Abstract:Introduction Evaluating the neural correlates of sensorimotor control deficits in cervical dystonia (CD) is fundamental to plan the best treatment. This study aims to assess kinematic and resting-state functional connectivity (RS-FC) characteristics in CD patients relative to healthy controls. Methods Seventeen CD patients and 14 age-/sex-matched healthy controls were recruited. Electromagnetic sensors were used to evaluate dystonic pattern, mean/maximal cervical movement amplitude and joint position error with eyes open and closed, and movement quality during target reaching with the head. RS-fMRI was acquired to compare the FC of brain sensorimotor regions between patients and controls. In patients, correlations between motion analysis and FC data were assessed. Results CD patients relative to controls showed reduced mean and maximal cervical range of motion (RoM) in rotation both towards and against dystonia pattern and reduced total RoM in rotation both with eyes open and closed. They had less severe dystonia pattern with eyes open vs eyes closed. CD patients showed an altered movement quality and sensorimotor control during target reaching and a higher joint position error. Compared to controls, CD patients showed reduced FC between supplementary motor area (SMA), occipital and cerebellar areas, which correlated with lower cervical RoM in rotation both with eyes open and closed and with worse movement quality during target reaching. Conclusions FC alterations between SMA and occipital and cerebellar areas may represent the neural basis of cervical sensorimotor control deficits in CD patients. Electromagnetic sensors and RS-fMRI might be promising tools to monitor CD and assess the efficacy of rehabilitative interventions.

Biomechanical modeling and optimal control of human posture

Abstract:The present work describes the biomechanical modeling of human postural mechanics in the saggital plane and the use of optimal control to generate open-loop raising-up movements from a squatting position. The biomechanical model comprises 10 equivalent musculotendon actuators, based on a 40 muscles model, and three links (shank, thigh and HAT-Head, Arms and Trunk). Optimal control solutions are achieved through algorithms based on the Consistent Approximations Theory (Schwartz and Polak, 1996), where the continuous non-linear dynamics is represented in a discrete space by means of a Runge-Kutta integration and the control signals in a spline-coefficient functional space. This leads to non-linear programming problems solved by a sequential quadratic programming (SQP) method. Due to the highly non-linear and unstable nature of the posture dynamics, numerical convergence is difficult, and specific strategies must be implemented in order to allow convergence. Results for control (muscular excitations) and angular trajectories are shown using two final simulation times, as well as specific control strategies are discussed.

Abstract:Abstract Gait initiation (GI), the transient period between quiet standing and locomotion, is a functional task classically used in the literature to investigate postural control. This study aimed to investigate the influence of an experimentally-induced alteration of cervical spine mobility (CSM) on GI postural organisation. Fifteen healthy young adults initiated gait on a force-plate in (1) two test conditions, where participants wore a neck orthosis that passively simulated low and high levels of CSM alteration; (2) one control condition, where participants wore no orthosis; and (3) one placebo condition, where participants wore a cervical bandage that did not limit CSM. Centre-of-pressure and centre-of-mass kinematics were computed based on force-plate recordings according to Newton’s second law. Main results showed that anticipatory postural adjustments amplitude (peak backward centre-of-pressure shift and forward centre-of-mass velocity at toe-off) and motor performance (step length and forward centre-of-mass velocity at foot-contact) were altered under the condition of high CSM restriction. These effects of CSM restriction may reflect the implementation of a more cautious strategy directed to attenuate head-in-space destabilisation and ease postural control. It follows that clinicians should be aware that the prescription of a rigid neck orthosis to posturo-deficient patients could exacerbate pre-existing GI deficits.

A Dynamic Computational Model of Head Sway Responses in Human Upright Stance Postural Control during Support Surface Tilt

Abstract:Human and humanoid posture control models usually rely on single or multiple degrees of freedom inverted pendulum representation of upright stance associated with a feedback controller. In models typically focused on the action between ankles, hips, and knees, the control of head position is often neglected, and the head is considered one with the upper body. However, two of the three main contributors to the human motion sensorium reside in the head: the vestibular and the visual system. As the third contributor, the proprioceptive system is distributed throughout the body. In human neurodegenerative brain diseases of motor control, like Progressive Supranuclear Palsy PSP and Idiopathic Parkinson's Disease IPD, clinical studies have demonstrated the importance of head motion deficits. This work specifically addresses the control of the head during a perturbed upright stance. A control model for the neck is proposed following the hypothesis of a modular posture control from previous studies. Data from human experiments are used to fit the model and retrieve sets of parameters representative of the behavior obtained in different conditions. The result of the analysis is twofold: validate the model and its underlying hypothesis and provide a system to assess the differences in posture control that can be used to identify the differences between healthy subjects and patients with different conditions. Implications for clinical pathology and application in humanoid and assistive robotics are discussed.

Abstract:This review argues neuronal mechanisms of postural control. Multi-sensory information such as somatosensory, visual, and vestibular sensation act on various areas of the brain so that adaptable postural control can be achieved. Automatic process of postural control, which is termed as postural reflexes including head–eye coordination accompanied by appropriate alignment of body segments, is mediated by the descending pathways from the brainstem. Cooperation of the vestibulospinal, reticulospinal, and tectospinal tracts contributes to this process. On the other hand, walking in unfamiliar circumstance requires cognitive process of postural control, which depends on knowledge of self-body, such as body schema, sense of postural verticality, and body motion in space. Such a bodily cognitive information is produced at the temporoparietal cortex. They are fundamental to sustention of vertical posture and construction of motor programs. The programs then run to execute anticipatory postural adjustment which is appropriate for achievement of goal-directed movements. The basal ganglia and cerebellum may affect both the automatic and cognitive processes of postural control through reciprocal connections with the brainstem and cerebral cortex, respectively. Consequently, impairments in cognitive function in addition to damages in the motor cortex, basal ganglia, and cerebellum may disturb appropriate postural control, resulting in falling.

Abstract:The vocal tract continuously employs tonic muscle activity in the maintenance of postural configurations. Gamma-band activity in the sensorimotor cortex underlies transient movements during speech production, yet little is known about the neural control of postural states in the vocal tract. Simultaneously, there is evidence that sensorimotor beta-band activations contribute to a system of inhibition and state maintenance that is integral to postural control in the body. Here we use electrocorticography to assess the contribution of sensorimotor beta-band activity during speech articulation and postural maintenance, and demonstrate that beta-band activity corresponds to the inhibition of discrete speech movements and the maintenance of tonic postural states in the vocal tract. Our findings identify consistencies between the neural control of posture in speech and what is previously reported in gross motor contexts, providing support for a unified theory of postural control across gross and fine motor skills.

Abstract:The tonic postural system controls the equilibrium of posture when the body is completely at rest and when it is moving. It is a complicated mechanism that can call into action a variety of modalities including the assistance of the feet, the skin, the eyes, the masticatory system, the Central Nervous System and the efferent muscles, with all elements working reciprocally. Orthodontists should make a postural assessment before undertaking any treatment. If problems are found they should be dealt with by a multi-disciplinary team. But in every case orthodontists should be aware that their efforts have an effect on posture and can modify the corporal schema of the patient. Orthodontists should pay special attention to patients with scoliosis or any deformity of the spinal column.

Abstract:The cerebellum is known to control the proper balance of isometric muscular contractions that maintain body posture. Current optogenetic manipulations of the cerebellar cortex output, however, have focused on ballistic body movements, examining movement initiation or perturbations. Here, by optogenetic stimulations of cerebellar Purkinje cells, which are the output of the cerebellar cortex, we evaluate body posture maintenance. By sequential analysis of body movement, we dissect the effect of optogenetic stimulation into a directly induced movement that is then followed by a compensatory reflex to regain body posture. We identify a module in the medial part of the anterior vermis which, through multiple muscle tone regulation, is involved in postural anti-gravity maintenance of the body. Moreover, we report an antero-posterior and medio-lateral functional segregation over the vermal lobules IV/V/VI. Taken together our results open new avenues for better understanding of the modular functional organization of the cerebellar cortex and its role in postural anti-gravity maintenance.

Abstract:The literature shows that low back pain causes a reduced lumbar range of movement, affecting patients’ proprioception and motor control. Nevertheless, studies have found that proprioception and motor control of the spine and posture are vague and individually expressed even in healthy young adults. This study aimed to investigate the standing posture and its modifications induced by an instinctive self-correction manoeuvre in subacute and chronic nonspecific low back pain (NSLBP) patients to clarify how NSLBP relates to body upright posture, proprioception, and motor control and how these are modified in patients compared to healthy young adults (121 healthy young adults: 57 females and 64 males). A cohort of 83 NSLBP patients (43 females, 40 males) were recruited in a cross-sectional observational study. Patients’ entire body posture, including 3D spine shape reconstruction, was measured using a non-ionising 3D optoelectronic stereophotogrammetric approach. Thirteen quantitative biomechanical parameters describing the nature of body posture were computed. The statistical analysis was performed using multivariate methods. NSLBP patients did not present an altered proprioception and motor control ability compared to healthy young adults. Furthermore, as for healthy subjects, NSLBP patients could not focus and control their posture globally. Proprioception and motor control in natural erect standing are vague for most people regardless of gender and concurrent nonspecific low back pain. Self-correction manoeuvres improving body posture and spine shape must be learned with specific postural training focusing on the lumbar spine.

Abstract:The human pelvis is such a unique structure that enables our upper body to work so perfectly with the two legs so as to control the body's balance in the complicated postures. The aim of this study is to establish a new dynamic body sway control model in the upright standing body position in coronal plane, and to reveal the possible control mechanisms underlying the body sway with special concerns on the roles that the pelvis and its muscles are performing during the sway. The plant of control model, the dynamics of human body, includes five parts, i.e. two ankles, two hips and one lumbosacral joint, which makes up a multi-link inverted pendulum system, and is driven by two pairs of muscles, the psoas major (PM) and glutaeus medius (GM). Body sway records from eight healthy young subjects showed that the angular sway scopes of the ankle on roll (lateral) plane are 0.94 +/- 0.36 degrees (eye-open) and 1.35 +/- 0.52 degrees (eye-closed) respectively, while in lumbosacral plane, the scopes are 0.99 +/- 0.41 degrees (eye-open) and 1.27 +/- 0.72 degrees (eye-closed). The ankle and lumbosacral sways were almost in the same degree, yet their phase difference was near pi, which means that the body trunk maintains perpendicular to horizon during the upright stance. Electromyographic (sEMG) activity from GM also showed the same evidence: the activated GM was always in the same side as the deviated center-of-pressure (COP). By assuming the corrective torque of posture is regulated by PID (proportional, integral and derivative) control, the body sway can be simulated by applying human physical parameters. Our study results demonstrated that the simulated traces are consistent with the experimental record, suggesting that the pelvis is an important structure for the posture maintenance and control, and the mechanism of balance keeping control during upright stance can be approximately taken as a PID control. The result also suggests a novel means for postural stability assessment in individual in the future.

Abstract:In recent years it has been conclusively shown how the position of the mouth in relation to the body affects the way of walking and standing. In particular, occlusion, the relationship between skull and jaw, swallowing and convergence of the eyes are in neuro-muscular relationship with the control and maintenance system of posture, integrating at different levels. This manuscript aims to be a summary of all the oral, occlusal and articular dysfunctions of TMJ with systemic and postural-muscular repercussions. Recent articles found in the literature that are taken into consideration and briefly analyzed represent an important starting point for these correlations, which are still unclear in the medical field. Posturology, occlusal and oral influences on posture, spine and muscular system are still much debated today. In the literature, there are articles concerning sports performance and dental occlusion or even the postural characteristics of adolescents or children in deciduous and mixed dentition. The temporomandibular joint, as the only joint of the skull, could therefore represent a site to pay particular attention to, and in some cases an ATM dysfunction could be a clue for the diagnosis of systemic pathologies, or it could be the repercussion.

Abstract:The functional Magnetic Resonance Imaging (fMRI) has emerged as a powerful noninvasive method for collecting large amount of data about activities in human brain. Analysis for fMRI is essential to the success in detecting cognitive states. Due to very high dimensionality of feature vectors, feature extraction should be considered as a critical step to preprocess fMRI data before the stage of cognitive state detection. Up to now, different feature extraction methods have been applied to this type of data and they require domain experts to specify the Regions of Interests (Rol). However, none of them can give a dominant approach for precisely detecting cognitive states. In this paper, incremental principal component analysis (iPCA) proves to be an efficient method of feature extraction for fMRI data without using domain experts. Our experimental results show that this approach gives a higher performance compared to other feature extraction methods which require domain experts to select Regions of Interests.

Abstract:This paper presents the anatomical and biomechanical aspects of chronic instability of the craniocervical junction (CCJ) with a discussion on clinical diagnostics based on mobility tests and provocative tests related to ligamentous system injuries, as well as radiological criteria for CCJ instability. In addition to the structural instability of the CCJ, the hypothesis of its functional form resulting from cervical proprioceptive system (CPS) damage is discussed. Clinical and neurophysiological studies have shown that functional disorders or organic changes in the CPS cause symptoms similar to those of vestibular system diseases: dizziness, nystagmus, and balance disorders. The underlying cause of the functional form of CCJ instability may be the increased activity of mechanoreceptors, leading to "informational noise" which causes vestibular system disorientation. Due to the disharmony of mutual stimulation and the inhibition of impulses between the centers controlling eye movements, the cerebellum, spinal motoneurons, and the vestibular system, inadequate vestibulospinal and vestibulo-ocular reactions occur, manifesting as postural instability, dizziness, and nystagmus. The hyperactivity of craniocervical mechanoreceptors also leads to disturbances in the reflex regulation of postural muscle tone, manifesting as "general instability". Understanding this form of CCJ instability as a distinct clinical entity is important both diagnostically and therapeutically as it requires different management strategies compared to true instability. Chronic CCJ instability significantly impacts the quality of life (QOL) of affected patients, contributing to chronic pain, psychological distress, and functional impairments. Addressing both structural and functional instability is essential for improving patient outcomes and enhancing their overall QOL.

Abstract:Posture and movements are our only physical means of interacting with the environment. As we express our thoughts and emotions through posture and movements, they indicate our will or intentions. Locomotion is representative of purposeful goal-directed behaviors that are initiated by signals arising from either volitional processing in the cerebral cortex or emotional processing in the limbic system. Regardless of whether the locomotion is volitional or emotional, it is accompanied by automatically controlled movement processes such as the adjustment of postural muscle tone and rhythmic limb movements that are unconsciously executed. Sensori-motor integration at the level of the brainstem and spinal cord plays major roles in this automatic control. Signals processed in the basal ganglia and the cerebellum act on the cerebral cortex, the limbic system and the brainstem so that locomotor behaviors are appropriately and precisely regulated depending on the behavioral context. The purpose of this review is to describe how purposeful locomotor behaviors are initiated, executed and regulated so as to enable locomotive subjects to interact with and adapt to the environment.

Abstract:The active control of the lumbar musculature provides a stable platform critical for postures and goal-directed movements. Voluntary and perturbation-evoked motor commands can recruit individual lumbar muscles in a task-specific manner according to their presumed biomechanics. Here, we investigated the vestibular control of the deep and superficial lumbar musculature. Ten healthy participants were exposed to noisy electrical vestibular stimulation while balancing upright with their head facing forward, left or right to characterize the differential modulation in the vestibular-evoked lumbar extensor responses in generating multidirectional whole-body motion. We quantified the lumbar muscles activation on the right side using indwelling (deep multifidus, superficial multifidus, caudal longissimus (L4) and cranial longissimus (L1)) and high-density surface recordings. We characterized the vestibular-evoked responses using coherence and peak-to-peak cross-covariance amplitude between the vestibular and electromyographic signals. Participants exhibited responses in all lumbar muscles. The vestibular control of the lumbar musculature exhibited muscle-specific modulations: responses were larger in the longissimus (combined cranio-caudal) compared to the multifidus (combined deep-superficial) when participants faced forward (p<0.001) and right (p=0.011) but not when they faced left. The high-density surface recordings partly supported this observation: the location of the responses was more lateral when facing right compared to left (p<0.001). The vestibular control of muscle sub-regions within the longissimus or the multifidus was similar. Our results demonstrate muscle-specific vestibular control of the lumbar muscles in response to perturbations of vestibular origin. The lack of differential activation of lumbar muscle sub-regions suggests the vestibular control of these sub-regions is co-regulated for standing balance.

Analysis of three-dimensional facet joint displacement during two passive upper cervical mobilizations

Abstract:Background: Understanding the 3D-kinematics of the upper cervical spine during manual mobilization is essential for clinical examination and therapy. Some information about rotational motion is available in literature but translational components are often ignored, complicating the understanding of the complex inter-segmental motions. Objectives: This study aims to describe the amount, trajectories and reproducibility of atlanto-occipital facet joints' displacement during a flexion-extension mobilization and of the atlanto-axial facet joints during an axial rotation mobilization. Design: Original research using quantitative data. Methods: 20 fresh frozen human cervical specimens were examined with a Zebris® CMS20 ultrasound-based motion tracking system. Two physiotherapists performed regionalmobilizations in flexion-extension and axial rotation. The amount of displacement and the trajectories were calculated along the XYZ axes. Difference between measurements was evaluated with the Friedman two-way ANOVA test. Intra- and inter-rater reliability were estimated through ICC scores. Results: 3D-displacement (2.6-23.4 mm) was larger at C1-C2 during axial rotation, Atlanto-occipital flexion displayed the greatest variability in the C0 trajectory. During a right rotation, the left C1 facet moved mainly forward, and the right C1 facet moved backward. During a left rotation, the left C1 facet moved backward, while the right C1 facet moved forward. Intra-tester and Inter-tester ICCs varied between 0.5 and 0.90 (p < 0.005). Conclusions: During passive spinal motion, there is an important variability in magnitude and trajectory of joints' displacement. Nevertheless, different clinicians may be able to achieve the same position at the end of the mobilization.

Abstract:Cervical dystonia is often refractory to all forms of therapy. Many patients, however, are able to transiently abolish their spasms following a specific gesture that presumably enhances sensory input. Such observations prompted us to develop a protocol to determine if various forms of sensory stimulation could modify the motor control patterns in cervical dystonia. Surface EMG recordings of multiple neck and trunk muscles were obtained in 11 consecutive cervical dystonia patients. Baseline patterns of voluntary and involuntary muscle activation were established during a series of motor and non-motor tasks. The tasks were repeated during the application of vibratory or electrical stimulation to select muscle groups or to cutaneous and mixed nerves. Analysis of the results was made on the basis of paper and computer recordings of the data. Sensory stimulation decreased involuntary muscle activity and reduced spasms in 5 subjects. However, objective or subjective improvement usually occurred only after specific stimuli were applied to specific anatomical sites. In these cases, the protocol identified the site at which a specific sensory stimulus could be applied to control the dystonia. We conclude that selective sensory stimulation can beneficially modify cervical dystonia in some patients. Such findings warrant further investigation of the use of sensory stimulation for control of cervical dystonia.
