Abstract:Background The impact of COVID-19 caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on patients with bronchiectasis in terms of symptoms, self-management and medical resource utilization was unknown. Objective To describe the impact of infection by SARS-CoV-2 on fluctuation of symptoms, self-management and medical resource utilization of patients with bronchiectasis during the pandemic of COVID-19. Methods This was a single-center cross-sectional questionnaire study performed in Peking University Third Hospital. An online questionnaire investigation addressing the impact of SARS-CoV-2 infection on respiratory symptoms, self-management and medical resource utilization was conducted among patients with bronchiectasis during the COVID-19 surge in December 2022 in Beijing, China. Results Five hundred patients with bronchiectasis, with 285 (57%) females, and a mean ( ± S D ) age of 57.9 ± 15.1 years, completed the telephone questionnaire. The reported prevalence of COVID-19 was 81.2% (406/500). Of the 406 COVID-19 patients, 89.2% experienced fever lasting mostly for no more than 3 days, 70.6 and 61.8% reported exacerbated cough and sputum production respectively, and 17.7% reported worsened dyspnea. Notable 37.4% of the patients with COVID-19 experienced symptoms consistent with the definition of an acute exacerbation of bronchiectasis. However, 76.6% (311/406) of the infected patients did not seek medical care but managed at home. Of the patients who visited hospitals, 26.3% (25/95) needed hospitalization and 2.1% (2/95) needed ICU admission. Multi-factors logistic regression analysis showed that younger age ( p = 0.012) and not using a bronchodilator agent( p = 0.022) were independently associated with SARS-CoV-2 infection, while a history of exacerbation of bronchiectasis in the past year ( p = 0.006) and daily use of expectorants ( p = 0.002) were associated with emergency visit and/or hospitalization for patients with bronchiectasis after SARS-CoV-2 infection. Conclusion During the COVID-19 surge, the infection rate of SARS-CoV-2 in patients with bronchiectasis was high, and most of the patients experienced new-onset or exacerbated respiratory symptoms, but only a minority needed medical visits. Our survey results further underscore the importance of patients’ disease awareness and self-management skills during a pandemic like COVID-19.

Abstract:Objective: Viral infections are an important cause of exacerbation in bronchiectasis patients. We aimed to determine the influence of the COVID-19 pandemic on adult bronchiectasis patients and whether there was a relationship between the clinical parameters and the COVID-19 infection. Patients and methods: In this retrospective observational study, 547 bronchiectasis patients were included. Demographic characteristics, vaccination status, Bronchiectasis Severity Index (BSI), FACED and Reiff scores, and clinical and laboratory parameters during COVID-19 infection were evaluated. Results: The median age was 56, and 49.2% of the patients were male. The COVID-19 infection rate was 27.6%. 431 (78.8%) patients had at least one dose of the COVID-19 vaccine. The patients were divided into two groups according to their COVID-19 infection status. Emergency admission was significantly higher in the COVID-19-infected group. There was no statistical difference with other clinical factors. The COVID-19-infected patients were divided into home treatment and hospital/intensive care unit (ICU) treatment groups. There was a statistically significant difference between the two groups regarding advanced age, male gender, presence of asthma, long-term oxygen therapy (LTOT) and non-invasive mechanic ventilator (NIMV) usage, sputum culture positivity, BSI and FACED scores, and multiple laboratory parameters (ferritin, C-reactive protein, eosinophil). In logistic regression analysis, BSI was found as a risk factor [OR 1.252 (1.077-1.456), p=0.004] and eosinophilia as a protective factor [OR 0.986 (0.973-0.999), p=0.030] for hospital/ICU admission. Conclusions: Frequent emergency visits might increase the risk of COVID-19 infection in bronchiectasis patients. BSI was found to be an independent risk factor, and blood eosinophilia could play a protective role in hospital/ICU admission for COVID-19 infection.

Abstract:Titanium osseointegrated implants for boneanchored hearing aids (BAHAs) have been in use since 1977. A series of 32 children who received implantation since 1990 is reported. The report focuses on the surgical aspects of BAHAs, predisposing factors, and prevention of complications in an unusual pediatric population. The records of 32 children who had undergone two‐stage implantation of a BAHA were retrospectively reviewed. The majority of the patients had craniofacial abnormalities. Of the 32 implantees, 29 use their BAHA at present. Five children failed to achieve osseointe‐gration, and eight patients have had revision surgery for lost abutments, trauma, or chronic skin problems. There were no differences between preimplantation and postimplantation bone or air conduction thresholds. The pediatric BAHA carries with it a unique set of challenges and problems but can be successfully implanted and maintained.

Abstract:Mild‐to‐moderate COVID‐19 is associated with an increase in frequency of bronchiectasis exacerbation as well as hospitalizations following recovery. Background and Objective Respiratory viral infection is a common trigger of bronchiectasis exacerbation. Knowledge of the intermediate to long‐term effect of COVID‐19 on bronchiectasis is poor. Methods A retrospective cohort study of patient records was conducted to assess the frequency of bronchiectasis exacerbation following recovery from mild‐to‐moderate COVID‐19. The exacerbation frequency at baseline, using 2019 and 2019–2021 data, was compared with that during the 1 year following recovery. Results A total of 234 adult patient records who had a confirmed diagnosis of bronchiectasis were identified, of whom 52 (22.2%) were classified as the COVID‐19 group. Patients with COVID‐19 had significantly more frequent annual exacerbations of bronchiectasis (total exacerbations and hospitalizations). Compared with 2019–2021 data, the total exacerbation frequency decreased by 0.1 ± 0.51 per year among non‐COVID‐19 patients but increased by 0.68 ± 1.09 per year among the COVID‐19 group (p

Abstract:The coronavirus disease (COVID-19) pandemic, which has been driven by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus, has caused significant morbidity and mortality worldwide. With a range of clinical responses from asymptomatic to severe, it has particularly targeted the elderly and those suffering from underlying disease conditions. Individuals with chronic obstructive pulmonary disease (COPD) are susceptible to a range of respiratory viral infections, and it is therefore not surprising that COPD is a risk factor for hospitalization, mechanical ventilation, severe disease, and mortality in COVID-19 (1). The effects of the COVID-19 pandemic have been globally devastating; however, as new viral strains emerge and population immunity increases, focus has turned to those groups with greater risk of developing severe disease determined early in the pandemic with risk factors including age, sociodemographic status, and preexisting comorbid conditions (2). Preexisting chronic respiratory diseases, such as asthma, interstitial lung disease, and COPD, are associated with increased risk of poor outcome although the mechanisms remain poorly described (3). The use of primary bronchial epithelial cells (PBECs) from individuals with a range of lung conditions has proved useful in understanding altered responses in these cells at baseline and following infection to delineate disease mechanisms (4). New insights published in this Journal by Johansen and colleagues (pp. 712–729), provide experimental evidence supporting increased susceptibility and severity of infection in patients with COPD, using a well-defined air–liquid interface model of PBECs from patients with COPD and healthy volunteers (5). The use of such patient-derived cell models provides a much clearer path to translation than cell line or murine models of disease, although it will be interesting to see how these findings translate to viral infection of type-II pneumocytes (AT2), which are tied to the development of acute respiratory distress syndrome. This will, of course, be technically challenging, but the use of alveosphere culture models may help to address this to some extent (6).

Abstract:Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is responsible for the respiratory distress condition known as COVID-19. This disease broadly affects several physiological systems, including the gastrointestinal, renal, and central nervous (CNS) systems, significantly influencing the patient's overall quality of life. Additionally, numerous risk factors have been suggested, including gender, body weight, age, metabolic status, renal health, preexisting cardiomyopathies, and inflammatory conditions. Despite advances in understanding the genome and pathophysiological ramifications of COVID-19, its precise origins remain elusive. SARS-CoV-2 interacts with a receptor-binding domain within angiotensin-converting enzyme 2 (ACE2). This receptor is expressed in various organs of different species, including humans, with different abundance. Although COVID-19 has multiorgan manifestations, the main pathologies occur in the lung, including pulmonary fibrosis, respiratory failure, pulmonary embolism, and secondary bacterial pneumonia. In the post-COVID-19 period, different sequelae may occur, which may have various causes, including the direct action of the virus, alteration of the immune response, and metabolic alterations during infection, among others. Recognizing the serious adverse health effects associated with COVID-19, it becomes imperative to comprehensively elucidate and discuss the existing evidence surrounding this viral infection, including those related to the pathophysiological effects of the disease and the subsequent consequences. This review aims to contribute to a comprehensive understanding of the impact of COVID-19 and its long-term effects on human health.

Abstract:Tuberculosis (TB) and Coronavirus Disease-19 (COVID-19) infection are two respiratory diseases that are of particular concern epidemiologically. Tuberculosis is one of the oldest diseases recorded in the history of mankind dating back thousands of years. It is estimated that approximately one quarter of the world's population is infected with latent Mycobacterium tuberculosis (LTBI). This contrasts with COVID-19, which emerged in late 2019. Data continues to accumulate and become available on this pathogen, but the long-term side effect of fibrotic damage in COVID-19 patients evokes parallels between this novel coronavirus and its ancient bacterial affiliate. This similarity as well as several others may incite inquiries on whether coinfection of individuals with latent TB and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) lead to excessive fibrosis in the lungs and thus the emergence of an active TB infection. While it is well understood how TB leads to structural and immunological lung complications including granuloma formation, fibrosis, and T cell exhaustion, less is known about the disease course when coinfection with SARS-CoV-2 is present. Past and present research demonstrate that IL-10, TNF-α, IFN class I-III, TGF-β, IL-35, and Regulatory T cells (T-regs) are all important contributors of the characteristics of host response to mycobacterium tuberculosis. It has also been noted with current research that IL-10, TNF-α, IFN class I, II, and III, TGF-β, ACE-2, and T-regs are also important contributors to the host response to the SARS-CoV-2 virus in different ways than they are to the TB pathogen. Both pathogens may lead to an unbalanced inflammatory immune response, and together a shared dysregulation of immune response suggests an increased risk of severity and progression of both diseases. We have reviewed 72 different manuscripts between the years 1992 and 2021. The manuscripts pertaining to the SARS-COV-2 virus specifically are from the years 2020 and 2021. Our literature review aims to explore the biomolecular effects of these contributors to pathogenicity of both diseases along with current publications on TB/COVID-19 coinfection, focusing on the pathogenicity of SARS-CoV-2 infection with both latent and active TB, as well as the challenges in treating TB during the COVID-19 pandemic. The compiled material will then aid the latticework foundation of knowledge for future research leading to a hopeful improved system of therapeutic strategies for coinfection.

Abstract:BACKGROUND: Bronchiectasis revealed by chest computed tomography in COPD patients and its comorbid effect on prognosis have not been addressed by large-sized studies. Understanding the presence of bronchiectasis in COPD is important for future intervention and preventing disease progression.METHODS: Observational studies were identified from electronic literature searches in Cochrane library, PubMed, ScienceDirect databases, American Thoracic Society and European Respiratory Society meeting abstracts. A systematic review and meta-analysis of studies was performed to summarize the factors associated with bronchiectasis in COPD patients. Primary outcomes included the risks for exacerbation frequency, isolation of a potentially pathogenic microorganism, severe airway obstruction and mortality. Odds ratios (ORs) were pooled by random effects models.RESULTS: Fourteen observational studies were eligible for the study. Compared with COPD without bronchiectasis, comorbid bronchiectasis in COPD increased the risk of exacerbation (1.97, 95% CI, 1.29-3.00), isolation of a potentially pathogenic microorganism (4.11, 95%CI, 2.16-7.82), severe airway obstruction (1.31, 95% CI, 1.09-1.58) and mortality (1.96, 95% CI, 1.04-3.70).CONCLUSIONS: The presence of bronchiectasis in patients with COPD was associated with exacerbation frequency, isolation of a potentially pathogenic microorganism, severe airway obstruction and mortality.

Abstract:The COVID-19 pandemic has led to an unprecedented surge in hospitalised patients with viral pneumonia. The most severely affected patients are older men, individuals of black and Asian minority ethnicity and those with comorbidities. COVID-19 is also associated with an increased risk of hypercoagulability and venous thromboembolism. The overwhelming majority of patients admitted to hospital have respiratory failure and while most are managed on general wards, a sizeable proportion require intensive care support. The long-term complications of COVID-19 pneumonia are starting to emerge but data from previous coronavirus outbreaks such as severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome (MERS) suggest that some patients will experience long-term respiratory complications of the infection. With the pattern of thoracic imaging abnormalities and growing clinical experience, it is envisaged that interstitial lung disease and pulmonary vascular disease are likely to be the most important respiratory complications. There is a need for a unified pathway for the respiratory follow-up of patients with COVID-19 balancing the delivery of high-quality clinical care with stretched National Health Service (NHS) resources. In this guidance document, we provide a suggested structure for the respiratory follow-up of patients with clinicoradiological confirmation of COVID-19 pneumonia. We define two separate algorithms integrating disease severity, likelihood of long-term respiratory complications and functional capacity on discharge. To mitigate NHS pressures, virtual solutions have been embedded within the pathway as has safety netting of patients whose clinical trajectory deviates from the pathway. For all patients, we suggest a holistic package of care to address breathlessness, anxiety, oxygen requirement, palliative care and rehabilitation.

Abstract:PURPOSE OF REVIEW: Chronic obstructive pulmonary disease (COPD) and COVID-19 have many potentially negative interrelationships, which may influence the course of infection and clinical outcomes. The aim of this review is to provide clinicians with an up-to-date perspective of the complex interactions between COPD and COVID-19.RECENT FINDINGS: We consider mechanisms that could increase SARS-CoV-2 infection susceptibility in COPD, including increased ACE2 expression, reduced antiviral defence and dysfunctional immunity. We review evidence that COPD is associated with worse clinical outcomes from COVID-19 in analyses that have adjusted for confounding factors, and describe the mechanisms responsible. We discuss the use of inhaled corticosteroids in the context of susceptibility to COVID-19, and consider the impact of COVID-19 on the usual care of COPD patients.SUMMARY: The current review highlights the evidence that COPD patients have worse outcomes from COVID-19, and the multiple mechanisms responsible.

Abstract:SARS-CoV-2 infection leading to Coronavirus Disease 2019 (COVID-19) has caused more than 762 million infections worldwide, with 10–30% of patients suffering from post-acute sequelae of SARS-CoV-2 infections (PASC). Initially thought to primarily affect the respiratory system, it is now known that SARS-CoV-2 infection and PASC can cause dysfunction in multiple organs, both during the acute and chronic stages of infection. There are also multiple risk factors that may predispose patients to worse outcomes from acute SARS-CoV-2 infection and contribute to PASC, including genetics, sex differences, age, reactivation of chronic viruses such as Epstein Barr Virus (EBV), gut microbiome dysbiosis, and behavioral and lifestyle factors, including patients' diet, alcohol use, smoking, exercise, and sleep patterns. In addition, there are important social determinants of health, such as race and ethnicity, barriers to health equity, differential cultural perspectives and biases that influence patients' access to health services and disease outcomes from acute COVID-19 and PASC. Here, we review risk factors in acute SARS-CoV-2 infection and PASC and highlight social determinants of health and their impact on patients affected with acute and chronic sequelae of COVID-19.

Abstract:Coronavirus disease (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has become a real pandemic. After a median incubation period of 5 days, the disease occurs in different stages, inducing upper and lower airway responses in mild disease (80–90% of patients) and progressing to bilateral pneumonia in severe disease (10–20%) ( 1 – 3 ). A subgroup of patients with severe COVID-19 develops acute respiratory distress syndrome, requiring mechanical ventilation on intensive care. COVID-19 patients with preexisting comorbid conditions such as chronic respiratory diseases have worse disease outcomes, including a higher incidence of the need for hospitalization, ICU admission, and mortality ( 3 , 4 ). However, it remains to be further investigated how preexisting chronic inflammatory airway diseases, such as asthma and chronic obstructive pulmonary disease (COPD), and their treatment might modify the risk for SARS-CoV-2 infection and development of COVID-19. Approximately 300 million individuals worldwide have asthma ( 5 ). Considering that a significant proportion of individuals with asthma is confronted with COVID-19, it is crucial to understand which asthma patients are particularly at risk and how inhaled corticosteroids (ICS)—the cornerstone of asthma treatment—may influence morbidity and mortality associated with COVID-19. Long-term treatment with systemic corticosteroids (e.g., in transplant patients) is immunosuppressive, increasing the risk and severity of viral infections. Because of the potential risk for worse disease outcomes, the World Health Organization does not recommend systemic corticosteroid treatment in COVID-19 ( 6 ), unless if indicated for other reasons such as acute asthma or COPD exacerbations requiring a short course of oral corticosteroids. These recommendations have caused doubt and uncertainty among patients with asthma and physicians on whether ICS therapy should be maintained during this pandemic. However, withdrawal of ICS treatment puts asthma patients at risk of severe exacerbations. A recent meta-analysis on COVID-19 outcomes in patients with chronic respiratory diseases using ICS concluded that there is currently insufficient evidence to abandon the well-established ICS treatment in asthma ( 7 ). There is thus an urgent need to elucidate demographic and clinical characteristics that determine disease outcomes of COVID-19 in asthma, to investigate the impact of ICS and to unravel the underlying pathogenic mechanisms.
Bronchodilator response in Post-COVID-19 patients undergoing pulmonary rehabilitation

Abstract:Introduction: SARS-CoV-2 infections can result in a broad spectrum of symptoms from mild to life-threatening. Long-term consequences on lung function are not well understood yet. Methods: In our study, we have examined 134 post-COVID patients (aged 54.83±14.4 years) with dyspnea on exertion as a leading symptom 6 weeks to 24 months after a SARS-CoV-2 infection for bronchodilator responsiveness during their stay in our pulmonary rehabilitation clinic. Results: Prior to bronchial dilation, six out of 134 patients (4.47%) presented an FEV1/FVC ratio below lower limit of normal (Z-score=-1.645) indicative of an obstructive airway disease. Following inhalation of a ß2-adrenergic agonist we measured a mean FEV1 increase of 181.5 mL in our cohort, which was significantly elevated compared to a historical control group (ΔFEV1 = 118 mL). 28.7% of the patients showed an increase greater than 200 mL and 12% displayed a significant bronchodilation response (>200 mL ΔFEV1 and >12% FEV1 increase). Interestingly, no significant difference in bronchial dilation effect was observed when comparing patients hospitalized and those non-hospitalized during the course of their SARS-CoV-2 infection. Conclusion: Our data provides evidence for increased prevalence of obstructive ventilatory defects and increased bronchodilator responsiveness in patients with persisting symptoms after COVID-19. Depending on the extent of this complication, post-COVID patients may benefit from an adapted ß2-inhalation therapy including subsequent reevaluation.

Abstract:The emerging disease of COVID-19 was announced as a pandemic in 2020, with wide prevalence worldwide. After the duration of the pandemic of the disease, reports based on the invasion of Aspergillus into patients' lungs with COVID-19 and their hospitalization in ICU were published by the researchers, which attracted the attention of other researchers to perform such studies. In this review, PubMed, ScienceDirect, Scopus, Springer, Wiley, ProQuest, Sid, Cochrane, and the search engine Google Scholar were searched for publications by the keywords include COVID-19, Aspergillus, SARS-CoV-2, Invasive pulmonary aspergillosis, fungal and viral co-infection or a combination of them. The data were extracted and descriptively discussed. Not many studies have been performed on the association between aspergillosis and COVID-19. However, although the results of the studies suggest some possible causes of these concomitant infections like underlying diseases and prolonged mechanical ventilation, they also recommend further studies. Since the diagnosis of common fungal and viral infections is difficult, the concurrent incidence of these two infections in patients becomes a therapeutic challenge. Accordingly, this issue increased the mortality rate in this group of patients, especially in those hospitalized in ICU. According to the recently performed studies, various problems, including underlying diseases and difficult diagnostic methods of some dangerous diseases like invasive aspergillosis, are discussed in patients with COVID-19.
Bacteria and viruses and clinical outcomes of asthma‐bronchiectasis overlap syndrome: A cohort study

Abstract:Abstract Background Despite the high prevalence of co‐existing bronchiectasis and asthma (asthma‐bronchiectasis overlap syndrome [ABOS]), little is known regarding the dominant pathogens and clinical correlates. Objective To investigate the bacteria and viruses which differentially dominate in ABOS (including its subtypes) compared with bronchiectasis alone, and determine their relevance with bronchiectasis severity and exacerbations. Methods This was a prospective observational cohort study conducted between March 2017 and August 2023. We included 81 patients with ABOS and 107 patients with bronchiectasis alone. At steady‐state baseline, patients underwent comprehensive assessments and sputum collection for bacterial culture and viral detection (quantitative polymerase‐chain‐reaction). Patients were followed‐up to record exacerbation and spirometry. Results Patients with ABOS had significantly higher symptom burden and exacerbation frequency than those with bronchiectasis alone. Despite similar pathogen spectrum, the rate of bacteria–virus co‐detection increased less substantially at acute exacerbations (AE) onset than at steady‐state compared with bronchiectasis alone. Pathogenic bacteria (particularly Pseudomonas aeruginosa ) were detected fairly common (exceeding 50%) in ABOS and were associated with greater severity of bronchiectasis when stable and conferred greater exacerbation risks at follow‐up. Viral but not bacterial compositions changed substantially at AE onset compared with clinical stability. Higher blood eosinophil count, moderate‐to‐severe bronchiectasis and non‐atopy were associated with higher odds of bacterial, but not viral, detection (all p < 0.05). Conclusion Detection of bacteria or virus is associated with bronchiectasis severity or clinical outcomes in ABOS. This highlights the importance of integrating sputum microbial assessment for ascertaining the dominant pathophysiology (atopy vs. infection) and longitudinal trajectory prediction in ABOS.

Abstract:During the coronavirus disease 2019 (COVID-19) pandemic, the exploration of microecology has been essential for elucidating the intricacies of infection mechanisms and the recovery of afflicted individuals. To decipher the interplay of microorganisms between the intestinal and respiratory tracts, we collected sputum and throat swabs and feces from COVID-19 patients and explored the mutual migration among intestinal and respiratory microorganisms. Using next-generation sequencing (NGS) technology, we investigated intestinal and respiratory microorganism intermigration in two patients with severe COVID-19 during their hospitalization. Notably, we observed an expedited recovery of microecological equilibrium in one patient harboring Mycobacterium avium . Comparative analyses between 32 healthy controls and 110 COVID-19 patients with different disease severities revealed alterations in predominant microorganisms inhabiting the respiratory and intestinal tracts of COVID-19 patients. Among the alterations, intestinal Bacteroides vulgatus (BV) was identified as a noteworthy microorganism that exhibited marked enrichment in patients with severe COVID-19. BV, when highly abundant, may inhibit the transitional growth of Escherichia coli / Enterococcus , indirectly prevent the overgrowth of salivary streptococci , and maintain lung/intestinal microecology stability. In summary, this study elucidates the bidirectional microbial intermigration between the intestinal and respiratory tracts in COVID-19 patients. These findings are expected to provide new ideas for the treatment and management of COVID-19, underscoring the essential role of microecology in infectious diseases. Nevertheless, a systematic study of the roles of BV in recovery from infection is required to gain a deeper understanding of the mechanisms of microbial migration.

Abstract:In December 2019, severe acute respiratory diseases due to severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) were reported from Wuhan province of China. The symptoms associated with Coronavirus Disease-2019 (COVID-19) range from mild upper respiratory tract infection to acute respiratory distress syndrome (ARDS). It was observed that symptoms and radiological findings continued in some patients after discharge. Comorbidities such as hypertension and diabetes, risk factors such as male gender and advanced age are common risk factors for severe COVID-19 and idiopathic pulmonary fibrosis (IPF). However, the role of anti fibrotictherapy and the scientific rationale for their continuation or discontinuation in IPF patients infected with SARS-CoV-2 have not been fully defined. Data from the COVID-19 pandemic and previous coronavirus infections such as severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome (MERS) suggest that there may be significant fibrotic changes following SARS-CoV-2 infection. In this article, the frequency of on going symptoms after COVID-19 pneumonia and the frequency of interstitial lung disease that may ocur were discussed. Inaddition, the prevention of interstitial changes in the lung, especially pulmonary fibrosis, and the decrease in respiratory functions are discussed in the light of the literature.

Abstract:Several risk factors for Coronavirus-2019 (COVID-19) disease have been highlighted in clinical evidence. Among the various risk factors are advanced age, metabolic illness such as diabetes, heart disease, and diseases of the respiratory system. Cystic Fibrosis (CF) is a rare disease with autosomal recessive transmission, characterised by a lack of synthesis of the CFTR channel protein, and multi-organ clinical symptoms mainly affecting the respiratory tract with recurrent pulmonary exacerbations. In view of the pathophysiological mechanisms, CF disease should be in theory considered a risk factor for SARS-CoV2 or severe COVID-19. However, recent clinical evidence seems to point in the opposite direction, suggesting that CF could be a protective factor against severe COVID-19. Possibly, the lack of presence or function of the CFTR channel protein could be linked to the expression of the membrane glycoprotein ACE-2, a key enzyme for the endocellular penetration of SARS-CoV-2 and related to the pathophysiology of COVID-19 disease. Furthermore, CFTR channel modulating agents could indirectly influence the expression of ACE-2, playing an important role in restoring the proper functioning of mucociliary clearance and the pulmonary microbiome in the host response to SARS-CoV-2 infection. In this review, the authors attempt to shed light on these important associations of issues that are not yet fully elucidated.

Abstract:The purpose of this study is to investigate the fit of zirconia cores and all-ceramic crowns prepared with different cervical margin designs. The radius of curvature between the axial wall and the occlusal surface was set to 1 mm in an abutment using the cervical shoulder marginal design (S) and to 0.2 and 0.5 mm in abutments with round shoulders (0.2RS and 0.5RS, respectively). The internal gaps of the cores were 45-138 μm (S), 41-141 μm (0.2RS), and 43-133 μm (0.5RS). The internal gaps of the all-ceramic crowns were 40-115 μm (S), 45-113 μm (0.2RS), and 42-126 μm (0.5RS). There were no significant differences in one-way ANOVA for any region in any marginal design before and after firing the porcelain. The marginal gaps between the all-ceramic crowns and dies were 27 ± 25 (S), 30 ± 29 (0.2RS), and 24 ± 27 μm (0.5RS), again with no significant differences in one-way ANOVA.
