Abstract:Objective: To synthesise the available evidence on the effects of interventions designed to improve the delivery of healthcare that reduces the greenhouse gas (GHG) emissions of healthcare. Design: Systematic review and structured synthesis. Search sources: Cochrane Central Register of Controlled Trials, PubMed, Web of Science and Embase from inception to 3 May 2023. Selection criteria: Randomised, quasi-randomised and non-randomised controlled trials, interrupted time series and controlled or uncontrolled before-after studies that assessed interventions primarily designed to improve the delivery of healthcare that reduces the GHG emissions of healthcare initiated by clinicians or healthcare services within any setting. Main outcome measures: Primary outcome was GHG emissions. Secondary outcomes were financial costs, effectiveness, harms, patient-relevant outcomes, engagement and acceptability. Data collection and analysis: Paired authors independently selected studies for inclusion, extracted data, and assessed risk of bias using a modified checklist for observational studies and the certainty of the evidence using Grades of Recommendation, Assessment, Development and Evaluation. Data could not be pooled because of clinical and methodological heterogeneity, so we synthesised results in a structured summary of intervention effects with vote counting based on direction of effect. Results: 21 observational studies were included. Interventions targeted delivery of anaesthesia (12 of 21), waste/recycling (5 of 21), unnecessary test requests (3 of 21) and energy (1 of 21). The primary intervention type was clinician education. Most (20 of 21) studies were judged at unclear or high risk of bias for at least one criterion. Most studies reported effect estimates favouring the intervention (GHG emissions 17 of 18, costs 13 of 15, effectiveness 18 of 20, harms 1 of 1 and staff acceptability 1 of 1 studies), but the evidence is very uncertain for all outcomes (downgraded predominantly for observational study design and risk of bias). No studies reported patient-relevant outcomes other than death or engagement with the intervention. Conclusions: Interventions designed to improve the delivery of healthcare that reduces GHG emissions may reduce GHG emissions and costs, reduce anaesthesia use, waste and unnecessary testing, be acceptable to staff and have little to no effect on energy use or unintended harms, but the evidence is very uncertain. Rigorous studies that measure GHG emissions using gold-standard life cycle assessment are needed as well as studies in more diverse areas of healthcare. It is also important that future interventions to reduce GHG emissions evaluate the effect on beneficial and harmful patient outcomes. Prospero registration number: CRD42022309428.

Abstract:ObjectiveTo review the international literature and assess the ways healthcare systems are mitigating and can mitigate their carbon footprint, which is currently estimated to be more than 4.4% of global emissions.DesignSystematic review of empirical studies and grey literature to examine how healthcare services and institutions are limiting their greenhouse gas (GHG) emissions.Data sourcesEight databases and authoritative reports were searched from inception dates to November 2023.Eligibility criteria for selecting studiesTeams of investigators screened relevant publications against the inclusion criteria (eg, in English; discussed impact of healthcare systems on climate change), applying four quality appraisal tools, and results are reported in accordance with PRISMA (preferred reporting items for systematic reviews and meta-analyses).ResultsOf 33 737 publications identified, 32 998 (97.8%) were excluded after title and abstract screening; 536 (72.5%) of the remaining publications were excluded after full text review. Two additional papers were identified, screened, and included through backward citation tracking. The 205 included studies applied empirical (n=88, 42.9%), review (n=60, 29.3%), narrative descriptive (n=53, 25.9%), and multiple (n=4, 2.0%) methods. More than half of the publications (51.5%) addressed the macro level of the healthcare system. Nine themes were identified using inductive analysis: changing clinical and surgical practices (n=107); enacting policies and governance (n=97); managing physical waste (n=83); changing organisational behaviour (n=76); actions of individuals and groups (eg, advocacy, community involvement; n=74); minimising travel and transportation (n=70); using tools for measuring GHG emissions (n=70); reducing emissions related to infrastructure (n=63); and decarbonising the supply chain (n=48).ConclusionsPublications presented various strategies and tactics to reduce GHG emissions. These included changing clinical and surgical practices; using policies such as benchmarking and reporting at a facility level, and financial levers to reduce emissions from procurement; reducing physical waste; changing organisational culture through workforce training; supporting education on the benefits of decarbonisation; and involving patients in care planning. Numerous tools and frameworks were presented for measuring GHG emissions, but implementation and evaluation of the sustainability of initiatives were largely missing. At the macro level, decarbonisation approaches focused on energy grid emissions, infrastructure efficiency, and reducing supply chain emissions, including those from agriculture and supply of food products. Decarbonisation mechanisms at the micro and meso system levels ranged from reducing low value care, to choosing lower GHG options (eg, anaesthetic gases, rescue inhalers), to reducing travel. Based on these strategies and tactics, this study provides a framework to support the decarbonisation of healthcare systems.Systematic review registrationPROSPERO: CRD42022383719.

Abstract:Background: Circular economy has raised great interest as a concept and as a development model worldwide. This concept aims to provide a substitute for the linear economic model, which was based on production and consumption, continuous growth, and resources depletion. Circular economy allows a greener economy with sustainable development and promotes more balanced societies. The healthcare sector is a major contributor to the climate crisis, with a carbon footprint representing 4.4% of global net emissions. It is thus essential to rethink the applicability of circular economy in healthcare. Methods: We conducted a scoping review guided by the Arksey and O’Malley methodological framework and utilised PRISMA-ScR checklist. A systematic search from MEDLINE complete, SCOPUS and Web of Science databases published between 1992 and 2022. Results: Through database searching a total of 1018 records were identified and 475 duplicates were removed. From the total search, 543 articles were screened by title/abstract according to the inclusion and exclusion criteria. After screening, 38 full-text articles were selected and assessed for eligibility. Forty-seven additional records were also identified through other sources and screened for eligibility. Other sources included: 12 articles from snowballing of previous papers; 9 articles following peer-reviewers suggestions; 19 reports from relevant organisations in CE and healthcare; two webpage, and one book. Conclusion: Specific areas were identified where hospitals could reduce their greenhouse gas emissions and consequently their negative environmental impact, namely through waste management, energy, water, transportation/travel, hospital design, food optimisation, green procurement and behaviour. Also, lack of staff awareness and knowledge of the environmental impact of healthcare, and hospitals sustainability were identified as major contributors.

Addressing Climate Health: A Practical Guide to Quantifying and Reducing Health Care-Associated Emissions

Abstract:Objective: The objective was to quantify annual greenhouse gas emissions from a surgical specialty hospital and identify high-yield areas to reduce emissions associated with patient care. Study design: Pre-post study, greenhouse gas inventory. Setting: Specialty hospital. Methods: A scope 1 and scope 2 greenhouse gas inventory of the Massachusetts Eye and Ear main campus for calendar years (CY) 2020, 2021, and 2022 was performed by assessing emissions attributable to on-site sources (scope 1) and purchased electricity and steam (scope 2). The associated carbon dioxide equivalent was then calculated using known global warming potentials and emission factors. Results: The major contributors to scope 1 and scope 2 emissions at our institution for CY 2020 to 2022 were waste anesthetic gases and purchased steam. These results were reviewed with hospital leadership and a plan was developed to reduce these emissions. Emission monitoring is ongoing to assess the efficacy of these interventions. Conclusion: Measuring scope 1 and scope 2 emissions at the facility level allows health care facilities to develop institution-specific interventions and compare data across health care organizations. Surgeons can lead on health care system sustainability by collaborating with clinical and nonclinical staff to measure emissions, developing targeted emissions-reduction interventions, and tracking progress with yearly assessments.

Abstract:Abstract Climate change is an existential threat to humanity. While the healthcare sector must manage the health-related consequences of climate change, it is a significant contributor to greenhouse gas emissions, responsible for up to 4.6% of global emission, aggravating global warming. Within the hospital environment, the three largest contributors to greenhouse gas emissions are the operating theatre, intensive care unit and gastrointestinal endoscopy. Knowledge of the health-related burden of climate change and the potential transformative health benefits of climate action is important to all health professionals, as they play crucial roles in effecting change. This article summarises the available literature on the impact of healthcare on climate change and efforts in mitigation, focusing on the intrinsic differences and similarities across the operating theatre complex, intensive care unit and gastrointestinal endoscopy unit. It also discusses strategies to reduce carbon footprint.

Abstract:To date, greenhouse gas (GHG) emissions, mitigation strategies and the accompanying health co-benefits in different economic sectors have not been fully investigated. The purpose of this paper is to review comprehensively the evidence on GHG mitigation measures and the related health co-benefits, identify knowledge gaps, and provide recommendations to promote further development and implementation of climate change response policies. Evidence on GHG emissions, abatement measures and related health co-benefits has been observed at regional, national and global levels, involving both low- and high-income societies. GHG mitigation actions have mainly been taken in five sectors: energy generation, transport, food and agriculture, household and industry, consistent with the main sources of GHG emissions. GHGs and air pollutants to a large extent stem from the same sources and are inseparable in terms of their atmospheric evolution and effects on ecosystem; thus, GHG reductions are usually, although not always, estimated to have cost effective co-benefits for public health. Some integrated mitigation strategies involving multiple sectors, which tend to create greater health benefits. The pros and cons of different mitigation measures, issues with existing knowledge, priorities for research, and potential policy implications were also discussed. Findings from this study can play a role not only in motivating large GHG emitters to make decisive changes in GHG emissions, but also in facilitating cooperation at international, national and regional levels, to promote GHG mitigation policies that protect public health from climate change and air pollution simultaneously.

Abstract:Healthcare systems (HSs) provide essential services to populations yet require a large amount of energy to perform duties and tasks. Specifically, 4.4% of global net carbon emissions can be attributed precisely to the activities of HSs. Therefore, it seems unlikely that the targets of zero carbon dioxide (CO 2 ) emissions by 2050, set by the Paris Agreement, will be met without rethinking the role and impact of HSs towards climate change. Here we summarize the available literature on CO 2 emissions depending on direct and indirect Hss activities. We show that the number of studies addressing this topic is still very limited and that most of the emissions derive from the supply chain HSs rely on to perform their tasks. Furthermore, we highlight how this topic is addressed unevenly among countries. Indeed, we found that less economically developed countries are less represented in the literature on this topic and that, even in economically developed countries, most of the studies come from English-speaking countries. In the discussion, we stress that health workers and policymakers should pay more attention to this issue in order to tackle climate change and related health issues, following the example of few virtuous countries. Finally, some potential impact mitigation strategies are discussed.

Abstract:1. Single-unit neuronal activity was recorded in the primary motor and superior precentral premotor areas of two rhesus monkeys during an arm reaching task. The task involved moving a cursor displayed on a video terminal using a draftsman's arm-type manipulandum. From a centrally located start box the animal was required to move to 1 of 48 target boxes at eight different directions (0-360 degrees in 45 degrees intervals) and six distances (1.4-5.4 cm in 0.8-cm increments). Both direction and distance for the upcoming movement were unpredictable. 2. The activity of 197 arm movement-related cells was recorded and evaluated for each of the 48 targets. Histological examination showed the cells to be primarily in the primary motor cortex or in the premotor area around the superior precentral sulcus. Each cell's discharge was aligned on movement onset and averaged over five trials for each target. Movement kinematics including hand path velocity were also determined. The task time was divided into three epochs, a premovement period (PT), a movement period (MT), and total time (TT = PT+MT). For each epoch the average firing was correlated with the direction and distance of the movement using various regression procedures. 3. An analysis of variance (ANOVA) showed that the majority of neurons were modulated significantly by movement direction in each of the three time periods, PT (73.7%), MT (68.3%), and TT (78.5%). The relationship of the firing to direction was fit to a cosine tuning function for each significantly modulated cell. In 86.3% of the cells the firing was correlated significantly with a cosine function of movement direction in TT. A cell's preferred direction varied little for different movement distances. The mean difference in preferred direction for the smallest possible change in distance (0.8 cm) was 12.8 +/- 11.4 degrees (SD) and 17.1 +/- 14.7 degrees for the largest change in distance (4.0 cm). 4. Correlation analysis revealed that the activity of the majority of cells was modulated significantly by distance along at least one direction in each of the three time periods, PT (46.8%), MT (68.8%), and TT (67.7%). Subsequently, a univariate linear regression model was used to quantify a cell's discharge as a function of distance. For the regressions of firing with distance with a statistically significant correlation (r > 0.8), the mean slope was 3.59 +/- 0.17 spikes.s-1.cm-1 for the total time. The existence of a significant distance modulation was not invariably correlated with a cell's preferred movement direction.(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract:This paper describes a sophomore/junior-level laboratory course on network administration. The course aims to introduce networking concepts while emphasizing observation and measurement of network traffic. It utilizes an isolated laboratory of PCs running Linux along with numerous hubs, switches, and routers that can be rewired into multiple configurations. Being isolated from the campus network allows students complete administrative access without compromising campus security. Groups of students assemble their own LAN, attach it to the lab's "internet" backbone via a router, and then provide numerous common services like HTTP, DNS, and remote access to the rest of the lab. Once the room has a functioning internet, students examine various issues like encryption with TLS and ssh, as well as caching and load issues in retrieving different types of data from a web server. This paper covers the goals of the course, the laboratory setup, the exercises used in the course, and our experiences teaching the course.

Abstract:As a result of the carbon-intensive nature of health care, hospital facilities are contributors to global warming. Health care's contributions to global warming and greenhouse gas emissions include those associated with carbon emissions, energy consumption, pharmaceuticals, travel-related emissions and procurement. This article presents a review of environmental impact of different medical services. The published research articles focused on estimating the carbon footprint of healthcare services are investigated here. Pharmaceuticals and energy consumption emerge as the primary contributors to the carbon emission. Carbon emissions from surgical activities and the treatment of patients in the Intensive Care Unit (ICU) stand out as the two most vital sectors contributing to carbon emissions. This review also discusses methods used for determining greenhouse gas emissions. Life cycle assessment and component analysis are the two most used methods for calculating emissions. This study also highlights the existing challenges related to estimation of carbon emission of different healthcare services and ways to overcome these challenges associated with carbon emission. The most challenging aspect of the estimation is that the data collection has been conducted under specific circumstances and limited to any particular region. As a result, the outcomes may vary for different circumstance and different area. Considering the maximum possible number of factors and areas can improve the quality of the outcome.

Abstract:Healthcare services have now become a fundamental requirement for all individuals owing to rising pollution levels and shifting lifestyles brought on by fast modernization. The hospital is a specialized healthcare facility where doctors, nurses, and other medical professionals offer their services. Academics and professionals have emphasized green operation initiatives such as green design, green purchasing, green supply chain, and green manufacturing to increase public awareness of environmental problems affecting company operations associated with healthcare for the quality of life. The purpose of this research is to use total interpretive structural modeling and MICMAC (matrix cross multiplication applied to a classification) analysis to investigate and analyze the elements impacting green operations strategies in healthcare. The data are gathered using a closed-ended questionnaire together with a scheduled interview. The components' interactions are explored using the total interpretive structural modeling technique, and the MICMAC analysis is used to rank and categorize the green operation strategy variables. The study is a novel effort to address and focus on stakeholders, vision and structure, resources, and capabilities. Green operations strategies have only been the subject of a small number of studies in the past, and those studies were mostly addressed at manufacturing-specific green strategies. Thus, by promoting energy efficiency programs, green building design, alternative sources of energy, low-carbon transportation, local food, waste reduction, and water conservation, the health sector can develop multiple key strategies to become more climate-friendly with significant health, environmental, and social co-benefits for quality of life.

Abstract:Economic evaluations aim to compare the costs and the results of health strategies to guide the public decision-making process. Cost estimation is, thus, a cornerstone of this approach. At present, few national evaluation agencies recommend incorporating the cost of greenhouse gas (GHG) emissions from healthcare actions into the calculation of healthcare costs. Our main goal is to describe and discuss the methodology for integrating the cost of GHG emissions into the field of applied economic evaluations. To estimate this cost, three steps are required: (1) identifying and quantifying the physical flows linked to the production and management of the outputs of healthcare interventions, (2) estimating the quantity of GHG that can be attributed to each physical flow, and (3) valuing these GHG emissions in monetary terms. Integrating the cost of GHG emissions into the calculation of the costs of healthcare interventions is both useful and relevant from a perspective of collective intergenerational well-being. This approach has been made possible thanks to the existence of accounting and monetary valuation methods for emissions. Agencies specialized in health economic evaluations could take up this issue to resolve ongoing questions, thus providing researchers with a methodological framework and public decision-makers with some key insights.

Abstract:Obesity is a growing issue worldwide, whose causes and consequences are linked to the environment and which therefore has a high carbon footprint. On the other hand, obesity surgery, along with other procedures in surgical suites, entails environmental consequences and responsibilities. We conducted a prospective comparative study on two groups of bariatric interventions (N = 59 and 56, respectively) during two consecutive periods of time (Oct 2021–March 2022), first without and then with specific measures aimed at reducing greenhouse gas emissions related to bariatric procedures by approximately 18%. These measures included recycling of disposable surgical equipment, minimizing its use, and curbing anesthetic gas emissions. Further and continuous efforts/incentives are warranted, including reframing the surgical strategies. Instead of comparing measurements, which is difficult at the present time, we suggest defining an ECO-SCORE in operating rooms, among other healthcare facilities.

Abstract:Background: There is a pressing need for healthcare to respond to the climate crisis. Family physicians, given their central role in community healthcare provision, are strategically placed to lead, support, and promote sustainable healthcare, yet guidance on how to do this is fragmented. Objective: To identify and evaluate toolkits and aids on sustainable healthcare to act as a curated resource for family physicians and their care teams interested in delivering evidence-based sustainable healthcare in their clinical practices. Methods: A scoping review was completed of the published and grey literature across 4 databases and 2 search engines to identify articles and aids/toolkits from 1990 to present. Toolkits were subsequently evaluated for purpose, evidence-base, implementation process, adaptability to family medicine, and outcome measures. Results: The search identified 17,751 articles. Screening resulted in 20 published articles and 11 toolkits. Most articles presented simple checklists to support greening clinic initiatives, 3 studies focussed on partial carbon footprint analyses, and 4 on educational initiatives. Toolkits ranged in sustainability topics and degree of depth covered, and adaptability and outcome measures. None of the resources identified have been formally evaluated for effectiveness. Conclusions: A range of aids exist to support greening of clinic operations; however, there is a significant gap in the literature for greening clinical care. Two toolkits were found to be comprehensive, one requiring tracking and reporting of sustainability initiatives. This scoping review provides a starting point for motivated family doctors and community clinics to initiate change and support more sustainable healthcare.
The green operating room: simple changes to reduce cost and our carbon footprint

Abstract:Generating over four billion pounds of waste each year, the healthcare system in the United States is the second largest contributor of trash with one-third produced by operating rooms. Our objective is to assess improvement in waste reduction and recycling after implementation of a Green Operating Room Committee (GORC) at our institution. A surgeon and nurse-initiated GORC was formed with members from corporate leadership, nursing, anesthesia, and OR staff. Initiatives for recycling opportunities, reduction of energy and water use as well as solid waste were implemented and the results were recorded. Since formation of GORC in 2008, our OR has diverted 6.5 tons of medical waste. An effort to recycle all single-use devices was implemented with annual solid waste reduction of approximately 12,860 lbs. Disposable OR foam padding was replaced with reusable gel pads at greater than $50,000 per year savings. Over 500 lbs of previously discarded batteries were salvaged from the OR and donated to charity or redistributed in the hospital ($9,000 annual savings). A "Power Down" initiative to turn off all anesthesia and OR lights and equipment not in use resulted in saving $33,000 and 234.3 metric tons of CO2 emissions reduced per year. Converting from soap to alcohol-based waterless scrub demonstrated a potential saving of 2.7 million liters of water annually. Formation of an OR committee dedicated to ecological initiatives can provide a significant opportunity to improve health care's impact on the environment and save money.
An umbrella review of health co-benefits from actions to reduce greenhouse gas emissions

Abstract:Background: There have been many modelled studies of potential health co-benefits from actions to reduce greenhouse gas emissions, but so far there have been no large-scale attempts to compare the magnitude of health and climate effects across sectors, countries, and study designs. Methods: As part of the Pathfinder Initiative project an umbrella review of studies was done, and 26 previous reviews were identified with 57 primary studies included. Studies included in the review were required to have quantified changes in greenhouse gas emissions and health effects (or risk factors) from defined actions to reduce climate effects. Study data were extracted and harmonised by standardising impact measures per 100 000 of the national population (or urban population for city-level actions), averaging effects over a 1-year period and aggregating actions into their respective sectors by use of a predefined framework. Findings: From 200 mitigation actions, the majority were in the agriculture, forestry, and land use sector (103 actions [52%]), followed by the transport sector (43 actions [22%]). The largest effects on greenhouse gas emissions were seen from actions in the energy sector, and these actions also had substantial health co-benefits in lower middle-income countries, although benefits were smaller in high-income settings. The greatest health benefits were seen from actions to change diets and introduce clean cookstoves. The major pathways to health were through reduced air pollution, healthier diets, and increased physical activity from switching to active travel modes. Effect sizes tended to be larger from national modelling studies and smaller from localised or implemented actions. Interpretation: The potential co-benefits to health from actions to reduce climate change are large, but most evidence still comes from modelling studies and from high-income and middle-income countries. There are also major context-dependent differences in the magnitude of effects found, so actions need to be tailored to the local context and careful attention needs to be paid to potential trade-offs and spillover effects. Abstract:Introduction: Nowadays, climate change is seen as a visible threat that we are asked to face every day by posing new political challenges. Hospital waste is estimated at 1% of solid waste and 2.1% of the annual greenhouse gas emissions of each country. More specifically, operating theaters are the main source of pollution coming from Hospitals, with anesthesia departments responsible for 25% of these pollutants. Method: The General Oncology Hospital of Athens "Saint Savvas" and in particular the Anesthesiology Department, in collaboration with the Surgery, participates in the global effort to reduce CO2 emissions to protect the environment, adopting the 5R program (Recycle, Rethink, Research, Reduce, Reuse) and the ESAIC (European Society of Anaesthesiology and Intensive Care) guidelines "How to reduce the carbon footprint in the surgery, in the hospital, on the planet". Qualitative data analysis was performed on outcomes Results: Decreased desflurane consumption per surgery by -7.31% and overall, in the year 2023 by -1.59%. Reduced battery consumption from -73.89% to -100%. From the money saved, the items of the Anesthesiology Department's medical supply warehouse increased by +16.56%, while the percentage value decreased by -11.39%. Conclusion: With the adoption of the 5R program, it seems that financial and material resources are saved and the consumption of volatile gases per operation is significantly reduced.

Abstract:Background and objectives: Public health co-benefits from curbing climate change can make greenhouse gas (GHG) mitigation strategies more attractive and increase their implementation. The purpose of this systematic review is to summarize the evidence of these health co-benefits to improve our understanding of the mitigation measures involved, potential mechanisms, and relevant uncertainties. Methods: A comprehensive search for peer-reviewed studies published in English was conducted using the primary electronic databases. Reference lists from these articles were reviewed and manual searches were performed to supplement relevant studies. The identified records were screened based on inclusion criteria. We extracted data from the final retrieved papers using a pre-designed data extraction form and a quality assessment was conducted. The studies were heterogeneities, so meta-analysis was not possible and instead evidence was synthesized using narrative summaries. Results: Thirty-six studies were identified. We identified GHG mitigation strategies in five domains - energy generation, transportation, food and agriculture, households, and industry and economy - which usually, although not always, bring co-benefits for public health. These health gains are likely to be multiplied by comprehensive measures that include more than one sectors. Conclusions: GHG mitigation strategies can bring about substantial and possibly cost-effective public health co-benefits. These findings are highly relevant to policy makers and other stakeholders since they point to the compounding value of taking concerted action against climate change and air pollution.
