Abstract:As we enter the era of digital interdependence, artificial intelligence (AI) emerges as a key instrument to transform health care and address disparities and barriers in access to services. This viewpoint explores AI's potential to reduce inequalities in cancer care by improving diagnostic accuracy, optimizing resource allocation, and expanding access to medical care, especially in underserved communities. Despite persistent barriers, such as socioeconomic and geographical disparities, AI can significantly improve health care delivery. Key applications include AI-driven health equity monitoring, predictive analytics, mental health support, and personalized medicine. This viewpoint highlights the need for inclusive development practices and ethical considerations to ensure diverse data representation and equitable access. Emphasizing the role of AI in cancer care, especially in low- and middle-income countries, we underscore the importance of collaborative and multidisciplinary efforts to integrate AI effectively and ethically into health systems. This call to action highlights the need for further research on user experiences and the unique social, cultural, and political barriers to AI implementation in cancer care.

Abstract:Artificial intelligence (AI) stands at the threshold of revolutionizing clinical oncology, with considerable potential to improve early cancer detection and risk assessment, and to enable more accurate personalized treatment recommendations. However, a notable imbalance exists in the distribution of the benefits of AI, which disproportionately favour those living in specific geographical locations and in specific populations. In this Perspective, we discuss the need to foster the development of equitable AI tools that are both accurate in and accessible to a diverse range of patient populations, including those in low-income to middle-income countries. We also discuss some of the challenges and potential solutions in attaining equitable AI, including addressing the historically limited representation of diverse populations in existing clinical datasets and the use of inadequate clinical validation methods. Additionally, we focus on extant sources of inequity including the type of model approach (such as deep learning, and feature engineering-based methods), the implications of dataset curation strategies, the need for rigorous validation across a variety of populations and settings, and the risk of introducing contextual bias that comes with developing tools predominantly in high-income countries.

Abstract:The development of effective, targeted therapies for cancer remains one of the primary goals in precision medicine, requiring the input of multiple disciplines across the diagnostic and therapeutic landscape. Despite the significant role of cancer care in medicine, there is a forecasted shortfall of physicians able to diagnose and treat patients along their cancer diagnosis journey, which is only projected to worsen in the coming decades. Artificial intelligence (AI), and its manifestations in both machine learning and deep learning, are poised to unburden physicians from many laborious and repetitive tasks in diagnostic analyses, while also providing insight into prospective therapeutic options. AI will increasingly facilitate decision-making as well as improve the accuracy and relevance of therapeutic recommendations. This collaborative interaction between algorithms and physicians promises to enhance the field of precision medicine and further enable optimal patient care.

This review describes the current state of the field, with a specific focus on clinical integration. AI applications are structured according to cancer type and clinical domain, focusing on the four most common cancers and tasks of detection, diagnosis, and treatment. These applications encompass various data modalities, including imaging, genomics, and medical records. We conclude with a summary of existing challenges, evolving solutions, and potential future directions for the field. Significance: AI is increasingly being applied to all aspects of oncology, where several applications are maturing beyond research and development to direct clinical integration. This review summarizes the current state of the field through the lens of clinical translation along the clinical care continuum. Emerging areas are also highlighted, along with common challenges, evolving solutions, and potential future directions for the field.

Abstract:Upon a diagnosis, the clinical team faces two main questions: what treatment, and at what dose? Clinical trials' results provide the basis for guidance and support for official protocols that clinicians use to base their decisions. However, individuals do not consistently demonstrate the reported response from relevant clinical trials. The decision complexity increases with combination treatments where drugs administered together can interact with each other, which is often the case. Additionally, the individual's response to the treatment varies with the changes in their condition. In practice, the drug and the dose selection depend significantly on the medical protocol and the medical team's experience. As such, the results are inherently varied and often suboptimal. Big data and Artificial Intelligence (AI) approaches have emerged as excellent decision-making tools, but multiple challenges limit their application. AI is a rapidly evolving and dynamic field with the potential to revolutionize various aspects of human life. AI has become increasingly crucial in drug discovery and development. AI enhances decision-making across different disciplines, such as medicinal chemistry, molecular and cell biology, pharmacology, pathology, and clinical practice. In addition to these, AI contributes to patient population selection and stratification. The need for AI in healthcare is evident as it aids in enhancing data accuracy and ensuring the quality care necessary for effective patient treatment. AI is pivotal in improving success rates in clinical practice. The increasing significance of AI in drug discovery, development, and clinical trials is underscored by many scientific publications. Despite the numerous advantages of AI, such as enhancing and advancing Precision Medicine (PM) and remote patient monitoring, unlocking its full potential in healthcare requires addressing fundamental concerns. These concerns include data quality, the lack of well-annotated large datasets, data privacy and safety issues, biases in AI algorithms, legal and ethical challenges, and obstacles related to cost and implementation. Nevertheless, integrating AI in clinical medicine will improve diagnostic accuracy and treatment outcomes, contribute to more efficient healthcare delivery, reduce costs, and facilitate better patient experiences, making healthcare more sustainable. Abstract:In the field of cancer genomics, the broad availability of genetic information offered by next-generation sequencing technologies and rapid growth in biomedical publication has led to the advent of the big-data era. Integration of artificial intelligence (AI) approaches such as machine learning, deep learning, and natural language processing (NLP) to tackle the challenges of scalability and high dimensionality of data and to transform big data into clinically actionable knowledge is expanding and becoming the foundation of precision medicine. In this paper, we review the current status and future directions of AI application in cancer genomics within the context of workflows to integrate genomic analysis for precision cancer care. The existing solutions of AI and their limitations in cancer genetic testing and diagnostics such as variant calling and interpretation are critically analyzed. Publicly available tools or algorithms for key NLP technologies in the literature mining for evidence-based clinical recommendations are reviewed and compared. In addition, the present paper highlights the challenges to AI adoption in digital healthcare with regard to data requirements, algorithmic transparency, reproducibility, and real-world assessment, and discusses the importance of preparing patients and physicians for modern digitized healthcare. We believe that AI will remain the main driver to healthcare transformation toward precision medicine, yet the unprecedented challenges posed should be addressed to ensure safety and beneficial impact to healthcare.

Abstract:Artificial intelligence (AI) is a transformative technology that is capturing popular imagination and can revolutionize biomedicine. AI and machine learning (ML) algorithms have the potential to break through existing barriers in oncology research and practice such as automating workflow processes, personalizing care, and reducing healthcare disparities. Emerging applications of AI/ML in the literature include screening and early detection of cancer, disease diagnosis, response prediction, prognosis, and accelerated drug discovery. Despite this excitement, only few AI/ML models have been properly validated and fewer have become regulated products for routine clinical use. In this review, we highlight the main challenges impeding AI/ML clinical translation. We present different clinical use cases from the domains of radiology, radiation oncology, immunotherapy, and drug discovery in oncology. We dissect the unique challenges and opportunities associated with each of these cases. Finally, we summarize the general requirements for successful AI/ML implementation in the clinic, highlighting specific examples and points of emphasis including the importance of multidisciplinary collaboration of stakeholders, role of domain experts in AI augmentation, transparency of AI/ML models, and the establishment of a comprehensive quality assurance program to mitigate risks of training bias and data drifts, all culminating toward safer and more beneficial AI/ML applications in oncology labs and clinics.

Abstract:Abstract Several factors, including advances in computational algorithms, the availability of high-performance computing hardware, and the assembly of large community-based databases, have led to the extensive application of Artificial Intelligence (AI) in the biomedical domain for nearly 20 years. AI algorithms have attained expert-level performance in cancer research. However, only a few AI-based applications have been approved for use in the real world. Whether AI will eventually be capable of replacing medical experts has been a hot topic. In this article, we first summarize the cancer research status using AI in the past two decades, including the consensus on the procedure of AI based on an ideal paradigm and current efforts of the expertise and domain knowledge. Next, the available data of AI process in the biomedical domain are surveyed. Then, we review the methods and applications of AI in cancer clinical research categorized by the data types including radiographic imaging, cancer genome, medical records, drug information and biomedical literatures. At last, we discuss challenges in moving AI from theoretical research to real-world cancer research applications and the perspectives toward the future realization of AI participating cancer treatment.

Abstract:Abstract Judgement, as one of the core tenets of medicine, relies upon the integration of multilayered data with nuanced decision making. Cancer offers a unique context for medical decisions given not only its variegated forms with evolution of disease but also the need to take into account the individual condition of patients, their ability to receive treatment, and their responses to treatment. Challenges remain in the accurate detection, characterization, and monitoring of cancers despite improved technologies. Radiographic assessment of disease most commonly relies upon visual evaluations, the interpretations of which may be augmented by advanced computational analyses. In particular, artificial intelligence (AI) promises to make great strides in the qualitative interpretation of cancer imaging by expert clinicians, including volumetric delineation of tumors over time, extrapolation of the tumor genotype and biological course from its radiographic phenotype, prediction of clinical outcome, and assessment of the impact of disease and treatment on adjacent organs. AI may automate processes in the initial interpretation of images and shift the clinical workflow of radiographic detection, management decisions on whether or not to administer an intervention, and subsequent observation to a yet to be envisioned paradigm. Here, the authors review the current state of AI as applied to medical imaging of cancer and describe advances in 4 tumor types (lung, brain, breast, and prostate) to illustrate how common clinical problems are being addressed. Although most studies evaluating AI applications in oncology to date have not been vigorously validated for reproducibility and generalizability, the results do highlight increasingly concerted efforts in pushing AI technology to clinical use and to impact future directions in cancer care.

Abstract:The World Health Organization (WHO), in their 2022 report, identified cancer as one of the leading causes of death, accounting for about 16% of deaths worldwide. The Cancer-Moonshot community aims to reduce the cancer death rate by half in the next 25 years and wants to improve the lives of cancer-affected people. Cancer mortality can be reduced if detected early and treated appropriately. Cancers like breast cancer and cervical cancer have high cure probabilities when treated early in accordance with best practices. Integration of artificial intelligence (AI) into cancer research is currently addressing many of the challenges where medical experts fail to bring cancer to control and cure, and the outcomes are quite encouraging. AI offers many tools and platforms to facilitate more understanding and tackling of this life-threatening disease. AI-based systems can help pathologists in diagnosing cancer more accurately and consistently, reducing the case error rates. Predictive-AI models can estimate the likelihood for a person to get cancer by identifying the risk factors. Big data, together with AI, can enable medical experts to develop customized treatments for cancer patients. The side effects from this kind of customized therapy will be less severe in comparison with the generalized therapies. However, many of these AI tools will remain ineffective in fighting against cancer and saving the lives of millions of patients unless they are accessible and understandable to biologists, oncologists, and other medical cancer researchers. This paper presents the trends, challenges, and future directions of AI in cancer research. We hope that this paper will be of help to both medical experts and technical experts in getting a better understanding of the challenges and research opportunities in cancer diagnosis and treatment.

Abstract:Objectives: The rapid advances in artificial intelligence (AI), big data, and machine learning (ML) technologies hold promise for personalized, equitable cancer care and improved health outcomes within the context of cancer and beyond. Furthermore, integrating these technologies into cancer research has been effective in addressing many of the challenges for cancer control and cure. This can be achieved through the insights generated from massive amounts of data, in ways that can help inform decisions, interventions, and precision cancer care. AI, big data, and ML technologies offer, either in isolation or in combination, unconventional pathways that facilitate the better understanding and management of cancer and its impact on the person. The value of AI, big data, and ML technologies has been acknowledged and integrated within the Cancer Moonshot program in the U.S. and the EU Beating Cancer Plan in Europe. Data sources: Relevant studies on the topic have formed the basis for this article. Conclusion: In a shifting health care environment where cancer care is becoming more complex and demanding, big data and AI technologies can act as a vehicle to facilitating the care continuum. An increasing body of literature demonstrates their impactful contributions in areas such as treatment and diagnosis. These technologies, however, create additional requirements from health care professionals in terms of capacity and preparedness to integrate them effectively and efficiently in clinical practice. Therefore, there is an increasing need for investment and training in oncology to combat and overcome some of the challenges posed by cancer control. Implications for nursing practice: AI, big data, and ML are increasingly integrated in various aspects of health care. As a result, health care professionals, including nurses, will need to adjust in an ever-changing practice environment where these technologies have potential applications in clinical settings to improve risk stratification, early detection, and surveillance management of cancer patients.

Abstract:Artificial Intelligence (AI) technology is based on theory and development of computer systems able to perform tasks that normally require human intelligence. In this context, deep learning is a family of computational methods that allow an algorithm to program itself by learning from a large set of examples that demonstrate the desired behavior. Application of these methods to medical imaging can assist pathologists in the detection of cancer subtype, gene mutations and/or metastases for applying appropriate therapies. The purpose of this study is to show the emerging application of AI in medical imaging to detect lung and breast cancer. Moreover, this study shows the comparative evolutionary pathways of this emerging technology for three critical cancers: lung, breast and thyroid. A main finding of this study is the recognition that, since the late 1990, the sharp increase of technological trajectories of AI technology applied in cancer imaging seems to be driven by high rates of mortality of some types of cancer (e.g., lung and breast) in order to find new techniques for a more accurate detection, characterization and monitoring as well as to apply efficiently anticancer therapies that increase the progression-free survival of patients: the so-called mortality-driven AI technological trajectories. Results also suggest that this new technology can generate a technological paradigm shift for diagnostic assessment of any cancer type. However, application of these methods to medical imaging requires further assessment and validation to assist pathologists to increase the efficiency of their workflow in both routine tasks and critical cases of diagnostics.

AI sorts big data, predicting people’s reactions to specific treatments, the right combinations of drugs, and possible side effects, therefore increasing the efficiency of the treatment process and decreasing negative outcomes. This article briefly presents the ethical issues and concerns that might arise due to the integration of AI in society, such as the privacy of data, the issues of bias in the algorithms, and the issues of interpretability of the AI systems. Nevertheless, there is no doubt that AI can bring qualitative changes in cancer care based on its potential to enhance patient prognosis and reduce health care costs, as well as become a defining feature of the standard of care.

Abstract:Breast cancer is the most prevalent cancer worldwide, affecting both low- and middle-income countries, with a growing number of cases. In 2024, about 310,720 women in the U.S. are projected to receive an invasive breast cancer diagnosis, alongside 56,500 cases of ductal carcinoma in situ (DCIS). Breast cancer occurs in every country of the world in women at any age after puberty but with increasing rates in later life. About 65% of women with the BRCA1 and 45% with the BRCA2 gene variants develop breast cancer by age 70. While these genes account for 5% of breast cancers, their prevalence is higher in certain populations. Advances in early detection, personalised medicine, and AI-driven diagnostics are improving outcomes by enabling a more precise analysis, reducing recurrence, and minimising treatment side effects. Our paper aims to explore the vast applications of artificial intelligence within the diagnosis and treatment of breast cancer and how these advancements can contribute to elevating patient care as well as discussing the potential drawbacks of such integrations into modern medicine. We structured our paper as a non-systematic review and utilised Google Scholar and PubMed databases to review literature regarding the incorporation of AI in the diagnosis and treatment of non-palpable breast masses. AI is revolutionising breast cancer management by enhancing imaging, pathology, and personalised treatment. In imaging, AI can improve the detection of cancer in mammography, MRIs, and ultrasounds, rivalling expert radiologists in accuracy. In pathology, AI enhances biomarker detection, improving HER2 and Ki67 assessments. Personalised medicine benefits from AI's predictive power, aiding risk stratification and treatment response. AI also shows promise in triple-negative breast cancer management, offering better prognosis and subtype classification. However, challenges include data variability, ethical concerns, and real-world validation. Despite limitations, AI integration offers significant potential in improving breast cancer diagnosis, prognosis, and treatment outcomes.
