Abstract:Abstract Background Before attending ophthalmology trainee courses in Zhongshan Ophthalmic Centre, the medical students from Sun Yat-sen University had finished two years of premedical education after the six-year medical courses including basic medical courses, clinical medical courses, clerkship, and research training in medical college. Integrated modular teaching using different problem-based teaching methods in ophthalmology was designed by the teaching steering committee of Zhongshan Ophthalmic Centre. This study aimed to evaluate the effectiveness and satisfaction scales of the integrated modular teaching among the trainee students. Methods A total of 100 medical students attending ophthalmology trainee courses in Zhongshan Ophthalmic Centre were enrolled and randomly allocated into 4 groups according to the teaching arrangement. The trainee courses consisted of several sessions delivered in multiple methods, such as “flipped classroom” session and team-based learning session. The pre- and post-class tests were delivered to evaluate the effectiveness of the integrated modular teaching. The satisfaction survey questionnaire was collected from all participants to investigate the degree of satisfaction. Results Compared with the first-day-test score, the total last-day-test score was significantly improved by a paired t- test ( t = 3.288, P = 0.001). Nineteen students obtained a significant improvement in ranking increased by more than 10 in the last-day-test, whereas they failed to obtain a higher average score for daily performance than other students ( t = 0.469, P = 0.654). According to the participant satisfaction questionnaires, these innovative teaching methods were considered as effective and satisfactory. Conclusions Integrated modular teaching in ophthalmology trainee courses is effective and appreciated by the medical college students.

Use of a flipped classroom in ophthalmology courses for nursing, dental and medical students: A quasi-experimental study using a mixed-methods approach

Abstract:Background: Traditional teaching is associated with dilemmas, such as low motivation to learn and passive learning. In contrast, use of a flipped classroom with the proper learning design has the potential to promote accelerated learning, bolster transmission of competencies (i.e., critical thinking, communication and problem-solving skills) and improve teamwork capacity, all of which are of great significance in professional healthcare practice, including nursing, dentistry and medicine. Objective: The aim of the study was to examine the effects of using a flipped classroom in ophthalmology courses for undergraduate nursing students, dental students and higher vocational medical students. Design: A quasi-experimental design was used with an intervention (n = 100) and a comparison group (n = 100), pre-/post-testing measures and a mixed-methods approach. Setting: A university in China. Participants: A total of 200 students were included. Methods: Students' self-rated learning ability was measured before and after the courses, and skill exams were performed after the courses. In addition, interviews were conducted with the clinical medical students concerning their experiences of the flipped classroom. Results: Students' self-rated learning ability improved significantly more in the intervention than in the comparison group, for the total scale and the three factors 'learning goals', 'self-efficacy and social learning' and 'problem-solving'. Skill exam scores were statistically significantly better in the intervention than in the comparison group. On the whole, the clinical medicine students felt the flipped classroom had a positive impact and improved their learning ability as well as knowledge in ophthalmology. Conclusions: Use of a flipped classroom for nursing, dental and clinical medical students in ophthalmology courses shows promising results in the form of students' skill exam scores and self-rated learning ability.
Practice and effect analysis of online course for ophthalmology teaching in eight-year program medical students

Abstract:Objective:To investigate the efficiency and potential improvement of online course for ophthalmology in eight-year program medical students.Methods:Totally 27 students enrolled in 2016 (experimental group) and 38 students enrolled in 2015 (control group) who studied ophthalmology course in Peking University Third Hospital in 2019 and 2020 were recruited as the participants.The experimental group adopted the teaching model of online course, the control group adopted the traditional teaching model. At the end of study, all students were tested by theoretical examination designed with the same rule. The general scoring rate and scoring rate in each teaching section, including anatomy of ophthalmology, disease of anterior segment, disease of posterior segment and optometry was calculated. Correlation between actual study time-teaching video ratio and scoring rate in each teaching section was analyzed. A questionnaire about the demands of online course on teaching organization ability, problems in lesson preparation, problems in the process of teaching and potential improvements in teaching ability was applied to the 29 teachers.Results:Compared with the control group, the general scoring rate [ (76.2±8.0)% vs. (70.0±10.7)%, P=0.015], scoring rate in section of disease of anterior segment [ (79.2±14.2)% vs. (71.0±13.9)%, P=0.024], disease of posterior segment [ (92.7±11.8)% vs. (52.0±15.9)%, P<0.01] and optometry [ (88.1±7.2)% vs. (79.2±13.4)%, P=0.001] was superior in the experimental group. However, the scoring rate in section of anatomy [ (37.0±17.6)% vs. (77.7±15.6)%, P<0.01] was inferior in the experimental group. The correlation between actual study time-teaching video ratio and scoring rate in the teaching section of optometry was statistically significant ( r=0.467, P=0.016). The questionnaire indicated that the teaching model of online course addressed higher requirements to teaching organization ability, lesson preparation and classroom interaction. Conclusions:Online course was of practical value in ophthalmology teaching. Attentions should be paid to improvements of strategy in online course of anatomy and elevation of teaching ability.
Evaluation of the Effectiveness of MOOC-based Flipped Learning in College Mental Health Curriculum

Abstract:Background and Aim: The flipped classroom redefines psychological health education by using technology for independent study before class and engaging in interactive and applied learning activities in class. Thus, two research objectives were proposed: (1) To compare the difference in the effectiveness of the new MOOC-based flipped learning on students' learning outcomes compared to traditional teaching in undergraduates’ mental health curriculum. (2) To explore students' attitudes to new MOOC-based flipped teaching methods. Materials and Methods: This study was designed as a pretest and posttest trial. The sample was 60 freshmen from two Sichuan University classes allocated to two groups using purposive samplings: group 1 for traditional learning, and group 2 for flipped learning. The performance test and semi-structured interview were employed in this study as research instruments. Both groups' pretest and posttest scores were obtained for learning outcome comparison. The students' attitudes to the new teaching methods were obtained through semi-structured interviews. Results: The mean pretest and posttest scores in group 1 were 58.87 and 82.17 respectively, while in group 2, they were 58.67 and 86.53 respectively. The mean improvement in knowledge score was significantly higher in group 2 (M=27.87, SD=3.14) compared to group 1 (M=23.30, SD=1.69). The level of statistical significance was P < .001. The results of the semi-structured interviews indicated that almost all participants had very positive opinions regarding flipped learning based on MOOCs. Conclusion: The research demonstrates that MOOC-based flipped learning increases the student learning effectiveness to obtain better score performance and make it worth promoting.
Comparison between flipped classroom and lecture-based classroom in ophthalmology clerkship

Abstract:Background: In recent years, the flipped classroom method of teaching has received much attention in health sciences education. However, the application of flipped classrooms in ophthalmology education has not been well investigated. Objective: The goal of this study was to investigate the effectiveness and acceptability of the flipped classroom approach to teaching ophthalmology at the clerkship level. Design: Ninety-five fourth year medical students in an ophthalmology clerkship were randomly divided into two groups. An ocular trauma module was chosen for the content of this study. One group (FG (flipped group), n = 48) participated in flipped classroom instruction and was asked to watch a recorded lecture video and to read study materials before a face-to-face class meeting. They used the in-class time for discussion. The other group (TG (traditional group), n = 47) was assigned to traditional lecture-based instruction. These students attended a didactic lecture and completed assigned homework after the lecture. Feedback questionnaires were collected to compare students' perspectives on the teaching approach they experienced and to evaluate students' self-perceived competence and interest in ocular trauma. Pre- and post-tests were performed to assess student learning of the course materials. Results: More students in the FG agreed that the classroom helped to promote their learning motivation, improve their understanding of the course materials, and enhance their communication skill and clinical thinking. However, students in the FG did not show a preference for this method of teaching, and also reported more burden and pressure than those from the TG. Students from the FG performed better on the post test over the ocular trauma-related questions when compared to those from the TG. Conclusions: The flipped classroom approach shows promise in ophthalmology clerkship teaching. However, it has some drawbacks. Further evaluation and modifications are required before it can be widely accepted and implemented. Flipped Classroom Case Learning vs Traditional Lecture-Based Learning in Medical School Ophthalmology Education: A Randomized Trial

Abstract:Purpose: To address the problem of teaching noncore specialties, for which there is often limited teaching time and low student engagement, a flipped classroom case learning (FCCL) module was designed and implemented in a compulsory 5-day ophthalmology rotation for undergraduate medical students. The module consisted of a flipped classroom, online gamified clinical cases, and case-based learning. Method: Final-year medical students in a 5-day ophthalmology rotation were randomized to the FCCL or a traditional lecture-based (TLB) module. The outcomes of subjective assessments (student-rated anonymous Likert scale questionnaire, scale 1 to 5, and course and teaching evaluation, scale 1 to 6) and objective assessments (end-of-rotation and post-MBChB multiple-choice questions, scale 0 to 60) were compared between the 2 groups. Results: Between May 2021 and June 2022, 216 students (108 in each group) completed the study. Compared with the TLB students, the students in the FCCL group rated various aspects of the course statistically significantly higher, including feeling more enthusiastic and engaged by the course and more encouraged to ask questions and participate in discussions (all P < .001). They also gave higher ratings for the instructional methods, course assignments, course outcomes, and course workload ( P < .001). They gave higher course and teaching evaluation scores to the tutors (5.7 ± 0.6 vs 5.0 ± 1.0, P < .001). The FCCL group scored higher than the TLB group on the end-of-rotation multiple-choice questions (53.6 ± 3.1 vs 51.8 ± 2.8, P < .001). When 32 FCCL students and 36 TLB students were reassessed approximately 20 weeks after the rotation, the FCCL group scored higher (40.3 ± 9.1) than the TLB group (34.3 ± 10.9, P = .018). Conclusions: Applying the FCCL module in ophthalmology teaching enhanced medical students' satisfaction, examination performance, and knowledge retention. A similar model may be suitable for other specialties.

Abstract:Purpose: This study aimed to measure student satisfaction with a revised ophthalmology delivery format, which due to the pandemic had previously relied on a remote online flipped classroom (OFC) format compared to a blended learning format. This educational strategy combined online learning with in-person seminars and practical patient centred sessions. Our previous investigations demonstrated a significant lack of student satisfaction with a curriculum solely reliant on a remote OFC, as such we hypothesised that a blended learning approach would result in improved levels of student satisfaction and knowledge gain. Methods: Non-randomised intervention study of two groups; group 1 = OFC group and group 2 = BL group, compared perspectives of 4th year ophthalmology students using a validated course evaluation questionnaire (CEQ). Results: A total of 59 students from the BL group (n = 257; response rate = 23.0%) and 28 from the OFC group agreed to participate in the study (n = 114; response rate = 24.6%). Participants in the BL group felt it was easier to determine the standard of work that was expected (77.42% v 60.71%) and demonstrated significantly increased satisfaction with staff motivation of students (95.16% v 64.29%, p <0.001) and provision of feedback (74.19% v 46.43%, p = 0.004), compared to the OFC group. Furthermore, students in the BL group also felt the course significantly improved their analytical skills (64.52% v 42.85%, p = 0.023) and ability to work as part of team (69.36% v 25%, p <0.001) as well as reporting reduced dissatisfaction with the level of choice afforded in terms of how they would learn (33.88% v 60.71%, p = 0.31) and the how they were assessed (59.68% v 89.28%, p = 0.004). No evidence of a statistical difference in exam score was observed. Conclusions: The COVID-19 pandemic necessitated an unavoidable pivot to online and distance learning, to meet the challenges presented by government mandates and social distancing requirements. Since many of these directives have been reversed, it is important to evaluate the effectiveness and learner perceptions' of the online and distance learning interventions. In this study we demonstrated a significant student preference for BL compared to the OFC approach, with comparable student performances determined by MCQ examinations. Our findings suggest a preference for reintroducing in-person and patient engagement activities in post-pandemic health professions education.

Abstract:Purpose This study aimed to measure stakeholder satisfaction with our usual delivery format, which previously relied on a blend of didactic lectures and clinical skills sessions compared to a revised format, which had more emphasis on online learning. We hypothesised that the online flipped classroom (OFC) would facilitate delivery of content in the wake of the pandemic, and result in improved levels of student satisfaction and knowledge gain. Design Non randomised intervention study. Group 1 = Traditional delivery (TD) and Group 2 = OFC group. Methods A validated course evaluation questionnaire (CEQ) compared perspectives of teaching faculty (n = 5) and students with the traditional delivery (TD) of the 4 th year ophthalmology clinical attachment and an OFC approach (TD n = 129 v OFC n = 114). Results The OFC group (n = 114; response rate = 24.6%) reported significantly reduced satisfaction with staff motivation of students and provision of feedback, compared to TD (n = 129; response rate = 17.8%). OFC students also felt it was harder to determine what standard of work was expected and found the course less beneficial at helping develop problem-solving skills. Students were dissatisfied with the level of choice afforded by the OFC, specifically how they would learn and assessment options. No significant difference in exam score was observed between the TD and OFC groups. For faculty (n = 5), there was no evidence of a difference between OFC and TD. Conclusions Students indicated a preference for the TD compared to the OFC approach. However, both delivery approaches led to comparable student performances as determined by MCQ examination.

Abstract:Background The flipped classroom blended learning model has been proven effective in the teaching of undergraduate medical courses as shown by student acceptance and results. Since COVID-19 necessitated the application of online learning in Histology practical for MBBS students, the effectiveness of the blended learning model on teaching quality has required additional attention. Methods A blended learning of histology practical was flipped in a virtual classroom (FVCR-BL) or in a physical classroom (FPCR-BL) in School of Medicine, Zhejiang University in China. Students were split into FVCR-BL group (n = 146) due to COVID-19 pandemic in 2020 or were randomly allocated into FPCR-BL group (n = 93) in 2021, and retrospectively, students with traditional learning in 2019 were allocated into traditional learning model in a physical classroom (PCR-TL) group (n = 89). Same learning requirements were given for 3 groups; all informative and summative scores of students were collected; a questionnaire of student satisfaction for blended learning activities were surveyed in 2021. Data of scores and scales were analyzed with Kruskal-Wallis test and Kolmogorov-Smirnov test in SPSS Statics software. Results The results clarified that FPCR-BL students obtained higher final exam scores and were more likely to engage in face-to-face interactions with instructors than FVCR-BL students. FPCR-BL and FVCR-BL students had higher classroom quiz scores than the PCR-TL students owing to the contribution of blended learning. The results of the questionnaire showed that participants of FPCR-BL positively rated the online learning and preview test, with a cumulative percentage of 68.31%, were more satisfying than other learning activities of blended learning. There were significant correlations (r = 0.581, P < 0.05) between online learning and the other three blended learning strategies. Conclusions In the flipped classroom with a blended learning process of histology practical, enhancing the quality of online learning boosts student satisfaction and improves knowledge learning; peer-to-peer interactions and instructor-to-peer interactions in the physical classroom improved knowledge construction.
Integration of a Massive Open Online Course on Clinical Kidney, Pancreas and Islet Transplantation into a Regular Medical Science Curriculum

Abstract:Introduction In January 2016 Leiden University Medical Center (LUMC) launched world’s first clinical oriented Massive Open Online Course (MOOC) on Kidney, Pancreas and Islet Transplantation. In this course several digital learning opportunities are offered to students, including reading materials, interactive patient cases, lectures, 3D animations of transplantation, discussions, serious games, and interviews with experts and patients. The electronic activities help learners to increase their knowledge, practice with simulated patient cases and develop several skills including clinical reasoning. The course is structured around four sections, covering before, during and short and long term after transplantation. The target audience for this online course is (bio)medical students and healthcare professionals who work in the (pre)clinical transplantation field. Materials and Methods Integration of the MOOC into the local curriculum took place in two regular courses in the second year of the Leiden medical school curriculum. For the first course, several movies and online assignments from the MOOC were used to replace traditional teaching; in the second course several movies, the discussion forum and a clinical patient case assignment have been offered. Students were asked to fill out a questionnaire using a 5-point Likert scale. Besides the two regular courses, the complete MOOC has been offered to students in the LUMC Honors track and the 2016 and 2017 international Leiden Oxford Transplantation Summerschool (LOTS). Results and Discussion Between January 2016 and November 2017 over 9500 learners of all ages signed up for the MOOC, of which over 6000 turned out to be active learners. Participants originated from over 90 countries. 62% of the participants were male and 78% of the participants had a bachelor’s degree or higher. For the courses in the Leiden curriculum, 325 students signed up for the MOOC to use materials during both courses. 56 students (17%) responded to the online questionnaire. Respondents indicated that the MOOC elements were an interesting addition to the face to face curriculum (score 3.3 ±0.9) and that the online lectures and discussion forums were inspiring (3.1 ±1). The students clearly did not explore other parts of the MOOC other than those that were mentioned in the assignments (2.5 ±1.4). For the Honors track and LOTS, 20 students signed up for the MOOC. Their engagement with the online materials and participation in discussion forums seems to be much higher compared to the medical school students. Conclusion Students indicate they like the online materials from the MOOC as an inspiring addition to traditional course materials. However, students in the medical school curriculum hardly explore other parts of the MOOC, while Honors track and LOTS students do that in great extent. Further research is needed to see how students can be more encouraged to gain as much as possible from the content rich resources in the MOOC.

Abstract:Objective To investigate the efficiency and existing problems of case-based learning approach for ophthalmology course in eight-year program medical students. Methods Case-based learning was used in 46 students of grade 2012 who studied ophthalmology course in Peking University Third Hospital in 2016. Later a questionnaire with 5-score questions was applied to evaluate the efficiency of case-based learning and to detect the existing problems. Results Forty-six students responded to the investigation, with a response rate of 100.0％. The general effectiveness and facilitation in clinical thoughts training was affirmed, with scores of (4.2±0.8),(4.1±0.9),(4.3±0.8),(4.2 ± 0.9) and (4.0 ± 1.1) in the items of overall satisfaction, acquisition of theoretical knowledge, ability to extract case information, ability to analyze clinical problems and diagnosis of typical cases, respectively. However, the activity of students and self-learning ability training was not satisfying, with scores of (3.9±1.1),(3.7±1.0),(3.6±1.0),(3.3 ± 1.2) and (3.7 ± 1.2) in the items of motivation in presentation, participation in class discussion, active study, ability in literature search and team learning efficiency,respectively. Conclusions Case-based learning was of practical value in clinical thoughts training in the ophthalmology course. Instructors should pay more attention to motivation of learning initiatives in students and the training of self-learning ability.

Abstract:Purpose: This study based on direct ophthalmoscopy (DO) teaching in medical curricula aimed to integrate both classic and simulator‐based DO techniques into the teaching of medical students. Methods: 110 medical students received a multiple‐choice questionnaire (four theory questions, six fundus photographs) prior to simulator‐based (EyeSi® Direct, Haag‐Streit Simulation, Germany) and classic DO training (Heine BETA®200, Heine Optotechnik GmbH, Germany). Simulator‐based DO training was performed in groups of up to eight students. Thereafter, they underwent an objective standardized clinical examination containing questions about (1) pupil dilation, (2) mydriasis contraindication, (3) ophthalmoscope adjustment and a practical test checking whether (1) optic nerve head, (2) macula, (3–6) upper/lower vessel arcade nasally versus temporally were identified correctly. Subsequently, classic DO was performed mutually and they reported the funduscopic structures they had recognized. Finally, the questionnaire was repeated 1 week later in combination with an evaluation of simulator‐based versus classic DO. Results: Students improved from an average of 4.0 ± 1.5/10 before to 8.8 ± 1.5/10 correctly answered questions after training ( p < 0.0001, Wilcoxon matched‐pairs test). 87|34|61 students named pupil dilating time mydriasis contraindication and adjusted the ophthalmoscope correctly. They successfully identified the optic nerve head ( n (simulator)| n (classic) DO: 110|68, p < 0.0001, Fisher's exact test), the macula (93|22, p < 0.0001), the upper (98|75, p = 0.0002) and lower temporal vessel arcade (101|91, p = 0.0672), the upper (84|56, p = 0.0001) and lower nasal vessel arcade (87|48, p < 0.0001). They wished for more simulator‐based training, indicated a higher interest in ophthalmology after training and stated an improvement of their ophthalmoscopy skills. Conclusion: A standardized simulator‐based approach improves medical students' skills in ophthalmoscopy and arouses professional interest in ophthalmology.

Abstract:Blended learning is a learning approach that combines face-to-face classroom lectures and e-learning. It has grown rapidly to be commonly used in medical institutions, especially in the local medical universities where there is lack of qualified teachers and instructional materials. Massive open online courses (MOOCs) are the latest revolution in e-learning and provides learners with access to quality educational resources. Nevertheless, there is seldom reports concerning how to effectively integrate MOOCs into blended learning in local universities, as well as the evaluation of knowledge outcomes. In order to achieve this aim, a blended learning approach was carried out in teaching pathophysiology in Guilin Medical University. This blended learning model was based on combination of Chinese University MOOC with case based learning (CBL), as an alternative to conventional learning. The medical students in the 2017 and 2018 classes received the blended learning method, while the medical students in the 2015 class received the traditional classroom instruction. The results showed that students in the 2017 and 2018 performed significantly better than students in the 2015 class at mid-term exam and the final exam. Perception surveys also revealed that both students and teachers had positive attitude toward blended learning, and they shared similar viewpoints of blended learning. A large proportion of students and teachers believed that the blended learning enhanced students' motivation to learn independently, improved their time management skills, and allowed them to experience personalized learning. Also, most students and teachers recognized that Chinese University MOOC provided substantial educational resources suitable for their need. In addition, teachers indicated that the blended learning improved student learning quality, facilitated interaction between teachers and students, and helped them to establish a student-centered model in teaching pathophysiology. Overall, the blended learning method that combines Chinese University MOOC with CBL is effective in enhancing students' achievement and motivation in pathophysiology than the traditional learning method, and helps to strengthen the cultivation of talent in local medical universities.

Abstract:Background & Objective: With the affluence of use and better accessibility, Massive Open Online Courses (MOOCs) have become essential in imparting education in the ongoing COVID scenario. The study aims to compare the effect of MOOCs versus face-to-face classrooms on medical students’ learning outcomes in anatomy exams at the end of the first year of a medical course. Materials & Methods: The cross-sectional study included 300 first-year Bachelor of Medicine and Bachelor of Surgery (MBBS) students in two batches (150 each). The first group underwent a one-year MOOCs group, and the second group experienced a one-year face-to-face classroom. The training program and study materials for the anatomy curriculum were kept the same for both groups by the standard National Medical Council curriculum of India. The assessment was standardized and based on summative University examinations. Results: The mean score in the university theoretical exam was significantly higher in the MOOCs group as compared to the classroom learning group (66.68 ± 6.61 vs 63.52 ± 7.97, P-value: 0.001), whereas the mean scores in the practical examination were significantly higher in classroom learning group as compared to MOOCs group (78.71 ± 6.28 vs. 74.19 ± 7.45, P-value: 0.001). The MOOCs group shows better learning in the cognitive domain. Conclusion: The classroom learning groups performed better in the practical examination, which assesses the psychomotor and affective domain of learning. Hence, a blended T/L method involving both classroom and online methods is recommended as India’s preferred approach to imparting medical education.
Motivational simulated teaching of clinical skills using formative assessment methods for medical undergraduate students: between-group evaluation of a simulated course in a Chinese medical college

Abstract:Objectives Teaching clinical skills is an important component of educational programmes for medical undergraduates. However, the extension of the interval between the completion of the course and qualification examination affects the performance of students in the skill examination. This study established a multisource evaluation system to determine whether formative assessment can enhance the instruction of clinical skills. Methods Formative assessment was introduced to the entire training course on clinical skills, in which diversified methods were used to observe the performance of students during training. Students in the experimental group received training for clinical skills using formative assessment (class of 2019, n=128), while students in the control group received traditional training without formative assessment (class of 2018, n=123). Both groups participated in the Objective Structured Clinical Examination (OSCE) conducted by Tongji Medical College, and the exam scores were taken as the objective measure of course outcome. After completing the course, all students in the experimental group were instructed to fill in a questionnaire to evaluate their experience in the training programme, as a subjective measure of course outcome. Results Compared with the control group, students in the experimental group received significantly better practical scores in the four clinical skills tested by the OSCE. The questionnaire results revealed that the majority of students who were trained using formative assessment methods considered the course helpful for learning, and appreciated the course for the clinical skills they had gained, and the opportunity to receive and give feedback to the instructors. Conclusions The findings of this study suggest that formative assessment methods are beneficial for learning clinical skills through simulated teaching, as shown by the improved objective clinical skills evaluated by the structured clinical examination, and the self-reported satisfaction with the learning process.

Abstract:Background The recent application of blended educational methods has impacted medical education and has drawn attention to a new teaching method. This teaching style presents unique opportunities and challenges. We investigated the effects of blended learning and traditional teaching methods on clinical skill development. Methods We sorted 200 medical students from Tongji Medical College at Huazhong University of Science and Technology into a control or experimental group. The control group was taught with a traditional lecture-based learning method and the experimental group was taught using a blended learning method. The two groups were compared after training to assess their theoretical and practical differences. A student satisfaction survey was given to participants in both groups. Results The results of the experimental group’s theoretical and practical assessments were found to be significantly higher ( p < 0.05) than that of the control group. The student satisfaction survey showed that blended learning was significantly more effective for acquiring relevant knowledge, enhancing student-centered learning and improving clinical practice. Conclusions Blended learning may address deficiencies in clinical skills, make up for limited time and space, and ensure learning efficiency and quality.

Eyesi direct ophthalmoscope simulator: an effective training tool for medical undergraduates

Abstract:Introduction: Non-ophthalmologists often lack sufficient operational training to use a direct ophthalmoscope proficiently, resulting in a global deficit of basic ophthalmological skills among general practitioners. This deficiency hampers the timely diagnosis, referral, and intervention of patients. Consequently, the optimization of teaching tools and methods to enhance teaching efficiency is imperative. This study explores the effectiveness of the Eyesi Direct Ophthalmoscope Simulator (Eyesi) as an innovative tool for fundus examination training. Methods: Medical undergraduates were randomly assigned to Group A or B (n = 168). All participants completed a pre-training questionnaire. Group A received Eyesi training, while Group B underwent traditional direct ophthalmoscope (TDO) training. Subsequently, participants answered questionnaires relevant to their respective training methods. Both groups exchanged training tools and completed a summary questionnaire. Results: After training, 54.17% of participants believed that images presented by the Eyesi were consistent with the real fundus. Group A scored significantly higher than Group B in fundus structure recognition and self-confidence in examination. The degree of mastery over fundus theory score increased from 6.10 ± 0.13 to 7.74 ± 0.16 (P < 0.001) in Group A, but Group B did not demonstrate a significant difference. We also compared undergraduates' tendencies for different learning purposes, 75.59% of participants preferred the Eyesi to TDO as a training tool, and 88.41% of participants were receptive to introducing the Eyesi in training. Conclusion: According to subjective participant feedback, Eyesi outperformed TDO in fundus observation, operational practice, and theoretical learning. It effectively equips undergraduates with fundus examination skills, potentially promoting the use of direct ophthalmoscopes in primary medical institutions.

Abstract:Background: To investigate the effect of the bridge-in, objective, preassessment, participatory learning, post assessment, and summary (BOPPPS) model combined with case-based learning (CBL) on ophthalmology teaching for five-year paediatric undergraduates. Methods: The effects of the BOPPPS model combined with CBL (BOPPPS-CBL) and traditional lecture-based learning (LBL) on ophthalmology teaching were compared among students in a five-year programme. The questionnaire surveys of the students were collected and statistically analysed after the class. The final examination scores, including on elementary knowledge and case analysis, in the two groups were analysed. Results: There were no statistically significant differences between the teachers and students in the baseline data. More students agreed that the BOPPPS-CBL model helped develop their problem-solving skills, analytical skills and motivation for learning better than the LBL model. There was no significant difference in learning pressure between the two groups. The final examination scores of the BOPPPS-CBL group were significantly higher than those of the LBL group. The overall course satisfaction of the BOPPPS-CBL group was obviously higher than that of the LBL group. Conclusions: The BOPPPS-CBL model is an effective ophthalmology teaching method for five-year paediatric undergraduates.
[Predisposing factors, clinical picture and mortality in volvulus of the small intestine].

Abstract:This retrospective study evaluated predisposing factors, clinical picture and the methods of treatment related to morbidity and mortality of 19 small bowel volvulus (SBV) who underwent operation at Belen Hospital (Trujillo-Peru) during the last 26 years (1966-1992). The SBV was 1.6% of all cases of intestinal obstruction in this period and 10.8% of all intestinal volvulus. The median age was of 43 +/- 20.5 years (range, 6 to 78 years) and the majority of them were between 41 and 60 years. Sixteen cases (84.2%) were men from Indian and Spanish extraction and most of them were farmers and came from the Sierra of the Department of La Libertad. Two cases (10.5%) had non-related antecedents previous surgery. In six patients (31.6%) the volvulus was less than seven day's duration and in thirty (68.4%) it was more eight day's duration with previous attacks of obstruction (median: 19.3 days, range: 17 hours to 94 days). Pain, vomiting and distention were present in almost all of these cases. The most frequent abdominal finding was distention. The location of the volvulus was: ileum, 12 cases (63.2%), root of mesentery, 4 cases (21%) and jejunum, 3 cases (15.8%). Gangrenous bowel was present in six patients (31.5) and gangrenous intestine with perforation in two cases (10.5%) who underwent resection of the involved segment with primary anastomosis. In this group one patient (5.2%) died of sepsis and the wound infection rate was of 37.5%. There was no statistically significant correlation with the duration of illness and the presence of gangrenous loops or the mortality rate (p > 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)
