Abstract:Breath test might be used as a new mode, non-invasive, and convenient diagnostic method, which can make the assessment of disease through detecting the range of the VOCs exhaled or consumed by the patients, and reflecting the metabolism of the corresponding tissue cell. Considering the location, lung cancer is closely related to VOCs, therefore, to find out the lung cancer-associated specific VOCs might be provided us a new idea and target for early diagnosis of lung cancer. The study confirmed that there are stable conclusion and good repeatability to analyze special VOCs from exhaled breath by SPME-GCMS. We analyzed exhaled breath VOCs of 243 samples by SPME-GCMS system, including 88 samples of lung cancer, 70 samples of lung benign disease, 85 samples of health person, and further researched the sensitivity and specificity of the diagnostic models based on different combinations of special VOCs by applying statistical approaches, such as cluster analysis or stepwise discriminant analysis. In the end, we selected age, as well as 4 to 6 VOCs, as the diagnostic models for lung cancer, including 2,6-dimethylnaphthalene, palmitic aldehyde, nonadecyl alcohol, etc, basically with high sensitivity and specificity more than 85% . Take one model for example, which is consisted of palmitic aldehyde, nonadecyl alcohol, 2,6-dimethylnaphthalene, age, and so on, its specificity is 94.9%, and 78.7% of early stage lung cancer patients were correctly identificated, however, 31.9% of them were misclassificated to advanced stage. The study found that several special VOCs expression are associated with pathological classification, lesion locations, surgery and smoking situation.
Optimization of Volatile Markers of Lung Cancer to Exclude Interferences of Non-Malignant Disease

Abstract:BACKGROUND: Breath analysis became promising for noninvasive diagnosis of cancer with sophisticated spectrometry technology introduced.OBJECTIVE: This study aimed to select volatile markers for lung cancer detection, which exclude the influences from nonmalignant lung diseases.METHODS: 171 subjects who were divided into three groups: patients with LC, patients with PNMD and healthy controls were enrolled in our studies as training cohort. The volatile organic compounds (VOCs) in their breath samples were analyzed with solid-phase micro-extraction/gas chromatography/mass spectrometry (SPME-GCMS). Markers were selected by receiver operating characteristic (ROC) curves. After that, 78 subjects with high morbidity of LC were employed as validation cohort. Their breath samples were analyzed by thermal desorption instrument/gas chromatography/mass spectrometry (TD-GCMS).RESULTS: Through a series of comparisons among lung cancer patients, pulmonary non-malignant diseases patients, and healthy participants in training cohort, Nonane, 5-(2-methyl-)propyl-; phenol, 2,6-di-tert-butyl-, 4-methyl-; dodecane, 2,6,11-trimethyl-; hexadecanal and pentadecane, 8-hexyl- were selected as markers for lung cancer diagnosis. Principal component analysis was employ to process data from validation cohort. As results, satisfied distinctions have been obtained with detection of these five selected markers, although the detection method is not identical with that used for training cohort.CONCLUSIONS: In conclusion, with optimization method described in this paper, breath test could be an effective method for diagnosis of lung cancer and avoid the interference of pulmonary non-malignant diseases.
A Study of the Volatile Organic Compounds Exhaled by Lung Cancer Cells in Vitro for Breath Diagnosis

Abstract:BACKGROUND. The specific volatile organic compounds (VOCs) exhaled by lung cancer cells in the microenvironment are the source biomarkers of lung cancer and also serve as direct evidence that the diagnosis of lung cancer by breath is possible. However, to the authors' knowledge, few articles published to date have provided accurate VOCs in the microenvironment, thereby leading to different points of view with regard to searching for biomarkers in the breath from lung cancer patients In this article, an innovative pathologic analysis method Of lung cancer and the early diagnosis of lung cancer at the cellular level were introduced for this purpose.METHODS. Solid-phase microextraction combined with gas chromatography is used as the detection system to determine the VOCs in the culture medium of several target cells, including different kinds of lung cancer cells, bronchial epithelial cells, tastebud cells, osteogenic cells, and lipocytes. As a result, each kind of cells has a unique chromatogram. There are 4 special VOCs that were found to exist in all culture mediums of lung cancer cells, which are the metabolic products of lung cancer cells and can be viewed as markets of lung cancer.RESULTS. The authors were able to determine a correlation between VOCs in the metabolic products of lung cancer cells and VOCs in the breath of lung cancer patients, some of whom had stage I and 11 disease, and eventually hope to certify the biomarkers in the breath of lung cancer patients.CONCLUSIONS. This research is significant and provides the basis for the noninvasive detection and the breath diagnosis of lung cancer using an electronic nose.

Abstract:Abstract Background: Breath analysis is a promising noninvasive screening technique that offers a wide range of probable possibilities to facilitate early diagnosis of lung cancer (LC). Method: Exhaled breath samples of 352 subjects were analyzed to detect C6∼C30 VOCs through thermal desorption coupled with gas chromatography-mass spectrometry (TD-GC-MS). Data were divided into three categories Lung Cancer (LC), Benign Pulmonary Disease (BPD) and Healthy control (HC). Statistical classification models were used to analyze VOCs to distinguish between HC, BPD and LC patient's early and advanced stages. Receiver operator characteristics (ROC) curves were used to select different VOCs to evaluate the accuracy, specificity, and sensitivity of the diagnostic model. Results: The analysis resulted that 10,9,11 and 10 VOCs have a significant correlation to discriminate Histology, stage, BPD from LC and HC from LC. The diagnostic model predicted accuracy, sensitivity, and specificity of NSCLC vs. SCLC is 92%, 96% and 90% with area under curve AUC>0.913. As of ROC curve with AUC>0.819 has distinguished Stage I-II vs. stage III-IV with accuracy, sensitivity, and specificity 72%, 84%, and 80%. Whereas the ROC curve with AUC>0.795 has distinguished BPD from LC with accuracy, sensitivity, and specificity 83%, 89%, and 84%. While ROC curve with AUC>0.945, accuracy, sensitivity and specificity 92%, 96%, and 90% have distinguished HC from LC. Conclusion: This study confirms that TD GC-MS is a sensitive and dependable method to distinguish the VOCs in exhaled breath. We have witnessed significant differences in breath of BPD, HC and LC subjects. Where 10,8,11 and 10 VOCs have distinguished between early from the advance stage, NSCLC from SCLC, BPD from LC and HC from LC. These VOCs has the potential to help improve LC diagnosis and prognosis. However, with the expansion of multi centers to other regions could result in more vital findings and will help improve breath analysis and the diagnosis standard. Screening and Bioinformatics Analysis of Lung Cancer Exhale Breath Biomarkers

Abstract:The exhale breath detection combined bioinformatics analysis method, including transcriptome, metabolic pathway and protein structure, was proposed to identify gas markers for screening and diagnosis of lung cancer. Lung cancer patients and healthy controls' samples were collected to performe GC-MS and ROC curve analysis which obtained ten specific VOCs. Differentially expressed genes were obtained by transcriptome analysis. The differentially expressed genes and relative metabolic pathways were consistent with in vivo biological process, which meant that these VOCs come from the metabolism of lung cancer patient. The sensitivity, specificity and overall accuracy of lung cancer diagnosis model established based on VOCs were 86.2%, 91.2% and 89.6%, respectively. Thus, the proposed method can distinguish normal people and lung cancer patients simply and effectively, providing convenient approach for early screening of lung cancer.

Abstract:Lung cancer is a prevalent and life-threatening disease worldwide. The primary diagnostic approach for lung cancer is the utilization of low-dose spiral CT scans. However, repeated scans can expose patients to harmful radiation. Consequently, there is growing interest in exploring alternative methods such as the analysis of exhaled volatile organic compounds (VOCs) for lung cancer detection. In this study, we employed a gas chromatography-mass spectrometry analyzer to identify and quantify a total of 108 VOCs of lung cancer patients. Our objective is to investigate the correlation between VOCs in exhaled breath and lung cancer. Through the application of orthogonal partial least squares-discriminant analysis and correlation analysis, we identified several VOCs, including acetone, ethanol, isopropanol, and ethyl acetate, which exhibited a strong association with lung cancer. Unlike the use of a single marker, our study employed a multi-parameter regression method, resulting in superior accuracy. A diagnostic model based on the neural network algorithm was established, demonstrating an accuracy of 93.5% after screening, surpassing the accuracy before screening at 81%. Furthermore, we optimize the model by incorporating the gender factor, leading to an accuracy exceeding 96%. Numerous studies have demonstrated that the analysis of VOCs in exhaled breath holds significant potential for effectively distinguishing lung cancer patients from healthy individuals. These findings emphasize the potential of respiratory analysis as a novel diagnostic tool for early detection of lung cancer.

Abstract:An electronic nose combining surface acoustic wave(SAW) sensor array and image recognition was developed for the noninvasive diagnosis of lung cancer.Respiratory VOCs(volatile organic compounds)of lung cancer patients,healthy people(control group) and chronic bronchitis asthma patients were detected,and 11 kinds of VOCs were viewed as biomarkers of lung cancer patients.To find the correlation between the special VOCs in metabolic product of lung cancer cells and VOCs in the breath of lung cancer patients,and to certify the biomarkers in lung cancer patients' breath,and eventually to provide basis for electronic nose for non-invasive detection and diagnosis of lung cancer,an innovative method for lung cancer pathology analysis and early diagnosis in cell level was introduced.

Abstract:A new method for lung cancer pathology analysis and early diagnosis in cell level was presented to find the special volatile organic compounds (VOCs) in the metabolic product of lung cancer cells. A gas detection system using solid phase micro-extraction combined with gas chromatography (SPME-GC) was designed to detect the VOCs existing in the culture medium of several target cells. Lung tissues and their side tissues from 13 lung cancer patients were cultured and observed by electron microscopy, besides, different kinds of cells including bronchial epithelial cells, taste bud cells, osteogenic cells and lipocytes from rats were cultured as control group. The results show that there are special VOCs in the culture medium of lung cancer cells, which are the metabolic product of lung cancer cells. The special VOCs can be viewed as biomarkers of lung cancer, so provide basis for electronic nose for non-invasive detection and diagnosis of lung cancer.

Abstract:OBJECTIVE:to study the characteristics of volatile organic compounds (VOCs) in exhaled breath of lung cancer patients, and therefore to explore its use in the diagnosis of the disease. METHODS:from February 2007 to September 2009, 55 patients with lung cancer, 21 patients with benign lung diseases and 30 healthy controls were enrolled in our study. The VOCs in exhaled breath were detected by the SPME (solid phase microextraction)-GC (gas chromatography) system. RESULTS:heptanal was detected in 45 patients of the lung cancer group, 1 of the benign lung disease group, and 1 of the healthy control group. The positive rate was higher in the lung cancer group as compared to the benign lung disease group and the healthy group combined (P < 0.05). The detection of heptanal was not related to age, smoking, histological type and staging (P > 0.05). CONCLUSION:heptanal maybe a useful marker in VOCs from patients with lung cancer.

Abstract:The method of breath detection for lung cancer has gained growing interests in recent years.It is simple,fast,non-invasive,no need of marks and non-contact.Based on the lung cancer biomarkers obtained from the volatile organic compounds in breath samples of lung cancer patients and controls,we analyzed 27 breath samples of lung cancer patients and 27 breath samples of controls using MOS-SAW sensor coupling electronic nose technique.After pre-processing and analysis of the data,multiple methods,including PCA,LDA,PLS and ANN,were employed to establish the diagnosis models.And the discrimination results of these models were compared.It was showed that the ANN method had the best performance with the sensitivity of 92.59% and the specificity of 88.89%.So this method can be considered as a candidate one for the diagnosis of lung cancer through breath detection.

Abstract:(1) Background: Lung cancer is silent in its early stages and fatal in its advanced stages. The current examinations for lung cancer are usually based on imaging. Conventional chest X-rays lack accuracy, and chest computed tomography (CT) is associated with radiation exposure and cost, limiting screening effectiveness. Breathomics, a noninvasive strategy, has recently been studied extensively. Volatile organic compounds (VOCs) derived from human breath can reflect metabolic changes caused by diseases and possibly serve as biomarkers of lung cancer. (2) Methods: The selected ion flow tube mass spectrometry (SIFT-MS) technique was used to quantitatively analyze 116 VOCs in breath samples from 148 patients with histologically confirmed lung cancers and 168 healthy volunteers. We used eXtreme Gradient Boosting (XGBoost), a machine learning method, to build a model for predicting lung cancer occurrence based on quantitative VOC measurements. (3) Results: The proposed prediction model achieved better performance than other previous approaches, with an accuracy, sensitivity, specificity, and area under the curve (AUC) of 0.89, 0.82, 0.94, and 0.95, respectively. When we further adjusted the confounding effect of environmental VOCs on the relationship between participants' exhaled VOCs and lung cancer occurrence, our model was improved to reach 0.92 accuracy, 0.96 sensitivity, 0.88 specificity, and 0.98 AUC. (4) Conclusion: A quantitative VOCs databank integrated with the application of an XGBoost classifier provides a persuasive platform for lung cancer prediction.
The analysis of volatile organic compounds biomarkers for lung cancer in exhaled breath, tissues and cell lines.

Abstract:Background: Volatile organic compounds (VOCs) biomarkers in breath provide a novel, noninvasive and quick approach to diagnosis lung cancer. The aim of the proposed study was to investigate the VOCs biomarkers in exhaled breath for lung cancer. Method: The VOCs in exhaled breath of 88 lung cancer patients, 70 lung benign disease and 85 healthy people were analyzed by Solid Phase Micro Extraction - Gas Chromatography Mass Spectrometry (SPME-GCMS). Three types of lung cancer cells and 18 lung cancer patients' tissues were cultured in vitro. The VOCs in the headspace of these cultivations were analyzed as an evidence of production mechanism of the VOCs in breath. Three lung cancer diagnosis models were constructed respectively in exhaled breath samples using Linear Discriminant Analysis (LDA). Leave one out cross validation was employed to evaluate these models. Results: 23 VOCs, whose areas under curve (AUC) of receiver operating characteristic (ROC) > 0.60 and p < 0.01, were confirmed as the VOCs biomarkers for lung cancer. Three diagnostic models based on 23 VOCs could easily discriminate lung cancer patients from controls with 96.47% sensitivity and 97.47% specificity. However, the discrimination between early stage and later stage lung cancer was not very obvious.
Breath carbonyl compounds as biomarkers of lung cancer

Abstract:Objective: Lung cancer dysregulations impart oxidative stress which results in important metabolic products in the form of volatile organic compounds (VOCs) in exhaled breath. The objective of this work is to use statistical classification models to determine specific carbonyl VOCs in exhaled breath as biomarkers for detection of lung cancer. Materials and methods: Exhaled breath samples from 85 patients with untreated lung cancer, 34 patients with benign pulmonary nodules and 85 healthy controls were collected. Carbonyl compounds in exhaled breath were captured by silicon microreactors and analyzed by Fourier transform ion cyclotron resonance mass spectrometry (FT-ICR-MS). The concentrations of carbonyl compounds were analyzed using a variety of statistical classification models to determine which compounds best differentiated between the patient sub-populations. Predictive accuracy of each of the models was assessed on a separate test data set. Results: Six carbonyl compounds (C(4)H(8)O, C(5)H(10)O, C(2)H(4)O(2), C(4)H(8)O(2), C(6)H(10)O(2), C(9)H(16)O(2)) had significantly elevated concentrations in lung cancer patients vs. Controls: A model based on counting the number of elevated compounds out of these six achieved an overall classification accuracy on the test data of 97% (95% CI 92%-100%), 95% (95% CI 88%-100%), and 89% (95% CI 79%-99%) for classifying lung cancer patients vs. non-smokers, current smokers, and patients with benign nodules, respectively. These results were comparable to benchmarking based on established statistical and machine-learning methods. The sensitivity in each case was 96% or higher, with specificity ranging from 64% for benign nodule patients to 86% for smokers and 100% for non-smokers. Conclusion: A model based on elevated levels of the six carbonyl VOCs effectively discriminates lung cancer patients from healthy controls as well as patients with benign pulmonary nodules.

Abstract:In patients with lung cancer, volatile organic compounds (VOCs) are excreted in exhaled breath. In this article, exhaled breath of 30 lung cancer paitients and 30 healthy people were collected, preconcentrated by solid-microextraction(SPME) and analyzed with gas chromatography and mass spectrometry (GC/MS). A predictive model composed of 5 VOCs out of 16 candidate VOCs detected in the lung cancer patients is constructed by discriminant analysis, with a sensitivity of 76.7% and specificity of 96.7%. We detected exhaled VOCs of 3 different lung cancer cell lines and human bronchial epithelial cell lines. 2-Tridicanone is considered the distinctive marker of lung cancer cells, which is found in lung cancer patients' exhaled breath as well. Compared to healthy people, patients with lung cancer had distinctive VOCs in their exhaled breath. The predictive model can work as diagnosis reference for lung cancer. VOCs found in lung cancer cell line help the cognition of the mechasim VOCs generating in lung cancer patients.

Abstract:The aim of the present study was to investigate the ability of breath analysis to distinguish lung cancer (LC) patients from patients with other respiratory diseases and healthy people. The population sample consisted of 51 patients with confirmed LC, 38 patients with pathological computed tomography (CT) findings not diagnosed with LC, and 53 healthy controls. The concentrations of 19 volatile organic compounds (VOCs) were quantified in the exhaled breath of study participants by solid phase microextraction (SPME) of the VOCs and subsequent gas chromatography-mass spectrometry (GC-MS) analysis. Kruskal–Wallis and Mann–Whitney tests were used to identify significant differences between subgroups. Machine learning methods were used to determine the discriminant power of the method. Several compounds were found to differ significantly between LC patients and healthy controls. Strong associations were identified for 2-propanol, 1-propanol, toluene, ethylbenzene, and styrene (p-values < 0.001–0.006). These associations remained significant when ambient air concentrations were subtracted from breath concentrations. VOC levels were found to be affected by ambient air concentrations and a few by smoking status. The random forest machine learning algorithm achieved a correct classification of patients of 88.5% (area under the curve—AUC 0.94). However, none of the methods used achieved adequate discrimination between LC patients and patients with abnormal computed tomography (CT) findings. Biomarker sets, consisting mainly of the exogenous monoaromatic compounds and 1- and 2- propanol, adequately discriminated LC patients from healthy controls. The breath concentrations of these compounds may reflect the alterations in patient’s physiological and biochemical status and perhaps can be used as probes for the investigation of these statuses or normalization of patient-related factors in breath analysis.

Traditional cancer detection techniques such as biopsies, imaging, and blood tests, though effective, often involve invasive procedures or are costly, time consuming, and painful. Recent advancements in technology have led to the exploration of VOC detection as a promising non-invasive and comfortable alternative. VOCs are organic chemicals that have a high vapor pressure at room temperature, making them readily detectable in breath, urine, and skin. The present study leverages artificial intelligence (AI) and machine learning algorithms to enhance classification accuracy and efficiency in detecting lung cancer through VOC analysis collected from exhaled breath air. Unlike other studies that primarily focus on identifying specific compounds, this study takes an agnostic approach, maximizing detection efficiency over the identification of specific compounds focusing on the overall compositional profiles and their differences across groups of patients. The results reported hereby uphold the potential of AI-driven techniques in revolutionizing early cancer detection methodologies towards their implementation in a clinical setting.

Breath Analysis for Lung Cancer Early Detection-A Clinical Study

Abstract:This clinical study presents a comprehensive investigation into the utility of breath analysis as a non-invasive method for the early detection of lung cancer. The study enrolled 14 lung cancer patients, 14 non-lung cancer controls with diverse medical conditions, and 3 tuberculosis (TB) patients for biomarker discovery. Matching criteria including age, gender, smoking history, and comorbidities were strictly followed to ensure reliable comparisons. A systematic breath sampling protocol utilizing a BIO-VOC sampler was employed, followed by VOC analysis using Thermal Desorption-Gas Chromatography-Mass Spectrometry (TD-GC/MS). The resulting VOC profiles were subjected to stringent statistical analysis, including Orthogonal Projections to Latent Structures-Discriminant Analysis (OPLS-DA), Kruskal-Wallis test, and Receiver Operating Characteristic (ROC) analysis. Notably, 13 VOCs exhibited statistically significant differences between lung cancer patients and controls. The combination of eight VOCs (hexanal, heptanal, octanal, benzaldehyde, undecane, phenylacetaldehyde, decanal, and benzoic acid) demonstrated substantial discriminatory power with an area under the curve (AUC) of 0.85, a sensitivity of 82%, and a specificity of 76% in the discovery set. Validation in an independent cohort yielded an AUC of 0.78, a sensitivity of 78%, and a specificity of 64%. Further analysis revealed that elevated aldehyde levels in lung cancer patients' breath could be attributed to overactivated Alcohol Dehydrogenase (ADH) pathways in cancerous tissues. Addressing methodological challenges, this study employed a matching of physiological and pathological confounders, controlled room air samples, and standardized breath sampling techniques. Despite the limitations, this study's findings emphasize the potential of breath analysis as a diagnostic tool for lung cancer and suggest its utility in differentiating tuberculosis from lung cancer. However, further research and validation are warranted for the translation of these findings into clinical practice.
Volatile Organic Compounds in Exhaled Breath: A Promising Approach for Accurate Differentiation of Lung Adenocarcinoma and Squamous Cell Carcinoma

Abstract:Lung cancer subtyping, particularly differentiating adenocarcinoma (ADC) from squamous cell carcinoma (SCC), is paramount for clinicians to develop effective treatment strategies. In this study, we aimed: (i) to discover VOC biomarkers for precise diagnosis of ADC and SCC, (ii) to investigated the impact of risk factors on ADC and SCC prediction, and (iii) to explore the metabolic pathways of VOC biomarkers. Exhaled breath samples from patients with ADC (n=149) and SCC (n=94) were analyzed by gas chromatography-mass spectrometry (GC-MS). Both multivariate and univariate statistical analysis method were employed to identify VOC biomarkers. Support vector machine (SVM) prediction models were developed and validated based on these VOC biomarkers. The impact of risk factors on ADC and SCC prediction was investigated. A panel of 13 VOCs was found to differ significantly between ADC and SCC. Utilizing the SVM algorithm, the VOC biomarkers achieved a specificity of 90.48%, a sensitivity of 83.50%, and an AUC value of 0.958 on the training set. On the validation set, these VOC biomarkers attained a predictive power of 85.71% for sensitivity and 73.08% for specificity, along with an AUC value of 0.875. Clinical risk factors exhibit certain predictive power on ADC and SCC prediction. Integrating these risk factors into the prediction model based on VOC biomarkers can enhance its predictive accuracy. This work indicates that exhaled breath holds the potential to precisely detect ADCs and SCCs. Considering clinical risk factors is essential when differentiating between these two subtypes.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Breath analysis is a promising noninvasive technique that offers a wide range of opportunities to facilitate early diagnosis of lung cancer (LC).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Method</h3><p>Exhaled breath samples of 352 subjects including 160 with lung cancer (LC), 70 with benign pulmonary nodule (BPN) and 122 healthy controls (HC) were analyzed through thermal desorption coupled with gas chromatography-mass spectrometry (TD-GC–MS) to obtain the metabolic information from volatile organic compounds (VOCs). Statistical classification models were used to find diagnostic clusters of VOCs for the discrimination of HC, BPN and LC patients' early and advanced stages, as well as subtypes of LC. Receiver operator characteristics (ROC) curves with 5-fold validations were used to evaluate the accuracy of these models.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The analysis revealed that 20, 19, 19, and 20 VOCs discriminated LC from HC, LC from BPN, histology and LC stages respectively. The calculated diagnostic indices showed a large area under the curve (AUC) to distinguish HC from LC (AUC: 0.987, 95 % confidence interval (CI): 0.976−0.997), BPN from LC (AUC: 0.809, 95 % CI: 0.758−0.860), NSCLC from SCLC (AUC: 0.939, 95 % CI: 0.875−0.995) and Stage III from stage III-IV (AUC: 0.827, 95 % CI: 0.768−0.886). The comparison between the high-risk groups (BPN and HC smokers) and early stages LC resulted in the AUC of 0.756 (95 %CI: 0.681−0.817) for BPN vs. early stage LC and AUC of 0.986 (95 % CI: 0.972−0.994) for HC smoker vs. early stage LC.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Volatome of breath of the LC patients was significantly different from that of both BPN patients and HC and showed an ability of distinguishing early from advance stage LC and NSCLC from SCLC. We conclude that the volatome has a potential to help improve early diagnosis of LC.</p>
