Abstract:We present an analysis of spatial and temporal spectra of the observations of scintillations in a laser beam (532 nm, ∼200 mW power) traveling along a 144 km path at an altitude of 2-2.4 km above sea level, just above the atmospheric boundary layer, between the islands of La Palma and Tenerife. The observations were performed during nighttime on July 18 and 21, 2011, by means of a telescope with an aperture diameter of 1 m. Strong scintillations were observed. We compared the temporal and spatial spectra of the scintillations. For the temporal spectra, we performed the correction for the aliasing effect due to the low frame rate. The 2D spatial spectra of the scintillations in the observation plane were found to be close to isotropic. This allowed for transforming them into 1D spectra. We found a good agreement between the temporal and 1D spatial spectra. This corroborates the applicability of the Taylor hypothesis of frozen turbulence for finite-size laser beams and strong scintillations.
Comparison of Particulate Air Pollution From Different Emission Sources and Incident Dementia in the US

Abstract:Importance: Emerging evidence indicates that exposure to fine particulate matter (PM2.5) air pollution may increase dementia risk in older adults. Although this evidence suggests opportunities for intervention, little is known about the relative importance of PM2.5 from different emission sources. Objective: To examine associations of long-term exposure of total and source-specific PM2.5 with incident dementia in older adults. Design, setting, and participants: The Environmental Predictors of Cognitive Health and Aging study used biennial survey data from January 1, 1998, to December 31, 2016, for participants in the Health and Retirement Study, which is a nationally representative, population-based cohort study in the US. The present cohort study included all participants older than 50 years who were without dementia at baseline and had available exposure, outcome, and demographic data between 1998 and 2016 (N = 27 857). Analyses were performed from January 31 to May 1, 2022. Exposures: The 10-year mean total PM2.5 and PM2.5 from 9 emission sources at participant residences for each month during follow-up using spatiotemporal and chemical transport models. Main outcomes and measures: The main outcome was incident dementia as classified by a validated algorithm incorporating respondent-based cognitive testing and proxy respondent reports. Adjusted hazard ratios (HRs) were estimated for incident dementia per IQR of residential PM2.5 concentrations using time-varying, weighted Cox proportional hazards regression models with adjustment for the individual- and area-level risk factors. Results: Among 27 857 participants (mean [SD] age, 61 [10] years; 15 747 [56.5%] female), 4105 (15%) developed dementia during a mean (SD) follow-up of 10.2 [5.6] years. Higher concentrations of total PM2.5 were associated with greater rates of incident dementia (HR, 1.08 per IQR; 95% CI, 1.01-1.17). In single pollutant models, PM2.5 from all sources, except dust, were associated with increased rates of dementia, with the strongest associations for agriculture, traffic, coal combustion, and wildfires. After control for PM2.5 from all other sources and copollutants, only PM2.5 from agriculture (HR, 1.13; 95% CI, 1.01-1.27) and wildfires (HR, 1.05; 95% CI, 1.02-1.08) were robustly associated with greater rates of dementia. Conclusion and relevance: In this cohort study, higher residential PM2.5 levels, especially from agriculture and wildfires, were associated with higher rates of incident dementia, providing further evidence supporting PM2.5 reduction as a population-based approach to promote healthy cognitive aging. These findings also indicate that intervening on key emission sources might have value, although more research is needed to confirm these findings.

Abstract:Background: The associations of PM2.5 mass and various adverse health outcomes have been widely investigated. However, fewer studies focused on the potential health impacts of PM2.5 components, especially for dementia and Alzheimer's diseases (AD). Methods: We constructed a nationwide population-based open cohort study among Medicare beneficiaries aged 65 or older during 2000-2018. This dataset was linked with the predicted levels of 15 PM2.5 components, including 5 major mass contributors (EC, OC, NH4+, NO3-, SO42-) and 10 trace elements (Br, Ca, Cu, Fe, K, Ni, Pb, Si, V, Zn) across contiguous US territory. Data were aggregated by ZIP code, calendar year and individual level demographics. Two mixture analysis methods, weighted quantile sum regression (WQS) and quantile g-computation (qgcomp), were used with quasi-Poisson models to analyze the health effects of the total mixture of PM2.5 components on dementia and AD, as well as the relative contribution of individual components. Results: Exposure to PM2.5 components over the previous 5 years was significantly associated with increased risks of both dementia and AD, with stronger associations observed for AD. SO42-, OC, Cu were identified with large contributions to the combined positive association of the mixture from both WQS and qgcomp models. Conclusion: We found positive associations between the 15 PM2.5 components and the incidence of dementia and AD. Our findings suggest that reducing PM2.5 emissions from traffic and fossil fuel combustion could help mitigate the growing burden of dementia and Alzheimer's disease.
Association between ambient fine particular matter components and subsequent cognitive impairment in community-dwelling older people: a prospective cohort study from eastern China

Abstract:Background: Fine particular matter (PM2.5) has been associated with dementia, but limited information is available regarding the association between PM2.5 components and dementia. Aims: We aimed to identify the major components of PM2.5 that affect cognitive function to further investigate its mechanism of action, and develop a prevention strategy for dementia. Methods: In this study, we included 7804 participants aged ≥ 60 years recruited from seven counties in Zhejiang province, eastern China. The participants completed the baseline survey between 2014 and 2015, and were followed up until the end of 2020. We adopted single-component robust Poisson regression models for analyses, and estimated relative risks and 95% confidence intervals describing associations between the chemical constituents of PM2.5 exposure and incident cognitive impairment in those who were free from cognitive impairment at baseline. Results: Significantly positive associations were observed between sulfate, nitrate, ammonium, and organic matter in PM2.5 and incident cognitive impairment across different exposure periods; the relative risks of 10-year exposure before enrollment ranged from 1.01 to 1.02. However, we did not find a significant association between black carbon and cognitive impairment. The point estimates of the relative risk values did not change substantially after performing the sensitivity analyses. Conclusions: Our findings strengthen the idea that long-term exposure to PM2.5 mass and its chemical components is associated with an elevated risk of incident cognitive impairment among older adults.
Association between short-term exposure to ambient air pollution and dementia mortality in Chinese adults

Abstract:Background Short-term exposure to ambient air pollution has been linked to an increased risk of mortality from a variety of causes, but its effects on mortality from dementia remain largely unknown. Objectives To investigate the association between short-term exposure to ambient air pollution and dementia mortality, and quantitatively assess the excess mortality. Methods In this time-stratified case-crossover study, 47,108 dementia deaths were identified in Jiangsu province, China during 2015–2019. Exposure to particulate matter with an aerodynamic diameter ≤ 2.5 μm (PM 2.5 ), PM 10 , sulfur dioxide (SO 2 ), nitrogen dioxide (NO 2 ), carbon monoxide (CO), and ozone (O 3 ) was assessed by extracting daily concentrations from a validated grid dataset based on each subject's residential address. Conditional logistic regression models were applied for exposure-response analyses. Results There were 47,108 case days and 159,852 control days during the study period. Each 10 μg/m 3 increase of lag 04-day exposure to PM 2.5 , PM 10 , and NO 2 was significantly associated with a 1.43 % (95 % CI: 0.77, 2.09 %), 1.06 % (0.59, 1.54 %), and 2.80 % (1.51, 4.10 %) increase in odds of dementia mortality, corresponding to an excess mortality of 4.87 %, 5.50 %, and 6.43 %, respectively. We estimated that reducing ambient air pollutant exposures to the WHO air quality guidelines would avoid up to 4.17 % of the dementia deaths, while the ambient air quality standards in China would only help avoid up to 0.39 %. Conclusions This study provides consistent evidence that short-term exposure to PM 2.5 , PM 10 , and NO 2 is associated with increased odds of dementia mortality, which can be translated to a considerable excess mortality. Our findings highlight a potential approach to prevent deaths from dementia by reducing individual exposures to ambient air pollution, especially in areas with high levels of ambient air pollution.

Abstract:Background: Although growing evidence has shown independent links of long-term exposure to fine particulate matter (PM2.5) with cognitive impairment, the effects of its constituents remain unclear. This study aims to explore the associations of long-term exposure to ambient PM2.5 constituents' mixture with cognitive impairment in Chinese older adults, and to further identify the main contributor. Methods: 15,274 adults >= 65 years old were recruited by the Chinese Longitudinal Healthy Longevity Study (CLHLS) and followed up through 7 waves during 2000-2018. Concentrations of ambient PM2.5 and its constituents (i.e., black carbon [BC], organic matter [OM], ammonium [NH4+], sulfate [SO42-], and nitrate [NO3-]) were estimated by satellite retrievals and machine learning models. Quantile-based g -computation model was employed to assess the joint effects of a mixture of 5 PM2.5 constituents and their relative contributions to cognitive impairment. Analyses stratified by age group, sex, residence (urban vs. rural), and region (north vs. south) were performed to identify vulnerable populations. Results: During the average 3.03 follow-up visits (89,296.9 person -years), 4294 (28.1%) participants had developed cognitive impairment. The adjusted hazard ratio [HR] (95% confidence interval [CI]) for cognitive impairment for every quartile increase in mixture exposure to 5 PM2.5 constituents was 1.08 (1.05-1.11). BC held the largest index weight (0.69) in the positive direction in the qg-computation model, followed by OM (0.31). Subgroup analyses suggested stronger associations in younger old adults and rural residents. Conclusion: Long-term exposure to ambient PM2.5, particularly its constituents BC and OM, is associated with an elevated risk of cognitive impairment onset among Chinese older adults.

Abstract:Background: Exposure to fine particulate matter air pollution (PM2.5) increases risk for dementia. However, it is unknown whether this relationship is mediated by dementia-related neuropathologic change found at autopsy. We aimed to examine relationships between PM2.5 exposure, dementia severity, and dementia-associated neuropathologic change. Methods: This cross-sectional study used harmonized demographic, clinical, genetic, and neuropathological data from autopsy cases collected from 1998 to 2022 at the Center for Neurodegenerative Disease Research brain bank, University of Pennsylvania. Cases who had common neuropathologic forms of dementia and complete data on neuropathologic measures, APOE genotype, and residential address were included in this study cohort. Dementia severity was measured by Clinical Dementia Rating-Sum of Boxes (CDR-SB) scores. Ten dementia-associated neuropathologic measures representing Alzheimer's disease, Lewy body disease, limbic-predominant age related TDP-encephalopathy, and cerebrovascular disease were graded or staged according to the consensus criteria. One-year average PM2.5 exposure prior to death was estimated using a spatiotemporal prediction model based on residential addresses as the primary exposure measure. Linear, logistic and structural equation models were used to examine the relationships between PM2.5, CDR-SB and neuropathologic measures. Results: A total of 861 autopsy cases were included (mean age at death 76.6 years [SD 10.3]; 481 [56%] male). Each 1µg/m3 increase in one-year average PM2.5 concentration prior to death was associated with significantly greater cognitive and functional impairment (increase in CDR-SB score of 0.78; 95% confidence interval [CI], 0.52-1.05), faster cognitive and functional decline (change in CDR-SB scores of 0.13; 95% CI, 0.09-0.16), more severe Alzheimer's disease neuropathologic change (ADNC; odds ratio [OR] of 1.07; 95% CI, 1.01-1.13), and a higher prevalence of large infarcts (OR, 1.17; 95% CI, 1.05-1.30). The relationship between PM2.5 exposure and CDR-SB was mediated by ADNC (change in CDR-SB score due to ADNC level of 0.36; 95% CI, 0.13-0.65). Conclusions: PM2.5 exposure may increase dementia risk by increasing ADNC. Measures that improve air quality may represent a population-level intervention for the prevention of dementia.

Abstract:Background: Exposure to PM 2.5 has been linked to neurodegenerative diseases, with limited understanding of constituent -specific contributions. Objectives: To explore the associations between long-term exposure to PM 2.5 constituents and neurodegenerative diseases. Methods: We recruited 148,274 individuals aged >= 60 from four cities in the Pearl River Delta region, China (2020 to 2021). We calculated twenty-year average air pollutant concentrations (PM 2.5 mass, black carbon (BC), organic matter (OM), ammonium (NH 4 + ), nitrate (NO 3 - ) and sulfate (SO 4 2- )) at the individuals' home addresses. Neurodegenerative diseases were determined by self -reported doctor -diagnosed Alzheimer's disease (AD) and Parkinson's disease (PD). Generalized linear mixed models were employed to explore associations between pollutants and neurodegenerative disease prevalence. Results: PM 2.5 and all five constituents were significantly associated with a higher prevalence of AD and PD. The observed associations generally exhibited a non-linear pattern. For example, compared with the lowest quartile, higher quartiles of BC were associated with greater odds for AD prevalence (i.e., the adjusted odds ratios were 1.81; 95% CI, 1.45-2.27; 1.78; 95% CI, 1.37-2.32; and 1.99; 95% CI, 1.54-2.57 for the second, third, and fourth quartiles, respectively). Conclusions: Long-term exposure to PM 2.5 and its constituents, particularly combustion -related BC, OM, and SO 4 2- , was significantly associated with higher prevalence of AD and PD in Chinese individuals. Environmental implication: PM 2.5 is a routinely regulated mixture of multiple hazardous constituents that can lead to diverse adverse health outcomes. However, current evidence on the specific contributions of PM 2.5 constituents to health effects is scarce. This study firstly investigated the association between PM 2.5 constituents and neurodegenerative diseases in the moderately to highly polluted Pearl River Delta region in China, and identified hazardous constituents within PM 2.5 that have significant impacts. This study provides important implications for the development of targeted PM 2.5 prevention and control policies to reduce specific hazardous PM 2.5 constituents.
Long-term Effects of PM2·5 on Neurological Disorders in the American Medicare Population: a Longitudinal Cohort Study.

Abstract:Background Accumulating evidence links fine particulate matter (PM2.5) to premature mortality, cardiovascular disease, and respiratory disease. However, less is known about the influence of PM2.5 on neurological disorders. We aimed to investigate the effect of long-term PM2.5 exposure on development of Parkinson's disease or Alzheimer's disease and related dementias. Methods We did a longitudinal cohort study in which we constructed a population-based nationwide open cohort including all fee-for-service Medicare beneficiaries (aged >= 65 years) in the contiguous United States (2000-16) with no exclusions. We assigned PM2.5 postal code (ie, ZIP code) concentrations based on mean annual predictions from a high-resolution model. To accommodate our very large dataset, we applied Cox-equivalent Poisson models with parallel computing to estimate hazard ratios (HRs) for first hospital admission for Parkinson's disease or Alzheimer's disease and related dementias, adjusting for potential confounders in the health models. Findings Between Jan 1, 2000, and Dec 31, 2016, of 63 038 019 individuals who were aged 65 years or older during the study period, we identified 1.0 million cases of Parkinson's disease and 3.4 million cases of Alzheimer's disease and related dementias based on primary and secondary diagnosis billing codes. For each 5 mu g/m(3) increase in annual PM2.5 concentrations, the HR was 1.13 (95% CI 1.12-1.14) for first hospital admission for Parkinson's disease and 1.13 (1.12-1.14) for first hospital admission for Alzheimer's disease and related dementias. For both outcomes, there was strong evidence of linearity at PM2.5 concentrations less than 16 mu g/m(3) (95th percentile of the PM2.5 distribution), followed by a plateaued association with increasingly larger confidence bands. Interpretation We provide evidence that exposure to annual mean PM2.5 in the USA is significantly associated with an increased hazard of first hospital admission with Parkinson's disease and Alzheimer's disease and related dementias. For the ageing American population, improving air quality to reduce PM2.5 concentrations to less than current national standards could yield substantial health benefits by reducing the burden of neurological disorders. Association of air pollution with dementia: a systematic review with meta-analysis including new cohort data from China

Abstract:It remains unclear whether a total exposure to air pollution (AP) is associated with an increased risk of dementia. Little is known on the association in low- and middle-income countries. Two cohort studies in China (in Anhui cohort 1402 older adults aged ≥ 60 followed up for 10 years; in Zhejiang cohort 6115 older adults followed up for 5 years) were conducted to examine particulate matter - PM2.5 associated with all dementia and air quality index (AQI) with Alzheimer's disease, respectively. A systematic literature review and meta-analysis was performed following worldwide literature searched until May 20, 2020 to identify 15 population-based cohort studies examining the association of AP with dementia (or any specific type of dementia) through PubMed, MEDLINE, PsycINFO, SocINDEX, CINHAL, and CNKI. The cohort studies in China showed a significantly increased relative risk (RR) of dementia in relation to AP exposure; in Anhui cohort the adjusted RR was 2.14 (95% CI 1.00-4.56) in people with PM2.5 exposure at ≥ 64.5 μg/m3 versus <63.5 μg/m3 and in Zhejiang cohort the adjusted RR was 2.28 (1.07-4.87) in AQI>90 versus ≤ 80. The systematic review revealed that all 15 studies were undertaken in high income countries/regions, with inconsistent findings. While they had reasonably good overall quality of studies, seven studies did not adjust smoking in analysis and 13 did not account for depression. Pooling all eligible data demonstrated that dementia risk increased with the total AP exposure (1.13, 1.08-1.19). Data analysis of air pollutants showed that the RR significantly increased with PM2.5 (1.06, 1.03-1.10 in 2nd tertile exposure; 1.13, 1.07-1.19 in 3rd tertile versus 1st tertile), PM10 (1.05, 0.86-1.29; 1.62, 0.60-4.36), carbon monoxide (1.69, 0.72-3.93; 1.52, 1.35-1.71), nitrogen dioxide (1.06, 1.03-1.09; 1.18, 1.10-1.28) and nitrogen oxides (1.09, 1.04-1.15; 1.26, 1.13-1.41), but not ozone. Controlling air pollution and targeting on specific pollutants would reduce dementia globally.
Associations of Particulate Matter with Dementia and Mild Cognitive Impairment in China: A Multicenter Cross-Sectional Study

Abstract:Ambient air pollution has been shown to be associated with the pathogenesis of dementia and mild cognitive impairment (MCI). However, few studies have examined these associations in well-characterized populations with low residential mobility, similar living habits, and a standardized assessment of both air pollution exposure and clinical outcome. This study examined the associations of long-term exposure to particulate matter (PM) air pollution with dementia and MCI, using data from the Chinese Veteran Clinical Research Platform. The cognitive function of elderly veterans from 277 communities in 18 Chinese cities was examined. Participants' daily exposures to aerodynamic diameters <= 2.5 mm(PM2.5) and <= 10 mm(PM10) during the 3 years prior to the survey were estimated using a satellite-based prediction. The adjusted odds ratios (ORs) and 95% confidence intervals of MCI associated with each 10 mu g/m(3) increase in PM2.5 and PM10 were 1.52 (1.39, 1.67) and 1.04 (1.00, 1.08), and those of dementia associated with PM2.5 and PM10 were 1.27 (1.11, 1.46) and 1.13 (1.05, 1.21), respectively. This demonstrates that long-term exposure to PM2.5 and PM10 can increase the prevalence of dementia/MCI among veterans in China. Higher ORs were observed for those with <= 9 years of educational attainment, those who actively attended physical activities, those who never smoked, former drinkers, and those who did not suffer from cerebral infarction. Improvement of ambient air quality, especially decreasing levels of PM2.5, may help to decrease the risk of dementia/MCI. Given the statistically significant association between PM and cognitive impairment demonstrated here, future studies should focus on examining the causal effect of PM pollution on dementia and MCI.

Abstract:BACKGROUND: Air pollution induces neurotoxic reactions and may exert adverse effects on cognitive health. We aimed to investigate whether air pollutants accelerate cognitive decline and affect neurobiological signatures of Alzheimer's disease (AD).METHODS: We used a population-based cohort from the Chinese Longitudinal Healthy Longevity Survey with 31,573 participants and a 10-year follow-up (5878 cognitively unimpaired individuals in Chinese Longitudinal Healthy Longevity Survey followed for 5.95 6 2.87 years), and biomarker-based data from the Chinese Alzheimer's Biomarker and Lifestyle study including 1131 participants who underwent cerebrospinal fluid measurements of AD-related amyloid-l3 (Al3) and tau proteins. Cognitive impairment was determined by education-corrected performance on the China-Modified Mini-Mental State Examination. Annual exposures to fine particulate matter (PM2.5), ground-level ozone (O3), and nitrogen dioxide (NO2) were estimated at areas of residence. Exposures were aggregated as 2-year averages preceding enrollments using Cox proportional hazards or linear models.RESULTS: Long-term exposure to PM2.5 (per 20 mg/m3) increased the risk of cognitive impairment (hazard ratio, 1.100; 95% CI: 1.026-1.180), and similar associations were observed from separate cross-sectional analyses. Exposures to O3 and NO2 yielded elevated risk but with nonsignificant estimates. Individuals exposed to high PM2.5 manifested increased amyloid burdens as reflected by cerebrospinal fluid-AD biomarkers. Moreover, PM2.5 exposure-associated decline in global cognition was partly explained by amyloid pathology as measured by cerebrospinal fluid-Al342/Al340, P-tau/Al342, and T-tau/Al342, with mediation proportions ranging from 16.95% to 21.64%.CONCLUSIONS: Long-term exposure to PM2.5 contributed to the development of cognitive decline, which may be partly explained by brain amyloid accumulation indicative of increased AD risk.

Abstract:Background: While emerging studies have examined long-term exposure to fine particulate matter (PM₂.₅) and its association with various measures of morbidity of dementia and Alzheimer´s disease (AD), evidence on short-term PM₂.₅ exposure and mortality caused by dementia or AD is sparse. Methods: We applied a time-stratified case-crossover design using conditional logistic regression to assess the association between ambient PM₂.₅ concentrations and daily all-cause dementia and AD mortality in Connecticut between 2005 to 2016, while adjusting for mean air and dewpoint temperature. Our analysis included cases whose principal cause of death was all-cause dementia or AD. The daily 24-hour average PM₂.₅ concentration was obtained from a well-validated model and the individual-level exposure was assigned based on the modeled PM₂.₅ level in a 1 × 1 km² grid which contains the residential address. We also explored potential effect modification by age and sex groups. Results: A total of 28,736 all-cause dementia deaths occurred during our study period, among which 9,826 were specially coded as AD deaths. A 10 μg/m³ increase in the 2-day moving average of ambient PM₂.₅ was significantly associated with all-cause dementia (Odds Ratio [OR] =1.06, 95% confidence interval [CI]: 1.03, 1.09) and AD (OR=1.07, 95% CI: 1.02, 1.12). We did not find effect modification by age (<80 years vs. ≥ 80 years) in all-cause dementia or AD. The estimated effect of PM₂.₅ was higher among the female subgroup compared to the male subgroup (P-value = 0.03) for mortality caused by all-cause dementia but not for AD. Conclusions: Our study suggests a significant association between short-term exposure to PM₂.₅ and an increase in mortality risks of all-cause dementia and AD. Further research is warranted on mechanisms that can explain the relationship between short-term PM₂.₅ exposure and mortality risks of dementia. Abstract:Air pollution exposure has been linked to various diseases, including dementia. However, a novel method for investigating the associations between air pollution exposure and disease is lacking. The objective of this study was to investigate whether long-term exposure to ambient particulate air pollution increases dementia risk using both the traditional Cox model approach and a novel machine learning (ML) with random forest (RF) method. We used health data from a national population-based cohort in Taiwan from 2000 to 2017. We collected the following ambient air pollution data from the Taiwan Environmental Protection Administration (EPA): fine particulate matter (PM 2.5 ) and gaseous pollutants, including sulfur dioxide (SO 2 ), carbon monoxide (CO), ozone (O 3 ), nitrogen oxide (NO x ), nitric oxide (NO), and nitrogen dioxide (NO 2 ). Spatiotemporal-estimated air quality data calculated based on a geostatistical approach, namely, the Bayesian maximum entropy method, were collected. Each subject's residential county and township were reviewed monthly and linked to air quality data based on the corresponding township and month of the year for each subject. The Cox model approach and the ML with RF method were used. Increasing the concentration of PM 2.5 by one interquartile range (IQR) increased the risk of dementia by approximately 5% (HR = 1.05 with 95% CI = 1.04–1.05). The comparison of the performance of the extended Cox model approach with the RF method showed that the prediction accuracy was approximately 0.7 by the RF method, but the AUC was lower than that of the Cox model approach. This national cohort study over an 18-year period provides supporting evidence that long-term particulate air pollution exposure is associated with increased dementia risk in Taiwan. The ML with RF method appears to be an acceptable approach for exploring associations between air pollutant exposure and disease.

Abstract:Importance Fine particulate matter air pollution (PM 2.5 ) has been consistently associated with cardiovascular disease, which, in turn, is associated with an increased risk of dementia. As such, vascular dysfunction might be a mechanism by which PM 2.5 mediates dementia risk, yet few prior epidemiological studies have examined this potential mechanism. Objective To investigate whether hypertension and stroke serve as mediators and modifiers of the association of PM 2.5 with incident dementia. Design, Setting, and Participants As part of the Environmental Predictors of Cognitive Health and Aging (EPOCH) Project, this cohort study used biennial survey data collected between 1998 and 2016 from respondents of the Health and Retirement Study (HRS), a nationally representative, population-based, cohort in the US. Eligible participants were those over 50 years of age who were free of dementia at baseline and had complete exposure, mediator, outcome, and demographic data from the HRS. Data analysis was conducted from August to November 2022. Exposures Exposure to PM 2.5 , calculated for the 10 years preceding each person's baseline examination according to residential histories and spatiotemporal models. Main Outcomes and Measures Incident dementia was identified using a validated algorithm based on cognitive testing and informant reports. The 4-way decomposition causal mediation analysis method was used to quantify the degree to which hypertension and stroke mediated or modified the association of PM 2.5 with incident dementia after adjustment for individual-level and area-level covariates. Results Among 27 857 participants (mean [SD] age at baseline, 61 [10] years; 15 747 female participants [56.5%]; 19 249 non-Hispanic White participants [69.1%]), 4105 (14.7%) developed dementia during the follow-up period (mean [SD], 10.2 [5.6] years). Among participants with dementia, 2204 (53.7%) had a history of hypertension at baseline and 386 (9.4%) received a diagnosis of hypertension during the follow up. A total of 378 participants (9.2%) had a history of stroke at baseline and 673 (16.4%) developed stroke over the follow-up period. The IQR of baseline PM 2.5 concentrations was 10.9 to 14.9 μg/m 3 . In fully adjusted models, higher levels of PM 2.5 (per IQR) were not associated with increased risk of incident dementia (HR, 1.04; 95% CI, 0.98 to 1.11). Although there were positive associations of prevalent stroke (HR, 1.67; 95% CI, 1.48 to 1.88) and hypertension (HR, 1.15; 95% CI, 1.08 to 1.23) with incident dementia compared with those free of stroke and hypertension during follow-up, there was no statistically significant association of PM 2.5 with stroke (odds ratio per IQR increment in PM 2.5 , 1.08; 95%CI, 0.91 to 1.29) and no evidence of an association of PM 2.5 with hypertension (odds ratio per IQR increment in PM2.5, 0.99; 95%CI, 0.92 to 1.07). Concordantly, there was no evidence that hypertension or stroke acted as mediators or modifiers of the association of PM 2.5 with incident dementia. Although the nonmediated interaction between PM 2.5 and hypertension accounted for 39.2% of the total excess association (95% CI, −138.5% to 216.9%), the findings were not statistically significant. Conclusions and Relevance These findings suggest that although hypertension may enhance the susceptibility of individuals to air pollution, hypertension and stroke do not significantly mediate or modify the association of PM 2.5 with dementia, indicating the need to investigate other pathways and potential mediators of risk.

Abstract:We investigated the association of long-term exposure to atmospheric PM2.5 with non-accidental and cause-specific mortality in Yinzhou, China. From July 2015 to January 2018, a total of 29,564 individuals aged ≥ 40 years in Yinzhou were recruited for a prospective cohort study. We used the Cox proportional-hazards model to analyze the relationship of the 2-year average concentration of PM2.5 prior to the baseline with non-accidental and cause-specific mortality. The median PM2.5 concentration was 36.51 μg/m3 (range: 25.57–45.40 μg/m3). In model 4, the hazard ratios per 10 μg/m3 increment in PM2.5 were 1.25 (95%CI: 1.04–1.50) for non-accidental mortality and 1.38 (95%CI:1.02–1.86) for cardiovascular disease mortality. We observed no associations between PM2.5 and deaths from respiratory disease or cancer. In the subgroup analysis, interactions were observed between PM2.5 and age, as well as preventive measures on hazy days. The observed association between long-term exposure to atmospheric PM2.5 at a relatively moderate concentration and the risk of non-accidental and cardiovascular disease mortality among middle-aged and elderly Chinese adults could provide evidence for government decision-makers to revise environmental policies towards a more stringent standard.

Abstract:B ACKGROUND : Research on the relationship between long-term exposure to particulate matter with aerodynamic diameter 2 . 5 lm (PM 2.5 ) and poor cognitive function is lacking in developing countries, especially in highly polluted areas. O BJECTIVES : We evaluated associations of long-term exposure to PM 2.5 with poor cognitive function in a diverse, national sample of older adults in China. M ETHODS : This analysis included data on 13,324 older adults (5,879 who were 65 - 79 years of age, 3,052 who were 80 - 89 years of age, 2,634 who were 90 - 99 years of age, and 1,759 who were 41 . 4 lg / m 3 ), adjusted HR values were 1.20 (95% CI: 1.09, 1.33), 1.27 (95% CI: 1.15, 1.41), and 1.21 (95% CI: 1.09, 1.34) for the second ( 0 . 001). Subgroup analyses suggested stronger associations between PM 2.5 and poor cog- nitive impairment in men than women. The association was positive in the 65- to 79- and 100-y age group but not signi fi cant and positive in the other two age groups with similar results. C ONCLUSION : PM 2.5 was identi fi ed as a risk factor for poor cognitive function in Chinese older adults. Abstract:Background: As a prodromal stage of dementia, significant emphasis has been placed on the identification of modifiable risks of mild cognitive impairment (MCI). Research has indicated a correlation between exposure to air pollution and cognitive function in older adults. However, few studies have examined such an association among the MCI population inChina. Objective: We aimed to explore the association between air pollution exposure and MCI risk from the Hubei Memory and Aging Cohort Study. Methods: We measured four pollutants from 2015 to 2018, 3 years before the cognitive assessment of the participants. Logistic regression models were employed to calculate odds ratios (ORs) to assess the relationship between air pollutants and MCI risk. Results: Among 4,205 older participants, the adjusted ORs of MCI risk for the highest quartile of PM2.5, PM10, O3, and SO2 were 1.90 (1.39, 2.62), 1.77 (1.28, 2.47), 0.56 (0.42, 0.75), and 1.18 (0.87, 1.61) respectively, compared with the lowest quartile. Stratified analyses indicated that such associations were found in both males and females, but were more significant in older participants. Conclusions: Our findings are consistent with the growing evidence suggesting that air pollution increases the risk of mild cognitive decline, which has considerable guiding significance for early intervention of dementia in the older population. Further studies in other populations and broader geographical areas are warranted to validate these findings.

Abstract:Aims:Stroke is a leading cause of death and disability for Americans, and growing evidence suggests that air pollution may play an important role. To facilitate pollution control efforts, the National Academy of Sciences and the World Health Organization have prioritized determining which air pollutants are most toxic. However, evidence is limited for the simultaneous effects of multiple air pollutants on stroke.Methods and results:We constructed a nationwide population-based cohort study, using the Medicare Chronic Conditions Warehouse (2000-2017) and high-resolution air pollution data, to investigate the impact of long-term exposure to ambient PM2.5, NO2, and ground-level O3 on incident stroke. Hazard ratios (HR) for stroke incidence were estimated using single-, bi-, and tri-pollutant Cox proportional hazards models. We identified ~2.2 million incident stroke cases among 17,443,900 fee-for-service Medicare beneficiaries. Per interquartile range (IQR) increase in the annual average PM2.5 (3.7 μg/m3), NO2 (12.4 ppb), and warm-season O3 (6.5 ppb) one-year prior to diagnosis, the HRs were 1.022 (95% CI: 1.017-1.028), 1.060 (95% CI: 1.054-1.065), and 1.021 (95% CI: 1.017-1.024), respectively, from the tri-pollutant model. There was strong evidence of linearity in concentration-response relationships for all three air pollutants in single-pollutant models. This linear relationship remained robust for NO2 and O3 in tri-pollutant models while the effect of PM2.5 attenuated at the lower end of concentrations.Conclusion:Using a large nationwide cohort, our study suggests that long-term exposure to PM2.5, NO2, and O3 may independently increase the risk of stroke among the US elderly, among which traffic-related air pollution plays a particularly crucial role.
