Abstract:OBJECTIVES:Intestinal tuberculosis (ITB) remains prevalent and a big health hazard in China. The aim of this study was to retrospectively analyse its clinico-pathological features. METHODS:Retrospective study of 85 consecutive ITB patients in two tertiary hospitals in East China. Relevant clinical, laboratory examination, radiological, endoscopic and histopathological features of ITB were recorded. RESULTS:The mean age was 37.3 ± 16.0 years; 56 patients (65.9%) were male. 67.1% had ITB secondary to pulmonary tuberculosis. The overall median length of hospital stay was 28 days and was significantly longer in patients with intestinal complications (P = 0.003) and malnutrition (P = 0.042). Abdominal pain (88.2%) and weight loss (75.3%) were the commonest symptoms. The positive rate of the purified protein derivative (PPD) test was 88.2%; of the T-spot, 85.7%. Histopathology revealed caseating granuloma in 70.6% and caseating necrosis in 24.7% of patients. The most commonly affected sites were the ileocecal valve (56, 65.9%), terminal ileum (40, 47.1%) and caecum (33, 38.8%). Only 17 (20%) patients were initially diagnosed as ITB, the other 68 patients were misdiagnosed. Six patients with caecum tuberculosis were misdiagnosed as appendicitis, four of whom had improper surgical procedures followed by post-operative intestinal fistulas; two died due to MODS. CONCLUSIONS:Diagnosis of ITB is often misdirected and delayed, which may lead to inappropriate treatment and high mortality. High diagnostic suspicion is necessary for patients with unexplained abdominal complaints. Diagnosis is not easy but could benefit coexisting pulmonary tuberculosis, T-spot, CT imaging, colonoscopy, pathological features, acid-fast bacilli and response to anti-tuberculosis therapy (ATT).

Abstract:AIM: To assess CT, MR manifestations and their diagnostic value in hepatic tuberculosis.METHODS: CT findings in 12 cases and MR findings in 4 cases of hepatic tuberculosis proved by surgery or biopsy were retrospectively analyzed.RESULTS: (1) CT findings: One case of serohepatic type of hepatic tuberculosis had multiple-nodular lesions in the subcapsule of liver. Parenchymal type was found in 10 cases, including multiple, miliary, micronodular and low-density lesions with miliary calcifications in 2 cases; singular, low-density mass with multiple flecked calcifications in 3 cases; multiple cystic lesions in 1 case; multiple micronodular and low-density lesions fusing into multiloculated cystic mass or "cluser" sign in 3 cases; and singular, macronodular and low-density lesion with multiple miliary calcifications in 1 case. One case of tuberculous cholangitis showed marked dilated intrahepatic ducts with multiple flecked calcifications in the porta hepatis. (2) MR findings in 4 cases were hypointense on both T1-weighted imagings and T2-weighted imagings in one case, hypointense on T1-weighted imagings and hyperintense on T2-weighted imagings in 3 cases. Enhanced MR in 3 cases was slightly shown peripheral enhancement or with multilocular enhancement.CONCLUSION: Various types of hepatic tuberculosis have different imaging findings, and typical CT and MR findings can suggest the diagnosis.
Demographic, Clinical, Radiological, and Surgical Outcome of Patients with Intestinal Tuberculosis: A Single-Center Retrospective Study

Abstract:Background: Intestinal tuberculosis (iTB) represents a potentially underrecognized clinical entity with limited clinical and radiological differentiating features. This study aims to assess the patterns of iTB clinical and radiological findings, along with the treatment approaches and the overall outcome. Methods: This retrospective cross-sectional study included patients with histopathologically confirmed iTB who presented with acute abdomen and were surgically managed between September 2005 and October 2023. Clinical and sociodemographic variables, imaging features, surgical treatments, and overall outcomes were retrospectively analyzed. Results: 96 patients with iTB were included, with a mean age of 36.1 ± 11.5 years and a relatively proportionate gender distribution. Abdominal pain was the most common presenting symptom (45.8%). The radiological features varied by the modality. Plain imaging showed non-specific findings, while ultrasonography showed loculated ascites (25%), and lymphadenopathy (22%). In computed tomography scans, multi-segmental symmetric intestinal thickening (53.1%) was the most prevalent finding. The most commonly performed surgical procedure was adhesiolysis (29.2%), with the ileocecal junction being the most commonly involved structure (39.6%). Histopathological examination of all the tissue biopsies revealed epithelioid granulomas. Postoperative complications occurred in 19 patients (19.8%), with surgical site infection being the most common complication (10.4%). Conclusion: Intestinal obstruction is an underrecognized manifestation of tuberculosis, particularly in endemic regions. The non-specific clinical presentation, coupled with the limited utility of laboratory and radiological tests, often leads to delayed recognition and treatment. Maintaining a high index of suspicion is essential, especially in younger patients, inhabitants of endemic areas, or those with laboratory findings indicative of chronic inflammation. Prompt recognition is crucial to ensure the timely initiation of anti-tuberculosis therapy and to optimize patient outcomes through appropriate follow-up.
Altered Intestinal Microbiota and Fecal Metabolites in Patients with Latent and Active Pulmonary Tuberculosis

Abstract:Background Pulmonary tuberculosis (PTB) is the main cause of infection-related mortality and the most common infectious disease that develops resistance to antibiotics. Gut microbiota and their associated metabolites are assumed to induce and influence the development of PTB. However, the alterations of gut microbiota and metabolites in TB patients is currently unclear. Methods Fecal samples were collected from 13 PTB patients, 13 LTBI patients, and 13 healthy controls (HC). 16S rRNA sequencing and metabolomics were used to analyze the changes in the intestinal microbiota and the composition of fecal metabolites in groups. Results Our findings indicated that the α-diversity of the gut microbiota in patients with PTB and LTBI decreases compared to HC, and at the phylum level, the relative abundance of Firmicutes decreases and the relative abundance of Bacteroides increases. And six genera were notably enriched in PTB patients and four in LTBI patients. Metabolomic analysis showed alterations in metabolite levels, such as short-chain fatty acids and amino acids. Conclusions we comprehensively explored the changes in the gut microbes and fecal metabolites in patients with PTB and LTBI from the perspective of the gut microbiota, which may provide potential diagnostic biomarkers and therapeutic targets for TB diagnosis and treatment.
Clinical Features, Differential Diagnosis and Intestinal Mucosal TB-PCR for Intestinal Tuberculosis and Crohn's Disease

Abstract:Objective To analyze the difference of clinical,endoscopic and pathological features between intestinal tuberculosis(ITB) and Crohn's disease (CD),and to evaluate the significance of tissue polymerase chain reaction for Mycobacterium tuberculosis(TB-PCR) assay for the differential diagnosis.Methods One hundred and two cases diagnosed as intestinal tuberculosis(42) and CD(60) from Jan.1994 to Dec.2006 were reviewed.The clinical manifestation,endoscopic,histopathological findings,and the result of intestinal mucosal TB-PCR were analyzed.Results Hematochezia was more frequently seen in CD patients(56.9%,33/58) than that in ITB(16.1%,5/31,P0.001).Extraintestinal manifestations such as oral ulcer,skin rash,arthralgia and peri-anal diseases,were seen in 29.3% CD patients compared with none in ITB.Under the endoscope,CD appeared as longitudinal/fissure-like ulcers for 44.9%(22/49),cobblestone mucosa 28.6%(14/49),and segmental lesion 51.0%(25/49),significantly higher than those in ITB(7.7%,3.9%,0,P=0.001,0.011,0.000 respectively).As for histopathological findings,lymphoid aggregation(34%,18/53) and microvasculitis(26.4%,14/53) were more common in CD compared with ITB(0,3.8%,P=0.001,0.016 respectively).82.4%(14/17)ITB patients got symptom improved after 1～2 weeks empirical antituberculous therapy compared with no effect in 5 CD patients after 4～8 weeks antituberculous therapy.There was no significant difference in the assay of intestinal mucosal TB-PCR between ITB group(11.5%,3/26) and CD group(14.3%,3/21) (P0.5).Conclusion We need to take account the clinical features,endoscope,radiological and histological findings to diagnose ITB or CD,and the significance of intestinal mucosal TB-PCR assay is limited for diffeventially diagnosing ITB and CD.For those cases difficult to differentiation,empirical antituberculous therapy and follow-up colonoscopy are necessary.

Abstract:As Crohn’s disease (CD) has become more and more common in developing countries, tuberculosis still remains relatively common, thus differentiating CD from intestinal tuberculosis poses a great challenge to clinicians’ worldwide. There have been no good diagnostic methods until now. Recently, more cases using the TSPOT.TB test, an interferon-gamma (IFN-c) release assay of whole blood, could be highly specific for Mycobacterium tuberculosis. A 61-year-old male had abdominal distension and irregular stooling for 20 years. For the past 10 years his abdominal pain was more noticeable and was accompanied with green mucosy stools. His condition became more severe 2 years ago with right lower quadrant (RLQ) pain, nausea, and vomiting. B ultrasound showed a hypoechoic mass in the right lower quadrant diagnosed as ‘‘appendix abscess.’’ A second B ultrasound showed disappearance of the mass following antibiotic therapy. However, during the last year the RLQ pain returned and he underwent surgery to relieve intestinal obstruction caused by appendicitis and intestinal adhesions. One month later a partial colonoscopic examination was performed but inflammation and stricturing prevented a full examination. Double contrast barium enema showed stricturing and shortening of the ileocecum, ascending colon, and hepatic flexure. A second colonoscopy was successful and revealed ulceration of the cecum and ascending colon. Histology showed chronic inflammation. At the same time the patient also complained of severe pain in his joints. The medical team suspected CD or intestinal tuberculosis (TB). Differentiating CD from TB can be quite difficult. In this case our patient had a negative tuberculin test, but a strong positive T-SPOT result. Later, because the patient also had articule cavity dropsy, we obtained fluid from the cavity, which was found to contain tuberculosis antibodies, suggesting he had intestinal TB. We commenced antituberculosis treatment, which improved the patient’s clinical symptom and endoscopic presentation (Fig. 1). A second case was a 35-year-old female suffering from recurrent abdominal pain for 2 years which became more severe over 20 days. Colonoscopy showed multiple ulcers in the ascending colon and ileocecus and was diagnosed with CD. The T-SPOT assay was performed and was positive. FIGURE 1. Abstract:Valgus tibial osteotomy is a well-accepted procedure for the treatment of unicompartmental degenerative joint disease of the knee. For a good result to be obtained, at least for degrees of valgus angulation with the leg in the weight-bearing position must be achieved. Experience with 20 procedures showed a tendency for undercorrection, which led to poor results. After analyzing the technical reasons for these poor results, we devised a detailed protocol that included patient selection, preoperative planning, and an operative technique that minimizes the chance of technical error.

Abstract:Abstract Background The purpose of this study was to examine the value of Xpert MTB/RIF assay and detection of additional Mycobacterium tuberculosis complex (MTBC) species antigens from intestinal tissue samples in differentiating intestinal tuberculosis (ITB) from Crohn’s disease (CD). Methods Several clinical specimens of intestinal tissue obtained by either endoscopic biopsy or surgical excision were used for mycobacteriologic solid cultures,Xpert MTB/RIF assays, immunohistochemistry, and histological examinations. Four antigens (38KDa, ESAT-6, MPT64, and Ag85 complex) of MTBC in the intestinal tissue were detected by immunohistochemical analysis. Results The study included 42 patients with ITB and 46 with CD. Perianal lesions and longitudinal ulcers were more common in patients with CD, while caseating granuloma and annular ulcers were more common in patients with ITB. The positive rate of MTBC detected by Xpert MTB/RIF in intestinal tissues of patients with ITB was 33.33%, which was significantly higher than that in patients with CD and that detected using acid-fast staining smears. It was also higher than that detected by tissue MTBC culture, but the difference was not statistically significant. The positive MPT64 expression rate in patients with ITB was 40.48%, which was significantly higher than that observed in patients with CD. The sensitivity of parallelly combined detection of tuberculosis protein MPT64 and Xpert MTB/RIF in diagnosing ITB was 50.0%. Conclusions The detection of Xpert MTB/RIF in intestinal tissue is a rapid and useful method for establishing an early diagnosis of ITB. The detection of MTBC using Xpert MTB/RIF and MPT64 antigen in intestinal tissues has a definitive value in the differential diagnosis ofITB and CD. The combination of these two methods can improve the detection sensitivity.

Abstract:Background: Abdominal tuberculosis (TB) remains an issue as it masquerades as many malignant or benign abdominal conditions. Objective: To analyze the clinical and laboratory features of abdominal TB retrospectively and discuss its management. Methods: The data of patients with a histopathologic diagnosis of abdominal TB seen from January 1, 2008, to February 1, 2019 were collected in The First Affiliated Hospital of Nanjing Medical University. Nodal, visceral, peritoneal, and mixed TB cases were included while excluding other forms of extra-pulmonary TB (EPTB). Results: A total of 21 patients presented having a median age of 49 years (interquartile range 33-57 years) with 12 females and 9 males. Ten presented with abdominal pain, whereas four had abdominal pain and distention. Weight loss was present in five and type 2 diabetes mellitus (DM) in three. Every patient received contrast-enhanced computed tomography (CE-CT) with positive results in all the cases. Seven patients received endoscopic ultrasound-guided fine-needle aspiration cytology examination (EUS-FNAC) and five had results positive for TB. Pre-operative diagnosis of abdominal TB was possible in seven; however, the majority (n = 14) underwent exploratory laparotomy, and all obtained a definitive diagnosis of TB. No deaths occurred. Conclusions: Both CE-CT and EUS-FNAC can aid in the timely diagnosis. Laparotomy is an invasive but efficient tool for the final diagnosis of abdominal TB.
Clinical Features and Diagnostic Approaches for Abdominal Tuberculosis: Five-Year Experience from a Non-Tuberculosis-designated Hospital in China

Abstract:Background and purpose: abdominal tuberculosis (TB) is a common form of extrapulmonary TB but it is still a diagnostic dilemma in clinical practice. This study aimed to highlight the clinical features and diagnostic approaches for abdominal TB. Methods: seventy cases of diagnosed abdominal TB were retrospectively collected between August 1st, 2015 and June 30th, 2020. They were classified as peritoneal TB, lymph node TB, gastrointestinal TB, visceral TB or mixed TB. Results: eighteen patients were diagnosed with peritoneal TB, nine with lymph node TB, five with gastrointestinal TB, two with visceral TB and 36 with mixed TB. More than 65 % of the patients had tuberculosis of other sites except the abdomen. The median diagnosis time was 60 days. Ascites (58.6 %), abdominal distension (48.6 %), weight loss (44.3 %) and fever (42.9 %) were the most common symptoms. The overall microbiological and histological detection rates were 70.0 % and 38.6 %, respectively. The non-ascite samples yielded a higher microbiological confirmation rate (63.6 %) than the total samples (40.8 %). Diagnosis was confirmed histologically in 18 patients (69.2 %). Forty-five cases (64.3 %) were clinically diagnosed. Invasive procedures such as surgery (6/7), percutaneous biopsy (7/7) and endoscopy in lymph node TB (4/5) had high confirmation rates. Conclusions: the diagnosis of abdominal TB should be reached by a combination of clinical, laboratory, radiological, microbiological and pathological findings.

Abstract:Abstract Background Anti-tumor necrosis factor (TNF) therapy increased the risk of tuberculosis(TB), and the characteristic clinical feature of western patients with inflammatory bowel disease (IBD) receiving anti-TNF therapy were predominantly extrapulmonary TB. We aimed to analyze the clinical features of patients with IBD who in TB-endemic area (China) that developed active tuberculosis after receiving anti-TNF therapy. Methods Patients from 9 hospital in 7 provinces developed active TB after the initial treatment with infliximab. Demographics, interval between the time at the first dose of anti-TNF therapy and active TB development, tests for latent TB infection (LTBI), concomitant medications, and the details of diagnosis were analyzed in our study. The diagnosis of active TB and LTBI were made based on comprehensive evaluation including TB skin test (TST), interferon gamma releasing assay (IGRA), X-ray test and computed tomography (CT). Results Totally 43 patients developed active TB during infliximab therapy. Among 43 patients, 6 patients had a past TB history and 11 patients were confirmed as LTBI before infliximab treatment. Six patients with LTBI received anti-TB prophylaxis as the same time as anti-TNF treatment. The mean time between initiation of infliximab therapy and active tuberculosis diagnosis in patients with LTBI was significantly shorter than patients without LTBI (216.9±83.2 days versus 410.9±220.3 days, p=0.042). Among 43 active TB patients, 29 patients (29/43, 67.4%) had isolated pulmonary tuberculosis and 11 patients (11/43, 25.6%) had pulmonary TB concomitantly with other organ involvement. Conclusion IBD patients with LTBI develop TB earlier than those without LTBI during anti-TNF treatment, and history of treatment and anti-TB prophylaxis didn’t fully prevent active TB. The most common disease site is lung in China, most of cases were diagnosed by imaging. Chinese physicians should be aware of the potential for TB development during anti-TNF therapy, especially pulmonary tuberculosis.
Intestinal tuberculosis with small bowel stricture and hemorrhage as the predominant manifestation: Three case reports

Abstract:BACKGROUND Intestinal tuberculosis is a chronic disease caused by Mycobacterium tuberculosis that mainly affects the ileum and cecum. Small bowel tuberculosis, characterized by predominant involvement of the small intestine, is an extremely rare condition with highly atypical clinical presentations, making diagnosis even more challenging. CASE SUMMARY We report three cases of small intestinal tuberculosis, two of the patients presented primarily with abdominal pain, and one presented with gastrointestinal bleeding. All patients underwent blood tests and imaging examinations. Small bowel endoscopy (SBE) revealed that the main lesions in these patients were intestinal stenosis or gastrointestinal bleeding caused by small intestinal ulcers. One patient ultimately underwent surgical treatment. Following a complex diagnostic process and comprehensive analysis, all patients were confirmed to have small intestinal tuberculosis and received standard antituberculosis treatment, leading to an improvement in their condition. CONCLUSION Patients with SBTs present with nonspecific symptoms such as abdominal pain, weight loss, and occasional gastrointestinal bleeding. Accurate diagnosis requires a thorough evaluation of clinical symptoms and various tests to avoid misdiagnosis and complications.

Is the Identification of Caseating Granuloma in the Intestine Indicative of Tuberculosis? a Rare Case of Crohn’s Disease

Abstract:BACKGROUND:Crohn's disease (CD) is a chronic intestinal inflammatory disorder, the etiology of which remains unknown, and is characterized by symptoms such as chronic abdominal pain, diarrhea, obstruction, and perianal lesions. Histopathology is widely regarded as the preferred method for diagnosing CD, although the histological diagnosis may lack specificity. The identification of granulomas is commonly believed to be the most reliable diagnostic indicator for CD, surpassing all other clinical features in significance. Nevertheless, research indicates that the detection rate of granulomas in CD exhibits considerable variability. Furthermore, granulomas can manifest in various specific infections including tuberculosis and Yersinia, as well as in a range of diseases characterized by macrophage reactions such as sarcoidosis and drug-induced enteritis. Granulomas associated with CD typically do not exhibit necrosis. However, the formation of caseous granulomas may occur as a result of secondary infections related to anti-CD drug treatment or perforation of the intestinal wall. CASE PRESENTATION:In this study, we present a case of a 28-year-old female patient diagnosed with CD exhibiting histologic granulomas, including both caseating and non-caseating forms, which demonstrated a positive response to medical treatment. CONCLUSION:In clinical practice, various forms of granulomas may indicate diverse underlying diseases, yet lack specificity. It is suggested that the presence of caseous granulomas should not be considered as a definitive exclusion criterion for the diagnosis when clinical, endoscopic, imaging and other histopathological features are consistent with CD. This study is the first report of caseous granulomas in CD without concomitant tuberculosis infection.

Abstract:Objective To investigate the status quo of tuberculosis (TB) screening among patients with inflammatory bowel diseases (IBD) before receiving infliximab (IFX) therapy in China.Methods IBD patients receiving IFX treatment during January 2009 to December 2016 from 22 hospitals nationwide were enrolled retrospectively.Clinical data of TB screening before IFX therapy,including previous TB history,tuberculin test (TST test),interferon-γrelease assay (IGRA) and chest image results (chest X-ray and chest CT) were all recorded.TB screening result whether was latent tuberculosis infection (LTBI)judged by the doctor and clinical data related to whether receiving prophylactic treatment were collected.Results A total of 1711 IBD patients were enrolled,including 1606 (93.9％) cases receiving TB screening with TST or IGRA combining chest X-ray or CT scan before IFX treatment,663 (38.7％) cases with IGRA combining chest CT scan,and only 504 (29.5％) cases with complete TST plus IGRA combining chest CT scan,while 77(4.5％) cases had no chest imaging screening and 66(3.9％) cases did not receive any screening for TST or IGRA.TB screening results before IFX treatment showed 7.1％(121/1711) of IBD patients were LTBI,in whom 27 (27/121,22.3％) IBD patients with LTBI were neglected by clinicians,presenting as omission of judgment for LTBI or failure to take prophylactic anti-TB treatment during IFX treatment.Conclusions The TB screening before IFX treatment in China is not sufficient at present.The LTBI prevalence of IBD patients in China may be on the low side and clinicians have no enough knowledge of LTBI.It is recommended that IBD patients should undergo a standard TB screening before IFX therapy.

Abstract:The misdiagnosis of intestinal tuberculosis (ITB), such as Crohn's disease (CD), and subsequent treatment with immunosuppressive therapies can lead to severe outcomes. However, the differential diagnosis between these two conditions can be challenging. We herein report a patient from Myanmar who was initially diagnosed with CD due to the presence of non-caseating granulomas. The patient's symptoms were aggravated with steroid treatment, eventually leading to a diagnosis of ITB. In the international medical community, we encounter patients from countries, such as Myanmar, where tuberculosis is endemic. Therefore, it is necessary to understand the epidemiological background of each country to accurately distinguish between CD and ITB.

Confluent granulomas and ulcers lined by epithelioid histiocytes: new ideal method for differentiation of ITB and CD? A meta analysis.

Abstract:BACKGROUND:There are few widely accepted criteria other than caseation, which has low sensitivity, for differentiating intestinal tuberculosis (ITB) and Crohn's disease (CD). OBJECTIVE:We performed a meta-analysis to evaluate the use of confluent granulomas and ulcers lined by epithelioid histiocytes as histological methods for differentiating ITB and CD, compared with that of caseation. METHODS:We searched PubMed, Medline, Embase, Web of Science, the Cochrane Library and Chinese Biomedicine Database for all relevant studies on the histological differentiation of ITB and CD. Sensitivity, specificity, and diagnostic odds ratio (DOR) were calculated for each study. Study quality and heterogeneity were assessed. Meta-regression analysis and sensitivity analyses were performed. RESULTS:Ten randomized trials involving 316 ITB and 376 CD patients were included. The results showed that analysis of caseation showed an overall weighted area under the curve (AUC) of 0.9966, overall sensitivity and specificity were 0.21 and 1.00, respectively, with a positive likelihood ratio (+LR) of 10.79, negative likelihood ratio(-LR) of 0.82 and DOR of 13.74. Confluent granulomas had a lower overall weighted AUC of 0.9381, sensitivity and specificity were 0.38 and 0.99, respectively, with a +LR of 16.29, -LR of 0.65 and DOR of 26.52. Overall weighted AUC for ulcers lined by epithelioid histiocytes was 0.9017, sensitivity and specificity were 0.41 and 0.94, respectively, with a +LR of 6.46, -LR of 0.54 and DOR of 13.17. Significant heterogeneity was noted for the studies. Meta-regression analysis showed that study source, publication year, size, design and quality did not affect heterogeneity. CONCLUSION:Confluent granulomas and ulcers lined by epithelioid histiocytes are helpful in distinguishing ITB from CD, which may provide a new method, other than caseating granulomas and acid-fast bacilli, to differentiate ITB and CD in mucosal biopsies.

Abstract:A young patient had serious hematochezia for nearly 2 months without obvious cause. Abdominal CT images showed rare features such as serious hemorrhage and multiple miliary nodules in the small bowel. The colonoscopy showed multiple lymphoid follicles in the terminal ileum. The laparotomy showed adverse adhesion in the abdominal cavity and multiple miliary noduli in the surfaces of small bowel wall. The pathology suggested tuberculosis. This patient received anti-TB therapy and the condition improved gradually. There are many variants of GI tuberculosis demonstrating uncommon and rare imaging features. It turned to be difficult in diagnosis when it showed the rare appearance, such as the great amount of intestinal bleeding and multiple noduli in our case. The reason why gastrointestinal bleeding might be attributed to the invasiveness of submucosal vessels by the tuberculosis bacteria. And the multiple noduli on the bowel wall might be the granuloma formation. Therefore, from the experience of our case, gastrointestinal tract might also be the first and only involved site, and it may cause great amount of bleeding to a life danger, even in young people.
New diagnostic strategies to distinguish Crohn's disease and gastrointestinal tuberculosis

Abstract:Purpose of review: Despite advances in our radiological, histological and microbiological armamentarium, distinguishing between Crohn's disease (CD) and intestinal tuberculosis (ITB), especially in a TB endemic country, continues to be a challenging exercise in a significant number of patients. This review aims to summarize current available evidence on novel diagnostic techniques which have a potential to fill the gap in our knowledge of differentiating between ITB and CD. Recent findings: Both ITB and CD are associated with altered host immune responses, and detection of these altered innate and adaptive immune cells has potential to distinguish ITB from CD. ITB and CD have different epigenetic, proteomic and metabolomic signatures, and recent research has focused on detecting these differences. In addition, the gut microbiome, which is involved in mucosal immunity and inflammatory responses, is considerably altered in both ITB and CD, and is another potential frontier, which can be tapped to discriminate between the two diseases. With technological advancements, we have newer radiological modalities including perfusion CT and dual-layer spectral detector CT enterography and evidence is emerging of their role in differentiating ITB from CD. Finally, time will tell whether the advent of artificial intelligence, with rapidly accumulating data in this field, will be the gamechanger in solving this puzzle of diagnostic dilemma between ITB and Crohn's disease. Summary: Recent advances need to be clinically validated before they can be used as novel diagnostic measures to differentiate Intestinal TB from CD.

Abstract:A retrospective study of all patients presenting with acute leukemia to a single institution during the period 1968-1988 was undertaken to determine the type and incidence of surgical complications. One hundred twenty-eight of 296 patients were identified as requiring a surgical consultation for complications occurring during the course of the disease and many surgical disciplines were involved. Operative intervention was often required for these complications. Pediatric surgical specialists need to be aware of the range of surgical complications that can occur in children with acute leukemia and they must work in close co-operation with pediatric medical oncologists to ensure optimal treatment for all patients.

Abstract:Abstract Background Detecting granulomas is valuable for the diagnosis of isolated intestinal tuberculosis (ITB). However, how to perform an effective endoscopic biopsy remains unclear. We aimed to explore the impact of the number and location of endoscopic biopsies on granuloma detection in patients with isolated ITB. Methods A prospective analysis was conducted in 50 patients with confirmed isolated ITB who underwent predetermined endoscopic biopsy and pathological tests. 50 patients with isolated ITB were included for analysis. The diagnosis was confirmed based on microbiological, endoscopic, and histopathological evidences. For each patient, eight endoscopic biopsy specimens were obtained from either ulcer bases (n=4) or ulcer margins (n=4). After fixation, the specimens were serially sectioned into 8 slices on average. The slices were then stained and examined by an experienced gastrointestinal pathologist who were blinded to the patient information. Results The detection rates of granulomas from 2, 4, 6, and 8 biopsy specimens were 10, 28, 50, and 82%, respectively (5/50 vs 14/50 vs 25/50 vs 41/50, P<0.00001). When 4 biopsy specimens were used, the granuloma detection rate with specimens from the ulcer base was significantly higher than that from the ulcer margin (41/50 vs 14/50, P<0.0001). 34% (17/50) of the patients had necrotising-confluent granuloma, and 12% (6/50) had confluent (>400 um) granuloma. In total, 205 granulomas were identified in 41 patients. Multi-granulomas were detected on 49 slices while mono-granulomas on were detected on 72 slices. 62.9% (129/205) of granulomas were detected in the ulcer base, 27.8% (57/205) in the lamina propria, and 9.3% (19/205) in the submucosa. Conclusion Using more than 8 or 4 biopsy specimens from the ulcer base improved the detection rate (over 50%) of necrotising or confluent granulomas for ITB. To improve the accuracy of diagnosis, clinicians should obtain ≥4 pieces of endoscopic biopsy from the ulcer base and conduct a deep biopsy into the lamina propria. Nevertheless, the integration of clinical characteristics and anti-TB responses is still required for suspected patients with undetected granulomas.
