Abstract:Sacrococcygeal teratoma (SCT) is a sacrococcygeal neoplasm derived from more than one primitive germ layer and is only occasionally encountered in adults. The primary treatment for all primary SCTs is surgical excision. The present study reports the case of a giant SCT in a middle-aged female with a history lasting >3 decades. Multi-staged surgical treatment was performed, including ileostomy plus tumor excision, four debridement plus flap repair procedures, and closure of the ileostomy. Follow-up showed improved quality of life without evidence of local recurrence after resection. The study also presents a brief overview of the relevant literature. To the best of our knowledge, this is the first report of multi-staged surgical treatment for giant SCT in an adult patient.
Laparoscopic transabdominal-sacrococcygeal approach for resection of Altman type III sacrococcygeal teratoma in adult women: A case report

Abstract:Introduction: Adult sacrococcygeal teratoma (SCT) is a rare disease that is not easily detected or easily missed, and its treatment is based on surgery, including transabdominal, transsacral, or a combination of both, but there are no clear guidelines for diagnosis and treatment. We share a case of Altman type III SCT in order to provide more reference protocols for the diagnosis and treatment of adult SCT, and more importantly to increase our understanding of different types of SCT cases in adults. Patient concerns: Our patient was a 31-year-old adult woman who underwent complete surgical resection of a cystic mature teratoma of the right ovary 8 years ago and is currently 13 months postpartum without menstruation, usually with a feeling of anal bulge, with symptoms such as constipation. Diagnosis: We diagnosed SCT by vaginal ultrasonography, computed tomography and magnetic resonance imaging (MRI); benign tumors were considered in the results of serum tumor markers. Interventions: We chose the surgical approach of laparoscopic transabdominal-sacrococcygeal approach to completely remove the patient SCT and coccyx. Outcomes: The location of SCT is concealed and the clinical symptoms are not obvious. Vaginal ultrasonography, CT and MRI can not only improve the diagnostic rate of SCT, but also understand the size and mass of SCT, providing an exact basis for clinicians to select the laparoscopic transabdominal-sacrococcygeal approach. Conclusion: Our sharing increases the reports of rare cases of teratoma with the same histological findings in different organ tissues of the same patient at different times, whether this occurs incidentally requires more case reports and further basic research; in addition, the laparoscopic transabdominal-sacrococcygeal approach is a safe and effective surgical approach for the treatment of Altman type III SCT in adults; finally, this case reminds us that SCT may not affect pregnancy and pregnancy outcomes and provides a reference for the selection of interventions for SCT with pregnancy.

Abstract:Background: Extragonadal abdominopelvic teratomas in adults are extremely rare, and those in males are exceedingly rare. These masses are most commonly found incidentally and require surgical excision for diagnostic confirmation after a thorough workup. Case presentation: This is a case report of a 49-year-old male who presented to a urology office with symptoms of hematuria, incidentally, found to have a pelvic mass on computed tomography urogram prompting colorectal surgical evaluation and subsequent laparoscopic complete excision. The clinical presentation, radiographic findings, and histopathological findings are described along with a literature review of extragonadal abdominopelvic mature cystic teratoma, also referred to as a sacrococcygeal teratoma. Discussion: A broad differential diagnosis was generated for this patient with a pelvic mass after complete work-up, consisting of a dermoid or epidermoid cyst, liposarcoma, or sacrococcygeal teratoma. Radiological features can aid in the diagnostic confusion that may present in the adult patient. Conclusion: Albeit rare in the male and adult population, sacrococcygeal teratoma is a plausible differential diagnosis for a pelvic mass. Underrepresented in the literature in regard to guidelines on management, complete surgical excision is the gold standard, with laparoscopy being a reasonable approach.

Abstract:Objective:To discuss early diagnosis and main points of surgery about sacrococcygeal teratoma in children.Methods : Information about the diagnosis and operative therapy of was annlyzed 56 cases sacrococcygeal teratoma in children.Results :There was no malignant teratoma in children under 5 months older.However,about 70% teratoma were malignant at the age of 1～3 years.All of the innocence teratoma cases were recovered after operation,but some cases might be come malignent if cutting of tumor tissue was incomplete during first operative therapy.And malignent teratoma cases were dead 1～3 years after of operation and chemotherapy.Conclusion: The key points for sacrococygeal teratoma are early period clinical diagnosic and thoroughly exairesis of tumor tissue.Maybe We can avoid malignant in some degree,and innocent teratoma may recover completely after operation.

Abstract:Background Sacrococcygeal teratoma is one of the most common congenital tumors in newborns and infancy. The incidence is 1 per 20,000–40,000 live births. Ultrasonography is an optimal method for prenatal screening and diagnosis of fetal sacrococcygeal teratoma. MRI can be used to assist in the diagnosis. However, sacrococcygeal teratoma in the twin pregnancy is rare. Case presentation We reported a case of one twin with sacrococcygeal teratoma in dichorionic-diamniotic twin pregnancy.One twin with sacrococcygeal teratoma was diagnosed at the second trimester by ultrasonic examination and another twin was normal. A regular and careful antenatal care was conducted by the multidisciplinary team. The parents refused to perform the fetal MRI and examine the chromosome of both twin.At 37 + 1 of gestation, planned cesarean section was performed. The healthy male co-twin (twin A) weighed 2880 g.The male twin with SCT (twin B) weighed 2900 g, complying with 6 × 3 × 3 cm cystic and solid mass in sacrococcygeal region. At four days of age twin B underwent excisional surgery of the sacrococcygeal teratoma and coccyx and discharged 7 days after surgery. The mother and both babies were followed up and are all in good health until now. Conclusion(s) Sacrococcygeal teratoma in twin pregnancy is rare. Early antenatal diagnosis is important. Once the sacrococcygeal teratoma is diagnosed, clinicians should be aware of the associated maternal and fetal complications. Expecting parents should be counseled by the multidisciplinary team about the management and prognosis of the STC twin and co-twin. Prompt surgical excision of the sacrococcygeal teratoma after birth should be suggested.

Abstract:Signcryption is a cryptographic primitive that fulfills both the functions of digital signature and public key encryption simultaneously in low cost compare with the traditional signature-then-encryption approach. In this paper, we propose a new method for providing more secure key generation against the outer and insider attack, which is based on the mouse movements. Tag KEM process is implemented using this True random number generator (TRNG) and it generates most secure and nondeterministic secret keys for data encryption which held in the Data encapsulation mechanism (DEM). We compare the statistical reports of the proposed system with the previous methods which is used to implements TKEM based on pseudo random number generators, and it works better in manner of security.

Abstract:Sacrococcygeal teratoma is the most common extragonadal germ cell tumor in the pediatric population, and accounts for approximately 70% of all teratomas in childhood.1,2 They present in two distinct phases, with most cases seen in neonates with large predominately exophytic tumors, often detected in utero on prenatal sonography or at birth. A smaller cohort presents in older infants and children with primarily hidden tumors in the pelvis which have a much higher rate of malignancy. The primary surgical objective is complete tumor resection without compromise to critical structures or function. Herein we outline the critical elements of tumor resection and management of sacrococcygeal germ cell tumors with a focus on the technical aspects of this tumor across a range of presentations.

Abstract:Extragonadal germ cell tumors (GCTs) are rare, accounting for 1% to 5% of all GCTs, with malignant sacrococcygeal yolk sac tumors (SCYSTs) being part of this uncommon subset. These tumors primarily occur in children and young adults and are known for their aggressive behavior, necessitating early diagnosis and prompt treatment. In this case report, we present the case of an 8-month-old female who presented with mild abdominal distension and a 3-week history of difficulty passing urine. Imaging revealed a pelvic mass originating from the coccyx, compressing the urinary bladder. The mass was surgically excised, and histopathological analysis confirmed a malignant yolk sac tumor with positive margins. The patient received adjuvant chemotherapy postsurgery. Despite treatment, follow-up scans showed residual tumor tissue, leading to a second tumor resection. Surveillance was continued using Alpha Fetoprotein (AFP) levels as a marker, which revealed a relapse of the tumor. Subsequent imaging confirmed a growing tumor, and the patient was scheduled for second-line chemotherapy. Management of SCYSTs is complex and requires a multidisciplinary approach, including surgeons, oncologists, radiologists, and pathologists. While relapse is rare, it poses a significant challenge for treatment, underscoring the need for clear and updated guidelines for managing relapsed cases.
Diagnosis and management of a sacrococcygeal teratoma at a tertiary hospital in northern Tanzania: A case report

Abstract:Introduction and importance: Sacrococcygeal teratoma is a rare extragonadal germ cell tumor occurring at the lower end of the back. It is congenital, and can be diagnosed in utero or just after birth. These tumors are sporadic in nature, more common in females than in males. They can be solid, cystic or a mixture of the two in consistency. Authors present this case to share their experience because these congenital tumors are rare and a lapse in appropriate management can be devastating because of chances of recurrence and the possibility of a malignant nature of the disease. Case presentation: Authors in this case present a baby that was referred to their center 5 days post vaginal delivery with a huge mass on the sacrococcygeal region. MRI revealed sacrococcygeal teratoma type II. Wide local excision was done. Tumor margins were microscopically negative. The patient recovered well and was discharged. The patient did not require adjuvant treatment. Clinical discussion: Routine obstetric ultrasound can diagnose sacrococcygeal teratoma with 100 % sensitivity. Fetuses with tumors <5 cm in size can be delivered vaginally. Tumors larger than that should be delivered at term through cesarean section. High risk sacrococcygeal teratomas can be delivered at 28 weeks of gestation by cesarean section followed by ex-utero intrapartum therapy. MRI is useful to assess the intrapelvic extent of the tumor and to plan management. Surgical intervention should be done after delivery. Conclusion: Intrauterine diagnosis is not an indication for termination of pregnancy. Surgical resection with negative margins provides a cure. Histological examination of the tumor is mandatory. For malignant sacrococcygeal teratomas, adjuvant chemotherapy is necessary for positive tumor margin after surgical resection. Positive margins may require chemotherapy. Neoadjuvant chemotherapy may be used for tumor debulking prior to surgery if the tumor infiltrates neighboring structures or is metastatic.
Clinical feature and surgical management of pediatric sacral tumors

Abstract:Objective Sacral tumors in children are very rare.The purpose of this research is to investigate and discuss the clinical characteristics, pathological types, therapeutic process and prognosis of such lesions.Methods From Jan.2000 to Dec.2013, the study retrospected 20 patients (≤14 years) who were diagnosed as sacral tumors.Each patient's hospital records, radiographic reports and pathological results were reviewed.The pathological types of these lesions were Ewing sarcoma/primitive neuroectodermal tumor (PNET, n=7), osteosarcoma (n=3), osteoblastoma (n=2), hemangioma (n=2), teratoma (n=1), chondroblastoma (n=1), schwannoma (n=1), aneurysmal bone cyst (n=1), eosinophilic granuloma (n=1) and hemangioendothelioma (n=1).Seven patients had sacral nerve impairment as initial symptoms and the rest 13 complained with sacrococcygeal pain.Eleven patients presented with presacral mass during rectal examination.All 20 patients received surgical treatment.The survival rate was described with Kaplan-Meier survival curve.Results Thirteen patients with sacrococcygeal pain had their symptoms all relieved.Three of the 7 patients with sacral nerve impairment experienced improvement of the symptoms while 3 patients found no much change.One patient found the symptom deteriorated and suffered ambulation dysfunction postoperatively.The mean 2-year survival rate was 70.6％, 5-year survival rate 64.7％.Conclusions Sacral tumors in children are very rare, and most of them are primary tumors like Ewing sarcoma/PNET or osteosarcoma, which are usually highly malignant.Once a child is suspected with sacral tumors, prompt radiological and pathological examinations should be performed and therapeutic strategy should be made in order to improve the survival and preserve the sacral nerve function as much as possible.
Analysis of the Prognosis of Sacrococcygeal Teratoma

Abstract:101 cases of sacrococcygeal teratoma were reviewed.Of them,25.8% were malignant and 74.3% were followed up.The pathology,operation age,clinical type and treatment were discussed.7.6% were malignant in patients aged younger than one year while 63.2% in those aged 1-2 years,and the incidence of malignancy remained unchange after 2 years of age.Recurrence occured in 12.3% of benign cases with incomplate removal and in 71% of malignant.Benign cases usually recurred within 2 years after operation,so the first follow up should not be later than this time.The average survival time was 8 months among the malignant cases and was 4 months among recurred cases.A complete removal of the tumor with the coccyx in the initial operation is the key to reduce the recurrence and malignant change.Surgery combined with chemotherapy and radiotherapy is suggested to improve the prognosis.
Sacrococcygeal teratomas: clinico-pathological characteristics and isochromosome 12p status

Abstract:The biological behavior of teratomas is highly variable, and morphologic features alone are insufficient to predict their clinical course. Prognostic factors that influence behavior include the following: patient sex, age, anatomic site, coincident neoplasm, and cytogenetic abnormalities. Gonadal teratomas have been well-characterized; postpubertal testicular teratomas are commonly associated with isochromosome 12p (i12p) and considered to nearly always carry a potential for malignant behavior, whereas ovarian and prepubertal testicular teratomas are i12p negative and predominantly benign in behavior. For extragonadal sites, such as sacrum and coccyx, clinical characteristics and i12p status are yet to be adequately characterized. As part of this study, we identified 19 sacrococcygeal teratomas in our surgical pathology archives from 1990 to 2012. Clinical records and slides were reviewed to confirm the original diagnosis. Gains in chromosome 12p, including i12p status were assessed in representative paraffin sections by fluorescence in situ hybridization. Our cases included 16 mature sacrococcygeal teratomas (11 prepubertal and 5 postpubertal) and three immature saccrococygeal teratomas (all prepubertal). Among mature teratomas, the average tumor size was larger in adults compared with prepubertal patients. A higher number of adult cases were recurrences (80% vs 21%), but only pediatric recurrences were managed with postoperative chemotherapy. All examined tumors were negative for i12p. 100% survival was documented in our cohort with a median follow-up of 6 years. We present a large series of sacrococcygeal teratomas and the first series to examine postpubertal adults at this anatomic site. All tumors lacked chromosome 12p gains, including i12p. Both pre- and postpubertal sacrococcygeal teratomas had a favorable outcome regardless of age or sex.
The Effect of Implicit and Explicit Motivation on Recall among Old and Young Adults

Abstract:Seventy-six elderly subjects aged sixty-five to eighty-seven and seventy-seven young adults aged twenty-five to forty were compared on implicit and explicit motive levels and on recall of introductions and working memory. Significantly fewer of the elderly than the young participants scored high in the implicit motives, n Affiliation and n Power, confirming results from U.S. national surveys. The surveys also demonstrated a significant decline with age in high levels of n Achievement, a decline not found here. The elderly participants showed major recall deficits on both tasks, but all three of the implicit motives studied were shown to enhance recall for the elderly, but not for the young adults. Eight elderly women scoring high on at least two of the three motives showed no recall deficits compared to the young women on two memory tasks. In old age implicit motive deficits contribute to poor memory but explicit commitments to have a good memory had no effect on recall.

Abstract:Sacrococcygeal teratoma is a congenital tumor of early infancy. Eighty percent are present in the first month of life. It is considered to be an extragonadal germ cell tumor that can be diagnosed in utero. It is considered to be a rare tumor in infancy and childhood. Incidences in girls are more common as compared to boys. Association with other congenital abnormalities of the gastrointestinal system, cardiovascular system, and urological manifestations may be present. Association with spinal dysraphism may be present. A complete systemic evaluation needs to be done before any surgical intervention. We hereby present you a case of a 5-day-old male child, born from a non-consanguineous marriage, who presented to us with a large swelling at the coccygeal region. The mass had a variegated consistency with an ear-like appendage over it with associated hair, bones, and necrotic tissue in it. The patient was evaluated with imaging (MRI) which revealed a heterogeneously enhancing mass at the sacrococcygeal region with mild extension into the pre-sacral space suggesting sacrococcygeal teratoma. After a complete evaluation for congenital abnormal abnormalities of other sites, gross total excision of the mass was performed. The post-operative condition of the baby was uneventful. A biopsy of the mass revealed a mature cystic teratoma. The baby is under follow-up at present.

Propofol Attenuates the Decrease of Dynamic Compliance and Water Content in the Lung by Decreasing Oxidative Radicals Released from the Reperfused Liver

Abstract:BACKGROUND: Remote pulmonary injuries after hepatic reperfusion are frequently caused by reactive oxygen species (ROS)-induced damage. The choice of anesthetics may affect the balance between oxidants and antioxidants, and propofol, a commonly used anesthetic, has an antioxidant effect. In this study, we developed a model to study pulmonary function with hepatic ischemia/reperfusion (I/R) manipulation, with the aim of defining remote pulmonary dysfunction after hepatic reperfusion and determining if propofol affects this dysfunction by altering ROS production from the liver or lungs. METHODS: Adult male rats weighing 160–250 g were randomly divided into four groups according to the type of surgery (sham or I/R) and the anesthetic administered (pentobarbital or propofol). To induce I/R, the portal vein and hepatic artery to the left and medial lobes of the liver were clamped. All of the measurements were done after 5 h of reperfusion, after 45 min of ischemia. Pulmonary function after hepatic I/R was determined by dynamic compliance, resistance and wet-to-dry ratio, and by histopathology. Hepato-cellular injuries were confirmed by alanine aminotransferase, whereas ROS production was measured from the inferior vena cava, jugular vein, and carotid artery. Products of lipid peroxidation, thiobarbiturate acid reactive substances and malondialdehyde, were measured in lung and hepatic tissues. RESULTS: Remote lung injury after hepatic I/R was shown by a significant decrease of Cdyn, and increases in resistance and the wet-to-dry ratio. ROS production was significantly increased and was highest in samples from the inferior vena cava. Thiobarbiturate acid reactive substances and malondialdehyde in the liver and serum alanine aminotransferase were significantly increased only in the I/R+pentobarbital group. All of the changes were significantly attenuated in the I/R+ propofol group (P = 0.05). With propofol infusion, there was decreased ROS production from the reperfused liver, with less hepato-cellular injury, followed by well-maintained pulmonary function. CONCLUSION: Remote pulmonary dysfunction and reperfusion injury in the liver were demonstrated in our rat model, as well as massive ROS production and lipid peroxidation. Propofol infusion attenuated remote pulmonary injury by lessening oxidative injury from the reperfused liver.
The Irish Experience with Sacrococcygeal Teratomas: Are Type IV Lesions More Common than We Think?

Abstract:Introduction Sacrococcygeal teratomas (SCTs) are rare tumors occurring in approximately 1 in 35,000 to 40,000 live births. The Altman classification is used to describe SCTs. There are four types, with type 1 predominantly external through to type IV which is a presacral, completely internal mass. As far as the authors are aware, this is the first study to focus on type IV SCT lesions. Materials and Methods Using ICD-10- AM (The International Statistical Classification of Diseases and Related Health Problems, Tenth Revision, Australian Modification) codes, we identified all patients in the Republic of Ireland with a diagnosis of SCT from 2004 to 2020. The following information was obtained for each patient: gender, time of diagnosis, clinical presentation, method of diagnosis, Altman classification, biomarkers, age at operation, surgical technique, pathology, recurrence, and age at most recent follow-up. Results There were 29 patients in total; 23 females (79%) and 6 males (21%). In total, 16 (55%) were diagnosed antenatally, 4 (14%) at less than 1 month, 4 (14%) less than 1 year, 3 (10%) age 2 to 4 years, and 2 (7%) were aged 5 to 12 years. In addition, 22 (76%) were mature teratomas, 2 (7%) immature teratomas, and 5 (17%) were malignant tumors. There were 6 (21%) type I lesions, 9 (30%) type II, 6 (21%) type III, and 8 (28%) type IV lesions. Conclusion In Altman's original 1974 study, type IV lesions were present in 10% of cases. As a result, this is the most frequently quoted figure. Type IV lesions were present in 28% of cases in our study. We propose that type IV lesions may be more common than the current literature suggests and consequently a higher index of suspicion of their presence should be entertained. Advances in the Prognostic Predication of Fetal Sacrococcygeal Teratoma

Abstract:Sacrococcygeal teratoma (SCT) is a common fetal and neonatal tumor in live births with an incidence of 1/40,000 to 1/35,000.And it has a male-to-female ratio of 1∶3 to 1∶4.The prognosis of SCT is generally fair after birth.However,the mortality of fetal SCT was reported at 16％to 63 ％ in the literature.Thus its prenatal diagnosis and prognostic indicators have become a hot focus of current researches.Here recent advances in its prognostic predication are reviewed.Various indicators such as tumor cystic proportion,tumor-fetal ratio (TFR),solid tumor volume divided by head volume (STV∶ HV),solid tumor volume index (STVI),tumor growth and cardiovascular profile score (CVPS) are discussed.In clinical practices,tumor cystic proportion and TFR are recommended.Or either STV∶ HV or STVI may be used.Tumor growth should be always monitored.CVPS may be monitored be to assess the extent of fetal heart failure to determine the timing of intervention.If high output heart failure occurs,fetal surgery should be performed in the first 28 weeks of pregnancy and therapeutic delivery should be considered after 28 weeks of pregnancy.Currently the prognosis of SCT fetus is still in an exploratory stage.Due to imperfect prenatal policies,more in-depth researches are warranted.
