Abstract:PurposeThe automatic detection of pulmonary nodules using CT scans improves the efficiency of lung cancer diagnosis, and false-positive reduction plays a significant role in the detection. In this paper, we focus on the false-positive reduction task and propose an effective method for this task. MethodsWe construct a deep 3D residual CNN (convolution neural network) to reduce false-positive nodules from candidate nodules. The proposed network is much deeper than the traditional 3D CNNs used in medical image processing. Specifically, in the network, we design a spatial pooling and cropping (SPC) layer to extract multilevel contextual information of CT data. Moreover, we employ an online hard sample selection strategy in the training process to make the network better fit hard samples (e.g., nodules with irregular shapes). ResultsOur method is evaluated on 888 CT scans from the dataset of the LUNA16 Challenge. The free-response receiver operating characteristic (FROC) curve shows that the proposed method achieves a high detection performance. ConclusionsOur experiments confirm that our method is robust and that the SPC layer helps increase the prediction accuracy. Additionally, the proposed method can easily be extended to other 3D object detection tasks in medical image processing.
Multiclassifier Fusion Based on Radiomics Features for the Prediction of Benign and Malignant Primary Pulmonary Solid Nodules

Abstract:Background: TO test the ability of a multiclassifier model based on radiomics features to predict benign and malignant primary pulmonary solid nodules. Methods: Computed tomography (CT) images of 342 patients with primary pulmonary solid nodules confirmed by histopathology or follow-up were retrospectively analyzed. The region of interest (ROI) of the images was delineated, and the radiomics features of the lesions were extracted. The feature weight was calculated using the relief feature selection algorithm. Based on the selected features, five classifier models were constructed: support vector machine (SVA1), random forest (RF), logistic regression (LR), extreme learning machine (ELM), and K-nearest neighbor (KNN). The precision, recall rate, and area under the receiver operating characteristic curve (AUC) were used to evaluate the prediction performance of each classifier. The prediction result of each classifier was first weighted, and then all the prediction results were fused to predict the nodule type of unknown images. The prediction precision, recall rate, and AUC of the fusion classifier and single classifier were compared. Cross-validation was used to evaluate the generalization of the fusion classifier, and t- and F-tests were performed on the five classifiers and fusion classifier. Results: For each ROI, 450 features in four major categories were extracted and were analyzed using the relief feature selection algorithm. According to the weights, 25 highly repetitive and nonredundant stable features that played a major role in pulmonary nodule classification were selected. The fusion classifier's prediction performance (prediction precision =92.0%, AUC =0.915) was superior to those of SVM (prediction precision =75.3%, AUC =0.740), RF (prediction precision =89.1%, AUC =0.855), 1,R (prediction precision =68.4%, AUC =0.681), ELM (prediction precision =87.0%, AUC =0.830), and KNN (prediction precision =77.1%, AUC =0.702). The fusion classifier showed the best null hypothesis performance in the 1-test (P=0.035) and F-test (P=0.036). Conclusions: The multiclassifier fusion model based on radiomics features had high prediction value for benign and malignant primary pulmonary solid nodules.
A Hybrid CNN Feature Model for Pulmonary Nodule Malignancy Risk Differentiation.

Abstract:The malignancy risk differentiation of pulmonary nodule is one of the most challenge tasks of computer-aided diagnosis (CADx). Most recently reported CADx methods or schemes based on texture and shape estimation have shown relatively satisfactory on differentiating the risk level of malignancy among the nodules detected in lung cancer screening. However, the existing CADx schemes tend to detect and analyze characteristics of pulmonary nodules from a statistical perspective according to local features only. Enlightened by the currently prevailing learning ability of convolutional neural network (CNN), which simulates human neural network for target recognition and our previously research on texture features, we present a hybrid model that takes into consideration of both global and local features for pulmonary nodule differentiation using the largest public database founded by the Lung Image Database Consortium and Image Database Resource Initiative (LIDC-IDRI). By comparing three types of CNN models in which two of them were newly proposed by us, we observed that the multi-channel CNN model yielded the best discrimination in capacity of differentiating malignancy risk of the nodules based on the projection of distributions of extracted features. Moreover, CADx scheme using the new multi-channel CNN model outperformed our previously developed CADx scheme using the 3D texture feature analysis method, which increased the computed area under a receiver operating characteristic curve (AUC) from 0.9441 to 0.9702.
MHSnet: Multi-head and Spatial Attention Network with False-Positive Reduction for Pulmonary Nodules Detection

Abstract:The mortality of lung cancer has ranked high among cancers for many years. Early detection of lung cancer is critical for disease prevention, cure, and mortality rate reduction. However, existing detection methods on pulmonary nodules introduce an excessive number of false positive proposals in order to achieve high sensitivity, which is not practical in clinical situations. In this paper, we propose the multi-head detection and spatial squeeze-and-attention network, MHSnet, to detect pulmonary nodules, in order to aid doctors in the early diagnosis of lung cancers. Specifically, we first introduce multi-head detectors and skip connections to customize for the variety of nodules in sizes, shapes and types and capture multi-scale features. Then, we implement a spatial attention module to enable the network to focus on different regions differently inspired by how experienced clinicians screen CT images, which results in fewer false positive proposals. Lastly, we present a lightweight but effective false positive reduction module with the Linear Regression model to cut down the number of false positive proposals, without any constraints on the front network. Extensive experimental results compared with the state-of-the-art models have shown the superiority of the MHSnet in terms of the average FROC, sensitivity and especially false discovery rate (2.98% and 2.18% improvement in terms of average FROC and sensitivity, 5.62% and 28.33% decrease in terms of false discovery rate and average candidates per scan). The false positive reduction module significantly decreases the average number of candidates generated per scan by 68.11% and the false discovery rate by 13.48%, which is promising to reduce distracted proposals for the downstream tasks based on the detection results.

Abstract:Abstract Pulmonary nodule false-positive reduction is of great significance for automated nodule detection in clinical diagnosis of low-dose computed tomography (LDCT) lung cancer screening. Due to individual intra-nodule variations and visual similarities between true nodules and false positives as soft tissues in LDCT images, the current clinical practices remain subject to shortcomings of potential high-risk and time-consumption issues. In this paper, we propose a multi-dimensional nodule detection network (MD-NDNet) for automatic nodule false-positive reduction using deep convolutional neural network (DCNNs). The underlying method collaboratively integrates multi-dimensional nodule information to complementarily and comprehensively extract nodule inter-plane volumetric correlation features using three-dimensional CNNs (3D CNNs) and spatial nodule correlation features from sagittal, coronal, and axial planes using two-dimensional CNNs (2D CNNs) with attention module. To incorporate different sizes and shapes of nodule candidates, a multi-scale ensemble strategy is employed for probability aggregation with weights. The proposed method is evaluated on the LUNA16 challenge dataset in ISBI 2016 with ten-fold cross-validation. Experiment results show that the proposed framework achieves classification performance with a CPM score of 0.9008. All of these indicate that our method enables an efficient, accurate and reliable pulmonary nodule detection for clinical diagnosis.

Abstract:Lung cancer is a global and dangerous disease, and its early detection is crucial for reducing the risks of mortality. In this regard, it has been of great interest in developing a computer-aided system for pulmonary nodules detection as early as possible on thoracic CT scans. In general, a nodule detection system involves two steps: (i) candidate nodule detection at a high sensitivity, which captures many false positives and (ii) false positive reduction from candidates. However, due to the high variation of nodule morphological characteristics and the possibility of mistaking them for neighboring organs, candidate nodule detection remains a challenge. In this study, we propose a novel Multi-scale Gradual Integration Convolutional Neural Network (MGI-CNN), designed with three main strategies: (1) to use multi-scale inputs with different levels of contextual information, (2) to use abstract information inherent in different input scales with gradual integration, and (3) to learn multi-stream feature integration in an end-to-end manner. To verify the efficacy of the proposed network, we conducted exhaustive experiments on the LUNA16 challenge datasets by comparing the performance of the proposed method with state-of-the-art methods in the literature. On two candidate subsets of the LUNA16 dataset, i.e., V1 and V2, our method achieved an average CPM of 0.908 (V1) and 0.942 (V2), outperforming comparable methods by a large margin. Our MGI-CNN is implemented in Python using TensorFlow and the source code is available from https://github.com/ku-milab/MGICNN.

Abstract:In this article, a 3-D detection framework for detecting nonsmall cell lung cancer (NSCLC) in 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) images and guided by a multimodality attention fusion is proposed. A total of 250 18F-FDG PET/CT scans between January 1, 2015 and December 31, 2019 of patients who had histopathologically proven NSCLC were acquired. A customized dual-path 3-D CenterNet is used for NSCLC detection. Moreover, we propose a multimodality attention module that adaptively refines the multimodality feature map fusion. Since the 3-D convolutional neural network (CNN) requires many graphic memories and sliding windows, a $384\times 384\times 32$ patch size is designed and used for training and testing. Fivefold cross-validation is applied for this study. The sensitivity and false positive per scan (FPPS) obtained by our proposed method are 0.96 and 1.04, respectively. Our method significantly outperforms 3-D CenterNet in terms of sensitivity $(P = 0.031)$ . This module demonstrates the potential to be implemented in other multimodality applications. Our result performs competitively against other lung cancer detections. Furthermore, case studies show that the proposed method can detect difficult-to-diagnose NSCLCs. Our result shows the proposed method can help radiologists and medical physicists diagnose NSCLC.
Postpartum Mental Health Promotion: Perspectives from Mothers and Home Visitors.

Abstract:OBJECTIVE The object of this study was to examine the implementation of the Towards Flourishing Mental Health Promotion Strategy, a demonstration project designed to promote the mental well-being of parents and their children that was added to an existing public health home visiting program. DESIGN AND SAMPLE Structured interviews were conducted with program stakeholders including 13 women receiving home visiting services in the postpartum period and 6 home visitors. MEASURES Thematic analysis of individual transcripts was conducted and results were compiled according to common themes. RESULTS The results indicate that women and home visitors perceived the integration of a mental health promotion strategy into an existing public health program as feasible, acceptable and useful. The strategy provides a mechanism for women and home visitors to dialog about mental health and appears to have early positive impacts on the women. Factors that facilitated and impeded the successful implementation of the strategy are described. CONCLUSION These results point to promising strategies to reach women early in the postpartum period to support their mental health. They also shed light on the barriers to supporting mental health, indicating the need to address stigma related to mental health and the social determinants of health.

Abstract:OBJECTIVE:The aim of this paper is to describe a novel algorithm for False Positive Reduction in lung nodule Computer Aided Detection(CAD).METHODS:In this paper, we describes a new CT lung CAD method which aims to detect solid nodules. Specially, we proposed a multi-kernel classifier with a ℓ2, 1 norm regularizer for heterogeneous feature fusion and selection from the feature subset level, and designed two efficient strategies to optimize the parameters of kernel weights in non-smooth ℓ2, 1 regularized multiple kernel learning algorithm. The first optimization algorithm adapts a proximal gradient method for solving the ℓ2, 1 norm of kernel weights, and use an accelerated method based on FISTA; the second one employs an iterative scheme based on an approximate gradient descent method.RESULTS:The results demonstrates that the FISTA-style accelerated proximal descent method is efficient for the ℓ2, 1 norm formulation of multiple kernel learning with the theoretical guarantee of the convergence rate. Moreover, the experimental results demonstrate the effectiveness of the proposed methods in terms of Geometric mean (G-mean) and Area under the ROC curve (AUC), and significantly outperforms the competing methods.CONCLUSIONS:The proposed approach exhibits some remarkable advantages both in heterogeneous feature subsets fusion and classification phases. Compared with the fusion strategies of feature-level and decision level, the proposed ℓ2, 1 norm multi-kernel learning algorithm is able to accurately fuse the complementary and heterogeneous feature sets, and automatically prune the irrelevant and redundant feature subsets to form a more discriminative feature set, leading a promising classification performance. Moreover, the proposed algorithm consistently outperforms the comparable classification approaches in the literature.

Abstract:Abstract The detection of pulmonary nodules has a low accuracy due to the various shapes and sizes of pulmonary nodules. In this paper, a multi-scale detection network for pulmonary nodules based on the attention mechanism is proposed to accurately predict pulmonary nodules. During data processing, the pseudo-color processing strategy is designed to enhance the gray image and introduce more contextual semantic information. In the feature extraction network section, this paper designs a basic module of ResSCBlock integrating attention mechanism for feature extraction. At the same time, the feature pyramid structure is used for feature fusion in the network, and the problem of the detection of small-size nodules which are easily lost is solved by multi-scale prediction method. The proposed method is tested on the LUNA16 data set, with an 83% mAP value. Compared with other detection networks, the proposed method achieves an improvement in detecting pulmonary nodules.

Although many deep learning-based algorithms make great progress for improving the accuracy of nodule detection, the high false positive rate is still a challenging problem which limited the automatic diagnosis in routine clinical practice. In this paper, we propose a novel pulmonary nodule detection framework based on a 3D Feature Pyramid Network (3DFPN) to improve the sensitivity of nodule detection by employing multi-scale features to increase the resolution of nodules, as well as a parallel top-down path to transit the high-level semantic features to complement low-level general features. Furthermore, a High Sensitivity and Specificity (HS$^2$) network is introduced to eliminate the falsely detected nodule candidates by tracking the appearance changes in continuous CT slices of each nodule candidate. The proposed framework is evaluated on the public Lung Nodule Analysis (LUNA16) challenge dataset. Our method is able to accurately detect lung nodules at high sensitivity and specificity and achieves $90.4\%$ sensitivity with 1/8 false positive per scan which outperforms the state-of-the-art results $15.6\%$.
A Lower False Positive Pulmonary Nodule Detection Approach for Early Lung Cancer Screening

Abstract:Pulmonary nodule detection with low-dose computed tomography (LDCT) is indispensable in early lung cancer screening. Although existing methods have achieved excellent detection sensitivity, nodule detection still faces challenges such as nodule size variation and uneven distribution, as well as excessive nodule-like false positive candidates in the detection results. We propose a novel two-stage nodule detection (TSND) method. In the first stage, a multi-scale feature detection network (MSFD-Net) is designed to generate nodule candidates. This includes a proposed feature extraction network to learn the multi-scale feature representation of candidates. In the second stage, a candidate scoring network (CS-Net) is built to estimate the score of candidate patches to realize false positive reduction (FPR). Finally, we develop an end-to-end nodule computer-aided detection (CAD) system based on the proposed TSND for LDCT scans. Experimental results on the LUNA16 dataset show that our proposed TSND obtained an excellent average sensitivity of 90.59% at seven predefined false positives (FPs) points: 0.125, 0.25, 0.5, 1, 2, 4, and 8 FPs per scan on the FROC curve introduced in LUNA16. Moreover, comparative experiments indicate that our CS-Net can effectively suppress false positives and improve the detection performance of TSND.

Moreover, the CT scans collected from multiple manufacturers may affect the robustness of Computer-aided diagnosis (CAD) due to the differences in intensity scales and machine noises. In this paper, we propose a novel self-supervised learning assisted pulmonary nodule detection framework based on a 3D Feature Pyramid Network (3DFPN) to improve the sensitivity of nodule detection by employing multi-scale features to increase the resolution of nodules, as well as a parallel top-down path to transit the high-level semantic features to complement low-level general features. Furthermore, a High Sensitivity and Specificity (HS2) network is introduced to eliminate the false positive nodule candidates by tracking the appearance changes in continuous CT slices of each nodule candidate on Location History Images (LHI). In addition, in order to improve the performance consistency of the proposed framework across data captured by different CT scanners without using additional annotations, an effective self-supervised learning schema is applied to learn spatiotemporal features of CT scans from large-scale unlabeled data. The performance and robustness of our method are evaluated on several publicly available datasets with significant performance improvements. The proposed framework is able to accurately detect pulmonary nodules with high sensitivity and specificity and achieves 90.6% sensitivity with 1/8 false positive per scan which outperforms the state-of-the-art results 15.8% on LUNA16 dataset.
An improved 3-D attention CNN with hybrid loss and feature fusion for pulmonary nodule classification

Abstract:Background and objective: Lung cancer has the highest cancer-related mortality worldwide, and lung nodule usually presents with no symptom. Low-dose computed tomography (LDCT) was an important tool for lung cancer detection and diagnosis. It provided a complete three-dimensional (3-D) chest image with a high resolution.Recently, convolutional neural network (CNN) had flourished and been proven the CNN-based computer-aided diagnosis (CADx) system could extract the features and help radiologists to make a preliminary diagnosis. Therefore, a 3-D ResNeXt-based CADx system was proposed to assist radiologists for diagnosis in this study. Methods: The proposed CADx system consists of image preprocessing and a 3-D CNN-based classification model for pulmonary nodule classification. First, the image preprocessing was executed to generate the normalized volumn of interest (VOI) only including nodule information and a few surrounding tissues. Then, the extracted VOI was forwarded to the 3-D nodule classification model. In the classification model, the RestNext was employed as the backbone and the attention scheme was embedded to focus on the important features. Moreover, a multi-level feature fusion network incorporating feature information of different scales was used to enhance the prediction accuracy of small malignant nodules. Finally, a hybrid loss based on channel optimization which make the network learn more detailed information was empolyed to replace a binary cross-entropy (BCE) loss. Results: In this research, there were a total of 880 low-dose CT images including 440 benign and 440 malignant nodules from the American National Lung Screening Trial (NLST) for system evaluation. The results showed that our system could achieve the accuracy of 85.3%, the sensitivity of 86.8%, the specificity of 83.9%, and the area-under-curve (AUC) value was 0.9042. It was confirmed that the designed system had a good diagnostic ability. Conclusion: In this study, a CADx composed of the image preprocessing and a 3-D nodule classification model with attention scheme, feature fusion, and hybrid loss was proposed for pulmonary nodule classification in LDCT. The results indicated that the proposed CADx system had potential for achieving high performance in classifying lung nodules as benign and malignant.
Automated Pulmonary Nodule Detection: High Sensitivity with Few Candidates.

Abstract:Automated pulmonary nodule detection plays an important role in lung cancer diagnosis. In this paper, we propose a pulmonary detection framework that can achieve high sensitivity with few candidates. First, the Feature Pyramid Network (FPN), which leverages multi-level features, is applied to detect nodule candidates that cover almost all true positives. Then redundant candidates are removed by a simple but effective Conditional 3-Dimensional Non-Maximum Suppression (Conditional 3D-NMS). Moreover, a novel Attention 3D CNN (Attention 3D-CNN) which efficiently utilizes contextual information is proposed to further remove the overwhelming majority of false positives. The proposed method yields a sensitivity of 95.8% at 2 false positives per scan on the LUng Nodule Analysis 2016 (LUNA16) dataset, which is competitive compared to the current published state-of-the-art methods.

Abstract:Detecting and accurately identifying malignant lung nodules in chest CT scans in a timely manner is crucial for effective lung cancer treatment. This study introduces a deep learning model featuring a multi-channel attention mechanism, specifically designed for the precise diagnosis of malignant lung nodules. To start, we standardized the voxel size of CT images and generated three RGB images of varying scales for each lung nodule, viewed from three different angles. Subsequently, we applied three attention submodels to extract class-specific characteristics from these RGB images. Finally, the nodule features were consolidated in the model's final layer to make the ultimate predictions. Through the utilization of an attention mechanism, we could dynamically pinpoint the exact location of lung nodules in the images without the need for prior segmentation. This proposed approach enhances the accuracy and efficiency of lung nodule classification. We evaluated and tested our model using a dataset of 1018 CT scans sourced from the Lung Image Database Consortium and Image Database Resource Initiative (LIDC-IDRI). The experimental results demonstrate that our model achieved a lung nodule classification accuracy of 90.11 %, with an area under the receiver operator curve (AUC) score of 95.66 %. Impressively, our method achieved this high level of performance while utilizing only 29.09 % of the time needed by the mainstream model.
Lung-YOLO: Multiscale Feature Fusion Attention and Cross-Layer Aggregation for Lung Nodule Detection

Abstract:Objective: Lung cancer is a significant public health problem worldwide, and its mortality and morbidity rates are among the highest of cancers. At the same time, early diagnosis of nodules can significantly improve the survival rate of patients. Therefore, this paper proposes the Lung-YOLO algorithm for lung CT image detection based on YOLOv6. Methods: First, to enable the network to detect nodules of different sizes and to minimize missed detections, we introduce the Multi-scale Dual-branch Attention (MSDA) mechanism in the feature extraction part of the network. The input features undergo continual dilation convolutions, effectively establish remote dependencies, and the fused multiscale contextual information expands the receptive field while enhancing the model's ability to detect targets of different sizes, which not only contains precise category and location information but also enables the allocation of attentional weights to generate more pixel-level attention. Then, the fused features access the dual-branch attention module to shift the model's attention to the target nodules, and the composed dual-branch structure captures cross-dimensional interactions and realizes inter-dimensional dependencies, effectively improving the detection performance. Second, during the feature transfer process, the original Bidirectional Feature Pyramid structure (RepBiFPAN) suffers from the loss of detailed information such as texture and color, making it challenging to localize target nodules accurately. To address this, we propose the Cross-layer Aggregation Module (CLAM), by cross-layer aggregating the multi-level feature layer of the backbone with the multi-level detection layer of the head, which preserves the multi-level fine-grained information that may be lost during the feature transfer process, which is crucial for the detection of small targets. Finally, the module proposed in this paper can be easily incorporated into any detection framework for plug-and-play. Results: Our method achieves accuracy, precision, recall, and mAP of 97.5 %, 96.5 %, 96.9 %, and 97.9 % on the LUNA16 dataset, and 95.1 %, 94.3 %, 93.4 %, and 95.9 % on the LIDC-IDRI dataset, respectively, surpassing many existing state-of-the-art detection methods. Moreover, the inference speed is 22.8 ms and 28.4 ms per image with 30.6 M parameters, respectively.
An End-to-End Framework for Integrated Pulmonary Nodule Detection and False Positive Reduction.

Abstract:Pulmonary nodule detection using low-dose Computed Tomography (CT) is often the first step in lung disease screening and diagnosis. Recently, algorithms based on deep convolutional neural nets have shown great promise for automated nodule detection. Most of the existing deep learning nodule detection systems are constructed in two steps: a) nodule candidates screening and b) false positive reduction, using two different models trained separately. Although it is commonly adopted, the two-step approach not only imposes significant resource overhead on training two independent deep learning models, but also is sub-optimal because it prevents crosstalk between the two. In this work, we present an end-to-end framework for nodule detection, integrating nodule candidate screening and false positive reduction into one model, trained jointly. We demonstrate that the end-to-end system improves the performance by 3.88% over the two-step approach, while at the same time reducing model complexity by one third and cutting inference time by 3.6 fold. Abstract:Lung cancer is one of the leading causes of cancer-related death worldwide. Early diagnosis can effectively reduce the mortality, and computer-aided diagnosis (CAD) as an important way to assist doctors has developed rapidly. In particular, automated pulmonary nodule detection in computed tomography (CT) images is crucial to CAD. It is a challenging task to quickly locate the exact positions of lung nodules. In this paper, a novel automated pulmonary nodule detection framework with 2D convolutional neural network (CNN) is proposed to assist the CT reading process. Firstly, we adjust the structure of Faster R-CNN with two region proposal networks and a deconvolutional layer to detect nodule candidates, and then three models are trained for three kinds of slices for later result fusion. Secondly, a boosting architecture based on 2D CNN is designed for false positive reduction, which is a classifier to distinguish true nodules from the candidates. The misclassified samples are still kept for retraining a model which boosts the sensitivity for pulmonary nodule detection. Finally, the results of these networks are fused to vote out the final classification results. Extensive experiments are conducted on LUNA16, and the sensitivity of nodule candidate detection achieves 86.42%. For the false positive reduction, the sensitivity reaches 73.4% and 74.4% at 1/8 and 1/4 FPs/scan, respectively. It illustrates that the proposed method can obviously achieve accurate pulmonary nodule detection.
