Abstract:Adults with inflammatory bowel disease (IBD) experience a significant decline in quality of life due to various symptoms. Exercise has emerged as a potential therapeutic approach to improve IBD management, but its effectiveness requires further investigation. This systematic review, adhering to PRISMA 2020 guidelines, explored the effects of exercise on IBD progression and its potential as a treatment in adults. A comprehensive search strategy was conducted across three databases and two registries from May 12, 2024, to May 22, 2024. Methodological rigor and potential bias were minimized through quality assessment using the Cochrane risk-of-bias tool 2 (RoB 2) for randomized controlled trials (RCTs), the Newcastle-Ottawa Scale (NOS) for cohort studies, and the Joanna Briggs Institute (JBI) critical appraisal checklist for studies evaluating the effectiveness of non-randomized interventions. This process yielded 12 high-quality studies for analysis. The review identified positive evidence from both RCTs and observational studies. Exercise interventions demonstrated improvements in cardiorespiratory fitness, disease activity, quality of life, and mental health in adults with IBD. Studies explored various modalities, including aerobic exercise, resistance training, and mind-body interventions. However, further research is needed to optimize exercise prescription and elucidate the underlying mechanisms of action. This review strengthens the evidence for exercise as a beneficial intervention for IBD patients. Personalized exercise programs based on individual needs hold promise for improved IBD management and patient outcomes. However, limitations exist due to study design variations and the need for long-term follow-up studies.

Abstract:Background This study investigated selected inflammatory responses to acute and chronic exercise in individuals with inflammatory bowel disease (IBD). Methods A systematic review and meta-analysis was conducted on all relevant exercise-based intervention publications with IBD participants. The study included articles that utilised a broad range of acute and chronic exercise interventions, with inflammatory biomarkers measured and symptoms documented, both pre- and post-exercise for those with IBD. The search was limited to studies published in English, the use of human participants, and primary studies, with no restrictions on date of publication or participant’s age. Articles were retrieved through the electronic databases: PubMed, SPORTDiscus, and Scopus. This study adhered to Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Results Six inflammatory markers were included in the meta-analysis which consisted of five studies. Exercise interventions resulted in no significant difference in IL-6 ( SMD = -0.09; 95% CI = -0.49, 0.30; P = 0.64), TNF-α ( SMD = 0.08; 95% CI = -0.31, 0.48; P = 0.68), CRP ( SMD = -0.04; 95% CI = -0.58, 0.50; P = 0.89), IL-17 ( SMD = 0.15; 95% CI = -0.45, 0.76; P = 0.62), leukocytes ( SMD = 0.40; 95% CI = -0.53, 1.33; P = 0.40) or lymphocytes ( SMD = 0.32; 95% CI = -0.33, 0.97; P = 0.33), thus, indicating exercise may have no effect on inflammatory markers in IBD. Bowel symptoms improved following regular moderate exercise that incorporated stress management. Conclusion Heterogeneity among the identified literature may have led to exercise interventions being ineffective in reducing inflammation. Although the limited number of eligible studies may reduce the reliability of results, it emphasises the need for additional research in this domain. Importantly, no adverse symptomatic responses to exercise indicate that exercise is safe for IBD patients.

Structured physical activity interventions as a complementary therapy for patients with inflammatory bowel disease – a scoping review and practical implications

Abstract:<h3 class="Heading">Background</h3><p class="Para">Patients with Inflammatory Bowel Disease (IBD) also suffer from a wide range of additional disorders, which may be caused by the disease, the side effect of the medication, or a lack of physical activity (PA). This results in reduced physical and psychological wellbeing. However, as known from other chronic diseases exercise could be utilized as supportive therapy for IBD patients. Main goals of this article are (a) collecting data of the effects structured physical activity interventions have on validated clinical parameters of IBD and health related symptoms, (b) developing activity recommendations for this clientele.</p><h3 class="Heading">Methods</h3><p class="Para">A scoping review was conducted, searching for relevant articles published until May 2018, which investigated the effects of structured exercise interventions in IBD patients. The heterogeneity of the outcomes and the interventions did not support a quantitative synthesis thus, a qualitative discussion of the studies is provided.</p><h3 class="Heading">Results</h3><p class="Para">After reviewing 353 records, 13 eligible articles were identified. Five studies investigated aerobic exercise, three studies resistance exercise, three studies mind-body therapies and two studies yoga. The quality of the studies is mixed, and the duration is rather short for exercise interventions. Only few studies assessed validated IBD activity markers or inflammatory biomarkers. Nevertheless, the patients showed an increase in fitness, bone mineral density (BMD), quality of life and a decrease of IBD induced stress and anxiety. No severe adversial events were reported.</p><h3 class="Heading">Conclusion</h3><p class="Para">Even though the evidence is limited the application of exercise interventions in IBD patients can be assumed to be safe and beneficial for the patients' overall-health, and IBD specific physical and psychosocial symptoms. But there is still a high demand for more thoroughly conducted studies, focussing on important clinical outcome parameters.</p>

Abstract:Abstract Background Moderate physical activity (PA) appears to be beneficial for inflammatory bowel disease (IBD), improving the symptoms of the disease and promoting the maintenance of remission. However, little is known about the impact of different types of PA on IBD activity. Methods IBD patients with stable treatment without steroids for 4 months were prospectively included and randomized into 3 groups: muscle strengthening exercises, aerobic exercises, and a control group. The impact of a 10-week training period with 2 sessions per week was evaluated. Following parameters were collected: clinical activity of the disease (Mayo score for ulcerative colitis or UC, Harvey-Bradshaw index or HBI for Crohn’s disease or CD, patient reported outcome or PRO for both), Godin Leisure Time Exercise Questionnaire or GLTEQ (assessing PA level), Metabolic Equivalent Task or MET (assessing sedentarity level), barriers to PA, quality of life assessment (by EuroQol 5 dimensions or EQ5D and Short health scale or SHS), Inflammatory Bowel Disease Fatigue (IBD-F), physical abilities according to exercise stress test and maximum strength test. Results Between January 2021 and September 2022, a total 33 patients were enrolled in the program. Of these, 24 patients (13 women, 17 CD, median age of 31.6 [IQR, 29.4- 46.4]) completed the program and were included in the analysis (dropout rate of 18.2%). Table 1 shows the characteristics of these patients at inclusion. Eight, six and ten patients were respectively randomized in the muscle strengthening, aerobic exercise and control groups. After the training period, 50% of patients in the muscle strengthening group and 66.7% of patients in the aerobic exercise group showed a clinical improvement (according Mayo score and HBI). Strengthening was associated with significant reduction in PRO rectal bleeding in UC patients (p<0.0001) and aerobic exercise was associated with a significant lowering of HBI (p=0.039). Among the 24 patients, 2 CD patients relapsed in the muscle strengthening group during the training period. Aerobic exercise significantly reduced the barriers to PA (p=0.037). In neither group did we find any positive or negative impact of PA on quality of life or fatigue. Strengthening exercise and aerobic exercise significantly improved VO2max (maximal oxygen consumption) and Pmax in the exercise stress test, respectively. Most of the maximum strength tests were improved in the muscle strengthening group. Conclusion PA was well tolerated and associated with an improvement of disease activity in the majority of the IBD patients. There may be a variable impact of different types of PA in CD and UC, which should be explored further in larger cohorts.

Abstract:Abstract Objective Cognitive behavioral therapy (CBT) is now included in the treatment of patients with inflammatory bowel disease (IBD) in many settings. However, different clinical trials report different outcomes without consensus. This study aims to evaluate the impact of CBT on the mental state, quality of life and disease activity of patients with IBD. Design Systematic review. Methods This systematic review searched eligible studies from 1946 to December 8, 2019, in MEDLINE, EMBASE, CINAHL, Cochrane library, ClinicalTrials.gov, PsycINFO, Web of Science for eligible randomized controlled trials (RCT). Results Among the initial identified 1807 references, 11 studies met inclusion criteria. CBT was shown to improve patient's quality of life and reduce the level of depression and anxiety post-intervention but was not sustained. Evidence is not enough for the effect of CBT on disease activity, or C-reactive protein level. Conclusions CBT has shown short-term positive psychological effects on IBD patients, but there is insufficient evidence for sustained physical and psychological improvements of IBD patients. Can exercise affect the course of inflammatory bowel disease? Experimental and clinical evidence

Abstract:The inflammatory bowel disease (IBD) consisting of Crohn's disease (CD) and ulcerative colitis (UC) are defined as idiopathic, chronic and relapsing intestinal disorders occurring in genetically predisposed individuals exposed to environmental risk factors such as diet and microbiome changes. Since conventional drug therapy is expensive and not fully efficient, there is a need for alternative remedies that can improve the outcome in patients suffering from IBD. Whether exercise, which has been proposed as adjunct therapy in IBD, can be beneficial in patients with IBD remains an intriguing question. In this review, we provide an overview of the effects of exercise on human IBD and experimental colitis in animal models that mimic human disease, although the information on exercise in human IBD are sparse and poorly understood. Moderate exercise can exert a beneficial ameliorating effect on IBD and improve the healing of experimental animal colitis due to the activity of protective myokines such as irisin released from working skeletal muscles. CD patients with higher levels of exercise were significantly less likely to develop active disease at six months. Moreover, voluntary exercise has been shown to exert a positive effect on IBD patients' mood, weight maintenance and osteoporosis. On the other hand, depending on its intensity and duration, exercise can evoke transient mild systemic inflammation and enhances pro-inflammatory cytokine release, thereby exacerbating the gastrointestinal symptoms. We discuss recent advances in the mechanism of voluntary and strenuous exercise affecting the outcome of IBD in patients and experimental animal models.

Abstract:Abstract Background Sarcopenia is prevalent in patients with IBD (inflammatory bowel disease). This study aimed to assess the effects of an 8-weeks combined aerobic and resistance training program on the quality of life, disease activity, muscle strength, and aerobic capacity in patients with IBD. (Registration ID: NCT05455762) Methods This study was a single-blind parallel-group randomized clinical trial. Inclusion criteria were patients 18 to 65 years old with a confirmed diagnosis of IBD. Exclusion criteria: active malignancy, consumption of systemic corticosteroids, and patients with moderate to severe physical activity more than 150 minutes/week. Patients were allocated to each group by block randomization with a 1:1 ratio. The intervention consisted of walking sessions on even days and six resistance exercises on odd days. The primary outcome was the patients' quality of life, measured by the Persian version of the ibdq-9 questionnaire. secondary outcomes included disease severity, muscle strength, 6-minute walk test, and 30-second sit-to-stand test. We measured muscle strength by a handheld dynamometer. Results Of 72 patients screened 64 were eligible for the study. During the follow-up period, 13 patients, 8 from the exercise group and 5 from the control group, were excluded from the study. 51 patients, 28 females and 23 males, with a mean age of 34.9 ± 10.57 were analyzed. Two groups were matched in demographic and IBD-related characteristics. Unlike the control group, the exercise group showed a significant increase in overall quality of life (40.29 to 47.75, p=0.005), particularly in intestinal and systemic symptoms domains (p=0.01 and p=0.004). the inter-group analysis found no significantly higher mean difference in QOL scores between the exercise and control groups (p=0.053). The Partial Mayo score in patients with ulcerative colitis (UC) significantly decreased in the intervention group but not in the control group. The Harvey-Bradshaw index in Crohn patients didn’t change in either group. There were no exercise-related serious complications. The 6-minute walk test distance significantly increased in the exercise group but not in the control group. The 30-second sit-to-stand test didn’t significantly change in either group. Of muscles examined (hand grip, shoulder abductor, hip flexor, knee extensor, and elbow flexor) only knee extension strength increased in both groups. Conclusion Combined resistance and aerobic exercise in patients with IBD is safe and can significantly improve the quality of life and aerobic capacity. This regimen also improved clinical severity in patients with UC but not in Crohn’s. Muscle strength did not significantly improve when comparing the two groups, possibly due to the short duration of the intervention.

Abstract:Inflammatory bowel diseases (IBDs), including both Crohn's disease (CD) and ulcerative colitis (UC), are chronic autoimmune diseases. Both CD and UC have relapsing and remitting courses. Although effective medical treatments exist for these chronic conditions, some patients do not respond to these traditional therapies. Patients are often left frustrated with incomplete resolution of symptoms and seek alternative or complementary forms of therapy. Patients often search for modifiable factors that could improve their symptoms or help them to maintain periods of remission. In this review, we examine both the published evidence on the benefits of exercise clinically and the pathophysiological changes associated with exercise. We then describe data on exercise patterns in patients with IBDs, potential barriers to exercise in IBDs, and the role of exercise in the development and course of IBDs. While some data support physical activity as having a protective role in the development of IBDs, the findings have not been robust. Importantly, studies of exercise in patients with mild-to-moderate IBD activity show no danger of disease or symptom exacerbation. Exercise has theoretical benefits on the immune response, and the limited available data suggest that exercise may improve disease activity, quality of life, bone mineral density, and fatigue levels in patients with IBDs. Overall, exercise is safe and probably beneficial in patients with IBDs. Evidence supporting specific exercise recommendations, including aspects such as duration and heart rate targets, is needed in order to better counsel patients with IBDs.

Abstract:Increased nigral iron content in the parkinsonian brain is now well documented and is implicated in the pathogenesis of this movement disorder. Free iron in the pigmented DA-containing neurons catalyze DA autoxidation and Fenton reaction to produce cytotoxic .OH, initiating lipid peroxidation and consequent cell damage. The present results clearly demonstrate that a regional increase in the levels of the "labile iron pool" can result in the degeneration of dopaminergic nigral neurons as reflected by a significant inhibition in the expression of tyrosine hydroxylase mRNA and DA depletion. Iron-complex-induced damage of dopaminergic neurons in the substantia nigra, might have resulted from a sequence of cytotoxic events including the .OH generation and lipid peroxidation as demonstrated in this study. This free-radical-induced oxidative nigral injury may be a reliable free-radical model for studying parkinsonism and may be relevant to idiopathic Parkinson's disease. This apparent nigral injury stimulated by Fe(2+)-citrate is more severe than that produced by ferric iron and its citrate complex. Moreover, these data indicate that Fe(2+)-citrate is as potent as MPP+ in causing oxidative injury to the substantia nigral neurons. However, the nigral toxicity of MPTP and its congeners are not progressive, while Fe(2+)-citrate complex may produce a progressive degeneration of the nigrostriatal neurons which is similar to the progression of ideopathic Parkinson's disease. Thus, this unique Fe(2+)-citrate complex animal model could be used for studying neuroprotective treatments for retarding or halting the progressive nigrostriatal degeneration caused by free radicals in the iron-rich basal ganglia.
The Effect of Probiotics on Treatments in Patients Affected by Inflammatory Bowel Disease: A Systematic Review

Abstract:Background: Gut dysbiosis has been associated with development and progression of inflammatory bowel disease (IBD). Over the last two decades, many publications have focused on the role of probiotics in IBD. However, studies regarding the treatment of IBD with probiotics have not yielded consistent results. Objective: To evaluate the efficacy and safety of probiotics in IBD therapy. Results: A total of seven studies met the criteria and were included in the meta-analysis, which included 879 participants. We finally selected five endpoints to evaluate the efficacy of probiotics, 3 indicators showed the statistical difference, including C-reactive protein (CRP)(OR: -2.45, 95% CI: -3.16, -1.73, P< 0.01), the number of fecal Bifidobacterium (OR: 3.37, 95%CI: 3.28, 3.47, P< 0.01) and Lactobacillus(OR: 2.00, 95%CI: 1.91, 2.09, P< 0.01) . The other two indicators including the disease activity of Crohn’s Disease (CD) (OR: -0.17, 95%CI: -1.46, 1.12, P=0.80) and ulcerative colitis(UC) (OR: -0.31, 95%CI: -1.36, 0.74, P=0.56) showed no statistical difference, while the OR reflected the beneficial relationship between probiotics and IBD. Conclusion: Supplementation of probiotics can improve clinical symptoms in IBD patients, reduce the serological inflammation level, and regulate the intestinal microenvironment, but more RCTs are needed.
P744 Evaluation of a lifestyle program based on physical activity on quality of life and fatigue in patients with Inflammatory Bowel Disease: a pilot study

Abstract:Abstract Background Patients with inflammatory bowel disease (IBD) tend to be less physical active, whilst maintaining an active lifestyle has been associated with enhanced disease control, diminished fatigue and improved quality of life. The primary objective was to evaluate the potential impact on physical fitness, fatigue, quality of life and disease control of a specially designed exercise program for IBD patients. Methods Patients with IBD, treated at two teaching hospitals in the southwest of the Netherlands, participated in a 16-week exercise program (IBD-Fit) between March until June 2023. The program was designed especially for patients with IBD to sport with peers and was adjusted to a patient's fitness level. Improvement of physical fitness (measured with body composition, grip strength, sit-and-reach test, Y-balance test and 6-minute walk test) was analysed. In addition, patients were asked to fill in validated questionnaires at baseline and at the end of the program (IBD-Quality of life, IBD-Fatigue and IBD-control), to assess the effect of the exercise program on quality of life, fatigue and disease control. For statistical analyses, a paired t-test or Wilcoxon matched paired rank test was used. Results A total of 27/32 patients completed the entire exercise program, with a mean age of 50.4 years (SD 12.5), 40.7% were male, 48.1% had Crohn’s disease and 51.9% had ulcerative colitis (table 1). After 16 weeks, body composition, such as BMI and muscle mass improved compared to baseline; fat percentage significantly decreased (32.3 to 29.7%; Z=-2.983; P=0.003). Physical fitness improved significantly, based on the sit-and-reach test (23 to 29 cm; Z=-3.03; P=0.002), the absolute reach distance of the Y-balance test (73.7 to 79 cm; Z=-2.77; P=0.006), and the mean distance during a 6-minute walk test (505.2 to 557.5 m; 95%CI 32.6 – 72.2; P<0.001). Grip strength showed no significant improvement. Quality of life improved on average with 8.0 points on the IBDQ. The Systemic Symptoms domain improved significantly (mean difference 2.7; 95%CI 0.1–5.4, P=0.043). The program significantly improved fatigue scores (Z = -2.296, P=0.022). Disease control showed no significant improvement after the program. Overall, patients were very satisfied with the exercise program. Average rating was 8.6 out of 10. Conclusion This pilot study in a small number of patients shows that a specially designed exercise program has a positive effect on IBD patients. These results underline the importance to broaden our standard of care and can be used as a steppingstone to implement the program on a larger scale and ultimately become part of the standard of care for patients with IBD.

Anti‐neutrophil cytoplasm antibodies in rheumatoid arthritis

Abstract:Anti‐neutrophil cytoplasm antibodies (ANCA) occur occasionally in rheumatoid arthritis (RA), but their incidence and clinical significance have been unclear. In this study we have investigated 58 patients with RA. In 22 patients the disease was inactive and the remaining 36 with active disease were further subdivided into those without clinical evidence of vasculitis (26), those with cutaneous vasculitis (8) and those with systemic vasculitis (2). ANCA were demonstrated by indirect immunofluorescence in 10 of the 58 patients (17%), While both pcrinuclear (pANCA) and cytoplasmic (cANCA) staining were detected, pANCA were more common (70%). Neutrophil‐specific anti‐nuclear antibodies (ANNA) were demonstrated in a further eight sera (14%) and ANA were detected on Hep‐2 cells in 30 of the 58 sera (52%). ELISAs for the detection of anti‐myeloperoxidase and anti‐elastase antibodies were then established. Five sera with pANCA and five that contained ANNA were negative for both anti‐myeloperoxidase and anti‐elastase antibodies, suggesting other as yet unidentified cytoplasmic antigens as the target molecules. However, anti‐myeloperoxidase or anti‐elastase antibodies were found in four sera that had homogeneous or speckled ANA on both Hep‐2 cells and neutrophils. One serum contained both antibodies. The presence of ANCA detected by indirect immunofluorescence or of anti‐myeloperoxidase or anti‐elastase antibodies in these patients with RA was not associated with disease activity nor with the demonstration of cutaneous vasculitis or renal disease (P NS). A possible association with systemic vasculitis remains to be confirmed. There is an incomplete correlation between indirect immunofluorescence patterns and antibody specificity in ELISA systems.

Abstract:Introduction Biologic and small-molecule therapies have revolutionised the treatment of moderate-to-severe inflammatory bowel disease (IBD). A significant proportion of patients experience early or delayed treatment failure. Patients with IBD with greater visceral obesity are less likely to respond to biologics. Sarcopenia has been identified as a predictor of disease severity and need for rescue therapy in acute severe ulcerative colitis. The aim of this study is to assess the feasibility of a physician-derived exercise programme in patients with IBD commencing biologic or small-molecule therapy in addition to the effect on physical fitness, body composition and objective measures of quality of life, fatigue scores and disease activity. Methods and analysis This is a randomised controlled feasibility study comparing the effects of a physician-derived exercise programme and standard medical care (biologic/small-molecule therapy) with standard care alone in patients with moderate to severe IBD. Patients with IBD in the intervention group will undergo a structured exercise programme for 20 weeks. Both IBD groups will carry out body composition, disease activity and quality-of-life assessments at baseline, week 12 and week 26. The primary objective is to assess the feasibility of the physician-derived exercise programme in patients with IBD commencing disease-modifying therapies. Secondary endpoints include a change in cardiorespiratory fitness, disease activity/inflammation, fatigue, health-related quality of life outcomes and body composition between the two IBD groups. Exploratory endpoints include validation of anterior thigh ultrasound for sarcopenia screening, assessment of proinflammatory cytokines and markers of immunometabolism. Ethics and dissemination This study has received ethical approval from the Beaumont Hospital Ethics committee on 22 October 2021 (reference number 21/21). Data generated or analysed during this study will be published as an article and supplementary appendix in relevant medical journals. Abstract:Abstract Background Inflammatory bowel disease (IBD), Crohn’s disease, and ulcerative colitis are chronic autoimmune lifelong diseases with fluctuating activity over time. The treatment includes medical therapy and surgery, however, there is no definite cure. Therefore, the quest for new and supplementary treatment options is imperative to improve patients’ general health and quality of life. Physical activity and exercise have been suggested to be elements in both the prevention and supplementary treatment of IBD; however, this is based on limited underpowered trials. Thus, the role of exercise as a treatment option still has to be settled. We aim to investigate the effect of a 12-week exercise intervention in adult patients with moderately active IBD on three categories of outcomes (1) disease-specific health-related quality of life (IBDQ); (2) general health status of the patients, i.e., waist circumference, disease activity by clinical scorings systems (Harvey Bradshaw Index, Simple Clinical Colitis Activity Index), blood pressure, blood lipids, and non-disease specific quality of life (EQ5D) scores; and (3) explorative outcomes on biomarkers (C-reactive protein and fecal calprotectin) plus different biomarkers of immunology (cytokine panel). Methods We will apply a superiority design in this open-label randomized clinical trial including 150 patients equally allocated to intervention and usual care. The intervention will be based on a 12-week aerobic exercise program and will include two supervised exercise sessions of 60 min per week, combined with one weekly home training session. We have defined a moderate exercise level as 60–80% of patients’ maximum heart rate. The patients in the intervention group will also be offered an online video lesson of 15–25 min on lifestyle guidance, and the same online video lesson will be offered in the comparator group. Questionnaires on quality of life will be forwarded electronically both at inclusion and at the end of the study, and the patients will have blood samples, and fecal samples for calprotectin at baseline, weeks 4 and 8, as well as after 12 weeks (study end). Discussion This will be a clinical trial investigating the effect of exercise on patients with Crohn’s disease and ulcerative colitis. This trial will add to the evidence on the possible effect of exercise and might clarify whether exercise can benefit as a supplementary treatment addendum. Thus, the trial may provide a new patient-active disease management approach. Trial registration ClinicalTrials.gov NCT04816812. Date of first registration: March 23, 2021.

Abstract:Abstract Background Individuals with quiescent inflammatory bowel diseases (IBD) can continue to have several symptoms. Physical activity (PA) can improve immunological response and psychological health. Purpose We performed a systematic review of trials investigating the safety and efficacy of PA as an adjunct therapy to manage persistent symptoms in quiescent or mildly active IBD. Method We searched for randomised controlled trials (RCTs) and nonrandomised non-controlled trials (non-RCTs) in eight databases, trial registries and conference proceedings, published in English between 2011 and 2021. We focused on trials in adults (>18 years) using any form of PA as an adjunct therapy to medical therapy. Outcomes of interest were health-related quality of life, fatigue, joint pain, abdominal pain, stress, anxiety, and depression. Result(s) From 10,862 retrieved citations, we identified seven RCTs and one non-RCT that met our inclusion criteria. All trials deemed PA safe for individuals with quiescent or mildly active IBD. Clinical heterogeneity was noted among the trials for all outcomes. Six RCTs utilized parallel-group design while one utilized a cross-over design. Seven trials provided partially or fully supervised PA interventions, and one provided no supervision. All trials used different types of PA, which varied between running, resistance training, yoga, and aerobic exercises. The trials used different parameters to measure PA intensity including Peak Power Output, Resistance Intensity Scale for Exercise, percentage of maximum heart rate, ability to talk while running. Heterogeneity was noted in the duration and frequency of PA. Conclusion(s) Even though PA is safe for individuals with quiescent or mildly active IBD, heterogeneity in available trials make it difficult to ascertain precise estimates of treatment effect. This also presents a challenge when determining the specific modes of PA that are beneficial for individuals with quiescent or mildly active IBD. This highlights the need for standardization of the definitions of parameters involved in PA in IBD research. Abstract:1. The presence of lipoprotein lipase has been confirmed in acetone-ether extracts of human adipose tissue. 2. The properties of the enzyme appear to be similar to that in rat adipose tissue though it is less active on a weight basis. 3. The presence of a separate monoglyceride lipase has been demonstrated in human adipose tissue. 4. In eluates of human adipose tissue the activity of lipoprotein lipase is dependent on heparin, whereas that of the monoglyceride lipase does not require heparin. 5. No consistent effect of insulin in vitro on the enzyme in human adipose tissue has been demonstrated. Possible reasons for this are discussed.
Perceptions, behaviours and barriers towards exercise practices in inflammatory bowel disease

Abstract:Inflammatory bowel disease (IBD), a chronic disease affecting the digestive tract, has a significant impact on health-related quality of life. Pharmaceutical treatment is typically adopted, yet exercise is increasingly becoming recognized as an adjunct therapy. This study aimed to explore the perspectives, behaviours, and barriers of IBD patients in terms of their exercise habits. A 16-item closed-ended questionnaire was completed by 463 adult IBD patients (Ulcerative colitis = 57.02%, Crohn’s dis-ease = 40.60% and Other = 2.38%) (Female = 76.67%, Male = 22.46 and Non-binary = 0.86%). The questionnaire was divided into three sections: baseline/demographic characteristics, disease characteristics, and exercise perceptions, beliefs, and behaviours. Significantly (P<0.001) more participants (63.07%) reported that they engage regularly with exercise compared to those who do not; however, engagement was significantly lower in female patients (59.72%) compared to males (74.04%). Respondents also rated significantly (P<0.001) that a combination of factors prevents engagement in exercise (74.30%). Moderate intensity exercise was the predominant (P<0.001) aerobic modality (39.04%), the majority (P<0.001) response was that patients undertake no resistance training (27.74%), and significantly more (P<0.001) patients indicated that they don’t know whether resistance training can influence IBD either positively (57.53%) or negatively (62.33%). Whilst it is encouraging that IBD patients are engaging regularly with exercise, the reduced levels of engagement in females and lack of knowledge/ engagement with resistance training, indicate that future implementation and educational developments are necessary to enhance exercise in females and resistance training engagement in all IBD patients.

Abstract:BACKGROUND: Improving health-related quality of life is a primary target of therapy for patients with inflammatory bowel disease. Physical activity has been demonstrated to improve health-related quality of life in several patient populations with chronic disease. There are very few studies investigating the effects of physical activity on health-related quality of life in inflammatory bowel disease. The primary purpose of this study is to investigate the effects of 10 weeks of moderate physical activity on health-related quality of life in patients with inflammatory bowel disease.METHODS: Thirty patients with mild to moderate IBD (Crohn's Disease Activity Index (CDAI) <220 or Rachmilewitz Index (RI) <11) were randomized 1:1 to either supervised moderate-intensity running thrice a week for 10 weeks or a control group who were not prescribed any exercise. Health-related quality of life, symptoms, and inflammation were assessed at baseline and after 10 weeks.RESULTS: Participants were 41 ± 14 years (73% female), had a body mass index of 22.8 ± 4.1 kg/m(2), and an average CDAI or RI of 66.8 ± 42.4 and 3.6 ± 3.1. No adverse events occurred during the 10-week training period. Health-related quality of life, reported as IBDQ total score, improved 19% in the intervention group and 8% in the control group. Scores for the IBDQ social sub-scale were significantly improved in the intervention group compared with controls (ΔIBDQsocial = 6.27 ± 5.46 vs. 1.87 ± 4.76, p = 0.023).CONCLUSION: Patients suffering from moderately active IBD are capable of performing symptom-free regular endurance exercise. Our data support the assumption that PA is feasible in IBD patients. PA may furthermore improve quality of life through improvements in social well-being, and may, therefore, be a useful adjunct to IBD therapy.

Abstract:INTRODUCTION & AIMS Inflammatory bowel disease (IBD) and irritable bowel syndrome (IBS), are characterised by intestinal inflammation that presents severe symptoms and continued complications throughout the lifespan. These gastrointestinal (GI) disorders have a growing prevalence worldwide with no known cure. Management strategies often require a ‘trial and error’ process before an effective pharmacal/s is established; the successful interim varies considerably with prevalent adverse side-effects and gradual decline in the effectiveness of medication. Accordingly, alternative mechanisms to complement management options, reduce side-effects, and improve well-being warrant investigation. Physical activity is known to be beneficial for a variety of inflammatory conditions; the advantageous effects are associated with the exercise-induced inflammatory response directly from active muscle tissue. Despite this knowledge, minimal studies have attempted to quantify the inflammatory effect of exercise. This study investigates the inflammatory response of acute exercise. METHODS Participants were allocated into one of two groups: GI disorders (IBD & IBS; n = 11) or apparently healthy control (AHC; n = 11). All participants completed three sessions: 1) initial familiarisation: questionnaires, anthropometry measures, DEXA scan, and a graded exercise test; 2) protocol 1: moderate-intensity continuous exercise; and 3) protocol 2: alternating high- and low-intensity exercise. Participants were randomly allocated to the acute exercise protocols. Each protocol had a fasted pre-exercise venous blood sample, 35-minutes cycle, fasted blood sample immediately post-, 2hrs post-, and 24hr post-exercise. RESULTS Significant differences were observed between groups in body mass index, waist and hip circumstances, percentage of fat mass, and VO2peak (p < .05). Analyses of venous blood samples will be presented. CONCLUSION With no known cure, lifestyle management is important for those with GI disorders. Exercise hosts several benefits; however, given the inflammatory nature of the condition, the acute responses of exercise are important for clinicians working with IBD/IBS patients.
