Abstract:Medical health care centers are envisioned as a promising paradigm to handle the massive volume of data of COVID-19 patients using artificial intelligence (AI). Traditionally, AI techniques often require centralized data collection and training the model in a single organization, which is most common weakness due to the privacy and security of raw data communication. To solve this challenging task, we propose a blockchain-based federated learning framework that provides collaborative data training solutions by coordinating multiple hospitals to train and share encrypted federated models without leakage of data privacy. The blockchain ledger technology provides the decentralization of federated learning model without any central server. The proposed homomorphic encryption scheme encrypts and decrypts the gradients of model to preserve the privacy. More precisely, the proposed framework: i) train the local model by a novel capsule network to segmentation and classify COVID-19 images, ii) then use the homomorphic encryption scheme to secure the local model that encrypts and decrypts the gradients, and finally the model is shared over a decentralized platform through proposed blockchain-based federated learning algorithm. The integration of blockchain and federated learning leads to a new paradigm for medical image data sharing in the decentralized network. The conducted experimental resultsdemonstrate the performance of the proposed scheme.

Abstract:Blockchain-based Federated Learning (BCFL) is gaining significant attention as a promising decentralized data sharing technology with privacy protection. Most existing BCFL frameworks loosely couple blockchain and Federated Learning (FL). FL transforms data sharing into model sharing, while blockchain decentralizes the model aggregation and handles security verification. However, this simplistic overlay of the two technologies often involves third-party blockchain peers, leading to inefficient workflows and potential privacy attacks from malicious blockchain peers. Some studies have proposed differential privacy with noise addition to prevent privacy attacks, yet most do not allow clients to customize privacy budgets, impacting data availability and limiting BCFL’s application in data sharing. To address these challenges, we first introduce a novel tightly-coupled BCFL framework that integrates training and mining at the client side. Under this framework, a totally decentralized data sharing process is established. Moreover, a Personalised Differential Privacy (PDP) mechanism is devised, enabling clients to add Laplace noise to model gradients based on custom privacy budgets. To achieve optimal privacy budgets, a privacy optimization mechanism based on Stackelberg games is proposed. It establishes utility functions for data requesters and providers, models the process of utility optimization as a Stackelberg game process, and obtains the optimal privacy budget while maximizing utility for all participants. This facilitates flexible and on-demand privacy-protected data sharing. Extensive experiments validate the effectiveness of our approach in enhancing system efficiency and facilitating flexible on-demand privacy-protected data sharing, further solidifying BCFL’s potential in decentralized data sharing scenarios.

Abstract:COVID-19 is a serious epidemic that not only endangers human health, but also wreaks havoc on the development of society. Recently, there has been research on using artificial intelligence (AI) techniques for COVID-19 detection. As AI has entered the era of big models, deep learning methods based on pre-trained models (PTMs) have become a focus of industrial applications. Federated learning (FL) enables the union of geographically isolated data, which can address the demands of big data for PTMs. However, the incompleteness of the healthcare system and the untrusted distribution of medical data make FL participants unreliable, and medical data also has strong privacy protection requirements. Our research aims to improve training efficiency and global model accuracy using PTMs for training in FL, reducing computation and communication. Meanwhile, we provide a secure aggregation rule using differential privacy and fully homomorphic encryption to achieve a privacy-preserving Byzantine robust federal learning scheme. In addition, we use blockchain to record the training process and we integrate a Byzantine fault tolerance consensus to further improve robustness. Finally, we conduct experiments on a publicly available dataset, and the experimental results show that our scheme is effective with privacy-preserving and robustness. The final trained models achieve better performance on the positive prediction and severe prediction tasks, with an accuracy of 85.00% and 85.06%, respectively. Thus, this indicates that our study is able to provide reliable results for COVID-19 detection.

Abstract:Federated learning-based medical data privacy sharing can promote the development of medical industry intelligence, but limited by its own security and privacy deficiencies, federated learning still suffers from a single point of failure and privacy leakage of intermediate parameters. To address these problems, this paper proposes a privacy protection framework for medical data based on blockchain and cross-silo federated learning, using cross-silo federated learning to establish a collaborative training platform for multiple medical institutions to enhance the privacy of medical data, introducing blockchain and smart contracts to realize decentralized federated learning to enhance trust between distrustful medical institutions and solve the problem of a single point of failure. In addition, a secure aggregation scheme is designed using threshold homomorphic encryption to prevent the privacy leakage problem during parameter transmission. The experimental and analytical results show that the accuracy of this paper's scheme is consistent with the original federated learning scheme, effectively deals with the problems of single-point failure and inference attacks of federated learning, improves system robustness, and is suitable for medical scenarios with more stringent requirements on security and accuracy.

Abstract:Improved data sharing between healthcare providers can lead to a higher probability of accurate diagnosis, more effective treatments, and enhanced capabilities of healthcare organizations. One critical area of focus is brain tumor segmentation, a complex task due to the heterogeneous appearance, irregular shape, and variable location of tumors. Accurate segmentation is essential for proper diagnosis and effective treatment planning, yet current techniques often fall short due to these complexities. However, the sensitive nature of health data often prohibits its sharing. Moreover, the healthcare industry faces significant issues, including preserving the privacy of the model and instilling trust in the model. This paper proposes a framework to address these privacy and trust issues by introducing a mechanism for training the global model using federated learning and sharing the encrypted learned parameters via a permissioned blockchain. The blockchain-federated learning algorithm we designed aggregates gradients in the permissioned blockchain to decentralize the global model, while the introduced masking approach retains the privacy of the model parameters. Unlike traditional raw data sharing, this approach enables hospitals or medical research centers to contribute to a globally learned model, thereby enhancing the performance of the central model for all participating medical entities. As a result, the global model can learn about several specific diseases and benefit each contributor with new disease diagnosis tasks, leading to improved treatment options. The proposed algorithm ensures the quality of model data when aggregating the local model, using an asynchronous federated learning procedure to evaluate the shared model's quality. The experimental results demonstrate the efficacy of the proposed scheme for the critical and challenging task of brain tumor segmentation. Specifically, our method achieved a 1.99% improvement in Dice similarity coefficient for enhancing tumors and a 19.08% reduction in Hausdorff distance for whole tumors compared to the baseline methods, highlighting the significant advancement in segmentation performance and reliability.

Abstract:Federated Learning (FL) is a machine learning methodology that allows remote devices to collectively train a learning system without sharing their data. FL-based methods provide enhanced secure privacy by transmitting only localized model variables, learned with local information, from dispersed devices to a centralized controller. There is a potential for a centralized server or malicious individuals to deduce or get sensitive private data by analyzing the structure and variables of regional learning networks. This study incorporates the FL process into the deep learning process of medical prototypes in an Internet of Things (IoT)-based medical facility. A Secured Medical Homomorphic Encryption Algorithm (SMHEA) is proposed in this research to ensure medical data privacy. Cryptographic primitives, such as masking and homomorphic cryptography, are used to enhance the security of local modeling. This prevents adversaries from deducing confidential health information via assaults like model restoration or modeling inversion. The primary determinant for assessing the regional modeling's contributions to the universal model during each training stage is the quality of the databases possessed by various individuals rather than the typically used metric of database dimension in deep learning. A dropout-tolerant approach is suggested, where the FL procedure would continue as long as the total amount of online customers remains over a certain level. By doing a security evaluation, it is evident that the suggested approach effectively ensures data privacy. Theoretical analysis is conducted on accuracy, computation time, and communication error. An example of clinical applications is the categorization of skin lesions using training photos from the HAM10000 medical database. The experimental findings demonstrate that the suggested system had favorable performance and privacy preservation outcomes compared to current methods.

However, a crucial challenge in training such models is the acquisition of vast amounts of data, which is often limited in fields like healthcare. In this domain, medical data is typically scattered across various sources such as hospitals, clinics, and wearable devices. The aggregated data collected from multiple sources in the healthcare domain is sufficient for training advanced deep learning models. However, these sources are frequently hesitant to share such data due to privacy considerations. To address this challenge, researchers have proposed the integration of blockchain and federated learning to develop a system that facilitates the secure sharing of medical records. Abstract:The rapid growth of the Internet of Medical Things (IoMT) has introduced significant security and privacy challenges in managing and protecting medical data. This paper proposes a secure federated cloud storage system designed to address these challenges using a hybrid heuristic attribute-based encryption (ABE) scheme integrated with a permissioned Blockchain. The proposed system enhances data confidentiality and integrity by first collecting medical information and then encrypting it with ABE using an optimal key generated by the Hybrid Mexican Axolotl with Energy Valley Optimizer (HMO-EVO). The encrypted data is securely stored in a permissioned blockchain, ensuring robust access control and protection against data breaches. For effective healthcare monitoring, the system employs federated learning with a Multi-scale Bi-Long Short-Term Memory and Gated Recurrent Unit (MBiLSTM-GRU) to predict diseases accurately. This federated approach allows for decentralized training of deep learning models, preserving patient data privacy while leveraging collective learning. Experimental results show that the proposed system outperforms conventional methods in terms of security, efficiency, and predictive accuracy. This research offers a comprehensive framework for secure medical data management, combining the strengths of federated learning and blockchain technology to address the critical issues of data ownership, regulatory compliance, and privacy in IoMT networks.

Abstract:Slow transit constipation (STC) is one kind of chronic constipation. Medical treatment is adopted by most patients, but many patients do not respond to conservative treatment and require surgical intervention. There are several problems in clinical paratice, including non-rigorous patient selection, less standard preoperative assessment, less standard operation or insufficient post operative treatments and follow-up, which may lead to the overtreatment or serious complications. As the changes of living habits and diet, the incidence of STC increases gradually. Therefore it is important to make a standardized surgical treatment of STC. Because lots of factors may cause STC and the pathophysiology of STC is complicated, multidisciplinary treatment is recommended. This paper will discuss the standardized surgical treatment of STC base on the new literature and author's experience, concerning operative indications, preoperative assessment, choice of operation procedure, postoperative evaluation and follow-up.

Abstract:This paper proposes Federated Learning (FL) based smart healthcare system where Medical Centers (MCs) train the local model using the data collected from patients and send the model weights to the miners in a blockchain-based robust framework without sharing raw data, keeping privacy preservation into deliberation. We formulate an optimization problem by maximizing the utility and minimizing the loss function considering energy consumption and FL process delay of MCs for learning effective models on distributed healthcare data underlying a blockchain-based framework. We propose a solution in two stages: first, offer a stable matching-based association algorithm to maximize the utility of both miners and MCs and then solve loss minimization using Stochastic Gradient Descent (SGD) algorithm employing FL under Differential Privacy (DP) and blockchain technology. Moreover, we incorporate blockchain technology to provide tempered resistant and decentralized model weight sharing in the proposed FL-based framework. The effectiveness of the proposed model is shown through simulation on real-world healthcare data comparing other state-of-the-art techniques.

Abstract:Medical image fusion generates a fused image containing multiple features extracted from different source images, and it is of great help in clinical analysis and diagnosis. However, training a deep learning model for image fusion usually requires enormous computing power, especially for large volumes of medical data. Meanwhile, the privacy of images is also a critical issue. In this paper, we propose a privacy-preserving blockchain-based medical image fusion (BMIF) framework. First, to ensure fusion performance, we design a new medical image fusion model based on convolutional neural network and Inception network, and integrate the proposed model into the consensus process of blockchain. Next, to save computing power of blockchain, we design a consensus mechanism by requesting consensus nodes to train the fusion model instead of calculating useless hash values in traditional blockchain. Then, to protect data privacy, we further present an efficient homomorphic encryption to realize the training of fusion model on encrypted medical data. Finally, we conduct theoretical analysis and extensive experiments on public datasets to evaluate the feasibility and the performance of our proposed BMIF. The results exhibit that BMIF is efficient and secure, and our medical image fusion network performs better than state-of-the-art approaches.

Abstract:At these times, internet of things (IoT) technologies have become ubiquitous in the healthcare sector. Because of the increasing needs of IoT, massive quantity of patient data is being gathered and is utilized for diagnostic purposes. The recent developments of artificial intelligence (AI) and deep learning (DL) models are commonly employed to accurately identify the diseases in real-time scenarios. Despite the benefits, security, energy constraining, insufficient training data are the major issues which need to be resolved in the IoT enabled medical field. To accomplish the security, blockchain technology is recently developed which is a decentralized architecture that is widely utilized. With this motivation, this paper introduces a new blockchain with DL enabled secure medical data transmission and diagnosis (BDL-SMDTD) model. The goal of the BDL-SMDTD model is to securely transmit the medical images and diagnose the disease with maximum detection rate. The BDL-SMDTD model incorporates different stages of operations such as image acquisition, encryption, blockchain, and diagnostic process. Primarily, moth flame optimization (MFO) with elliptic curve cryptography (ECC), called MFO-ECC technique is used for the image encryption process where the optimal keys of ECC are generated using MFO algorithm. Besides, blockchain technology is utilized to store the encrypted images. Then, the diagnostic process involves histogram-based segmentation, Inception with ResNet-v2-based feature extraction, and support vector machine (SVM)-based classification. The experimental performance of the presented BDL-SMDTD technique has been validated using benchmark medical images and the resultant values highlighted the improved performance of the BDL-SMDTD technique. The proposed BDL-SMDTD model accomplished maximum classification performance with sensitivity of 96.94%, specificity of 98.36%, and accuracy of 95.29%, whereas the feature extraction is performed based on ResNet-v2

Abstract:Medical data is often highly sensitive in terms of data privacy and security concerns. Federated learning, one type of machine learning techniques, has been started to use for the improvement of the privacy and security of medical data. In the federated learning, the training data is distributed across multiple machines, and the learning process is performed in a collaborative manner. There are several privacy attacks on deep learning (DL) models to get the sensitive information by attackers. Therefore, the DL model itself should be protected from the adversarial attack, especially for applications using medical data. One of the solutions for this problem is homomorphic encryption-based model protection from the adversary collaborator. This paper proposes a privacy-preserving federated learning algorithm for medical data using homomorphic encryption. The proposed algorithm uses a secure multi-party computation protocol to protect the deep learning model from the adversaries. In this study, the proposed algorithm using a real-world medical dataset is evaluated in terms of the model performance.

Abstract:Securing Electronic Medical Records (EMRs) is one of the most critical applications of cryptography over the Internet due to the value and importance of data contained in such EMRs. Although blockchain-based healthcare systems can provide security, privacy, and immutability to EMRs, several outstanding security and latency issues are associated with existing schemes. For example, some researchers have used the blockchain as a storage tool which increases delay and adversely affects the blockchain performance since it stores a copy of each transaction. A distributed ledger also requires appropriate space and computational power with increased data size. In addition, existing healthcare-based approaches usually rely on centralized servers connected to clouds, which are vulnerable to denial of service (DoS), distributed DoS (DDoS), and collusion attacks. This paper proposes a novel hybrid-deep learning-based homomorphic encryption (HE) model for the Industrial Internet of Medical Things (IIoMT) to cope with such challenges using a consortium blockchain. Integrating HE with the proposed IIoMT system is a vital contribution of this work. The use of HE while outsourcing to the cloud the storage provides a unique facility to perform any statistical and machine learning operation on the encrypted EMR data, hence providing resistance to collusion and phishing attacks. Our proposed model uses a pre-trained hybrid deep learning model in the cloud and deploys the trained model into blockchain-based edge devices in order to classify and train local models using EMRs. This is further conditioned on the private data of each edge and IoT device connected with the consortium blockchain. All local models obtained are aggregated to the cloud to update a global model, which is finally disseminated to the edge nodes. Our proposed approach provides more privacy and security than conventional models and can deliver high efficiency and low end-to-end latency for users. Comparative simulation analysis with state-of-the-art approaches is carried out using benchmark performance metrics, which show that our proposed model provides enhanced security, efficiency, and transparency.

Abstract:Federated learning, leveraging distributed data from multiple nodes to train a common model, allows for the use of more data to improve the model while also protecting the privacy of original data. However, challenges still exist in ensuring privacy and security within the interactions. To address these issues, this paper proposes a federated learning approach that incorporates blockchain, homomorphic encryption, and reputation. Using homomorphic encryption, edge nodes possessing local data can complete the training of ciphertext models, with their contributions to the aggregation being evaluated by a reputation mechanism. Both models and reputations are documented and verified on the blockchain through consensus process, which then determines the rewards based on the incentive mechanism. This approach not only incentivizes participation in training, but also ensures the privacy of data and models through encryption. Additionally, it addresses security risks associated with both data and network attacks, ultimately leading to a highly accurate trained model. To enhance the efficiency of learning and the performance of the model, a joint adaptive aggregation and resource optimization algorithm is introduced. Finally, simulations and analyses demonstrate that the proposed scheme enhances learning accuracy while maintaining privacy and security.

The suggested framework integrates several innovative technologies to make healthcare systems safe and privacy-preserving. The framework leverages the blockchain system, a distributed and unchangeable ledger, to secure the integrity, traceability and openness of private medical information. Patient privacy is better protected as a result, and there is less chance of data breaches or unauthorized access. The system makes use of the Linear Discriminant Analysis (LDA), Decision Tree, Extra Tree Classifier, AdaBoost, and Federated Deep Extreme Machine Learning algorithms to increase the accuracy and efficacy of illness prediction. This method allows for collaborative learning across many healthcare organizations without disclosing raw data, protecting privacy. The system obtains a thorough awareness of patient health, allowing for the early diagnosis of diseases and the development of individualized treatment suggestions. To further detect and reduce possible security risks in the IoMT environment, the framework also includes intrusion detection methods. Protecting patient data and infrastructure, the system can quickly identify and react to unauthorized actions or threats. High accuracy and privacy protection are shown by the results, making it appropriate for Healthcare 5.0 applications. The findings have important ramifications for researchers, politicians, and healthcare professionals who are seeking to develop safe and privacy-conscious healthcare systems.

Abstract:The rapid advancements in technology have paved the way for innovative solutions in the healthcare domain, aiming to improve scalability and security while enhancing patient care. This abstract introduces a cutting-edge approach, leveraging blockchain technology and hybrid deep learning techniques to revolutionize healthcare systems. Blockchain technology provides a decentralized and transparent framework, enabling secure data storage, sharing, and access control. By integrating blockchain into healthcare systems, data integrity, privacy, and interoperability can be ensured while eliminating the reliance on centralized authorities. In conjunction with blockchain, hybrid deep learning techniques offer powerful capabilities for data analysis and decision making in healthcare. Combining the strengths of deep learning algorithms with traditional machine learning approaches, hybrid deep learning enables accurate and efficient processing of complex healthcare data, including medical records, images, and sensor data. This research proposes a permissions-based blockchain framework for scalable and secure healthcare systems, integrating hybrid deep learning models. The framework ensures that only authorized entities can access and modify sensitive health information, preserving patient privacy while facilitating seamless data sharing and collaboration among healthcare providers. Additionally, the hybrid deep learning models enable real-time analysis of large-scale healthcare data, facilitating timely diagnosis, treatment recommendations, and disease prediction. The integration of blockchain and hybrid deep learning presents numerous benefits, including enhanced scalability, improved security, interoperability, and informed decision making in healthcare systems. However, challenges such as computational complexity, regulatory compliance, and ethical considerations need to be addressed for successful implementation. Abstract:The advancement of Internet and Communication Technologies (ICTs) has led to the era of Industry 4.0. This shift is followed by healthcare industries creating the term Healthcare 4.0. In Healthcare 4.0, the use of IoT-enabled medical imaging devices for early disease detection has enabled medical practitioners to increase healthcare institutions' quality of service. However, Healthcare 4.0 is still lagging in Artificial Intelligence and big data compared to other Industry 4.0 due to data privacy concerns. In addition, institutions' diverse storage and computing capabilities restrict institutions from incorporating the same training model structure. This paper presents a secure multi-party computation-based ensemble federated learning with blockchain that enables heterogeneous models to collaboratively learn from healthcare institutions' data without violating users' privacy. Blockchain properties also allow the party to enjoy data integrity without trust in a centralized server while also providing each healthcare institution with auditability and version control capability.

Abstract:Data mining, integration, and utilization are the inevitable trend of the Internet of Medical Things (IoMT) in the context of big data. With the increasing demand for data privacy, federated learning has emerged as a new paradigm, which enables distributed joint training of medical data sources without leaving the private domain. However, federated learning is suffering from security threats as the shared local model will reveal original datasets. Privacy leakage is even more fatal in healthcare because medical data contains critically sensitive information. In addition, open wireless channels are susceptible to malicious attacks. To further safeguard the privacy of IoMT, we propose a comprehensive privacy-preserving federated learning scheme with a tactful dropout handling mechanism. The proposed scheme leverages blind masking and certificateless proxy re-encryption (CL-PRE) for secure aggregation, ensuring the confidentiality of the local model and rendering the global model invisible to any parties other than clients. It also provides authentication of uploaded models while protecting identity privacy. Compared with other relevant schemes, our solution has better performance on functional features and efficiency, and is more applicable to IoMT systems with many devices.
