Abstract:Background Adolescent pregnancy is defined as pregnancy before the age of 20 and is the leading cause of mortality in girls ages 15 to 19 worldwide. Adolescent pregnancies are associated with increased maternal, obstetric, and neonatal risks, however data is limited. Our objective was to compare the risk of adverse obstetrical outcomes in teenage pregnancies to adult pregnancies. Methods We conducted a population-based retrospective cohort study using a linked maternal-infant database of livebirths in Florida from 2000 to 2019. Inclusion criteria included singleton pregnancies with complete pregnancy and delivery information. Outcomes of interest included preterm birth (PTB), premature rupture of membranes (PROM), hypertensive disorders of pregnancy (HTNP), fetal growth restriction (FGR), cesarean births (CB), adequate prenatal care, and postpartum hemorrhage (PPH). Modified Poisson regression was used to calculate adjusted relative risks (aRR) and 95% confidence intervals (CI) representing the association between maternal age groups and adverse obstetrical outcomes, using ages 25-29 as the reference group. A p-value < 0.05 was considered statistically significant. Results Of the 3,616,879 deliveries included in the study, adolescent pregnancy (10-19 year olds) comprised 8.1% of deliveries. Demographic and clinical characteristics by maternal age are presented in Table 1. Teenage pregnancies had higher rates of PTB (10-14 year olds aRR: 1.65, 95% CI: 1.51-1.79; 15-19 year olds aRR: 1.20, 95% CI: 1.18-1.21) and FGR (10-14 year olds aRR: 1.22, 95% CI: 1.01-1.48; 15-19 year olds aRR: 1.28, 95% CI: 1.25-1.32) compared to 25-29 year olds. However, they were less likely to have a CB (10-14 year olds aRR: 0.59, 95% CI: 0.55-0.63; 15-19 year olds aRR: 0.64, 95% CI: 0.64-0.65) and have adequate prenatal care (10-14 year olds aRR: 0.60, 95% CI: 0.57-0.63; 15-19 year olds aRR: 0.83, 95% CI: 0.82-0.83). Younger teen deliveries (10-14 year olds) had an increased risk of HTNP (aRR: 1.15, 95% CI: 1.06-1.26) and those in the older teen delivery group (15-19 year olds) had an increased risk of PPH (aRR:1.14, 95% CI: 1.11-1.17). Complete results are presented in Table 2. Conclusions Adolescent mothers are at increased risk for adverse obstetrical outcomes such as FGR and PTB. HTNP is selectively increased in those deliveries with maternal age < 15 year of age, while PPH is increased in deliveries of mothers 15-19 years of age. Teenagers are less likely to have adequate prenatal care and have a CB. These results suggest that tailoring prenatal and intrapartum care in teenage pregnancies may be warranted, however larger studies are needed to corroborate our findings.

Epidemiology of Birth Defects in Teenage Pregnancies: Based on Provincial Surveillance System in Eastern China.

Abstract:BackgroundHealthcare for adolescents and birth defects (BD) prevention are highlighted public health issues. The epidemiology of birth defects in teenage pregnancies has not been studied extensively. ObjectivesTo investigate the prevalence trend and spectrum of BDs among teenage mothers. MethodsThis observational study covered all births registered in the BD surveillance system in Zhejiang Province, China, during 2012-2018. The annual change in the prevalence of BDs among adolescent mothers was estimated. Crude relative ratios using the BD categories in teenage pregnancies were calculated and compared with those in women aged 25-29 years. ResultsOverall, 54,571 BD cases among 1,910,977 births were included in this study, resulting in an overall prevalence of 234.64 to 409.07 per 10,000 births from 2012 to 2018 (P-trend < 0.001) in total population. The prevalence of birth defects in teenage pregnancies increased from 247.19 to 387.73 per 10,000 births in 2012-2018 (P-trend = 0.024). The risks of neural tube defects (relative risk [RR] = 3.15, 95% confidence interval [CI] 2.56, 3.87), gastroschisis (RR = 7.02, 95% CI 5.09, 9.69), and multiple birth defects (RR=1.27, 95% CI 1.07, 1.52) were higher in teenage pregnancies than those in women aged 25-29 years. ConclusionsWe found a distinctive spectrum of BDs, with higher proportions of fatal or multiple anomalies in infants born to teenage mothers than in those born to adults aged 25-29 years. These results emphasize the importance of providing adolescents with better access to reproductive and prenatal care.

Abstract:Programme evaluation is a complex but integral component of a nursing education programme. It is an ongoing process of collecting and describing data which provides the basis for decision making. Specifically, evaluative data can be used to prepare for accreditation visits; account for budgetary expenditures; answer requests for information; develop faculty and staff; and examine the planned and actual effects of the programme within the community and make changes accordingly. There are several programme evaluation models available to guide the evaluation process, but no single model is best and nurse educators must consider a variety of variables. Ideally, the selection of a model should be based on the purpose of the evaluation, programme needs, material and spatial resources, and personnel time, as well as the needs and desires of key interest groups. Several considerations are also required before the model is implemented. These include determining specific priorities for evaluation since all aspects of a programme (conceptual framework, philosophy, programme goals, student characteristics, graduates' performance, faculty and administrative expertise, as well as adequacy of resources) usually cannot be evaluated simultaneously; how the evaluation should proceed; time frames for specific evaluation projects; and personnel responsible for the evaluative activities.

Abstract:Objective To describe utilization of prenatal care and outcomes of low birth weight and preterm birth among adolescent births in Mexico. Methods We used birth certificate data and included live births to individuals 10–24 years, 2008–2019. Our outcomes were binary measures of adequate prenatal care, low birth weight, and preterm birth. We compared adolescents (10–14 years, 15–16, and 17–19) to those 20–24 years. We included individual‐, clinical‐, and municipality‐level variables, used multivariable logistic regression, and calculated adjusted predicted probabilities. We also tested whether receipt of prenatal care moderated the relationship of age with preterm birth and low birth weight. Results We included a total of 12 106 710 births to women 10–24 years. The adjusted predicted probability of adequate prenatal care increased with age: 56.07% (95% CI 55.82–56.31%) adjusted probability among adolescents 10–14 years compared with 65.51% (95% CI 65.48%–65.55%) among individuals 20–24 years. Receipt of adequate prenatal care in part mitigated disparities in preterm birth and low birthweight across all age groups: 7.30% (95% CI 7.17%–7.43%) adjusted probability of delivering a preterm infant among those 10–14 years who received adequate prenatal care compared with 9.37% (95% CI 9.20%–9.53%) among those 10–14 years without adequate prenatal care. Conclusion In Mexico, adolescent pregnancies are associated with inadequate prenatal care as well as higher odds of preterm delivery and low birth weight. Youngest adolescents (10–14 years) have the highest probability of adverse outcomes. Adequate prenatal care may help partially mitigate disparities in poor perinatal outcomes.

Clinical analysis of obstetric outcomes for one hundred cases with adolescent pregnancy

Abstract:Objective To appraise the characteristics of the obstetric outcomes of adolescent pregnancy. Methods From 1990 to 2002, one hundred consecutive adolescent women admitted for delivery in our hospital were enrolled with analysis and comparison to the adult mothers (n=22 0078) in the same period for the obstetric outcomes. Results The rate of adolescent mother increased steadily from 0. 005% in 1992 to 1.09% in 2002; the ratio of spontaneous delivery in adolescent pregnancy (82%) was higher than that in mature pregnancy (55.4%); the ratio of cesarean section in adolescent pregnancy (16%) was lower than that in mature pregnancy (38. 50%, P＜0. 000 1); the rate of the premature labor (＜37 w) in the adolescent mother (14%) was significantly lower than that in mature pregnancy (2.8%, P＜0. 001); the rate of precipitate labor in adolescent pregnancy was 13%, higher than that in mature pregnancy 6.8% (P＜0.01); the rate of severe preeclampsia and eclampsia in the ado- lescent group was 5%, higher than that in mature group 1. 96% (P＜0. 001). There were no significant diffe rences in birth weight of newborn, neonatal asphyxia and neonatal mortality rate between the two groups; also there were no significant differences on premature rupture of membranes and postpartum hemorrhage. Conclusion The rate of adolescent mother was gradually rising year by year, and almost all of them tend to give rise to sponta- neous delivery, premature labor even precipitate labor. There are no significant differences on the outcomes of new- born and the rate of premature rupture of membranes and postpartum hemorrhage, but are prone to have pre- eclampsia and eclampsia.
Association Between Postterm Pregnancy and Adverse Growth Outcomes in Preschool-Age Children.

Abstract:Background Postterm pregnancy has been associated with higher risk of perinatal mortality and morbidity, but its long-term health effects on offspring are poorly understood. Objectives The aim of the study was to investigate the prospective associations between maternal postterm pregnancy and adverse growth outcomes in children. Methods The Jiaxing Birth Cohort is part of a large population-based health surveillance system in China and recruited pregnant females resident in the Jiaxing area between 1999 and 2013; newborns were followed up for a median duration of 5.8 y until they went to school. Mother-child pairs with maternal gestational information and offspring's anthropometric data at 4-7 y old were included. Postterm pregnancy was defined as maternal gestational age >= 42 and <47 wk, and its associations with offspring obesity, overweight/obesity, and thinness during childhood were determined by using Poisson regression models. Results Of the 101,505 included mother-child pairs, 2369 (2.3%) children were born at postterm. Children born at postterm had significantly lower BMI-for-age z score, weight-for-age z score, and height-for-age z score than those born at term; the mean difference (95% CI) was -0.11 (-0.15, -0.06), -0.17 (-0.21, -0.13), and -0.16 (-0.20, -0.12), respectively. When comparing postterm with term pregnancy, the multivariable-adjusted RRs and 95% CIs among preschool-age children were 0.87 (0.68, 1.11) for obesity, 0.82 (0.72, 0.94) for overweight/obesity, and 1.18 (1.09, 1.28) for thinness, respectively. These risk estimates were robust in sensitivity analyses, but were attenuated in several subgroups stratified by age, sex, mode of delivery, and fetal distress. Conclusions Postterm pregnancy was associated with a higher risk of thinness, and a lower risk of overweight/obesity, as well as lower growth parameters in preschool-age children. These findings imply that postterm pregnancy may impede the long-term growth of offspring.

Abstract:BACKGROUND: Both young and advanced maternal age pregnancies have strong associations with adverse pregnancy outcomes; however, there is limited understanding of how these associations present in an urban environment in China. This study aimed to analyze the associations between maternal age and pregnancy outcomes among Chinese urban women.METHODS: We performed a population-based study consisting of 60,209 singleton pregnancies of primiparous women whose newborns were delivered after 20 weeks' gestation between January 2012 and December 2015 in urban areas of China. Participants were divided into six groups (19 or younger, 20-24, 25-29, 30-34, 35-39, 40 or older). Pregnancy outcomes include gestational diabetes mellitus (GDM), preeclampsia, placental abruption, placenta previa, premature rupture of membrane (PROM), postpartum hemorrhage, preterm birth, low birthweight, small for gestational age (SGA), large for gestational age (LGA), fetal distress, congenital microtia, and fetal death. Logistic regression models were used to assess the role of maternal age on the risk of adverse pregnancy outcomes with women aged 25-29 years as the reference group.RESULTS: The risks of GDM, preeclampsia, placenta previa, and postpartum hemorrhage were decreased for women at a young maternal age and increased for women with advanced maternal age. Both young and advanced maternal age increased the risk of preterm birth and low birthweight. Young maternal age was also associated with increased risk of SGA (aOR 1.64, 95% CI 1.46-1.83) and fetal death (aOR 2.08, 95% CI 1.35-3.20). Maternal age over 40 years elevated the odds of placental abruption (aOR 3.44, 95% CI 1.47-8.03), LGA (aOR 1.47, 95% CI 1.09-1.98), fetal death (aOR 2.67, 95% CI 1.16-6.14), and congenital microtia (aOR 13.92, 95% CI 3.91-49.57). There were positive linear associations between maternal age and GDM, preeclampsia, placental abruption, placenta previa, PROM, postpartum hemorrhage, preterm birth, LGA and fetal distress (all P for linear trend < .05), and a negative linear association between maternal age and SGA (P for linear trend < .001). The analysis of the associations between maternal age and adverse fetal outcomes showed U-shape for preterm birth, low birth weight, SGA, fetal death and congenital microtia (all P for quadratic trend < .001).CONCLUSIONS: Advanced maternal age predisposes women to adverse obstetric outcomes. Young maternal age manifests a bidirectional effect on adverse pregnancy outcomes. The findings may contribute to improving women's antenatal care and management.

Adverse Perinatal Outcomes Among Teenage Mothers Delivering at a Tertiary Referral Hospital in Southwestern Uganda: Prevalence and Associated Factors

Abstract:Introduction: Each year, millions of teenagers in low-resource areas experience unintended pregnancies, many of which result in childbirth. These pregnancies often carry an increased risk of negative perinatal outcomes. Objectives: The study determined the prevalence and factors associated with adverse perinatal outcomes among teenagers delivering at a tertiary referral hospital in southwestern Uganda. Methods: This cross-sectional study was carried out in the Department of Obstetrics and Gynecology. We consecutively included all teenagers (13-19 years) in the postnatal ward who delivered. Descriptive statistics were used to summarize demographic and outcome data, and multivariable logistic regression analysis was used to identify factors associated with adverse perinatal outcomes. Results: Overall, 327 participants were enrolled. The mean age was 18.4 (SD 1.1) years, while the mean number of antenatal care (ANC) visits attended was 4.6 (SD 1.9). Less than half delivered by cesarean 136 (41.6%) and 16 (4.9%) were HIV seropositive. Approximately 140 (42.8%) participants had adverse perinatal outcomes, including neonatal death (7, 2.1%), APGAR score at five minutes <7 (44, 13.5%), or low birth weight <2.5 kg (52, 15.9%). ANC attendance was mildly protective against adverse perinatal outcomes (aOR 0.91 (95% CI 1.14, 3.01), p=0.03). Feeling indifferent toward the pregnancy was associated with increased odds of one or more adverse perinatal outcomes compared to feeling happy about the pregnancy (aOR 3.39 (95% CI 1.11, 10.37), p=0.02). Participants with a history of prior miscarriage had increased odds of adverse perinatal outcomes (aOR 9.03 (95% CI 2.45, 25.53), p=0.04). Conclusions: Nearly half of teenagers experienced adverse perinatal outcomes, and a history of prior miscarriage was a significant risk factor for adverse perinatal outcomes, while ANC was protective. Prospective cohort studies to explore the newborn and child developmental outcomes among children born to teenage mothers are also recommended.

Abstract:Abstract We aimed to describe the characteristics of adolescent pregnancy, determine its effect on adverse maternal and perinatal outcomes and explore whether that association varies with gestational age with the goal of proposing specific recommendations for adolescent health in China. This study included 2,366,559 women aged 10–24 years who had singleton pregnancies between 2012 and 2019 at 438 hospitals. Adolescent pregnancy was defined as younger than 20 years of age. We used multivariable logistic regression to estimate the effects. Women aged 20–24 years served as the reference group in all analyses. The proportion of rural girls with adolescent pregnancies rebounded after 2015 even though common-law marriage in rural areas decreased. Higher risks of eclampsia (adjusted odds ratio (aOR) 1.87, 95% confidence interval (CI) 1.57 ~ 2.23), severe anaemia (aOR 1.18, 95% CI 1.09 ~ 1.28), maternal near miss (MNM; aOR 1.24, 95% CI 1.12 ~ 1.37), and small for gestational age (SGA; aOR 1.30, 95% CI 1.28 ~ 1.33) were observed when gestational age was > 37 weeks. Adolescent pregnancy was independently associated with increased risks of other perinatal outcomes. Further implementation of pregnancy prevention strategies and improved health care interventions are needed to reduce adolescent pregnancies and prevent adverse fertility outcomes among adolescent women in China at a time when adolescent fertility rate is rebounding.

Adolescent pregnancies in Turkey: a single center experience

Abstract:BACKGROUND: Adolescent pregnancies are more likely to be complicated with adverse perinatal outcomes. OBJECTIVE: Assess the sociodemographic and clinical characteristics of adolescents who have delivered singleton newborns. DESIGN: Retrospective cohort SETTINGS: A tertiary training and research hospital in Turkey. PATIENTS AND METHODS: This was a review of adolescents and adults who delivered singleton newborns at a tertiary health center between January 2018 and June 2022. Pregnant adolescents were aged <20 years. MAIN OUTCOME MEASURES: Adverse maternal and perinatal outcomes. SAMPLE SIZE: 2233 pregnant women (754 adolescents and 1479 adults) RESULTS: Turkish nationality was significantly less prevalent in pregnant adolescents than pregnant adults ( P =.001). Oligohydramnios, fetal growth restriction, perineal injury and postpartum intravenous iron treatment were significantly more prevalent in pregnant adolescents than pregnant adults ( P <.05 for all). The neonates born to adolescent mothers had significantly lower birth weight and first minute Apgar score than the neonates born to adult mothers ( P =.001 for both). Small for gestational age, need for intensive care and death were significantly more prevalent in neonates born to adolescent mothers than those born to adult mothers ( P =.001 for all). Compared with pregnant adults, pregnant adolescents had a significantly higher risk of oligohydramnios ( P =.001), preterm delivery ( P =.024), intravenous iron treatment ( P =.001), and small for gestational age ( P =.001). CONCLUSION: Due to the refugee population received by Turkey, it would be prudent to expect more frequent adolescent pregnancies. Adolescent pregnancies are more likely to be complicated with low birth weight, oligohydramnios, preterm delivery, postpartum iron treatment, lower Apgar scores, need for neonatal intensive care and neonatal death. Abstract:Background and Objective: The current study aimed to determine the association of maternal age and obstetric complications and adverse birth outcomes among the adolescent and adult pregnant women. Methods: A cross-sectional study was conducted involving about 150 pregnant women (75 adolescent and 75 adult) who were purposively selected from three maternity centers of Dhaka city. All of them were primigravida experiencing their third trimester of pregnancy. They were interviewed twice (first visit during third trimester and second visit after delivery) with a semi-structured questionnaire. Statistical analysis was done by IBM SPSS Statistics version 20.0. Adjusted odds ratios were achieved by conducting binomial logistic regression analysis. Results: The mean age of the adolescent pregnant girls and adult pregnant women were 16.54±2.38 and 25.23±5.67 years of age, respectively. It was explored that adolescent pregnant girls had higher risks for anemia, preeclampsia, eclampsia, severe fever, jaundice, abnormal position of the baby and urinary problems during gestation. On the contrary, they were observed with lower risks for gestational diabetes, excessive bleeding and leakage of amniotic fluid during pregnancy. During delivery, adolescent group had higher risks for excessive bleeding and placental abruption. Findings of the study also showed that pregnant adolescents had higher risks of various adverse birth outcomes like preterm delivery, still birth, small for gestational age (SGA), very low birth weight (LBW), and low birth weight (LBW). Conclusion: Adolescent pregnancy in Dhaka city had augmented the risk of obstetric complications during pregnancy, during delivery as well as adverse birth outcomes.

Abstract:Background Adolescence entails a series of situations that can affect both the health of the mother and the child, and constitute a health problem, which should not be considered only in terms of the present, but also in the future, due to the complications it entails. The objective of this study was to identify sociodemographic and psychosocial risk factors (individual, interpersonal and contextual) common among adolescents undergoing a pregnancy of any gestacional age, who come to the Hospital "José Eleuterio González". Methods A cross-sectional study was carried out. The study population consisted of adolescent patients treated at the Hospital Dr. José E. González in the resolution of pregnancy or prenatal control. The selected population was surveyed on personal, interpersonal and contextual variables, through the Apgar Score, The National Survey Adolescents and the Stanley Coopersmith Self- esteem Inventory. Results A total of 400 patients who completed the questionnaire were included. The average age was 17.74 ± 1.65 years. A total of 221 patients were found between 16 and 18 years old (middle adolescence), which represents 55.2% of the total sample. Regarding the family environment in which the patients we included in the study live, it was found that 177 of the cases had parents who were teenage parents. This means that being the child of teenage parents is a risk factor for this sociological phenomenon to occur in the next generation (p< 0.02), 99% of the cases share the house with their parents, in addition to 325 and 159 of the patients share the room and the bed respectively. Regarding the level of education of their parents and their partner, it was found that 51% (p< 0.04) had education up to high school. In 249 cases the level of study achieved in the patients was up to high school (p< 0.001) corresponding to 63%. The 59% of the cases mentioned having had more than one sexual partner, which represents a high risk of getting pregnant (p< 0.001), with the average start of an active sexual life being 15.5 ± 1.4 years. Conclusions It was found the same academic degree in mother ́s patient, her family and her partner, the deficiency of contraceptive use despite their knowledge, been daughter of adolescent parents and having had several sexual partners as the factors of greatest risk for the incidence of adolescent pregnancy. The medium-low and low socioeconomic stratum were predominant, with a direct economic dependence on the parents. Early pregnancy is a trigger for school dropouts. Social, school, home and general self-esteem was not significant for any age group. In our population, the family is functional, and patients have a support network.

Abstract:Background: Childhood obesity has increased significantly in recent decades. Objective: The objective was to examine the perinatal risk factors related to childhood obesity. Design: In a prospective study, 89 women with normal glucose tolerance (NGT) or gestational diabetes mellitus (GDM) and their offspring were evaluated at birth and at 8.8 +/- 1.8 y. At birth, obstetrical data, parental anthropometric measures, and neonatal body composition were assessed; at follow-up, diet and activity were assessed and laboratory studies were conducted. Weight was classified by using weight for age and sex, and body composition was measured by using dual-energy X-ray absorptiometry. In childhood, data were analyzed as tertiles and prediction models were developed by using logistic and stepwise regression. Results: No significant differences in Centers for Disease Control and Prevention weight percentiles, body composition, and most metabolic measures were observed between children of mothers with NGT and GDM at follow-up. Children in the upper tertile for weight had greater energy intake (P = 0.02), skinfold thickness (P = 0.0001), and leptin concentrations (P < 0.0001) than did those in tertiles 1 and 2. Children in the upper tertile for percentage body fat had greater waist circumference (P = 0.0001), insulin resistance (P = 0.002), and triglyceride (P = 0.009) and leptin (P = 0.0001) concentrations than did children in tertiles 1 and 2. The correlation between body fat at birth and follow-up was r = 0.29 (P = 0.02). The strongest perinatal predictor for a child in the upper tertile for weight was maternal pregravid body mass index (BMI; kg/m(2)) >30 (odds ratio: 3.75; 95% CI: 1.39, 10.10; P = 0.009) and for percentage body fat was maternal pregravid BMI >30 (odds ratio: 5.45; 95% CI: 1.62, 18.41; P = 0.006). Conclusion: Maternal pregravid BMI, independent of maternal glucose status or birth weight, was the strongest predictor of childhood obesity.

Abstract:Bone marrow-derived stromal stem cells (BMSC) can differentiate along a variety of mesenchymal lines, including mesangial cells. For determining whether BMSC can be induced to differentiate along podocytic lines in vitro, canine BMSC were cultured on plastic, type I collagen, and NC1 hexamers of type IV collagen from normal and Alport canine glomerular basement membrane. Results were compared with a mouse podocyte cell line. In the case of the podocyte line, differentiation occurred on all three matrices as indicated by the expression of synaptopodin and CD2-associated protein (CD2AP) and organization of myosin heavy chain IIA into a linear pattern. BMSC proliferated equally well on all matrices, but cells that were grown on type IV collagen NC1 hexamers became larger and stellate. Evidence for podocytic differentiation occurred on all three collagen matrices as indicated by the redistribution of myosin IIA to a linear pattern and expression of synaptopodin, CD2AP, and alpha-actinin. A punctate distribution of CD2AP was seen only in cells that were grown on normal and Alport glomerular basement membrane NC1 hexamers. Differentiated podocytes expressed the alpha1, alpha2, and alpha5 chains of type IV collagen but at higher levels in cells that were grown on NC1 hexamers. Similar results were obtained in BMSC for the alpha1 and alpha2 chains only. The alpha3, alpha4, and alpha6 chains were never detected in the podocyte line or BMSC. These results indicate that BMSC undergo a degree of podocytic differentiation in vitro and greater when grown on type IV collagen NC1 hexamers than type I collagen. Alport and normal NC1 hexamers seem equally permissive to BMSC growth and differentiation, suggesting that these processes are not influenced specifically by the alpha3/alpha4/alpha5 network. BMSC may be useful in the development of stem cell-based reconstitution of glomeruli that are damaged by disease and for gene therapy of genetic glomerular diseases such as Alport syndrome.
Adverse pregnancy outcomes attributable to overweight and obesity across maternal birth regions: a Swedish population-based cohort study

Abstract:Background: Whether there are differences in the contribution of overweight and obesity to adverse pregnancy outcomes between migrant and non-migrant women in high-income countries, which might increase health inequalities, remains unclear. Therefore, in this study, we aimed to estimate the contribution (including the proportion and number of attributable cases) of overweight and obesity to a wide range of adverse pregnancy outcomes in Swedish-born and migrant women. Methods: This population-based cohort study used nationwide population registries in Sweden. All outcomes and covariates were collected from the Medical Birth Register (delivery and maternal characteristics), National Patient Register (inpatient and specialised outpatient care), the Cause of Death Register (all deaths in Sweden), the Longitudinal Integrated Database for Health Insurance and Labour Market Studies (socioeconomic data), and the Total Population Register (maternal birth country data). Women with missing records of BMI at the first antenatal visit, country of birth, or covariates, were excluded from the study. BMI was measured during the first antenatal visit. Maternal country of birth was categorised into Sweden and seven super-regions. The proportion (ie, population attributable fractions [PAFs]) and the number of adverse pregnancy outcomes attributable to overweight and obesity were calculated, adjusting for maternal age, gestational age at first antenatal visit, maternal parity, smoking status, maternal somatic conditions, child's sex, socioeconomic and demographic variables. Findings: We identified 2 228 416 singleton pregnancies between Jan 1, 2000, and Dec 31, 2020 of 1 245 273 women. 254 778 (11·4%) pregnancies with missing records of BMI at the first antenatal visit, country of birth, or covariates were excluded, which resulted in a final analytical cohort of 1 973 638 pregnancies carried by 1 164 783 women. The overall mean maternal age of the study population was 30·8 years (SD 5·1). As estimated by PAFs, overweight and obesity contributed to a large proportion of adverse pregnancy outcomes: gestational diabetes (52·1% [95% CI 51·0-53·2]), large-for-gestational age (36·9% [36·2-37·6]), pre-eclampsia (26·5% [25·7-27·3]), low Apgar score (14·7% [13·5-15·9]), infant mortality (12·7% [9·8-15·7]), severe maternal morbidity (henceforth referred to as a near-miss event; 8·5% [6·0-11·0]), and preterm birth (5·0% [4·4-5·7]) in the total study population. PAFs varied between maternal birth regions. Interpretation: Interventions to reduce overweight and obesity have the potential to mitigate the burden of adverse pregnancy outcomes and possibly reduce inequalities in reproductive health. Therefore, public health practice and policy should prioritise efforts to prevent overweight and obesity among women of childbearing age. Funding: Swedish Research Council.
Early perinatal outcomes of babies born to adolescent mothers at two maternity hospitals in Mogadishu, Somalia

Abstract:Background: Adolescent motherhood remains a major problem in developing countries. We set out to describe the perinatal outcomes of infants born to adolescent mothers and to determine factors associated with birth asphyxia among these infants in Mogadishu, Somalia. Methods: This cross-sectional study involved adolescent mothers who presented in labor. Sociodemographic and medical data was collected and mother-infant pairs were followed up at 24hrs. Poor perinatal outcomes were: low birth weight, birth asphyxia, death or hospitalization after 24 hours. Data was entered into Epi data 3.1 and analysed using STATA version 12.0. Results: Of the 270 adolescents, mean age was 17.7 years (±1.19), 89% were married, 94% were unemployed and 54% had not received formal education. Of the 270 babies born, 70 (25.9%) had poor perinatal outcomes which included; 35 (12.9%) died; of whom 30 (11.1%) were stillbirths. Thirty-two infants (13.3%) had birth asphyxia and 18 (6.7%) had low birth weight. Prolonged labor (p-value=<0.001) and preterm birth (p-value=0.023) were significantly associated with birth asphyxia while living in Mogadishu was protective (p- value=0.018). Conclusions: About one in four adolescent mother’s babies had poor perinatal outcomes. Prolonged labor and preterm delivery and were associated with birth asphyxia while residing closer to the facilities was protective. Abstract:Regional cerebral blood flow (rCBF) was measured in 13 patients with various cerebral disease by N-isopropyl-p-[123I]iodoamphetamine (IMP) SPECT with the microsphere model (rCBF (CbST)), which was compared with 133Xe inhalation SPECT (rCBFXe). Continuous withdrawal of arterial blood was performed for 5 minutes and SPECT image was obtained from 25 min to 55 min after the tracer injection. The reconstructed counts at 5 min was corrected using entire cerebral counts collected at 5 min, 20 min, and 60 min after the tracer injection. As compared with rCBFXe, rCBF (CbST) was underestimated in high flow areas and overestimated in low flow areas. Regional CBF was also measured using one minute short time SPECT images at 5 min (rCBF (Cb5)), 20 min (rCBF (Cb20)), and 60 min (rCBF (Cb60)) after the injection. Regional CBF tended to be underestimated with time from the tracer injection due to the wash-out from brain tissue to blood. In comparison with rCBF (Cb5), rCBF (CbST) was underestimated in high flow areas and overestimated in low flow areas. This error was considered to be due to the correction of reconstruct counts using entire cerebral counts, because of the kinetic behavior of 123I-IMP was different in each region of the brain.

Abstract:To evaluate outcomes related to pregestational obesity and excessive weight gain during pregnancy. This retrospective cohort was conducted from August to December 2020. Participants were divided into four groups: non-obese with non-excessive weight gain (n=765, 45.9 %), obese with non-excessive weight gain (n=190, 11.4 %), non-obese with excessive weight gain (n=532, 31.9 %), and obese with excessive weight gain (n=179, 10.7 %). The outcomes were evaluated for gestational diabetes (GDM), pregnancy-induced hypertension (PIH), newborn large for gestational age (LGA) and cesarean delivery. A p-value of <0.05 was considered significant. The odds of GDM were significant in groups 2 (CR, 3.6; 95 %CI, 2.5–5.2) and 4 (CR,1.9; 95 %CI, 1.3–2.9). The odds of PIH in groups 3 (CR, 1.7; 95 %CI, 1–2.6) and 4 (CR,3.1; 95 %CI, 1.9–5.2) and those of LGA newborns in groups 2 (CR, 2.0; 95 %CI, 1.2–3.3), 3 (CR, 2.6; 95 %CI, 1.9–3.7), and 4 (CR, 3.2; 95 %CI, 2–5) were high. The odds of GDM were higher in participants with pregestational obesity, while the odds of PIH were higher in participants with excessive weight gain. All groups analyzed, except the reference group, had greater chances of lga newborns. the form of delivery was not affected.

Abstract:Aims: Pregnancies are increasingly affected by young-onset type 2 diabetes mellitus (YT2DM), an aggressive phenotype associated with a higher vascular risk profile compared to type 1 diabetes mellitus (T1DM). We compared pregnancy outcomes to illuminate areas where differing management guidance might be needed. Methods: This retrospective single-centre study (2010-2019) included 259 singleton pregnancies affected by pregestational T1DM (N=124) or YT2DM (N=135) diagnosed at < 40 years. Primary outcomes included preterm delivery, large for gestational age (LGA) infants, and pre-eclampsia. Results: The YT2DM cohort were older, with more obesity, greater apparent sociodemographic disadvantage, and lower measures of pregnancy preparedness. Overweight/obesity were also prevalent in the T1DM cohort (46 % affected). The second/third trimester mean HbA1c measurements were significantly higher in the T1DM cohort. Pre-eclampsia and preterm delivery rates were similar between the cohorts. Significantly lower rates of LGA infants, NICU admission, neonatal hypoglycaemia, and neonatal respiratory distress were seen in the YT2DM cohort (p < 0.05 for all). Conclusions: In pregnancy, YT2DM appears to be the lower-risk cohort compared to T1DM despite higher obesity rates. Gaps in achieving glycaemic targets exist for both subtypes but particularly for T1DM. The relative impact of increasing BMI in pregnancies affected by T1DM requires further elucidation.

Abstract:Background: More than 70% of pregnant adolescents in developing countries experience inappropriate gestational weight gain (GWG). Objective: To determine the association of the number of antenatal care visits (ANC) with GWG, birth weight, and their differences between two countries. Methods: A prospective study was conducted in two cohorts of adolescents, one from Mexico and one from Colombia. The study calculated pregestational body mass index (BMI), obtained GWG and birth weight, and collected socioeconomic characteristics. Birth weight was categorized according to gestational age. A total of 690 mother-child pairs were included, of which 42.6% were Colombian and 57.4% Mexican. Results: The study found no association between socioeconomic characteristics and GWG or birth weight. Colombian adolescents were more likely to experience insufficient GWG (68%), compared with 36% of Mexican adolescents. Colombian adolescents who attended fewer than eight ANC visits were at increased risk of insufficient GWG, whereas Mexican adolescents were at increased risk of excessive GWG. Mexican adolescents who began their pregnancies overweight or obese were at increased risk of excessive GWG. Fewer than eight ANC visits were associated with small for gestational age (SGA) in the Mexican cohort. Conclusions: Inadequate numbers of ANC visits were associated with excessive and insufficient GWG, and SGA. Promoting ANC in adolescent pregnancy is essential to prevent suboptimal GWG and SGA. This study highlights the need for interventions targeting pregnant adolescents from low socioeconomic backgrounds, prioritizing early initiation of prenatal care (first trimester) and a drastic reduction in the high rates of cesarean sections in this group.
