Trail-Making Test, and the Behavior Rating Inventory of Executive Function-Parents (BRIEF). Multivariate analysis of variance was used.Results: The results showed that after controlling for the effect of ADHD symptoms, the ADHD + learning difficulties group was still significantly worse than the pure ADHD group, which was, in turn, worse than the control group on full intelligence quotient (98.66 +/- 13.87 vs. 105.17 +/- 14.36 vs. 112.93 +/- 13.87, P < 0.001). The same relationship was also evident for shift function (shifting time of the Trail-Making Test, 122.50 [62.00,194.25] s vs. 122.00 [73.00, 201.50] s vs. 66.00 [45.00, 108.00] s, P < 0.001) and everyday life executive function (BRIEF total score, 145.71 +/- 19.35 vs. 138.96 +/- 18.00 vs. 122.71 +/- 20.45, P < 0.001) after controlling for the effect of the severity of ADHD symptoms, intelligence quotient, age, and gender. As for the age groups, the differences among groups became nonsignificant in the 12-14 years old group for inhibition (meaning interference of the Stroop Color-Word Test, 18.00 [13.00, 25.00] s vs. 17.00 [15.00, 26.00] s vs. 17.00 [10.50, 20.00] s, P = 0.704) and shift function (shifting time of the Trail-Making Test, 62.00 [43.00, 97.00] s vs. 53.00 [38.00, 81.00] s vs. 101.00 [88.00, 114.00] s, P = 0.778).Conclusions: Children and adolescents with ADHD and learning difficulties have more severe cognitive impairment than pure ADHD patients even after controlling for the effect of ADHD symptoms. However, the differences in impairment in inhibition and shift function are no longer significant when these individuals were 12-14 years old.
