Abstract:Background Social media has become a dominant educational resource for breast reconstruction patients. Rather than passively consuming information, patients interface directly with other users and healthcare professionals. While online information for breast reconstruction has been analyzed previously, a robust analysis of patient questions on online forums has not been conducted. In this study the authors use a machine learning approach to analyze and categorize online patient questions regarding breast reconstruction. Methods Realself.com was accessed and questions pertaining to breast reconstruction were extracted. Data collected included date of question, poster's location, question header, question text, and available tags. Questions were analyzed and categorized by two independent reviewers. Results 522 preoperative questions were analyzed. Geographic analysis is displayed in Fig. 1. Questions were often asked in the pre-mastectomy period (38.3%), however, patients with tissue expanders currently in place made up 28.5%. Questions were often related to reconstructive method (23.2%), implant selection (19.5%), tissue expander concerns (16.7%). Questions asked in the post-lumpectomy period were significantly more likely to be related to insurance/cost and reconstructive candidacy (p < 0.01). The "Top 6" patient questions were determined by machine learning analysis, the most common of which was "Can I get good results going direct to implant after mastectomy?" Conclusions Analysis of online questions provides valuable insight and may help inform our educational approach to our breast reconstruction patients. Our findings suggest that questions are common throughout the reconstructive process, and do not end after the initial consultation. Patients most often want more information on their reconstructive options, implant selection, and the tissue expansion process.

Abstract:Background: Online reviews have become modern versions of the word-of-mouth recommendation, and prospective patients are increasingly consulting them before making decisions about their surgical care. The authors’ objectives were to (1) identify trends in the use of online reviews, and (2) important reasons for patient satisfaction and dissatisfaction with aesthetic surgery. The authors selected breast augmentation as the primary procedure of interest. Methods: Reviews of the top 10 to 20 most reviewed plastic surgeons in each of six large metropolitan areas were obtained from Google, Yelp, and RealSelf. Reviews were assessed for predefined dimensions of satisfaction and dissatisfaction. Results: A total of 1077 breast augmentation reviews were obtained. Ratings were distributed bimodally, with peaks at five stars and one star. The majority of reviews were positive (87.5 percent). Relative popularity of Google versus Yelp varied across geographic regions, and average rating varied by platform. Between 2011 and 2016, the number of online reviews for breast augmentation grew at an average rate of 42.6 percent per year. Aesthetic outcome was the most commonly cited dimension (69.8 percent of reviews), whereas cost was mentioned in only 7.8 percent of reviews. A substantial minority of negative Yelp (37 percent) and Google (9.4 percent) reviews were written by patients who did not actually undergo surgery. Free-text analysis of heterogeneous reviews (containing positive and negative attributes) classified dimensions as critical, redeemable, or protective. Conclusion: As the influence of online review platforms continues to grow, understanding drivers of positive and negative reviews may help surgeons improve patient satisfaction.

Capitalizing on social media: An evaluation of the public's preferences for plastic surgery social media content

Abstract:Background: The effectiveness of utilizing social media platforms to promote clinical practices and attract patients has been well established. This study aimed to assess what plastic surgery social media content and educational material the public is most responsive to. Methods: An anonymous 25-question survey was distributed using REDCap and Amazon's Mechanical Turk to ascertain demographic information, patterns of social media use, levels of interest in plastic surgery, and preferences for plastic surgery content. Results: Of 401 participants, the typical respondent was between 25 and 34 years old and on social media daily. Almost half of the respondents (46.1%) have intentionally viewed plastic surgery content on social media, of which most used Instagram (71.1%) and Facebook (55.4%). Participants grouped as either younger or older than 35 years old were equally likely to have viewed plastic surgery content (p = 0.33). Content categories with the highest interest were before and after results (mean Likert weight 4.00 ± 1.10), patient testimonials (3.73 ± 1.15), and recovery process (3.67 ± 1.14). Content on celebrities (2.89 ± 1.17), comedic videos (2.79 ± 1.19), and surgeons' private lives (2.51 ± 1.08) received negative interest. Photo posts (51.4%) were preferred more than video posts (27.2%). Before and after results on social media were the most influential factor in the selection of a plastic surgeon (45.9%). Conclusions: The importance of social media for plastic surgeons to be able to interact with patients is at unprecedented heights. Understanding patterns of the public's social media content preferences will help plastic surgeons optimize their social media reach and influence their target audience.
Preoperative Patient Guidance and Education in Aesthetic Breast Plastic Surgery: A Novel Proposed Application of Artificial Intelligence Large Language Models

Abstract:Background: At a time when Internet and social media use is omnipresent among patients in their self-directed research about their medical or surgical needs, artificial intelligence (AI) large language models (LLMs) are on track to represent hallmark resources in this context. Objectives: The authors aim to explore and assess the performance of a novel AI LLM in answering questions posed by simulated patients interested in aesthetic breast plastic surgery procedures. Methods: A publicly available AI LLM was queried using simulated interactions from the perspective of patients interested in breast augmentation, mastopexy, and breast reduction. Questions posed were standardized and categorized under aesthetic needs inquiries and awareness of appropriate procedures; patient candidacy and indications; procedure safety and risks; procedure information, steps, and techniques; patient assessment; preparation for surgery; postprocedure instructions and recovery; and procedure cost and surgeon recommendations. Using standardized Likert scales ranging from 1 to 10, 4 expert breast plastic surgeons evaluated responses provided by AI. A postparticipation survey assessed expert evaluators' experience with LLM technology, perceived utility, and limitations. Results: The overall performance across all question categories, assessment criteria, and procedures examined was 7.3/10 ± 0.5. Overall accuracy of information shared was scored at 7.1/10 ± 0.5; comprehensiveness at 7.0/10 ± 0.6; objectivity at 7.5/10 ± 0.4; safety at 7.5/10 ± 0.4; communication clarity at 7.3/10 ± 0.2; and acknowledgment of limitations at 7.7/10 ± 0.2. With regards to performance on procedures examined, the model's overall score was 7.0/10 ± 0.8 for breast augmentation; 7.6/10 ± 0.5 for mastopexy; and 7.4/10 ± 0.5 for breast reduction. The score on breast implant-specific knowledge was 6.7/10 ± 0.6. Conclusions: Albeit not without limitations, AI LLMs represent promising resources for patient guidance and patient education. The technology's machine learning capabilities may explain its improved performance efficiency.
Which Groups of Plastic Surgery Patients Are Impacted by Social Media Use? An In-Depth Review of Social Media Engagement

Abstract:Background: Empowerment is the process by which patients gain greater control of their health through active and informed decision-making. Greater patient empowerment has been shown to be positively correlated with improved health care outcomes and experiences. It is unclear how social media affect plastic and reconstructive surgery (PRS) patients' health care decision-making. This study aimed to help quantify how social media sites influence levels of PRS patient empowerment. Methods: In this cross-sectional study, a modified Cyber Info-Decisional Empowerment Scale (CIDES) survey was distributed through Amazon Mechanical Turk (MTurk) to US adults. Sociodemographic characteristics, PRS history, and social media usage data were collected. Wilcoxon signed-rank and Kruskal-Wallis tests were used to assess for heterogeneity for categorical variables. ANOVA and t tests were used to evaluate differences in means for Likert scale-based responses. Results: A total of 473 survey responses were included. The participants were grouped based on their surgical history: cosmetic [187 (39.5%)], reconstructive [107 (22.6%)], both cosmetic and reconstructive [36 (7.6%)], or non-PRS [143 (30.2%)]. There was increased empowerment depending on the online resources used. Social media use was associated with significantly greater empowerment in six of seven CIDES categories. Of the social media platforms, Facebook was associated with higher empowerment in three of seven CIDES categories. Conclusion: Social media use appears to have a positive impact on PRS patient empowerment, which may reflect better patient decision-making and autonomy when consulting with their plastic surgeon.

Abstract:The problems posed by exercise at high altitude for oxygen diffusion across the blood-gas barrier have been recognized since the early part of this century. But the successful ascent of Mt. Everest (altitude 8848 m) by two climbers without supplementary oxygen in 1978 focused attention on this issue. We have therefore carried out a theoretical study of gas exchange under these conditions of extreme hypoxia. Calculations of oxygenation along the pulmonary capillaries show that, even at rest, there is an alveolar-end capillary PO2 of about 6 torr caused by diffusion limitation, and this widens rapidly on mild exercise. As the oxygen uptake is increased, the PO2 of mixed venous blood falls to very low values. If we assume a minimal value of 15 torr, a maximal oxygen uptake on the summit of less than 700 ml/min is predicted. VO2max is extremely sensitive to barometric pressure, and to a lesser extent to lung diffusing capacity.

Abstract:Abstract Background Reduction mammoplasty relieves macromastia symptoms while improving breast aesthetics, though the ideal breast aesthetically has been shown to differ culturally in previous crowdsourcing studies. Better understanding these differences can aid in setting postoperative expectations. Objectives The aim of this study was to characterize the ideal reduction mammoplasty according to demographics such as gender, ethnicity, socioeconomic status, and education. Methods A crowdsourcing platform was used to collect 10,169 de-identified responses. Users completed one of three surveys, either a preoperative, postoperative, or preoperative and postoperative paired survey. The preoperative and postoperative surveys addressed 10 breast measurements including upper breast slope, projection proportion, nipple position, breast width, and breast fullness. The paired pre- and postoperative survey assessed nipple areolar complex (NAC), chest fit, symmetry improvement, and scarring. Results Preoperative images were rated more aesthetic than postoperative images. This was consistent across all demographics evaluated. Female, African American, Asian, participants aged 55+, and participants with no high school degree or a graduate degree found the most improvement in breast symmetry (p = 0.001, p = 0.002, p = 0.027, p < 0.001, p = 0.01). Male and Hispanic participants were most likely to see no change in symmetry (p = 0.008, p = 0.04), and South Asian participants found breasts less symmetric postoperative (p < 0.001). There were significant demographic differences in aesthetic ratings of NAC, scarring, and breast fit. Conclusions Perceived breast aesthetics after reduction mammoplasty vary significantly across demographics including gender, ethnicity, age, socioeconomic status, and educational achievement. Surgeons should consider demographics when planning each patient’s reduction mammoplasty.

A Comparative Assessment of Three Planes of Implant Placement in Breast Augmentation: A Bayesian Analysis

Abstract:Background: Techniques based on three planes of implant placement, including the subglandular (SG), subpectoral (SP), and subfascial (SF) planes are used for breast augmentation. The placement that offers the greatest balance of risks and benefits is unclear. This study presents a systematic review with a Bayesian network meta-analysis to compare different implant placement techniques for augmentation mammaplasty. Methods: A systematic literature search was performed. We estimated the odds ratios (ORs) for capsular contractures, hematomas, seromas, infections, reoperation rates, rippling, nipple numbness, malplacements, ruptures, and asymmetry among the different interventions. Muscle movement events and satisfaction rates were also evaluated. Results: A total of 19 studies (25,744 cases) were included. SG placement significantly increased the incidence of capsular contractures (SP vs. SG: OR 0.42; 95% credible interval [CrI] 0.28-0.63; SF vs. SG: OR 0.41; 95% CrI 0.17-0.97), hematomas (SF vs. SG: OR 0.22; 95% CrI 0.06-0.63), and seromas (SF vs. SG: OR 0.04; 95% CrI 0.00-0.81) compared to other placement techniques. Muscle movement only occurred in the SP group, but it did not increase the risk of subsequent malplacements, asymmetries, or ruptures. Most patients were highly satisfied with their surgical results. Comparisons did not show significant differences in the remaining results. Conclusions: Our evidence suggests that SG placement increases the risk of capsular contractures, hematomas, and seromas. The SP and SF planes were safe and effective for controlling total complication rates and achieving high satisfaction rates; however, the long-term benefits of the SF technique require further research. Abstract:Abstract: In order to determine highly immunogenic severe acute respiratory syndrome coronavirus (SARS‐CoV) epitope peptides capable of inducing long‐lasting immunity, we first screened immunoglobulin‐G (IgG) antibodies reactive to 197 different overlapping 15‐mers from the SARS‐CoV proteins in the sera of three infected patients. Forty‐two peptides among them were reactive to the sera from all three patients. Consequently, we tested for the reactivity of these 42 peptides to patients' sera (n = 45) at 6‐month post‐infection. The significantly higher levels of IgG antibodies specific to three (S791, M207 and N161) of 42 peptides were detectable in the post‐infection sera from 23 (51%), 27 (60%) and 19 (42%) of 45 patients, respectively. These three peptides, recognized by their long‐lasting immunity, may provide a better understanding of the immunogenicity of SARS‐CoV.

Abstract:Background: Breast image plays an important role in a woman's identity. Breast aesthetics has strong social, emotional, and cultural connotations. Objectives: We aimed to understand one's breast satisfaction, what is seen as ideal, and the gap between these two facets. We also studied the influence of several factors on these conceptions, including clinical and demographic characteristics, self-esteem, social media use, and pornography. We also studied the motivational factors driving the recourse to surgery. Methods: An online questionnaire of 64 questions divided into 5 sections was shared on social media to all women over the age of 18. Three versions were available: French, Dutch, and English. The questionnaire was accessible for 45 days (from 12/18/2021 to 01/31/2022). Results: A total of 382 complete responses were analyzed. The average age was 30.6 years; the most frequent breast size was a B cup (29.06%), while the size considered ideal was a C cup (49.48%). Social media and pornography seem to influence the conception of ideal breast; the subgroup that uses them tends to choose a larger breast size as ideal. Smoking and the education level also seem to influence this conception. Furthermore, 26.96% of women would benefit from breast surgery if they did not have to consider the financial aspect. Conclusions: The ideal breast concept is highly dependent on regional, cultural, and religious factors. It is interesting to note the influence that other less studied factors, such as pornography, smoking habits, and social media, may have. The Influence of Social Media and Easily Accessible Online Information on the Aesthetic Plastic Surgery Practice: Literature Review and Our Own Experience

Abstract:IntroductionPatients interested in aesthetic plastic surgery procedures increasingly seek advice on social media and rely on easily accessible online information. The investigatory goal was to determine the impact of this phenomenon on the everyday aesthetic plastic surgery practice.Methods and PatientsFive hundred consecutive patients completed a questionnaire prior to their consultation with a plastic surgeon at our clinic. A questionnaire was also completed by 128 plastic surgeons practising in 19 different countries. A literature review was performed.ResultsAlmost all patients (95 %) used the internet to collect information prior to consultation, for 68 % of them it being their first search method. Social media were used by 46 % of patients and 40 % of these were strongly influenced when choosing a specific doctor. The majority of plastic surgeons (85 %) thought the information found on social media could lead to unrealistic expectations. However, 45 % of plastic surgeons believed that their consultations became easier after the advent of social media, 29 % found them more difficult. A literature review showed a high percentage of poor quality internet websites regarding plastic surgery and an increase in use of social media among plastic surgeons.ConclusionThe internet and social media play an important and growing role in plastic surgery. This results in more informed patients but may create unrealistic expectations. Even if the internet provides ample information, it cannot replace the face-to-face consultation, which always should remain a detailed process, covering both risks and limitations of alternative procedures. Molecular Characterization of Salt Tolerant Rhizobial Strains Induced by Gamma Rays Using RAPD Markers

Abstract:Rhizobia are soil-borne bacteria that form nodules with legume roots and convert nitrogen into ammonia. Legume plants are often a major part of native or agricultural ecosystems, which increas nitrogen in low fertility soils, e.g. saline soils. The major target of this study was to characterize salt-tolerant rhizobial strains with high symbiotic efficiency. We choose ten strains of rhizobia that were previously mutagenized. Obtained results showed that 40 krad was the highest dose of gamma rays supported by tested strains. The irradiated clones at 50 krad were more tolerant to salt stress compared to control strains, which proved that gamma irradiation was effective in changing physiological and phenotypic characteristics via random mutations. The rhizobial strains treated with gamma rays were investigated using randomly amplified polymorphic DNA profiles to differentiate the DNA patterns among gamma treated rhizobia and parental strains. The RAPD profiles showed different, similar or even the same RAPD patterns for both types of rhizobial strains. The increase of N2 fixation parameters with some mutants was related to tolerance acquisition that protects effective symbiosis against damage induced by abiotic stresses. Inoculation with mutant isolates resulted in higher total nitrogen content than parental strains. According to the results of this study it can be recommended that inoculation of Vicia faba with effective Rhizobium strains can result in significant increase in yield due to higher nitrogen fixation ability. Assessing Patient Satisfaction Following Facelifts with Social Media Reviews

Abstract:Background: Facelifts are one of the most common facial aesthetic surgery procedures. Patient satisfaction determines success of most aesthetic surgery but has been historically difficult to assess. Objective: This study evaluated reviews by facelift patients on the aesthetic surgery social media website RealSelf.com to determine positive and negative factors underlying patient satisfaction following facelifts. Methods: Facelift reviews were gathered from RealSelf.com with an automated web crawler. Reviews were categorized as positive or negative and by the primary and secondary reasons for the positive or negative review. Patient "worth it" and star ratings, physician specialty, and cost of procedure were also collected. Results: A total of 2153 facelift reviews were collected. Overall, 1986 (92.24%) were positive and 167 (7.76%) were negative. The most common overall reasons for a positive review were aesthetic results (n=1571, 79.10%) and bedside manner (n=1488, 74.92%). The most common overall reasons for a negative review were outcome (n=137, 82.04%) and bedside manner (n=82, 49.10%). Most facelifts were performed by plastic surgeons (n=1796, 83.42%). The greatest 5-star rating percentages were seen for oral and maxillofacial surgeons (n=29, 93.55%), otolaryngologists (n=96, 92.31%), and plastic surgeons (n=1642, 91.43%). Of patients who provided a "worth it" rating, 1216 (91.91%) stated that their facelift was "worth it." Conclusion: Overall patient sentiment toward facelifts was positive. The factors most commonly affecting a positive patient experience were bedside manner and aesthetic results. Negative patient reviews were primarily attributed to dissatisfaction with aesthetic outcomes. Social media serves as a valuable tool for evaluating patient satisfaction with aesthetic surgery. Abstract:Various surgical approaches and pedicles have been described to ensure safe and satisfactory results in reduction mammaplasty. Although different breasts require different techniques, complications are common. This study aims to assess the incidence of complications following primary bilateral reduction mammaplasties across a diverse range of pedicle methods within one of the largest single-center cohorts to date, utilizing machine learning methodologies. A retrospective review of primary bilateral reduction mammaplasties at a single surgical center between January 2016 and March 2020 was performed. Patient medical records and surgical details were reviewed. Complications were compared among three different pedicles. Binary recursive partitioning (CART) machine learning was employed to identify risk factors. In total, 1021 patients (2142 breasts) met the inclusion criteria. The superomedial pedicle was the most frequently utilized (48.0%), with an overall complication rate of 21%. While pedicle-based subgroups demonstrated significant demographic variance, overall complication rates differed most between the inferior (24.9%) and the superomedial pedicle (17.7%). Statistical analysis identified resection weight as the sole significant independent risk factor (OR 1.001, p = 0.007). The machine learning model revealed that total resection weights exceeding 1700 g significantly increased the risk of overall complications, while a sternal notch to nipple (SNN)-distance > 36.5 cm correlated with complications involving the nipple-areola complex (NAC). Higher resection weights are associated with elevated complication rates. Preoperative assessment utilizing SNN-distance can aid in predicting NAC complications.

Abstract:In this study, we assess the trade-off between size, gain and safety of implantable antennas for telemetry in the medical implant communications service (MICS) band. An implantable antenna is proposed, and an algorithm is applied to miniaturize it considering single- and stacked-patch geometries. Gain and specific absorption rate (SAR) performance of all three antennas is compared as a function of size. To speed-up design and provide generic results, skin-cube implantation is assumed. Validation inside an anatomical head model is also performed.

Cross-Sectional Analysis of Instagram Use in American Plastic Surgery Practices

Abstract:Background: The growth of social media has transformed advertising within plastic surgery. Recent studies have characterized these developments, but objective analysis is needed. Methods: This is a cross-sectional analysis of online media use by American Society of Plastic Surgeons members who received board certification in 2000, 2005, 2010, 2015, or 2019. Online searches and StatShow revealed social media and website metrics. Metropolitan-based practices were determined using Department of Agriculture continuum codes. Descriptive and quantitative analyses were used to make inferences regarding study aims. Results: This study included 811 surgeons. A total of 58.6 percent had practice websites and 43.9 percent had professional Instagram accounts. Instagram use was widespread across subspecialties and there was no significant difference in the number of followers by subspecialty ( p = 0.34). Year of certification had no significant effect on the number of followers ( p = 0.12); however, recently certified and seasoned members had the fewest. The top 1 percent of surgeons had more followers than the remaining 99 percent combined. Those with metropolitan-based practices had significantly higher website traffic ( p = 0.01) but no difference in the number of followers ( p = 0.88). There was no evidence that the number of followers or posts per month correlated with website traffic ( R 2 = 0.004 and 0.036, respectively). Conclusions: The study demonstrates findings from a cross-sectional analysis of plastic surgeons from different training backgrounds, regions, and tenure. The use of Instagram in professional practice is widespread but there is no correlation between its use and increased website traffic.

Abstract:Objective: Facial asymmetries can significantly affect patient satisfaction post-surgery. Social media can impact a patient's perception of their asymmetries. To gather insights into the practices, perceptions, and approaches concerning facial asymmetry among facial plastic surgeons. To understand the impact of social media on perceptions of facial asymmetry. Design and participants: An AAFPRS survey consisting of 46 questions that sought to understand facial plastic surgeons' perceptions of facial asymmetry, their consultation process, their patients' perspectives on their own facial asymmetries, and patient satisfaction. The survey was distributed to 1,269 members of AAFPRS across community hospitals, private practices, and academic hospitals. Sixty-seven members completed the survey. Findings: The survey revealed that 31% (21) of surgeons face challenges in managing patient expectations regarding facial asymmetry. 12% (8) of surgeons felt that patients did not initially recognize their own asymmetries. Eighty-seven percent (58) of surgeons emphasize the natural occurrence of asymmetry during consultations to set realistic expectations. Conclusion: This study emphasizes the need for thorough patient education during initial consultations to align expectations with achievable outcomes. Surgeons should explain the natural occurrence of asymmetry clearly and use digital imaging to show patients realistic previews of surgical results.
