Abstract:Objective:To observe the dynamic changes in the salivary microbiota of children with dental caries and those who were caries-free and to analyze the functional differences in the oral microecology of the two groups during the course of sugar metabolism and the synthesis and transport of multiple amino acids.Methods:Ten children with dental caries and 10 caries-free children were enrolled. We employed Illumina metagenomics technology to analyze the composition and function of salivary microbiome in children with and without caries. Six months later, PacBio single-molecule long-read sequencing technology was used to analyze the changes over time in the oral microbial communities of the two groups. We studied the patterns of change in the oral microbial communities under diseased or healthy conditions and attempted to offer a comprehensive interpretation of children's oral microbiota in terms of its composition and functions.Results:The composition of the oral microbiota of children with or without dental caries changed significantly over time. At the phylum level, changing trends in the salivary microbial communities of children with dental caries were in line with those in caries-free children. In these microbial communities, increased proportions of Firmicutes and decreased proportions of Actinobacteria and Bacteroidetes were found in the two groups. At the genus level, however, the two groups showed significantly different changes of the salivary microbial communities. Upward trends in the abundance of Lactobacillus, Methylobacterium, and Megasphaera were found in the caries group, while the abundance of these genera in the caries-free group showed downward trends. At the species level, L. fermentum, L. gasseri, L. oris, S. downei, and some other species belonging to the genus Lactobacillus showed upward trends in the saliva of children with caries, while they consistently stayed at a relative low level of abundance in caries-free children. The abundance of S. gordonii and S. mutans decreased to a certain extent in children with dental caries, but the abundance of S. gordonii and S. mutans in caries-free children were always at a low level. Species such as S. mutans and C. gracilis were positively correlated to the sum of decayed, missing and filled teeth (dmft), while N. flavescens was negatively correlated to dmft. gltA, icd, and mqo, the key genes related to tricarboxylic acid (TCA) cycle, gudB, a glutamate synthesis-related gene, and argAB/C/J, arginine synthesis-related genes, were significantly increased in caries-free children. In addition, the abundance of the NADH dehydrogenase-related gene nuoB/C/D/E/H/I/J/K/L/M in the electron transport chain increased significantly in caries-free children.Conclusion:Dynamic changes were found in the oral microbiota of children with or without caries. The trends of microbial shifts over time were associated with the oral health status. Oxidative phosphorylation and the synthesis and transport of amino acids such as glutamate and arginine in the oral microecology were more active in caries-free children.
Quantitative analysis of biofilm bacteria according to different stages of early childhood caries

Abstract:Objective: Monitoring selected key species related to health or disease may facilitate caries risk assessment and discovery of novel ecological preventive and therapeutic approaches. This study aimed at quantifying Actinomyces naeslundii, Bifidobacterium spp., Lactobacillus acidophilus, Lactobacillus casei group, Streptococcus gordonii, Mitis group and Streptococcus mutans by quantitative polymerase chain reaction (qPCR) in dental biofilm from Brazilian children with different stages of early childhood caries (ECC). Design: Seventy-five preschool children were clinically evaluated by ICDAS criteria and divided into groups: caries-free (CF; n = 20), enamel caries lesions (ECL; n = 17) and dentine caries lesions (DCL; n = 38). Plaque samples from all children were collected for detection and quantification of the selected bacteria. Results: L. acidophilus and L. casei group were absent in almost all plaque samples. No differences in relative proportions of A. naeslundii, Mitis group and S. gordonii were observed in any stage of caries. However, S. mutans and Bifidobacterium spp. were present at higher concentrations in the biofilm of children with DCL (p < 0.001). Multivariate analysis showed that S. mutans and Bifidobacterium spp. were strongly associated with biofilm in children with DCL. Conclusion: Differences were observed in the proportion of acidogenic and aciduric bacteria with dental caries progression. The data indicate that S. mutans and Bifidobacterium spp. in dental biofilm may be involved in some progression processes for ECC.
[Quantitative Analysis of Streptococcus Mutans and Its Proportion in the Dental Plaque of Different Caries-Susceptible Children].

Abstract:OBJECTIVE:To determine the quantity of Streptococcus mutans (S. mutans) and the ratio of S. mutans to total bacteria in the dental plaque of different caries-susceptible children.METHODS:Dental plaque samples from 26 children aged 3 years old to 4 years old were collected. The quantities of S. mutans and total bacteria were determined by TaqMan real-time fluorescence quantitative polymerase chain reaction (PCR). The ratio ofS. mutans to total bacteria in children with and without caries was calculated and statistically analyzed.RESULTS:In children with and without caries, the quantities of S. mutans were 1.33 x 10(5) and 1.16 x 10(3) CFU x mg(-1), the total bacteria were 7.17 x 10(7) and 1.01 x 10(8) CFU x mg(-1), and the ratios of S. mutans to total bacteria were 0.058 6 and 0.018 6, respectively. Significant differences were observed in the quantities of S. mutans and the total bacteria as well as in the ratios of S. mutans to total bacteria of the two groups (P = 0.033, 0.418, 0.008).CONCLUSION:The quantities of total bacteria of the two groups show negligible difference. However, the quantity of S. mutans and the ratio of S. mutans to total bacteria in caries-susceptible children are higher than those in caries-free children. Therefore, the ratios of S. mutans to total bacteria in plaque are closely associated with the prevalence of dental caries in children.
Quantitative Analysis of Salivary Oral Bacteria Associated with Severe Early Childhood Caries and Construction of Caries Assessment Model

Abstract:To construct a saliva-based caries risk assessment model, saliva samples from 176 severe early childhood caries (S-ECC) children and 178 healthy (H) children were screened by real-time PCR-based quantification of the selected species, including Streptococcus mutans, Prevotella pallens, Prevotella denticola and Lactobacillus fermentum. Host factors including caries status, dmft indices, age, gender, and geographic origin were assessed in their influence on abundance of the targeted species, which revealed host caries status as the dominant factor, followed by dmft indices (both P < 0.01). Moreover, levels of S. mutans and P. denticola in the S-ECC group were significantly higher than those in the healthy group (P < 0.001 for S. mutans and P < 0.01 for P. denticola). Interestingly, the co-occurrence network of these targeted species in the S-ECC group differed from that from the healthy group. Finally, based on the combined change pattern of S. mutans and P. pallens, we constructed an S-ECC diagnosis model with an accuracy of 72%. This saliva-based caries diagnosis model is of potential value for circumstances where sampling dental plague is difficult.

Abstract:Dental caries remains the most common chronic disease in children, and the respective etiology is not fully understood. Though Streptococcus mutans is an important factor in the initiation and progression of caries, its presence is not always associated with the disease. The existence of caries discordant populations, in which S. mutans counts do not correlate with caries experience, poses a challenging problem. This study explored the possible correlation of S. mutans and other microorganism levels on caries-associated ecology of caries-concordant and discordant populations. A total of forty-seven children were analyzed in this study and stratified into four clinical groups based on their S. mutans levels in saliva (HS/LS: High/low S. mutans ) and caries experience. Streptococcus mutans levels were determined by culture-based selective plating. The salivary microbiome of caries concordant and discordant populations was investigated by 16S rRNA gene sequencing and downstream bioinformatics analysis. The salivary microbial communities significantly clustered based on S. mutans levels and independent of their caries experience. In addition to S. mutans levels, significant differences in the abundance of other species were observed between HS and LS groups. Interestingly, disease-associated species such as Veillonella dispar, Streptococcus spp., and Prevotella spp. were significantly increased in HS groups and may contribute, in combination with S. mutans , to the caries progression. Furthermore, health-associated species exhibited higher abundance in the LS groups, such as Veillonella rogosae, Haemophilus sp., and Alloprevotella spp. but their possible contribution to the caries process remains to be elucidated. This study provides evidence that S. mutans may play a role in shaping the salivary microbial community. Our results highlight that future caries research should consider additional species as health/disease microbial markers in conjunction with S. mutans to improve diagnosis and caries management of the caries-discordant population.

Abstract:Background : Alterations of oral microbiota are the main cause of the progression of caries. The goal of this study was to characterize the oral microbiota in childhood caries based on single-molecule real-time sequencing. Methods : A total of 21 preschoolers, aged 3-5 years old with severe early childhood caries, and 20 age-matched, caries-free children as controls were recruited. Saliva samples were collected, followed by DNA extraction, Pacbio sequencing, and phylogenetic analyses of the oral microbial communities. Results : Eight hundred and seventy six species derived from 13 known bacterial phyla and 110 genera were detected from 41 children using Pacbio sequencing. At the species level, 38 species, including Veillonella spp., Streptococcus spp., Prevotella spp., and Lactobacillus spp., showed higher abundance in the caries group compared to the caries-free group (p < 0.05). The core microbiota at the genus and species levels was more stable in the caries-free micro-ecological niche. At follow-up, oral examinations 6 months after sample collection, development of new dental caries was observed in 5 children (the transitional group) among the 21 caries free children. Compared with the caries-free children, in the transitional and caries groups, 6 species, which were more abundant in the caries-free group, exhibited a relatively low abundance in both the caries group and the transitional group (p < 0.05). We conclude that Abiotrophia spp., Neisseria spp., and Veillonella spp., might be associated with healthy oral microbial ecosystem. Prevotella spp., Lactobacillus spp., Dialister spp., and Filifactor spp. may be related to the pathogenesis and progression of dental caries.

Abstract:In this paper, the author explores the idea that psychoanalysis at its core involves an effort on the part of patient and analyst to articulate what is true to an emotional experience in a form that is utilizable by the analytic pair for purposes of psychological change. Building upon the work of Bion, what is true to human emotional experience is seen as independent of the analyst's formulation of it. In this sense, we, as psychoanalysts, are not inventors of emotional truths, but participant observers and scribes. And yet, in the very act of thinking and giving verbally symbolic ‘shape’ to what we intuit to be true to an emotional experience, we alter that truth. This understanding of what is true underlies the analytic conception of the therapeutic action of interpretation: in interpreting, the analyst verbally symbolizes what he feels is true to the patient's unconscious experience and, in so doing, alters what is true and contributes to the creation of a potentially new experience with which the analytic pair may do psychological work. These ideas are illustrated in a detailed discussion of an analytic session. The analyst makes use of his reverie experience‐for which both and neither of the members of the analytic pair may claim authorship‐in his effort to arrive at tentative understandings of what is true to the patient's unconscious emotional experience at several junctures in the session.

Abstract:Early childhood caries (ECC) is the most common chronic infectious oral disease in preschool children worldwide. It is closely related to the caries activity (CA) of children. However, the distribution characteristics of oral saliva microbiomes in children with different CA are largely underexplored. The aim of this study was to investigate the microbial community in saliva of preschool children with different CA and caries status, and to analyze the difference of microbial community in saliva of children with different CA and its correlation with ECC. Subjects were divided into 3 groups based on the Cariostat caries activity test: Group H, high CA (n=30); Group M, medium CA (n = 30); Group L, low CA (n=30). Questionnaire survey was used to explore the related influencing factors of CA. According to the caries status (on the basis of decayed mising filled teeth), these subjects were divided into caries-free group (dmft=0, n=19), caries-low group (0 < dmft ≤ 4, n=27) and caries-high group (dmft > 4, n=44). Microbial profiles of oral saliva were analyzed using 16S rRNA gene sequencing. There were significant differences in the microbial structure (P < 0.05). Scardovia and Selenomonas were the biomarkers of both H group and high caries group. The genus Abiotrophia and Lautropia were the biomarkers of both the L group and the low caries group, while the Lactobacillus and Arthrospira spp. were significantly enriched in the M group. The area under the ROC curve of the combined application of dmft score, age, frequency of sugary beverage intake, and the genus Scardovia, Selenomonas, and Campylobacter in screening children with high CA was 0.842. Moreover, function prediction using the MetaCyc database showed that there were significant differences in 11 metabolic pathways of salivary microbiota among different CA groups. Certain bacteria genera in saliva such as Scardovia and Selenomonas may be helpful in screening children with high CA.

Abstract:INTRODUCTION Dental caries is one of the most common infectious diseases affecting the oral cavity. Among the oral bacteria, mutans streptococci have been implicated as major cariogenic bacteria as they can produce high levels of dental caries causing substances such as lactic acid and extracellular polysaccharides. AIM The aim of the study was to detect the presence of Streptococcus mutans and Streptococcus sobrinus in dental plaque by using Polymerase Chain Reaction (PCR) method, quantification of these micro-organisms using Modified Sucrose-Bacitracin (SB-20M) agar medium and to correlate their presence in Caries Active (CA) and Caries Free (CF) pre-school children. MATERIALS AND METHODS Sixty-eight pre-school children, in the age group of 3-5 years were divided equally into 34 CA and 34 CF children. Dental plaque samples were obtained for detection of these microorganisms by PCR method and quantification was done using SB-20M culture medium. The data was analyzed using statistical software SPSS version 16. For statistical analysis, the frequencies and means of Colony Forming Units (CFU) were used with CI = 95%. For bivariate analysis, Fisher exact test was used at 5% level of significance. The comparison of mean of number of CFU of S. mutans and S. sobrinus was made by Mann Whitney U test and Spearman's Rho test at 1% level of significance was used for correlation between dmft and CFU in CA group. RESULTS The results showed that S. sobrinus was significantly higher in CA group as compared to CF group whereas S. mutans showed no significant difference. On quantification of these micro-organisms, S. sobrinus was present in significantly higher numbers in CA group as compared to CF group. On correlating the CFU/ml of the micro-organisms with the dmft index, both the micro-organisms showed a positive correlation. CONCLUSION We conclude that S. mutans and S. sobrinus were detected in higher numbers in CA children as compared to CF children. PCR is a sensitive, specific, rapid and an effective method for the detection of oral microorganisms.

Abstract:Objective Assessing the relationship among saliva mutans streptococci,saliva flow rate,saliva buff capacity,oral hygiene and caries status in primary teeth,and to observe the efficacy of Dentocult SM and Dentobuff Strip as a risk indicator for predicting caries experience in preschool children.Methods Among 1018 examined 4-year old and 5-year old children,to screen 204 ones whose dmft≥6 in caries group,and 237 ones in caries free group.Saliva mutans streptococci,saliva flow rate,saliva buff capacity,and debris index were tested for all children.The methods of Dentocult SM and Dentobuff Strip were used.Results ①The difference of the saliva flow rate between two groups was not statistically significant,so was the saliva buffer capacity.②The debris index was 2.22±0.50 in caries free group and 2.37±0.33 in caries group respectively.The difference between two groups was highly statistical significant.③Four scores (0,1,2,3) were used to record the level of saliva mutans streptococci.The proportion of score 0,1,2,3 was 33.3%,19.4%,34.6%,12.7% in caries free group,and 2.9%,11.8%,27.0%,58.3% in caries group respectively.The difference between two groups was highly statistical significant.The mean dmft of children in each score level was 0.46±1.68,3.26±4.89,3.66±4.99,7.10±4.42,respectively.The difference between score 0 group and score 1 group,and between score 2 group and score 3 group was highly statistical significant.There is no statistical difference between score 1 group and score 2 group.Conclusion The study demonstrated the strong positive association between more caries and higher level of saliva mutans streptococci and poor oral hygiene.The Dentocult SM test was effective for predicting caries risk in preschool children.

Abstract:Background Streptococcus mutans (S. mutans) is a potential pathogenic bacteria of dental caries. However, the level of S. mutans is low in some children with severe early childhood caries (SECC) Aim To evaluate the effect of S. mutans level on dental microbiome and cariogenesis. Methods The oral microbiota was compared between caries-free group (CF) and SECC group.16S rRNA gene sequencing was used for S. mutans level bacterial community analysis. The candidate bacteria that were closely related with S. mutans abundance were identified and confirmed by absolute quantitative real-time PCR in clinical dental plaque samples from CF and SECC groups. Results Through in-depth analysis of dental plaque microorganism, Leptotrichia, Selenomonas and Prevotella_7 were found in the S. mutans-low group (p < 0.05) and Porphyromonas, Selenomonas_3 were found in the S. mutans-high group (p < 0.05). Through quantitative real-time PCR, Leptotrichia, Selenomonas and Prevotella_7 were identified as the potential biomarkers of SECC when S. mutans was at a low level. Conclusion Leptotrichia, Selenomonas and Prevotella_7 are identified as potential biomarkers in SECC with a low abundance or without S. mutans. Our study may shed light on the understanding of caries occurrence in SECC with low abundance of S. mutans.
QPCR Screening of Pediatric Saliva Samples to Evaluate Effects of Dental Sealants on Cariogenic Bacteria Streptococcus Mutans and Scardovia Wiggsiae

Abstract:Dental caries is an alarmingly prevalent multifactorial disease worldwide but is most common as a chronic disease among children and adolescents. Within the oral cavity there are hundreds bacterial species, including specific cariogenic pathogens such as Streptococcus mutans and Scardoviawiggsiae. Although dental caries are highly prevalent, it may also be preventable using methods such as dental sealants or fluoride treatment, which have proven to be highly effective in reducing caries risk – although there is limited evidence on the exact effect of these treatments on S. wiggsiaeinparticular. Due to the lack of conclusive evidence, this study is aimed to evaluate the microbial burden of S. mutans and S. wiggsiae among pediatric saliva samples to analyze the effects dental sealants have on these oral microbiota. Saliva samples derived from pediatric patients pre- and post-sealant were obtained from an existing repository. DNA was extracted and screened using qPCR and primers specific for the cariogenic organisms, S. mutans (SM) and S. wiggsiae (SW). These data demonstrated an overall reduction in SW among Post-sealant samples (11.14 U/uL) compared with Pre-sealant samples (11.68 U/uL), as well as a reduction in the overall number of samples harboring S. wiggsiae (n=8). The Pre-sealant average amount of SM (14.54 U/uL) was slightly increased among the Post-sealant samples (15.67 U/uL) with half of the samples analyzed (n=13) exhibiting slight increases in SM levels, and nearly one-third (n=8) exhibiting slight decreases in SM levels. The data from this study may suggest that dental sealants have an impact on certain cariogenic pathogens in the oral microbiota. Whether or not the impact is positive or not in light of the increase in S. mutans remains an observation that should be further analyzed. More research is required to address the limitations found in this study in order to localize the exact effect on the teeth with dental sealants.
The Prevalence of Candida albicans and Malassezia globosa in Preschool Children with Severe Early Childhood Caries: A Case-Control Study

Abstract:Background: This cross-sectional study aimed to identify the prevalence of Candida albicans and Malassezia globosa in children with severe early childhood caries and caries-free children in Hong Kong. Methods: This study first recruited a total of 80 children aged between 48 and 72 months old, 40 children with severe early childhood caries, and 40 caries-free children. The children were then further divided into four groups, with 20 children in each group: Group 1: Severe early childhood caries—C. albicans, Group 2: Severe early childhood caries—M. globosa, Group 3: Caries-free—C. albicans and Group 4: Caries-free—M. globosa. Saliva, plaque, and caries lesion samples were collected from participants with severe early childhood caries, while only saliva and plaque samples were collected from caries-free participants. Caries status of the primary molars was assessed using WHO's decayed, missing, and filled tooth index, and the severity of cavitated lesions was determined based on International Caries Diagnosis and Assessment System criteria as caries code 5 or 6. The samples were analyzed using an Internal Transcribed Space and Quantitative Real-Time Polymerase Chain Reaction. Results:C. albicans was more prevalent in saliva and plaque samples of severe early childhood caries than in the caries-free group. Proportion of C. albicans in both saliva and plaque samples differed significantly between severe early childhood caries and caries-free groups (p < 0.05). Within the severe early childhood caries group, the proportion of children with C. albicans varied between 6 and 46%. No significant difference in M. globosa load was found between plaque samples of the severe early childhood caries and caries-free groups (p = 0.159). Conversely, no significant difference in M. globosa load was observed between saliva samples of severe early childhood caries and caries-free groups (p = 0.051). Conclusions: This study demonstrated a strong association between C. albicans and severe early childhood caries. M. globosa was detected in both the caries-free and severe early childhood caries groups, albeit at low levels.

Abstract:Oral microbiota plays a vital role in maintaining the homeostasis of oral cavity. Dental caries are among the most common oral diseases in children and pathogenic bacteria contribute to the development of the disease. However, the overall structure of bacterial communities in the oral cavity from children with dental caries has not been explored deeply heretofore. We used high-throughput barcoded pyrosequencing and PCR-denaturing gradient gel electrophoresis (DGGE) to examine bacterial diversity of oral microbiota in saliva and supragingival plaques from 60 children aged 3 to 6 years old with and without dental caries from China. The multiplex barcoded pyrosequencing was performed in a single run, with multiple samples tagged uniquely by multiplex identifiers. As PCR-DGGE analysis is a conventional molecular ecological approach, this analysis was also performed on the same samples and the results of both approaches were compared. A total of 186,787 high-quality sequences were obtained for evaluating bacterial diversity and 41,905 unique sequences represented all phylotypes. We found that the oral microbiota in children was far more diverse than previous studies reported, and more than 200 genera belonging to ten phyla were found in the oral cavity. The phylotypes in saliva and supragingival plaques were significantly different and could be divided into two distinct clusters ( p < 0.05). The bacterial diversity in oral microbiome analyzed by PCR-DGGE and barcoded pyrosequencing was employed to cross validate the data sets. The genera of Streptococcus , Veillonella , Actinomyces , Granulicatella , Leptotrichia , and Thiomonas in plaques were significantly associated with dental caries ( p < 0.05). The results showed that there was no one specific pathogen but rather pathogenic populations in plaque that significantly correlated with dental caries. The enormous diversity of oral microbiota allowed for a better understanding of oral microecosystem, and these pathogenic populations in plaque provide new insights into the etiology of dental caries and suggest new targets for interventions of the disease.
Prevalence of Streptococcus mutans and Lactobacillus fermentum and their association with caries and dietary habits in preschool Thai children

Abstract:Aims: To detect the prevalence of Streptococcus mutans and Lactobacillus fermentum in plaque obtained from severe early childhood caries (S-ECC) and caries-free (CF) groups and to assess any association between their prevalence, feeding habits and oral hygiene practices within each group. Study design: Sixty-four supra-gingival plaque samples were collected from S-ECC and CF groups (CF = 30, S-ECC = 34). All participants were asked about feeding habits and oral hygiene care behaviour by direct questionnaire. Methods: After DNA extraction, S. mutans and L. fermentum were amplified using standard PCR. Nested PCR was performed as a second identification method for S. mutans. Results: Mean age in CF and S-ECC was 2.29 ± 0.42 and 2.49 ± 0.41 years, respectively. Mean dmft in S-ECC was 4.51 ± 0.72. By standard PCR, no S. mutans was detected in CF group, while 10 % was detected in S-ECC group. By nested PCR, S. mutans was detected for 48 and 50 % in CF and S-ECC, respectively. L. fermentum was detected for 16 and 40 % in CF and S-ECC, respectively. Conclusion: Nested PCR gave a higher detection frequency than standard PCR but not significant (p ≤ 0.05). The prevalence of S. mutans was not different between the two groups, while the prevalence of L. fermentum was significantly associated with S-ECC (p ≤ 0.05).
The oral bacterial microbiome of occlusal surfaces in children and its association with diet and caries

Abstract:Dental caries is the most prevalent disease in humans globally. Efforts to control it have been invigorated by an increasing knowledge of the oral microbiome composition. This study aimed to evaluate the bacterial diversity in occlusal biofilms and its relationship with clinical surface diagnosis and dietary habits. Anamneses were recorded from thirteen 12-year-old children. Biofilm samples collected from occlusal surfaces of 46 permanent second molars were analyzed by 16S rRNA amplicon sequencing combined with the BLASTN-based search algorithm for species identification. The overall mean decayed, missing and filled surfaces modified index [DMFSm Index, including active white spot lesions (AWSL)] value was 8.77±7.47. Biofilm communities were highly polymicrobial collectively, representing 10 bacterial phyla, 25 classes, 29 orders, 58 families, 107 genera, 723 species. Streptococcus sp_Oral_Taxon_065, Corynebacterium matruchotii, Actinomyces viscosus, Actinomyces sp_Oral_Taxon_175, Actinomyces sp_Oral_Taxon_178, Actinomyces sp_Oral_Taxon_877, Prevotella nigrescens, Dialister micraerophilus, Eubacterium_XI G 1 infirmum were more abundant among surfaces with AWSL, and Streptococcus gordonii, Streptococcus sp._Oral_Taxon_058, Enterobacter sp._str._638 Streptococcus australis, Yersinia mollaretii, Enterobacter cloacae, Streptococcus sp._Oral_Taxon_71, Streptococcus sp._Oral_Taxon_F11, Centipeda sp._Oral_Taxon_D18 were more abundant among sound surfaces. Streptococcus mutans was detected on all surfaces in all patients, while Streptococcus sobrinus was detected only in three patients (mean relative abundances 7.1% and 0.6%, respectively). Neither species differentiated healthy from diseased sites. Diets of nine of the subjects were scored as high in fermentable carbohydrates (≧2X/day between meals). A direct association between relative abundances of bacteria and carbohydrate consumption was observed among 18 species. High consumption of fermentable carbohydrates and sound surfaces were associated with a reduction in bacterial diversity. PCoA plots displayed differences in bacterial community profiles between sound and diseased surfaces. Our study showed that, in addition to mutans streptococci, other species may be associated with the initiation of dental caries on occlusal surfaces, and that biofilm diversity of tooth surfaces is influenced by carbohydrate consumption and a surface's health status.

Association between untreated caries and cariogenic bacteria in adolescents in Taiwan

Abstract:Background/purpose There is a paucity of research focused on salivary bacteria analyzed through real-time polymerase chain reaction (qPCR) among adolescents. The current study determined the quantity of Streptococcus mutans (SM) and Lactobacillus (LB) in saliva obtained from Taiwanese adolescents and investigated the association between the oral bacteria and untreated dental caries. Materials and methods This cross-sectional study recruited Taiwanese students aged 10−18. Saliva was collected using a Salivette kit and then analyzed through qPCR. The relative quantification values of SM and LB were coded based on mean fold ratios, with values > 2 coded as high and other values coded as low. Untreated dental caries was assessed through standard oral examinations. Univariate and multivariate logistic regression models were used to estimate the association between the levels of bacteria in the saliva of the study participants and the presence of untreated caries. Results The study involved 421 adolescents. 56 (13.3%) had both SM and LB values of >2 and were coded as having high levels of bacteria, whereas the other 365 (86.7%) students were coded as having low levels. The multivariate logistic regression analysis revealed that adolescents who had high combined salivary SM and LB levels had an odds ratio of having untreated dental caries of 2.05 (95% CI = 1.09, 3.86, P = 0.027) compared with those who had low salivary SM and LB levels. Conclusion The results of the present study indicate that salivary SM and LB levels are significantly associated with adolescents having untreated caries.

Abstract:OBJECTIVE:To investigate the relationship between Mutans streptococcus (MS), Lactobacilli (LB), pH value and buffer capacity of saliva and early childhood caries (ECC).METHODS:A total of 178 children aged from 42 to 54 months were recruited from 14 urban kindergartens in Beijing. The ECC group contained 87 children with more than 5 decayed teeth, and the control group was composed of 91 caries-free children. Unstimulated (UWS, 2 ml) and stimulated (SWS, 2 ml) whole saliva were collected in each subject. The pH value and buffer capacity of saliva were measured using an electro-acidimeter (+/- 0.01pH).RESULTS:MS and LB were isolated from 96.6% and 79.3% of children with ECC, which were significantly higher than those (63.7%, 27.5%) of caries-free children (P < 0.05) respectively. The counts of MS and LB in children with ECC were approximately 10 times higher than that in caries-free children. Initial pH value and buffer capacity of SWS were significantly higher than that of UWS (P < 0.001) in both groups. The pH value and buffer capacity of both UWS and SWS in ECC children were significantly higher than caries-free children (P < 0.05). There were no significant correlations between MS, LB and pH value and buffer capacity of saliva in caries-free children. Significant correlation (r = 0.249, P < 0.05) was found between the numbers of MS and buffer capacity of stimulated saliva in ECC children.CONCLUSIONS:MS and LB were important pathogens for ECC. Lower initial pH value and buffer capacity of saliva may be an important factor of ECC.
