Abstract:This descriptive study was designed to investigate the pelvic floor muscle strength (PFMS) of women aged 18 to 49 years and to examine the factors that may have an effect on PFMS. The study was conducted on 258 women who visited a gynecology outpatient clinic between January 2019 and January 2020, who met the research criteria, and who agreed to participate in the study. The data were collected using the Sociodemographic Characteristics Information Form. The Modified Oxford Scale (MOS) and a perineometer were used to evaluate the PFMS of the women. The mean PFMS value measured using the perineometer was 31.56 ± 12.17 cmH2O (moderate pressure). The PFMS values were 20.00 to 29.9 cmH2O (weak pressure) and 30.00 to 39.9 cmH2O (moderate pressure) in 23.6% of the women, respectively. The PFMS values measured with MOS were of grade 3 strength (moderate pressure) in 23.6% of the women and grade 2 strength (weak pressure) in 23.3%. A statistically significant strong correlation was found between the perineometer measurement and the women's MOS values. Moreover, a statistically significant difference was found between the PFMS values measured with the perineometer, MOS scores, and women's age groups, educational status, marital status, employment status, income status, persistent cough, use of nicotine, alcohol and coffee consumptions, chronic constipation, history of frequent urinary tract infections, regular exercise, body mass index, history of pregnancy, mode of delivery, use of episiotomy at birth, perineal rupture at birth, use of forceps vacuum at birth, multiple pregnancies, delivery of a baby weighing ≥4,000 g, treatment during pregnancy, hysterectomy, menopause, frequency of sexual intercourse, and pain during sexual intercourse (p < .05). We conclude that most of the women in the study had weak to moderate PFMS, that the evaluation of PFMS with the MOS positively overlapped with the perineometric measurements, and that a number of sociodemographic and obstetric variables act as risk factors that affect PFMS. The PFMS of all women should be assessed as part of their routine gynecological examinations.
Observe and Analyze the Rehabilitation of Multiparous Pelvic Floor Muscles in Postpartum

Abstract:Objective:To analyze the effects of mutiparity on the pelvic floor muscle and the best time of postpartum for pelvic floor rehabilitation intervention.Methods:By questionnaire survey and pelvic floor functional checking on women with 42 days postpartum clinic routine health checking,and following up pelvic floor muscles force exam in postpartum 3 months,6 months and 1 year,426 cases were included in the puerpera group,including primipara 325 cases as primiparous group,two birth multipara 101 cases as multiparous group,the damage of pelvic floor muscles(≤level Ⅱ) was compared between the two groups,the difference between multiparity and primiparity were compared.Results:①In postpartum 42 days,type Ⅰ muscle fiber damage rate was lower in multiparous group than that in primiparous group(P ＜0.05),there was no significant difference on the muscle fiber damage ratio (muscles force below Ⅲ) between the two groups (P ＞ 0.05).②)In postpartum 3 months,6 months,1 year there was no significant difference in proportion of muscle damage between the two groups (P ＞ 0.05).③Multiparous group pelvic floor muscle damage ratio decreased slowly and gradually in the four periods,there was no statistical difference between adjacent period (P ＞ 0.05),the pelvic muscle damage ratio appeared significant statistical difference between postpartum 6 months and postpartum 42 days(P ＜ 0.01).④In primiparous group,pelvic floor muscle damage ratio decreased significantly in postpartum 6 months,postpartum 42 days、3 months,6 months,the differces were statistically significant among the three times(P ＜ 0.05),there was no statistical difference between postpartum 6 months and 1 year(P＞0.05).Conclusions:Type Ⅰ muscle fibers suffer injury more in Primipara than that in multipara,multiparous pelvic floor muscle recovery process is more slowly than primipara's,we should pay attention to pelvic muscle recovery within 6 months after delivery.

Abstract:Objective To assess pelvic floor muscle (PFM) strength and influencing factors among healthy women at different life stages. Design Multicentre cross‐sectional study. Setting Fourteen hospitals in China. Population A total of 5040 healthy women allocated to the following groups (with 1680 women per group): premenopausal nulliparous, premenopausal parous and postmenopausal. Methods The PFM strength was evaluated by vaginal manometry. Multivariate logistic regression was used to determine the influencing factors for low PFM strength. Main Outcome Measures Maximum voluntary contraction pressure (MVCP). Results The median MVCP values were 36, 35 and 35 cmH2O in premenopausal nulliparous (aged 19–51 years), premenopausal parous (aged 22–61 years), and postmenopausal (aged 40–86 years) women, respectively. In the premenopausal nulliparous group, physical work (odds ratio, OR 2.05) was the risk factor for low PFM strength, which may be related to the chronic increased abdominal pressure caused by physical work. In the premenopausal parous group, the number of vaginal deliveries (OR 1.28) and diabetes (OR = 2.697) were risk factors for low PFM strength, whereas sexual intercourse (<2 times per week vs. none, OR 0.55; ≥2 times per week vs. none, OR 0.56) and PFM exercise (OR 0.501) may have protective effects. In the postmenopausal group, the number of vaginal deliveries (OR 1.32) and family history of pelvic organ prolapse (POP) (OR 1.83) were risk factors for low PFM strength. Conclusions Physical work, vaginal delivery, diabetes and a family history of POP are all risk factors for low PFM strength, whereas PFM exercises and sexual life can have a protective effect. The importance of these factors varies at different stages of a woman's life.

The impact of acute and chronic strenuous exercise on pelvic floor muscle strength and support in nulliparous healthy women

Abstract:BACKGROUND: Strenuous physical activity, which is known to increase intraabdominal pressure and theoretically places stress on the pelvic floor, may affect pelvic support in nulliparous women.OBJECTIVE: The aims of this study were to: (1) examine the differences in maximal vaginal descent (MVD), vaginal resting pressure (VRP), and pelvic floor muscle strength (PFMS) between women who habitually perform strenuous exercise vs women who refrain from performing strenuous exercise; and (2) compare MVD, VRP, and PFMS before and immediately following physical activity in the strenuous and nonstrenuous groups separately.STUDY DESIGN: Participants were healthy nulliparous women ages 18-35 years who were habitual strenuous or nonstrenuous exercisers. Women in the strenuous group participated in CrossFit (CrossFit, Inc., Washington, DC) at least 3 days per week for at least 6 months. We assessed anthropometric and body composition values using standardized procedures. Participants completed the Pelvic Organ Prolapse Quantification examination and pelvic muscle strength assessment before and again within 15 minutes of completing exercise (CrossFit for the strenuous group and self-paced walking for the nonstrenuous). A research nurse masked to study group assignment recorded MVD, defined as the greatest value of anterior, posterior, or apical support, and VRP and PFMS using a perineometer. Maximal PFMS was recorded as the highest pressure measured in 3 vaginal contraction trials. Data were analyzed using parametric and nonparametric tests as appropriate. P < .05 was considered significant.RESULTS: Seventy nulliparous women participated in the study, 35 in each group. The mean age was 24.77 ± 4.3 years. Compared to the nonstrenuous group, strenuous participants were heavier (64.70 ± 7.78 kg vs 60.6 ± 8.99 kg, P = .027), had lower percent body fat (23.36 ± 5.88% vs 27.55 ± 7.07%, P = .003), and had higher handgrip strength (20.78 ± 5.97 kg vs 16.04 ± 11.04 kg, P = .001). Before exercise, there were no significant differences in VRP (P = .167), MVD (P = .49), or maximal PFMS (P = .773) between the strenuous and nonstrenuous groups. Immediately following exercise, we observed significant increases in MVD in both the strenuous (P = .008) and nonstrenuous (P = .025) groups, indicating marginal decreases in support. VRP significantly decreased in both groups after exercise. Maximal PFMS did not change significantly in either group after exercise.CONCLUSION: After an exercise bout typical for each group, vaginal support and VRP decreased slightly in both groups. Based on preexercise measures, chronic strenuous exercise demonstrated neither beneficial nor deleterious effects on pelvic floor strength or support. While strenuous women had greater grip strength than nonstrenuous women, PFMS was not significantly greater, suggesting that targeted pelvic floor muscle strengthening, rather than general muscle fitness, is needed to maximize PFMS.

Investigation and Influencing Factors on Pelvic Floor Muscle Strength of 929 Adult Females in Gynecological Outpatient Department

Abstract:Objective: To investigate the present situation of pelvic floor muscle strength, and to analyze the factors affecting pelvic floor muscle strength. Methods: The data of patients who were admitted into the general outpatient department of gynecology, Peking University People's Hospital from October 2021 to April 2022 were collected, and the patients who met the exclusion criteria were included in this cross sectional study. The patient's age, height, weight, education level, defecation way and defecation time, birth history, maximum newborn birth weight, occupational physical activity, sedentary time, menopause, family history and disease history were recorded by questionnaire. Morphological indexes such as waist circumference, abdomen circumference and hip circumference were measured with tape measure. Handgrip strength level was measured with grip strength instrument. After performing routine gynecological examinations, the pelvic floor muscle strength was evaluated by palpation with modified Oxford grading scale (MOS). MOS grade>3 was taken as normal group and ≤3 as decreased group. Binary logistic regression was used to investigate the related factors of deceased pelvic floor muscle strength. Results: A total of 929 patients were included in the study, and the average MOS grade was 2.8±1.2. By univariate analysis, birth history, menopausal time, defecation time, handgrip strength level, waist circumference and abdominal circumference were related to the decrease of pelvic floor muscle strength (all P<0.05). By binary logistic regression analysis, the level of handgrip strength (OR=0.913, 95%CI: 0.883-0.945; P<0.001) was correlated with normal pelvic floor muscle strength; waist circumference (OR=1.025, 95%CI: 1.005-1.046; P=0.016), birth history (OR=2.224, 95%CI: 1.570-3.149; P<0.001), sedentary time> 8 hours (OR=2.073, 95%CI: 1.198-3.587; P=0.009) were associated with the decrease of pelvic floor muscle strength. Conclusions: The level of handgrip strength is related to the normal pelvic floor muscle strength of females, while the waist circumference, birth history and sedentary time>8 hours are related to the decrease of pelvic floor muscle strength of females. In order to prevent the decrease of pelvic floor muscle strength, it is necessary to carry out relevant health education, enhance exercise, improve the overall strength level, reduce daily sedentary time, maintain symmetry, and carry out comprehensive overall intervention to improve pelvic floor muscle function.
Impact of Different Body Positions on Bioelectrical Activity of the Pelvic Floor Muscles in Nulliparous Continent Women

Abstract:We examined pelvic floor muscles (PFM) activity (%MVC) in twenty nulliparous women by body position during exercise as well as the activation of abdominal muscles and the gluteus maximus during voluntary contractions of the PFMs. Pelvic floor muscle activity was recorded using a vaginal probe during five experimental trials. Activation of transversus abdominis, rectus abdominis, and gluteus maximus during voluntary PFM contractions was also assessed. Significant differences in mean normalized amplitudes of baseline PFM activity were revealed between standing and lying ( P < 0.00024 ) and lying and ball-sitting positions ( P < 0.0053 ). Average peak, average time before peak, and average time after peak did not differ significantly during the voluntary contractions of the PFMs. Baseline PFM activity seemed to depend on the body position and was the highest in standing. Pelvic floor muscles activity during voluntary contractions did not differ by position in continent women. Statistically significant differences between the supine lying and sitting positions were only observed during a sustained 60-second contraction of the PFMs.

Abstract:Background Pelvic floor muscle strength is well-known to be associated with female sexual function. However, there were a few studies that reported on the relationship between pelvic floor muscle strength and female sexual function in pregnant women, and the presented results were inconsistent. Nulliparae represent a specific cohort with simplicity to exclude confounding factors that are caused by parity. The present study aimed to explore the association of pelvic floor muscle strength and sexual function based on the Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ-12) of nulliparae during pregnancy. Methods This is the second analysis of the baseline data from a randomized controlled trial (RCT), which aimed to study the protective efficacy of pelvic floor muscle training on stress urinary incontinence at 6th week postpartum (registration number: ChiCTR2000029618). Nulliparae aged 20–40 years with singleton pregnancy before 16 weeks of gestation were enrolled in this study, and data, including participants' demographic information, the Modified Oxford Scale (MOS), and PISQ-12, were collected. Eligible nulliparae were divided into two groups: Group MOS > 3 and Group MOS ≤ 3. Demographic information of the two groups was compared. Sexual function based on the PISQ-12 scores of the two groups was compared. A comparison of the PISQ-12 scores between the two groups was calculated by the Mann–Whitney U -test using SPSS version 23.0. Results A total of 735 eligible nulliparae were enrolled in this study. Along with MOS grading up, PISQ-12 scores tended to get lower. Of the 735 nulliparae, there were 378 and 357 participants included in Group MOS > 3 and Group MOS ≤ 3, respectively. The PISQ-12 scores of Group MOS > 3 were significantly lower than those of Group MOS ≤ 3 (11 vs. 12, p < 0.001). The scores of the frequency of feeling sexual desire, orgasm achievement, sexual excitement, sexual activity satisfaction, sexual intercourse pain, fear of urinary incontinence, and negative emotion reactions with the sexual intercourse of Group MOS > 3 were lower than those of Group MOS ≤ 3 ( p < 0.05). Conclusion Pelvic floor muscle strength was positively associated with sexual function based on the questionnaire of young nulliparae during their first trimester. Up to half of the nulliparae during the first trimester were suffering from weak pelvic floor muscle strength and nearly a quarter of the nulliparae were facing this weakness combined with sexual dysfunction. Correlation of Postpartum Pelvic Floor Dysfunction with Contractility of Pelvic Floor Muscles and Related Factors

Abstract:Objective: To analyze the correlation of postpartum pelvic floor dysfunction with the contractility of pelvic floor muscles and related factors. Methods: A cohort of 102 patients in postpartum 42-60 days with symptoms of pelvic floor dysfunction,including 66 of postpartum urinary incontinence( UI) and 36 of pelvic organ prolapsed( POP),were recruited. 100 postpartum women with normal pelvic floor function were recruited as a control group. A questionnaire survey was conducted for the general situation of the subjects. The correlation of postpartum pelvic floor dysfunction and contractility of pelvic floor muscles was determined through the electromyographic recording of pelvic floor,measuring the sustained and rapid contractility as the function of pelvic floor class Ⅰ and Ⅱ muscle,respectively. Results: 1The age,pre-delivery body mass index,newborn baby weight,vaginal birth rate and a history of abdominal pressure in the study group were significantly higher than those in the control group( P 0. 05; P 0. 01). 2Both the sustained and rapid contractilities of pelvic floor muscle in the study group were significantly lower than those in the control group( P 0. 01). 3The rapid contractility in UI patients was significantly lower than that in POP patients and control group( P 0. 01),and the sustained contractility in POP patients was significantly lower than that in UI patients and control group( P 0. 01). Conclusions: Age,pre-delivery BMI,newborn body weight,and vaginal delivery,may be related factors of postpartum pelvic floor dysfunction. The postpartum pelvic floor dysfunction may be associated with the declined contractility of pelvic floor muscle,especially the POP with class I muscle,and the UI with class II muscle.
At What Point in the Menstrual Cycle Are the Pelvic Floor Muscles at Their Weakest?

Abstract:Pelvic floor muscle (PFM) strength is a critical factor for optimal pelvic floor function. Fluctuations in strength values based on different phases of the menstrual cycle (MC) could signify a need for a paradigm shift in evaluating, approaching, and planning training. This research aims to examine and contrast the pelvic floor muscle strength during different phases of the menstrual cycle. A prospective observational study employing digital assessment with the modified Oxford scale and vaginal dynamometry measurements was performed, in order to assess the baseline strength and the contraction strength of the PFMs in eumenorrheic females at three different phases of the MC: the early follicular phase (EFP), the late follicular phase (LFP), and the mid-luteal phase (MLP). During two complete cycles, tympanic temperature and body weight were measured and the urinary luteinizing hormone concentration was tested to determine the time of ovulation. In total, 216 dynamometric measurements of PFM strength were obtained from eighteen nulliparous women (25.72 ± 5.03 years). There were no differences between the baseline strength (p = 0.886) and the contraction strength (p = 0.756) with the dynamometric speculum. In the post hoc analysis, the baseline strength, contraction strength, and strength showed no significant differences between MC phases. As no differences in PFM strength in women were found, the PFMs do not seem to be weaker at any time during the menstrual cycle. It appears that the assessment, establishment, and monitoring of a PFM training program could be initiated at any point in the cycle.
Evaluation of Pelvic Floor Muscle Function (PFMF) in Cervical Cancer Patients with Querleu–Morrow Type C Hysterectomy: a Multicenter Study

Abstract:Introduction To evaluate the pelvic floor muscle function (PFMF) of cervical cancer patients after type QM-C hysterectomy and to explore the relationship between decreased PFMF and related factors. Methods This was a multi-centered retrospective cohort study. 181 cervical cancer patients who underwent type QM-C hysterectomy were enrolled from 9 tertiary hospitals. Strength of PFMF were measured using neuromuscular apparatus (Phenix U8, French). Risk factors contributing to decreased PFMF were analyzed by univariate and multivariate ordinal polytomous logistic regression. Results Totally 181 patients were investigated in this study. 0–3 level of type I muscle fibre strength (MFSI) was 52.6% (95/181), 0–3 level of type IIA muscle fibre strength (MFSIIA) was 50% (91/181). Subjective stress urinary incontinence was 46% (84/181), urinary retention was 27.3% (50/181), dyschezia was 41.5% (75/181), fecal incontinence was 9% (18/181). ① MFSI: Multivariate ordinal polytomous logistic regression shows that the follow-up time ( p < 0.05), chemotherapy and radiotherapy ( p = 0.038) are independent risk factors of MFSI’s reduction after type QM-C hysterectomy. ② MFSIIA: multivariate ordinal polytomous logistic regression shows that the follow-up time ( p < 0.05) are independent risk factors of MFSIIA’s reduction after type QM-C hysterectomy. The pelvic floor muscle strength (PFMS) increased after 9 months than in 9 months after operation, which showed that the PFMS could be recovered after operation. Conclusions We advocate for more attention and emphasis on the PFMF of Chinese female patients with cervical cancer postoperation. Peking University People's Hospital PFMF after QM-C hysterectomy has not been analyzed by current study. The contribution is that patients with radical hysterectomy should do pelvic floor rehabilitation exercises in 3 months after operation. Clinical Trails NCT number of this study is 02492542.
Clinical significance of pelvic floor muscle strength screening in postpartum women

Abstract:Objective To investigate the changes of pelvic floor muscle strength and its clinical significance in postpartum women.Methods There totally 1 008 healthy women who delivered in our hospital from March 2011 to Sepetember 2011 involved in this study.All women underwent pelvic floor muscle strength screen in 6~8 weeks postpartum and their obstetric factors were recorded.Results In vaginal delivery group and cesarean section group,the abnormal rates of type I muscle fiber strength were 36.3%(200/551) and 39.4%(180/457);the abnormal rates of type II muscle fiber strength were 36.5%(201/551) and 39.4%(180/457).There were no significant difference between the two groups(P>0.05).Prenatal body mass index(BMI) before labor was related to the type I muscle fiber(OR=-0.095,P<0.05).Age,neonatal birth weight,duration of the second stage of labor,parity,instrument delivery and the status of perineum were not relative with pelvic floor muscle strength.Women who were older or had longer duration of second stage tended to have higher abnormal ratio of Type I,II muscle fiber.Women with no episiotomy tended to have lower abnormal ratio of type I,II muscle fiber.The selective cesarean section group had lower abnormal ratio of type I,II muscle fiber compared with emergency cesarean section group,but there were no statistic difference.Conclusions Cesarean section can not protect the pelvic floor;prenatal maternal body mass index can affect postpartum pelvic floor muscle fiber type I muscle strength,weight control during pregnancy should be recommended.

Abstract:Background The pelvic floor musculature (PFM) plays a role that influences bladder, intestinal and sexual functions and when this musculature is affected, it can develop several symptoms with high prevalence in the female population. For women of reproductive age, every month passed by menstrual periods could have dysmenorrhea, a source of chronic pelvic pain originating from a difficult menstrual flow. There are few data on the influence of dysmenorrhea on other PFM disorders. Objective To correlate dysmenorrhea with PFM disorders in young nulliparous women. Methods A descriptive, observational, cross-sectional study with a quantitative approach was carried out. Enrollment was optional, with women aged between 18 and 30 years old, nulliparous, who had never been pregnant, had already experienced their first sexual intercourse and who did not have their menstruation on the day of the assessment. The evaluation was carried out through the application of tests (socio-clinical, International Consultation on Incontinence Questionnaire-Short Form, Pelvic Floor Distress Inventory, Visual Analog Scale (VAS) and, later, through the physical examination of the strength of the PFM through the Perfect scheme. Data were analyzed according to the sample's normality distribution, comparing the groups with and without pelvic floor dysfunction according to the presence of dysmenorrhea using the t-test for independent samples. The Statistical Program for Social Science program (version 23) was used, considering a significance level of 5%. Results The sample was fixed by 45 women (median age 21 years). The prevalence of dysmenorrhea was 77.7% with pain intensity 5 (2 - 6.50) on the VAS scale. The main symptoms of premenstrual tension were those of an emotional nature, such as irritability (84.4%), anxiety (73.3%), desire to cry (71.1%) and sadness (71.1%). The sample showed muscle weakness with a median of 3 (2 – 3) in the Perfect scheme, and 24.4% of the sample reported some type of urinary incontinence, and 17.7% had constipation. The group with dysmenorrhea had worse vaginal (p=0.04) and intestinal (p=0.03) dysfunctions. There was interference between dysmenorrhea, vaginal (R=0.81) and intestinal (R=0.57) disorders. Conclusion Dysmenorrhea is prevalent in young nulliparous women with an association between vaginal and intestinal dysfunctions, this population presents pelvic floor muscle weakness and symptoms of premenstrual tension predominantly of an emotional nature. Implications In scientific terms, this study demonstrated that PFM dysfunctions are a reality among young nulliparous women and that women with dysmenorrhea every month may be more subject to vaginal and intestinal dysfunctions with possible chronic implications for their health. In clinical terms, it is important to consider the complaint of dysmenorrhea in the evaluation and to question and physically assess the bladder, vaginal, sexual, and intestinal functions that may be associated.

Evaluation of Factors Affecting the Reduction in Postpartum Pelvic Floor Muscle Tension and The Therapeutic Effect

Abstract:Purpose: This study investigated the risk factors associated with reductions in postpartum pel-vic floor muscle tension and evaluated improvements in pelvic floor muscle tension with electrical stimulation combined with biofeedback technology and Kegel exercis-es. Methods: We conducted a case-control study of 170 women with postpartum follow-up at Ningbo Li Huili Hospital from April 2019 to May 2020. According to the Oxford modified scale assessment(MOS), 94 and 76 women were included in the abnormal pelvic floor muscle tension group and control group, respectively. The two groups were trained by pelvic floor training. The changes in pelvic floor muscle tension before and after treatment in the two groups were analyzed to evaluate the effect of postpartum pelvic floor rehabilitation training. Results: The EMG value of the fast muscle contraction stage was negatively correlated with age and neonatal weight (P <0.05). The EMG values of slow muscle contraction and endurance were negatively correlated with weight gain during pregnancy but posi-tively correlated with age and BMI at delivery (P <0.05). The muscle tension of the abnormal muscle tension group was significantly improved and significantly higher than that of the control group (P <0.05) after the two groups received the intervention. Conclusion: The factors affecting postpartum pelvic floor muscle tension include age, delivery times, BMI at delivery and neonatal weight. Electrical stimulation combined with the biofeedback technique and Kegel exercises in the early postpartum period are effec-tive means to reduce the incidence of Pelvic floor disorders.
Pelvic Floor Muscle Strength in the First Trimester of Primipara: A Cross-Sectional Study

Abstract:Background: Pelvic floor muscle (PFM) weakness is associated with stress urinary incontinence. Pregnancy is an important risk factor for PFM weakness. Studies evaluating PFM strength in the first trimester of pregnancy are still lacking. Our study aimed to describe pelvic floor function of the primipara in the first trimester of gestation and investigate the risk factors for PFM weakness. Methods: Primiparas aged 20~40 years with a singleton pregnancy less than 14 weeks of gestation were recruited, and data were collected via questionnaires on items that were suggested as associated with PFM weakness, followed by Modified Oxford Scale (MOS) on genital hiatus and perineal body and pelvic floor ultrasound evaluation for the thickness of the left and right levator ani muscles (LAM), right–left diameter of the levator hiatus (LH), and LH area. Participants were divided into three groups by MOS >3, =3, and <3 for data analysis. Results: A total of 380 participants completed the questionnaires and examinational analysis, of whom, 228, 98, and 54 were divided into Group 1, Group 2, and Group 3, respectively. The three groups were significantly different in the number of gestations and abortions, toilet types, and the right–left diameter of the LH (p < 0.05). Logistic regressive analysis showed that squatting toilet dominant (OR = 3.025; 95% CI: 1.623~5.638; p < 0.001) and a larger right–left diameter of the LH (OR = 1.065; 95% CI: 1.026~1.105; p = 0.001) were significantly associated with PFM weakness. Conclusions: Squatting toilet dominancy and longer right–left diameter of the LH are significantly associated with PFM weakness in primiparas in the first trimester. Sitting toilets should be recommended to women, especially pregnant women. Abstract:Aims: Early evaluation of pelvic floor muscle (PFM) in postpartum women is important for the treatment of stress urinary incontinence (SUI). Digital vaginal palpation and electromyography (EMG) evaluation based on Glazer protocol are widely used for the assessment of PFM. However, the correlation among digital palpation, EMG, and morbidity of postpartum SUI is still unclear. This study aims to investigate the relationship between postpartum SUI and PFM examinations. Methods: This hospital-based cross-sectional study included 1,380 parturients during September 2016 to January 2018. We collected the clinical characteristics, PFM strength, and EMG variables of parturients 6–8 weeks after birth. Then the correlation among the results of EMG, digital palpation, and the occurrence of SUI was analyzed. Results: There is no significant difference in digital palpation scores of PFM strength between SUI and non-SUI parturients. The EMG values were closely related to SUI: the multivariate logistic regression revealed that the most reliable evaluation indicators of postpartum SUI were pelvic floor contractile amplitude of endurance contraction (B = 0.021, p = 0.019) and pretest resting baseline (B = 0.056, p = 0.019). Correlation analysis demonstrated that the contraction variables of EMG had a significant correlation with the digital palpation PFM strength in postpartum women (r = 0.467–0.545, p < 0.001). Conclusion: The EMG proved to be reliable in assessing the PFM function in postpartum women. The decreased PFM activity, according to EMG, was correlated with postpartum SUI. Although digital palpation scores were positively correlated with EMG results, no correlation was observed with SUI incidence.
Do women have an accurate perception of their pelvic floor muscle contraction? A cross-sectional study

Abstract:Aims: To assess women's self-perception of their pelvic floor muscle (PFM) contraction and its agreement with an assessed PFM contraction. Further, to assess a possible correlation between women's self-perception and reports of urinary incontinence (UI) and between PFM contraction and severity of UI. Methods: A cross-sectional observational study including 82 women. The study was conducted in a basic healthcare unit in Brazil. PFM contraction was assessed by a physiotherapist and estimated by women using the Modified Oxford Scale (MOS). UI symptoms were assessed using a validated questionnaire (ICIQ-UI-SF). A descriptive analysis of the data was performed. The weighted κ coefficient, Spearman's correlation coefficient, and Fisher's exact test were used to analyze data. Results: Eighty-two women with a mean age of 46.83 (±17.94) were analyzed. The majority (98.8%) believed they were able to voluntarily contract their PFM, but only 33% correctly estimated their PFM considering the examiner assessment as reference. No agreement (κ = 0.139, P = .087) was found between the examiner's classification and the women's estimation of their PFM contraction. Women's self-perception did not correlate with the ICIQ-IU-SF (r's = .011, P = .922). A moderate negative correlation was found between the assessed PFM contraction and the ICIQ-UI-SF score (r's = -.406, P = .00). Conclusion: Most of the women did not show an accurate self-perception of PFM contraction. No correlation between women's self-perception and the ICIQ-UI-SF score was found, but a negative correlation was found between the assessed PFM contraction and the ICIQ-UI-SF score.
Difference in pelvic floor muscle strength among pregnant women and its effect on labour and postpartum stress urinary incontinence

Abstract:Objective:To investigate the difference in pelvic floor muscle strength among pregnant women and their mastery level of pelvic floor muscle training (PFMT) and explore the effect of pelvic floor muscle strength on labour and postpartum stress urinary incontinence (SUI).Methods:A total of 220 singleton primiparae who received routine antenatal assessment and delivered in Peking University Shougang Hospital from September 2018 to March 2021 were selected. All participants underwent pelvic floor muscle strength assessment by vaginal palpation and PFMT instruction at the 28th week of gestation and were divided into a strong strength group (group A), a moderate strength group (group B), and a weak strength group (group C). Parameters related to muscle strength as well as correlation of muscle strength with delivery and SUI were analyzed in the three groups.Results:Approximately 51.1% (113) of cases could perform PFMT correctly, while 18.2% (40 cases) could not , and the remaining 30.7% (67 cases) had weak muscle strength or insufficient endurance. Of the 220 women included, 135 had full-term vaginal delivery (group A, 75 cases; group B, 42 cases; group C, 18 cases), while 10 had premature vaginal delivery, and 75 had caesarean delivery. Body height was significantly higher in group A than in groups B and C (P＜0.05), while there were significant differences in pre-pregnancy body weight and body mass index (BMI) between groups A and C (P＜0.05). Pelvic floor muscle strength was positively correlated with body height, pre-pregnancy body weight, and BMI. Length of the first stage of labour in group A was shorter significantly than that in group C (P＜0.05). The incidence of SUI in women who underwent vaginal delivery did not differ significantly among the three groups (P＞0.05), but the incidence of SUI in women who underwent caesarean delivery in strong group was significantly lower in group A than in group B (P＜0.05).Conclusion:There are individual differences in pelvic floor muscle strength among pregnant women, which may be related to body height, pre-pregnancy body weight, and BMI. Strong pelvic floor muscle strength could help shorten the first stage of labour and reduce the incidence of SUI after caesarean delivery. Accurate PFMT is of great clinical importance in these women.
Association of pre‐pregnancy body mass index and gestational weight gain on postpartum pelvic floor muscle morphology and function in Chinese primiparous women: A retrospective cohort study

Abstract:Objective Our study aimed to investigate the association between maternal pre‐pregnancy body mass index (BMI), gestational weight gain (GWG), and impaired pelvic floor muscle (PFM) morphology and function during the early postpartum period. Methods This retrospective cohort study was conducted at Shanghai First Maternity and Infant Hospital from December 2020 to December 2022. A total of 1118 primiparous women with singleton pregnancies who underwent vaginal deliveries and participated in postpartum PFM assessments were included. Maternal pre‐pregnancy BMI and GWG were considered as exposures. PFM morphology and function impairment were the primary outcomes. PFM morphology impairment, defined as levator ani muscle avulsion, was assessed using transperineal ultrasound. PFM function impairment, manifested as diminished PFM fiber strength, was assessed through vaginal manometry. Multivariable logistic regression analysis was employed to calculate adjusted odds ratios (aOR) with 95% confidence intervals (CI). Restricted cubic spline models were used to validate and visualize the relationship. Results Women with lower pre‐pregnancy BMI were at an increased risk of levator ani muscle avulsion (aOR = 1.73, 95% CI: 1.10–2.70, P = 0.017), particularly when combined with excessive GWG during pregnancy (aOR = 3.20, 95% CI: 1.15–8.97, P = 0.027). Lower pre‐pregnancy BMI was also identified as an independent predictor of PFM weakness (aOR = 1.53, 95% CI: 1.08–2.16, P = 0.017 for type I fiber injuries). Notably, regardless of the avulsion status, both underweight and overweight/obese women faced an elevated risk of reduced PFM strength (aOR = 1.74, 95% CI: 1.17–2.59, P = 0.006 for underweight women with type I fiber injuries; aOR = 1.67, 95% CI: 1.06–2.64, P = 0.027; and aOR = 1.73, 95% CI: 1.09–2.76, P = 0.021 for overweight/obese women with type I and type II fibers injuries, respectively). Conclusions Both lower and higher pre‐pregnancy BMI, as well as excessive GWG, were strongly associated with PFM impairments. These findings highlighted the critical importance of comprehensive weight management throughout pregnancy to effectively promote women's pelvic health.
