Abstract:Abstract Older adults have difficulty adapting to new visual information, posing a challenge to maintain balance during walking. Virtual reality can be used to study gait adaptability in response to discordant sensorimotor stimulations. This study aimed to investigate age-related modifications and propensity for visuomotor adaptations due to continuous visual perturbations during overground walking in a virtual reality headset. Twenty old and twelve young subjects walked on an instrumented walkway in real and virtual environments while reacting to antero-posterior and medio-lateral oscillations of the visual field. Mean and variability of spatiotemporal gait parameters were calculated during the first and fifth minutes of walking. A 3-way mixed-design ANOVA was performed to determine the main and interaction effects of group, condition and time. Both groups modified gait similarly, but older adults walked with shorter and slower strides and did not reduce stride velocity or increase stride width variability during medio-lateral perturbations. This may be related to a more conservative and anticipatory strategy as well as a reduced perception of the optic flow. Over time, participants adapted similarly to the perturbations but only younger participants reduced their stride velocity variability. Results provide novel evidence of age- and context-dependent visuomotor adaptations in response to visual perturbations during overground walking and may help to establish new methods for early identification and remediation of gait deficits.

Abstract:Navigating public environments requires adjustments to one s walking patterns to avoid stationary and moving obstacles. It is known that physical inactivity induces alterations in motor capacities, but the impact of inactivity on anticipatory locomotor adjustments (ALA) has not been studied. The purpose of the present study was to compare ALAs and related muscle co-contraction during a pedestrian circumvention task between active (AA) and inactive young adults (IA). Thirteen AA and thirteen IA were placed in a virtual environment simulating a public park. Participants circumvented virtual pedestrians walking towards them. Walking speed, onset of deviation, clearance, foot placement strategies and muscle co-contraction were analysed. IA exhibited slower walking speeds compared to the AA during circumvention condition but not during unobstructed walking condition. The distance at the onset of trajectory deviation was larger for IA. Both groups increased co-contraction for pedestrian circumvention at the ankle and left hip and IA displayed greater ankle co-contraction overall. No significant group differences were observed in minimum clearance. This study suggests that an inactive lifestyle influences ALAs by inducing a cautious behavior during pedestrian circumvention.

Abstract:PURPOSE Use of gold nanoparticles (NPs) in radiation therapy has been found promising due to the possibility of selective radiation dose enhancement within a tumor, and favorable biological compatibility. Recent studies demonstrated that the biological effect exceeds the expectations based on the physical energy deposition solely, thus pointing toward biochemical factors. Since free radicals are responsible for the majority of the biological damage, evaluation of their radiation induced free radicals near the gold-tissue interface offers a more relevant estimate of the biological damage in NP-aided radiation treatments. METHODS Samples in the capillaries were irradiated with clinical Ir-192 HDR source. Since free radicals are very short-lived (∼nanoseconds), we used a spin trap agent, high purity DMPO dissolved in water, to trap hydroxyl radicals generated by radiolysis of water. The formed spin adducts retain spin properties of free radicals for a prolonged time (∼hours), especially at low temperatures. Radicals were detected via electron spin resonance (ESR) spectrometry technique. Quantification of free radicals generated in water and in the presence of a thin gold wire was done by double integration of the obtained spectra. RESULTS Our Monte Carlo simulations demonstrated that the dose enhancement region is limited to a close vicinity (less than 1mm) of the gold-tissue interface still suitable for free radical measurement with ESR spectroscopy. We have established a protocol for quantitative ESR measurements. The radical generation was found to be proportional to the radiation dose, and strongly enhanced by the presence of gold. CONCLUSIONS Free radicals generated by radiolysis of water can be measured quantitatively with ESR technique. Our evaluation of radiation induced free radical generation in water with nanoparticles sheds some light on the effects of NPs in biological systems. Abstract:Aging is a crucial factor influencing postural stability control and contributing to frequent falls, yet its underlying mechanisms remain incompletely understood. This study aims to explore the effects of aging on postural stability control by comparing differences in postural stability and node strength of electroencephalogram (EEG) brain network between elderly and young people under the conditions of congruent and incongruent visual-vestibular sensory inputs. Eighteen elderly volunteers without neuromuscular disorders and eighteen young individuals participated in the present study. Virtual reality (VR) technology was employed to manipulate visual rotation stimuli (clockwise and counterclockwise), and a horizontal rotating platform was used for vestibular rotation stimuli (clockwise). Based on the directional disparity of sensory input in the horizontal plane, visual-vestibular input consistency was categorized as congruent and incongruent. Postural stability was assessed by the center of pressure (COP) trajectory, and EEG signals were collected and analyzed using directed network analysis to observe EEG brain network node connectivity strength. The results revealed that, under conditions of incongruent visual-vestibular sensory inputs, the elderly exhibited significantly inferior postural stability performance in terms of COP anterior-posterior (Y-axial) sway speed, total path length, anterior-posterior and medial-lateral sample entropy, compared to the young adults. Moreover, the node connectivity strength of visual cortex in the elderly was notably higher, while node connectivity strength of superior temporal cortex was significantly lower than that in the young adults. These findings suggest that the elderly have a heightened reliance on visual information in postural control and an impaired ability to cope with sensory conflicts arising from incongruent visual-vestibular sensory inputs, leading to compromised postural stability. The outcomes of this study hold significant implications for future assessments of balance function in the elder and fall prevention trainings.
Towards an explanation of age-related difficulties in crossing a two-way street

Abstract:Crossing a two-way street is a complex task that involves visual, cognitive and motor abilities, all of which are known to decline with ageing. In particular, older pedestrians may experience difficulties when crossing two-way streets because of incorrect gap acceptance choices and impossible or unperceived evasive actions. To understand the overrepresentation of older pedestrians in crash statistics, several experimental studies have sought to identify traffic-related factors as well as those related to the abilities of the individuals themselves. However, none of these studies has required participants to actually walk across an experimental two-way street with curbs, which is a particularly challenging situation for older pedestrians. To fill this research gap, a quasi-experiment was conducted in a simulator including a total of 58 healthy aged participants (25 younger-old [age 60-72] and 33 older-old [age 72-92]) and 25 young adults (aged 18-25 years). Participants carried out a street-crossing task in a simulated two-way traffic environment; curbs were present on both sides of the experimental street. Participants also undertook a battery of tests to assess their visual and cognitive abilities. In addition, during the experiment, the participants' gait parameters were recorded. In line with earlier findings, the older-old group of participants made a higher number of decisions that led to collisions with approaching cars compared with the other groups. The two groups of older participants experienced specific difficulties when vehicles were in the far lane or when they approached rapidly. A regression analysis identified visual acuity, speed of processing (assessed using the UFOV(®) test), and step length as significant predictors of collisions. Our results have implications for understanding the difficulties experienced by older pedestrians and allow to draw up several recommendations for improving their safety.

Abstract:Studies are reported on a patient who developed thrombocytopenic purpura following quinidine therapy. Quinidine caused lysis of the patient’s platelets in vitro within 40 minutes and complete inhibition of clot retraction. Platelet lysis preceded platelet agglutination. Increasing the concentration of the sodium citrate used as anticoagulant inhibited the in vitro action of the quinidine. No thrombocytolytic effect of the patient’s serum with quinidine could be shown when tested against normal blood. A test dose of quinidine administered orally after the patient had recovered from the thrombocytopenia produced a profound decrease in the number of platelets within 90 minutes, associated with a prolongation of the bleeding time and disappearance of clot retraction. The in vitro hypersensitivity to quinidine persisted for at least four weeks after recovery from the purpura; after six weeks it could no longer be demonstrated.

Abstract:Older adults who report a fear of falling are more likely to subsequently fall, yet, some gait anxiety-related alterations may protect balance. We examined the effect of age on walking in anxiety-inducing virtual reality (VR) settings. We predicted a high elevation-related postural threat would impair gait in older age, and differences in cognitive and physical function would relate to the observed effects. Altogether, 24 adults (age (y) = 49.2 (18.7), 13 women) walked on a 2.2-m walkway at self-selected and fast speeds at low (ground) and high (15 m) VR elevation. Self-reported cognitive and somatic anxiety and mental effort were greater at high elevations (all p < 0.001), but age- and speed-related effects were not observed. At high VR elevations, participants walked slower, took shorter steps, and reduced turning speed (all p < 0.001). Significant interactions with age in gait speed and step length showed that relatively older adults walked slower ( β = − 0.05, p = 0.024) and took shorter steps ( β = − 0.05, p = 0.001) at self-selected speeds at high compared to low elevation settings. The effect of Age on gait speed and step length disappeared between self-selected and fast speeds and at high elevation. At self-selected speeds, older adults took shorter and slower steps at high elevation without changing step width, suggesting that in threatening settings relatively older people change gait parameters to promote stability. At fast speeds, older adults walked like relatively younger adults (or young adults walked like older adults) supporting the notion that people opt to walk faster in a way that still protects balance and stability in threatening settings.

Abstract:Cognitive abilities decline with healthy ageing which can have a critical impact on day-to-day activities. One example is road crossing where older adults (OAs) disproportionally fall victim to pedestrian accidents. The current research examined two virtual reality experiments that investigated how the complexity of the road crossing situation impacts OAs (N = 19, ages 65–85) and younger adults (YAs, N = 34, ages 18–24) with a range of executive functioning abilities (EFs). Overall, we found that OAs were able to make safe crossing decisions, and were more cautious than YAs. This continued to be the case in high cognitive load situations. In these situations, safe decisions were associated with an increase in head movements for participants with poorer attention switching than participants with better attention switching suggesting these groups developed compensation strategies to continue to make safe decisions. In situations where participants had less time to make a crossing decision all participants had difficulties making safe crossing decisions which was amplified for OAs and participants with poorer EFs. Our findings suggest more effort should be taken to ensure that road crossing points are clear of visual obstructions and more speed limits should be placed around retirement or care homes, neither of which are legislated for in the UK and Australia.

Effects of age on balance control during walking

Abstract:Objective: To determine the effects of aging on balance control during walking. Design: Two-group repeated-measures design. Setting: Gait laboratory in Australia. Participants: Convenience sample of 20 healthy older subjects (mean age, 72y) and 20 healthy young subjects (mean age, 24y). Interventions: Changes in locomotor performance in response to perturbations to balance were quantified for healthy older adults compared with healthy young adults for (1) straight line walking at preferred speed, (2) straight line walking at fast speed, (3) figure-of-eight walking at preferred speed, and (4) figure-of-eight walking while performing a secondary motor task. Main outcome measures: Gait speed, stride length, cadence, and double-limb support duration, using a footswitch system. Results: Healthy older people screened for pathology had gait patterns comparable to young adults for straight line walking at preferred speed. However, multivariate analysis of variance (MANOVA) showed a significant interaction between age and speed when balance was perturbed by requiring subjects to change from walking at preferred to fast speeds (Pillai-Bartlett trace=.259, F(4,35)=3.06, P<.029, partial eta(2)=.259). This occurred because older people did not increase their speed (F(1,38)=7.65, P<.01, partial eta(2)=.168) or stride length (F(1,38)=12.23, P<.01, partial eta(2)=.243) as much as did the young adults. MANOVAs did not show statistically significant interactions between age and turning conditions or age and dual task conditions, although older people walked more slowly and with shorter steps when turning or performing a secondary task. Conclusions: Balance strategies during gait are task specific and vary according to age. In response to challenges to balance imposed by the requirement to change from preferred to fast walking, older people did not increase their speed and stride length to the same extent as did younger adults. This was possibly a strategy to maintain their stability.
Space matters: virtual pedestrians with mobility constraints affect individuals' avoidance behaviours

Abstract:Walking in urban settings requires people to negotiate crowds. In these situations, people typically want to maintain a level of personal space around themselves. Recent work on one-versus-one interactions demonstrated that whether one of the pedestrians looked distracted or interacted with an object (e.g., stroller, bike) predicted the medial-lateral separation between them as they walked past each other. However, this work did not distinguish between the type of object interaction (or mobility constraint) and thus, it is unclear whether different constraints have different effects on avoidance behaviours. Here we tested the hypothesis that the type of object an approaching pedestrian held or pushed would affect the extent of path deviation, which would also depend on whether that pedestrian was distracted. To address this hypothesis, we created an immersive virtual environment that consisted of a 3.5-m-wide paved urban path. Participants had to walk and avoid colliding with approaching virtual pedestrians that often held a shopping bag or pushed a bike or stroller while looking straight ahead or off to the side as if distracted. Distraction did not affect avoidance behaviours. However, participants increased medial-lateral separation with the virtual pedestrian at the time of crossing when a stroller was present compared to the other mobility constraints. The type of mobility constraint also differentially affected onset of deviation and rate of progression before and after a path deviation. These results support the idea that characteristics of the obstacle to avoid (in this case, a virtual pedestrian) influence collision avoidance behaviours.

At Crossroads in a Virtual City: Effect of Spatial Disorientation on Gait Variability and Psychophysiological Response among Healthy Older Adults

Abstract:Introduction: Aging has been associated with a decline in cognitive and motor performance, often expressed in multitasking situations, which could include wayfinding. A major challenge to successful wayfinding is spatial disorientation, occurring mostly at crossings. Although gait changes have been observed in various dual-task conditions, little is known about the effect of disorientation on gait and psychophysiological response among older adults during wayfinding. The study aimed at identifying the effect of spatial disorientation on gait variability and psychophysiological response among healthy older adults during wayfinding in a controlled environment. Method: We analyzed data of 28 participants (age 70.8 ± 4.6, 18 female), 14 experimental and 14 controls. Participants performed a wayfinding task consisting of 14 major decision points (7 intersections) within a virtual environment (VE) projected on a 180° screen while walking on a self-paced treadmill equipped with a marker-based optical motion-capture system. The VE was held constant for the controls and manipulated for the experimental participants. Disorientation was identified based on a customized annotation scheme. Variability in gait, including the coefficient of variation (CV), was measured as the primary endpoint. Psychophysiological response measures, including heart rate variability (RMSSD) and skin conductance response (SCR), were continuously monitored as secondary endpoints and estimates of cognitive effort. Linear Mixed Effects models were applied to hypothesis-driven outcome measures extracted from decision points. Results: Walking speed and step length decreased when disoriented (p < 0.05), while stride time, stance time, walking speed CV, stance time CV, SCR amplitude, and SCR count increased when disoriented (p < 0.05). A higher RMSSD was associated with being disoriented at crossings (p < 0.05). SCR count was greater in the older experimental group (p < 0.001), including when disoriented (p < 0.001). Discussion/conclusion: The results provide evidence for the impact of spatial disorientation on changes in gait pattern and psychophysiological response among older adults during wayfinding. Location also had implications for the effect of disorientation on gait and cognitive effort. This gives further insight into the substrates of real-world navigation challenges among older adults, with an emphasis on viable features for designing situation-adaptive interventional devices aiding independent mobility.

Abstract:Background Gait adaptability training programs to prevent falls in healthy older adults can be proposed in virtual reality. The development of training programs requires the characterization of the target population. Research question Before proposing an innovative training program to develop gait adaptability behavior of healthy older adults in fully immersive virtual reality, we had to compare gait adaptability behavior between healthy older adults and young adults in virtual reality. Methods Twenty healthy older adults (with no fall history) and twenty young adults performed a goal-directed locomotion task in a totally secure virtual reality set-up. Gait adaptability behavior was investigated via a set of measures taking into account gait speed, pointing accuracy, and the evolution of the relationship between the participant and the environment through both inter-trial and trial-by-trial analyses. Mann-Whitney tests and linear regressions were performed to determine potential age differences. Results The results reveal some common and specific strategies in gait adaptability behavior between healthy older and young adults. In both populations, successive gait adjustments depend on the state of the agent-environment system. However, older adults walked more slowly than young adults (p <.001) with a greater coupling at the end of the target approach (p =.003). Significance In the context of fall prevention in healthy older adults, fully immersive VR appears as a relevant tool to propose relevant gait training programs to improve gait adjustments.

Abstract:Same day PCR amplification and sequencing is desired in situations where one needs to sequence a number of PCR products. The rapid, high-yield purification of PCR products via the use of high performance, anion-exchange chromatography yields sequencing results comparable to those obtained from techniques requiring subcloning of the PCR product. This can be achieved by standard dideoxynucleotide sequencing technology without the need to prepare prelabeled primers and additional internal primers or to gel purify the PCR product. In addition, this chromatographic technique offers the potential of isolating several PCR products from the same amplification mixture.
Motor inhibition during voluntary gait initiation in young and older adults

Abstract:Based on a novel approach, this study explores feasibility and relevance of an inhibition task for studying age-related differences in motor inhibition during gait initiation. When presented with a go-signal, young adults (YA, n = 24) and older adults (OA, n = 55) were required to promptly initiate gait. Participants completed 3 blocks of 12 gait trials. Each block contained 3 stop trials in which the go-signal was followed by a stop-signal that required the person to block gait initiation and remain standing. Stop-signals were presented randomly and with different delays. Data analyses focused on changes in the centre of pressure (COP) and success of motor inhibition. Compared to go-trials, stop trials resulted in a marked decrease of timing and amplitude of COP displacement. Overall success rate of motor inhibition was low (29% in YA vs. 19% in OA) and decreased with increasing COP displacement. Inhibitory success was associated with two strategies: a pro-active cautious COP displacement; and the inhibition of further COP displacement after a stop-signal. Results demonstrate age-related differences in adaptive behavior as well as boundaries beyond which neither old nor young persons were successful. This study yields important insights into motor inhibition during gait and essential input nto further studies.

Abstract:Background: Changes in cognitive control are considered potential factors affecting voluntary motor movements during gait initiation (GI). Simulating environments with higher cognitive resource demands have an effect on the stability of GI task performance, which is of significant importance for assessing fall risk in the older adults and devising fall risk management measures in multiple environments. This study aims to reveal the influence of complex cognitive competitive environment with increased cognitive demands on the dynamic stability during GI in the older women. Methods: Twenty-three older females and twenty-three younger females performed walking tests under three conditions: voluntary initiation (SI), visual light reaction time task (LRT), and cognitive interference + visual light reaction time task (C + LRT). Eight cameras (Qualisys, Sweden, model: Oqus 600) and three force plates (Kistler, Switzerland, model: 9287C) are used to obtain kinematic and kinetic data. To recorde the trajectory of center of pressure (CoP) and the position of the foot placement, and compute the anterior–posterior (A-P) and medio-lateral (M-L) dynamic stability at the onset and end moments of the single-leg support by means of center of mass (CoM) and gait spatiotemporal parameters. Results: Older women responded to the effect of complex environments involving cognitive competition on body stability by prolonging the lateral displacement time of the CoP during the anticipatory postural adjustments (APAs) phase, reducing step length and velocity, and increasing step width and foot inclination angle. Conclusion: Complex initiation environments lead to competition for cognitive resources in the brain, resulting in decreased stability of GI motor control in older adults. The higher the complexity of the cognitive resource demands environment, the lower the stability of GI in older adults, and the greater the effect on their M-L stability at the onset of stepping.

Abstract:Postural instability marks a prevalent symptom of Parkinson's disease (PD). It often manifests in increased body sway, which is commonly assessed by tracking the Center of Pressure (CoP). Yet, in terms of postural control, the body's Center of Mass (CoM), and not CoP is what is regulated in a gravitational field. The aim of this study was to explore the effect of early- to mid-stage PD on these measures of postural control in response to unpredictable visual perturbations. We investigated three cohorts: (i) 18 patients with early to mid-stage PD [Hoehn & Yahr stage (1-3); 1.94 ± 0.70]; (ii) a group of 15 age-matched controls (ECT); and (iii) a group of 12 young healthy adults (YCT). Participants stood on a force plate to track their CoP, while the movement of their entire body was recorded with a video-based motion tracking system to monitor their CoM. A moving room paradigm was applied through a head-mounted virtual reality headset. The stimulus consisted of a virtual tunnel that stretched in the anterior-posterior direction which either remained static or moved back and forth in an unpredictable fashion.We found differences in mean sway amplitude (MSA) and mean velocities of CoP and CoM between the groups under both conditions, with higher MSA of CoP and CoM for PD and higher mean velocities of both variables for PD and ECT when compared with YCT. Visual perturbation increased mean CoP velocity in all groups but did not have effects on mean CoM velocity or MSA. While being significantly lower for the young adults, the net effect of visual perturbation on mean CoP velocity was similar between patients with PD and age-matched controls. There was no effect of the visual perturbation on mean CoM velocity for any of the groups.Our simultaneous assessment of CoP and CoM revealed that postural control is reflected differently in CoM and CoP. As the motion of CoM remained mostly unaffected, all groups successfully counteracted the perturbation and maintained their balance. Higher CoP velocity for PD and ECT revealed increased corrective motion needed to achieve this, which however was similar in both groups. Thus, our results suggest increased effort, expressed in CoP velocity, to be an effect of age rather than disease in earlier stages of PD.

Abstract:THE INTERNATIONAL CLASSIFICATION of Diseases (ICD) is revised decennially to improve its usefulness and to bring it up to date. Unfortunately, the revision of code books used for classification of diseases creates problems. Not only do code numbers change, but the categories sometimes include different entities. The resulting break in comparability of cause-of-death statistics presents a problem for those who study trends in mortality. The effects of changes on mortality statistics resulting from earlier revisions of the ICD have been reported previously (1-3). Our concern is with the impact on the 1968 cancer mortality statistics of the introduction of the eighth revision of the ICD in 1965 (4) that was adapted for use in the United States (5). When it was observed that U.S. lung cancer deaths had increased nearly twice as much from 1967 to 1968 as they had increased in the previous 4 or 5 years-from 5 to approximately 10 percent-a research project was devised to study all sites of cancer, as well as of the lung.
Characteristics of personal space during obstacle circumvention in physical and virtual environments

Abstract:It is not known how the flexible protective zone maintained around oneself during locomotion (personal space or PS; see [Gérin-Lajoie M, Richards CL, McFadyen BJ. The negotiation of stationary and moving obstructions during walking: anticipatory locomotor adaptations and preservation of personal space. Motor Control 2005;9:242-69]) is modulated with walking speed, whether both sides of the PS are symmetrical, and whether the circumvention of physical and virtual obstructions elicit the same use of such PS. Personal space was measured in ten adults as they circumvented a cylindrical obstacle that was stationary within their path. Both left and right passes were performed at natural self-selected, slow and fast walking speeds. The same circumvention task was also performed at natural speeds in an immersive virtual environment (VE) replicating the same obstruction scenario. The shape and size of PS were maintained across walking speeds, and a smaller PS was generally observed on the dominant side. The general shape and lateral bias of the PS were preserved in the VE while its size was slightly increased. The systematic behavior across walking speeds and types of environment and the lateral bias suggest that PS is used to control navigation. This study deepens our understanding of normal adaptive walking behavior and has implications for the development of better tools for the assessment and retraining of locomotor capacity in different populations, from people with walking deficits to elite athletes. Since the PS behavior was shown to be robust in the VE used for this study, the virtual reality technology is proposed as a promising platform for the development of such assessment and retraining applications.
