Abstract:A 54-year-old Chinese woman presented with a 10-year history of repeated paroxysmal palpitations. She was diagnosed with paroxysmal supraventricular tachycardia by 12-lead electrocardiogram and was advised to undergo catheter-based radiofrequency ablation. During the procedure, a rare complication occurred that was diagnosed as a right internal mammary artery penetrating injury. After appropriate emergency treatment with arterial embolization and membrane-covered stent implantation, the patient was out of immediate danger of haemorrhaging. Follow-up computed tomography angiography of the subclavian artery at 3 months after she was discharged from hospital revealed stent-graft patency with no evidence of in-stent thrombosis or stent stenosis. No problems were observed at the 6-month follow-up visit.
[Risk analysis of gastrointestinal bleeding in patients with acute ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention]

Abstract:Objective: Of all spontaneous bleeding complications in patients with acute ST-elevation myocardial infarction (STEMI), upper gastrointestinal bleeding (UGIB) is the most common and of specific interest, because it can be prevented by several prophylactic measures. The purpose of this study was to investigate the in-hospital incidence, associated outcomes, and predictors of UGIB after STEMI. Methods: In this retrospective study, we analyzed the records of 2 791 patients with acute STEMI admitted to Beijing Anzhen Hospital Affiliated to Capital Medical University between January 2018 and January 2022. The patients were divided into the UGIB group (n=61) and non-UGIB group (n=2 730) according to the presence or absence of upper gastrointestinal hemorrhage, respectively. Baseline clinical conditions, coronary lesions, in-hospital deaths, and in-hospital adverse events were compared between the two groups. Logistic regression analysis was also performed for risk factors that could lead to UGIB. Results: The in-hospital incidence of UGIB after STEMI was 2.2% (61/2 791). Hospital stay was significantly longer in the UGIB group [8(6, 12) days vs. 5 (4, 7) days, Z=3.28, P<0.001] and in-hospital mortality was significantly higher in the UGIB group than in the non-UGIB group (9.8% vs. 0.8%, χ2=0.63, P=0.001). Patients with UGIB were significantly older than those without UGIB (63±11 years vs. 58±11 years, t=-3.75, P<0.001). The serum creatinine level of UGIB patients was significantly higher than that of non-UGIB patients [(80(62, 98) mmol/L vs. 73(64, 84) mmol/L, Z=1.68, P=0.007], the red blood cell count of UGIB patients was significantly lower than that of non-UGIB patients [4.1(3.8, 4.6)×1012/L vs. 4.6(4.2, 4.9)×1012/L, Z=2.61,P<0.001], and the hemoglobin concentration of UGIB patients was significantly lower than that of non-UGIB patients [129(109, 141) g/L vs. 141(130, 152) g/L, Z=2.52,P<0.001]. Brain natriuretic peptide levels were significantly higher in UGIB patients than in non-UGIB patients [331(165, 644) ng/L vs. 181(89, 333) ng/L, Z=2.42,P<0.001]. Logistic regression analysis showed that age (OR=1.045, 95%CI 1.009-1.082, P=0.013); hemoglobin (OR=1.594, 95%CI 1.150-2.210, P=0.005); hematocrit (OR=0.181, 95%CI 0.060-0.546, P=0.002); and mean hemoglobin concentration (OR=0.845, 95%CI 0.752-0.951, P=0.005) were independent risk factors for UGIB in patients with STEMI. Logistic regression analysis of risk factors for in-hospital death revealed that concurrent UGIB was an independent risk factor for in-hospital death in patients with STEMI (OR=2.954, 95%CI 0.635-13.751, P=0.024). Conclusions: The incidence of in-hospital UGIB in STEMI patients was 2.2%, and the in-hospital mortality rate of STEMI complicated with UGIB increased to 9.8%. Concurrent UGIB was an independent risk factor for in-hospital death in patients with STEMI. The most important predictors of in-hospital UGIB in patients with STEMI were age, hemoglobin, hematocrit, and mean hemoglobin concentration.
Lateral Position Intubation Followed by Endoscopic Ultrasound-Guided Angiotherapy in Acute Esophageal Variceal Rupture: A Case Report.

Abstract:BACKGROUND Massive esophageal variceal bleeding can be catastrophic, leading to high morbidity and mortality. Patients experiencing massive esophageal variceal bleeding are at high risk of aspiration and hemorrhagic shock in acute episodes. Intubation and bleeding control are the two essential steps for resuscitation of these patients.CASE SUMMARY A 47-year-old male patient was admitted to our hospital with upper digestive tract bleeding. He was diagnosed with alcohol-induced liver cirrhosis and consequent esophagogastric varices. As he did not show a good response to somatostatin and Sengstaken-Blakemore tube placement, the patient was scheduled for endoscopic angiotherapy under anesthesia. Preoperative assessment showed an ASA physical status of III and Child-Pugh classification B. However, massive hemorrhage occurred just after induction of anesthesia. Intubation by video-guided laryngoscopy in the lateral decubitus position was attempted twice and was successful. After that, an experienced endoscopic ultrasound（EUS） specialist performed angiotherapy and occluded the culprit vessel. An ultra-thin gastroscope was then inserted into the endotracheal tube to extract the blood observed in the lobar bronchi. The patient suffered hemorrhagic shock with an estimated blood loss of 1500 m L in 20 min and remained in the intensive care unit for two days. The patient was discharged from our hospital eight days later without major complications.CONCLUSION Intubation in the lateral decubitus position and EUS-guided treatment can be life-saving procedures in patients with massive upper gastrointestinal hemorrhage.
The Use of the Hemostatic Agent Zhelplastan in Combination with a Granulated Sorbent in the Treatment of Patients with Mallory-Weiss Syndrome

Abstract:Background: Mallory-Weiss syndrome (MWS) is one of the common causes of acute upper gastrointestinal bleeding. The aim of our research was to develop a method for the treatment of MWS, which would improve the efficiency of local hemostasis, reduce the risk of re-bleeding and shorten the treatment time. Methods and Results: The study involved 102 patients with MWS. All patients were divided by random sampling into two groups: the main group (MG) and control group (CG). Patients of both groups were comparable in age, gender, concomitant diseases, type of bleeding, size of bleeding defect, and duration of observations. The MG included 52 patients, who were given the developed method of MWS treatment. Patients with ongoing bleeding were treated with a combined method of endoscopic hemostasis: active bleeding was initially stopped by infiltration or coagulation (argon-plasma coagulation was preferred), and then to increase the reliability of hemostasis and prevent the resumption of bleeding, powdered Zhelplastan 0.2g was applied to the rupture area with an insufflator, followed by Sephadex G-25 0.3g (Patent RF No2633925). In patients with signs of unstable hemostasis, the developed method of cytoprotective action on a clot or thrombosed vessel was used by applying a local hemostatic, Zhelplastan 0.2g, to the defect area in combination with a granular sorbent, Sephadex G-25 0.3g, during therapeutic endoscopy to prevent the resumption of bleeding. In patients with stable stopped bleeding, a hemostatic powdered Zhelplastan was applied to the defect area using an endoscope in combination with Sephadex G-25, according to the developed method. In the CG (n=50), well-known methods of arresting endoscopic bleeding (pricking with aminocaproic acid, vasoconstrictors, argon-plasma coagulation, etc.) were used without local hemostatics and granular sorbents. Summing up the results of patients with Mallory-Weiss syndrome in the compared groups, in the MG final hemostasis was performed in 51(98.1%) patients. In the MG, bleeding resumed in 1(1.9%) patient. There were no emergency operations or deaths. In the CG, the bleeding was finally stopped in 43(86%) patients, and it resumed in 7(14%) patients. Emergency surgery was required for 1(2%) patient. In the CG, the mortality rate was 4%. Conclusion: In MWS, the use of the developed method of endoscopic treatment, including pneumoinsufflation of a powdered hemostatic agent in combination with a granular sorbent, against the background of conservative therapy, improved the results of treatment, increased the reliability of local hemostasis, reduced the number of recurrent bleedings, and excluded emergency operations.
A Surviving Case of Myocardial Infarction with Ventricular Septal Rupture and Ventricular Aneurysm following Gastrointestinal Bleeding

Abstract:A 55-year-old man presented to the emergency department with worsening shortness of breath 1 month after a gastrointestinal bleed. He had congestive heart failure, and an electrocardiogram suggested ischemic heart disease involvement. Echocardiography revealed a ventricular septal defect complicated by a left ventricular aneurysm in the inferior-posterior wall. Conservative treatment was started, but hemodynamic collapse occurred on the third day of admission and coronary angiography revealed a revascularizing lesion in the right fourth posterior descending coronary artery. Subsequently, his hemodynamic status continued to deteriorate, even with an Impella CP® heart pump, so ventricular septal defect patch closure and left ventricular aneurysm suture were performed. His condition improved and he was discharged on day 23 of admission and was not readmitted within 6 months after the procedure. Hemodynamic management of ventricular septal defects requires devices that reduce afterload, and clinicians should be aware of the risk of myocardial infarction after gastrointestinal bleeding.
A National Study to Evaluate Trends in the Utilization of Nerve Reconstruction for Treatment of Neonatal Brachial Plexus Palsy [Outcomes Article]

Abstract:Background: Approximately 4 to 34 percent of infants born with neonatal brachial plexus palsy do not recover spontaneously and require surgery. Despite the increasing availability of microsurgical nerve repair, the authors hypothesize that this condition remains undertreated and that uninsured children and children with public insurance are less likely to receive treatment than those with private insurance. Methods: The authors used a national sample of inpatient hospital discharge data from the Healthcare Cost and Utilization Kids Inpatient Databases for the years 1997, 2000, 2003, and 2006. Relevant discharges were identified using the International Classification of Diseases, Ninth Revision diagnosis code 767.6 for neonatal brachial plexus palsy and procedure codes relating to nerve surgery. Weighted frequencies were calculated to generate national estimates for neonatal brachial plexus palsy births and nerve surgery procedures for these patients. Results: A total of 21,758 births with neonatal brachial plexus palsy and 721 admissions for nerve surgery were identified. Over time, utilization of nerve surgery procedures has generally increased (1.1 percent in 1997 to 3.2 percent in 2006). Treatment with nerve surgery varied significantly according to insurance status—3.8 percent among private insurance discharges, 2.9 percent among Medicaid insurance discharges, and 0.7 percent among self-pay/uninsured records (p < 0.001). The mean age among nerve surgery patients was 235 ± 75 days, and the mean total charges associated with microsurgical intervention was $24,534 ± $30,460. Conclusions: Over the past decade, approximately 3.3 percent of neonatal brachial plexus palsy births have undergone some form of primary microsurgical nerve surgical intervention, which may reflect underutilization of these procedures and limited access to care. Insurance status plays a significant role in the use of nerve surgery procedures, as neonates without private insurance were less likely to receive nerve surgery procedures than those with private insurance.

Abstract:BACKGROUND Gastric wall necrosis is a rare complication of endoscopic treatment for bleeding gastric ulcer, which may exacerbate the patient’s condition once it occurs and may even require surgical intervention for treatment. CASE SUMMARY A 59-year-old man was admitted to our department with melena. Endoscopy revealed a giant ulcer in the gastric antrum with a visible vessel in its center, which was treated with sclerosants and tissue glue injection and resulted in necrosis of the gastric wall. CONCLUSION Injection of sclerosants and tissue glue may lead to gastric wall necrosis, which is a serious complication. Therefore, before administering this treatment to patients, we should consider other more effective methods of hemostasis to avoid gastric wall necrosis.

Abstract:Reductions in treatment time are attractive for patients and dialysis centers. However, there are concerns related to treatment adequacy and increased intradialytic symptoms. Treatment times were progressively reduced in 12 stable patients on standard 4-h acetate hemodialysis, solute clearances being increased proportionately, until the tolerance limit manifested by increased complications or 2.5 h of treatment was reached. Once shortened schedules (mean reduction approximately 31%) were achieved, a 2-month surveillance period was initiated, followed by 2 additional months of bicarbonate therapy, treatment time being unchanged. Rapid bicarbonate hemodialysis was associated with a significantly lower incidence of hypotension, nausea, and vomiting than rapid acetate or standard acetate therapies. Fluid removal was comparable with that of standard treatment, and serum chemistry levels remained stable. Based on the success of these studies treatment times have been reduced in greater than 200 patients with high-efficiency bicarbonate therapy for long-term evaluations.

Abstract:Objective:To evaluate the clinical value of endoscopic hemostasis in acute nonvariceal upper gastrointestinal bleeding.Methods:This was a retrospective study of 223 patients with acute non-variceal upper gastrointestinal bleeding and receiving endoscopic treatment who were admitted to Peking University Third Hospital between January 1,2005 and December 31,2009.Endoscopic diagnosis,lesion location,lesion size and stigmata of recent hemorrhage were recorded.Stigmata of recent hemorrhage was evaluated by Forrest classification.All the patients were scored by Rockall for rehemorrhage and death risk.Endoscopic treatment comprised medicine aspersing,injection,thermocoagulation,clips and combination therapy.Results: Hemorrhagic lesions of Forrest Ia-Ⅱb were selected for endoscopic treatment,in which 214 patients(96.0%,214/223) underwent first endoscopic hemostasis successfully,while rehemorrhage occurred in 34 patients(15.2%,34/223).The first hemostatic achievement rate was 80.7%(180/223).And 17 patients received surgery or died because of endoscopic treatment failure.Total effective rate of endoscopic treatment was 92.4%(206/223).The total effective rates of Rockall high-risk group,moderate-risk group and low-risk group were 80%(40/50),95.7%(156/163) and 100%(10/10) respectively.The effective rates of epinephrine injection and combination therapy were 92.6%(137/148) and 77.6%(38/49) respectively.The rehemorrgagic rates of epinephrine injection and combination therapy were 14.2%(21/148) and 18.4%(9/49) respectively.The proportion of combination therapy in the second attempt at endoscopic therapy was 65.0%(13/20),and the achievement rate was 61.5%(8/13).Conclusion: Endoscopic hemostatic therapy is the preferred emergency treatment in acute nonvariceal upper gastrointestinal bleeding.Endoscopic treatment should be used for emorrhagic lesions of Forrest Ia-Ⅱb.Endoscopic therapy could be completely hemostatic in Rockall low-risk group.Rockall score directly influences endoscopic treatment effectiveness.
Upper gastric tract bleeding due to cholecystoduodenal fistula: a case report

Abstract:A 50-year-old man was admitted to our hospital because of a single episode of hematemesis followed by melena for about 14 hours. On gastroendoscopy a 1.2 x 1.4 cm2 deep ulcer was visualized in the back wall of the first part of the duodenum with active bleeding. Digital subtracted angiography showed an obvious leakage of contrast agents from the gallbladder artery. So hemobilia and cholecystoduodenal fistula with upper gastric tract bleeding were diagnosed. Cholecystectomy was performed after the duodenal fistula was resected and repaired. Common bile duct was intubated with a T-tube after no bleeding and stones were verified inside. A pathologic examination confirmed the presence of chronic cholecystitis with the formation of suppurate granulomatosis. The postoperative course was uneventful. He has been doing well without any recurrence of bleeding for the last 7 months.

Abstract:A 36-year-old male presented with dizziness and melena 5 years post laparoscopic Roux-en-Y gastric bypass (LRYGB). Initial treatments with proton pump inhibitors and blood transfusions was followed by esophagogastroduodenoscopy (EGD) and CT angiography, identifying an active arterial bleed at the gastrojejunal anastomosis. Super-selective celiac artery coil embolization successfully controlled the bleeding. This case highlights the importance of considering late-onset gastrointestinal bleeding as a potential complication post-LRYGB. Early diagnosis and intervention, including advanced radiological techniques, are crucial for successful management and optimal patient outcomes.

Abstract:Upper gastrointestinal (GI) hemorrhage presents a substantial clinical challenge. Initial management typically involves resuscitation and endoscopy within 24 h, although the benefit of very early endoscopy (< 12 h) for high-risk patients is debated. Treatment goals include stopping acute bleeding, preventing rebleeding, and using a multimodal approach encompassing endoscopic, pharmacological, angiographic, and surgical methods. Pharmacological agents such as vasopressin, prostaglandins, and proton pump inhibitors are effective, but the increase in antithrombotic use has increased GI bleeding morbidity. Endoscopic hemostasis, particularly for nonvariceal bleeding, employs techniques such as electrocoagulation and heater probes, with concerns over tissue injury from monopolar electrocoagulation. Novel methods such as Hemospray and Endoclot show promise in creating mechanical tamponades but have limitations. Currently, the first-line therapy includes thermal probes and hemoclips, with over-the-scope clips emerging for larger ulcer bleeding. The gold probe, combining bipolar electrocoagulation and injection, offers targeted coagulation but has faced device-related issues. Future advancements involve combining techniques and improving endoscopic imaging, with studies exploring combined approaches showing promise. Ongoing research is crucial for developing standardized and effective hemorrhage management strategies.

An extremely dangerous case of acute massive upper gastrointestinal bleeding: a case report

Abstract:Background: Upper gastrointestinal (GI) bleeding is a severe acute disease of gastroenterology department. Fish bone is the most common food-related foreign body. However, fish bone piercing the esophagus, causing the mediastinal abscess that corroded the left subclavian artery, resulting delayed but high-risk massive upper gastrointestinal bleeding is very rare. Case presentation: We report a 54-year-old man who was diagnosed with delayed but high-risk massive upper GI bleeding that was the result of a fish bone piercing the esophagus, causing a mediastinal abscess that corroded the left subclavian artery. He was saved effectively by early and timely multidisciplinary collaboration. Conclusion: A fish bone-caused mediastinal abscess that corrodes the left subclavian artery and induces delayed but high-risk massive upper GI bleeding is very rare. In addition to routine consideration of upper GI bleeding, medical history, endoscopy and CT are helpful for achieving a diagnosis. Importantly, early and timely multidisciplinary collaboration can effectively save critically ill patients.

Abstract:Postoperative delayed bleeding of submucosal tunnel is a rare complication after peroral endoscopic myotomy (POEM) for esophageal achalasia. However, once it occurs it can be fatal. Thus, early diagnosis and managements are most critical for patient outcome. Here, we showed emergency endoscopic diagnosis and hemostasis for delayed bleeding of submucosal tunnel after POEM in a 25-year-old male. This patient did not have any coagulation disorder before POEM and underwent POEM successfully. After discharge, he complained of progressive serious retrosternal pain from the first day after surgery and also suddenly had vomiting of fresh blood on the third day. Emergency gastroscopy was performed immediately for exploration. Hematoma was found along the submucosal tunnel and the covering mucosa was very swelling. After removing the metal clips of mucosal entry, a large number of blood clots were discovered in the submucosal tunnel, and were removed. The active bleeding points were identified and coagulated with hemostatic forceps. However, on the third day after first endoscopic hemostasis, there was major blood drainage from nasogastric tuble. A Sengstaken–Blakemore tube was placed into the stomach and lower part of the esophagus to compress the bleeding spot. Intermittence deflation of the balloons was done every 24 hours. The gastric balloon of Sengstaken–Blakemore tube was finally deflated on the first day after placement, and the esophageal balloon was finally released on the second day. Successful hemostasis was achieved and no blood transfusion was necessary. This case may provide a better understanding of delayed bleeding after POEM with an emphasis on its early features and effective managements. Vomiting of fresh blood and progressive serious retrosternal pain were the major early manifestations in patients with delayed bleeding of submucosal tunnel. Emergency endoscopic diagnosis and hemostasis should be taken as early as possible. It should be worth mentioning that a Sengstaken–Blakemore tube is particularly effective for hemostasis by compression.

Abstract:Endoscopic sphincterotomy is an essential procedure during endoscopic retrograde cholangiopancreatography (ERCP) for the diagnosis or treatment of pancreaticobiliary disease; however, postendoscopic sphincterotomy bleeding occasionally occurs and is a potentially life-threatening adverse event [1]. Recently, a novel self-assembling peptide matrix (PuraStat; 3-D Matrix Europe SAS, France), which is approved for hemostasis in gastrointestinal endoscopic procedures, was launched [2]. Because of its characteristics, this product is not effective in achieving hemostasis of pooled blood, as the peptide matrix assembles when the solution comes into contact with body fluids. For effective hemostasis, the peptide matrix gel should be applied as close as possible to the bleeding point of the injured vessel wall. Herein, we report the effective application of PuraStat for hemostasis of postendoscopic sphincterotomy bleeding while securing an endoscopic field of view with the gel immersion technique [3] and red dichromatic imaging (RDI; Olympus, Japan) [4] [5] ([Video 1]). Video 1 A novel self-assembling peptide matrix (PuraStat) is successfully applied to achieve hemostasis of postendoscopic sphincterotomy bleeding, with the endoscopic field of view secured with the gel immersion technique and red dichromatic imaging. An 85-year-old man underwent ERCP for acute cholangitis due to a common bile duct stone. The stone was successfully removed with a balloon extractor following endoscopic sphincterotomy ([Fig. 1]). After removal of the stone, bleeding occurred at the endoscopic sphincterotomy site ([Fig. 2]); therefore, we performed hemostasis using the peptide matrix gel. We used the gel immersion technique and RDI to secure the endoscopic field of view and detect the bleeding point ([Fig. 3]). As the flowing blood does not spread and mix with the gel, we applied the peptide matrix gel and put the endoscope into the gel. RDI was used to provide color contrast between the bleeding point and the pooled peripheral blood. The peptide matrix was applied directly onto the bleeding point using a dedicated catheter ([Fig. 4]), and complete hemostasis was achieved ([Fig. 5]). The patient's clinical course was uneventful after the procedure. Understanding the characteristics of various devices and techniques can lead to an effective and safe procedure. Abstract:During the past century, the incidence of myocardial infarction has markedly increased worldwide. Percutaneous coronary intervention with stent implantation is often considered as the first-choice treatment, especially in emergency cases. Current guidelines recommend delayed elective noncardiac surgery for such vulnerable patients. However, few suggestions are available regarding the exact treatment strategy for patients who have already undergone percutaneous coronary intervention but suddenly need emergent noncardiac surgery for an unrelated reason. We herein present a case involving a patient with acute myocardial infarction who had undergone implantation of a drug-eluting stent and developed an ileal perforation due to fish bone ingestion 3 days postoperatively. After carefully balancing the risks of stent thrombosis and uncontrollable bleeding, dual antiplatelet therapy and low-molecular-weight heparin were given with close monitoring. Emergency laparotomy and partial small bowel resection surgery were then performed, after which the patient eventually recovered. This case indicates a possible management strategy for patients with acute myocardial infarction complicated by emergency noncardiac surgery.
Renal pelvis mucosal artery hemorrhage after percutaneous nephrolithotomy: a rare case report and literature review

Abstract:Background: Following a percutaneous nephrolithotomy (PCNL) procedure, the most common complications are considered to be intraoperative and postoperative bleeding. Many patients with postoperative bleeding can be treated conservatively, causing the perirenal hematoma to resolve spontaneously. The major causes of severe postoperative bleeding are pseudoaneurysms, arteriovenous fistula, and segmental arterial injury. Typically, the first choice of treatment to manage severe bleeding complications is selective angioembolization (SAE) because of the very high success rate associated with this procedure. Case presentation: This clinical case involves a 56-year-old man who underwent dual-channel PCNL treatment after diagnosing a left kidney staghorn stone and urinary tract infection. The operation was successful, with no apparent signs of bleeding. Tests revealed continued decreasing hemoglobin levels following the procedure. After the conservative treatment failed, renal angiography was performed immediately, indicating renal pelvis mucosal artery hemorrhage. In the three hours post-surgery, the SAE still failed to prevent bleeding. Further discussions led to formulating a new surgical plan using a nephroscope to enter the initial channel where hemostasis began. The hemostasis origin was found precisely in the mucosal artery next to the channel during the operation and was successfully controlled. Conclusions: This case reveals there is poor communication and inadequate discussions about the potential failures of an SAE procedure. Swift clinical decision-making is imperative when dealing with high-level renal trauma to prevent delays in surgery that can threaten the safety of patients.

Abstract:BACKGROUND Acute non-variceal upper gastrointestinal bleeding (ANVUGIB) constitutes a prevalent emergency within Gastroenterology, encompassing 80%-90% of all gastrointestinal hemorrhage incidents. This condition is distinguished by its abrupt onset, swift progression, and notably elevated mortality rate. AIM To gather clinical data from patients with ANVUGIB at our hospital in order to elucidate the clinical characteristics specific to our institution and analyze the therapeutic effectiveness of endoscopic hemostasis. METHODS We retrospectively retrieved the records of 532 patients diagnosed with ANVUGIB by endoscopy at our hospital between March 2021 and March 2023, utilizing our medical record system. Data pertaining to general patient information, etiological factors, disease outcomes, and other relevant variables were meticulously collected and analyzed. RESULTS Among the 532 patients diagnosed with ANVUGIB, the male-to-female ratio was 2.91:1, with a higher prevalence among males. Notably, 43.6% of patients presented with black stool as their primary complaint, while 27.4% had hematemesis as their initial symptom. Upon admission, 17% of patients exhibited both hematemesis and black stool, while most ANVUGIB patients primarily complained of overt gastrointestinal bleeding. Urgent routine blood examinations at admission revealed that 75.8% of patients had anemia, with 63.4% experiencing moderate to severe anemia, and 1.5% having extremely severe anemia (hemoglobin < 30 g/L). With regard to etiology, 53.2% of patients experienced bleeding without a definitive trigger, 24.2% had a history of using gastric mucosa-irritating medications, 24.2% developed bleeding after alcohol consumption, 2.8% attributed it to improper diet, 1.7% to emotional excitement, and 2.3% to fatigue preceding the bleeding episode. Drug-induced ANVUGIB was more prevalent in the elderly than middle-aged and young individuals, while bleeding due to alcohol consumption showed the opposite trend. Additionally, diet-related bleeding was more common among the young age group compared to the middle-aged group. Gastrointestinal endoscopy identified peptic ulcers as the most frequent cause of ANVUGIB (73.3%), followed by gastrointestinal malignancies (10.9%), acute gastric mucous lesions (9.8%), and androgenic upper gastrointestinal bleeding (1.5%) among inpatients with ANVUGIB. Of the 532 patients with gastrointestinal bleeding, 68 underwent endoscopic hemostasis, resulting in an endoscopic treatment rate of 12.8%, with a high immediate hemostasis success rate of 94.1%. CONCLUSION ANVUGIB patients exhibit diverse characteristics across different age groups, and endoscopic hemostatic treatments have demonstrated remarkable efficacy.
