Abstract:It is generally accepted that human bipedal upright stance is achieved by feedback mechanisms that generate an appropriate corrective torque based on body-sway motion detected primarily by visual, vestibular, and proprioceptive sensory systems. Because orientation information from the various senses is not always available (eyes closed) or accurate (compliant support surface), the postural control system must somehow adjust to maintain stance in a wide variety of environmental conditions. This is the sensorimotor integration problem that we investigated by evoking anterior-posterior (AP) body sway using pseudorandom rotation of the visual surround and/or support surface (amplitudes 0.5-8 degrees ) in both normal subjects and subjects with severe bilateral vestibular loss (VL). AP rotation of body center-of-mass (COM) was measured in response to six conditions offering different combinations of available sensory information. Stimulus-response data were analyzed using spectral analysis to compute transfer functions and coherence functions over a frequency range from 0.017 to 2.23 Hz. Stimulus-response data were quite linear for any given condition and amplitude. However, overall behavior in normal subjects was nonlinear because gain decreased and phase functions sometimes changed with increasing stimulus amplitude. "Sensory channel reweighting" could account for this nonlinear behavior with subjects showing increasing reliance on vestibular cues as stimulus amplitudes increased. VL subjects could not perform this reweighting, and their stimulus-response behavior remained quite linear. Transfer function curve fits based on a simple feedback control model provided estimates of postural stiffness, damping, and feedback time delay. There were only small changes in these parameters with increasing visual stimulus amplitude. However, stiffness increased as much as 60% with increasing support surface amplitude. To maintain postural stability and avoid resonant behavior, an increase in stiffness should be accompanied by a corresponding increase in damping. Increased damping was achieved primarily by decreasing the apparent time delay of feedback control rather than by changing the damping coefficient (i.e., corrective torque related to body-sway velocity). In normal subjects, stiffness and damping were highly correlated with body mass and moment of inertia, with stiffness always about 1/3 larger than necessary to resist the destabilizing torque due to gravity. The stiffness parameter in some VL subjects was larger compared with normal subjects, suggesting that they may use increased stiffness to help compensate for their loss. Overall results show that the simple act of standing quietly depends on a remarkably complex sensorimotor control system.
Velocity dependence of sensory reweighting in human balance control

Abstract:The relative contributions of proprioceptive, vestibular, and visual sensory cues to balance control change depending on their availability and reliability. This sensory reweighting is classically supported by nonlinear sway responses to increasing visual surround and/or surface tilt amplitudes. However, recent evidence indicates that visual cues are reweighted based on visual tilt velocity rather than tilt amplitude. Therefore, we designed a study to specifically test the hypothesized velocity dependence of reweighting while expanding on earlier findings for visual reweighting by testing proprioceptive reweighting for standing balance on a tilting surface. Twenty healthy young adults stood with their eyes closed on a toes-up/-down tilting platform. We designed four pseudorandom tilt sequences with either a slow (S) or a fast (F) tilt velocity and different peak-to-peak amplitudes. We used model-based interpretations of measured sway characteristics to estimate the proprioceptive sensory weight (Wprop) within each trial. In addition, root-mean-square values of measured body center of mass sway amplitude (RMS) and velocity (RMSv) were calculated for each tilt sequence. Wprop, RMS, and RMSv values varied depending on the stimulus velocity, exhibiting large effects (all Cohen's d >1.10). In contrast, we observed no significant differences across stimulus amplitudes for Wprop (Cohen's d: 0.02-0.16) and, compared with the differences in velocity, there were much smaller changes in RMS and RMSv values (Cohen's d: 0.05-0.91). These results confirmed the hypothesized velocity, rather than amplitude, dependence of sensory reweighting.NEW & NOTEWORTHY This novel study examined the velocity dependence of sensory reweighting for human balance control using support surface tilt stimuli with independently varied amplitude and velocity. Estimates of the proprioceptive contribution to standing balance, derived from model-based interpretations of sway characteristics, showed greater sensitivity to changes in surface tilt velocity than surface tilt amplitude. These results support a velocity-based mechanism underlying sensory reweighting for human balance control.

Abstract:Objective Are people with a characteristically large physiological sway rendered particularly unstable when standing on a moving surface? Is postural sway in standing individuals idiosyncratic? In this study, we examine postural sway in individuals standing normally, and when subtle continuous sinusoidal disturbances are applied to their support platform. We calculate consistency between conditions to verify if sway can be considered characteristic of each individual. We also correlate two different aspects of participants’ responses to disturbance; their sway velocity and their regulation of body orientation. Methods Nineteen healthy adults (age 29.2 ± 3.2 years) stood freely on footplates coaxially aligned with their ankles and attached to a motorized platform. They had their eyes closed, and hips and knees locked with a light wooden board attached to their body. Participants either stood quietly on a fixed platform or on a slowly tilting platform (0.1 Hz sinusoid; 0.2 and 0.4 deg). Postural sway size was separated into two entities: (1) the spontaneous sway velocity component (natural random relatively rapid postural adjustments, RMS body angular velocity) and (2) the evoked tilt gain component (much slower 0.1 Hz synchronous tilt induced by the movement of the platform, measured as peak-to-peak (p-p) gain, ratio of body angle to applied footplate rotation). Results There was no correlation between the velocity of an individual’s sway and their evoked tilt gain ( r = 0.34, p = 0.15 and r = 0.30, p = 0.22). However, when considered separately, each of the two measurements showed fair to good absolute agreement within conditions. Spontaneous sway velocity consistently increased as participants were subjected to increasing disturbance. Participants who swayed more (or less) did so across all conditions [ICC (3,k) = 0.95]. Evoked tilt gain also showed consistency between conditions [ICC (3,k) = 0.79], but decreased from least to most disturbed conditions. Conclusion The two measurements remain consistent between conditions. Consistency between conditions of two very distinct unrelated measurements reflects the idiosyncratic nature of postural sway. However, sway velocity and tilt gain are not related, which supports the idea that the short-term regulation of stability and the longer-term regulation of orientation are controlled by different processes.

Abstract:Background: Falls and related injuries are critical issues in several disease states, as well as aging, especially when interactions between vestibular and visual sensory inputs are involved. Slow support surface tilt (0.6 deg/s) followed by subjective postural horizontal (SPH) assessments have been proposed as a viable method for assessing otolith contributions to balance control. Previous assessments of perceived body alignment to vertical, including subjective visual vertical, have suggested that visual inputs are weighted more when vestibular information is near the threshold and less reliable during slow body tilt. To date, no studies have examined the influence of visual stimuli on slow roll-tilt postural responses and the SPH. Therefore, this study investigated how dynamic visual cues, in the form of circular vection (CV), influence postural responses and the perception of the horizontal during and after support surface tilt. Methods: Ten healthy young adults (6 female, mean age 23) wore a head-mounted display while standing on a tilting platform. Participants were asked to remain upright for 30 s, during which (1) the visual scene rotated, inducing roll CV clockwise (CW) or counter-clockwise (CCW) at 60°/s; (2) the platform only (PO) rotated in roll to test SPH (0.6°/s, 2°, CW or CCW); (3) a combination of both; or (4) neither occurred. During SPH trials, participants used a hand-held device to reset the position of the platform to 0.8°/s to their perceived SPH. The angular motion of body segments was measured using pairs of light-emitting diodes mounted on the head, trunk and pelvis. Segment motion, prior to platform motion, was compared to that at peak body motion induced by platform motion and when SPH had been set. Results: When the support surface was tilted 2°, peak upper body tilt significantly increased for congruent CV and platform tilt and decreased at the pelvis for incongruent CV when compared to PO, leading to significant differences across body segments for congruent and incongruent conditions (p ≤ 0.008). During PO, participants' mean SPH deviated from horizontal by 0.2°. The pelvis deviated 0.2°, the trunk 0.3°, and the head 0.5° in the direction of initial platform rotation. When platform tilt and CV directions were congruent or incongruent, only head tilt at SPH reset under congruent conditions was significantly different from the PO condition (1.7° vs. 0.5°). Conclusions: Roll CV has a significant effect on phasic body responses and a less significant effect on tonic body responses to lateral tilt. The SPH of the support surface was not altered by CV. Responses during tilt demonstrated enhanced reactions for congruent and reduced reactions for incongruent CV, both different from responses to CV alone. Tonic body displacements associated with SPH were changed less than those during tilt and were only slightly larger than displacements for CV alone. This study supports the hypothesis of weighted multisensory integration during dynamic postural tasks being highly dependent on the direction of visual cues during tilt and less dependent on tonic SPH offsets. These techniques could be used to examine vestibular and visual interactions within clinical populations, particularly those with visual vertigo and dizziness.

Abstract:Human and humanoid posture control models usually rely on single or multiple degrees of freedom inverted pendulum representation of upright stance associated with a feedback controller. In models typically focused on the action between ankles, hips, and knees, the control of head position is often neglected, and the head is considered one with the upper body. However, two of the three main contributors to the human motion sensorium reside in the head: the vestibular and the visual system. As the third contributor, the proprioceptive system is distributed throughout the body. In human neurodegenerative brain diseases of motor control, like Progressive Supranuclear Palsy PSP and Idiopathic Parkinson's Disease IPD, clinical studies have demonstrated the importance of head motion deficits. This work specifically addresses the control of the head during a perturbed upright stance. A control model for the neck is proposed following the hypothesis of a modular posture control from previous studies. Data from human experiments are used to fit the model and retrieve sets of parameters representative of the behavior obtained in different conditions. The result of the analysis is twofold: validate the model and its underlying hypothesis and provide a system to assess the differences in posture control that can be used to identify the differences between healthy subjects and patients with different conditions. Implications for clinical pathology and application in humanoid and assistive robotics are discussed.

Identification of human balance control responses to visual inputs using virtual reality

Abstract:Human upright balance is maintained through feedback mechanisms that use a variety of sensory modalities. Vision senses information about the position and velocity of the visual surround motion to improve balance by reducing the sway evoked by external disturbances. This study characterized the effects of visual information on human anterior-posterior body sway in upright stance by presenting perturbations through a virtual reality system. This made it possible to use a new visual perturbation signal, based on trapezoidal velocity pulses, whose amplitude and velocity could be controlled separately. To date, the influences of visual field position and velocity have only been studied independently due to the experimental limitations. The hip displacement, ankle torques, shank angles, and surface EMGs of four major ankle muscles were measured bilaterally as outputs. We found that the root mean square (RMS) hip displacement (body angle) increased systematically with visual input amplitude. However, for each amplitude, the RMS body angle increased when input velocity was changed from 2 to 5 degrees per second (dps) and then decreased from 5 to 10 dps. Spectral analysis was used to compute frequency response over a frequency range from 0.04 to 0.6 Hz. The gain of body sway relative to the perturbation increased with frequency, whereas the coherence declined. Moreover, as the stimulus amplitude increased, the gain generally decreased, whereas the mean coherence values always increased. The mean gains and mean coherence values were greatest for the velocity of 5 dps. This study presents a novel experimental approach to study human postural control and augments our knowledge of how visual information is processed in the central nervous system to maintain balance.NEW & NOTEWORTHY In this paper, we developed a new methodological approach to study the effects of visual information on dynamic body sway. We used VR to apply visual perturbations to induce AP body sway. We designed a new visual stimulus waveform based on trapezoidal velocity pulses whose peak-to-peak amplitude and velocity could be modulated independently. Subsequently, we investigated how the amplitude and velocity of visual field motion influence the postural responses evoked in healthy adults.
Sensory integration of a light touch reference in human standing balance

Abstract:In upright stance, light touch of a space-stationary touch reference reduces spontaneous sway. Moving the reference evokes sway responses which exhibit non-linear behavior that has been attributed to sensory reweighting. Reweighting refers to a change in the relative contribution of sensory cues signaling body sway in space and light touch cues signaling finger position with respect to the body. Here we test the hypothesis that the sensory fusion process involves a transformation of light touch signals into the same reference frame as other sensory inputs encoding body sway in space, or vice versa. Eight subjects lightly gripped a robotic manipulandum which moved in a circular arc around the ankle joint. A pseudo-randomized motion sequence with broad spectral characteristics was applied at three amplitudes. The stimulus was presented at two different heights and therefore different radial distances, which were matched in terms of angular motion. However, the higher stimulus evoked a significantly larger sway response, indicating that the response was not matched to stimulus angular motion. Instead, the body sway response was strongly related to the horizontal translation of the manipulandum. The results suggest that light touch is integrated as the horizontal distance between body COM and the finger. The data were well explained by a model with one feedback loop minimizing changes in horizontal COM-finger distance. The model further includes a second feedback loop estimating the horizontal finger motion and correcting the first loop when the touch reference is moving. The second loop includes the predicted transformation of sensory signals into the same reference frame and a non-linear threshold element that reproduces the non-linear sway responses, thus providing a mechanism that can explain reweighting.
Reactive Balance Control in Response to Perturbation in Unilateral Stance: Interaction Effects of Direction, Displacement and Velocity on Compensatory Neuromuscular and Kinematic Responses

Abstract:Unexpected sudden perturbations challenge postural equilibrium and require reactive compensation. This study aimed to assess interaction effects of the direction, displacement and velocity of perturbations on electromyographic (EMG) activity, centre of pressure (COP) displacement and joint kinematics to detect neuromuscular characteristics (phasic and segmental) and kinematic strategies of compensatory reactions in an unilateral balance paradigm. In 20 subjects, COP displacement and velocity, ankle, knee and hip joint excursions and EMG during short (SLR), medium (MLR) and long latency response (LLR) of four shank and five thigh muscles were analysed during random surface translations varying in direction (anterior-posterior (sagittal plane), medial-lateral (frontal plane)), displacement (2 vs. 3 cm) and velocity (0.11 vs. 0.18 m/s) of perturbation when balancing on one leg on a movable platform. Phases: SLR and MLR were scaled to increased velocity (P<0.05); LLR was scaled to increased displacement (P<0.05). Segments: phasic interrelationships were accompanied by segmental distinctions: distal muscles were used for fast compensation in SLR (P<0.05) and proximal muscles to stabilise in LLR (P<0.05). Kinematics: ankle joints compensated for both increasing displacement and velocity in all directions (P<0.05), whereas knee joint deflections were particularly sensitive to increasing displacement in the sagittal (P<0.05) and hip joint deflections to increasing velocity in the frontal plane (P<0.05). COP measures increased with increasing perturbation velocity and displacement (P<0.05). Interaction effects indicate that compensatory responses are based on complex processes, including different postural strategies characterised by phasic and segmental specifications, precisely adjusted to the type of balance disturbance. To regain balance after surface translation, muscles of the distal segment govern the quick regain of equilibrium; the muscles of the proximal limb serve as delayed stabilisers after a balance disturbance. Further, a kinematic distinction regarding the compensation for balance disturbance indicated different plane- and segment-specific sensitivities with respect to the determinants displacement and velocity.

Abstract:Postural control in quiet standing is often explained by a reflexive response to optical flow, the apparent motion of environmental objects in a visual scene. However, moving room experiments show that even small-amplitude body sway can evoke odd sensations or motion sickness, indicating that a consciousness factor may also be involved. Studies targeting perception of self-motion, vection, typically use rapid visual stimuli moving in a single direction to maintain a constant feeling of vection, and there are few studies of vection using low-speed sinusoidal visual stimuli similar to human pendular movement. In the present study we searched for changes in postural control during periods of vection during quiet standing. Participants (N = 19, age = 20.4 ±1.1 years) were shown dynamic visual stimuli in the form of sinusoidally expanding and contracting random dots, and the stimuli speed and visual field were manipulated. Posture was continually evaluated using Center of Pressure (CoP) measurements. Participants were also asked to report feelings of vection, both by pressing a button during the trial and through an overall rating at the end of each trial. Using repeated-measures ANOVA, we assessed changes in the CoP and vection variables between experimental conditions, as well as possible interactions between the variables. The results show that postural reaction and vection were both affected by the visual stimuli and varied with speed. The peripheral visual field was found to couple to stronger feeling of vection and better quality of postural control. However, no significant relationship between postural control and vection, nor evidence of vection interaction to the relationship between optical flow and postural control, was found. Based on our results we conclude that for postural stability during quiet standing, visual cues dominate over any potential consciousness factor arising due to vection.

Abstract:Body sway responses evoked by a horizontal acceleration of a level and firm support surface are particular in that the vestibular information on body-space angle BS resembles the proprioceptive information on body-foot angle BF. We compared corresponding eyes-closed responses of vestibular-able (VA) and vestibular-loss (VL) subjects, postulating a close correspondence. In contradistinction to previous studies, we used an unpredictable (pseudorandom) stimulus and found that the eyes-closed and eyes-open responses of the VA closely resembled those of the VL subjects, as expected. We further conclude that the vestibular signals coding head linear translation in VA subjects has in this case too little functional relevance to cause a notable difference between the subject groups.

Abstract:The role of vision in stabilizing balance has been studied exhaustively. Other studies have shown that non-supportive light touch of the fingertip with a surface also can significantly stabilize postural balance. We have studied how vision and cutaneous information jointly affect balance. We used a head-mounted display to simulate a virtual room that rotated about a vertical axis centered with the standing subjects z-axis. Subjects viewing the displays rotational displacement soon experienced self-motion and displacement. We assessed how the moving visual input destabilized posture and how it interacted with touch cues that stabilized posture. A novel result is how balance is influenced by the onset of visual motion and the illusion of self-rotation. We discovered that motion perceptions are coupled with stochastic aspects of balance. Changes in the perception of types of motion (none, environment, and self-rotation) distinctively influence metrics that encode for the stochasticity of balance and do not influence those that filter the stochasticity out. We reconfirmed the significant effects of touch in stabilizing balance and discovered how it interacts with the visual perception of motion. We also found lingering effects of past motion perception, which keep influencing the stochasticity of balance even when visual motion is long stopped. Our findings provide insights into multisensory interaction effects in postural balance and suggest novel future research directions.
Is it me or the train moving? Humans resolve sensory conflicts with a nonlinear feedback mechanism in balance control

Abstract:Humans use multiple sensory systems to estimate body orientation in space. A predominant concept for the underlying multisensory integration (MSI) is the linear summation of weighted inputs from individual sensory systems. The sensory contributions change depending on context and environmental conditions. These changes are typically attributed to 'reweighting' by some higher order mechanism. We provide evidence for a conceptually different mechanism that combines 1) sensory inputs with fixed weights and 2) multisensory corrections, if the reference of the sensory inputs move in space and are therefore unreliable. This reconstruction of the sensory reference frame motion (RFM) allows humans to use vision and automatically counteract erroneous inputs, e.g. when looking at a moving train. The proposed RFM estimator contains a nonlinear dead-zone that blocks corrections at slow velocities. We first demonstrate that this mechanism accounts for changes in sensory contributions. Secondly, we hypothesized that such a nonlinearity would distort balance responses to perturbations in a very specific way. We predicted such distortions in body sway using a balance control model. Experiments using visual scene movements as a specific implementation of RFMs confirmed the predictions. The findings indicate that the central nervous system resolves sensory conflicts by an internal reconstruction of the cause of the conflict. Thus, the mechanism links the concept of causal inference to shifts in sensory contributions, providing a cohesive picture of MSI for the estimation of body orientation in space.

Different mechanisms of contextual inference govern associatively learned and sensory-evoked postural responses

Abstract:Human standing balance relies on the continuous monitoring and integration of sensory signals to infer our body's motion and orientation within the environment. However, when sensory information is no longer contextually relevant to balancing the body (e.g., when sensory and motor signals are incongruent), sensory-evoked balance responses are rapidly suppressed, much earlier than any conscious perception of changes in balance control. Here, we used a robotic balance simulator to assess whether associatively learned postural responses are similarly modulated by sensorimotor incongruence and contextual relevance to postural control. Twenty-nine participants in three groups were classically conditioned to generate postural responses to whole-body perturbations when presented with an initially neutral sound cue. During catch and extinction trials, participants received only the auditory stimulus but in different sensorimotor states corresponding to their group: 1) during normal active balance, 2) while immobilized, and 3) throughout periods where the computer subtly removed active control over balance. In the balancing and immobilized states, conditioned responses were either evoked or suppressed, respectively, according to the (in)ability to control movement. Following the immobilized state, conditioned responses were renewed when balance was restored, indicating that conditioning was retained but only expressed when contextually relevant. In contrast, conditioned responses persisted in the computer-controlled state even though there was no causal relationship between motor and sensory signals. These findings suggest that mechanisms responsible for sensory-evoked and conditioned postural responses do not share a single, central contextual inference and assessment of their relevance to postural control, and may instead operate in parallel.

These changes might be read as the small beginnings of a larger and more gradual swing back toward a greater respect for hand and eye and image building in the brain. For some time the major contributions of visual thinkers have been eclipsed in many fields by theoretical approaches that did not lend themselves to pictures or images or imagined models or hands-on manipulation. For a long time, we have been told with confidence that visual approaches were old fashioned and somehow primitive. Modern scientists and mathematicians, we have been told, did not need images. Pictures and diagrams were mainly for non-professionals and lay persons. But we now see that things may be going back in the opposite direction. With new visualization technologies and an emerging sense of the missed opportunities using the old narrow methods researchers in many fields are becoming aware that in order to do really creative work, they may need to go back to visual approaches once again. Perhaps we are traveling back in spirit to an earlier time, a time where much of the most advanced and creative work is done by visual thinkers using visual methods and technologies. Once again, pictures may not only be for children!
Dynamic changes of brain networks during standing balance control under visual conflict

Abstract:Stance balance control requires a very accurate tuning and combination of visual, vestibular, and proprioceptive inputs, and conflict among these sensory systems may induce posture instability and even falls. Although there are many human mechanics and psychophysical studies for this phenomenon, the effects of sensory conflict on brain networks and its underlying neural mechanisms are still unclear. Here, we combined a rotating platform and a virtual reality (VR) headset to control the participants' physical and visual motion states, presenting them with incongruous (sensory conflict) or congruous (normal control) physical-visual stimuli. Further, to investigate the effects of sensory conflict on stance stability and brain networks, we recorded and calculated the effective connectivity of source-level electroencephalogram (EEG) and the average velocity of the plantar center of pressure (COP) in healthy subjects (18 subjects: 10 males, 8 females). First, our results showed that sensory conflict did have a detrimental effect on stance posture control [sensor F (1, 17) = 13.34, P = 0.0019], but this effect decreases over time [window*sensor F (2, 34) = 6.72, P = 0.0035]. Humans show a marked adaptation to sensory conflict. In addition, we found that human adaptation to the sensory conflict was associated with changes in the cortical network. At the stimulus onset, congruent and incongruent stimuli had similar effects on brain networks. In both cases, there was a significant increase in information interaction centered on the frontal cortices (p < 0.05). Then, after a time window, synchronized with the restoration of stance stability under conflict, the connectivity of large brain regions, including posterior parietal, visual, somatosensory, and motor cortices, was generally lower in sensory conflict than in controls (p < 0.05). But the influence of the superior temporal lobe on other cortices was significantly increased. Overall, we speculate that a posterior parietal-centered cortical network may play a key role in integrating congruous sensory information. Furthermore, the dissociation of this network may reflect a flexible multisensory interaction strategy that is critical for human posture balance control in complex and changing environments. In addition, the superior temporal lobe may play a key role in processing conflicting sensory information.

Abstract:The present study aimed at investigating the effects of an artificial head position-based tongue-placed electrotactile biofeedback on postural control during quiet standing under different somatosensory conditions from the support surface. Eight young healthy adults were asked to stand as immobile as possible with their eyes closed on two Firm and Foam support surface conditions executed in two conditions of No-biofeedback and Biofeedback. In the Foam condition, a 6-cm thick foam support surface was placed under the subjects' feet to alter the quality and/or quantity of somatosensory information at the plantar sole and the ankle. The underlying principle of the biofeedback consisted of providing supplementary information about the head orientation with respect to gravitational vertical through electrical stimulation of the tongue. Centre of foot pressure (CoP) displacements were recorded using a force platform. Larger CoP displacements were observed in the Foam than Firm conditions in the two conditions of No-biofeedback and Biofeedback. Interestingly, this destabilizing effect was less accentuated in the Biofeedback than No-biofeedback condition. In accordance with the sensory re-weighting hypothesis for balance control, the present findings evidence that the availability of the central nervous system to integrate an artificial head orientation information delivered through electrical stimulation of the tongue to limit the postural perturbation induced by alteration of somatosensory input from the support surface.

The effect of a concurrent cognitive task on cortical potentials evoked by unpredictable balance perturbations

Abstract:Background: Although previous studies suggest that postural control requires attention and other cognitive resources, the central mechanisms responsible for this relationship remain unclear. To address this issue, we examined the effects of altered attention on cortical activity and postural responses following mechanical perturbations to upright stance. We hypothesized that cortical activity would be attenuated but not delayed when mechanical perturbations were applied during a concurrent performance of a cognitive task (i.e. when attention was directed away from the perturbation). We also hypothesized that these cortical changes would be accompanied by alterations in the postural response, as evidenced by increases in the magnitude of anteroposterior (AP) centre of pressure (COP) peak displacements and tibialis anterior (TA) muscle activity. Healthy young adults (n = 7) were instructed to continuously track (cognitive task) or not track (control task) a randomly moving visual target using a hand-held joystick. During each of these conditions, unpredictable translations of a moving floor evoked cortical and postural responses. Scalp-recorded cortical activity, COP, and TA electromyographic (EMG) measures were collected. Results: Results revealed a significant decrease in the magnitude of early cortical activity (the N1 response, the first negative peak after perturbation onset) during the tracking task compared to the control condition. More pronounced AP COP peak displacements and EMG magnitudes were also observed for the tracking task and were possibly related to changes in the N1 response. Conclusion: Based on previous notions that the N1 response represents sensory processing of the balance disturbance, we suggest that the attenuation of the N1 response is an important central mechanism that may provide insight into the relationship between attention and postural control.

Abstract:To date, the human balance assessment is mainly achieved by measuring the center of pressure (COP) trajectory of upright posture. However, COP-based metrics provide inconclusive results under certain circumstances, due to the lack of relevance with the body balance mechanism. This paper developed a novel framework to measure the variability in body sway based on the Poincaré analysis to enhance the balance assessment. It provided nonlinear parameters SD1, SD2, and their ratio SD1/SD2 as quantitative indicators of the local and global balance regulation capacities. 136 healthy adults were categorized into young and elderly groups and received test on a force platform. COP signals were recorded for 30 s under eyes-opened and eyes-closed vision conditions. The results were analyzed using the two-way mixed-design analysis of variance and show that the technology is able to reveal the effect of age and vision on the balance control by SD1, SD2, and SD1/SD2. SD1 mainly responded to vision, and SD2 was more influenced by age. Due to the information about the subtle adjustment during quiet standing, these Poincaré indexes surpassed the conventional COP measurements in demonstrating the balance control behavior. It can be concluded that the supposed method of estimating the local and global variabilities is a valuable approach for the balance assessment.

Abstract:Human balance control relies on various sensory modalities, and conflict of sensory input may result in postural instability. Virtual reality (VR) technology allows to train balance under conflicting sensory information by decoupling visual from somatosensory and vestibular systems, creating additional demands on sensory reweighting for balance control. However, there is no metric for the design of visual input manipulations that can induce persistent sensory conflicts to perturb balance. This limits the possibilities to generate sustained sensory reweighting processes and design well-defined training approaches. This study aimed to investigate the effects that different onset characteristics, amplitudes and velocities of visual input manipulations may have on balance control and their ability to create persistent balance responses. Twenty-four young adults were recruited for the study. The VR was provided using a state-of-the-art head-mounted display and balance was challenged in two experiments by rotations of the visual scene in the frontal plane with scaled constellations of trajectories, amplitudes and velocities. Mean center of pressure speed was recorded and revealed to be greater when the visual input manipulation had an abrupt onset compared to a smooth onset. Furthermore, the balance response was greatest and most persistent when stimulus velocity was low and stimulus amplitude was large. These findings show clear dissociation in the state of the postural system for abrupt and smooth visual manipulation onsets with no indication of short-term adaption to abrupt manipulations with slow stimulus velocity. This augments our understanding of how conflicting visual information affect balance responses and could help to optimize the conceptualization of training and rehabilitation interventions.

Abstract:Somatosensory feedback is essential for motor control, yet sensations from one's own movements are often suppressed. Despite extensive research on movement-induced tactile suppression, its mechanisms remain unclear. Most studies focus on simple upper-limb movements, leaving the generalization to other body parts unexplored. This study examines tactile processing on the lower limb during balance control by varying feedback processing demands. Participants experienced visual perturbations in a virtual room challenging their posture. Tactile sensitivity was assessed using vibrotactile stimuli to the lower leg at different times around the perturbation. We found that postural behavior is both predictively tuned before and reactively adjusted to expected perturbations. Our results provide evidence that tactile sensitivity changes according to feedback processing demands on the lower limb. Such dynamic sensory modulation could reflect the continuous up- and down-regulation of feedback signals to accomplish the task at hand.
