Acute Effects of Ambient PM2.5 on Lung Function among Schoolchildren.

Abstract:Previous studies have found that fine particulate matter (PM2.5) air pollution is associated with decreased lung function. However, most current research focuses on children with asthma, leading to small sample sizes and limited generalization of results. The current study aimed to measure the short-term and lag effects of PM2.5 among school-aged children using repeated measurements of lung function.This prospective panel study included 848 schoolchildren in Zhejiang Province, China. Each year from 2014–2017, two lung function tests were conducted from November 15th to December 31st. Daily air pollution data were derived from the monitoring stations nearest to the schools. A mixed-effects regression model was used to investigate the relationship between PM2.5 and lung function. The effect of PM2.5 on lung function reached its greatest at 1-day moving average PM2.5 exposure. For every 10 μg/m3 increase in the 1-day moving average PM2.5 concentration, Forced Vital Capacity (FVC) of children decreased by 33.74 mL (95% CI: 22.52, 44.96), 1-s Forced Expiratory Volume (FEV1) decreased by 32.56 mL (95% CI: 21.41, 43.70), and Peak Expiratory Flow (PEF) decreased by 67.45 mL/s (95% CI: 45.64, 89.25). Stronger associations were found in children living in homes with smokers. Short-term exposure to PM2.5 was associated with reductions in schoolchildren’s lung function. This finding indicates that short-term exposure to PM2.5 is harmful to children’s respiratory health, and appropriate protective measures should be taken to reduce the adverse effects of air pollution on children’s health.

Abstract:Background/Aim: Previous research indicated the detrimental effect of air pollution on respiratory systems. We investigated the association between air pollution and respiratory symptoms in school children based on our school cohort in Hong Kong starting from 2012. Methods: This is a retrospective study conducted with our baseline data: 1735 participants from 17 schools across Hong Kong were included in this cross-sectional analysis. Parents-administrated questionnaires were used to collect information on non-infectious rhinitis, asthma, bronchitis, sinusitis and a wide range of confounders. Data of air quality for each school were retrieved from nearest Hong Kong Environmental Protection Department (HKEPD) monitoring stations. The pollutant concentrations for past 12 months were averaged to estimate the exposure, including particulates (PM2.5, PM10, TSP) and gaseous pollutants (SO2, NO2, O3). A two-level logistic regression model was used for data analysis. Results: In total, 27.3% (473 of 1735) students reported rhinitis occurrence in past 12 months. The simple regression result demonstrated higher likelihood of rhinitis occurrence is significantly associated with higher concentration of NO2 (OR: 1.008, 95%CI: 1.001, 1.014) and PM2.5 (OR: 1.092, 95%CI: 1.011, 1.180). The statistical significance remained after adjusting for asthma, gender and age. For other symptoms, no effect is observed. Conclusions: The result indicated higher concentrations of ambient pollutants in school area may contribute to the occurrence of rhinitis in school children.
The Association Between High Ambient Air Pollution Exposure and Respiratory Health of Young Children: A Cross Sectional Study in Jinan, China

Abstract:There is growing concern about health effects of high air pollution in less-developed countries. Children represent a population at increased risk for air pollution-related respiratory conditions. This study investigated the relationship between high ambient air pollution exposure and respiratory health of young schoolchildren. From 2014 to 2016 in Jinan, China, 2532 primary school children in grades three to five from two different schools with different levels of air pollution were included in the study. Levels of ambient air pollution exposure including PM10, PM2.5, NO2, SO2, CO and O-3 were measured continuously at the two schools. A questionnaire about children's respiratory health was conducted every year. Among them, about 150 randomly selected children also performed lung function tests two times a year at the beginning of November and middle of December. Annual average exposure levels of PM2.5 (66.8-79.1 vs 90.0-107.7 mu g/m(3)), PM10 (129.5-177.3 vs 198.1-218.6 mu g/ m(3)), NO2 (45.3-53.2 vs 45.0-56.2 mu g/m(3)), SO2 (29.8-56.5 vs 40.5-80.3 mu g/m(3)), CO (1.3-1.5 vs 1.4-1.7 mg/m(3)) and O-3 (84.8-120.2 vs 61.1-128.1 mu g/m(3)) in the heavy pollution primary school were significantly higher than the light one. The higher air pollution exposure was related to increased prevalence of respiratory diseases of young children in the last year, especially allergic rhinitis. The increased odds of lung function impairment associated with exposure to higher air pollution, could be up to 171.5% (aOR = 2.715; 95% CI = 1.915-3.849) for PEF < 75% predicted in 2014. However, after short-termexposure for 1.5 month or a week, paired comparison for parameters of the same child showed different results. The association between high ambient air pollution exposure and respiratory health of young children is closely related to exposure time and dose and may be fluctuate and complex. Abstract:Objective To investigate the effect of air pollution on respiratory health in school-aged children in the main urban area of Chongqing, China. Methods The main urban area of Chongqing was divided into polluted area and clean area according to the air pollution data shown on the Environmental Protection Agency Website of Chongqing between 2010 and 2015. A cluster sampling method was used to select 695 third- or fourth-grade children from 2 primary schools in the clean or polluted area as study subjects, with 313 children from the clean area and 382 children from the polluted area. Pulmonary function was examined for all children and a standard American epidemiological questionnaire (ATS-DLD-78-C) was used to investigate the prevalence of respiratory diseases and symptoms. Results Compared with the clean area, the polluted area had significantly higher concentrations of inhalable particles (PM10), fine particulate matter (PM2.5), and nitric oxide (NOX) (P<0.05). The multivariate logistic regression analysis was performed after adjustment for confounding factors, and the results showed that compared with those in the clean area, the children in the polluted area had significantly higher risks of cough (OR=1.644), cough during cold (OR=1.596), expectoration during cold (OR=2.196), persistent expectoration (OR=1.802), and wheezing (OR=2.415). The boys and girls in the clean areahad significantly higher forced vital capacity and forced expiratory volume in one second than those in the polluted area (P<0.05). Conclusions Air pollution in the main urban area of Chongqing is associated with the increased prevalence of respiratory symptoms in school-aged children and has certain effect on children's pulmonary function.
Effects of Fine Particulate Matter and Its Constituents on Asthmatic and Allergic Symptoms in Preschool Children: A Cross-Sectional Study in Six Cities of China

Abstract:Background/Aim: Few studies examined the effects of specific PM2.5 constituents on childhood asthma and allergy in China. We aimed to investigate the effects of various PM2.5 constituents on asthmatic and allergic symptoms in preschool children in China.Methods: A cross-sectional study was conducted among 48219 preschool children in six cities (Shanghai, Nanjing, Chongqing, Changsha, Urumqi and Taiyuan) based on China , Children ,Home and Health (CCHH) project in China. Information on allergic symptoms as well as demography were collected by questionnaires. Data for ambient PM2.5 concentration and its 5 constituents: BC, NH4+, SO42-, NO3- and OC were obtained from the global burden disease project to 205 kindergartens at a spatial resolution of 0.01×0.01(1km×1km) in six cities. A hierarchical logistic regression model was performed to explore the associations between kindergarten-level PM2.5 components and individual-level outcomes of asthmatic and allergic symptoms controlling for demography and other covariates.Results: A total of 30759 participants (51.7% male) were finally completed the study due on the missing data. Both the single-pollutant and two-pollutants model showed that ambient PM2.5 per 10ug/m3 increase positively associated with the prevalence of diagnosed asthma by an adjusted odds ratio (AOR) and 95% confidence interval(CI) of 1.18(1.01-1.35) and 1.13(1.02-1.24), respectively. Furthermore, the concentration of SO42- and NO3- were significantly associated with the prevalence of asthma and current rhinitis by the AOR for each 1ug/m3 increase in SO42-, and NO3- concentrations of 1.09(1.03-1.15) for asthma and 1.04(1.00-1.07) for current rhinitis, respectively.Conclusion: Components of PM2.5, especially NO3- and SO42- exposure were associated with asthmatic and allergic diseases in preschool children in China. Considering the related sources and combustion, it may represents an preventable risk factor for childhood asthma and allergic diseases.

Abstract:School‐age children are particularly susceptible to exposure to air pollutants. To quantify factors affecting children's exposure at school, indoor and outdoor microenvironmental air pollutant concentrations were measured at 32 selected primary and secondary schools in Hong Kong. Real‐time PM10, PM2.5, NO2 and O3 concentrations were measured in 76 classrooms and 23 non‐classrooms. Potential explanatory factors related to building characteristics, ventilation practice, and occupant activities were measured or recorded. Their relationship with indoor measured concentrations were examined using mixed linear regression models. Ten factors were significantly associated with indoor microenvironmental concentrations, together accounting for 74%, 61%, 46% and 38% of variations observed for PM2.5, PM10, O3 and NO2 microenvironmental concentrations respectively. Outdoor concentration is the single largest predictor for indoor concentrations. Infiltrated outdoor air pollution contributes to 90%, 70%, 75% and 50% of PM2.5, PM10, O3 and NO2 microenvironmental concentrations, respectively, in classrooms during school hours. Interventions to reduce indoor microenvironmental concentrations can be prioritized in reducing ambient air pollution and infiltration of outdoor pollution. Infiltration factors derived from linear regression models provide useful information on outdoor infiltration and help address the gap in generalizable parameter values that can be used to predict school microenvironmental concentrations.

[A Panel Study on the Acute Respiratory Effects of Ambient Particles of Different Sizes on Children in Shanghai].

Abstract:Objective: To explore the association between size-fractionated particle number concentrations (PNC) and respiratory health in children. Methods: From November 2018 to June 2019, there were 65 children aged 6-9 years from an elementary school in shanghai recruited in this panel study with three rounds of follow-up. The forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), and exhaled nitric oxide (FeNO) levels were measured and buccal mucosa samples of children were collected at each follow-up visit. The level of PNC, temperature and humidity of the elementary school was monitored from 3 days before each physical examination to the end of the physical examination. The linear mixed effects model was used to analyze the association between PNC and indicators of respiratory health in children. Results: Linear mixed effects model analysis revealed that, at lag 2 day, an interquartile range increase in PNC for particles measuring 0.25-0.40 μm was associated with the absolute changes in FVC, FEV1 and abundance-based coverage estimator (ACE) about -60.15 ml (95%CI:-88.97 ml, -31.32 ml), -34.26 ml (95%CI:-63.22 ml, -5.31 ml), -6.00 (95%CI:-9.15, -2.84) and percentage change in FeNO about 12.10% (95%CI: 3.05%, 21.95%), respectively. These adverse health effects increased with the decrease of particle size. Conclusion: The short-term exposure to particulate matter is associated with reduced lung function, buccal microbe diversity and higher airway inflammation level among children. These adverse health effects may increase with the decrease of particle size.

Abstract:Although studies have investigated the individual effects of air pollution, land use types, and parental mental health on children's respiratory health, few studies have examined the effects of these risk factors simultaneously in children aged <2 years. We investigated the effects of exposure to air pollution, land use types surrounding residences, and parental mental health on the frequent occurrence of respiratory symptoms in children aged <2 years in the Greater Taipei area. Participants were recruited from an ongoing Taiwanese birth cohort study. We analyzed the data of the participants who had been recruited from January 2011 to April 2014 and had responded to the follow-up questionnaires at 6, 12, and 24 months. Self-administered questionnaires were used to collect participants' sociodemographic background and health, such as respiratory symptoms, and parental mental health. Pre- and postnatal pollution levels were estimated using the spatial interpolation technique (ordinary kriging) at children's residential addresses. Land use types surrounding participants' homes were evaluated by performing buffer analysis. Multiple logistic regression analyses were conducted to examine the effects of risk factors on the frequent occurrence of child respiratory symptoms in children aged 6, 12, and 24 months. We included 228, 360, and 441 children aged 6, 12, and 24 months, respectively. Our results indicated that postnatal exposure to PM2.5 and O3 was positively associated with children's respiratory symptoms. Traffic-related land-use types, sports facilities, and commercial land surrounding homes exerted adverse effects on children's respiratory symptoms, whereas the presence of schools in the neighborhood was beneficial. Parental mental health was also associated with children's respiratory symptoms. Postnatal exposure to air pollution and land use types surrounding residences were associated with respiratory health in children aged <2 years. The residential environment is a critical factor affecting children's respiratory health of children aged <2 years.

Abstract:Background: Children are vulnerable to the respiratory effects of air pollution, and their lung function has been associated with long-term exposure to low air pollution level in developed countries. However, the impact of contemporary air pollution level in developing countries as a result of recent efforts to improve air quality on children's lung function is less understood. Methods: We obtained a cross-sectional sample of 617 schoolchildren living in three differently polluted areas in Anhui province, China. 2-year average concentrations of air pollutants at the year of spirometry and the previous year (2017–2018) obtained from district-level air monitoring stations were used to characterize long-term exposure. Forced vital capacity (FVC), forced expiratory volume in 1 second (FEV 1 ), and forced expiratory flow between 25 and 75% of FVC (FEF 25−75 ) were determined under strict quality control. Multivariable regression was employed to evaluate the associations between air pollution level and lung function parameters, overall and by demographic characteristics, lifestyle, and vitamin D that was determined by liquid chromatography tandem mass spectrometry. Results: Mean concentration of fine particulate matter was 44.7 μg/m 3 , which is slightly above the interim target 1 standard of the World Health Organization. After adjusting for confounders, FVC, FEV 1 , and FEF 25−75 showed inverse trends with increasing air pollution levels, with children in high exposure group exhibiting 87.9 [95% confidence interval (CI): 9.5, 166.4] mL decrement in FEV 1 and 195.3 (95% CI: 30.5, 360.1) mL/s decrement in FEF 25−75 compared with those in low exposure group. Additionally, the above negative associations were more pronounced among those who were younger, girls, not exposed to secondhand smoke, non-overweight, physically inactive, or vitamin D deficient. Conclusions: Our study suggests that long-term exposure to relatively high air pollution was associated with impaired lung function in children. More stringent pollution control measures and intervention strategies accounting for effect modification are needed for vulnerable populations in China and other developing countries.

Abstract:Background: Few studies have evaluated the effects of ambient air pollution exposure on lung function, especially in areas with high air pollution levels. Objectives: To investigate the associations of annual concentrations of particulate matter with diameters < 2.5 mu m (PM2.5) and nitrogen dioxide (NO2) with adult lung function in Shanghai, China. Methods: We included 5276 permanent residents aged >= 20 years. Annual residential exposure to PM2.5 and NO2 was estimated by validated satellite-based and land use regression models, respectively. The effects of PM2.5 and NO2 on lung function were estimated separately using multivariable linear regression, adjusting for potential confounders. Results: Higher exposure to PM2.5 and NO2 was significantly associated with lower forced vital capacity (FVC), inspiration capacity (IC), and vital capacity (VC). An increase of 10 mu g/m(3) in the annual average PM2.5 exposure was associated with a 45.83 ml (95% CI: -82.59, -9.07) lower FVC, 1.36 (95% CI: -2.42, -0.29) lower FVC of % predicted (FVC%pred), 121.98 ml (95% CI: -164.38, -79.57) lower IC, and 89.12 ml (95% CI -124.94, -53.3) lower VC. For NO2, an increase of 10 mu g/m3 in the annual average concentration was associated with 26.65 ml (95% CI: -46.29, -7.00) lower FVC, 0.70 (95% CI: -1.27, 0.13) lower FVC%pred, 65.26 ml (95% CI: -87.76, -42.76) lower IC, and 45.88 ml (95% CI: -65.03, -26.73) lower VC. The estimated effects on FEV1 were -10.25 ml (95% CI: -40.92, 20.42) and -0.29% (95% CI: -1.40, 0.82) per 10 mu g/m(3) increase in PM2.5 and -0.74 ml (95% CI: -17.13, 15.65) and 0.01% (95% CI: -0.58, 0.61) per 10 mu g/m(3) increase in NO2, which were not statistically significant. Stratified analysis showed that the estimated effects of PM2.5 were greater in the healthy subgroup than the COPD patients. Obese individuals were more susceptible to adverse effects of PM2.5 and NO2 on lung function. Education level showed no or only weak evidence of modification of the associations between air pollution and lung function. Conclusion: In this study, long-term exposure to ambient air pollutants was significantly associated with impaired lung function, presenting as restrictive ventilatory patterns.
Long-term Exposure to PM2.5 and Children's Lung Function: a Dose-Based Association Analysis.

Abstract:Background: The current literature is still not consist regarding the effect of long-term exposure to PM2.5 and children's lung function, partly due to inadequate or inaccurate exposure assessment. In this study, we aim to investigate the associations between long-term exposure to PM2.5, estimated as average daily dose (ADD), and lung function in school-age children. Methods: We recruited 684 participants of 7-12 years old from the city of Lanzhou located in northwestern China. Participants underwent spirometric tests for lung function and responded to a questionnaire survey. Detailed information about individual air exposure and personal information were collected, including length of school hours, home address, age, gender, etc. Combining the spatial distribution of PM2.5 concentrations in the past 5 years and individual time-activity data, we estimated annual ADD for 5 years preceding the lung function tests and 5-year average ADD, respectively. We used multiple linear regression models to examine the associations between ADD values and lung function, controlling for a range of individual-level covariates. Results: The 5-year average ADD among all the participants was 50.5 mu g/kg-d, with higher values estimated for children living in the urban area than the suburban area, for boys than girls, and for children whose parents received a lower education attainment. We found that a 1 mu g/kg-d increment in ADD of PM2.5 was associated with a 10.49 mL (95% CI: -20.47, -0.50) decrease in forced vital capacity (FVC) and a 7.68 mL (95% CI: -15.80, -0.44) decrease in forced exploratory volume in 1 second (FEV1). Among the annual ADDs estimated for the preceding 5 years, the immediate past year prior to lung function measurement had the greatest effect on lung function. The effect was greater in girls than in boys. We found no associations between annual exposure of PM2.5 (instead of ADD) and lung function when defined concentration was used as an exposure variable. Conclusions: Long- term PM2.5 exposure, when estimated as exposure dose averaged over a year or longer, was associated with statistically significant reductions in FVC and FEV1 in children of elementaryschool age. Future studies may consider the use of individual-level dose estimates (as opposed to exposure concentrations) to improve the dose-response assessment.
Effects of air pollution on asthma hospitalization rates in different age groups in Hong Kong

Abstract:Aims: To assess the relationship between levels of ambient air pollutants and hospitalization rates for asthma in Hong Kong (HK). Methods: This is a retrospective ecological study. Data of daily emergency hospital admissions to 15 major hospitals in HK for asthma and indices of air pollutants [sulphur dioxide (SO(2)), nitrogen dioxide (NO(2)), ozone (O(3)), particulates with an aerodynamic diameter of <10 microm particulate matter (PM(10)) and 2.5 microm (PM(2.5))] and meteorological variables from January 2000 to December 2005 were obtained from several government departments. Analysis was performed by the generalized additive models with Poisson distribution. The effects of time trend, season, other cyclical factors, temperature and humidity were adjusted. Autocorrelation and overdispersion were corrected. Results: Altogether, 69 716 admissions were assessed. Significant associations were found between hospital admissions for asthma and levels of NO(2), O(3), PM(10) and PM(2.5). The relative risks (RR) for hospitalization for every 10 microg/m(3) increase in NO(2), O(3), PM(10) and PM(2.5) were 1.028, 1.034, 1.019 and 1.021, respectively, at a lag day that ranged from cumulative lag 0-4 to 0-5. In a multi-pollutant model, O(3) was significantly associated with increased admissions for asthma. The younger age group (0-14 years) tended to have a higher RR for each 10 microg/m(3) increase in pollutants than those aged 15-65 years. The elderly (aged >/=65 years) had a shorter 'best' lag time to develop asthma exacerbation following exposure to pollutants than those aged <65 years. Conclusion: Adverse effects of ambient concentrations of air pollutants on hospitalization rates for asthma are evident. Measures to improve air quality in HK are urgently needed.

Abstract:BACKGROUND: Males and females exhibit different health responses to air pollution, but little is known about how exposure to air pollution affects juvenile respiratory health after analysis stratified by allergic predisposition. The aim of the present study was to assess the relationship between air pollutants and asthmatic symptoms in Chinese children selected from multiple sites in a heavily industrialized province of China, and investigate whether allergic predisposition modifies this relationship.METHODOLOGY/PRINCIPAL FINDINGS: 30139 Chinese children aged 3-to-12 years were selected from 25 districts of seven cities in northeast China in 2009. Information on respiratory health was obtained using a standard questionnaire from the American Thoracic Society. Routine air-pollution monitoring data was used for particles with an aerodynamic diameter ≤10 µm (PM(10)), sulfur dioxide (SO(2)), nitrogen dioxides (NO(2)), ozone (O(3)) and carbon monoxide (CO). A two-stage regression approach was applied in data analyses. The effect estimates were presented as odds ratios (ORs) per interquartile changes for PM(10), SO(2), NO(2), O(3), and CO. The results showed that children with allergic predisposition were more susceptible to air pollutants than children without allergic predisposition. Amongst children without an allergic predisposition, air pollution effects on asthma were stronger in males compared to females; Current asthma prevalence was related to PM(10) (ORs = 1.36 per 31 µg/m(3); 95% CI, 1.08-1.72), SO(2) (ORs = 1.38 per 21 µg/m(3); 95%CI, 1.12-1.69) only among males. However, among children with allergic predisposition, more positively associations between air pollutants and respiratory symptoms and diseases were detected in females; An increased prevalence of doctor-diagnosed asthma was significantly associated with SO(2) (ORs = 1.48 per 21 µg/m(3); 95%CI, 1.21-1.80), NO(2) (ORs = 1.26 per 10 µg/m(3); 95%CI, 1.01-1.56), and current asthma with O(3) (ORs = 1.55 per 23 µg/m(3); 95%CI, 1.18-2.04) only among females.CONCLUSION/SIGNIFICANCE: Ambient air pollutions were more evident in males without an allergic predisposition and more associations were detected in females with allergic predisposition.

Abstract:Evidence concerning short-term acute association between air pollutants and hospital admissions for respiratory diseases among children in a multi-city setting was quite limited. We conducted a time-series analysis to evaluate the association of six common air pollutants with hospital admissions for respiratory diseases among children aged 0-14 years in 4 cities (Guangzhou, Shanghai, Wuhan and Xining), China during 2013-2018. We used generalized additive models incorporating penalized smoothing splines and random-effect meta-analysis to calculate city-specific and pooled estimates, respectively. The exposure-response relationship curves were fitted using the cubic spline regression. Subgroup analyses by gender, age, season and disease subtype were also performed. A total of 183,036 respiratory diseases hospitalizations were recorded during the study period, and 94.1% of the cases were acute respiratory infections. Overall, we observed that increased levels of air pollutants except O-3, were significantly associated with increased hospital admissions for respiratory disease. Each 10 mu g/m(3) increase in PM2.5, SO2 and NO2 at lag 07, PM10 at lag 03 and per 1 mg/m(3) increase in CO at lag 01 corresponded to increments of 1.19%, 3.58%, 2.23%, 0.51% and 6.10% in total hospitalizations, respectively. Generally, exposure-response relationships of PM2.5 and SO2 in Guangzhou, SO2, NO2 and CO in Wuhan, as well as SO2 and NO2 in Xining with respiratory disease hospitalizations were also found. Moreover, the adverse effects of these pollutants apart from PM2.5 in certain cities remained significant even at exposure levels below the current Chinese Ambient Air Quality Standards (CAAQS) Grade II. Children aged 4-14 years appeared to be more vulnerable to the adverse effects of PM2.5, SO2 and NO2. Furthermore, with the exception of O-3, the associations were stronger in cold season than in warm season. Short-term exposure to PM2.5, SO2, NO2 and CO were associated, in dose-responsive manners, with increased risks of hospitalizations for childhood respiratory diseases, and adverse effects of air pollutants except PM2.5 held even at exposure levels below the current CAAQS Grade II in certain cities.

Abstract:Background Evidence of the effects of fine particulate matter (PM2·5) and its chemical constituents on childhood pneumonia is scarce. We aimed to investigate the effects of PM2·5 and its chemical constituents on doctor-diagnosed pneumonia in preschool children in China. Methods A cross-sectional study was done in six Chinese cities (ie, Shanghai, Nanjing, Chongqing, Changsha, Urumqi, and Taiyuan), based on the China, Children, Homes and Health project (2011–12), and included 30 759 preschool children across 205 preschools. Information on the prevalence of life-ever pneumonia, demographic characteristics, and home environmental factors were collected by validated questionnaires. Questionnaires were answered by parents or guardians of the children, and completed questionnaires were returned within 1 week, under the guidance of trained project investigators. The annual levels of ambient air pollutants (PM2·5, ozone, and dust) and five major PM2·5 chemical constituents (ie, black carbon, organic carbon, and three water-soluble ions [ammonium, NH4+; sulphate, SO42−; and nitrite, NO3−]) were obtained from a combination of satellite remote sensing, chemical transport modelling, and ground-based monitors (spatial resolution of 0·01° × 0·01° [ie, 1 km × 1 km]). The ambient air pollutants and chemical constituents were allocated to children according to geocoded preschool addresses. A hierarchical multiple regression model was done to evaluate the associations between PM2·5 and chemical components and childhood pneumonia at the preschool level and at the individual level, after adjusting for covariates. The study was approved by the ethics committee of the School of Public Health, Fudan University, Shanghai, China. Findings The prevalence of life-ever doctor-diagnosed pneumonia was significantly different between the six cities (p=0·004). Apart from family history of allergy, parental smoking, indoor dampness, and interior decoration, we found the prevalence of diagnosed pneumonia was significantly associated with the ambient PM2·5 (per 10 μg/m3) by an adjusted odds ratio (OR) of 1·10 (95% CI 1·00–1·21). The secondary chemical components, NH4+, SO42−, and NO3− (per 1 μg/m3), were significantly associated with childhood pneumonia, as shown by adjusted ORs of 1·08 (1·02–1·14), 1·04 (1·00–1·08), and 1·05 (1·01–1·09), respectively. Stratified analyses showed children who lived in urban areas or who were breastfed for less than 6 months had increased risk of pneumonia by ambient NH4+, SO42−, or NO3− exposure. Interpretation The chemical constituents of PM2·5, especially the water-soluble parts of NH4+, SO42−, and NO3−, were significantly associated with childhood pneumonia in China. This association indicates that these constituents might be important environmental triggers of childhood pneumonia. Abstract:The urbanization and industrialization in China is accompanied by bad air quality, and the prevalence of asthma in Chinese children has been increasing in recent years. To investigate the associations between ambient PM2.5 levels and asthmatic and allergic diseases or symptoms in preschool children in China, we assigned PM2.5 exposure data from the Global Burden of Disease (GBD) project to 205 kindergartens at a spatial resolution of 0.1° × 0.1° in six cities in China (Shanghai, Nanjing, Chongqing, Changsha, Urumqi, and Taiyuan). A hierarchical multiple logistical regression model was applied to analyze the associations between kindergarten-level PM2.5 exposure and individual-level outcomes of asthmatic and allergic symptoms. The individual-level variables, including gender, age, family history of asthma and allergic diseases, breastfeeding, parental smoking, indoor dampness, interior decoration pollution, household annual income, and city-level variable-annual temperature were adjusted. A total of 30,759 children (average age 4.6 years, 51.7% boys) were enrolled in this study. Apart from family history, indoor dampness, and decoration as predominant risk factors, we found that an increase of 10 μg/m3 of the annual PM2.5 was positively associated with the prevalence of allergic rhinitis by an odds ratio (OR) of 1.20 (95% confidence interval [CI] 1.11, 1.29) and diagnosed asthma by OR of 1.10 (95% CI 1.03, 1.18). Those who lived in non-urban (vs. urban) areas were exposed to more severe indoor air pollution arising from biomass combustion and had significantly higher ORs between PM2.5 and allergic rhinitis and current rhinitis. Our study suggested that long-term exposure to PM2.5 might increase the risks of asthmatic and allergic diseases or symptoms in preschool children in China. Compared to those living in urban areas, children living in suburban or rural areas had a higher risk of PM2.5 exposure.

Abstract:Ambient air pollutants are health hazards to children. This study comprised 773,504 emergency department visits (EDVs) at 0-14 years of age with respiratory diseases in southern China. All air pollutants were positively associated with EDVs of total respiratory diseases, especially pneumonia. NO2, PM10, and PM2.5 had intraday effects and cumulative effects on asthma EDVs. The effect of SO2, PM10, and PM2.5 on pneumonia EDVs was stronger in girls than in boys. The effect of NO2 on acute upper respiratory tract infection EDVs was greater in children aged 0-5 years old; however, the effect of PM10 on acute upper respiratory tract infection EDVs was greater in the 6-14 years group. In a two-pollutant model, NO2 was associated with bronchitis and pneumonia, and PM10 was associated with acute upper respiratory tract infection. In this time-series study, NO2 and PM10 were risk indicators for respiratory diseases in children.

Abstract:There was limited evidence regarding the association between short-term exposure to ambient particulate matter (PM) and respiratory outpatient visits among children at a multicity level. In this study, a time-series study was conducted among children aged 0-14 years in five Chinese cities from 2013 to 2018. City-specific effects of fine particles (PM2.5), inhalable particles (PM10) and coarse particles (PM10-2.5) were estimated for time lags of zero up to seven previous days using the overdispersed generalized additive models after adjusting for time trends, meteorological variables, day of the week and holidays. Meta-analyses were applied to pool the overall effects, while the exposure-response (E-R) curves were evaluated using a cubic regression spline. The overall effects of PM were significantly associated with total and cause-specific respiratory outpatients among children, even at PM2.5 and PM10 levels below the current Chinese Ambient Air Quality Standards (CAAQS) Grade II. Each 10 μg/m3 increment in PM2.5, PM10 and PM10-2.5 at lag 07 was associated with a 1.39% (95% CI: 0.38%, 2.40%), 1.10% (95% CI: 0.38%, 1.83%) and 2.93% (95% CI: 1.05%, 4.84%) increase in total respiratory outpatients, respectively. An E-R relationship was observed except for PM2.5 in Beijing and PM10 and PM10-2.5 in Shanghai. The effects of PM were stronger in cold season in 3 southern cities, while it was stronger in transition season in 2 northern cities. In conclusion, short-term PM exposures were dose-responsive associated with increased respiratory outpatient visits among children, even for PM2.5 and PM10 levels below current CAAQS II in certain cities.
